Abstract:

The sense of loss of a mother leads Anna Morgan to her imagined black identity. Being a Creole from Dominica, Morgan is alienated both in her home country and in London. Du Bois’s notion of double consciousness substantiates Morgan’s sense of alienation. The racial issue here is not only socially constructed, but it is also personally constructed, as Morgan does not consider England as her homeland although she is as white as English people. people. The character is struggling from identity conflict, as she internalizes the impact of the British colonialization to the black people in Dominica. She feels more black than white because of the image of blackness that she creates from the image of her mother and black women in general, as nurturing, warm and domestic. In addition to that, the geographic location contributes to Morgan’s sense of loss.

Abstract:
Since the outbreak of COVID-19, language teachers have been asked to rapidly react and adapt to constantly changing teaching environments in order to understand their students’ needs in L2 learning and make judgments in conditions of uncertainty. The COVID-19 outbreak has highlighted the need to understand how teachers’ identities evolve during such difficult times and situations. In response to this need, this study reports the findings from my qualitative case study on a Korean English teacher’s identity shift. Drawing upon Foucault’s (1983) notion of ethical self-formation, I examined how the Korean English teacher negotiated and developed her identity to adjust to drastically changing working environments as she weighted the benefits and challenges of online and offline education, particularly for novice Korean EFL learners. Data were collected through various sources from an experienced Korean English teacher, called Anna, at a regional foreign language center in South Korea over the course of two years. Due to the pandemic, she had to make the transition from offline to online teaching. Further, her center closed one year after the outbreak of the pandemic, and she was reassigned as a travelling teacher in a multi-school program for underachieving English students. The findings reveal that Anna became more agentive in searching for and utilizing multiple resources for teaching, showing her reflective and action-oriented practices to involve in ethical, practiced, and productive identity work (Miller, Morgan, & Medina, 2017). The findings contribute to expanding our understanding of the transformative potentials of language teachers’ identity.

Abstract:

Abstract:
From Colin Radford we have inherited the ‘fiction paradox’: How can we be moved by creatures of fiction? Answers recently offered by aestheticians presuppose a false theory of emotion and are inconsistent with emotional phenomenology as well as facts about human evolution, physiology, and anatomy. I argue that Kendall Walton's ‘Charles’ can genuinely fear the slime and yet avow that he does not consider it dangerous—all without being irrational. The solutions offered by Morreall, Moran, Hyslop, Boruah, Lamarque, and Neill are rejected as inadequate. Knowledge of how we can be moved probably lies outside philosophy altogether—in cognitive neuroscience.

Abstract:
Background:In chronic rheumatic diseases, non-adherence to treatment is associated with a progression of disease and an increased morbidity (1). In spondyloarthritis (SpA), improving patients’ knowledge on their subcutaneous biologic disease-modifying antirheumatic drugs (bDMARDs) is a key factor to enhance medication adherence (2). The patient information has to ensure the acquisition of safety skills regarding their treatment management. Objectives:To evaluate the impact of a pharmacist’s educational interview on knowledge and therapeutic adherence of subcutaneous bDMARDs in patients with SpA.Methods:Population and study design:consecutive adult patients with well-controlled axial SpA, stable on subcutaneous bDMARDs were enrolled in a randomized, controlled, single-center, open-label, 6-months trial.Intervention:A pharmacist’s educational interview provided information on bDMARDs management at baseline in the intervention group (IG) and at month 6 (M6) in the control group (CG). A booklet containing essential information was given to the patient.Intervention allocation:After