Zusammenfassung: Der selektive Mutismus ist eine Kommunikationsstörung in der Kindheit, bei der das Kind trotz seiner Fähigkeit zu sprechen in spezifischen sozialen Situationen nicht spricht. Praktische Richtlinien für die Diagnostik und Therapie von Kindern mit selektivem Mutismus wurden auf der Basis einer Literaturübersicht erstellt. Es gibt eine Vielzahl behavioraler Therapiemethoden zur Behandlung des selektiven Mutismus wie z.B. das Kontingenzmanagement, shaping, stimulus-fading, escape-avoidance, self-modeling oder lerntheoretische Ansätze. Eine klare diagnostische Einordnung des Störungsbildes als Angststörung oder oppositionelle Verhaltensstörung ist jedoch ein wesentlicher erster Schritt, um eine effektive Behandlung zu entwickeln.

Dargestellt werden die multimodale therapeutische Vorgehensweise sowie weiterführende psychosoziale Maßnahmen. Die Schwerpunkte des verhaltenstherapeutischen Vorgehens mit einem Aufbau sprachlicher Ausdrucksmöglichkeiten, dem Abbau der Sprechangst in sozialen Situationen sowie der Generalisierung auf nicht-therapeutische Situationen wird detailliert dargestellt. Die Kasuistik wird in den aktuellen Stand der Literatur zum elektiven Mutismus eingeordnet.

Zusammenfassung Die vorliegende Übersicht soll die wichtigsten Aspekte im klinischen Umgang mit der prolongierten, schweren Bewusstseinsstöung (SBS) zusammenfassen. Ausgehend von den klinischen Syndromen und der interdisziplinären Befunderhebung werden Assessments dargestellt, die zur Statuserhebung und Verlaufsbeurteilung geeignet sind. Auf die Rolle laborchemischer, elektrophysiologischer bzw. bildgebender Zusatzdiagnostik zur Beurteilung der Hirnschädigung und zur Prognoseabschätzung wird ebenso eingegangen. Neurorehabilitative Funktionstherapien sowie medikamentöse, nicht invasive und invasive Stimulationsverfahren werden vorgestellt. Abschließend werden – unter Berücksichtigung der aktuellen Evidenz – die bislang vorliegenden Erkenntnisse zum mittel- und langfristigen Erkrankungsverlauf dieser Patienten zusammengefasst. Zusammenfassung: Die neurologische Frührehabilitation hat im letzten Jahrzehnt stark an Bedeutung gewonnen. In diesem Artikel wird ein neuropsychologisches Behandlungskonzept für Patienten mit schwersten Bewusstseinsstörungen (Wachkoma, akinetischer Mutismus, stuporartige Antriebsstörung und Somnolenz) zur Diskussion gestellt. Schwerpunkt des Konzepts ist eine Konzentration auf die Handlungsebene und eine Interpretation der Syndrome als schwere Form der Negativsymptomatik, während die übliche Einordnung als Aufmerksamkeitsstörung in Frage gestellt wird. Die für die einzelnen Syndrome vorhandene Literatur zu neuroanatomischen Ursachen, zu neuropsychologischen Modellvorstellungen und zu funktionellen bzw. pharmakologischen Behandlungsansätzen wird jeweils kurz dargestellt und es werden daraus spezifische neuropsychologische Therapievorschläge abgeleitet. Aus der Analyse folgt, dass die Neuropsychologie einen wesentlichen Beitrag zur Behandlung dieser schwerst beeinträchtigten Patienten der Phase B der neurologischen Rehabilitation leisten könnte, bis heute aber kaum empirische Daten über die Wirksamkeit vorliegen.

Die Erforschung der Langzeiteffekte von psychischen Störungen im Kindes- und Jugendalter ist ein wichtiger Bestandteil der Entwicklungspsychopathologie. Nach einer kurzen Skizze der relevanten Begriffe, die in der Verlaufsforschung bedeutsam sind, werden in einem ersten Teil Ergebnisse zum Verlauf heterotyper (diagnostisch heterogener) klinischer Kohortenstudien dargestellt. Im umfangreicheren zweiten Teil werden die Befunde aus Langzeituntersuchungen an homotypen (homogenen) Diagnosegruppen zusammengefasst. Im Einzelnen werden Langzeitbefunde zum Verlauf und zur Prognose von ADHS, Angststörungen, Depression, Störungen des Sozialverhaltens, Anorexia nervosa, Bulimia nervosa, Autismus-Spektrum-Störungen, Schizophrenie und selektivem Mutismus referiert. Die Befunde stellen in erster Linie eine Bestätigung der Vulnerabilitätshypothese dar, welche in früh manifesten Störungen des Kindes- und Jugendalters häufig die Vorläufer von psychischen Störungen im Erwachsenenalter sieht. Die Diskussion fokussiert auf den Stellenwert früh beginnender Störungen im Rahmen der allgemeinen psychischen Morbidität sowie den noch nicht befriedigend erkennbaren Stellenwert von Interventionen.

