AbstractIn 1998 a mile-station was discovered along the ancient coastal road 1400 m east of Abu Kammash, a centre identified with ancient Pisida on the basis of the Roman itineraries: the Itinerarium Antonini records a distance of 54 miles from Sabratha; the Tabula Peutingeriana of only 53. The Department of Antiquity of Sabratha in collaboration with the Archaeological Mission of the University Roma Tre recovered a milestone of Caracalla (216 AD) and another of Diocletian and Maximian (290-292 AD). Both bear the number of 54 miles, a distance evidently reckoned from the city of Sabratha, in perfect accordance with the figure of the Itinerarium Antonini.

Il presente contributo riflette sulla Convenzione di Oviedo in prospettiva diacronica, analizzando l’evoluzione dei principi guida della bioetica e della biogiuridica nel passaggio di millennio. Vengono prese come pietre miliari la Convenzione medesima e la Dichiarazione UNESCO di Parigi del 2005 su Diritti Umani e Bioetica. L’emersione progressiva del principio di vulnerabilità viene interpretata come frutto dell’influsso del pensiero femminile nel dibattito e come lento ma consistente affermarsi di una concezione della giustizia meno liberal-individualistica e sempre più solidale e relazionale. ---------- This article thinks about the Oviedo Convention in a diachronic perspective, analyzing the evolution of the guiding principles of bioethics and biolaw in the millennium. The same convention and the 2005 Paris UNESCO Declaration on Human Rights and Bioethics are taken as milestones. The progressive emergence of the principle of vulnerability is interpreted as the result of the influence of women’s thinking in the debate and as slow but consistently affirming a less liberal-individualistic conception of justice and more and more relational conception.

Se ofrece un detallado estudio acerca de dos miliarios ya conocidos y procedentes, respectivamente, de las localidades de Ahigal y Coria (Cáceres, España). En el primer caso, nuestra reinterpretación nos lleva a descartar la atribución tiberiana que ha sido tradicionalmente propuesta para el miliario de Ahigal, señalando que en realidad nos hallamos ante un hito augusteo. En el segundo caso, tratamos de probar que, frente a las dudas expresadas por parte de ciertos investigadores, efectivamente nos hallamos ante un miliario de Augusto. En función de estas consideraciones, y proponiendo a modo de hipótesis la relación del miliario augusteo de Coria con el iter ab Emerita Asturicam, planteamos algunas ideas tratando de reevaluar el papel de Augusto en la comúnmente denominada Vía de la Plata.

Tuberculosis is an infectious disease caused by Mycobacterium tuberculosis that can affect all organs and tissues. The clinical forms of primary cutaneous tuberculosis are tuberculosis primary complex (tuberculosis chancre) and tuberculosis cutis miliaris (miliary cutaneous tuberculosis). Secondary cutaneous tuberculosis includes lupus vulgaris, tuberculosis cutis verrucosa, scrofuloderma, tuberculosis cutis officials, and metastatic tubercular abscesses. The most common form is lupus vulgaris. Cutaneous tuberculosis is a rare form of tuberculosis presenting with nonspecific lesions. It is difficult to diagnose due to its non-specific clinical characteristics thus it can be confused with many other diseases. In this paper, we reviewed five patients with cutaneous tuberculosis who presented to our clinic within the past one year and we aimed to emphasize that cutaneous tuberculosis can manifest in different clinical forms.

Abstract:
Introduction: Miliary tuberculosis is an increasingly rare form (2% to 8%), defined by the hematogenous or lymphatic dissemination of the tubercle bacillus from a pulmonary or extra-pulmonary focus but which remains serious and can lead to acute respiratory failure due lesional pulmonary edema. Patients and Methods: This is a retrospective study of 101 cases of miliary tuberculosis hospitalized between January 1989 and July 2019 in the pneumology department of “20 Aout” in Casablanca. Results: The average age is 43 years with a predominance of men (64%), the history of tuberculosis is found in 8 cases and tuberculosis contagion in 10 cases. The common symptom in all cases (100%) is an alteration of the general condition, fever is present in 76% of cases, dry cough is found in 75.5% of cases and dyspnea is found in 38.3% of cases. Among the causes of immunosuppression, we noted: 3 cases of neoplasia, 5 cases of diabetes, 5 cases of pregnancy and 4 cases of system diseases. The chest X-ray (frontal view) found an aspect of pulmonary miliaria in all cases associated with pleurisy in 18 cases, an excavated opacity in 3 cases and pneumothorax in 1 case. Tuberculin intradermal reaction (tIDR) was positive in 35 cases. Antibacillary treatment was started urgently in all cases. The outcome was good in the majority of cases with 10 deaths. Further work-up allowed confirmation of the diagnosis either by direct isolation of the koch bacillus or by biopsy in 47.52% of cases.

