Esta pesquisa aborda a rela??o entre o corpo e a est?tica, compreendida como padr?o corporal, com o objetivo de analisar o corpo masculino na Revista Men?s Health. A Revista em pauta ? uma publica??o mensal da Editora Abril, estando presente em mais de 43 pa?ses. A metodologia utilizada ? a an?lise de conte?do como proposta por Bardin (1979), visando identificar sentidos sobre o corpo masculino divulgado nessa m?dia, especificamente na se??o Fitness. O corpus de an?lise foi composto por 12 edi??es da revista, veiculadas de janeiro a dezembro do ano 2011. Elaboramos fichas de identifica??o para todas as mat?rias contidas no sum?rio da Se??o Fitness e, em seguida, fizemos os perfis das mat?rias construindo cinco categorias tem?ticas: Apar?ncia, Investimentos no corpo, Individualismo, Consumo, Bem-estar. A Men?s Health, atrav?s de suas imagens e discursos, apresenta v?rios conselhos e recomenda??es que apontam caminhos e atitudes a serem seguidos, influenciando o homem a ser jovem, belo e saud?vel. A partir da an?lise realizada, pode-se afirmar que na Revista Men?s Health a apar?ncia encontra-se ligada a uma ideia de um corpo magro e musculoso. Para a obten??o do corpo propagado pela revista, s?o necess?rios v?rios investimentos e pr?ticas de consumo. Abstract:
Composites are in high demand; however, fasteners are often required for joining process and can reduce their advantages. One solution is adhesive bonding, but uncertainty exists regarding long term durability and the ability to interrogate bonds noninvasively. One potential solution to qualify bond integrity over its service life is to dope an adhesive with magneto-electric nanoparticles (MENs). MENs can yield output magnetic signatures that are influenced by bond quality and damage state. In this study, adhesives have been doped with MENs prior to bonding at 1% volume concentration. For optimum implementation, this health monitoring system should be evaluated for effects of the MENs on the mechanical properties. Lap-shear testing was conducted to assess changes in the bond strength from addition of the nanoparticles. End-notched flexure (ENF) tests were also conducted for fracture mechanism evaluation. Results showed an increase of 12% in shear strength as a function of MENs loading concentration. In addition, a feasibility study of output magnetic signature as a function of elevated temperature and humidity were evaluated for MENs doped and un-doped adhesives. Results gave an order of magnitude change in magnetic signal as a function of exposure time.

Abstract:
Prostate cancer is one of the most prominent diseases in men’s health. It is inherently 'male', given the exclusivity of the prostate gland to men’s bodies and its physiological connection to testosterone and male sexuality. The biomedical complexities of prostate cancer continue to be unravelled and researched and are often connected to identifying causes, the virtues of screening and treatment modalities. However, despite the biological male 'sex' link, most of the prostate cancer research is not connected with research on gender relations, men and masculinities. The net outcome is that men’s lives and illness experiences are absent in much of the prostate cancer research. This PhD thesis Prostate cancer: Anglo-Australian heterosexual perspectives, is an ethnographic study of thirty-five Anglo-Australian men diagnosed with prostate cancer. Participants shared their experiences of living with prostate cancer in the context of health promotion, health services and in relation to their sexuality and intimate relationships. Through participant photographic novella and in-depth semi-structured interviews, rich cultural insights are provided. A social constructionist gender analysis is used in this research that shows how the social constructions of masculinity interconnect and occasionally collide with prostate cancer throughout the illness trajectory.

