Other task was to describe the diagnosis, acute treatment, aftercare, and give an overview of the brain stroke departments and rehabilitation centres in the Czech Republic. A special task was to describe care organization of patients with vascular cerebral stroke in a Faculty hospital in Hradec Králové. Main findings: Brain stroke is a disease emerging by block of the blood vessels in the brain (cerebral ischemia) or her rupture (cerebral haemorrhages). As a result, there is insufficient oxygen supply to the brain and damage of brain tissue. The most common causes are atherosclerosis and arterial hypertension. Abstract:
Introduction: The recurrence of the cerebral ischemic stroke after a history of TIA or ischemic stroke is 3-4% per year. One way of reducing the risk of reccurence is using antiplatelet therapy. The aim of our study was to investigate the effect of aspirin. Even though, newer antiplatelet drugs were developed, their risk/benefit profile has not been proved to be better than aspirin. Reasons for using aspirin in secondary prevention are: the longest experience, clearly proven effect in many studies and low price. On the other hand, aspirin prevents only 25% of strokes, thus there is wide space for searching for causes of failed therapy and alternative therapeutic ways. Noncompliance of aspirin use and embolic events are usually indicated as the most common causes of an ineffective therapy. The goal of our study was to find the antiplatelet therapy effectivity in patients with history of stroke treated with aspirin in daily dose of 100mg. We assured 100% compliance among these patients and as much as possible minimalized a likelihood of embolic causes of strokes. The thesis deals with with handwriting in adult patients after stroke. The main aim is to design and evaluate a therapeutic program for stroke patients focused on hand function and handwriting. The secondary aims are to translate Handwirting Assessment Battery for Adults into the Czech language by the method of back translation and to determine patient satisfaction with the program. The theoretical part of the thesis briefly summarizes the issue of stroke. General knowledge about handwriting is followed by a summary of handwriting difficulties of stroke patients. The theoretical part is concluded by available handwriting examinations and different ways of occupational therapy intervention. The practical part of the thesis was a case study. Five patients participated in the four- week therapeutic program. The designed program includes task-specific training and Constraint Induced Movement Therapy (CIMT) elements. The tools of the Jebsen Taylor Hand Function Test, the Frenchay Arm Test and Score for Visual Evaluation of Functional Task of the Hand were used to assess the effect of the program on hand function. The diploma thesis deals with the speech therapy of people after stroke. The aim of this diploma thesis is the analysis of the communication abilities of people after stroke. On the basis of this analysis was created for two person therapeutic material, which was verified during speech therapy. The diploma thesis is devided into two parts. The theoretical part of the thesis analyses the issue of stroke and neurogenic communication disorders on the basis of the current state of knowledge. The practical part of the thesis is performed with qualitatively focused research with the use of case studies, observation and creating own therapeutic material. Case studies describe speech therapy and work with material. The research shows that the therapeutic material is usable in practice. Title: Functional assessment after experiemantally induced focal cerebral ischemia in rat. Objectives: The aim of this diploma thesis was to design and test a set of motor tests, which provide an effective assessment of motor deficit, which forms after small ischemic lesion of primary motor cortex. Methods: Experiment was performed in 16 Long Evans rats from breeding in Institute of Physiology, Academy of Sciences of the Czech Republic. Rats were devided into two groups (experimental and control). Ischemic lesion was induced by a photothrombotic stroke model to affect a small area - a motor cortex. For functional assessment we used five motor tests - Reaching test, Bar holding test, Rotarod test, Ladder rung walking test and Oper field test. For data analysis we used program Sigma Stat3.5®SPSS and Microsoft Excel 2010. Results: Our findings demonstrate that a small lesion of motor cortex is objectively measurable by Reaching test, which focuses on grip and fine motor skills. Other tests assess mainly gross motor skills and there we did not find a significant difference between experimental and control group. Recently, the literature concerned with the possibilities and limitations of working memory training has been growing rapidly. Nonetheless, there are still no clear answers about the principles of its effectiveness or transfer effect. The main questions we ask are about effectiveness of cognitive training in patients after stroke or TBI. To our knowledge this group hasn't been studied in this context yet. To do this, we compare two types of cognitive training - extensively studied N-back training (n=11) and still more popular group cognitive therapy (n=9) with a placebo control group (n=5) who recieves "training" in a simple computer game. The placebo control group then continues in N-back training. Our hypothesis is that after 3 weeks the two trainings should lead to significantly higher gains in cognitive tests scores than the placebo condition. Also, we expected N-back to be more effective than group cognitive training in domains more closely related to executive control. We tested attention, fluid intelligence, short-term and working memory. We also recorded participants well-being. The goal of this thesis was to describe cognitive profile of mild cognitive impairment (MCI) in patients witch ischemic stroke (IST). The theoretical introduction focuses on IST, the concept of MCI, and neuropsychological test of cognitive deficits related to IST. The empirical part of the thesis describes the research results. 