Studies by the World Health Organisation have shown that mental illness is an international health concern across the globe, with one in four people (25%) suffering from mental disorders in both developed and developing countries. In many African countries traditional health practitioners are the health care providers of choice for individuals, families and communities. The aim of this study was to explore methods used by Shona speaking traditional health practitioners in the prevention of mental illness in Bulawayo, Zimbabwe. A qualitative research design was used in the present study. Ten Shona speaking traditional health practitioners (male=9; female=1) were selected through purposive sampling and requested to participate in the study. Data was collected using semi-structured interviews and analysed through thematic content analysis. It was found that traditional healers tend to commonly understand and conceptualise mental illness in terms of the causes instead attaching nosological labels to these conditions. The findings of the study also show that most of the traditional health practitioners interviewed had similar methods of preventing mental illness both in families and individuals. Culture was found to be central in shaping how the traditional health practitioners understand and prevent mental illness. Ancestors were found to be pivotal in specifically determining the methods to prevent mental illness for each client. The study is concluded by recommending closer collaboration between the dominant Western health care system and traditional healing in order to improve mental health care provision in Zimbabwe.

On average 322 million people worldwide are affected by depression. It is one of the leading causes of death in the United States and is often paired with anxiety. Although there are several ways to treat both depression and anxiety, the most popular way is through the use of anti-depressant medication. Typical alternative methods are talk therapy and acupuncture, which brings me to my question: Are anti-depressants truly the most effective way to treat depression or are they ultimately the most cost effective for patients suffering from the mental illness because they are the type of treatment most covered by insurance companies?

Abstract:
中風後抑鬱症( Post-strok Depression, PSD )的發生率,在中風後患者中佔有較大比例,嚴重影響患者的康復。國外研究結果顯示中風後抑鬱症的發病率為28%-56%而國內臨床相關文獻提示中風後抑鬱的患病率大致為20% - 50%。臨床中存在有許多繼發於中風的抑鬱患者,診療未受到關注的現狀。而一些接受治療的病人,也因長期口服抗抑鬱藥導致副作用愈發突出。與此同時,針灸治療抑鬱的臨床優勢已經得到國內外醫學界的重視與肯定。本文著重以近10 年來針灸治療中風後抑鬱症的文獻為主進行研究,總結關於該病因病機及有特色針灸療法的最新研究進展,歸納選穴處方的規律特點,進而在已有研究基礎上融人自己的見解,對於PSD 這種身心疾病的治療提出相關值得探討的問題與對今後研究的展望。新近的國內外研究提示,在針灸治療中風後抑鬱領域目前主要有毫針針刺和特種針法,並結合電針、配合藥物以及心理健康教育等方法,可以促進中風後患者抑鬱情緒的調整,且一定程度上也加強其他肢體功能以及神經功能的康復。同時針刺在治療的臨床研究方面也已取得較大進展,部分實驗已設立自身前後對照研究及其他藥物或針刺對照比較觀察,在診斷及評定療效標準時使用了量表及統計分析,增強了療效的可信度及可比性。一些課題也已進入實驗室研究,從神經遞質、神經內分泌方面探求針刺治療PSD 的機制,並取得一定的進展。本研究得出今後應該加強隨機大樣本對照的前瞻性設計實驗的結論,認為中風後抑鬱症的臨床分型、穴位配伍、針刺手法、刺激量大小以及療效評價等方面,均將成為針灸治療本病的繼續研究之探索方向。

Abstract:
This project presents dance as treatment for Orthorexia Nervosa, an eating disorder defined as an unhealthy obsession with healthy eating. Eating disorders disconnect body, mind, and spirit of an individual, and dance therapeutically connects these aspects. The specific effects of orthorexia on the body, mind, and spirit are analyzed; supported by evidence from research sources such as literature of books and scholarly journals, videos, an interview with board-certified dance/movement therapist Rachel Gonick-Mefferd, and a series of interviews with Dr. Thomas Doyle, in which he supplied a case study exemplifying dance as treatment for orthorexia. Conclusively, eating disorders and specifically orthorexia affect one’s entire being — physical, mental, emotional, social, spiritual health — and interfere with one’s entire life and daily functioning. Dance, as a holistic therapeutic approach, is effective in addressing and remedying every single one of these elements, healing one’s whole self. Therefore, it is suggested that dance may be an effective treatment for orthorexia.