O estudo analisa os discursos de homens e da revista Mens Health acerca do corpo, saúde e sexualidade. Para a construção dos discursos dos homens, realizamos entrevistas semi-estruturadas com 19 homens leitores e nove não leitores. E dois eventos de grupos focais que reuniram 11 homens no total. Foi entrevistado também o editor da revista. Os principais conceitos norteadores deste trabalho foram os de gênero, sexualidade, poder e masculinidades hegemônicas e subalternas. Evidenciou-se que a revista está fortemente atrelada à sociedade de consumo ao estimular a inserção dos homens em um mercado de produtos e serviços até então estranhos a esse gênero. E que suas concepções sobre saúde estão relacionadas a de bem-estar e de individualização que se articulam com os discursos hegemônicos que vêm dando sentido às concepções de saúde e doença atualmente. A publicação investe fortemente na ideia de um corpo musculoso que proporcionará ganhos sociais, sexuais e profissionais aos sujeitos, nem sempre atrelado às questões de saúde. Ela ratifica a heterossexualidade do leitor projetado, expondo o corpo feminino e o sexo heterossexual e silenciando sobre outras formas de sexualidade. Por isso consideramos que a revista se vincula a uma concepção tradicional da masculinidade. Seus discursos, no entanto, não são monolíticos ou isentos de contradição, e também manifestam nuances relativas a um modelo mais contemporâneo de masculinidade, como quando apresenta a ideia de uma nova pedagogia da sexualidade e a valorização dos cuidados estéticos e de saúde com o corpo, aspectos considerados pouco próximos da masculinidade tradicional. Com relação aos discursos dos homens, evidenciou-se que a classe social e a geração são as variáveis mais importantes nas suas concepções sobre corpo, saúde e sexualidade masculina. Que, entre os não leitores, de modo geral, há evidências mais fortes de flexibilização com relação aos padrões mais tradicionais entre os homens mais jovens e/ou de classes mais altas. Enquanto os homens com idade acima dos 30 anos e das classes populares estão mais atrelados às concepções tradicionais. Entre os leitores, observou-se uma grande reflexividade com relação aos discursos da revista demonstrando que eles vêm se apropriando de forma importante dos discursos da revista e ressignificando suas concepções e práticas sobre os três temas da pesquisa a partir desses discursos. E, assim como os discursos da revista, os discursos dos homens, leitores ou não, também apresentaram aspectos contraditórios, ora demonstrando mais afiliação a um novo modelo de masculinidade, ora ao modelo mais tradicional.
The study analyzes the speeches of men and Men's Health magazine about the body, health and sexuality. For the construction of discourses of men, we conducted semi-structured interviews with 19 men and nine readers not readers. And two events focus groups that gathered 11 men in total. Was also interviewed the magazine's editor. The main drivers of this work were the concepts of gender, sexuality, power and hegemonic and subordinate masculinities. And that his views on health are related to wellness and individualization that articulate with the hegemonic discourses that are giving meaning to the concepts of health and illness today. The publication invests heavily in the idea of a muscular body that will provide professionals subject to social gains, sexual, and not always linked to health issues. She confirms heterosexuality designed the reader, exposing the female body and heterosexual sex and silencing of other forms of sexuality. Therefore we consider that the magazine is linked to a traditional conception of masculinity. His speeches, however, are not monolithic or free from contradiction, and also express nuances on a more contemporary model of masculinity, as when he presents the idea of ​​a new pedagogy of sexuality and appreciation of aesthetic and health care with the body aspects considered next bit of traditional masculinity. With regard to the resources of men, it became clear that social class and generation are the most important variables in their conceptions about body, health and male sexuality. That, among non-readers in general, there is stronger evidence of flexibility with respect to more traditional patterns between younger and / or more upper class men. While men above the age of 30 years and the lower classes are more tied to traditional concepts. Among readers, there was a big reflexivity with regard to the resources of the magazine showing that they are appropriating important form of speeches and magazine redefines its conceptions and practices of the three research themes from these speeches. And just like the speeches of the journal, the speeches of men, or not readers also showed contradictory aspects, sometimes showing more affiliation to a new model of masculinity, sometimes the more traditional model.