64 patients with various types of brain lesions after first-ever stroke were examined within 3-6 months after hospital admission. The cognitive performance was assessed across five cognitive domains: memory, attention, executive functions, visuospacial abilities, and language. All cognitive domains were impaired to the similar extend; the impairment quite severe, and ranged from 1.5 to 2.0 SD below the mean of the control group. The results showed that right hemisphere lesions were associated with visuospacial impairment whereas left hemisphere lesions were associated with language impairment. Also, it was examined which combination of tests differentiates best between patients from the experimental groups and the control group, and among individual experimental groups. It is necessary to develop and use sensitive and valid tests that can detect mild cognitive deficits in patients after ischemic stroke, which could be useful for rehabilitation planning. Need of nutritional intervention in patients after stroke Summary Aims: To identify how many patients after acute stroke have accurate oral intake of energy and proteins and how is this condition changed after one month of hospitalization. To evaluate the association between dysphagia and food intake. Methods: The evaluation was conducted by observations of food intake, eaten food records, calculating of energy and nutrients intake and comparing to needed energy (by Harris-Benedicts equation) and proteins. GUSS screening tool was used to evaluation of swallowing, ADL test for self-sufficiency assessment. Data included antropometrical indicators (weight, height, arm circumference) and nutritional screenings in Thomayer hospital in Prague. Results: Of the 35 patients, 54 % had adequate oral intake of energy and 23 % adequate intake of proteins in the first week after stroke. Adequate oral intake of energy was indicated in 66 % of patients and adequate intake of proteins in 11 % after one month of hospitalization. There were no significant improvements of oral energy intake (p=0,2891, α=0,05) or protein intake (p=0,1336, α=0,05) after one month of hospitalization. 84,4 % of patients suffered from dysphagia in first week after stroke and 60,6 % after one month of hospitalization. The Abstract The Title: Effect of Peroneal Nerve Functional Electrical Stimulation on Gait in Post-stroke Patients The Goal: The aim of this work was to compare two different ways of applying functional electrical stimulation of the peroneal nerve as to their impacts on the gait speed and performance and the spastic paresis of lower leg in adult patients suffering from chronic stages of stroke accompanied by foot drop. The Method: An open, randomized, controlled clinical pilot study combining elements of both controlled and pragmatic experiments in comparing two different clinical approaches to peroneal nerve functional electrical stimulation. With the first group of patients (group A, consisting of 14 subjects), the intervention was carried out as an outpatient, strictly controlled and intense training of gait with electrical stimulation over the period of four weeks, five times a week; the B-group patients (13 subjects) were all provided with their own stimulator which they applied continuously throughout all their daily routines at their homes and community over the equal period of time. Present situation The theoretical part of the diploma thesis is divided into four separate chapters. The first chapter deals with cerebrovascular accident (CVA). Another chapter describes the consequences of illness in various areas of human life. The third chapter defines the concept of follow-up care and the fourth chapter deals with diagnosed obesity. Aims of the work and research questions The aim of the diploma thesis is to map out the influence of obesity on follow-up care of patients after cerebrovascular accident. Research questions: 1. How does obesity affect patient mobility after CVA? 2. How does obesity affect the hygienic care of patients after CVA? 3. How does obesity affect normal daily activities of CVA patients? 4. What is the impact of obesity on developing self-sufficiency in CVA patients? Quality research was done through semi-structured interviews with CVA patients who were hospitalized in the aftercare department. Other interviews were conducted with general nurses providing nursing care to CVA patients. Research group The research group consisted of 11 general nurses working at different follow-up care hospital stations, and 16 patients who were hospitalized at these departments after the CVA. Results By analyzing the information obtained, it has been determined that obesity affects the patients mobility especially after the cerebrovascular event. At the same time, it represents a physical and psychological burden for nursing staff, but also for patients. The problem has been the fact, that most patients do not perceive obesity as a condition affecting their whole life. Conclusion The diploma thesis has provided insight into the problematic of the influence of obesity on the follow-up care in patients after cerebrovascular accident. The findings should lead to a reflection on the issue of prevention of obesity in terms of both CVA risk and follow-up care, but also the need for educational activities.

