Abstract:
Chapter 7, “Motivational Interviewing: Theory and Practice,” provides the theoretical and practice foundation for motivational interviewing (MI). MI employs a client-centered counseling style for achieving behavior change by facilitating exploration and resolution of ambivalence. The trans-theoretical model of change is discussed, as is the “spirit of motivational interviewing,” which highlights the practitioner’s way of being with clients. The chapter presents skills and techniques to enhance older adults’ ability to work through ambivalence and develop a sustainable plan for action. In addition, later-life problems in functioning that benefit from the use of motivational interviewing are summarized, including substance abuse/misuse, chronic disease management, smoking cessation and diet, exercise and weight control. Contextual issues related to clinical practice, and special considerations for use of MI with older clients, are discussed. The chapter includes a case example of motivational interviewing with an older woman with alcohol misuse issues.

Abstract:
In Chapter 8, “Motivational Interviewing: Evidence-Based Practice,” the research on the efficacy of motivational interviewing (MI) for achieving behavioral change in older adults is summarized and evaluated. Only meta-analyses and randomized controlled trails (RCTs) were included in the review. Fifteen RCTs were found that investigated the use of MI for enhancing behavioral change in older adults related to substance abuse/misuse, exercise, diet, and smoking. Beginning research on the use of MI with older clients suggests its efficacy for promoting behavioral change in this population. Results of randomized control trials reveal that MI is as effective as, or more effective than, other psychosocial interventions in fostering healthier behavior and improved mood. The studies discussed in this review were conducted in differing formats—face-to-face, telephone, and a combination of the two. The success of the telephone-based format, as well as the face-to-face approach, reveals the utility of MI interventions with homebound elders.

Abstract:
To maximize their effectiveness, rehabilitation programs for chronic pain require patient motivation and involvement. Motivational Interviewing (MI) is a patient-centered approach that incorporates four key processes designed to reduce patient ambivalence and enhance motivation to engage in self-consistent adaptive behaviors. In this chapter, the essential processes of engaging, focusing, evoking, and planning, along with their associated techniques, are described, and examples (short clinical vignettes) are provided of how MI operates in practice. The chapter concludes with a review of the empirical data in support of the beneficial effects of MI in chronic pain populations. Although additional research is warranted, the available evidence demonstrates the efficacy of MI for facilitating adaptive responding such as increased patient participation and engagement in physical exercise.

This chapter focuses on the use of brief interventions for the treatment of alcohol and other substance use disorders and risky use. The authors provide definitions of brief interventions and a rationale for their use. They review the evidence base for brief interventions across primary care, emergency medical, college, and correctional settings, and include analysis of the impact of brief intervention on drinking and drug use and the relative costs of such services. They also describe several widely used frameworks or organizing structures for brief interventions including FRAMES (provide feedback, emphasize responsibility, give advice, menu of options, express empathy, support self-efficacy), SBIRT (screening, brief intervention, and referral to treatment), and the five As (ask, assess, advise, assist, arrange). Finally, the authors discuss the therapeutic approach of motivational interviewing as an interaction style that can be used within the context of many brief intervention structures.