Abstract:
Promoting Alternative Thinking Strategies (PATHS) is an intervention program for children with behavioral and emotional deficits, designed for use, and shown to be effective when used in the classroom a minimum of 3 times per week. However, in some settings, as in the current study, PATHS is being used just once per week. The purpose of this quantitative study was to determine whether PATHS was beneficial in helping elementary school aged children improve their behavioral and emotional health when implemented once per week in a group therapy setting. PATHS was developed based on cognitive behavioral theory, which focuses on improving internalizing symptoms of mental health disorders (thoughts) as well as the externalizing symptoms (behaviors). A one-way, repeated measures ANOVA was utilized to analyze archival data of 193 scores, collected over a single school year. Results indicated that elementary school aged children who received PATHS once per week in a group setting showed a decrease in aggression and disruptive behaviors, and an increase in concentration and attention as well as social and emotional competence. Social change implications could involve the results of the study informing how we might promote overall emotional and behavioral well-being in children. At the organizational level, the expansion of the use of PATHS at reduced costs and time within other settings will extend these benefits to more children with behavioral and emotional deficits. Future studies are suggested to examine further the effectiveness of PATHS when implemented in other programs and alternative ways.

Abstract:
School-Wide Positive Behavior Interventions and Support (SWPBIS) is an evidence- based application of Applied Behavior Analysis (ABA) to address and prevent problem behaviors from occurring, promote pro-social behaviors, and create a positive learning environment for all students. There are many secondary interventions that have been utilized within the framework of SWPBIS that have high success rates. However, the research conducted on the use of Check-In/Check-Out (CICO), a secondary intervention, has focused its attention more on its effectiveness in public elementary schools. There is a lack of research evaluating the effectiveness of CICO in alternative school settings. This study provides an extension to the literature by examining the effects of the CICO program with school-aged children residing in a mental health treatment facility. Using a concurrent multiple baseline across participants design, students were exposed to a CICO intervention strategy in which problem behaviors were targeted for reduction and academic engagement was targeted for acquisition. All three students showed substantial decreases in problem behavior and increases in academic engagement when the CICO intervention was in place.

Abstract:
Background. Primary and secondary prevention trials have shown that implantable cardioverter-defibrillators (ICD) reduce the risk of cardiac death, but concerns have been raised regarding the psychological well-being of ICD patients. Anxiety can affect a significant proportion of these patients, but there is limited information about prevalence and determinants of anxiety after the implementation of the more recent guidelines for ICD implantation. Several behavioral interventions have been effective in improving anxiety in these patients, however the efficacy of mindfulness-based interventions (MBI) has not been investigated in ICD patients, and there is limited information regarding the characteristics of pre-intervention, “dispositional” mindfulness in patients with cardiovascular disease never exposed to mindfulness training. The aims of this dissertation project were: 1) To determine the feasibility of a randomized clinical trial of a phone-administered, mindfulness-based training program, as measured by recruitment and retention rates, treatment adherence and fidelity; 2) To evaluate the current baseline prevalence and determinants of anxiety in the study population and 3) To describe the correlates of dispositional mindfulness in the study population. Methods. The study was conducted at the Electrophysiology Service at the UMass Memorial Medical Center. All consecutive patients who recently underwent an ICD procedure or received ICD shocks were screened for eligibility to participate in a pilot randomized controlled trial in which an eight session, phone-delivered, weekly MBI was compared to a usual care condition. Assessments were performed at baseline and post-intervention. A cross-sectional design was used for aims 2 and 3. Anxiety was assessed using the Hospital Anxiety and Depression Scale; a shortened version of the Five Facets of Mindfulness questionnaire was used to evaluate mindfulness. Results. Thirty patients (21 M, 9 F; mean age 63.1 ±10.3 years) were enrolled in the study. The methods ultimately adopted to screen, recruit, and retain study participants were feasible to conduct and satisfactory to ICD outpatients, and the study intervention was safe. Phone delivery resulted in excellent retention rates and limited costs. Assessments of treatment fidelity showed that the content of the intervention was delivered as intended in almost 100% of cases. The study findings do not show a decrease in the overall prevalence of anxiety in ICD patients compared with earlier cohorts; anxiety was associated with young age, low socio-economic status and previous psychological morbidity, but not with ICD-related factors including prior shock delivery. Finally, baseline mindfulness was most strongly associated with previous psychological morbidity (in particular, depression), and current anxiety symptoms. Conclusion. Psychological morbidity appears to be the major determinant of anxiety in the patients currently enrolled in the study. Dispositional mindfulness is inversely associated with current anxiety and depression and with prior psychological morbidity, supporting the hypothesis of a modulating role of mindfulness on the processing of negative emotions. A phone-delivered, individual MBI is feasible, acceptable to patients and can be adequately delivered by trained instructors. The findings from this dissertation work support the need for larger clinical trials of MBI in ICD patients.