The submitted diploma thesis shows overview of the problem using upper extremity orthoses for adult with spasticity after the stroke. The aim of the thesis is to explore possibilities of using orthoses at different stages of the spasticity development. The theoretical part includes main information about the important point from central motoneuron disorder, it means spasticity. There is information about materials, production, availability and usability in various types of orthoses. Main part of the theoretical section shows research studies and available literature, which interprets using kinds of orthoses in various time when the spasticity was developed. This study aims to evaluate the effects of functional electrical stimulation on motor and functional abilities of paretic upper limb in patients after stroke. Specifically detects the immediate effect of functional electrostimulation on the increase of the active range of motion, reduction of the hand spasticity degree and the improvement of functional performance. The research covered a total of 6 patients after first stroke in the basin aretria middle cerebral artery. The research sample had a homogeneous distribution of the paresis heaviness from light, moderate, to severe. The research was conducted in total of three weeks, every working day, 60 minutes a day, with 30 minutes of indicated electrostimulation and 30 minutes of testing of monitored parameters. Patients were evaluated using the Modified Frenchay scale, Modified Tardieu scale and the range of motion goniometer. Research showed in 60 observations that the functional electrical stimulation has immediate and significant effect on increasing the active range of dorsal flexion motion of the wrist (p-value 0.043). The average improvement on 95% interval estimation is 0.08. OF MASTER THESIS Tittle of master thesis: Use of individual types of positioning on patients after stroke in the acute and subacute phase of rehabilitation from the perspective of occupational therapy. Stroke is one of the diseases that most affects the human population. It can result in temporary disability, permanent consequences and death. The patient's positioning after a stroke can affect many risks of secondary changes. The aim of this work is to find out how occupational therapists are involved in the positioning of patients in the acute and subacute phase after a stroke in stroke centers in the Czech Republic and how the positioning in stroke centers takes place. The literature and recommended procedures show that the correct positioning after a stroke in the acute and subacute phase prevents the development of immobilization syndrome and secondary changes. Positioning patients after a stroke can affect muscle tone, shoulder pain, prevent subluxation in the joints, maintain passive and active range in the joints, etc. In patients after a stroke, agitation should occur from the affected side to support the return of sensorimotor functions and to involve the affected parties to activities. We evaluated the effectiveness of two virtual reality therapies (VRT) with visual biofeedback, Armeo Spring® upper limb exoskeleton (Armeo) and Homebalance® interactive system (Homebalance), in early post-stroke rehabilitation. Using a randomized controlled study design, participants within 30 days after stroke with arm paresis (Armeo study) or with balance problem (Homebalance study) were assigned either to the respective intervention group (Armeo IG n=25; mean age 66.5 years, and Homebalance IG n=25; mean age 69.6 years) performing VRT instead of conventional physiotherapy or to the control group (Armeo CG, n=25, mean age 68.1 years, and Homebalance CG, n=25, mean age 65.9 years) having conventional physiotherapy only. Montreal Cognitive Assessment (MoCA), Functional Independence Measure (FIM), Fugl Mayer Assessment-Upper Extremity Scale (FMA-UE), Modified Rivermead Mobility Index (m-RIM) and Berg Balance Scale (BBS) were performed before and after the 3-week therapy with 12 therapies. Title: The effect of regular training in Locomat system changing the stereotype of gait of patients after the cerebrovascular accident Objectives: The purpose of this thesis is to evaluate whether it is appropriate to use the Locomat system in therapy of gait of patients after the cerebrovascular accident, what is the effect of regular training of these patients in the Locomat system on their stereotype of gait and whether the training in the Locomat system of patients after the cerebrovascular accident is more effective in comparison with other physiotherapeutic practices. Methods: The thesis was processed in the Kladruby Rehabilitation Centre on 14 probands, who were divided after the entrance examination in order to form two equivalent homogenous groups. The first group of seven members was experimental, the second one was control. At the beginning of the study the rate of gait disorder was specified at each proband using standardized method of Wisconsin Gait Scale, they also underwent 10 - Meter Walk Test and 6 - Minute Walk Test. Cerebrovascular accident (CVA, stroke) is a disease that affects the central nervous system. Many people, even younger in age, have suffered the stroke, and especially in developed countries. For adults it falls into the most common causes of disability. Greater attention is now focused on medical rehabilitation, but all the elements of rehabilitation are important. The status after stroke results in physical, psychological and social consequences. Therefore, early and coordinated rehabilitation is important, since it increases the possibility of the person´s integration. The diploma thesis aims to describe particular components of coordinated rehabilitation granted to the patients after the CVA in the treatment and rehabilitation centre Chvaly. More specifically, the author tries to analyse the coherence of the individual components of coordinated rehabilitation. In the theoretical part, the author focuses at first on the definition of the CVA, follows up with a general treatise about the coordinated rehabilitation, and then she specifies coordinated rehabilitation for patients after the CVA, and briefly describes the facility under examination. To achieve the objective there has been used qualitative research, the method of questioning, the technique of a semistructured interview. The research sample includes people after the CVA, and the staff of the treatment and rehabilitation centre Chvaly, applying specific elements of rehabilitation. Research indicates that in the examined centre they use therapeutic and social elements, where the treatment dominates. The treatment center puts emphasis on self-sufficiency of a person after the CVA, working with the family and a multidisciplinary team, which is also perceived by the clients after the CVA. The results of the diploma thesis can serve as a feedback for the examined treatment centre, they can be used to improve the quality of coordinated rehabilitation. This work can be used as a source of information for people after the CVA and their families.