Background Miliary tuberculosis (TB) is a fatal form of tuberculosis with severe clinical symptoms and complications. The mortality rate from this disease remains high, therefore, it is important to identify the risk factors for miliary TB for early detection and treatment.Objective To identify risk factors for miliary tuberculosis in children.Methods A case-control study of children aged 0-14 years with miliary TB was conducted in Dr. Soetomo Hospital from 2010 to 2015. Data were taken from medical records. Case subjects were children with miliary TB, and control subjects were children with pulmonary TB . Patients with incomplete medical records were excluded. Case subjects were identified from the total patient population; control subjects were included by purposive sampling, with case:control ratio of 1:1. Potential risk factors were age, nutritional status, BCG immunization status, and history of contact with TB patients. Statistical analyses were done with Chi-square and logistic regression tests. P values < 0.05 were considered to be statistically significant.Results A total of 72 children were analyzed, with 36 case and 36 control subjects. Nutritional status had a significant associatation with miliary TB in children (OR 3.182; 95%CI 1.206 to 8.398; P=0.018) in both bivariate and multivariate analyses. The probability of a child with moderate or severe undernutrition developing miliary TB was 76.09%. Other factors were not significantly associated with miliary TB.Conclusion Nutritional status is significantly associated with miliary TB in children, and moderate or severe undernutrition increases the risk for developing miliary TB.

Abstract:
Miliary tuberculosis (TB) is a fatal form of TB. Although drug resistance in TB patients has increased worldwide, there is limited information on drug resistance in miliary TB. This study from Pakistan evaluated drug susceptibility pattern among miliary TB patients of a high TB-burden country. All adult patients with miliary TB, admitted between 1994 and 2001, were identified using a computerized database. Culture-positive isolates were evaluated for drug susceptibility using middle brook 7H10 agar according to National Committee for Clinical Laboratories Standard criteria. Of 110 patients diagnosed with miliary TB, 32 (30%) were culture positive (yielding 35 culture isolates). The sources of positive cultures were sputum (37%), cerebrospinal fluid (18%), lymph nodes (12%), bone marrow (9%), bronchial wash (9%), urine (6%), lungs (6%) and liver (3%). Isoniazid resistance was found in three (9%) isolates. All the isolates were sensitive to rifampicin, ethambutol, pyrazinamide and streptomycin. Despite a worldwide increase in TB drug resistance, patients with miliary TB have infection with drug-sensitive mycobacterium. First-line anti-TB drugs should be used as initial therapy in miliary TB patients.

Miliary pattern on radiographs are attributed always to tuberculosis even though the differential diagnoses of miliary pattern is very wide. This pattern is most commonly caused by infectious diseases and rarely by malignancies. Primary lung cancers presenting with miliary shadows is extremely rare. Here we report a case of 50-year old female, presenting with 15 days of symptoms and CXR PA view showing bilateral diffuse miliary nodules. HRCT Chest confirmed the miliary pattern and also showed a mass in left lower lobe. Microbiological tests for tuberculosis and fungal infections were negative. CECT Chest gave impression of left lower lobe suspected malignant lung mass with pulmonary metastasis. Malignancy was confirmed by CT guided FNAC as Adenocarcinoma lung. PET scan didnot reveal metastasis in other organs. CONCLUSION : Miliary pattern does not always indicate tuberculosis and other possibilities should be evaluated.

Abstract:
Miliary tuberculosis (TB) is a severe form of disseminated TB. In developing countries childhood miliary TB is a significant health problem. Pulmonary TB is the most common type of TB in children. Of all TB cases miliary TB accounts for about 1%. We report a case of paediatric miliary TB with acute presentation in a eleven years old female who presented with fever and cough since 3 days and rapid breathing since one day. Clinical examination showed hepatosplenomegaly, on auscultation chest was clear and air entry bilateral equal, chest x-ray was suggestive of miliary shadows suggestive of TB and it was confirmed by HRCT chest. Laboratory studies revealed anaemia and elevated erythrocyte sedimentation rate (ESR). Tuberculin test was negative, gastric lavage for acid-fast bacilli Gene-Xpert was positive for mycobacterium tuberculosis. Fundus examination revealed bilateral small, white sub retinal opacities with the classical appearance of choroidal tubercles. The diagnosis of Miliary TB was confirmed and the patient was started on anti TB therapy.

Purpose To describe a patient with Leber miliary aneurysms who fulfilled the criteria for clinically definite multiple sclerosis (MS) in remission. Methods Case report. Results We performed a complete ophthalmologic examination in a 45-year-old woman who was referred to our department for retinal vascular abnormalities in her left eye. Twenty-eight years previously, the patient had been diagnosed with MS. Following a complete ophthalmologic examination, the patient was diagnosed with Leber miliary aneurysms and coincident MS in remission. To our knowledge, there are no previous reports describing association between Leber miliary aneurysms and MS. Conclusions Leber miliary aneurysms have been found in association with multiple ocular and systemic syndromes. In this report, we suggest another possible systemic association (MS) with Leber miliary aneurysms

Dissemination of tuberculosis (TB) is known as miliary tuberculosis. When miliary tuberculosis lacks classic radiographic and clinical features, it can be labeled cryptic miliary tuberculosis and may mimic metastatic cancer. This unusual presentation of an already atypical form of TB often delays diagnosis. We present a case of a 58-year-old female who presented with presumed metastatic carcinoma, who was ultimately diagnosed with both primary breast cancer and disseminated TB. This case emphasizes the need for a high index of suspicion for miliary tuberculosis in patients with presumed or proven malignancy.