ZusammenfassungWir berichten über einen 61jährigen Patienten mit progredientem dementieilen Syndrom, der unter der Verdachtsdiagnose einer Demenz vom Alzheimer-Typ (DAT) zur Hirn-SPECT-Untersuchung mit Tc-99m-Bicisat (ECD) vorgestellt wurde. Zum Untersuchungszeitpunkt bestanden neben dem Vollbild einer Demenz eine ausgeprägte Apraxie und Aphasie bei unspezifischen Veränderungen im EEG sowie der neuroradiologischen Bildgebung. In der Hirn-SPECT-Untersuchung fanden sich für eine DAT untypische ausgedehnte, vorwiegend rechtshemisphärische Perfusionsstörungen. Im weiteren Verlauf rasche Progredienz des Krankheitsbildes mit Entwicklung eines akinetischen Mutismus sowie Nachweis erhöhter Werte der neuronspezifischen Enolase und des 14-3-3-Proteins im Liquor. Der Patient verstarb schließlich unter dem Bild einer Decortication. Aufgrund des klinischen Verlaufs sowie der Liquorbefunde wurde, da eine autoptische Befundsicherung nicht vorliegt, die Diagnose einer wahrscheinlichen Creutzfeldt-Jakob-Erkrankung gestellt. Die vorgestellte Kasuistik unterstreicht aus unserer Sicht den Wert der Hirn-SPECT in der differentialdiagnostischen Abgrenzung dementieller Zustandsbilder.

ZusammenfassungEine Hypoglykämie liegt bei einem Blutzuckerspiegel <40 mg/100 ml vor und kann durch zahlreiche Ursachen hervorgerufen werden, meist im Zusammenhang mit inadäquater Insulingabe und/oder unzureichender Nahrungsaufnahme beim Patienten mit insulinpflichtigem Diabetes mellitus. Als weitere Faktoren sind der endogene Insulinüberschuss (Insulinom), die Hormondefizienz (GH, Kortisol), der Alkoholmissbrauch, die Medikamenteneinnahme (Amphetamine u.a.), Leber-, Nieren-und Stoffwechselerkrankungen zu berücksichtigen.Bei anhaltender Hypoglykämie kommt es neurophysiologisch zu einer verminderten und schließlich fehlenden hirnelektrischen EEG-und evozierten Potenzial-Aktivität sowie zu epileptischen Entladungen. Pathomorphologisch finden sich Mikrovakuolationen, Schrumpfungen und Zellnekrosen sowohl an Neuronen als auch an Gliazellen. Biochemisch tritt bei Hypoglykämie eine eskalierende Schädigungskaskade mit Anstieg des intrazellulären Kalziums und Freisetzung freier Radikale sowie destruktiver Enzyme auf. Die Oxygenation und der zerebrale Blutfluss bleiben jedoch erhalten. Klinisch werden bei Hypoglykämie delirante Syndrome, epileptische Anfälle, transiente neurologische Symptome, anhaltende fokal-neurologische Zeichen und in schweren Fällen Stupor und Koma beobachtet, die z.T. in schwere neurologische Defektsyndrome (akinetischer Mutismus, apallisches Syndrom) einmünden. Aufgrund der erheblichen Schädigungspotenz von Hypoglykämien kommt der Prophylaxe große Bedeutung zu. Bei Risikopatienten ist ein intensives Hypoglykämiewahrnehmungstraining sinnvoll.