(v AJ) Nursing care for patients with dysphagia after stroke is nowadays a very serious problem. This disease is particularly dangerous for patients because of the risk of imminent aspiration pneumonia. In 2015, Czech Republic has developed a standard treatment plan for patients with dysphagia after stroke and following this plan a new screening method was put into practice, which would prevent aspiration pneumonia (Salt, 2015). This method is called THE GUGGING SWALLOWING SCREEN - GUSS test. After a positive GUSS test result a standardized nursing care should follow, which would involve the whole medical team (doctors, health nurses, speech therapists, physiotherapists and orderly). The aim of this work is to verify whether the GUSS test and subsequently the determined process of the standard treatment together with the nursing care meets and the needs of the nurses. Another objective is to verify whether the nurses would want to add, innovate or make necessary updates based on the already acquired experiences. Furthermore, taking into consideration the frequent collaboration of nurses with the speech therapists, this study also aims to evaluate and asses the cooperation between nurses and the speech therapists. Constraint induced movement therapy is one of many neurorehabilitation methods, based on principle of overcoming the learned non-use phenomenon by a specific therapeutic protocol including some behavioral techniques. A classification of currently available studies on the topic have been made on the model of literature review, with an in-depth focus on using Constraint induced movement therapy in occupational therapy for adult patients after stroke who have upper limb motor disorders. The present study is structured into two sections: a theoretical part and an analytic-descriptive one. In theoretical part history of the method is presented together at Basic method principles and detail contents of a therapeutic protocol. The analytical and descriptive section is aimed to analyse international studies important and relevant with respect to an occupational therapy practice. In this second section are also proposed many possible activities of shaping and task practice techniques which can be used by occupational therapists. Title: Neurological deficit after focal cerebral ischemia in rats - pharmacological intervention Objectives: The goal of the thesis was to determine the effect of 7-nitroindazole, a selective inhibitor of neuronal nitric oxide synthase, after focal ischemic stroke in rats. Methods: Twenty adult male Wistar rats were used in this experiment. The rats were randomly divided into four groups: ischemic stroke was given to half of them, the rest were sham operated. 10 animals were given 7-nitroindazole (25mg/kg) to protect neuronal ischemic brain damage. After a few weeks the rats were tested with a set of behavioral tests: Ladder rung walking test, Bar holding test, Rotarod test and Open field test. To evaluate the volume of brain damage the stereotactic method was used. The brain sections were cut and compared with atlas. This study was supported by Institute of Physiology, Academy of Sciences ČR in Prague. Results: The present results show that the 7-nitroindazole has no side effects on healthy rats. The long-term effect on rats after ischemic stroke was not proved. There were a few positive trends observed such as an increase of locomotor speed, increased explorative behaviour and better coordination outcome on RotaRod. The focus of the thesis is on the informal caregiver and family, who cares for a family member who has had a stroke. This topic is very relevant because CMP is one of the most common causes of disability and death, both in the Czech Republic and worldwide. The thesis consists of theoretical and practical part. The theoretical part is focused on the study of literature and is divided into 4 basic areas. The first area characterizes the concept of brain and stroke, describes the causes, distribution, symptoms, treatment and consequences of this health and life-threatening event. The second area is the division of coordinated rehabilitation, which is necessary after a stroke, whether it be medical, social, pedagogical, work rehabilitation or spa care. The third area focuses on poststroke therapies such as physiotherapy, speech therapy, occupational therapy and psychology. The next area focuses on forms of social assistance, without which home care for a close person would not be possible, and includes a care subsidy, a special aid subsidy, a mobility subsidy, a disability card and pension. Last but not least, terms such as formal and informal care are explained. The method of qualitative research using semi-structured interview technique was chosen for the elaboration of the practical part. The research group consisted of seven members of the South Bohemian Region who take care of a close member of their family for a long time at home. The interviews were recorded and then literally rewritten. In my research, I tried to answer the question: "What are the family's difficulties in caring for family members after having suffered from CMP and what are their most common problems?" In the discussion, I summarized the findings and hypothesised, based on research. The results of the diploma thesis can serve as a source of information for persons after the stroke and thein families and help to educate the public.