Although miliary tuberculosis (TB) is well known, the incidence of miliary TB accompanying paravertebral abscess is extremely rare in adolescent children. We report a case of paravertebral TB abscess and miliary TB in a 17-year-old male initially presenting with fever, general weakness, back pain, sweating, cough, dyspnea and weight loss. The patient was diagnosed as paravertebral TB abscess and miliary TB. The anti-tuberculous drugs were started and the follow-up imaging showed that the lesions had disappeared without surgery. Although seldom observed, TB should be kept in mind in the differential diagnosis of paravertebral abscess.

Il 21 aprile 1986 è stato pubblicato un nuovo documento pontificio sula pastorale dei militari. Si tratta del'a costituzione apostolica Spirituali militari curae con la quale si adempie quanto il nuovo Codice di Diritto Canonico acenna ned can. 569 sui cappelani militari. Nel documento sono state opportunamente riviste le norme relative alla cura pastorale dei militari fin qui vigenti, contenuti nella Istruzione Solemne semper del 23 aprile 1951. Nello suo studio l’autore présenta e commenta la suddetta costituzione apostolica. Dopo aver indicato la genesi, il carattere e la struttura del documento si tratta della organizzazione dei ordinariati militari nonchè dell’ufficio dell’ordinario militare. Secondo l’autore la più notevole novità della costituzione apostolica consiste nel riconoscere l’ordirariato militare come la Chiesa particolare, giuridicamente assimiliata ad una diocesi. Le altre nuove disposizioni del documento fanno solo le consequenze di quel principio.

Latar belakang: Perubahan iklim dan suhu dapat menimbulkan biang keringat (miliaria) pada bayi. Biang keringat adalah ruam kecil yang terasa gatal menyebabkan iritasi kulit. Hal ini dapat diatasi dengan tanaman obat yaitu minyak jintan hitam (Nigella sativa oil) yang berfungsi untuk menghentikan inflamasi atau peradangan. Tujuan: Untuk mengetahui pemberian minyak jintan hitam (Nigella sativa oil) sebagai pengobatan biang keringat (miliaria) pada Bayi. Metode: Karya Tulis Ilmiah ini merupakan jenis penelitian studi kasus. Data diperoleh dari wawancara, observasi atau pengamatan dan studi kepustakaan. Instrumen yang digunakan adalah lembar observasi, lembar check list. Hasil: 3 partisipan mengalami kesembuhan (60%), 2 partisipan mengalami perubahan menjadi miliaria profunda (40%). Kesimpulan: Penerapan pemberian minyak jintan hitam (Nigella sativa oil) efektif mengobati biang keringat (Miliaria) pada bayi. Abstract:
Miliary shadows on chest imaging have wide differential diagnoses. The most common etiology is infectious, such as miliary tuberculosis (TB) and histoplasmosis, but miliary shadows can be the presentation of sarcoidosis, pneumoconiosis, and secondary metastasis to the lungs from primary cancers of the thyroid, kidney, and trophoblasts as well as sarcomas. Here we present the case of a 35-year-old Indian male who presented with a 2-month history of dry cough and shortness of breath. Chest imaging showed diffuse bilateral miliary nodules. The initial impression was that of miliary pulmonary TB. Subsequent bronchoscopy with a transbronchial biopsy confirmed the diagnosis of pulmonary mucinous adenocarcinoma with brain metastasis, which is a rare and unusual presentation of primary lung cancer. The tumor was positive for ALK5A4 and PD-L1, and the patient was started on tyrosine kinase inhibitor immunotherapy, with a favorable response.

