Abstract:
This chapter discusses the role and responsibilities of monitors/CRAs seeking to improve awareness of their role in a global industry becoming more and more technologically focused. How the role is changing through the introduction of risk based monitoring. Often the only liaison between the Sponsor and each study site, CRAs are crucial to the successful completion of a clinical trial and need to be armed with appropriate knowledge and training to conduct their visits. Successful monitoring requires experience, people skills, management ability and knowledge – of the protocol, CRFs, study drug/device, therapeutic area, regulations and SOPs. In this chapter this spectrum of roles and responsibilities is presented in a concise and understandable format. Questions such as: What type of person makes a good monitor? EDC and/or paper? How – and how often – to monitor? are addressed. There are helpful tips and strategies on a variety of topics, notably preparing a monitoring plan; how to identify and assess potential investigators; preparing for a Study Initiation Visit; eCRFs and remote monitoring; thorough Source Data Verification; how to report monitoring visits plus example checklists associated with site visits and review of the Investigator Site File. The guidance provided in this chapter should help CRAs perform their essential role in encouraging good, high quality research - and provide an insight into the CRA role to those whose work is being monitored.

Abstract:
‘Ventilator Monitoring’ describes that group of functions that enables us to understand the functional status of a ventilator system and the ventilated patient. This chapter begins by introducing general monitoring concepts, describing the operation of the flow sensors and oxygen sensors that make the measurements, which are displayed as numerical monitoring parameters, waveforms, dynamic loops, and trend curves. The chapter details common monitoring parameters for pressure, flow, volume, time, and oxygen concentration. Examples of normal and abnormal ventilator graphics are shown. Finally, the chapter details each typical monitoring parameter and gives background information about its significance.

The subject of how growth should be monitored and recorded was addressed by the Royal College of Paediatrics and Child Health Growth Chart working group in the process of developing and implementing the UK-World Health Organization growth charts between 2008 and 2012. The group developed chart instructions and supporting educational material, drawing on the collective views of focus groups, stakeholder meetings, and the considered views of the working group. These have been expanded or modified where there is newer relevant evidence and the section is also informed by the 2008 National Institute for Health and Care Excellence guideline on maternal and child nutrition and the new guideline on faltering growth.

Full text
Abstract:
Haemostasis is a dynamic process to stop bleeding after vessel wall damage. Platelets form a platelet plug via activation, adherence, and aggregation processes. The coagulation proteins are activated one-by-one, cascading towards fibrin polymerization, a process controlled by thrombin generation. Fibrinolysis is the process responsible for fibrin mesh degradation, which is also controlled by thrombin. Besides procoagulant proteins, anticoagulant proteins maintain a balance in the haemostatic system. Measuring platelet count and function can be done as part of the monitoring of haemostasis, while coagulation times are measured to assess the coagulation proteins. Degradation products of fibrin and lysis times give information about fibrinolysis. Point-of-care monitoring provides simple, rapid bedside testing for platelets and for whole blood using viscoelasticity properties. In trauma-induced coagulopathy (TIC) platelet counts and coagulation times are still common practice to evaluate haemostasis, but point-of-care measurements are being used more and more. Medication interfering with haemostasis is frequently used in intensive care unit patients. Each (group of) drug(s) has its own monitoring tests either based on classical or novel techniques.

Abstract:
Chapter 25 introduced some basic generic principles applicable to many measurement and monitoring techniques. Chapter 43 introduces those principles not covered in Chapter 25 and discusses in detail the clinical applications and limitations of the many monitoring techniques available to the modern clinical anaesthetist. It starts with non-invasive blood pressure measurement, including clinical and automated techniques. This is followed by techniques of direct blood pressure measurement, noting that transducers and calibration have been discussed in Chapter 25. This is followed by electrocardiography. There then follows a section on the different methods of measuring cardiac output, including the pulmonary artery catheter, the application of ultrasound in echocardiography, pulse contour analysis (LiDCO™ and PiCCO™), and transthoracic electrical impedance. Pulse oximetry is then discussed in some detail. Depth of anaesthesia monitoring is then described, starting with the electroencephalogram and its application in BIS™ monitors, the use of evoked potentials, and entropy. There then follow sections on gas pressure measurement in cylinders and in breathing systems, followed by gas volume and flow measurement, including the rotameter, spirometry, and the pneumotachograph, and the measurement of lung dead space and functional residual capacity using body plethysmography and dilution techniques. The final section is on respiratory gas analysis, starting with light refractometry as the standard against which other techniques are compared, infrared spectroscopy, mass spectrometry, and Raman spectroscopy (the principles of these techniques having been introduced in Chapter 25), piezoelectric and paramagnetic analysers, polarography and fuel cells, and blood gas analysis.

Abstract:
The outside of the operating room (OOOR) environment is fraught with challenges and often requires a great deal of flexibility without compromising patient care. The expertise and skill of the modern anesthesiologist is increasingly required when anesthesia is administered for procedures performed OOOR. This chapter focuses on the physics, physiology, limitations, and recommendations for standard physiological monitors that should be utilized in the OOOR environment. A special emphasis is placed on pulse oximetry and capnography. By implementing standards for monitoring that are similar to standards used in the operating room, the safe delivery of an anesthetic for procedures in the OOOR environment can be consistently achieved.

Pulmonary function tests in critical illness 90End-tidal CO2 monitoring 92Pulse oximetry 94Pulmonary function test results in critically ill patients can be important prognostically and guide ventilatory and weaning strategies. However, they are not straightforward to measure in mechanically ventilated patients and remain limited to dynamic volumes. Fortunately, most modern mechanical ventilators are able to calculate and display static and dynamic lung volumes, together with derived values for airway resistance, compliance and flow/volume/time curves. The ability to monitor these changes after altering ventilatory parameters has enabled more sophisticated adjustments of ventilation, to prevent potentially damaging mechanical ventilation....

Intracranial pressure monitoring 130Intracranial perfusion 132EEG and CFAM monitoring 134Other forms of neurological monitoring 138In adults, the normal resting intracranial pressure (ICP) is 0–10mm Hg. ICP may rise to 50mm Hg or so during straining or sneezing, with no impairment in function. It is not, therefore, ICP alone that is important but rather the interpretation of the measurement in pathological conditions. Many of the clinicopathological changes associated with brain injury are the result of pressure differences between the intracranial compartments, with consequent shift of brain structures, rather than the absolute level of ICP....