In the diploma thesis called Nursing Care as a Complication Prevention Tool in the Patient Stroke Center, we dealt with various aspects of nursing care in the stroke center. The development of nursing research in the stroke center lags behind the development of rapidly growing medical knowledge. With the development of medical care, nursing care should respond flexibly. The research intent was to get comprehensive information on preventing complications in patients in the stroke center using nursing care. We have investigated the complications that may arise in patients with acute stroke and how they can be affected by nursing care. We have set three goals: to find out what stroke complications can be influenced in the acute phase by nursing care, to identify nursing activities by nurses in stroke centers to prevent complications and to identify obstacles and limits of preventive nursing care in the stroke center. These objectives correspond to five identified research questions. We were interested in the opinions of the staff on the provided nursing care and we also wanted to know the perception of the care provided by the patients themselves. We have tried to identify key nursing care sites, identify weaknesses and possible shortcomings, and have sought to find the basis for possible improvement in care. The qualitative research strategy was chosen to achieve the objectives of the thesis. The technique of data collection was an individual depth semi-structured interview with ten respondents. The interviews were subjected to qualitative analysis using the ATLAS.ti program. By coding, a total of 104 codes have been identified, grouped into 20 categories. We identified 18 complications that can occur with a stroke patient. For all these complications, we have found interventions to be prevented or alleviated. The results also show that nurses working in the stroke center should have specialized education for intensive care rather than college education. Specialized education in neurological issues, which nurses can acquire through post-graduate stroke seminars and exercises, is also suitable. We have identified systemic barriers to nursing care, including lack of material, lack of time and lack of staff. As obstacles on the part of nursing staff we have identified nursing indifference, poor mood, poor collaboration with the patient, hurried care, physical limitations (back pain, fatigue), lack of awareness, knowledge or skills and inadequate teamwork. As obstacles on the part of the patient we have evaluated speech disorders, perception disorder, unconsciousness, faulty adaptation, lack of cooperation, lack of understanding, distrust, negation, aggression, restlessness, disorientation, apathy, depression, overvaluation of own abilities, conflict with the patient's family. TITLE Efficacy of physiotherapeutic interventions in stroke patients with pusher and neglect syndrome. OBJECTIVES The aim of the thesis is to summarise current literature concerning two peculiar phenomena following stroke - pusher syndrome and neglect. Furthermore the work focuses on evaluation of efficacy of physiotherapeutic interventions. METHODS The thesis represents a critical review of a literature and it consists of three parts. The first one summarize issues concerning the pusher syndrome and neglect in general. In a special part you can find a detailed description of physiotherapeutic methods that are widely used in a rehabilitation treatment of these two neuronal symptoms. This work is concluded by evaluating results of effects of the therapeutic interventions that are discussed with respect to the future research. RESULTS Up till now there are no clinical trials concerning efficiacy of physiotherapeutic methods in a treatment of a pusher syndrome. The similar situation seems to be in case of a neglect syndrome as number of available clinical trials testing the efficiacy of physiotherapeutic techniques is very limited. The topic of this diploma thesis is speech therapy and intervention of people after stroke with a diagnosis of aphasia. The thesis consists of four chapters. The theoretical framework is presented in three chapters, the fourth one is an empirical part of thesis. The first theoretical chapter deals with the concept of communication and defines the basic terminology, it also describes the area of the central and peripheral nervous system and its influence on the human's communication ability. The aim of the study was to evaluate the effect of specific rehabilitation concept on the motor deficit in patients after stroke. This concept is used at the Department of inpatient rehabilitation FN Motol, the main component is exercise on the principles of dynamic neuromuscular stabilization (DNS) and is based on the principles of developmental kinesiology. The study included 12 patients after stroke. Probands participated in the 3 week therapy, exercise was carried out twice a day. Significant (p <0.05) improvement in motor function of the arm, hand, leg and foot was demonstrated by using Student`s paired t-test. There was a statistically significant improvement in postural control and reducing shoulder pain (all assessed using a specific scale: Chedoke McMaster Stroke Assessment). Changes in the degree of spasticity were evaluated by Modified Ashworth scale. A statistically significant improvement in the degree of spasticity was observed in these muscle groups: the elbow flexors , adductors of lower limb, knee extensors and hamstrings. It has been shown that the concept of dynamic neuromuscular stabilization can be advantageously used in selected patients after stroke. It is appropriate to supplement the DNS therapy with training of ADL.