A Tuberculose (TB), causada por Mycobacterium tuberculosis, um bacilo intracelular, aeróbico, transmitido por secreções respiratórias, constitui um importante problema de saúde pública. M. tuberculosis infecta preferencialmente os lóbulos inferiores dos pulmões, podendo se disseminar para outros órgãos, provocando a tuberculose miliar. Este estudo objetivou realizar um levantamento dos casos de Tuberculose no Município de Santa Fé do Sul, no período de janeiro de 2018 a janeiro de 2021, conhecendo o perfil epidemiológico dos infectados para subsidiar estratégias de controle. Foram coletados dados notificados pelo Sistema de Informação de Agravos e Notificação. A incidência de TB no período analisado foi de quatorze casos, sendo cinco do sexo feminino e nove do masculino, estando 50% na faixa etária de 45 a 64 anos. Embora 50% da amostra apresentasse alguma comorbidade, não foram encontradas comorbidades em comum. Aposentados, desempregados e “do lar” constituíram a maioria dos infectados. A tuberculose pulmonar foi a manifestação em 92% dos casos, havendo um caso de tuberculose pleural. O tratamento realizado combinou Rifampicina e Isoniazida, não havendo relatos de recidiva ou retratamento, evidenciando eficácia. Os dados de incidência estão de acordo com a literatura em relação ao sexo, idade, manifestações da doença e tratamento. Conclui-se que as ações municipais vêm sendo eficazes no tratamento, não havendo tuberculose miliar no período e um caso de óbito, relacionado ao usuário de drogas. Os resultados poderão instituir ações para otimizar o controle da doença, uma vez que no âmbito municipal tem-se um relacionamento mais estreito com o paciente com acompanhamento mais efetivo. EPIDEMIOLOGICAL SURVEY OF TUBERCULOSIS CASES IN THE MUNICIPALITY OF SANTA FÉ DO SUL (SP) ABSTRACT Tuberculosis (TB), caused by Mycobacterium tuberculosis, an intracellular, aerobic bacillus transmitted by respiratory secretions, constitutes an important public health problem. M. tuberculosis mostly infects the lower lobes of the lungs, and may spread to other organs, causing miliary tuberculosis. This study aimed to conduct a survey of Tuberculosis cases in the Municipality of Santa Fé do Sul, in the period from January 2018 to January 2021, understanding the epidemiological profile of those infected in order to support control strategies. It was collected data notified by the Notifiable Diseases Information System. The incidence of TB in the period analyzed was fourteen cases, five female and nine male, with 50% in the age range of 45 to 64 years. Although 50% of the sample had some comorbidity, no common comorbidities were found. Retired, unemployed and "housewives" constituted the majority of the infected. Pulmonary tuberculosis was the manifestation in 92% of the cases, with one case of pleural tuberculosis. The treatment given combined Rifampicin and Isoniazid, with no reports of recurrence or retreatment, showing efficacy. The incidence data are in agreement with the literature regarding gender, age, disease manifestations and treatment. We conclude that the local measures have been effective in the treatment, with no miliary tuberculosis in the period and one case of death, which was related to a drug user. The results may help to implement actions to strengthen the control of the disease, since at the local health care level there is a closer relationship with the patient and a more effective follow-up. Desain penelitian yang digunakan adalah pre eksperimental (One Group Pretest Postest Design). Populasi dalam penelitian ini adalah semua ibu yang memiliki balita sebanyak 25 responden dengan menggunakan total sampling. Pengumpulan data menggunakan lembar checklist. Analisis data menggunakan uji Wilcoxon Signed Rank Tets dengan hasil menunjukan bahwa ada pengaruh pendidikan kesehatan terhadap kemampuan ibu dalam penanganan biang keringat (miliaria) pada balita dengan (p value = 0.000 < ɑ=0.05) yang berarti bahwa ada pengaruh pendidikan kesehatan terhadap kemampuan ibu dalam penanganan biang keringat (miliaria) pada balita di Desa Candirejo Kecamatan Ponggok. Diharapkan hasil penelitian ini bisa dijadikan sebagai masukan informasi dan meningkatkan kemampuan ibu dalam penanganan biang keringat (miliaria).

Background: Lung lesions often appear in patients with sarcoidosis; however, miliary opacities are rare. We present the case of a 55-year-old Indian man who presented with dyspnea and low-grade fever. </P><P> Discussion: Miliary Tuberculosis (TB) was initially suspected, despite the direct microscopic examination from bronchoalveolar lavage was negative for acid-fast bacilli because imaging showed miliary opacities, and transbronchial lung biopsy revealed the presence of typical caseating granulomas. Antitubercular treatment with the classic four-drug regimen was initiated. However, the patient did not improve and cultures were negative for Mycobacterium growth. The diagnosis of sarcoidosis was made only after a negative culture and clinical and histopathological re-evaluation of the case. Conclusion: Although miliary sarcoidosis is rare, physicians should consider sarcoidosis in the differential diagnosis with conditions like tuberculosis, malignancy, and pneumoconiosis when patients present with miliary opacities who do not respond to the traditional treatment.

Miliary histoplasmosis is a rare presentation that may mimic miliary tuberculosis. We report a case of miliary histoplasmosis in a 52-year-old male who was being treated with hydroxychloroquine, methotrexate, and sulfasalazine for his rheumatoid arthritis and presented to the emergency department with shortness of breath and fevers. Computed tomography (CT) chest revealed miliary pulmonary nodules. Urine Histoplasma antigen and serum Histoplasma antigen were negative; however, Coccidioides immitis complement immunofixation assay and Coccidioides IgM were positive. The patient was initiated on treatment for pulmonary coccidioidomycosis and immunosuppression was held. However, a few days later, Histoplasma capsulatum was isolated from cultures from bronchoscopy. This case highlights the difficulty in diagnosing histoplasmosis in immunocompromised patients and the importance of having a broad differential diagnosis for miliary pulmonary nodules. Tissue culture and histopathology remain the gold standard for the diagnosis of histoplasmosis. Further research needs to be conducted to determine the optimal duration of histoplasmosis treatment in immunocompromised patients.