一、背景 中風是臨床常見病多發病,病死率與致殘率均高,流行病學調查結果顯示,我國中風發病率為(109.17〜217)/10萬,中風發病6個月以後仍遺留程度不同的偏癱、麻木、言語蹇澀不利、口眼喎斜、癡呆、情感障礙等後遺症,患者生活品質嚴重降低,其後遺症之一情感障礙以中風後抑鬱症(post stroke depression,PSD)為主。隨著中風的發病率持續增高,中風後抑鬱症同樣呈上升趨勢,國內外研究顯示,中風發病後6個月至2年間PSD的發生率和嚴重程度最高,發病率為30%〜70%,高於其他肢體殘疾患者人群抑鬱發生率(15.1%〜22.5%)及普通人群抑鬱發生率(3%〜6%)。中風後抑鬱症能推遲神經功能缺損恢復,延長恢復時間並使勞動力喪失,降低患者的生活品質,加重患者的精神痛苦,而且使患者對康復方案的實施缺乏積極性和主動性,嚴重影響治療和健康,甚至還會導致患者死亡。近年來,隨著中風患者存活率的提高、神經康復的普及和神經心理學的發展,中風後抑鬱症也越來越引起人們的廣泛關注。中西醫治療中風後抑鬱症有著較好的療效,尤其是針灸治療中風後抑鬱症有著效果顯著、不良反應很少出現、明顯提高患者的生存率和生活品質等優勢。筆者將对近年電針治療中風後抑鬱症的临床文獻做一次評價和探討。 二、目的 通過對近五年針灸治療中風後抑鬱症的國內外臨床研究文獻進行全面檢索,通過對其48篇相關文章的方法學品質評價,對該治療方法進行分析總結,以使電針在治療中風後抑鬱症發揮更好的作用,從而為臨床醫療服務。 三、方法 採用中國生物醫學文獻光碟資料庫(CBM disc)、中國期刊全文資料庫(CNKI,期刊全文資料庫,學位論文資料庫)、中文科技期刊全文資料庫(VIP),檢索近5年國內有關電針治療中風後抑鬱症的臨床研究文獻,用循證醫學的方法對所收集的符合標準的文獻進行系統評價和分析,並對其文獻品質以及目前電針治療中風後抑鬱症的臨床取穴規律、方法等進行總結,為臨床實踐提供更可靠的依據。 四、結果 在收集的電針治療中風後抑鬱症的文獻中,同時採用國家級的HAMD和CCMD-3有28篇文獻,占58.3%。有中醫診斷標準的文獻有12篇,占25.0%。共同使用HAMD和抑鬱自評量表(SDS)作為評價指標的文獻有14篇,占29.2%。文獻中治療中風後抑鬱症的十大常用穴位為百會、太沖、内關、印堂、神庭、合穀、足三裡、四神聰、神門、三陰交。 五、結論 我國近5年來電針治療中風後抑鬱症的文獻表明電針治療有效,但文獻中採用的隨機方法的品質及可信度較低,盲法幾乎未被應用。這些問題在一定程度上阻礙和減弱了電針治療中風後抑鬱症的推廣和普及。為了提高電針治療中風後抑鬱症的臨床水準,需要採用高品質的臨床研究設計,從而為臨床實踐提供更為可靠的依據。 六、關鍵字 電針 中風後 腦卒中 抑鬱症 臨床研究