This diploma thesis deals with the issue of robotic-assisted therapy, namely with the mechanical exoskeleton robotic device Armeo Spring, and it is focused on an acute stroke population. The aim of this study is to evaluate the efficiency of a robotic-assisted therapy using Armeo Spring device and to compare it with the efficiency of a common individual occupational therapy focused on improving motor function of upper extremity of patients with the acute stroke. There were 19 patients involved in the research, who were hospitalized in early rehabilitation acute beds. They were selected on the basis of entry criteria and further divided into two groups. An experimental group included 10 participants (n=10), a control group included 9 participants (n=9). The patients in the experimental group using Armeo Spring device underwent four to five 30-minutes therapies per week for three week period, the control group underwent the same amount of individual occupational therapies focused on improving motor function of a paretic upper limp. The diploma thesis explores the use biofeedback balls called SunBall® as a possible alternative in hand diagnostics and grip therapy in occupational therapy practice. This thesis includes theoretical knowledge about stroke with emphasis on upper limb from the point of view of occupational therapist. It describes the types of grips and discusses the use of a robotic biofeedback in occupational therapy practice. It includes basic information about SunBall® machine, which is used in the practical part of the thesis. The practical part of the thesis evaluates the benefit of the machine in interventions with patients after stroke in subacute and chronic phase. It also presents its possibilities of use in hand diagnostics. Performing these results objectively was achieved by the use of specific occupational therapy examination and also by standardized method, such as modified Ashworth scale, Jamar dynamometer or goninometric measurement. As the results of this thesis show, there is an apparent positive influence on the upper limb and grip therapy according to the disability of probands. There was no success in finding an appropriate application of the measured modalities of the machine in the area of upper limb and grip diagnostics.

This thesis deals with the effectiveness of individual vs. group cognitive therapies in stroke patients by training sheets, and pencil-paper exercises. The thesis is divided into theoretical and practical part. The theoretical part of the thesis describes the issue of this disease and the current study of cognitive training. The main aim of the practical part is to compare the effectiveness of group and individual cognitive training by means of intensive cognitive training in patients after stroke (within a year after a stroke). Another aim of the study is to find out how patients were satisfied with cognitive function by using the Schwartz scale of therapy. The study included 20 patients who were selected based on predetermined entry criteria. Subsequently, they were divided into two groups of 10 patients. Group 1 underwent individual cognitive therapy and group 2 underwent group therapy. In both groups, cognitive therapy was performed three times a week for one month. The time of one therapy was always 35-40 minutes. Patients were examined at baseline with the Montreal Cognitive Test and Schwartz's scale of therapy evaluation was added to the test at the end of the test. This diploma thesis deals with the topic of nutritional support in patients with neurological diseases (stroke and Critical-Illness-Polyneuropathy). The aim of this thesis is to find out, how implemented unified system of nutritional support affects the well-being of patients at neuro-rehabilitation clinic Asklepios Schlossberg Klinik in Bad König. There were observed 55 patients (30 men and 25 women) for 8 weeks. The theoretical part of this thesis describes basic components of nutrition, energy expenditure and needs, selected neurological diseases, dysphagia, malnutrition. The emphasis is put on nutrition in intensive neurological care. The practical part of the thesis analyses data obtained by the observation. There are observed changes of body weight, BMI and laboratory values of total protein, albumin and glycaemia in serum. These data are divided by sex, age, type of disease and initial BMI values. The observed patients had mainly overweight and obesity, which have lost a part of their excess weight due to catabolism. At the beginning of the observation 13 patient had normal BMI value, this number of patients rose to 24 after eight weeks. The main objective of this thesis is to raise awareness of swallowing disorders among occupational therapists. Despite the fact that the occupational therapist's intervention for patients with swallowing disorder is common practice among occupational therapists abroad, this issue still is not well known