5079 Background: Insulin-like growth factor binding protein, IGFBP4, was shown to be highly expressed across all stages of epithelial ovarian cancer (EOC) and serum levels are elevated in EOC. Moreover, IGFBP4 levels are ~3x greater in women with malignant pelvic masses. We investigated whether ascites volume and the presence of miliary disease in combination with serum levels of IGFBP4 are independent predictors of survival. Methods: A prospective and retrospective analysis was performed. Patients were enrolled at the time of cytoreductive surgery. Ascites volume was either absent, <500 cc (low), or >= 500 cc (high), and the presence of miliary disease was recorded. The IGFBP4 cutoff was 1064.5 ug/ml based upon previous results. The Kaplan-Meier product limit method was used to estimate PFS probabilities. The Cox proportional hazards model was used to estimate hazard ratios (HR) and corresponding 95% CI. Results: 57 cases were included in the analysis of ascites volume and miliary disease. Cytoreductive outcomes were complete gross resection (44.8%), optimal (<=1cm residual disease; 44.8%), and suboptimal ( >1cm residual disease; 10.3%). Histologic subtypes: papillary serous (n=35; 61.4%), mucinous (n=15; 26.3%), endometrioid (n=4; 7.0%), and clear cell (n=3; 5.3%). Stage distribution was 21.1% I/II, and 78.9% III/IV. PFS was unaffected by ascites volume (p=0.341) or miliary disease. Among this cohort, 29 had IGFBP4 levels available for a separate analysis. Patients with high IGFBP4 and miliary disease were 5.5 times as likely to recur compared with patients with miliary disease and low IGFBP4 (HR=5.55 [0.77, 39.82]), and the statistical significance was borderline (p<0.088). No statistically significant differences were detected between rates of recurrence among patients with high and low IGFBP4 values in combination with ascites volume. Conclusions: These exploratory studies suggest that patients with high IGFBP4 serum levels and miliary disease were > 5 times as likely to recur compared to women with miliary disease and low IGFBP4 levels. Future studies examining these variables using a larger population and examining the biologic basis of this relationship are planned.

Dipergunakan 30 tanaman ylang-ylangumur 4 tahun sebagai contoh. Pengamatan dilakukan tiap bulan denganparameter padat populasi (telur, larva, pupa dan imago) dan intensitasserangan. Selain itu juga dilakukan pengamatan skala rumah kaca untukmengetahui siklus hidup dan kebutuhan makan. Dari hasil penelitiandiketahui padat populasi A. miliaris dan A. atlas pada tanaman Ylang-ylang mencapai 40,94 dan 30,86 ekor/tan, terdiri dari populasi larva, pupa,dan imago masing-masing sebesar 26,70; 8,02; dan 6,22 ekor/tan pada A.miliaris dan 21,97; 5,21; dan 3,68 ekor/tan pada A. atlas, serta populasitelur 11,24 ootheca/tan pada jenis A. miliaris dan 15,54 butir/tan pada A.atlas. Introduction. Miliary tuberculosis (TB) is a rare and potentially fatal form of disseminated TB. It is caused by a widespread haematogenous dissemination of Mycobacterium tuberculosis from an active caseous focus to different organs. Sometimes it can have an acute presentation with a rapidonset clinical deterioration and death. Miliary TB complicated with an acute respiratory distress syndrome (ARDS) requiring mechanical ventilation (MV) is rare, even in countries with a high incidence of TB. Case report. A 35-yearold woman with Down syndrome (DS) was admitted to the Clinic for Pulmonology, Clinical Centre Kragujevac, due to an evaluation of cough and weight loss during last 2 months. Laboratory findings revealed anaemia, leukocytosis, elevated C-reactive protein (CRP) and hypoalbuminemia. A chest x-ray showed bilateral reticulonodular shadows, predominantly in the mid and lower right lung lobes. A purified protein derivative (PPD) skin test and induced sputum smear for acid-fast bacilli (AFB) were both negative. On the fifth day following admission, her health condition suddenly declined, and after developing a moderate ARDS, she was put on the mechanical ventilation. Due to a high clinical suspicion of miliary TB and the fact that her life was compromised, an empirical anti-tuberculosis therapy was initiated. Despite all therapeutic and supportive measures, the patient expired 3 days later. The diagnosis of miliary TB was established post-mortem. Conclusion. Miliary TB should be kept in mind in patients with DS due to immunosuppression associated with deficient cell-mediated immunity. The development of ARDS as a complication of miliary TB is difficult to identify due to a low causal association. High clinical suspicion and a chest radiograph with a typical appearance of miliary pattern justify the initiation of empirical anti-tuberculosis treatment in such patients, as an attempt to change poor prognosis.

It is a serious illness with nonspecific clinical manifestations and typical chest radiographic findings may not be seen until late in the course of the disease. It is often associated with maternal immunocompromised status. Here, we report a case of miliary tuberculosis in a nonimmunocompromised mother presenting as fever of unknown origin in immediate puerperium. Prolonged workup of eight weeks led to the diagnosis of miliary tuberculosis as the cause of postpartum fever that responded well to antituberculous drugs.

