Abstract:
105 Background: ACTS-GC trial demonstrated that S-1 is effective as adjuvant chemotherapy for Japanese patients who have undergone curative D2 gastrectomy for gastric cancer and were diagnosed with pathological stage II disease. However, stages T1N2M0 and T1N3M0, which are classified as part of Stage II, were excluded from the ACTS-GC trial. The aim of the present study was to identify the unfavorable subset of patients with T1N2M0 and T1N3M0 gastric cancer for whom surgery alone is the standard treatment. Methods: The present study examined 59 patients who were diagnosed with T1N2M0 or T1N3M0 gastric cancer at Kanagawa Cancer Center and Yokohama City University Hospital between January 2000 and June 2010. Univariate and multivariate analyses were performed to identify risk factors for overall survival using a Cox proportional hazards model. Results: When overall survival was compared by the log-rank test, a significant difference was observed with regard to macroscopic tumor diameter. A macroscopic tumor diameter greater than 30mm was regarded as a critical point of classification considering the survival. Mulitivariate Cox’s proportional hazard analyses demonstrated that macroscopic tumor diameter was the only significant independent prognosticator. The five-year survival was 60.0% in patients with a macroscopic tumor diameter < 30mm, and 84.6% in those with a macroscopic tumor diameter > 30mm (P = 0.027). Conclusions: Among T1N2M0 and T1N3M0 gastric cancer patients for whom surgery alone is the standard treatment, having a small T1N2-3 tumor of less than 30 mm in diameter was the sole risk factor for gastric cancer survival. These tumors might be another target for adjuvant chemotherapy.

Abstract:
542 Background: Numerous studies have documented the toxicities of endocrine therapy (ET) for early breast cancer (EBC) and their deleterious impact on quality of life and adherence. However, little is known about the factors that underlie patient's susceptibility to report toxicities. The identification of risk factors for toxicities from ET is important as it would allow early targeting of symptom management interventions for women more vulnerable to adverse effects of ET. This prospective study aims to examine the impact of pre-treatment perceptions of EBC, ET beliefs and fear of breast cancer (BC) recurrence (FBCR) on toxicities reported after 6 months of ET. Methods: Women diagnosed with EBC completed a survey prior to initiating endocrine therapy, then at 3, 6 and 12 months. Standardized self-report instruments were used to assess EBC perceptions, ET beliefs, FBCR and toxicities. Clinical and treatment variables were also evaluated. Univariate analyses and mulitivariate regression were conducted to identify factors associated (p<0.1) with side effects at 6 months. Results: Since 9/2010, 173 patients have consented and 84 (mean age = 60 y) have completed the questionnaires at baseline and after 6 months of ET. Controlling for age, none of the clinical or treatment variables (stage of disease, type of surgery, receipt of chemotherapy and radiation therapy) were significant univariate predictors of toxicities. In multiple regression, stronger perceptions that BC has serious consequences on their lives (β=0.218, p<0.05), greater concerns about the adverse effects of ET (β=0.215, p<0.05) and higher levels of FBCR (β=0.316, p<0.01) at baseline were associated with higher levels of reported toxicities. Conclusions: Baseline psychological factors predicted level of patient-reported toxicities to a larger extent than clinical/treatment factors. How patients perceived their illness, their beliefs about ET side effects and their fear of cancer recurrence are strongly associated with side effects experienced after 6 months of ET. These results could facilitate the identification of a subgroup of patients for early interventions to improve symptom management.

Background: The geriatri are an age group that gets more attention in the world of health, this is because the geriatri are vulnerable to various diseases. One of the diseases that are still widely experienced by the geriatri and still relatively high in Indonesia is gout arthritis or gout. Implementing gout arthritis prevention behavior with encouragement and support from the role of the family, as well as always maintaining the consumption pattern of preventative foods and triggers uric acid can reduce the risk of gout arthritis disease in the geriatri. Aim: To analyze the relationship of family roles and consumption patterns with the prevention behavior of gout arthritis in the geriatri. Method: This research uses quantitative analysis with observational analytical research type and Cross Sectional approach. The research was conducted at the Elderly Police Health Center Manyar Gresik Regency. The population of poly geriatri visitors as many as 88 geriatri. The subjects were taken by 52 seniors using purposive sampling techniques that took into account inclusion and exclusion criteria. The data was collected through interviews using family role questionnaires, FFQ (Food Frequency Questionnare) and gout arthritis prevention behavior. Data analysis using Spearman Rank test, Multiple Correlation test and Multiple Linear Regression test. Results: Based on bivariate analysis with spearman rank test shows that there is a meaningful or significant relationship between family role (p= 0.000), consumption pattern (p< 0.05) with gout arthritis prevention behavior and with multiple correlation test shows that there is a meaningful relationship between family role, consumption pattern with gout arthritis prevention behavior (p= 0.001). Analysis of mulitivariate tests with multiple linear regression tests showed the most influential variable was the role of the family (p= 0.000). Conclusion: There is a relationship between the role of the family and consumption patterns with the prevention behavior of gout arthritis in the geriatri. However, the role of the family is more related to disease prevention behavior in the geriatri. Nutrition education to geriatri patients and their families is very necessary as a source of information to prevent gout arthritis in the geriatri in Gresik Regency Manyar Health Center.

