A number of rheumatological diseases are manifest in the hand and wrist. Many are associated with considerable systemic inflammation. This produces chronic ill-health, poor wound healing, and osteoporosis. Attempts to eliminate inflammation at the earliest opportunity are vital. Multiple involved joints with high acute phase markers (CRP and erythrocyte sedimentation rate raised) and early morning stiffness are key features. Multi-disciplinary management, involving the rheumatologist and hand therapist is essential. With improving medical management, surgical problems are becoming far less common. Typical conditions include metacarpophalangeal joint malalignment (ulnar drift, sagittal band subluxation), spontaneous arthrodesis, tenosynovitis, tendon rupture, and proximal interphalangeal joint imbalance (swan neck and boutonnière deformity).

This chapter proposes a comprehensive yet evolutionary reform of the global monetary non-system that evolved out of the breakdown of the original Bretton Woods arrangement in the early 1970s. The recent North Atlantic financial crisis showed how dysfunctional the current international monetary and financial architecture is for managing today’s global economy, and led to calls to reform it. Proposals for reform in this chapter include: (i) a global reserve system that mixes the multi-currency arrangement with an active use of the International Monetary Fund’s Special Drawing Rights; (ii) stronger mechanisms of macroeconomic policy cooperation, including management of the exchange rate system and capital account regulations; (iii) additional automatic balance-of-payments financing facilities, and the complementary use of swap and regional arrangements; (iv) a multilateral sovereign debt workout mechanism; and (v) major reforms of the system’s governance.

Abstract:
The study of musteloids requires different perspectives and techniques than those needed for most mammals. Musteloids are generally small yet travel long distances and many live or forage underground or under water, limiting the use of telemetry and direct observation. Some are arboreal and nocturnal, facilitating telemetry but limiting observation, trapping, and many non-invasive techniques. Large sexual size dimorphism arguably doubles sample sizes for many research questions. Many musteloids defend themselves by expelling noxious chemicals. This obscure group does not attract funding, even when endangered, further reducing rate of knowledge gain. Nonetheless, passive and active radio frequency identification tags, magnetic-inductance tracking, accelerometers, mini-biologgers and some GPS tags are tiny enough for use with small musteloids. Environmental DNA can document presence of animals rarely seen. These technologies, coupled with creative research design that is well-grounded on the scientific method, form a multi-dimensional approach for advancing our understanding of these charismatic minifauna.

Abstract:
This paper adds quantitative analysis to the study by Crompton et al. (2020), in which various alternative regulatory arrangements regarding the petrol price in South Africa were explored. We use a multi-sector dynamic computable general equilibrium model for South Africa to conduct our economic impact analysis. Five scenarios are modelled, first individually to correctly calibrate the shocks, and then cumulatively to find the overall economy-wide effects of the proposed reforms. Under the most comprehensive set of reforms to the determination of petrol prices, which seeks to emulate market forces, the South African economy is seeing substantial benefits. GDP is expected to rise by 0.67 per cent and real wages by over 1.1 per cent relative to the baseline. Refineries are assumed to shrug off reforms targeted at removing pure profits earned via the import parity price (Basic Fuel Price) methodology by accepting a slightly lower rate of return, enabling them to meet the expected increase in demand for petrol on the back of the lower consumer prices achieved via the reforms. Whilst job losses at fuel service stations may be expected as a result of reduced revenues and margins, increased activity and job opportunities in the rest of the economy, facilitated through cheaper trade and transport margins, will more than offset those losses.

This book provides an analysis of the global monetary system and the necessary reforms that it should undergo to play an active role in the twenty-first century. As its title indicates, its basic diagnosis is that it is an ad hoc framework rather than a coherent system—a ‘non-system’—which evolved after the breakdown of the original Bretton Woods arrangement in the early 1970s. The book places a special focus on the asymmetries that emerging and developing countries face within the current system, and therefore on the development dimensions of the global monetary system and of global monetary reform. The book proposes a comprehensive yet evolutionary reform of the system that includes: (i) provision of international liquidity through a system that mixes the multi-currency arrangement with a more active use of the IMF’s Special Drawing Rights (SDRs), the only true global currency that has been created; (ii) stronger mechanisms of macroeconomic policy cooperation, including greater cooperation in exchange rate management, and freedom to manage capital flows as a complement to counter-cyclical macroeconomic policy and other instruments of financial regulation; (iii) additional automatic balance-of-payments financing facilities, and the complementary use of swap and regional arrangements; (iv) a multilateral sovereign debt workout mechanism; and (v) major reforms of the system’s governance, based on a more representative apex organization, more equitable participation of emerging and developing countries in decision-making, and a network of global, regional, inter-regional, and sub-regional organizations.

Abstract:
This book examines the links between economic growth, changing employment conditions, and the reduction of poverty in Latin America in the 2000s. Its analysis answers the following broad questions: Has economic growth resulted in gains in standards of living and reductions in poverty via improved labour market conditions in Latin America in the 2000s, and have these improvements halted or been reversed since the international crisis of 2008? How do the rate and character of economic growth, changes in the various employment and earnings indicators, and changes in poverty and inequality indicators relate to each other? Our contribution is an in-depth study of the multi-pronged growth–employment–poverty nexus based on a large number of labour market indicators (twelve employment and earnings indicators and four poverty and inequality indicators) for a large number of Latin American countries (sixteen of them). The book presents a positive and hopeful set of findings for the period 2000 to 2012–13. Economic growth took place and brought about improvements in almost all labour market indicators and consequent reductions in poverty rates. But not all improvements were equal in size or caused by the same things. Some macroeconomic factors were associated with changes in labour market conditions, some of them always in the welfare-improving direction and others always in the welfare-reducing direction. Most countries in the region suffered a deterioration in at least some labour market indicators as a consequence of the international crisis of 2008, but the negative effects were reversed very quickly in most countries.