Miliary tuberculosis (TB) is a severe form of disseminated TB that could be fatal without treatment. It remains a significant health problem in endemic countries such as Indonesia. A-3-month-old child was admitted to the hospital with chief complain of shortness of breath, cough, and intermittent low-grade fever. Significant clinical findings were lack of weight gain, previous contact with confirmed cases of TB, and severe malnutrition. Significant laboratory findings were anemia, elevated CRP, and hypoalbuminemia. Radiological findings were suggestive of miliary TB with bilateral diffuse reticulonodular lung lesion on chest CT-scan. Diagnosis of miliary TB is difficult because of nonspecific, atypical symptoms and varied clinical signs. It is needed multiple diagnostic approach, especially for children. Miliary TB remain to be a difficult case that require an early intervention despite difficulties in its diagnosis, to prevent morbidity and mortality of patient.

Abstract Miliary tuberculosis (TB) presents a major challenge following a renal transplant in humans. In the current report, we described a patient with disseminated TB following renal transplantation. The article presents the case of a 38-year-old man who presented an 8-month history of fever, chills, sweating, low-back pain and significant weight loss. Chest radiography and computed tomography (CT) scan showed miliary nodules distributed in the two lungs. The transbronchial lung biopsy revealed a granulomatous reaction with caseous necrosis. Magnetic resonance imaging (MRI) of the brain found multiple tuberculomas. Also, MRI of the lumbosacral was indicative of a psoas abscess. Therefore, miliary pulmonary, cerebral and spinal TB was confirmed. The patient was started on an anti-TB regimen and paravertebral aspiration was also done. The patient’s condition improved considerably. In conclusion, this case report can remind us of the importance of maintaining a high clinical suspicion and performing a thorough workup to establish a timely diagnosis and treatment of miliary TB.

După mai mult de douăzeci de ani de la reintroducerea preoților militari în rândul forțelor armate, se pot observa ceva inadvertențe, mai mici sau mai mari, atunci când facem o comparație între scopul reglementărilor adoptate în domeniul regimului preoților militari și modul în care acestea sunt puse în practică. Factorii de decizie din forțele armate par să facă o analogie totală între statutul clerului militar și reglementările care privesc salariații civili angajați cu contract de muncă, iar acest fapt generează o anumită frustrare în rândul preoților militari. Membrii acestui corp se așteptau la un tratament, chiar și atunci când se ia în calcul managementul personalului, similar cu al celorlalți ofițeri, ai căror colegi sunt, singura diferență fiind că preoții militari sunt personal necombatant. Prezentul articol reexplică metodele complexe și interdependente de interpretare a legislației și arată soluția necesară și imperativă de a pune lucrurile pe făgașul lor normal. Nu este nevoie de niciun fel de efort, sacrificii sau pierderi. Numai de bunăvoință și bună-credință.

Perubahan sistem perdagangan internasional menuju liberalisasi, seperti ASEAN menjadi Masyarakat Ekonomi ASEAN pada akhir tahun 2015, memunculkan banyak peluang dan sekaligus juga tantangan-tantangan bagi kondisi neraca pembayaran Indonesia. Peluang yang dimaksud adalah Indonesia dapat memperluas jangkauan pasarnya ke negara-negara ASEAN lainnya. Dampak positif lain dari adanya AEC adalah investor Indonesia dapat memperluas jangkauan investasinya keluar negeri, demikian sebaliknya, Indonesia juga dapat menarik investor dari negara-negara ASEAN yang lain untuk berinvestasi di Indonesia. Sedangkan tantangannya adalah, dengan tingkat persaingan perdagangan yang semakin ketat, maka akan semakin besar defisit neraca perdagangan Indonesia dengan negara ASEAN lainnya, dan bagaimana Indonesia dapat meningkatkan daya tarik investasinya. Penelitian ini dilakukan untuk mengetahui dampak Masyarakat Ekonomi ASEAN (MEA) terhadap Neraca Pembayaran Indonesia pada tahun 2017. Dari hasil analisis data, pada tahun 2016, terjadi penurunan jumlah ekspor barang dari USD 148.365 juta pada tahun 2015, menjadi USD 144.441 juta pada tahun 2016. Defisit neraca perdagangan jasa pada tahun 2016 turun menjadi USD6,5 miliar dari USD8,7 miliar pada tahun 2016. Surplus investasi langsung meningkat dari USD3,0 miliar pada triwulan II-2016 menjadi USD5,2 miliar pada triwulan III-2016 didukung oleh sentimen positif terhadap prospek perekonomian domestik. Perkembangan investasi portofolio sisi kewajiban pada triwulan III-2016 dipengaruhi oleh peningkatan neto pembelian investor asing atas surat berharga berdenominasi rupiah, terutama SUN dan saham Transaksi investasi lainnya pada triwulan III-2016 mengalami defisit USD2,3 miliar, lebih rendah dibandingkan dengan periode sebelumnya yang mencatat defisit sebesar USD3,7 miliar, namun berkebalikan dengan surplus di triwulan III-2015 sebesar USD0,4 miliar. Dari hasil analisis diatas, dapat diambil kesimpulan bahwa dalam jangka pendek (satu tahun), pemberlakuan Masyarakat Ekonomi ASEAN belum terlalu berdampak pada kondisi neraca pembayaran Indonesia.

