Abstract:
The 2001 World Health Organization (WHO) classification of hematopoietic and lymphoid neoplasms categorized “the leukemias” into two major groupings—myeloid and lymphoid. Myeloid neoplasms, which are the primary focus of this chapter, include acute myeloid leukemia (AML), myelodysplastic syndromes (MDS), and myeloproliferative neoplasms (MPN). Lymphoid neoplasms are mostly reviewed as part of non-Hodgkin lymphoma in Chapter 40 of this volume, although descriptive patterns and selected etiologic studies are briefly discussed in this chapter because of historical trends. Worldwide, leukemias are ranked eleventh among all cancer types, comprising approximately 2.5% of all malignancies. Exposure to ionizing radiation and certain chemical carcinogens (e.g., cytotoxic chemotherapy, benzene, formaldehyde) are the most consistently associated risk factors for MDS and/or AML. Radiation has been linked with CML, and cigarette smoking with AML. Fewer risk factors have been identified for MPNs. Some evidence implicates increased risks of AML in rubber workers, farmers, and other agricultural workers.

Our understanding of myeloproliferative neoplasms (MPN) disorders, a group of clonal haematological malignancies characterized by excessive accumulation of one or more myeloid cell lineages, has grown considerably over the past four decades. Even more importantly is the speed at which many of these findings were translated to accord survival benefits to our patients with MPN, in particular chronic myeloid leukaemia (CML), polycythaemia vera (PV), essential thrombocythaemia (ET), and primary myelofibrosis (PMF). This text offers a detailed evidence-based guide to MPN in an easily accessible format, structure to facilitate learning specialist information presenting core information in ‘bite size’ chunks. Each chapter summarizes the state-of-the art preclinical and clinical knowledge, and its impact on the clinical management of patients with MPN.

Abstract:
This chapter challenges the assumption that a society should first deal with its past before moving on to a new future, arguing instead that settling on a shared future provides the basis for dealing with the past. A key question is what kinds of processes can take reconciliation forward and turn it into a political reality characterized by durability and deep-seated institutional change toward inclusivity and fairness. I highlight four mechanisms created during the South African transition that I consider the most important and relevant to reconciliation. These platforms—the National Peace Accord (NPA), the Convention for a Democratic South Africa (CODESA), the Multi-Party Negotiating Forum (MPNF), and the TRC—carried out vitally important work, expanding the political transition across lines of political conflict. Analyzing the first three mechanisms in terms of their inclusivity and fairness occupies most of the chapter, as well as to understand how they built on, and complemented, one another.
