Abstract:
By the early ninth century, the responsibility for a series of social, religious and political transformations had become an integral part of running the Carolingian empire. This became especially clear when, in 813/4, Louis the Pious and his court seized the momentum generated by their predecessors and broadened the scope of these reforms ever further. These reformers knew they represented a movement greater than the sum of its parts; the interdependence between those wielding imperial authority and those bearing responsibility for ecclesiastical reforms was driven by comprehensive, yet still surprisingly diverse expectations. Taking this diversity as a starting point, this book takes a fresh look at the optimistic first decades of the ninth century. Extrapolating from a series of detailed case studies rather than presenting a new grand narrative, it offers new interpretations of contemporary theories of personal improvement and institutional correctio, and shows the self-awareness of its main instigators as they pondered what it meant to be a good Christian in a good Christian empire.

Abstract:
How do great athletes defy the power law of practice, according to which improvements in skill eventually plateau? To solve this puzzle, this book presents a theory of ‘continuous improvement’ which emphasizes the role that conscious processes play in maintaining and advancing skilled performers’ movement capacities. It argues that continuous improvement requires the use of processes such as abstract thought and bodily awareness in order to strategically alter and improve habitual movements in response to contextual demands. The book also elucidates a number of strategies that might be used to improve an athlete’s attentional control and help them switch their focus when they realize they have adopted task-irrelevant thoughts. Finally, it presents a range of methodological approaches that might be used by researchers to better understand the attentional flexibility that characterizes skilled action across training and performance contexts.

Rapid response systems (RRSs) are an important part of safety structure and this chapter aims to provide a basic understanding of the patient safety and quality movement, medical error and adverse events, and the role of the rapid response team (RRT) in identifying and reporting threats to patient safety. It offers a brief introduction of just culture, disclosure, and second victim.

Abstract:
This chapter, “Complex Systems and Approaches to Quality Improvement,” serves as an introduction to complex systems management and current thinking in improvement science. It explains the context behind quality improvement (QI) initiatives, beginning with a discussion of the ultimate goals of this movement. It then briefly reviews the history of QI development and early leaders in the field. The universal elements of a successful QI or patient safety project are discussed, followed by the exploration of commonly encountered barriers to systems and individual improvement. The chapter also highlights the differences between QI work and traditional research study. Selected tools to examine and prevent risk are explored, including informal approaches, Deming’s model for improvement, Six Sigma, healthcare failure mode and effects analysis (HFMEA), and root cause analysis (RCA).