written consent, the study treatment was randomly allocated via a computer program by simple randomization, with an allocation ratio of 1:1.Outcome measures:The change of a weighted knowledge score (0 – 100) concerning the bDMARDs management and the change in the Medication Possession Ratio (MPR) at M6 were primary outcomes. The changes in disease activity (BASDAI) and patients’ satisfaction regarding the pharmacists’ interview were secondary outcomes.Statistics:Changes in knowledge score, MPR and BASDAI were compared between the two groups using the T-Student test. Statistical analysis was performed in intention-to-treat. Missing data was handled with multiple imputations. Results:Patients’ characteristics at baseline were comparable among the 89 included patients (46 in IG, 43 in CG). The means ± SD of the knowledge score were 75.3 ±14.2 versus 73.0 ±13.2 and 86.3 ±12.6 versus 76.0 ±14.1 in the IG versus CG at baseline and at M6, respectively. The patient’s knowledge score improved at a greater magnitude in the IG (+11.0 ±11.5 versus +3.0 ±10.6 in the IG versus the CG respectively, p<0.0001). The MPR at baseline were very high in both groups (92.9 ±14.6% versus 96.6 ± 15.6% in the IG versus the CG, respectively). There was a trend in a better adherence (+2.2 ±13.9 versus -0.6 ±18.9 in the IG versus the CG in the MPR score respectively, p = 0,691). The disease activity (changes in BASDAI) remained stable during the study in both groups. All the patients were mostly or totally satisfied by the pharmacists’ interview. Conclusion:Pharmacists’ educational interview on subcutaneous bDMARDs is effective in improving the knowledge of patients with SpA on their treatment. Regarding therapeutic adherence, a trend in favor of an improvement was observed in the intervention group but did not reach the statistically significance. Nevertheless, the results observed in this study are an argument to propose to include the pharmacists in the multidisciplinary team in charge of the management of patients with SpA.References:[1] Bluett J, Morgan C, Thurston L et al. Abstract:
A mezőgazdaság globalizálódása jelentősen átalakította a kukorica világkereskedelmét. Az elmúlt évtizedekben tapasztalható piaci növekedés, a szabadkereskedelmi egyezmények számának emelkedése, valamint területi koncentráció indokolttá teszi a szomszédsági hatások vizsgálatát. A szomszédsági hatások irodalma korábban szinte kizárólag az ipari és a szolgáltatási szektorra koncentrált. A tanulmány célja a térbeli függőség, mint a szomszédsági hatásokra utaló jelenség bemutatása a kukorica világpiacán 1996 és 2015 közötti időszakban. A cikk Moran-féle globális és lokális I mutató segítségével elemzi a térbeli függőséget a kukorica piacon, kitérve annak időbeli dinamikájára. Eredményeink megerősítették, hogy a kukoricapiac bővülése több térségben előidézte a térbeli függőség erősödését.

Abstract:
In recent years, historians of the Augustan period have done much to rehabilitate the posthumous reputation of Queen Anne, a monarch traditionally viewed as dull, weak, reactionary, and easily led. Beginning in the 1920s with the work of W. T. Morgan, continuing with that of G. M. Trevelyan and G. S. Holmes, and culminating in the definitive biography by Edward Gregg, Anne has gradually emerged as a figure to be reckoned with. We have come to see her as a tenacious and often skillful navigator, charting a middle course between the opposing shoals of the Whig and Tory parties, in an attempt to preserve freedom of maneuver for the postrevolutionary monarchy. This article will explore a heretofore neglected aspect of the queen's political helmsmanship: the attempt to make her person and crown a focus for national (i.e., English) unity through the revival and exploitation of royal ritual and symbol. It will be argued below that Anne—alone among the later Stuarts—made extensive use of the arsenal of ceremonial paraphernalia, what David Cannadine has called “the theatre of power,” which is normally associated with her Tudor and early Stuart predecessors. This essay is thus intended not only to contribute to the ongoing reassessment of Anne's political role but also to help fill a gap between the wealth of fine work on pageantry at those earlier courts and the work Linda Colley and others have done on the reign of George III.