Ein Qualitätsrahmen für Suchtbehandlungsangebote: Ergebnisse einer Concept Mapping Strategie Ziel: Empirische Identifikation von Qualitätskriterien, -clusters und -dimensionen für Programme der Suchtbehandlung. </P><P>Methode: Drei Interessengruppen (Stakeholders N=90) generierten 667 Ideen, die zu 70 Statements zusammengefasst und mit mulitivariaten statistischen Techniken analysiert wurden. </P><P>Ergebnisse: Neun Cluster mit zwei Dimensionen »Best Practice« und »Effektivität« wurden identifiziert. Die drei wichtigsten Qualitätscluster waren »Klientenorientierung«, »Behandlungsangebot« und »Einstellung der Behandler«. Die drei wichtigsten Statements waren: »Effektivität des Programms«, »respektvolle Interaktion mit dem Patienten« und »leichter Zugang zu den Behandlungsangeboten«. </P><P>Schlussfolgerungen: Der empirisch abgeleitete Qualitätsrahmen für Suchtbehandlungsangebote ist spezifisch und umfassend. Es zeigen sich Übereinstimmungen und Ergänzungen zu bestehenden Zertifizierungs- und Qualitätssicherungssystemen.

Jenis penelitian ini adalah eksperimen semu dengan desain penelitian yaitu Non Equivalent Posttest Only Control Group Design. Populasi penelitian ini adalah seluruh siswa kelas IV Gugus X Kecamatan Buleleng. Sampel ditentukan dengan tenik random sampling. Teknik pengumpulan data menggunakan teknik tes berupa tes pilihan ganda dalam mengukur hasil belajar IPA dan non tes berupa angket untuk mengukur multiple intelligence siswa. Teknik analisis data menggunakan MANOVA (Mulitivariat Analysis of Variance). Hasil penelitian menunjukkan bahwa: Pertama, terdapat pengaruh yang signifikan model pembelajaran think pair share terhadap terhadap multiple intelligence siswa yang ada pada pelajaran IPA. Kedua, terdapat pengaruh yang signifikan model pembelajaran think pair share terhadap hasil belajar siswa pada pelajaran IPA. Physical activities and snack consumption of rural and urban obese adolescents in Bantul PurposeThis study to determine the difference of physical activity and snack consumption in adolescent obesity in urban and rural areas in the district of Bantul.MethodThis study used a case control study design, with a population of high school teenagers in Bantul. Cases are obese adolescents who were diagnosed at screening, while the control is not obese adolescents. Data analysis included univariable consist of frequency distribution, bivariable consist of chi-square and t-test and also logistic regression test for mulitivariabel analysis.ResultsAdolescents who do light physical activities are more likely have obesity almost 5 times than those who do physical activity in normal. Kind of snack and obesity have correlation in statistic and practical, which is many research prove that obesity found 2 times than adolescents who consume fried foods.ConclusionThere are differences of mean in snack weight and snack intake in obese adolescents. The low physical activity, high snack frequency, type of fried foods, snack weight, and snack intake are most likely to increase the incidence of obesity.

Akumulasi kasus HIV/AIDS di Provinsi Jawa Tengah, tahun 1993 - 2008 adalah pada usia 20 - 24 tahun sekitar 12,54% dan usia 25 - 29 tahun sekitar 37,31% merupakan kategori remaja dan dewasa muda. Di Kabupaten Magelang kasus HIV positif pada remaja mulai muncul tahun 2008 dan selalu muncul pada tahun berikutnya. Remaja sangat membutuhkan informasi tentang seksualitas dan peran ibu sangat penting. Tujuan penelitian ini adalah untuk mengetahui determinan perilaku ibu yang meliputi umur, pekerjaan, pendidikan, pengetahuan kesehatan reproduksi remaja, persepsi kemampuan diri dan sikap dalam pendidikan seksualitas. Jenis penelitian adalah survei dengan pendekatan potong lintang. Populasi terjangkau adalah ibu yang mempunyai anak remaja berusia 10 - 14 tahun dan mengikuti program Bina Keluarga Remaja percontohan di Kabupaten Magelang. Pemilihan sampel menggunakan klaster sampling dan berjumlah 92 orang. Pengumpulan data menggunakan kuesioner. Analisis bivariat menggunakan uji kai kuadrat dan analisis mulitivariat menggunakan regresi logistik. Hasil penelitian menunjukkan mayoritas ibu belum memberikan pendidikan seksualitas dengan baik. Variabel yang berhubungan adalah pendidikan ibu, pengetahuan ibu tentang kesehatan reproduksi remaja, persepsi kemampuan diri ibu dan sikap ibu. Persepsi kemampuan diri ibu merupakan variabel yang paling berpengaruh terhadap perilaku ibu dalam memberikan pendidikan seksualitas.Accumulation of HIV/ AIDS cases in Central Java province in 1993 - 2008 is at the age of 20 - 24 years at 12.54% and aged 25 - 29 years at 37.31% is the category of adolescent and young adults. This study was surveyed with cross sectional model. The population is mothers of adolescents aged 10 - 14 years and as member of Bina Keluarga Remaja program pilot in Magelang. The selection of samples using cluster sampling and numbered 92 people. Collecting data using a questionnaire. Bivariate analysis used chi squared test and multivariate used logistic regression. The results showed the majority of mother have not provided good sexuality education. There were relations between mother’s education level, mother’s level of knowledge about adolescent reproductive health, mother’s perception and mother’s attitudes with mother’s behavior in providing sexuality education. Mother’s perception is the most influential variable on mother’s behavior in providing sexuality education.

