Full textThe actual gold standard to exclude the malignant nature of thyroid nodules in the clinical routine is represented by thyroid Fine Needle Aspirations (FNAs) biopsies. Thyroid FNAs are safe and cost-effective. Approximately the 20-30% of cases have an indeterminate for malignancy final report. These patients undergo diagnostic (and not therapeutic) thyroidectomy, but after surgery the 80% of these thyroid nodules are benign. This overtreatment has of course important consequences in the quality of life of the patients and high healthcare costs. The application of -omics techniques might have a potential role in the research for new diagnostic markers able to discriminate benign from malignant nodules, thus minimizing the challenging cases of indeterminate for malignancy. Mass spectrometry is one of the most important analytical tools able to obtain information regarding the molecular composition of a sample, the presence of biomolecules and their abundance. Among the different proteomics approaches able to extract the molecular alterations of the different type of specimen’s lesion, Matrix-Assisted Laser Desorption/Ionization (MALDI) Mass Spectrometry Imaging (MSI) was strongly emerging. MALDI-MSI represents an ideal technology that enables to explore the spatial distribution of biomolecules within tissue, integrating molecular and traditional morphological information while preserving the integrity of the analysed tissue. Various studies applied MALDI-MSI technology for prognostic purposes and for in real time diagnostic setting, showing the usefulness, advantages and applicability of MALDI-MSI in different fields of pathology. Due to the promising results recently obtained with MALDI-MSI in the identification of proteomic signals able to differentiate between benign and malignant cases from the analysis of thyroid tissue after surgery , the idea was to apply for the first time MALDI-MSI on real thyroid FNAs biopsies. Preliminary to the clinical study, the protocol for the proteomic MALDI-MSI analysis was optimised to avoid degradation, alteration phenomena, contamination and artefacts formation. The methodological improvement of the protocol in a complicated field as thyroid cytological specimens played an important role in this study. Challenging technical aspects, such as i) the interference of haemoglobin due to the high vascularization of the thyroid organ and ii) the stability of the samples over time before the analysis from a morphological and proteomic point of view, were overcome through two studies that were planned and analysed as part of the thesis. The clinical study for the detection of the potential cluster of signals with discriminant capability was originally planned to involve a large sample of thyroid nodules, however, due to the slow enrolment rate of malignant cases, the thesis contains only the results of a preliminary analysis. Eighteen subjects contributed to the training set with 9 benign and 9 malignant thyroid nodules. The statistical model was based on data of 81 specific region of interest, according to the morphological triage performed by the pathologist in order to overcome false information deriving from non thyrocytes cells. The validation phase was performed on 11 patients with different type of lesions (i.e. benign, indeterminate and malignant). Results are very promising and highlight the possibility to introduce MALDI-MSI as a complementary tool for the diagnostic characterization of thyroid lesions. A methodological aspect that emerged from the peculiarity of the proteomic analysis was also investigated. A review of the most used indices for the assessment of the similarity between mass spectra profiles was performed and a new measure was proposed. A simulation study was implemented in order to identify the best similarity measure to use in comparing proteomic profiles.

Este trabalho apresenta uma proposta metodológica para um diagnóstico ambiental em estabelecimentos de saúde, visando, com ênfase nos critérios de gerenciamento ambiental, identificar os pontos positivos e negativos do manejo de resíduos de serviços de saúde dos estabelecimentos, a fim de poder propor ações de melhorias no desempenho ambiental dos mesmos. O modelo Diagnóstico Ambiental em Estabelecimentos de Saúde # DAES, proposto, tem como fundamentação teórica, para a elaboração de seus critérios, a legislação vigente sobre o manejo e disposição final de resíduos de serviços de saúde (RSS), constantes nas Resoluções da Agência de Vigilância Sanitária e Conselho Nacional de Meio Ambiente (RDC 33/03 e, 5/93 e 283/01, respectivamente), contando ainda, com as disposições das Normas da Associação Brasileira de Normas Técnicas (ABNT) sobre o assunto. O gerenciamento das rotinas e procedimentos dos estabelecimentos, com base nestas legislações, permite administrar as relações entre as atividades do estabelecimento e o meio ambiente. Assim, buscando validar o modelo proposto, aplicou-se, o mesmo, em 26 estabelecimentos de saúde de Santa Catarina, o que produziu um diagnóstico ambiental do setor. Com base no modelo aplicado foi possível constatar que a ferramenta é de fácil aplicação e análise, além de poder ser considerada uma ferramenta de sensibilização para a administração, quanto a necessidade de melhoras no gerenciamento ambiental do estabelecimento, mostrando-se adequada para uma avaliação inicial do desempenho ambiental do mesmo.

Resumo: Representações externas são muito utilizadas no ensino de bioquímica. As vias metabólicas constituem um tipo de representação externa e envolvem habilidades visuais; estão, portanto, intimamente relacionados à literacia visual (capacidade de interpretar imagens). Há uma preocupação crescente em diagnosticar a dificuldade dos estudantes com a bioquímica. Este tipo de diagnóstico é importante para poder propor soluções pedagógicas que possam facilitar o aprendizado. O presente trabalho tem como objetivo descrever o desenvolvimento e a aplicação de um teste informatizado para diagnosticar a literacia visual, relacionada ao estudo de vias metabólicas, de estudantes de cursos das áreas biológica e da saúde. Inicialmente foram desenvolvidos testes piloto com itens (modelos e questões) que contemplavam habilidades visuais para a compreensão de vias metabólicas mais frequentemente representadas em livros didáticos de bioquímica. Os testes piloto foram aplicados em 2007 e 2008 com turmas de graduação de cursos da área biológica e da saúde. Os resultados obtidos serviram de subsídio para que alguns itens fossem reformulados e outros adicionados. Os resultados obtidos foram tratados com uso do modelo de Rasch, tanto no processo de equalização dos testes piloto (para definir os itens do teste final), como para o tratamento dos resultados obtidos com a calibração de itens-respondentes após as aplicações do teste final. Três grupos participaram da aplicação do teste final e compreenderam 79 estudantes distribuídos entre os cursos de educação física (UNICAMP), enfermagem (UNICAMP) e fisioterapia/nutrição (FAJ). A partir dos dados obtidos utilizou-se o processo de calibração de itens-respondentes, proposta pelo modelo de Rasch, obtendo-se a dificuldade dos itens e as habilidades visuais apresentadas pelos grupos. Os resultados observados sugerem que entre as dificuldades apresentadas pelos estudantes no aprendizado de metabolismo pode ser incluída a dificuldade em se compreender as representações externas de vias metabólicas. Essa dificuldade pode ser minimizada com explicações dos modelos antes de se entrar nos conteúdos específicos. O teste produzido permite o diagnóstico de estudantes ou de grupos. A necessidade de desenvolver práticas de visualização para melhorar a compreensão de conceitos bioquímicos pelos estudantes mostrou-se um aspecto importante no estudo de vias metabólicas
Abstract: External representations are used in teaching of biochemistry. The metabolic pathways are a type of external representation and involve visual skills, are therefore closely related to visual literacy (ability to interpret images). There is an increasing concern regarding the diagnosis of students with difficulty to the biochemistry, this type of diagnosis is important to be able to propose solutions that would facilitate learning. This paper aims to describe the development and implementation of a computerized test to diagnose the visual literacy, related to the study of metabolic pathways for undergraduate students enrolled in courses in the biological and health. Initially, pilot tests have been developed with (models and items) that looked visual skills for the understanding of metabolic pathways most often represented in textbooks of biochemistry. The pilot tests were applied in 2007 and 2008 on undergraduate classes of various courses in the biological and health. The results served as a subsidy for some items were reformulated and others added. The results are processed using the Rasch model, both in the process of equalizing the pilot tests (for set of the final test) and for the treatment of the results obtained with calibration of items-respondents the applications for the final test. Three groups participated in the implementation of the final test and included 79 students distributed among the courses of physical education (UNICAMP), nursing (UNICAMP) and physiotherapy/nutrition (FAJ). From the data we used the calibration of items-respondents proposed by Rasch model resulting in the difficulty of items and visual skills presented by the groups. The results suggest that among the difficulties presented by the students in the learning of metabolism may be included the difficulty in understanding the external representations of metabolic maps. This difficulty can be minimized with the explanations of the models before they enter the specific content. The test produced allows the diagnosis of students or groups. The need of developing imagery to improve the understanding of biochemical concepts by students were an important aspect in the study of metabolic pathways

Full textIn the master‘s thesis, the scientifie literature of Lithuanian and forein authors, regarding bankruptcy of the companies, is analyzed and systematized. The reasons of the bankrupcy, orginating in the company and their consequences for the country‘s economics and discussed. The methods of the bankruptcy forecasting, according to the theoretical aspect are analyzes. In the practical part, the comparative analysis of the statistical data and generalization is carried out. The reasons and consequences of the alteration are disputed as well. In addition, the evaluation of the bankruptcy reasons of the next door to the bankruptcy companies is performed. In the empirical part of the present paper, the bankruptcy forecasting are distinguished. After analyzing, the raised hypothesis has proved out, that there is not a single particular model of the bankruptcy forecasting, which would suit all companies.

Resumo: A classe de modelos Birnbaum-Saunders (BS) surgiu em problemas de fadiga dos materiais (que 'e um dano estrutural que ocorre quando um material é exposto a estresse e tensão). Nos últimos tempos, este modelo tem sido aplicado em áreas fora do contexto de fadiga dos materiais e engenharia, como por exemplo, em ciências da saúde, ambiental, florestal, demográficas, atuarial, financeira, entre outras. Tendo em vista que a distribuição BS tem a propriedade de descrever processos de degradação acumulativa. Neste trabalho, apresentamos um estudo do modelo BS baseado na distribuição skewnormal. Como subproduto consideramos o modelo de regressão linear log-Birnbaum-Saunders (log-BS). Para obter as estimativas de máxima verossimilhança usamos o algoritmo EM. Além disso, apresentamos um estudo de análise de influência global e local, através da metodologia de Zhu e Lee (2001) para dados incompletos. Ilustramos a metodologia proposta com dados encontrados na literatura.
Abstract: The class of models Birnbaum-Saunders (BS) appeared in problems of fatigue of materials (which is a structural damage that occurs when a material is exposed to stress and tension). Recently, this model has been applied in areas outside the context of fatigue of materials and engineering, for example in health sciences, environmental, forestry, demographic, actuarial, financial, among others. As the BS distribution has the property to describe cumulative degradation processes. In this work we present a study of the BS model based on Skew-normal distribution. As a byproduct consider the model of linear regression log-Birnbaum-Saunders (log-BS). To obtain estimates of maximum likelihood we use the EM algorithm. Furthermore, we present a study of the analysis of global and local influence, through the method of Zhu and Lee (2001) to incomplete data. Illustrate the proposed methodology with data found in literature.