among occupational therapists in the Czech Republic. This diploma thesis is a part of the GAJU project entitled "Coordinated rehabilitation of patients with brain injury (reg. number GAJU 128/2016/S)". At the time when the thesis was elaborated, only the patients who suffered the cerebrovascular accident were engaged in the project, since in the hospital, no suitable patient after brain injury who would need a multidisciplinary team in home care had ever been sought. The selection of the patients was based on the communicative skills criteria e.g. if the skills to participate in the directed conversation based on the Functional Independence Measure Questionnaire (FIM), WHO Disability Assessment Schedule 2.0 and the follow-up questions concerning social issues, physiotherapeutic methods and techniques were preserved. Within the framework of the project, the research has not been finished yet. Given the above mentioned reasons, this thesis is therefore mainly focused on the identification of patients' needs after the cerebrovascular accident (CVA), on the period starting from its first incidence during the first three months period after the discharge from the hospital to home care. The complex results will be presented as a part of the publication activity of the project. The thesis comprises a theoretical and a practical part. The theoretical part presents a comprehensive view on the care for the patients who suffered the cerebrovascular accident. I addressed both the basic characteristics of the cerebrovascular accident and the organisation of the care for the patients in the Czech Republic; furthermore, the coordinated rehabilitation team, the role of a social worker, possibilities of social help provided by the welfare system to patients and their families, and the psychical condition of the patient. In the last chapter, I outline the general concept of human needs according to A. Maslow and describe important needs of the sick people. As far as the research part is concerned, my goal is to identify the needs of patients in home care after the brain injury. In connection with the aim of the thesis, two research questions were defined. The first question is: "What are the needs of the patients after brain injury?". The second question is: "What is the role of a social worker in meeting the needs of a patient with brain injury?" The qualitative research strategy was used; the instructional dialogue technique with patients after brain injury in the South Bohemia Region within the project. When analyzing the data collected from the patients during the first three months after the discharge from the hospital to the home care, seven major patient needs emerged need of help, need of psychical equilibrium, need of self-reliance, need of being without paint, need of recovering to the condition before CVA incidence, and need of establishing a new system of activities. Two of these needs, the need of recovering to the condition before CVA incidence and the need of establishing a new system of activities could be considered as principal, because the other needs are in general heading towards them. Furthermore, the analysis of the results showed the role of a social worker by means of which these needs can be saturated The results of the present thesis are used as a partial part from which the complex results of coordinated rehabilitation of patients after brain injury of the GAJU project are complied. The needs analysis of patients brought the proposals of dealing with/saturation of these needs and therefore had a direct impact on the lives of the respondents. The results of the research will become a part of the publication outputs of the above-mentioned projects.

Abstract:
Cerebro-vascular accident often affects parts of brain responsible for spatial orientation. Optimal integration of afference from visual, somatosensory and vestibular system is necessary for maintaining balance and often in the end for the functional indepencence of the patient. Examination of subjective vertical is a sensitive signifier for spatial orientation and the ability to discern graviception. By using clinical examination of subjective visual vertical it is possible to objectify and quantify graviception disorder excluding visual context. Current level of understanding in this field is briefly summarized in theoretical part of this study. In practical part subjective visual vertical examination by bucket-method was for statistically comparison of 29 patients after a cerebro- vascular accident with 34 healthy seniors. Hypotheses about the influence of a side of a lesion and a time elapsed since the lesions were also tested. The study concludes that the perception of verticality in patients after cerebro- vascular accident can be severely affected. Additionally, the study highlights the reliability of examination of patients after cerebro-vascular accident using clinically practical bucket-method.