Anemia terjadi di seluruh dunia, terutama di negara berkembang dan pada kelompok sosio-ekonomi rendah, yaitu pada 45 % wanita. Anemia merupakan salah satu penyebab tidak langsung kematian ibu. Berbagai upaya dilakukan, namun belum menunjukkan hasil maksimal. Perlu upaya lain berupa pemberian jus alpukat untuk mengatasi anemia kehamilan. Penelitian ini bertujuan mengetahui pengaruh pemberian jus alpukat terhadap kadar hemoglobin dan jumlah eritrosit ibu hamil. Jenis penelitian Quasi experimental design, dilaksanakan bulan November s.d. Desember 2017 di Puskesmas Sindang. Jumlah responden 33 ibu hamil trimester II, kadar Hb < 11 g/dL ditentukan dengan purposive sampling. Responden mengisi kuesioner, kemudian dilakukan pemeriksaan kadar Hb dan jumlah eritrosit 3 kali yaitu pre, hari ke-7, dan 14 setelah intervensi. Analisis data dilakukan secara deskriptif, analisis bivariat menggunakan independent dan paired sample t-test serta analisis mulitivariat menggunakan repeated measures ANOVA. Mengonsumsi jus alpukat selama 14 hari efektif meningkatkan kadar Hb dan jumlah eritrosit. Hasil uji beda tidak terdapat perbedaan kadar Hb dan jumlah eritrosit antara kelompok kontrol dengan intervensi. Hb rata-rata kelompok kontrol 10,375 g/dL sedangkan intervensi 10,653 g/dL (nilai p=0,986). Jumlah eritrosit rata-rata pada kelompok kontrol yaitu 3,455 juta/mm3 sedangkan intervensi 3,614 juta/mm3 (nilai p=0,763). Uji multivariat terlihat perbedaan antara kelompok kontrol dengan intervensi. Pada kelompok kontrol kadar Hb rata-rata (p=0,441) sedangkan intervensi (p=0,023). Untuk jumlah eritrosit rata-rata pada kelompok kontrol (p=1,000) sedangkan intervensi (p=0,043). Simpulan peneltian adalah terdapat pengaruh positif pemberian jus alpukat selama 14 hari terhadap peningkatan kadar Hb dan jumlah eritrosit rata-rata, namun tidak ada perbedaan antara kelompok intervensi dan kelompok kontrol. Abstrak Anemia occurs worlwide, especially in developing countries and in low socioeconomic groups. i.e. in 45% of women. Anemia is one of the causes of bleeding. Various attempts have been made, but have not shown the maximum results. Need other efforts in the nutrition form of avocado juice to overcome the anemia of pregnancy. This research aims to determine the effect of avocado juice on hemoglobin and erythrocyte levels of pregnant women. The type of this research is Quasi Experimental design which implemented in November to December 2017 with 33 respondents of trimester II pregnant women who have HB<11g/dL and determined by purposes sampling. Repondents filled out questionnaires to determine their characteristics and performed laboratorium checkup 3 times, pre, on day 7 and 14 after intervention. The data analysis was done descriptively, hypothesis testing using independent sample t-test, to know the effectiveness of avocado juice using paired sample t-test and to know the difference of mean of Hb and erythrocytes using ANOVA repeated measures test. Consuming avocado juice for 14 days was effective in increasing the average of Hb and erythrocyte levels. LSD post hoc test results showed significant differences in Hb and erythrocytes mean (Hb p-value = 0.023 and erythrocytes p-value = 0.043) in the intervention group between pre and day 14 post intervention. While in the control group p-value p= 0.441 for Hb and erythrocytes p=1.000. The conclusion of this research that there was a positive effect of consuming avocado juice for 14 days in increasing the average of Hb and erythrocyte levels, but the hypothesis test result showed that there was no significant difference between the intervention group and the control group.
