Dunn, Lauren E., Joshua Z. Willey, and Ronald M. Lazar. Neuroprotection for Mechanical Circulatory Support. Edited by David L. Reich, Stephan Mayer, and Suzan Uysal. Oxford University Press, 2017. http://dx.doi.org/10.1093/med/9780190280253.003.0012.

Abstract:
This chapter examines the adverse neurological events associated with mechanical circulatory support (MCS), which includes left ventricular assist devices (LVAD) and percutaneous devices for cardiac disease. The most frequently encountered neurological events are ischemic and hemorrhagic stroke, as well as intracerebral hemorrhage (ICH), heart failure and cardiovascular disease. The management of acute cerebrovascular conditions in this population poses unique challenges, given concomitant anticoagulation usage, hemodynamically unstable patients, and lack of randomized controlled trials investigating these clinical scenarios. Other acute neurological events include cerebral hyperperfusion and cerebral air embolism. This chapter describes these complications and their risk factors, and the available evidence-based and institutional management strategies.

Full text
Abstract:
When cardiac arrest occurs, blood flow to the vital organs diminishes rapidly. Chest compressions are an essential element of cardiopulmonary resuscitation (CPR), yet they achieve, at best, one-third of the normal cardiac output. The speed of initiating CPR, as well as its quality is critical to patient outcomes. Optimal chest characteristics of compressions are defined as pushing hard (depth > 5 cm) and fast (compression rate 100–120/min). Pressure should be released fully between sequential chest compressions and interruptions in chest compressions should be minimized. Even short interruptions in CPR around the time of attempted defibrillation can be harmful. CPR feedback and prompt devices can be used to monitor the quality of CPR. Studies have shown these devices can improve the quality of CPR, but do not improve overall survival. Mechanical chest compression devices may be usefully deployed when it is difficult or unsafe to perform manual CPR, but there is no evidence that the routine deployment of these devices improves outcome. Vasoactive drugs improve coronary perfusion pressure and increase the chances of return of spontaneous circulation. However, there is no definitive evidence that they improve long-term survival. Recent data have raised the possibility that adrenaline may worsen long-term outcomes.

Karzmark, C. J., Robert J. Morton, and James Lamb. A Primer on Theory and Operation of Linear Accelerators in Radiation Therapy. Medical Physics Publishing, 2017. http://dx.doi.org/10.54947/9781930524965.

Full text
Abstract:
By the mid-1950s, a linear accelerator suitable for treating deep-seated tumors was built in the Stanford Microwave Laboratory and installed at Stanford Hospital. It served as a prototype for commercial units that were built later. Since that time, medical linear accelerators gained in popularity as major radiation therapy devices, but few basic training materials on their operation had been produced for use by medical professionals. C.J. Karzmark, a radiological physicist at Stanford University, was involved with medical linacs since their development, and he agreed to collaborate with Robert Morton of the Center for Devices and Radiological Health (formerly the Bureau of Radiological Health), U.S. Food and Drug Administration, in writing the first edition of this primer. Since its first appearance, this book has become a classic. This third edition takes into account the significant advances occurring in radiotherapy linacs since the second edition was published in 1998. Again, the level of treating these advances is simplified so that the audience of radiation therapists as well as physicians, engineers, and physicists can benefit.

Full text
Abstract:
Mechanical circulatory support (MCS) offers life-saving treatment options for patients with advanced heart failure, cardiogenic shock, and acute cardiopulmonary failure. With the development of short-term, long-term, left-sided, right-sided, biventricular, and cardiopulmonary support options, the complexity surrounding this field has evolved substantially in the past decade. This textbook seeks to provide a logical and systematic framework for managing patients who require MCS therapies. In chapters 2–9, a comprehensive approach to selecting and preparing patients for MCS interventions is described. In chapters 10–23, the range of currently available devices is reviewed with updates on the most up-to-date clinical experiences based on recently published outcomes. Chapters 24–30 describe the current state of the art in perioperative strategies for achieving long-term success. Finally, chapters 31–46 outline the myriad of clinical challenges that often occur in the postoperative period, including some of the frequently encountered adverse events that are unique to the physiology associated with this technology. The fundamental principles included in this textbook will provide a framework for the MCS provider that can serve as a road map amidst the rapidly evolving landscape created by the technology pipeline within this industry.

Full text
Abstract:
Semiconductor quantum structures are at the core of many photonic devices such as lasers, photodetectors, solar cells etc. To appreciate why they are such a good fit to these devices, we must understand the basic features of their band structure and how they interact with incident light. This book takes the reader from the very basics of III-V semiconductors (some preparation in quantum mechanics and electromagnetism is helpful) and shows how seemingly obscure results such as detailed forms of the Hamiltonian, optical transition strengths, and recombination mechanisms follow. The reader does not need to consult other references to fully understand the material, although a few handpicked sources are listed for those who would like to deepen their knowledge further. Connections to the properties of novel materials such as graphene and transition metal dichalcogenides are pointed out, to help prepare the reader for contributing at the forefront of research. The book also supplies a complete, up-to-date database of the band parameters that enter into the calculations, along with tables of optical constants and interpolation schemes for alloys. From these foundations, the book goes on to derive the characteristics of photonic semiconductor devices (with a focus on the mid-infrared) using the same principles of building all concepts from the ground up, explaining all derivations in detail, giving quantitative examples, and laying out dimensional arguments whenever they can help the reader’s understanding. A substantial fraction of the material in this book has not appeared in print anywhere else, including journal publications.

Coeckelbergh, Mark. Robot Ethics. The MIT Press, 2022. http://dx.doi.org/10.7551/mitpress/14436.001.0001.

Full text
Abstract:
A guide to the ethical questions that arise from our use of industrial robots, robot companions, self-driving cars, and other robotic devices Does a robot have moral agency? Can it be held responsible for its actions? Do humans owe robots anything? Will robots take our jobs? These are some of the ethical and moral quandaries that we should address now, as robots and other intelligent devices become more widely used and more technically sophisticated. In this volume in the MIT Press Essential Knowledge series, philosopher Mark Coeckelbergh does just that. He considers a variety of robotics technologies and applications—from robotic companions to military drones—and identifies the ethical implications of their use. Questions of robot ethics, he argues, are not just about robots but are, crucially, about humans as well. Coeckelbergh examines industrial robots and their potential to take over tasks from humans; “social” robots and possible risks to privacy; and robots in health care and their effect on quality of care. He considers whether a machine can be moral, or have morality built in; how we ascribe moral status; and if machines should be allowed to make decisions about life and death. When we discuss robot ethics from a philosophical angle, Coeckelbergh argues, robots can function as mirrors for reflecting on the human. Robot ethics is more than applied ethics; it is a way of doing philosophy.