This study uses a qualitative description. The analysis used in this study came from the interactive model of analysis by Hubberman and Milles. Data collection is done by observation, interview and documentation techniques. Based on the results of the study it was found that children experience Selective Mutism disorders. Children tend to be quiet when at school, while at home children are more active. The teacher's way of dealing with children who have Selective Mutism disorders is by doing an interactive approach to children. Parents also often have to introduce children to the new environment. Abstrak: Tujuan penelitian ini adalah (1) mendeskripsikan gambaran umum tentang gangguan Mutisme Selektif pada subjek penelitian, (2) mendeskripsikan faktor-faktor penyebab subjek mengalami gangguan Mutisme Selektif, (3) mendeskripsikan upaya yang dilakukan guru dan orang tua untuk mengatasi subjek dengan gangguan Mutisme Selektif. Penelitian ini menggunakan deskripsi kualitatif. Analisis yang digunakan dalam penelitian ini berasal dari model interaktif analisis Hubberman dan Milles. Pengumpulan data dilakukan dengan teknik observasi, wawancara dan dokumentasi. Berdasarkan hasil penelitian ditemukan bahwa anak mengalami gangguan Mutisme Selektif. Anak cenderung diam ketika di sekolah, sedangkan di rumah anak lebih aktif. Cara guru untuk mengatasi anak yang memiliki gangguan Mutisme Selektif adalah dengan cara melakukan pendekatan secara interaktif kepada anak. Orang tua juga harus sering mengenalkan anak dengan lingkungan baru.

<p><strong>Abstract:</strong> Selective mutism disorder is a condition in which children refuse to speak in certain social conditions but are able to speak smoothly in other situations. The focus of this research is to determine the characteristics of children who experience impaired mutism in children in TK Al Muhajirin Malang City. The research methods used are qualitative with a case study approach. The results showed that the characteristics of selective mutism disorder are: communicating using non-verbal language; Lack of eye contact when asked to communicate. The child is able to complete aspects of proficiency according to developmental stages that do not require verbal communication ability. The child will look anxious, afraid, excessive anxiety when in the new circled.</p><strong>Abstrak:</strong><em> </em>Gangguan mutisme selektif merupakan kondisi dimana anak menolak untuk berbicara dalam kondisi sosial tertentu, tetapi mampu berbicara dengan lancar di situasi lain. Fokus penelitian ini adalah untuk mengetahui karakteristik anak yang mengalami gangguan mutisme selektif pada anak di TK Al Muhajirin Kota Malang. Metode penelitian yang digunakan merupakan kualitatif dengan pendekatan studi kasus. Hasil penelitian menunjukkan bahwa karakteristik gangguan mutisme selektif, yaitu berkomunikasi menggunakan bahasa nonverbal; kurang adanya kontak mata saat diajak berkomunikasi; anak mampu menyelesaikan aspek kemampuan sesuai tahapan perkembangan yang tidak membutuhkan kemampuan berkomunikasi secara verbal; anak akan terlihat cemas, takut, kekhawatiran berlebihan ketika berada di lingkungan baru.

Akinetic mutism is described in various clinical presentations but typically is defined as a state wherein the patient appears awake but does not move or speak. It can be divided into two different subtypes; the most common subtypes depend on the lesion location, mesencephalic-diencephalic region, also called apathetic akinetic mutism (somnolent mutism), and those involving the anterior cingulate gyrus and adjacent frontal lobes called hyperpathic akinetic mutism. The pathway of akinetic mutism is believed to originate from circuits that link the frontal and subcortical structures. This case reports a 48-year-old African American female with bilateral anterior cerebral artery stroke and akinetic mutism with coexisting thyroid storm. This patient with bilateral anterior cerebral artery infarcts presented with characteristics that are typical for akinetic mutism such as having intact eye movements but an inability to respond to auditory or visual commands. With the incidence of bilateral anterior cerebral artery (ACA) ischemic stroke being rare and the incidence of akinetic mutism secondary to ischemic stroke even rarer, we suspect that this patient potentially had a unilateral occlusion of anomalous anterior cerebral vasculature.