Background: Sonolysis is an important factor in therapeutic use of ultrasound in patients with cerebrovascular diseases, it is ultrasound induced lysis of thrombus or embolus. The aim of this work was to asses safety and efficacy of therapeutic ultrasound effect (sonolysis) in acute stroke patients and in patients undergoing carotid artery intervention. First partial aim of this work was to confirm the safety and efficacy of endovascular sonolysis by using the EkoSonic Endovascular System in subjects with acute ischemic stroke. Second partial aim of this work was to test the clinical efficacy of sonolysis for reducing the risk of incidence of new brain ischaemic lesions detected on brain magnetic resonance imaging in patients undergoing elective CEA or CAS for severe internal carotid stenosis. In addition, we assessed the effects of sonolysis on cognitive function, morbidity, and mortality at 30 days post-surgery. Methods: Patients with acute ischemic stroke and occlusion of the middle cerebral artery or basilar artery were enrolled consecutively to the prospective study tested safety and efficacy of endovascular sonolysis using the EkoSonic Endovascular System (EKOS) in patients with acute stroke. The Action Research Arm Test (ARAT) is a standardized tool, which is used for fine and gross motor skills evaluation of hemiparetic patients after stroke. It is widely used abroad thanks to its excellent psychometric properties and easy administration. It is also used in many foreign studies to evaluate the function of upper extremity, especially for neurological patients. Unfortunately, so far it has been used only sporadically in the Czech Republic. In the theoretical part of this thesis the ARAT and the Modified Frenchay Scale (referred as "MFS") is described. In the practical part, the MFS is compared with the ARAT. The main aim of this diploma thesis is a pilot comparison of sensitivity of upper limb motor skills evaluation done by ARAT and MFS. Thirty patients after stroke were tested in the Rehabilitation Center Kladruby in three weeks. They were further divided into three intervention groups according to the severity of the paresis (light, medium, heavy). The analysis of the results shows that the sensitivity of both diagnostic tools is statistically comparable. According to the Spearman correlation coefficient, the ARAT and the MFS correlate very strongly in range of 0.96-0.98. The thesis evaluates the effect of four-week home therapy using stabilometric platform Nintendo Wii Balance Board and tablet with integrated software HomeBalance in patients with brain injury. Software HomeBalance has been developed for the treatment of balance disorders and cognitive function. Manipulating the platform and the tablet may also develop grip and handling abilities of hand, both will be evaluated in this work as well. The theoretical part describes new trends in rehabilitation of cognitive-motor interference, dual-task therapy and therapy using technological equipments. Methods: A total of 9 subjects underwent four weeks of therapy using a therapeutic set HomeBalance. The control group of 12 subjects received therapy without any technological equipment. Patients participated in the initial examination, home therapy and final examination. As an evaluation of the therapy Mini-BESTest, Timed Up and Go test, Montreal cognitive assessment, 9- hole peg test, and also two games from HomeBalance software and questionnaire assessing satisfaction with therapy have been used. Results: After treatment the whole group improved in only two tests evaluated: in Mini- BESTest and the time of diagnostic scene in HomeBalance software. Title: Survey of self sufficiency of patiens after stroke. Objectives: The aim of the thesis is evaluation of patiens after stroke by questionnaire reseach with using standard questionnaire. Methods: For getting information for research was used quantitative method by questionnaire research. It was used questionnaire FIM. Results: The research group was contained from 222 patients. Average age was 74,2 (SD 12,26) years and average time of hospitalization was 11,5 (SD 7,69) days. Total score of self sufficiency at admission was 63 (SD 36) points and at discharge 83 (SD 42) points. Self sufficiency was improved about 32% in total result. Paired t-test corfirmed that the total self sufficiency was improved, it means that motor and cognitive functions were better on level of statistical meaning 0,01. This thesis is occupied with the issues of development of the fine motor skills and graphomotorics of an individual suffering from the central mobility disorder. The main aim of the thesis is to discover and describe what development possibilities of these abilities and skills are. The theoretical part deals with basic terms related to diagnosis of Cerebrovascular accident (CVA) and its rehabilitation options. There are also described the fine motor skills, graphomotorics, grasping process, classification of grasps and their development. There is a separate chapter that is engaged only with development of the fine motor skills and graphomotorics. The thesis also contains evaluation surveys that are applied in practice and that are important for comparison before and after a therapeutic intervention. The practical part is based on qualitative oriented methodology and it operates with data gained from five case studies. Based on comparison of values gained from the entry and control examinations, particular data have been elaborated. Method: The research for this thesis involved non-experimental pre-research and quasiexperiments. The thesis' theoretical hypothesis was developed on the basis of previous studies that had looked at the effect of brain damage on patients' upper limb function. A total of twenty patients between the ages of 20 - 59 were selected to participate in the empirical study reported in this thesis. All participating patients had suffered damage to their cerebral cortex. The causes of their brain damage included strokes, trauma and tumor. The participants were divided into two groups of ten using the Nottingham Sensory Assessment. Participants with sensory disorders were allocated to the experimental group. Participants without sensory disorders were placed in the control group. Secondary prevention in poststroke patiens can lead to reduce risk of recurrence or extend lifetime like in coronary heart disease (CHD). The principles of secondary prevention are well implemented in population with CHD, but the situation in poststroke patients is quite different. The assesment of secondary prevention in poststroke patiens and a comparison with patient with CHD was selected for a goal of this study. The study population consisted of Czech patients examined in the framework of well-defined surveys in patients after their first ischemic stroke. Patients represented pooled Czech samples of the project EUROASPIRE Stroke Modul or Stroke Survey in 2007 and in 2013. Better results in acute care were detected. Admissions in stroke unit in 2013 were realized more often than 2007 and fibrinolisis was aplicated more frequently too. The subject of interest is primarily the results of the assessment obtained through the Functional Independence Measure (FIM) and the Montreal Cognitive Assessment.
