Full textIn the first part, we were interested in the criteria of oxygen supply/demand imbalance involved in the occurrence of a type 2 infarction. We explored in 610 patients the association between the parameters of these criteria and the occurrence of acute myocardial injury and type 2 infarction, as well as the correlation between these parameters and the extent of myocardial injury. Our results did not show any association between the importance of oxygen mismatch and the occurrence of acute myocardial injury. There was also no correlation with the magnitude of such injury. Therefore, we could not define strict restrictive thresholds that could be considered a significant myocardial stressor. In the second part, we compared the short-term and the long-term outcomes of patients admitted with an oxygen supply/demand imbalance condition according to the presence of myocardial injury or type 2 infarction and assessed the association of these pathological entities with mortality and major cardiovascular events. In this population of 824 patients, the occurrence of myocardial injury or type 2 infarction led to high in-hospital mortality of more than 20% and was significantly associated with it after adjustment for patient characteristics. In the follow-up of survivors, the outcome was dependent on comorbidities without the involvement of the occurrence of these initial myocardial injuries, with mortality rates of 27 to 35% and major cardiovascular events of 23 to 40%. We proposed to compare these results in another study, conducted prospectively, with a standardized 6-month follow-up of patients admitted for oxygenation failure, the methods of which are detailed here. This cohort consists of 670 patients whose data are currently being analyzed. Finally, in the third part, we focused on the 675 elderly patients, who represent more than 80% of our cohort, to determine the factors associated with the occurrence of these myocardial injuries and type 2 infarction according to age classes. We found very dependent patient profiles in these classes, linked to the epidemiological changes of aging. However, the individualization of type 2 myocardial infarction within acute myocardial lesions was not obvious, nor was the impact on mortality, which was essentially based on the burden of comorbidities

Full textIt is well known that normal aging is accompanied by a decline in episodic memory. Following a traumatic brain injury (TBI), even from mild severity (MTBI), the most frequent complaints concern episodic memory and attention. Currently, the combined effects of aging and MTBI on episodic memory and related processes (ie, attention and executive control processes) remain to deepen. The first objective of this project was to evaluate the impact of MTBI on memory performance in relation with the executive control and attentional processes in the elderly. The second objective was to assess whether the presence of a MTBI in the elderly interfere with the achievement of life habits and physical activity. To meet the first objective, a paradigm of episodic memory manipulating the level of attention (full, divided) and semantic congruence between word pairs (semantically related, semantically unrelated) during the learning phase, was administered. For the second objective, the level of everyday functioning was examined using questionnaires assessing the realization of life habits, the physical activity and memory complaints in daily life. The results of this thesis demonstrate the presence of an associative deficit exacerbated by the presence of the MTBI in addition to a subtle dysfunction of the executive control system affecting encoding and retrieval in episodic memory. In addition, the results indicate the presence of functional complaints and a decrease in physical activity following an MTBI. This project provides a better understanding of the effects of MTBI in the elderly and the posibility to make recommendations to provide more appropriate intervention to the cognitive profile of the population.

Full textThe first objective of this study was to describe subjective burden (emotional experience of the caregiving role), objective burden (changes in responsibilities or activities since the elder’s injury) and psychological distress (anxious and depressive symptoms) of close relatives of elderly persons sustaining traumatic brain injury (TBI), at four months post-injury. The second objective was to explore if difficulties reported by relatives regarding the injured person, perceived social support, the age of the relative and the severity of TBI are predictors for subjective burden and psychological distress of these relatives. Telephone interviews and questionnaires were administered by trained doctoral students to a sample of 46 close relatives of elderly persons with mild to severe TBI. Results show that a significant proportion of participants (27%) experience a significant subjective burden, almost all (88%) report spending more time caring for their loved one or have less time for themselves in at least one area of life and almost a quarter suffer from significant anxious and/or depressive symptoms. Exploratory regression analyses suggest that a higher number of difficulties perceived in the injured elder and a lower perceived social support are good predictors of subjective burden and psychological distress. A younger age predicts higher subjective burden, but the severity of the TBI neither predicts subjective burden nor psychological distress. To our knowledge, this is the first quantitative study providing information on caregivers of elderly sustaining a TBI. These results provide a better understanding of their caregiving experience and contribute to guide clinical interventions (p.ex. offer a realistic prognosis and supporting the development of coping strategies) and underline the need of more longitudinal quantitative and qualitative research.

Full textBalance disorders, as the fall, remain frequently in the elderly, with adverse consequences on the autonomy. Three randomized trials have been conducted to identify the parameters distinguishing the “fallers autonomous elderly” and to analyze the impact of an osteopathic treatment. The 1st study concerns 33 elderly patients: 15 fallers (68,3 ± 2,7 years) and 18 no-fallers (67,7 ± 2,5years). Three stabilometric parameters and three clinical tests distinguish the two populations (p <0,05). Moreover, the osteopathic treatment improves the evaluated characteristics of fallers elderly (no significant difference). The 2nd study concerns 40 fallers and lombalgic elderly: 20 receiving an osteopathic treatment (69,5 ±3,9 years) and 20 without the osteopathic treatment (69,9 ± 3,4 years). This study identifies that the lumbar mobility is restricted for every fallers elderly and the osteopathic treatment improves the lumbar motion just after the treatment, as well as seven days after it (p ≤ 0,01). The 3rd study concerns 34 elderly patients: 17 fallers (71,3 ± 3,5 years) and 17 no-fallers (71,5 ± 4,2years). Four cinematic parameters distinguish the two populations (p ≤ 0,04). The coefficient ofdetermination R2 and the Gait Variability Index confirm that distinction. The osteopathic treatment improves the evaluated characteristics of fallers elderly (no significant difference)