Abstract:
The study reported in this thesis describes and analyses the experience of 15 men with long term mental illness, living and/or working at a rural farm and hostel complex. The research was set in the context of recent studies and literature focussing on the transfer of care of people with mental illness from large psychiatric hospitals to community based care and the consequent quality of life issues. A phenomenological approach was chosen as the most appropriate for the study, in order to understand the meaning of the residents' experience. Data was obtained from three sources: in-depth interviews with 2 key informants, 50 hours participant observation involving 15 residents and 8 staff and focussed or semi-structured interviews with 10 participants. The Ethnograph computer program was utilised to facilitate and expedite analysis of the resulting large volume of data.

This paper presents a literature review on the effectiveness of complementary and alternative therapies (CAT’s) on depression with specific reference to Reiki and depression. The concept of complementary and alternative medicine (CAM) is explained. The use of complementary and alternative medicine is widespread and it is noted that those with psychiatric disorders are more likely to use CAM than those with other diseases. The discussion then focuses on Reiki, providing some background information to orientate the reader. The studies, reports and claims regarding the use and efficacy of Reiki are enumerated and evaluated with the aim of trying to establish whether Reiki has been used and therefore could be used to treat depression in women.

Prisons in the South African context are considered to be places of correction for deviant individuals. The perception of the functioning of a correctional space is the correction of a particular type of functioning and the construction of another. One of the underlying assumptions of a penal system is the notion of a subject who is able to undergo a process of normalisation. This dissertation is directed towards the exploration of a different type of space that has been created within the correctional system at the Diepkloof Prison. It will be suggested herein that creative workshops, run within the prison by an outside facilitator, have succeeded in subverting the normalising discourse of the penal system, and have helped to facilitate a different type of healing experience within the confines of the institutional space. In order to place the discussion of the workshopping process within a sound theoretical framework, various theoretical questions regarding the shift from modernist to postmodern psychology are explored in some depth. It will be argued that the ideas emerging from social constructionist and narrative psychology, deconstructionist notions of language and subjectivity, as well as post-structuralist ideas on disciplinary power, offer a theoretical framework from within which to understand the healing process that occurs in the workshops themselves.

This thesis, which includes a dual case study, explores the clinical use of art therapy with women with eating issues from a feminist perspective. It provides a critique of the existing art therapy literature, and suggests alternative approaches which may be incorporated into an art therapy intervention, to increase its relevance to this client group. It demonstrates the need for flexibility and creativity on the part of the art therapist, particularly with regard to the provision of structure and containment. It offers an example of qualitative research methods which were easily incorporated into clinical practice, as a means to introducing clients’ voices into art therapy narratives as well as evaluating practice. These research methods are suggested for future art therapy research projects. This study found that a short term feminist informed art therapy intervention was able to meet its goals of increasing clients’ self awareness and insight into their eating issues. The use of art therapy was found to be one of the strategies which contributed to the success of this intervention. Other important aspects included: the adoption of a feminist model; the use of journalling; a flexible approach; a concern with what lay beneath the eating issue; and a focus on self-care and nurture.

This research study shows that young HIV sufferers undergo disturbances involving the mind, body and spirit causing a variety of future adjustment difficulties, problems and coping abilities. This is attributed to fear, loss, grief, guilt, denial, anger, anxiety, depression, stress, physiological changes, pain and hypochondria. This study indicates that they are particularly fearful about being isolated and rejected. They are overwhelmed by thoughts of loss of everything, accompanied by profound feelings of grief. Guilt and self-reproach affect their health further, having an effect on them both physically and mentally. The uncertainty of their future having contracted HIV intensifies anger with themselves as well as others. This also brings about anxiety due to lack of information and appropriate support systems in Soweto...

