Abstract:
Neuroscience challenges our beliefs about agency and autonomy because it seems to imply that we have no control of our behavior: most brain processes are not transparent to us, we have no direct access to the efferent system, and we only experience the sensorimotor consequences of our unconscious motor plans. In this chapter, Walter Glannon argues that although unconscious processes drive many of our actions, this does not imply that conscious mental states have no causal role in our behavior and that we have no control over it. He argues that some degree of unconscious neural constraint on conscious mental states is necessary to modulate thought and action and promote flexible behavior and adaptability to environmental demands. He maintains that a nonreductive materialist account of the mind–brain relation makes it plausible to claim that mental states can cause changes in physical states of the brain.

An expanded position model is presented that enables to study the following features of the self as society: meta-positions as “leadership” positions in the self, offering a broad and long-term scope in a multipositioned, democratically organized self; promoter positions stimulating its further development; power distance and emotional distance as basic dimensions in the mini-society of the self; the accessibility of I-positions allowing a free entrance so that they can be experienced in their inside qualities; their exit providing a way to leave a position and preventing the feeling of being locked up in an I-prison; a procedure to get in touch with “shadow” positions as undesirable or rejected parts of the self; and different types of boundaries of positions (soft, rigid, spongy, and flexible), with flexible ones as optimal for the communication between I-positions in complex social and societal situations.

Our nation increasingly relies on family members or friends (i.e., informal caregivers) for needed care and support as we age. Family caregivers typically assume their caregiving role willingly and reap personal fulfilment from helping a family member, developing new skills, and strengthening family relationships. For these benefits, however, caregivers often sacrifice their own health and well-being. Depression, anxiety, poor physical health, and compromised immune function are more common among family caregivers than in adults not providing such care. The REACH OUT (Resources for Enhancing Alzheimer’s Caregiver Health: Offering Useful Treatments) program is a multicomponent, tailored, and flexible intervention for caregivers of people with dementia focused on the evidence-based therapeutic strategy of problem solving. This work is designed to guide clinicians through the process and provide them the necessary tools to share with caregivers with the goal of enhancing caregiver physical and mental health. Five common risk areas (home safety, caregiver health, social support, challenging behaviors, and emotional well-being) are described in the manual; and interventions are described that respect the nuances of each risk area. By beginning with an individualized risk assessment and being flexible to the needs and issues of the caregiver, the REACH OUT intervention helps clinicians identify risk areas and provide caregivers with tailored action plans to reduce risk and promote well-being.

As young people become adults and establish their independence, they make critical choices about relationships, education, and vocation. This transition is even more challenging for young people with histories of trauma, mental illness, and poor social supports, such as many youth aging out of the foster care system. Dishearteningly high rates of school dropout, homelessness, incarceration, and suicide in the 15- to 25-year-old age group have focused increasing attention on the needs of this population. This chapter presents strategies to engage these transition-age youth. Flexible but reliable, youth-led and future-oriented care breaks down treatment barriers so that youth can develop and achieve personal goals. Attention to the housing, vocational, and social support needs of these young people is crucial to their survival and success.

This chapter discusses previous experiences with telemental health, the importance of a cross-cultural understanding of mental health, and the potential psychological barriers to cross-cultural telepsychiatric treatment in low- and middle-income countries. The cross-cultural aspect of the patient–therapist relationship is almost absent in recent research on telemental health. The majority of studies regarding telemental health have been conducted in Western countries. This is an obvious worry, because patients’ health beliefs, attitudes, and behaviors are rooted in their specific cultural setting, and failure to consider this can have negative consequences for patients. A sociotechnical approach to developing a dynamic, iterative, and flexible system for telemental health could increase its sufficiency, applicability, and utility by striving to include social, human, cultural, and technological aspects, and ultimately could provide a much needed framework for development and implementation of telemental heath.

The health care industry is evolving in response to increasing costs, poor health outcomes, and consumer dissatisfaction. Integrated care is a critical component of health care evolution, recognizing the particular burden of untreated behavioral health issues on both physical and behavioral health care outcomes and costs. Telehealth, the use of technology to provide care at a distance, is a flexible process adaptable to a variety of integrated care applications and models. Telehealth (telepsychiatry, telebehavioral health, telemental health) can help maximize limited resources and expand the reach of psychiatrists and other limited specialty providers. As evidenced in a strong and growing research base, telehealth can support integrated care, as well as forward its application in new and innovative ways across the integration continuum. This chapter provides examples of current telehealth supported care integration models along the Substance Abuse and Mental Health Services Administration (SAMHSA)’s articulated integration continuum.