Abstract:
Review of Secret Feminist Agenda, Season 4 By Andi Schwartz and Morgan Bimm The Secret Feminist Agenda podcast was first encountered by then-graduate student Andi Schwartz as assigned ‘reading’ in a Queer Pedagogies seminar. The seminar was part of a student-run initiative facilitated by co-reviewer, Morgan Bimm, who started the seminar series as a critical response to a lack of teaching resources available to graduate students. The podcast’s aims and sensibilities spoke to our experiences and values both then, as first-generation university students and now, as emerging feminist media scholars. Secret Feminist Agenda is recorded and produced by Dr. Hannah McGregor, an Assistant Professor of publishing at Simon Fraser University. Secret Feminist Agenda is McGregor’s second podcast, which she began in 2017 with the aim of bridging academia and feminism and forging connections between feminists.[1] In addition to producing the Secret Feminist Agenda podcast, podcasting has become an integral part of McGregor’s pedagogy[2] and research; she co-founded the SSHRC-funded Amplify Podcast Network to develop guidelines for peer reviewing podcasts. The original goals of the podcast, bridging academia and feminism and forging connects with feminists, remain the driving force behind season four, which is further organized around the principle of “keeping it local.” Season four consists of 30 episodes, half of which offer long-form interviews with feminists in academia, art, sex therapy, podcasting, Canadian literature, comedy, and more, which effectively highlight the various forms that feminism can take and offer a window into feminist friendships and community. While the theme “keeping it local” was challenged by the onset of the COVID-19 pandemic (interviews could no longer be conducted in person), the podcast consistently succeeded in prompting listeners to think about space and place as they relate to feminism and community. In our review, we were struck by the following three themes: 1) critiquing the expert(ise); 2) the spaces and places of feminist thought; and 3) the politics and affects of community space. In form, the scholarly podcast acts as a critique of the existing structures of academia. Through interviews with feminists like Dawn Serra and Khairani Barokka, the notion of expertise is critiqued alongside academia’s role in perpetuating myths of excellence through citational and syllabi-building practices. Such critiques highlight the importance of DIY media, like podcasts, as spaces through which expertise can be critiqued and other points of view are circulated. Solo-recorded “minisodes” often engage with more personal or affective topics; though we debated the merits of these episodes, we came to the conclusion that introducing affect and the personal into scholarship is both an important feminist project and a vital challenge to existing ideas about academic rigour.[3] Through interviews with feminists across fields, including sex therapy (Episode 4.2), comedy (Episode 4.6), podcasting (Episode 4.8), and art (Episode 4.4), the podcast demonstrates the many places and spaces in which feminist thought is fostered; indeed, that feminist thought and critique does not belong solely to the academy. The complexities of public intellectualism or public feminism are compellingly discussed in Episode 4.7: Trans Rights are Human Rights through the lens of cancelled and protested “gender identity debates” scheduled for public spaces across Canada. Campaigns to cancel these events are framed by some as an attack on ‘free speech’ and thus, perhaps, an attack on healthy public intellectual exchange, but these activist efforts are themselves an example of public modes of feminist thought. This and other discussions throughout season four of Secret Feminist Agenda highlight the multiple spaces of feminist thought and the multiple complexities of thinking feminism in public. In the spirit of “keeping it local,” season four offers rich discussions of the politics and affects of community space. A favourite example is episode 4.14 with Hilary Atleo of Iron Dog Books in Vancouver, which explores the connection between small business and housing costs as well as the power of systems to foster or destroy community and communal affinities. Episode 4.15, a minisode about World Obesity Day, further demonstrates the malleability of (virtual) space via political intervention, and how the political occupation of space can foster solidarities and positive, communal feelings. The COVID-19 pandemic hit Canada midway through the season, around episode 4.16 with Kai Cheng Thom, whose work frequently engages with notions of disposability, accountability, and harm within queer communities. The intersection of Thom’s work and COVID-19 serves as an acute reminder of both the affective and material significance of community, and the potential devastation of losing it. In addition to these themes, the podcast incites interesting questions about the feminist and scholarly potential of the podcasting form. McGregor and colleagues have developed podcast peer review guidelines as a mechanism for folding podcasts into the institutional understanding of rigour, and we further understand Secret Feminist Agenda as rigorous in its feminist politics of accessibility and the feminist practice of critique. Podcasts can be understood as a feminist medium in that they often feature grassroots and DIY production, have a wider reach than more sanctioned forms of scholarship, and have the capacity to bolster women’s, feminized, and otherwise marginalized voices. The feminist and scholastic merits of podcasting were explicitly discussed in episode 4.20 with Stacey Copeland and minisode 4.21, “Introducing the Amplify Podcast Network.” As Copeland and McGregor discuss, women’s voices have long been interpreted as unintelligent and unauthoratitive. Podcasting, with its grassroots and DIY sensibilities, has the potential to instill confidence in women, feminized and otherwise marginalized folks through building a practice of speaking; McGregor notes how podcasting has bolstered her own confidence in both academic and non-academic spaces.[4] Oriented toward low theory and feminist media scholarship, we are perhaps already primed to welcome podcasts into the scholarly fold. In our view, Secret Feminist Agenda is exemplary of the benefits wrought by bridging traditional academic knowledges with low theory, community, and collaborative practices. It is our hope that, as academia becomes better acquainted with podcasts, they retain their radical potential, rather than become another research output taxing already overburdened academics. [1] McGregor started her first podcast, Witch, Please, as a collaboration with her friend and former colleague, Marcelle Kosman, in 2015. [2] In a review of season two of SFA, Anna Poletti suggests that the work done through the podcast is more akin to teaching than research (Poletti, 2019). [3] In a review of season two of SFA, Carla Rice noted that the minisodes are where the podcast “shines,” writing with admiration of McGregor’s ability to address these more affective topics from both a personal and “big picture” perspective (Rice, 2019). [4] Similar arguments have been made by podcaster-academics, Raechel Tiffe and Melody Hoffman, who hosted the podcast, Feminist Killjoys, Phd, among others (Tiffe & Hoffman, 2017).