It is can affect academic performance, social relations, and lead to the development of more serious psychological disorders. Based on these conditions, necessary to reduce the symptoms of selective mutism with an intervention program. This study used a behavioral intervention with fading and prompting in a school setting. This intervention involves the collaboration of teachers and parents. The study was conducted with a single case experimental study on subjects aged 4 years who were indicated selective mutism symptoms. An observation, interviews, and the Selective Mutism Questionnaire (SMQ) used as diagnostic and examination tools. Introduction Mutism is the inability or unwillingness to speak, resulting in an absence or marked paucity of verbal output. Mutism is a common manifestation of psychiatric, neurological, and drug-related illnesses. Psychiatric disorders associated with mutism include schizophrenia, affective disorders, conversion reactions, dissociative states, and dementias. Neurological disorders causing mutism affect the basal ganglia, frontal lobes, or the limbic system. Objectives Outline the importance of setting a differential diagnosis of mutism in the Emergency Room. Methods Review of scientific literature based on a relevant clinical case. Results Male, 58 years old. He has lived in a residence for 3 months due to voluntary refusal to ingest. Diagnosed with paranoid personality disorder. He is refered to the Emergency Service due to sudden mutism. During this day, he has been stable and suitable with a good functionality. For 3 hours he is mutist, oppositional attitude and stiff limbs, refusing to obey simple orders. Hyperalert and hyperproxia. Not staring. After ruling out organic pathology: normal blood tests, negative urine toxins and cranial CT without alterations, he was admitted to Psychiatry for observation and, finally, he was diagnosed with Psychotic Disorder NOS. Conclusions Mutism most often occurs in association with other disturbances in behavior, thought processes, affect, or level of consciousness. The most common disorder of behavior occurring with mutism is catatonia. The differential diagnosis of mutism is complex. In some cases the diagnosis will be clarified only by careful observation and after a neurological evaluation. Published studies show neurological disorders presenting with mutism can be misdiagnosed as psychiatric. Disclosure No significant relationships.

This article advances mutism as a creative mode and conceptual tool to treat silence in cinema. Whereas mutism can be a productive concept for the study of auditory and visual absence in a broader theoretical framework, my focus here is on the silence of film characters. Drawing on correspondences between the critical and the clinical in Gilles Deleuze’s writing, I argue that adopting mutism as a tool of film-philosophy permits us to supersede traditional film analysis and examine silence not as an interpretive condition at the level of the plot but as a critical-clinical mode of experimentation related to the symptomatology of life. After presenting a brief clinical history of “elective” and “selective” mutism, I then draw on the suppressed understanding of “elective mutism”. I demonstrate that “elective mutism”, which might appear to be a negative and powerless disorder in its clinical presentation, transforms itself into an affirmative and empowering (dis)order that inspires creative experimentation in cinema. In postwar films with mute protagonists, such as Ingmar Bergman’s Persona (1960), “elective mutism” engenders a negative mode of existence in its symptomatology, which can be analysed according to the tenets of Theodor W. Adorno’s philosophy of negativity. By contrast, Deleuze’s philosophy of affirmativity and the concept of minor cinema allow for a renewed positive examination of “elective mutism” in more recent films with silent characters, such as Elia Suleiman’s Palestinian quartet (1996; 2002; 2009; 2019). Approaching minor cinema as both a cinema of experimentation and minoritarian cinema, I contend that in films made by minorities mutism becomes more pronouncedly “elective” in an affirmative sense. As a mode of protest and resistance, “elective mutism” is not only a rejection of the present status quo but also an expression of futurity. Finally, I maintain that the concept of “elective mutism” can relate to and stimulate the discussion of many contemporary sociopolitical phenomena.

Abstract. The symptoms of selective mutism disorder in children cause it to be less able to speak in social situations where children are expected to speak. Interventions in the form of fading stimulus are expected to teach children new behaviors so that they are more adaptive in social situations. This study aims to determine the effectiveness of stimulus fading methods in reducing symptoms of selective mutism disorder in children. This study uses a single case experimental design with treatment in the form of the application of fading stimulus intervention in one 4-year-old boy who experienced symptoms of selective mutism disorder. The instruments of data collection were observation techniques of tallying behavior and rating scale, selective mutism questionnaire, and school speech questionnaire and analyzed descriptively. The results showed that after giving intervention within 10 days, the subject experienced significant progress, the subject became more expressive, could mingle and interact with others. Therefore, it can be concluded that the stimulus fading method can be used to reduce the symptoms of selective mutism disorder in children. Gejala selective mutism disorder pada anak menyebabkannya kurang mampu berbicara dalam situasi sosial dimana anak diharapkan dapat berbicara. Penelitian ini bertujuan untuk mengetahui efektivitas metode stimulus fading dalam menurunkan gejala selective mutism disorder pada anak. Penelitian ini menggunakan metode single case experimental design dengan perlakuan berupa penerapan intervensi stimulus fading pada satu orang anak laki-laki berusia 4 tahun yang mengalami gejala selective mutism disorder. Instrumen pengumpulan data berupa teknik observasi behavior tallying dan rating scale, selective mutism questionnaire, dan school speech questionnaire dan dianalisis secara deskriptif. Hasil penelitian menunjukkan bahwa setelah pemberian intervensi selama dalam waktu 10 hari, subjek mengalami kemajuan yang signifikan, subjek menjadi lebih ekspresif, dapat berbaur dan berinteraksi dengan orang lain. Oleh karena itu, dapat disimpulkan bahwa metode stimulus fading dapat digunakan untuk menurunkan gejala selective mutism disorder pada anak.Kata kunci: Anak; Stimulus Fading: Selective Mutism Disorder

