About 14 per cent of the human body is protein, so a growing child, or pregnant woman must have protein intake to increase the total amount of protein in the body, or foetus, as it grows. But why does an adult, whose body weight does not change, require protein in the diet? ‘Protein nutrition’ explains that proteins contain the element nitrogen in their constituent amino acids. Nitrogen balance is the difference between the intake of nitrogen-containing compounds in the diet and the excretion of nitrogen-containing compounds from the body. There is a requirement for dietary protein as the continual breakdown of tissue proteins in the body needs replacement by newly synthesized protein.

Impaired hepatic function is a common event in intensive care unit patients and as the liver plays a central role in drug metabolism and excretion this may lead to profound changes in pharmacokinetics. Underlying mechanisms are altered enzyme function of phase I and phase II metabolism, altered transporter protein function together with cholestasis and hepatic perfusion disorders. Moreover, multidrug therapy may lead to induction and inhibition of these enzymes and transporter proteins. In addition, changes in plasma protein binding and volumes of distribution of drugs are common. Altogether, these changes may not only lead to sometimes unpredictable plasma levels of xenobiotics, but also to drug-induced liver injury when hepatocellular accumulation of noxious substances occurs. Concomitant renal dysfunction may further complicate this situation. Pharmacodynamic alterations might also occur. In conclusion, the clinician must carefully evaluate medication given to patients with hepatic failure. Therapeutic drug monitoring should be performed wherever available to guide therapy.

As a polymer of many amino acids, any given protein can, in principle, adopt a huge number of configurations. In reality, however, the biologically stable protein adopts a single configuration that is stable over time. Thermodynamically, this configuration must represent a Gibbs free energy minimum. This chapter therefore explores how the thermodynamics and kinetics of protein folding and unfolding can be investigated experimentally (using, for example, chaotropes, heating or ligand interactions), and how these measurements can be used to enrich our understanding of protein configurations and stability.

Abstract:
This chapter shows how St. Louis March on Washington Movement (MOWM) used a major rally to launch a sustained campaign for the integration of African American workers into St. Louis's burgeoning wartime economy. A public prayer demonstration, pickets, and marches helped advance the position of black workers in a number of businesses with defense contracts, including U.S. Cartridge, the world's largest bullet manufacturer. The rising tide of African American workers at U.S. Cartridge came at the confluence of many currents, including a labor shortage, the threat of federal intervention through EO 8802, and constant grassroots pressure from MOWM. By necessity, St. Louis MOWM stepped in as an arbiter of workplace dissension at job sites when greater numbers of African American workers were met by an increase of animosity.

Some mothers are unable, or choose not, to breastfeed; bottle-feeding carries certain risks that can be minimized by following simple rules. Formula must satisfy all the nutritional needs of an infant and recommendations for the composition of infant formula have been adopted in the Codex Alimentarius. The two main types of formula differ in protein composition (whey or casein predominant). Follow-on formula are designed for infants from 6–12 months of age. Soy protein is an alternative to cow milk protein, but because of its high phytoestrogen content, should not be used before 6 months. Unmodified cow’s milk as the main drink before 12 months of age is associated with iron deficiency. ‘Complementary feeding’ embraces all solid and liquid feeds other than breast milk and infant formula. There is considerable international variation in practice with regard to introduction of complementary feeds, but in general this should not be before 17 weeks, and not after 26 weeks.

Along with energy and protein, the body needs two further groups of nutrients in the diet, in relatively small amounts: mineral salts and vitamins. ‘Vitamins and minerals’ explains how these micro-nutrients are essential for maintenance of normal health, growth, and metabolic integrity. Vitamin D and niacin are the only vitamins that can be synthesized by the body; all other vitamins must be provided in the diet. The most important minerals are iron and calcium, but other trace elements are required in small amounts. Iron is needed for synthesis of the protein haemoglobin, which transports oxygen in red blood cells, and calcium is required for bone formation and regulating the activity of muscle.