Abstract:
Bevezetés: A mentális képességek és a teljesítmény kapcsolatát több sportág vizsgálata során is alátámasztották (Dosil, 2006, Russo & Ottoboni, 2019, French & Thomas, 1987). A technikai képességek és tudás, valamint a fizikai állóképesség és fittség mellett a taktika a sportteljesítmény összetevője (Moran & Toner, 2017, Wagner, Finkenzeller, Würth & von Duvillard, 2014). A kutatás célja annak megállapítása, hogy milyen hatást gyakorol egy videó eszközökön alapuló módszer kézilabda játékosok taktikai tudatosságára, technikai kivitelezéseire és fegyelmi teljesítményére. Módszertan: A vizsgálatban öt, U12-es kézilabdázó fiatal fiú vett részt, azonban sérülés miatt négy játékossal fejeződött be az intervenció. A beavatkozás Vickers (2000) video modeling módszerére épült, négy ülésből tevődött össze. A kívánt irányú változás a négyből három sportoló esetében mindhárom vizsgált dimenzióban rövid távon megjelent különböző hatásmértékekkel (∆ hatásmérték index = [0,07;2,89]), ám az intervenció befejeztével, hosszabb távon ezek a kívánatos irányban történő változások visszarendeződtek (∆- index= [-0,5;2,23]).

Abstract:
Background:There are several reports that cytomegalovirus (CMV) reactivation resulted in more co-infections affecting survival in rheumatic disease, and CMV reactivation can lead to infections in granulomatosis with polyangiitis patients by inducing CD4+CD28- T cell and depressing naïve T cell populations.1-4Despite this evidence, the prognostic value of CMV reactivation for severe infections in patients with connective tissue disease are still unknown. Objectives:The aim of this study was to examine prognostic factors for severe infection during the early phase of treatment, especially in CMV reactivation, in patients with antineutrophil cytoplasmic autoantibody (ANCA)-associated vasculitis (AAV) who received initial high dose corticosteroid therapy (prednisolone > 0.8mg/kg/day).Methods:We analyzed the data of 88 consecutive hospitalized patients newly diagnosed with AAV at our hospital from January 2006 to March 2019 in this retrospective cohort study. There were 32 patients with CMV reactivation during remission induction therapy compared to 56 patients without CMV reactivation. CMV reactivation was defined by the detection of CMV pp65 antigen in blood samples, and CMV positive cells ≥ 5 per 3.0 × 105polymorphonuclear neutrophils (PMNs). The variable for severe infections within 180 days with apvalue < 0.1 in univariate analysis were selected for multivariate analysis using the Cox regression model. The positive predictive value (PPV) and positive likelihood ratio (PLR) of CMV reactivation for subsequent severe infections were also analyzed. Results:Patients with CMV reactivation, compared to those without, had a higher prevalence of MPO-ANCA, renal manifestation and renal impairment at diagnosis, received hemodialysis (HD), higher revised five factor score (FFS), older age, higher Birmingham Vasculitis Activity Score at diagnosis, and higher initial doses of corticosteroids (CS) at baseline. Revised FFS ≥ 2, renal involvement, high initial serum creatinine (≥ 1.5 mg/dl) at diagnosis, received HD, and CMV reactivation were associated with severe infections in the univariate analysis, although receiving cyclophosphamide or rituximab was not. Among these variables, CMV reactivation (Hazard ratio [HR] 3.50; 95% confidence interval [CI]: 1.22-10.10;p= 0.02) and high initial serum creatinine at diagnosis (HR 8.09; 95%CI: 2.00-32.73;p< 0.01) were independent risk factors for severe infections within 180 days. (Table 1) The PPV and PLR of CMV reactivation for subsequent severe infections were 35% and 1.91. When including higher initial serum creatinine, PPV and PLR for subsequent severe infections was 67% and 7.26.Table 1.Cox regression analysis for severe