Abstract:
In many cultures and religions of the world the belief in transcendental realities, like God, the Devil and other benevolent and malevolent spirits are widespread. These realities are constructed in different ways, depending upon context. In light of the development Western industrial societies have undergone, it follows that the belief in transcendental realities may have given way to beliefs that are steeped more, in empiricism. However, understanding the belief in transcendental realities seems to be gaining renewed interest in various social science disciplines in light of the fact that claims of experiences of spirit possession are escalating. The implication this understanding has on the treatment a patient receives is likely to depend upon the training of the practitioner consulted. It follows that the experiences of patients who believe they are possessed by a spirit is as equally important to practitioners of psychology as it is to theologians. Psychologists would likely ascribe a spirit possession to a psychological experience; while theologians will attest to the presence of a spiritual illness. This project focussed on Free Church pastors in Germany and their perceptions of spirit possession and mental illness. To explore Free Church pastors understanding of spirit possession and mental illness is critical in light of the overlap of symptoms. Misdiagnosis may result in a client receiving treatment which may not be appropriate. Interviews with Free Church pastors were conducted. The results were analysed and 4 themes were identified. Based on these interviews conclusions could be drawn which ultimately made it clear that the German free church pastors’ theological training needs to be supplemented in the area of psychology and that the pastors are unable to cope in the area of ‘spirit possession or mental illness’

Abstract:
Background. A sizable amount of individuals in the community are presented with various kinds of physical and mental health problems which are either undetected, untreated or inadequately treated, due to the limitations on the availability and accessibility of the services in the existing health care system, or to other social and personal reasons. The current study evaluated the effectiveness of a newly developed modality of health-enhancing treatment---the Mindfulness-based Dejian Mind-Body Intervention, as compared to that of a Group Psychoeducational Treatment, in alleviating depressive mood and improving physical health of adult individuals in the community.
Conclusions. Findings of the current study suggest that compared with the Group Psychoeducational Treatment, Dejian Mind-Body Intervention might be more effective in enhancing the emotional and physical health of community individuals presented with moderate to severe depressive mood and/or problems with bowel functioning.
Method. Forty adult volunteers with various degree of depressive mood and physical problems who expressed interest in receiving either Dejian Mind-Body Intervention or Group Psychoeducational Treatment were recruited in the current study. They were matched for gender, age, education and level of depression, and were randomly assigned to either treatment group.
Results. Both the Dejian Mind-Body Intervention and Group Psychoeducation Treatment were effective in bringing about a significant reduction in depressive mood iv among treatment completers. However, differential effectiveness emerged among those presented with moderate to severe depressive mood, where Dejian Mind-Body Intervention resulted in significantly greater treatment-related reduction in depressive mood compared with the Group Psychoeducational Treatment. Besides, Dejian Mind-Body Intervention brought about significant increase in an objective QEEG measure of positive affect, and improvements in physical health (i.e., bowel functioning) that were not evidenced in the Group Psychoeducation Treatment.

Abstract:
This thesis examines how military members and veterans with Operational Stress Injuries are treated by Canadian justice systems. It suggests a correlation between mental injuries sustained on operations by military personnel and propensities for military and societal misconduct. By comparing civilian and military processes with American justice counterparts, a plan to improve the existing Canadian legal landscape is proposed. Using an analysis of the underlying philosophy and purpose of military justice, a problem solving diversionary court is recommended, along with legislative and policy amendments. The use of a consent-based “Treatment Standing Court Martial” would place military justice officials parallel to civilian justice alternative measures programs, and in a better position to break the cycle of recidivism among veterans by addressing root causes. Education to reduce stigma along with military-civilian partnerships are also advocated to enhance the detection of mental illness and to foster early treatment for military personnel and veterans. The overall goals of the work include: reducing recidivism, improving operational efficiency and taking care of military members, veterans and their families.

Abstract:
This research journey examines the process of co-creating, along with my co-travellers (research participants), a more holistic approach to trauma debriefing. Whilst incorporating elements of Mitchell's (1983) Critical Incidence Stress Debriefing model (C.I.S.D.), our point of difference has been to adopt a narrative pastoral approach. This was achieved by introducing spirituality and narrative therapy practices. Our research pathway has been further shaped by adopting a qualitative research approach within a postmodern, social construction discourse. My research curiosity was invited by both the mind, body and soul divide expressed in psychology; and the healing possibilities of spirituality I have witnessed in both my own and in the lives of others. While my co-travellers predominantly preferred to express their spirituality in terms of Christianity, our narrative pastoral approach to trauma debriefing is offered to people of all religious persuasions.