The paper discusses the issue of selective mutism, which is an anxiety disorder. Literature analysis shows that studies on selective mutism, i.e. an individual’s failure to speak in specific social situations, are missing, especially in Polish psychological and educational literature. The paper presents mainly data on the epidemiology and aetiology of selective mutism, emphasising the need to initiate therapy as soon as possible after the diagnosis. According to epidemiological data, the number of children with selective mutism is growing. This prevents educational success, and, above all, emotional and social development. Unfortunately, symptoms indicative of selective mutism are very often underestimated or confused with other developmental problems. It should be emphasised that early and accurate diagnosis is very important as selective mutism can give rise to social anxiety, depressive disorders and many other problems. The paper describes the diagnostic specifiers of selective mutism and the current trends in explaining some of the causes of the disorder, especially in the context of polyvagal theory and the concept of Bar-Haim and Henkin (with the focus placed on the aspects that are rarely cited in the literature). The disorders and difficulties usually associated with selective mutism are also mentioned. We expect that the paper will contribute to the reflection on the need to develop effective protocols to properly manage the disorder and eliminate its effects. Our literature review shows that empirical studies on the efficacy of selective mutism therapy in children and adolescents are still missing.

The aim of this study is to focus on the special educational and developmental needs of a child with selective mutism. Selective mutism is important for human social functioning and is a serious threat to the mental health and well-being of students. Unfortunately, not many teachers are aware of this fact.The first part of the study discusses the diagnostic specifiers of selective mutism in the context of ICD-10 and DSM-5. The next section discusses the special educational and development needs of children/ students who experience symptoms of selective mutism. Then the author presents a description of the social functioning of children with selective mutism in the context of ICF-CY (The International Classification of Functioning, Disability and Health for Children and Youth). In the last paragraph of this study, the author discusses selected techniques that enable the effective inclusion of a child into a peer group and create an environment that favors the learning of a child with selective mutism. Next the author describes the range of psychological and pedagogical assistance for students with selective mutism in the kindergarten / school.

Epileptic discharges are not considered a part of the clinical picture of selective mutism, and electroencephalography is generally not recommended in its work-up. This report describes 6 children with selective mutism who were found to have a history of epilepsy and abnormal interictal or subclinical electroencephalography recordings. Two of them had benign epilepsy of childhood with centro-temporal spikes. The mutism was not related in time to the presence of active seizures. While seizures could be controlled in all children by medications, the mutism resolved only in 1. Although the discharges could be coincidental, they might represent a co-morbidity of selective mutism or even play a role in its pathogenesis. Selective mutism should be listed among the psychiatric disorders that may be associated with electroencephalographic abnormalities. It can probably be regarded as a symptom of a more complicated organic brain disorder.

Abstract Mutism and neurobehaviour symptoms are well known features, which may occur following surgical excision of mass lesion of various histopathologies in the posterior cranial fossa, during the postoperative period. Mutism may be rarely associated with ataxia of eyelid closure and paresis of external ocular muscles. However, internuclear ophthalmoparesis is not reported in association with mutism following posterior cranial fossa surgery. We report an 8-year–boy, who developed internuclear ophthalmoparesis following suboccipital craniectomy for decompression of vermian medulloblastoma. The clinical features, aetiopathogenesis, management of transient internuclear ophthalmoparesis associated with mutism and pertinent literature is reviewed in short.

Mutismo cerebelar é uma condição rara associada a cirurgias na fossa craniana posterior. É descrita como complicação com maior frequência em crianças, ocorrência após ressecção de tumores cerebelares, principalmente os situados na linha média. O status mental e a cognição não são significativamente acometidos. Relata-se um caso de mutismo transitório em criança de 4 anos de idade após ressecção cirúrgica de um ependimoma cerebelar. No segundo dia de pós-operatório, observou-se quadro de mutismo com recuperação completa 3 meses depois. O substrato anatômico desta complicação é discutido e uma breve revisão da literatura é realizada. Este caso vem somar-se aos poucos casos relatados na literatura desta rara complicação da cirurgia de fossa posterior em crianças.