Abstract:
In Chapter 4, “Cognitive Behavioral Therapy: Evidence-Based Practice,” research on the use of CBT with older adults is summarized and evaluated. Fifteen meta-analyses and systematic reviews published between 2000 and 2015 are presented. The most developed area of research on CBT effectiveness with older clients is on depression and anxiety outcomes. A smaller, but still substantial, literature focuses on outcomes of CBT for insomnia. While research on the effectiveness of CBT with older adults is not as encompassing as that for younger populations, it is, nonetheless, substantive and, thus far, more advanced than that of most other psychotherapeutic interventions used with older adults. Research suggests that CBT with older clients is as effective as other psychotherapeutic interventions, with superior results in decreasing the negative outcomes of anxiety. In administration, CBT is a flexible approach that has utility across a variety of settings and contexts.

This chapter is concerned with a semantic (as opposed to ontological) approach to metaphysics, developed by Michael Dummett and Crispin Wright, that takes truth as fundamental, and explicates debates about realisms in terms of truth. On this approach realism is fundamentally concerned with the objectivity of truth, where objectivity does not consist in the existence of entities. The chapter shows that Dummett worked with three separable criteria for the objectivity of truth, which support a subtle and flexible framework for characterizing various degrees of realism. It argues that Dummett’s so-called “manifestation” arguments against semantic realism can handle many objections that have been brought against them. It discusses Wright’s minimalism about truth, his four semantic criteria of realism, their inter-relations, and their connections to Dummett’s criteria. It concludes with reflections on the meta-philosophical status of the semantic approach: the reasons in favor of pursuing it and its adequacy to metaphysical reflection.

There is currently a rather broad consensus that the competencies that are required to solve word problems involve: (a) a well-organized and flexibly accessible knowledge base involving the relevant factual, conceptual, and procedural knowledge that is relevant for solving word problems; (b) heuristic methods, i.e. search strategies for problem analysis and transformation which increase the probability of finding a solution; (c) metacognition, involving both metacognitive knowledge and metacognitive skills; (d) positive task-related affects, involving positive beliefs, attitudes, and emotions; and (e) meta-affect, involving knowledge about one’s affects and skills for regulating one’s affective processes. The present chapter reviews and discusses research that provides a view on how individual differences in performance on word problems can be related to each of these components.

While attention controls the internal, mental focus of attention, motor control directs the bodily control focus. Our nervous system is structured in a cascade of interactive control loops, where the primary self-stabilizing control loops can be found directly in the body’s morphology and the muscles themselves. The hierarchical structure enables flexible and selective motor control and the invocation of motor primitives and motor complexes. The learning of motor primitives and complexes again adheres to certain computational systematicities. Redundant behavioral alternatives are encoded in an abstract manner, enabling fast habitual decision making and slower, more elaborated planning processes for realizing context-dependent behavior adaptations. On a higher level, behavior can be segmented into events, during which a particular behavior unfolds, and event boundaries, which characterize the beginning or the end of a behavior. Combinations of events and event boundaries yield event schemata. Hierarchical combinations of event schemata on shorter and longer time scales yield event taxonomies. When developing event boundary detectors, our mind begins to develop environmental conceptualizations. Evidence is available that suggests that such event-oriented conceptualizations are inherently semantic and closely related to linguistic, generative models. Thus, by optimizing behavioral versatility and developing progressively more abstract codes of environmental interactions and manipulations, cognitive encodings develop, which are supporting symbol grounding and grammatical language development.

Es necesario instru­mentar una política fiscal contracíclica y monetaria flexible en América Latina para incentivar el creci­miento y disminuir la desigualdad del ingreso, dado que una política fiscal contracíclica y redistributiva induce el crecimiento estable y sostenido en los países en desarrollo, en tanto que la consolidación fiscal lo desacelera. El objetivo de este libro es ana­lizar los efectos de la política fiscal procíclica que se ha instrumentado desde la década de los noventa, analizando la evidencia para algunas economías de América Latina y de México, en particular. Se aborda la problemática desde un enfoque heterodoxo que nos permite entender el papel que han desempeña­do variables claves como la deuda, el gasto públi­co, y los impuestos en dichas economías. Este libro seguramente aportará elementos para la discusión de alternativas de política que puedan coadyuvar al crecimiento económico, sobre todo ante la crisis generalizada que se empieza a gestar actualmente.
