Other task was to describe the diagnosis, acute treatment, aftercare, and give an overview of the brain stroke departments and rehabilitation centres in the Czech Republic. A special task was to describe care organization of patients with vascular cerebral stroke in a Faculty hospital in Hradec Králové. Main findings: Brain stroke is a disease emerging by block of the blood vessels in the brain (cerebral ischemia) or her rupture (cerebral haemorrhages). As a result, there is insufficient oxygen supply to the brain and damage of brain tissue. The most common causes are atherosclerosis and arterial hypertension. Abstract:
Introduction: The recurrence of the cerebral ischemic stroke after a history of TIA or ischemic stroke is 3-4% per year. One way of reducing the risk of reccurence is using antiplatelet therapy. The aim of our study was to investigate the effect of aspirin. Even though, newer antiplatelet drugs were developed, their risk/benefit profile has not been proved to be better than aspirin. Reasons for using aspirin in secondary prevention are: the longest experience, clearly proven effect in many studies and low price. On the other hand, aspirin prevents only 25% of strokes, thus there is wide space for searching for causes of failed therapy and alternative therapeutic ways. Noncompliance of aspirin use and embolic events are usually indicated as the most common causes of an ineffective therapy. The goal of our study was to find the antiplatelet therapy effectivity in patients with history of stroke treated with aspirin in daily dose of 100mg. We assured 100% compliance among these patients and as much as possible minimalized a likelihood of embolic causes of strokes. The thesis deals with with handwriting in adult patients after stroke. The main aim is to design and evaluate a therapeutic program for stroke patients focused on hand function and handwriting. The secondary aims are to translate Handwirting Assessment Battery for Adults into the Czech language by the method of back translation and to determine patient satisfaction with the program. The theoretical part of the thesis briefly summarizes the issue of stroke. General knowledge about handwriting is followed by a summary of handwriting difficulties of stroke patients. The theoretical part is concluded by available handwriting examinations and different ways of occupational therapy intervention. The practical part of the thesis was a case study. Five patients participated in the four- week therapeutic program. The designed program includes task-specific training and Constraint Induced Movement Therapy (CIMT) elements. The tools of the Jebsen Taylor Hand Function Test, the Frenchay Arm Test and Score for Visual Evaluation of Functional Task of the Hand were used to assess the effect of the program on hand function. The diploma thesis deals with the speech therapy of people after stroke. The aim of this diploma thesis is the analysis of the communication abilities of people after stroke. On the basis of this analysis was created for two person therapeutic material, which was verified during speech therapy. The diploma thesis is devided into two parts. The theoretical part of the thesis analyses the issue of stroke and neurogenic communication disorders on the basis of the current state of knowledge. The practical part of the thesis is performed with qualitatively focused research with the use of case studies, observation and creating own therapeutic material. Case studies describe speech therapy and work with material. The research shows that the therapeutic material is usable in practice. Title: Functional assessment after experiemantally induced focal cerebral ischemia in rat. Objectives: The aim of this diploma thesis was to design and test a set of motor tests, which provide an effective assessment of motor deficit, which forms after small ischemic lesion of primary motor cortex. Methods: Experiment was performed in 16 Long Evans rats from breeding in Institute of Physiology, Academy of Sciences of the Czech Republic. Rats were devided into two groups (experimental and control). Ischemic lesion was induced by a photothrombotic stroke model to affect a small area - a motor cortex. For functional assessment we used five motor tests - Reaching test, Bar holding test, Rotarod test, Ladder rung walking test and Oper field test. For data analysis we used program Sigma Stat3.5®SPSS and Microsoft Excel 2010. Results: Our findings demonstrate that a small lesion of motor cortex is objectively measurable by Reaching test, which focuses on grip and fine motor skills. Other tests assess mainly gross motor skills and there we did not find a significant difference between experimental and control group. Recently, the literature concerned with the possibilities and limitations of working memory training has been growing rapidly. Nonetheless, there are still no clear answers about the principles of its effectiveness or transfer effect. The main questions we ask are about effectiveness of cognitive training in patients after stroke or TBI. To our knowledge this group hasn't been studied in this context yet. To do this, we compare two types of cognitive training - extensively studied N-back training (n=11) and still more popular group cognitive therapy (n=9) with a placebo control group (n=5) who recieves "training" in a simple computer game. The placebo control group then continues in N-back training. Our hypothesis is that after 3 weeks the two trainings should lead to significantly higher gains in cognitive tests scores than the placebo condition. Also, we expected N-back to be more effective than group cognitive training in domains more closely related to executive control. We tested attention, fluid intelligence, short-term and working memory. We also recorded participants well-being. The goal of this thesis was to describe cognitive profile of mild cognitive impairment (MCI) in patients witch ischemic stroke (IST). The theoretical introduction focuses on IST, the concept of MCI, and neuropsychological test of cognitive deficits related to IST. The empirical part of the thesis describes the research results. 64 patients with various types of brain lesions after first-ever stroke were examined within 3-6 months after hospital admission. The cognitive performance was assessed across five cognitive domains: memory, attention, executive functions, visuospacial abilities, and language. All cognitive domains were impaired to the similar extend; the impairment quite severe, and ranged from 1.5 to 2.0 SD below the mean of the control group. The results showed that right hemisphere lesions were associated with visuospacial impairment whereas left hemisphere lesions were associated with language impairment. Also, it was examined which combination of tests differentiates best between patients from the experimental groups and the control group, and among individual experimental groups. It is necessary to develop and use sensitive and valid tests that can detect mild cognitive deficits in patients after ischemic stroke, which could be useful for rehabilitation planning. Need of nutritional intervention in patients after stroke Summary Aims: To identify how many patients after acute stroke have accurate oral intake of energy and proteins and how is this condition changed after one month of hospitalization. To evaluate the association between dysphagia and food intake. Methods: The evaluation was conducted by observations of food intake, eaten food records, calculating of energy and nutrients intake and comparing to needed energy (by Harris-Benedicts equation) and proteins. GUSS screening tool was used to evaluation of swallowing, ADL test for self-sufficiency assessment. Data included antropometrical indicators (weight, height, arm circumference) and nutritional screenings in Thomayer hospital in Prague. Results: Of the 35 patients, 54 % had adequate oral intake of energy and 23 % adequate intake of proteins in the first week after stroke. Adequate oral intake of energy was indicated in 66 % of patients and adequate intake of proteins in 11 % after one month of hospitalization. There were no significant improvements of oral energy intake (p=0,2891, α=0,05) or protein intake (p=0,1336, α=0,05) after one month of hospitalization. 84,4 % of patients suffered from dysphagia in first week after stroke and 60,6 % after one month of hospitalization. The Abstract The Title: Effect of Peroneal Nerve Functional Electrical Stimulation on Gait in Post-stroke Patients The Goal: The aim of this work was to compare two different ways of applying functional electrical stimulation of the peroneal nerve as to their impacts on the gait speed and performance and the spastic paresis of lower leg in adult patients suffering from chronic stages of stroke accompanied by foot drop. The Method: An open, randomized, controlled clinical pilot study combining elements of both controlled and pragmatic experiments in comparing two different clinical approaches to peroneal nerve functional electrical stimulation. With the first group of patients (group A, consisting of 14 subjects), the intervention was carried out as an outpatient, strictly controlled and intense training of gait with electrical stimulation over the period of four weeks, five times a week; the B-group patients (13 subjects) were all provided with their own stimulator which they applied continuously throughout all their daily routines at their homes and community over the equal period of time.