infections within 180 days.Univariate analysisMultivariate analysisPotential prognostic factorsHR (95% CI)P valueHR (95% CI)P valueAge ≥ 651.36 (0.48-3.71)0.580Male1.23 (0.50-3.04)0.648Past history of lung disease0.39 (0.11-1.36)0.140Past history of diabetes mellitus0.64 (0.15-2.77)0.550Lung involvement1.76 (0.67-4.62)0.254Renal involvement†3.68 (1.22-11.10)0.021Serum Cr ≥ 1.5 at diagnosis9.50 (3.40-26.49)< 0.0018.09 (2.00-32.73)0.003Hemodialysis4.85 (1.73-13.54)0.0030.96 (0.31-2.97)0.950BVAS ≥ 201.50 (0.59-3.81)0.393Revised FFS ≥ 24.40 (1.28-15.13)0.0180.83 (0.16-4.27)0.818MPSL pulse therapy1.16 (0.47-2.86)0.746Received CYC or RTX1.54 (0.55-4.27)0.409CMV reactivation5.10 (1.93-13.48)0.0013.50 (1.22-10.06)0.020† “Renal involvement” was excluded in the multivariate analysis to avoid multicollinearity. Conclusion:Our study shows that there should be focus on subsequent severe infections when CMV reactivation is detected during early phase of treatment, especially in renal-impaired patients with ANCA-associated vasculitis.References:[1]Gardiner BJ et al. Abstract:
Background:Anifrolumab treatment resulted in improved British Isles Lupus Assessment Group (BILAG)–based Composite Lupus Assessment (BICLA) response rates in patients with systemic lupus erythematosus (SLE) in the phase 3 TULIP-2 and TULIP-1 trials.1,2In addition, annualized flare rates were lower among the groups treated with anifrolumab compared with placebo.1,2Objectives:TULIP-2 and TULIP-1 data were analyzed to assess the effects of anifrolumab on the number of SLE flares and time to first flare during 52 weeks of treatment. Methods:The randomized, double-blind, placebo-controlled TULIP-2 and TULIP-1 trials evaluated efficacy and safety of intravenous anifrolumab 300 mg vs placebo every 4 weeks for 48 weeks, with the primary endpoints assessed at Week 52, in patients with moderate to severe SLE despite standard-of-care treatment. Flares were defined as ≥1 new BILAG-2004 A or ≥2 new (worsening) BILAG-2004 B domain scores compared with the prior month’s visit. Time to first flare was evaluated using a Cox proportional hazards model. Annualized flare rate was analyzed using a negative binomial regression model. Results:In TULIP-2 (anifrolumab, n=180; placebo, n=182) and TULIP-1 (anifrolumab, n=180; placebo, n=184), fewer patients experienced ≥1 BILAG-2004 flare in the anifrolumab groups (TULIP-2: 31.1%, n=56; TULIP-1: 36.1%, n=65) compared with the placebo groups (TULIP-2: 42.3%, n=77; TULIP-1: 43.5%, n=80; Figure 1). Results favoring anifrolumab were observed in time to first flare (TULIP-2: hazard ratio [HR] 0.65, 95% confidence interval [CI] 0.46–0.91 and TULIP-1: HR 0.76, 95% CI 0.55–1.06; Figure 2) and BILAG-based annualized flare rates (TULIP-2: adjusted rate ratio 0.67, 95% CI 0.48–0.94 and TULIP-1: rate ratio 0.83, 95% CI 0.60–1.14) across both trials. Conclusion:Across 2 phase 3 trials, we observed reductions in the total number of flares and annualized flare rates, as well as prolongation of time to first flare with anifrolumab treatment compared with placebo. These results support the potential of anifrolumab to reduce disease activity and reduce flares, benefiting patients with SLE.References:[1]Morand EF, et al.N Engl J Med. 2020;382:211–221.[2]Furie RA, et al.Lancet Rheumatol.2019;1:e208–e219.Disclosure of Interests:Richard Furie Grant/research support from: AstraZeneca, Biogen, Consultant of: AstraZeneca, Biogen, Eric F. Abstract:
BackgroundPatients with cutaneous lupus erythematosus (CLE) experience disfiguring and painful lesions that can lead to psychological distress and significant impacts on quality of life.1 Treatment of patients with systemic lupus erythematosus (SLE) with anifrolumab, a type I interferon receptor antagonist, was associated with CLE Disease Area and Severity Index–Activity (CLASI-A) improvements compared with placebo through Week 52 in the phase 3 TULIP-1 and TULIP-2 SLE trials.2,3 CLASI assesses overall skin improvement and may be driven by erythema over the other components.4ObjectivesTo better understand the effect of anifrolumab on mucocutaneous SLE manifestations by analyzing the individual domains of CLASI-A using pooled data from the TULIP trials.MethodsTULIP-1 (NCT02446912) and TULIP-2 (NCT02446899) were randomized, double-blind, placebo-controlled, 52-week trials that evaluated efficacy and safety of intravenous anifrolumab administered every 4 weeks in patients with moderate to severe SLE despite standard therapy.2,3 In a post hoc analysis, individual CLASI-A domains (erythema, scale/hypertrophy, alopecia, and mucosal) were assessed at Week 24 (time point chosen to ensure adequate duration for improvement in slow-remitting manifestations [eg, scale, alopecia]) in 2 patient subgroups: 1) the “chronic” mucocutaneous subgroup (resembling chronic/discoid CLE), defined as patients with a baseline erythema score ≥4 and scale score ≥2, and alopecia score ≥1 or baseline mucosal lesions or ulceration score of 1; and 2) the “extended” mucocutaneous subgroup (resembling all CLE subtypes), defined as patients who met the “chronic” criteria or those who had a baseline erythema score ≥8.ResultsAcross the pooled TULIP trials, 360 patients received anifrolumab 300 mg and 366 patients received placebo. In patients with assessments at Week 24 in the “chronic” (anifrolumab n=58, placebo n=50) and “extended” (anifrolumab n=104, placebo n=96) subgroups, anifrolumab led to a greater mean percent reduction from baseline compared with placebo in erythema (chronic: −63.6% vs −39.9%; extended: −63.7% vs −41.2%) and scale/hypertrophy (chronic: −72.2% vs −42.6%; extended: −45.3% vs −7.3%). Anifrolumab-treated patients in both subgroups had no worsening in alopecia (chronic: 93.3% [56/60] vs 96.0% [48/50]; extended: 95.3% [102/107] vs 95.8% [92/96]) or mucous membrane (chronic: 95.0% [57/60] vs 96.0% [48/50]; extended: 96.3% [103/107] vs 94.8% [91/96]) from baseline vs placebo (Table 1).Table 1.Skin Responses at Week 24 Compared With BaselineCriteria, n (%)Chronic subgroupExtended subgroupAnifrolumab 300 mg (n=60)Placebo (n=50)Anifrolumab 300 mg (n=107)Placebo (n=96)Erythema score reduction≥25%53 (88.3)32 (64.0)93 (86.9)68 (70.8)≥50%42 (70.0)22 (44.0)71 (66.4)47 (49.0)≥60%34 (56.7)18 (36.0)61 (57.0)32 (33.3)Scale/hypertrophy score reduction≥10%49 (81.7)34 (68.0)55 (51.4)38 (39.6)≥25%47 (78.3)30 (60.0)53 (49.5)34 (35.4)≥50%46 (76.7)28 (56.0)51 (47.7)31 (32.3)No new/worsened lesions in any individual body area44 (73.3)26 (52.0)81 (75.7)56 (58.3)Alopecia≥1-point decreasea25 (41.7)19 (38.0)35 (32.7)27 (28.1)No worsening56 (93.3)48 (96.0)102 (95.3)92 (95.8)Mucosal lesion/ulceration1-point decreaseb25 (41.7)13 (26.0)39 (36.4)23 (24.0)No worsening57 (95.0)48 (96.0)103 (96.3)91 (94.8)aIf baseline score ≥1.bIf baseline score =1.ConclusionIn the phase 3 TULIP trials, SLE patients with mucocutaneous manifestations treated with anifrolumab experienced numerical improvements in erythema and scale/hypertrophy and no worsening in alopecia or mucous membrane CLASI-A domains compared with placebo, regardless of whether skin disease was classified by chronic or extended definitions. These encouraging data support further evaluation of anifrolumab in patients with CLE.References[1]Klein R. J Am Acad Dermatol. 2011;64:849–58.[2]Furie RA. Abstract:
Abstract:
There’s a strange forgetfulness around the term fictocriticism as it’s used in Australia now - for fictocriticism made its appearance here in the writing (mostly non-academic) of women very well aware of those strange, exciting and provocative texts emanating first of all from France and then later from Canada from the late seventies onward - most influential were Helene Cixous’ manifesto ‘The Laugh of the Medusa’ and her polemical essay ‘Castration or Decapitation’, Luce Irigaray’s first two books and in particular the collection of essays This Sex Which Is Not One. In Australia, an awareness of this writing was manifest soon after in the work of a number of writers in Frictions, published by Sybylla Press in 1982 & edited by myself & Alison Tilson - one of the first collections of Australian women’s (fiction) writing edited from a feminist perspective (and still trying to figure out exactly what that meant). I would cite in this context the contributions of Sneja Gunew, Wendy Morgan, Kerryn Goldsworthy & myself most obviously - but also, less obviously, Anna Couani & Kathleen Mary Fallon. Meaghan Morris’ very critical reading of ecriture feminine which appeared in Hecate in 1978 was crucial, and the influence of Marion Campbell’s first novel, Lines of Flight (1985) is incalculable. Much of this writing blends essay and fiction, shifts suddenly between fiction and poetry, makes use of indeterminate forms like the prose poem, and also of lists, fables, cliches - all manner of literary detritus.

Abstract:
Background: The use of cytokine-blocking agents has been proposed to modulate the inflammatory response in patients with COVID-19. Tocilizumab and anakinra were included in the local protocol as an optional treatment in critically ill patients with acute respiratory distress syndrome (ARDS) by SARS-CoV-2 infection. This cohort study evaluated the effects of therapy with cytokine blocking agents on in-hospital mortality in COVID-19 patients requiring mechanical ventilation and admitted to intensive care unit. Methods: The association between therapy with tocilizumab or anakinra and in-hospital mortality was assessed in consecutive adult COVID-19 patients admitted to our ICU with moderate to severe ARDS. The association was evaluated by comparing patients who received to those who did not receive tocilizumab or anakinra and by using different multivariable Cox models adjusted for variables related to poor outcome, for the propensity to be treated with tocilizumab or anakinra and after patient matching. Results: Sixty-six patients who received immunotherapy (49 tocilizumab, 17 anakinra) and 28 patients who did not receive immunotherapy were included. The in-hospital crude mortality was 30,3% in treated patients and 50% in nontreated (OR 0.77, 95% CI 0.56-1.05, p=0.069). The adjusted Cox model showed an association between therapy with immunotherapy and in-hospital mortality (HR 0.40, 95% CI 0.19-0.83, p=0.015). This protective effect was further confirmed in the analysis adjusted for propensity score, in the propensity-matched cohort and in the cohort of patients with invasive mechanical ventilation within 2 hours after ICU admission. Conclusions: Although important limitations, our study showed that cytokine-blocking agents seem to be safe and to improve survival in COVID-19 patients admitted to ICU with ARDS and the need for mechanical ventilation. *Modena Covid-19 Working Group (MoCo19): Intensive Care Unit: Massimo Girardis, Alberto Andreotti, Emanuela Biagioni, Filippo Bondi, Stefano Busani, Giovanni Chierego, Marzia Scotti, Lucia Serio, Annamaria Ghirardini, Marco Sita, Stefano De Julis, Lara Donno, Lorenzo Dall’Ara, Fabrizio Di Salvo, Carlotta Farinelli, Laura Rinaldi, Ilaria Cavazzuti, Andrea Ghidoni, Antonio Buono, Elena Ferrari, Daniela Iseppi, Anna Maria Ardito, Irene Coloretti, Sophie Venturelli, Elena Munari, Martina Tosi, Erika Roat, Ilenia Gatto, Marco Sarti.Immuno-Lab: Andrea Cossarizza, Caterina Bellinazzi, Rebecca Borella, Sara De Biasi, Anna De Gaetano, Lucia Fidanza, Lara Gibellini, Anna Iannone, Domenico Lo Tartaro, Marco Mattioli, Milena Nasi, Annamaria Paolini, Marcello Pinti. Abstract:
