Abstract:
Nowadays herbal medicines are popular all over the world. Drawback is consistency in the quality of the product over a range of time. Any considerable changes in the quality of the product over a time must be detectable. So stability testing is needed to ensure the quality of herbal products which is an evidence for the quality of the finished product. Present study reports the deviation in Pharmacopoeial constants of Hutabhugadi curna (HC) and Sitopaladi curna (SC) when stored at a recommended storage conditions and examined for a period of time. The variation in the Pharmacopoeial physico-chemical constants of the curna (fine powder) was observed during 0 (initial), 3rd and 6th month. Average shelf life is proposed for HC and SC based on 10% variation in physico-chemical tests specified in pharmacopoeia while compared to initial value. It was found that HC showed average ±10% variation in the values at 11.41 months while SC showed it at 22.58 months. This method of shelf life assessment can be used as routine shelf life check for polyherbal herbal formulations though this method is not bioavailability based assay applicable for single herb formulations.

Clara ha sido profesora de inglés durante 5 años en el Colegio Santa María La Blanca y los últimos tres años ha realizado las funciones de coordinadora del departamento de bilingüismo. Guillermo cuenta con una experiencia de dos años como profesor de Primaria en el Colegio Monte Tabor y otros tres años como profesor de español en la India. Ambos tienen claro que pese a ser muy lenta y a largo plazo, no hay mejor herramienta que la educación para crear agentes de cambio en una de las sociedades menos desarrolladas, tanto económica como socialmente, de todo el planeta.

L’opuscule La Vision en Dieu constitue le point de départ pour une lecture plus élargie de la philosophie de Nicolas de Cues en tant que theorèse de l’Absolu, lequel, nonobstant son inconnaissance dernière, néanmoins se réfléchit en quelque façon dans la pluralité de la pensée humaine, qui dans et à partir du monde cherche à l’atteindre. Nous présentons dans ses principaux moments le processus gnosique de la pensée dans sa dialéctique de ascensus-descensus entre l’intelligible et le sensible, l’un et le multiple, sans oublier ni le côté mystique ni le côté déjà bien moderne de cette philosophie. Ainsi, nous exposons soit la méthodologie conduisant à l’intuition noétique de l’Absolu, soit les instruments conceptuels qui permettent à l’homme de se construire, en tant que microcosmos, sa représentation du monde. Pour conclure, nous soulignons le caractère déjà bien moderne de l’anthropologie du Cardinal, qui nous révéle bien le lieu historial de sa philosophie, à demi chemin entre le Moyen-Âge et la Modernité.

Sebbene l'autenticit delle tre lettere sia stata accettata dalla maggior parte degli studiosi colombiani, l'intera corrispondenza stata negata da alcuni, e le due lettere a Colombo da altri. In giuoco almeno una parte della stima per la scoperta del Mondo Nuovo. Martins stato spesso identificato col canonico portoghese Fernando de Roritz, che stato testimone al testamento di Niccol di Cusa insieme con Paolo dal Pozzo Toscanelli, ma nessun'altra prova della sua conoscenza con Toscanelli stata finora conosciuta. Nuovi documenti adesso confermano la presenza del Roritz a Firenze nel 1459, quando egli ha servito accanto al Toscanelli come medico del cardinale Giacomo di Portogallo ed stato testimone al testamento del cardinale, e di nuovo nel 1466, quando stato testimone all'atto di dote della cappella del cardinale in San Miniato al Monte.

ABSTRCT | ITA Confrontarsi con l’acqua come ambito del progetto richiede un esercizio di sguardo e di concettualizzazione – intellettuale, grafica e spaziale – in grado di verificare e rinnovare molte delle tassonomie con cui comunemente si dispongono le forme, gli stati e i comportamenti di questa materia. Il testo che segue riflette sull’opportunità di interrogare le separazioni tra terra e acqua e volgersi piuttosto a considerare il paesaggio come un’inevitabile condizione anfibia, trovando indizi nelle ricerche di Anuradha Mathur e Dilip da Cuhna e nei progetti di alcuni luoghi iconici del patrimonio monumentale di Roma. Sebbene lontanissimi, per collocazione geografica e temporale, i due riferimenti convergono nel destabilizzare alcune convinzioni accreditate – ad esempio, l’esistenza dei fiumi, la separazione tra fiumi e città o tra edifici e spazi aperti – dimostrando che il paesaggio è il dominio dell’umidità con concentrazioni differenziate e che occorre aggiornare l’immaginario idrologico corrente per renderlo capace di generare nuovi strumenti ideativi e operativi, utili al progetto di paesaggio. ABSTRCT | ENG Dealing with water as a field of design asks for making practices of gaze and conceptualization – of ideas, graphics, and spaces – to verify and renovate many taxonomies we usually consider for setting the shapes, states, and behaviours of this substance. The following paper ponders the chance to question the parting between water and land, and to consider landscape as an unescapable amphibious condition, looking for clues both in the research of Anuradha Mathur and Dilip da Cuhna and in the projects of some iconic sites of the monumental heritage in Rome. Even though they are so far, for geographical and temporal belonging, the two references converge toward destabilizing some granted ideas – such as, the existence of rivers, the separation between rives and cities, or between buildings and open spaces – giving evidence that landscape is the domain of wetness with different concentration and that we need to update our current hydrological imaginary to make it able to generate new conceiving and operating tools, useful to landscape design.

Abstract:
Children in situations of destitution who become institutionalized commonly display developmental disorders, including delayed growth. The aim was to evaluate the environmental quality of the casas cuna of the Department of Chuquisaca (Plurinational state of Bolivia) in children aged 0 to 2 years old after receiving an early stimulation program based on psychomotor therapy. Thirty-six children who were institutionalized at shelter homes in the Department of Chuquisaca were selected to receive sessions of psychomotricity over a five-month period. The Infant/Toddler Home Observation for Measurement of the Environment (IT-HOME) scale and the Attachment During Stress Scale (ADS) were used. The adult−child relationship with factors of responsiveness (−0.89; p = 0.037), acceptance (0.57; p = 0.024), organization (−1.03; p < 0.001), learning material (−2.57; p < 0.001) and involvement (−1.92; p < 0.001) scored below expectations, showing that environmental indicators are a poor stimulation for children growing up in shelter homes. Improvements were found in the children’s development after receiving this therapy. In conclusion, an early stimulation program based on psychomotor therapy over five months provided favorable results for the acquisition of skills for communication, motor development and social skills, which positively affect the psychomotor development.

No comêço, esta nova doutrina teve razões utilitárias: quefazer medir a quantidade de Informação contida em determinada mensagem?Como definir a Informação transportada por um sistema de sinais telegráficos:pontos, traços e pausas do código Morse, por exemplo? Enfim, comoencontrar o código ótimo, o sistema de símbolos caoazes de traduzir a mensagemem sinais telegráficos da maneira mais econômica possível, sem nadaperder da Informação que se deve transmitir? Êste exemplo indica as preocupaçõesde um engenheiro telegráfico. ABSTRACTObjectives: to investigate how the colostomy linked to the Support Coordination Health of Disabled People of the State Department of Health in Montes Claros/MG is perceived socially and evaluate their quality of life; to verify the location and number of deletions/day in their influence daily practices and to determine whether, in that it feels meets, the colostomy is socially reintegrated. Method: this is a descriptive and exploratory study from quantitative approach using WHOQOL-bref scale assessing overall quality of life and a questionnaire developed by the authors. Research universe: 40 colostomists Montes-Clarenses. Casuistical: 19 patients studied. Research Ethics Committee of the State University of Montes Claros: Protocol 802. Results: mean scores/high quality of life and general acceptance of physical appearance. Statistical significance for the variable meaning of life 'versus' use of life. Conclusions: colostomy as a positive factor for longevity and rehabilitation of patients in this study, however, with direct consequences on the quality of life, quality of life such as neutral by most subjects, revealed a statistical significance (p = 0.001) in meaning to life versus enjoyment of life, need to deepen the problem that involves the fact that colostomy and their situations, especially qualitatively. Método: estudo descritivo exploratório, quantitativo utilizando-se da escala WHOQOL–bref avaliando a qualidade de vida geral e o questionário elaborado pelos autores. O universo de pesquisa foi 40 colostomizados Montes-Clarenses cuna casuística foi de 19 pacientes estudados após a aprovação pelo Comitê Ética em Pesquisa da Universidade Estadual de Montes Claros com protocolo 802. Resultados: escores médios/altos de qualidade de vida geral e aceitação de aparência física. Significância estatística para a variável sentido para a vida ‘versus’ aproveitamento de vida. Conclusões: colostomia como fator positivo para longevidade e reabilitação dos pacientes em estudo, porém, com reflexos diretos sobre a qualidade de vida destes; qualidade de vida referida como neutra pela maioria dos sujeitos; evidenciou-se significância estatística (p= 0,001), em sentido para a vida versus aproveitamento da vida; necessidade de aprofundamento na problemática que envolve o fato de ser colostomizado e suas conjunturas, sobretudo qualitativamente. Abstract:
Blood loss is the major cause of postoperative mortality and morbidity associated with hepatic resection. A prospective multicenter study was conducted to determine if ultrasonic dissectors (USD) were useful in hepatic resection and could reduce this hemorrhagic risk. Forty-seven hepatic resections were performed in 42 consecutive patients during a two month period in 11 public, surgical centers. Twenty-one patients had primary or secondary malignancies, six had benign tumors, two had biliary cysts, one had cholangiocarcinoma, one had Caroli’s disease, and 11 had hydatid cysts of the liver. Two different USD devices were evaluated (CUSA System-Lasersonics and NIIC-DX 101 T). The hepatic resections tested included a wide range of procedures. Each surgeon had the possibility of choosing between the USD and his own usual technique for each operative step and according to local conditions. The average volume of blood infused, irrespective of the underlying pathology or the procedure performed, was 1.0 L (range 0-4.8 L). Fourteen patients required no transfusions. No operative or immediate postoperative deaths were recorded. Five major complications, all unrelated to the use of the USD, developed in three patients. Access to intra and extraparenchymal arterial and venous tributaries and particularly the control of the hepatic veins were facilitated by USD. While transection of hepatic parenchyma was neither easier nor faster than with conventional techniques, it was found to be less hemorrhagic. Overall appraisal was expressed on an analog scale; the USD was found to be helpful or very helpful in 75 percent of all resections. With regard to the pathology being treated, total or partial excision of hydatid cysts was greatly enhanced by the use of the USD while this benefit was not found for wedge resections of other hepatic lesions. With regard to user friendliness and maintenance, the NIIC-DX 101 T device was preferred. We conclude that the USD facilitates formal hepatic resections. Converging opinions emerging from various surgical centers reinforce this conclusion.

Background:Tofacitinib (TOF), an oral JAK inhibitor, is approved for the treatment of rheumatoid arthritis (RA) either as monotherapy or in combination with background methotrexate (MTX). Despite the current evidence of efficacy from randomized controlled trials and open-label long-term extension studies, evidence of effectiveness and safety in real-world settings is limited, not only in Argentina but also in Latin America.Objectives:To describe effectiveness, safety and persistence of TOF therapy in RA patients from public and private medical centers from Argentina. In addition, establish prognostic factors for clinical remission at 3 months and TOF monotherapy at 12 months.Methods:A retrospective, observational and multicentre study was performed from an analysis of medical records of 10 medical centers. RA patients (ACR/EULAR, 2010) and age ≥ 18 years who had received or are under treatment with TOF until June 2020 were included. The data collection was done on a standard database that included baseline data and at 3, 6 and 12 months. Clinical remission was defined as DAS28-ESR < 2,6. Adverse events, treatment duration, TOF treatment persistence at last visit and discontinuation cause were assessed. Comparison to baseline values was performed using Wilcoxon sign for numerical variables and McNemar´s test for categorical variables. Treatment persistence was analyzed using Kaplan Meier´s technique. Multivariate analysis was performed using R software and its library packages (Lme4, Tidyverse and ggpubr). A p value < 0.05 was considered significant.Results:A total of 167 patients were included (78.4% were female). At baseline, the median age was 53 years (IQR 43-63 years), median disease duration was 4 years (IQR 2-13 years). RF was positive in 85.6% of patients, ACPA in 80.8% and structural radiological damage was present in 71.8%. Previous use of MTX was 97%, leflunomide 74.8% and biologic therapy 42.5% (28.74% 1 biologic, 11.98% 2 biologics and 1.8% ≥ 3 biologics). TOF dose: 48% 11 mg/day and 52% 5 mg BID. A statistically significant difference was observed not only in disease activity (p<0.0001) but also in the requirement of MTX and PDN (p<0.0001) in the 12 months evaluated. Remission significantly increased from baseline to month 3 and to a much lesser extent to month 6 (p < 0.001). The mean duration of treatment with TOF was 20.10 ± 15.25 months. Treatment persistence was 93.84% at 3 months and 91.24% at 6 months. In those patient who achieved REM at month 3, a statistically significant differences in duration of RA (p 0.0002), structural radiological damage (p 0.011), basal disease activity (p 0.018) and prior treatment with biological therapy (p 0.017) was found when compared with patients who remained active. Furthermore, in univariate logistic regression analysis, 5 years or more of disease duration was associated with a 3 times higher risk of not achieving clinical remission at 3 months (odds ratio = 0.35, 95% CI = 0.15-0.83). In the multivariate logistic regression analysis, previous biological therapy was the only predictor associated with a decrease in the probability of clinical remission (p < 0.008). Adverse events were registered in 26 patients (herpes zoster, n = 9).Conclusion:The effectiveness of TOF was observed not only in the clinical response achieved but also in the dose titration or withdrawal of MTX and PDN. The safety profile did not show any difference from long-term extension studies. At 12 months, 86.81% of the patients persisted with TOF therapy.
