Abstract:
Abstract In 2016, the World Health Organization set „Viral hepatitis elimination by 2030” as a goal, which is predicated upon 2 advances: on the one hand, the efficacy of hepatitis B vaccine and, on the other, the sustained viral response to direct-acting antiviral (DAA) therapy for infection with hepatitis C virus (HCV). Romania also adopted this initiative considering the available results confirming the efficiency of this policy. Viral clearance results in reduced morbidity and mortality by liver injury and also by any associated HCV infection manifestations. In order to fulfill the target stated by the WHO any patient with detectable viremia must be treated. Unlike interferon-based regimes, DAA therapy for HCV infection is applicable to all groups of patients, regardless of the severity of their liver injury. Other advantages are high efficiency levels, proved in clinical trials as well as in the real world, oral administration route, good safety profile, a very good rate of sustained virological response in patients with decompensated cirrhosis and the possibility of prescribing them to children older than 8. Despite the remarkable progress, an ideal antiviral therapy has not yet been discovered. The direct-acting antiviral therapy in use today has 3 targets: NS3/4A, NS5A, NS5B. None of the drugs is designed to be used alone. For convenience, the pharmaceutical industry has developed pills containing combinations of 2 or 3 active drugs, which completely changed the hepatitis C treatment paradigm.

We look at certain aspects of Carl Schmitt’s political and legal doctrine that were seen as relevant in XX century and are currently reflected in modern internal political processes. We analyze a range of concepts having a special interpretation in decisionism and offer examples from modern legal practice to support the concepts in question.

Abstract:
In modern-day Athens the fortification walls, built both in ancient times and subsequent periods, are not currently noticeable to the casual observer, despite having defined for long periods the city’s life. In fact, the majority of modern Athenians and visitors do not know of their existence since their remains are inconspicuous as a result of aggressive modern building schemes. While these remains are not materially magnificent, they are so historically as a mirror of the city’s fate.

Abstract:
Using a safe subset of C++ is a promising direction for increasing the safety of the programming language while maintaining its performance and productivity. In this paper, we examine how close existing C/C++ code is to conforming to a safe subset of C++. We examine the rules presented in existing safe C/C++ standards and safe C/C++ subsets. We analyze the code characteristics of 5.8 million code samples from the Exebench benchmark suite, two C/C++ benchmark suites, and five modern C++ applications using a static analysis tool. We find that raw pointers, unsafe casts, and unsafe library functions are used in both C/C++ code at large and in modern C++ applications. In general, C/C++ code at large does not differ much from modern C++ code, and continued work will be required to transition from existing C/C++ code to a safe subset of C++.

Abstract:
Aim. Assess the current diagnostic value of clinical laboratory markers of liver damage in chronic viral hepatitis C. Materials and methods. Comprehensive clinical examination of 194 patients with the diagnosis chronic hepatitis C and 73 almost healthy faces was conducted. Results. According to the clinical examination, 91% of patients have hepatomegaly. According to the results of elastography, the subgroup without fibrosis F0 49 (25%) people, with fibrosis of stages F1-F3-145 (75%) patients. The occurrence of HCV genotypes was: HCV-1-33%, HCV-2-12% and HCV-3-55%. The virusemia indicator showed large variations of values. During the research it was established that indirect indicators of a fibrosis of a liver: levels of AST and ALT, GGTP, a direct and general bilirubin, the APRI index are authentically exceeded by indexes of control group while the level of thrombocytes and de Ritis’s coefficient authentically decrease. Conclusions. The current course of chronic hepatitis C occurs with hepatomegalia, often associated with gastrointestinal and cardiovascular damage. The most common virus among patients is the HVC-3 genotype virus. When examining indirect markers of fibrosis in viral hepatitis C, the APRI test is an early predictor and has high predictive value. Platelet levels decrease with viral liver damage. As a consequence, the APRI index increases and the de Ritis ratio decreases.

Abstract:
AbstractAlthough Naples was one of Europe’s largest cities (after London and Paris), studies of the management of its water supply during the early modern period are sorely lacking, despite growing interest in the subject at both an Italian and European level. Naples was perhaps unique in relying on a vast and tortuous underground network of reservoirs, cisterns, channels and conduits, accessed by well shafts, all fed by an ancient aqueduct. The present study outlines and evaluates the Neapolitan water supply as it existed in the period, analysing the archival records of the municipal tribunal responsible for the city’s infrastructure, the ‘Tribunale della Fortificazione, Acqua e Mattonata’, and its various ‘Appuntamenti’ (proposals), ‘Conclusioni’ (decisions) and edicts. This is interwoven with reference to pertinent printed accounts, from contemporary guide books to medical regimens and health manuals. We examine both water quantity, in terms of availability and accessibility (by looking at the structure and its management, and the technicians responsible for its maintenance) and water quality (by looking at contemporary attitudes and perceptions). In the process we are able to question the widespread view of early modern Naples as chaotic and uncontrolled, governed by a weak public authority, as well as widely held assumptions about the “inertia” of the pre-modern hydro-social system more generally.

Abstract:
The article examines the main aspects of the adjustment of political and work immigrants from the South Caucasus (Georgia, Armenia, Azerbaijan) who settled in Switzerland in the 1990s and 2000s. Field ethnographic materials were collected in 2018-2019. In the course of the study, the peculiarities of the perception of the culture, lifestyle, and the value system of the Swiss were analyzed. Separately, the author examined the types of immigrants’ employment in the new country. The article’s main focus is the age factor in the adjustment and integration processes. The study also addresses the problems of the preservation of Caucasian cultures and languages under the conditions of immigration and the formation of communication environments for South Caucasians in Switzerland. The author concludes that the South Caucasian immigrants are united by professional and social criteria rather than ethnic kinship. The adults adapt to the new conditions, and the youth integrates and acquires a Swiss identity. Elements of Caucasian cultures, languages and environment are gradually being lost in Switzerland.

The formation and disintegration of the U.S.S.R. was one of the vital historical events of the 20th century. The Central Asian republics were established by the Soviet government in the 1920s and 1930s as a result of the national and territorial state delimitation, and the borders between them were provisional. As the republics obtained political independence, the process of defining state borders between the new sovereign countries began. This process turned out to be difficult for Uzbekistan, among others, but the republic was mostly able to mutually resolve interstate borders issues based on “uti possidetis.”

The article presents the basic principles of antiviral therapy for hepatitis C. The main objectives and importance of antiviral therapy for hepatitis C are considered. The concept of a sustained virological response is given. The necessity of determining the causal relationship between infection with the hepatitis C virus and liver damage is substantiated. The article discusses the principles of antiviral therapy using interferon and interferon-free therapy with direct-acting antiviral drugs in both patients with acute and chronic hepatitis C.