SPAM Kota Pekanbaru diharapkan mampu melayani kebutuhan air bersih Kota Pekanbaru dengan kapasitas 750 lpd dengan estimasi investasi Rp 750 miliar yang mencakup 5 kecamatan. Fokus penelitian ini adalah untuk menjelaskan bagaimana investasi skema SPAM Kota Pekanbaru dievaluasi agar tujuan pembangunan infrastruktur publik mencapai manfaatnya. Penelitian ini merupakan penelitian kuantitatif komparatif dengan studi kasus menggunakan metode Monte Carlo, Value for Money, dan T-Test. Hasil penelitian menunjukkan perbandingan alokasi risiko pemerintah akan menanggung semua risiko termasuk risiko peningkatan biaya operasional dan pemeliharaan sebesar 345.289 miliar dengan total 670.084 miliar, dengan pengadaan skema KPBU risiko biaya tersebut dialihkan ke mitra swasta. VfM yang diperoleh dalam proyek tersebut sebesar Rp589.537 miliar.

In miliary TB, the role of steroid remains to be unclear. Till date, extremely limited studies have been conducted to evaluate corticosteroids’ role in miliary TB. We report a patient with miliary TB and co-existing organizing pneumonia (OP), a condition that was managed successfully by anti-TB chemotherapy in adjunct with the corticosteroid. To the best of our knowledge, this association has not been reported or extremely infrequently reported in the existing literature. The adjunctive use of corticosteroids does not appear to diminish the efficacy of ‘adequate’ anti-tuberculosis therapy. The use of corticosteroids has significant short- and long-term benefits in most forms of tuberculosis.

Background: It is the purpose of this article to describe a suspected association of inadvertent vaccination with varicella vaccine during early pregnancy with the subsequent development of in utero miliary fetal tissue calcifications and fetal hydrops detected by sonogram at 15 weeks of gestation.Case: This is a case presentation of a pregnant patient who received varicella vaccination during the same menstrual cycle that she became pregnant, and is supplemented by a literary review. The fetus developed miliary fetal tissue calcifications and fetal hydrops detected by a targeted sonogram at 15 weeks gestation.Conclusion: Varicella vaccination during early pregnancy may be a cause of miliary fetal tissue calcifications and fetal hydrops.

Penerapan tapping box memberikan dampak yang sangat positif terhadap besaran jumlah penerimaan daerah dibandingkan dengan sebelum adanya pemasangan alat tersebut. Hal ini terliha pada penerimaan pajak hotel tahun 2018 sebesar Rp. 8,76 miliar meningkat menjadi Rp. 12,7 miliar pada tahun 2019. Penerimaan pajak restoran tahun 2018 sebesar Rp. 11,2 miliar menjadi Rp. 16 miliar meningkat pada tahun 2019. Kemudian penerimaan pajak hiburan pada tahun 2018 sebesar Rp. 7,2 miliar meningkat menjadi Rp. 8,5 miliar pada tahun 2019. Jumlah pemasangan tapping box di Kota Kendari juga meningkat yang awalnya 76 unit menjadi 464 unit hingga Oktober tahun 2020. Maka dari itu, perlu meningkatkan kerjasama dari berbagai pihak dalam menyelenggarakan program penerapan tapping box untuk mengoptimalkan penerimaan pajak hotel, restoran dan hiburan supaya terciptanya efektifitas dan efisiensi dalam sebuah inovasi dalam administrasi perpajakan untuk membangun sumber daya manusia perpajakan yang produktif dan berbasis digital.

A seventy year old woman had a permanent VVI pacemaker inserted in 1983 for complete heart block and presented ten years later with purulent discharge from the generator pocket. During a prolonged pyrexia illness, she developed renal and respiratory failure and her illness was complicated by recurrent ventricular fibrillation. The patient died on her 31st hospital day. Subsequent histological and microbiological investigation revealed widespread miliary tuberculosis which included involvement of myocardial tissue, great vessels and the pacemaker pocket. To our knowledge, this is the first reported occurrence of miliary tuberculosis involving a permanent pacemaker system. Furthermore, the granulomatous myocarditis which occurred as part of the miliary picture is a rare occurrence and possibly explains the recurrent ventricular fibrillation.