Zinc is an essential mineral involved in gene expression, cell growth and division, neurotransmission, and reproductive and immune functions. It is crucial for periods of growth, including pregnancy and lactation, infancy, childhood, and adolescence. Daily requirements in pregnancy increase by ~40, so adequate zinc stores should be ensured prior to pregnancy so that the high requirements of the developing fetus can be met. Inadequate zinc intake often accompanies general protein/calorie malnourishment but is also seen in individuals consuming poor quality diets. Diets low in animal protein and/or high in phytates (e.g. in whole grains) provide limited amounts of bioavailable zinc. Prenatal vitamins containing zinc are advisable with such diets. Zinc is required for male fertility, and supplementation may improve fertility in subfertile men.

Monogenic chylomicronemia is an autosomal recessive condition characterized by severely elevated fasting triglyceride that carries lifelong elevated risk of developing pancreatitis. The majority of cases are caused by mutations in the LPL gene encoding lipoprotein lipase, the enzyme primarily responsible for chylomicron clearance. Mutations in genes encoding associated proteins (APOC2, APOA5, GPIHBP1 and LMF1) may also present with a very similar phenotype. Current management, which includes restriction of dietary fat intake and standard pharmacologic interventions, has met with limited success, but new therapies under development may prove to be more effective.

The pathophysiology, clinical approach, and management of the common causes of metabolic acidosis and alkalosis are discussed. In metabolic acidosis, a quantitative estimate of the extracellular volume (ECFV) is required to determine its content of bicarbonate in a patient with ECFV contraction. Buffering of H+ must occur by the bicarbonate buffer system in muscle to avoid binding to intracellular proteins, this requires low muscle capillary PCO2; acid gain type of metabolic acidosis is detected by the finding of new anions in blood and/or urine. The urine osmolal gap is the best indirect test to assess [NH4+] in urine. In metabolic alkalosis, Cl− depletion alkalosis is misleading. Deficits must be defined as HCl, KCl, and/or NaCl. A quantitative assessment of ECFV helps determine the contribution of individual deficits of Cl− salts. There is no tubular maximum for HCO3− reabsorption. Angiotensin II and the usual pH in proximal convoluted tubule cells, the two major stimuli for NaHCO3 reabsorption, must be removed/ changed for NaHCO3 to be excreted.

During development, this single cell differentiates into a complex multicellular organism composed of various cells and tissues each carrying out specialized functions. Although each cell contains a genome of data it needs to select the relevant information from this genetic blueprint to fulfil its own specific function. ‘Molecular interactions’ shows that proteins must be produced in the right place and at the right time. This requires regulation of gene expression in conjunction with a myriad of bio-molecular interactions to coordinate this.

Amino acids present in the different biological fluids belong to two groups: the protein group, with the 21 classical amino acids constituting the backbone of the protein, and the nonprotein group, appearing in different metabolic pathways as intermediate metabolites. It is important to know and to be able to recognize the latter, as they are the markers of many inherited metabolic diseases. Three kinds of pathways must be considered: the catabolic pathways, the synthesis pathways, and the transport pathways. A disorder on a catabolic pathway induces an increase of all metabolites upstream and so an increase of the starting amino acid in all fluids. Any disorder on the synthetic pathway of a particular amino acid will induce a decrease of this amino acid in all fluids. When a transporter is located on a plasma membrane, its deficiency will result in normal or low concentration in plasma concomitant to a high excretion in urine.

Niacin (as nicotinamide) is a component of coenzyme systems that function in the reductive biosynthesis of fatty acids and steroids, including cholesterol, and are involved in cell signalling. Niacin deficiency is rare, as the daily requirement can usually be met by food sources, and also via synthesis from tryptophan, which is present in dietary proteins. The prevalence of niacin deficiency is higher in populations consuming mainly corn or sorghum as a dietary staple. Corn contains niacin, but only in a bound form that is nutritionally unavailable. The additional needs for niacin during pregnancy are mirrored by the increased energy intake needs, and dietary supplementation is only necessary in cases of overall poor nutritional intake.

Ammonia, an end-product of protein and amino acid catabolism toxic to the brain, must be removed quickly from the circulation. Its removal is achieved in two steps: glutamine synthesis and urea synthesis. Hyperammonemia results from either an excess of production or defective elimination. There are two main etiologies of hyperammonemia: inherited or acquired. Inherited causes are mainly related to defective elimination while acquired ones result either from excess production or deficient detoxification. Good laboratory diagnostic tools are necessary to make the right diagnosis.