We describe iron deficiency and silent intracranial tuberculomas with pulmonary miliary tuberculosis in 13-year old girl which can be rare seen, particularly in immunocompetent children. She presented as respiratory infection with lack of menstrual bleeding. She didn’t receive BCG vaccination. Chest roentgenogram showed miliary pattern while MRI of brain revealed parenchymal tuberculomas. Empirical 4 antitubercular drug treatments were initiated. Control showed a decrease in size and number of brain tuberculomas. Silent brain tuberculomas with miliary tuberculosis could be rare seen, particularly in immunocompetent children. It remains an open question whether the irregular menstrual bleeding and iron deficiency was associated with the drop of her immunity and increased risk for military tuberculosis. ObjectivesOvarian cancer patients with miliary disease have the lowest rates of complete surgical resection and poorest survival. Adjunct surgical techniques may potentially increase rates of complete surgical resection. No studies have evaluated the use of these techniques in primary debulking surgery for ovarian cancer patients with miliary disease. The aim of this study was to examine the use of adjunct surgical techniques during primary debulking surgery for patients with advanced epithelial ovarian, fallopian tube, and primary peritoneal cancer with miliary disease.MethodsMedical records of patients with International Federation of Gynecology and Obstetrics (FIGO) stages IIIC–IVB epithelial ovarian, fallopian tube, or primary peritoneal cancer with miliary disease undergoing primary debulking surgery from January 2010 to December 2014 were reviewed. Adjunct surgical techniques were defined as ultrasonic surgical aspiration, argon enhanced electrocautery, thermal plasma energy, and traditional electrocautery ablation. Patients undergoing surgery with and without these devices were compared with respect to demographics, operative characteristics, postoperative complications, residual disease, progression free survival and overall survival.ResultsA total of 135 patients with miliary disease underwent primary debulking surgery, of which 30 (22.2%) patients used adjunct surgical techniques. The most common devices were ultrasonic surgical aspiration (40%) and argon enhanced electrocautery (36.7%). The most common sites of use were diaphragm (63.3%), pelvic peritoneum (30%), bowel mesentery (20%), and large bowel serosa (20%). There were no differences in age, stage, primary site, histology, operative time, surgical complexity, or postoperative complications for patients operated on with or without these devices. Volume of residual disease was similar (0.1–1 cm: 60% with adjunct techniques versus 68.6% without; complete surgical resection: 16.7% with adjunct techniques versus 13.3% without; p=0.67). For patients with ≤1 cm residual disease, median progression free survival (15 versus 15 months, p=0.65) and median overall survival (40 versus 55 months, p=0.38) were also similar.ConclusionAdjunct surgical techniques may be incorporated during primary debulking surgery for patients with advanced epithelial ovarian, fallopian tube, or primary peritoneal cancer with miliary disease; however, these do not improve the rate of optimal cytoreduction.

Alat analisis yang digunakan yaitu Location Quotient (LQ) dan Icremental Capital Output Ratio (ICOR). Analisis LQ digunakan untuk menentukan sektor potensial dan ICOR digunakan untuk menentukan kebutuhan investasi dari setiap sektor. Data yang digunakan merupakan data sekunder yang diambil secara langsung dari website Badan Pusat Statistik (BPS).Hasil penelitian ini menyimpulkan terdapat delapan sektor unggulan yang ada di Kabupaten Lamongan yaitu (1) sektor pertanian, kehutanan, perikanan, (2) sektor pengadaan air, pengelolaan sampah, limbah dan daur ulang, (3) sektor konstruksi, (4) sektor informasi dan komunikasi, (5) sektor real estate, (6) sektor administrasi pemerintahan, pertahanan dan jaminan sosial wajib, (7) sektor jasa kesehatan dan kegiatan sosial, (8) sektor jasa lainnya.Hasil perhitungan ICOR dapat digunakan untuk menghitung kebutuhan investasi untuk mendorong pertumbuhan output sebesar 1%. Two cases of miliary tuberculosis and elevated levels of cancer antigen 125 Ovarian carcinoma is one of the most dangerous malignancies in women. The serum level of cancer antigen (CA) 125, as a tumour marker, is useful in the diagnosis of ovarian cancer. CA 125 serum level is also elevated in ascites (1), ovarian tube abscess (2,3), biliary duct cancer and periampullary tumours (4), cholangitis (5), cancer of pancreas (6) and cervical adenocarcinoma (7). Additionally, sometimes the serum level of CA 125 increases spontaneously during the menstrual cycle (8,9). In bone marrow transplantation, this marker is a sensitive index in the diagnosis of veno-occlusive disease (10). We describe an elevated serum level of CA 125 in two cases of tuberculosis (TB): one with cryptogenic miliary TB, and the other with miliary TB and meningitis.

Facial multiple miliary osteoma cutis is a variant of osteoma cutis usually occurring in women with a previous history of acne vulgaris. Successful ablative laser treatment has been reported using both CO2 and Er:YAG lasers among other invasive treatment modalities, like surgical removal. We report a patient with biopsy-proven facial multiple miliary osteoma cutis responding to non-ablative Q-switched Nd:YAG laser therapy. The patient had excellent cosmesis without textural changes or hypopigmentation despite her Asian background after three sessions over 6 months. Multiple miliary osteoma cutis is therefore now amenable to non-surgical non-ablative therapy by using Q-switched Nd:YAG laser therapy reducing the risk of textural changes and hypopigmentation, especially in dark complexion and high-risk individuals. To our knowledge, this treatment approach has not been previously reported.