This thesis proposes that the idea expressed in Juvenal s quotation Mens Sana in Corpore Sano, produced in the Ancient Greek-Roman civilization includes undestandings of body, heath and education that were reapropriated and reorganized by scientific and pedagogical theories in the XIX and XX centuries. These theories, especially the ones that received contributions from the biomedical sciences, have influenced Physical Education in the search of becoming a science. In order to realize this reapropriation, we have analyzed the transformations in the concepts of body, health and education produced in the area discourse, aiming at pointing out elements to the configuration of a theory that is being called Corpore Sano. The theory constitutes in possible systematization of scientific, philosophical and pedagogical concepts, and as such, an understanding of the scientific fundaments of Physical Education. The corpus of this analysis was composed by 148 articles that were published in Revista Brasileira de Ci?ncias do Esporte digitalized in the period between 1979 and 2003 and selected according to theme: body, biology, physical activity, effort physiology and health. The analysis of the content and the referential interpretation allowed the combination of philosophical reflection with attention to the empiric field as a comprehensive dimension. Based on the corpus of the analysis it is possible to configure meta-arguments about the concepts of body, health and education in the scientific production of Physical Education. As a metaresearch. Our study did not intend to judge the analyzed production, but look for theoretical elements that may generate a reflection about the scientific rationality in Physical Education and developments in the pedagogical field and in body practices. Such a reflection might be recognized as a theory of the living body, susceptible to modifications and interrogations that are proper of knowledge and practices of Physical Education. It is a theory rejects the idea of the complete truth and holds the comprehension that the truth is built historically through relations among different kinds of knowledge and Physical Education practices

Abstract:
A saúde do homem surge como questão de Saúde Pública frente às elevadas taxas de mortalidade masculina quando comparadas às taxas de mortalidade feminina. Há algumas décadas, os estudos sobre homens buscam relacionar essas estatísticas com a questão de gênero, demonstrando que o padrão de comportamento adotado pelos homens, baseado na masculinidade hegemônica, e socialmente imposto, está relacionado ao comportamento do homem em relação ao processo saúde-doença. Este estudo pretende conhecer a população masculina adulta usuária de um serviço de atenção primária à saúde o Centro de Saúde Escola Barra Funda Dr. Alexandre Vranjac (CSEBF-AV) e o modo como essa população se relaciona com o serviço: quem são esses homens, quais são as suas demandas e como eles estão inseridos na unidade. A partir do projeto de pesquisa intitulado Homens, violência e saúde: uma contribuição para a linha de pesquisa e intervenção em gênero, violência doméstica e saúde selecionou-se amostra de 237 homens usuários do CSEBF-AV. Os homens foram individualmente entrevistados a partir do instrumento questionário e seus prontuários lidos a partir da ficha de leitura de prontuários, buscando-se conhecer aspectos sócio-demográficos, de saúde e de uso do serviço desses homens segundo o tipo de matrícula/ porta de entrada no serviço: moradores e trabalhadores da área de abrangência da unidade, homens em situação de exclusão social e homens cuja entrada no serviço esteve vinculada a condições de saúde-doença relacionadas à vigilância epidemiológica (VE). Os dados coletados foram analisados nos programas STATA v. 9 e Epi Info v. 6.04b. Os homens, em sua maioria, pertenciam ao grupo etário de 35 a 54 anos, tinham 8 a 11 anos de estudo, pertenciam aos estratos sociais C e D, tinham relacionamento estável, moravam com a família em casa/ apartamento e estavam empregados. Em relação às questões de saúde, a maioria dos homens entrevistados mostrou boa percepção de sinais e sintomas, queixas gerais (43,5%) como principal demanda em saúde, uso importante de bebidas alcoólicas (62,4%), uso do preservativo masculino para prevenção de infecções sexualmente transmissíveis (54%) e opção (do casal) por laqueadura ou pílula anticoncepcional como método contraceptivo (41,4%). Em relação ao uso do serviço a maioria dos homens fazia uso de consultas programadas (70%) e buscava primeiramente um centro de saúde frente a um problema de saúde (57,8%). Observamos que moradores e trabalhadores da área apresentaram padrão de uso do serviço e de demandas semelhantes, enquanto que os homens seguidos pela VE apresentaram padrão específico de uso do serviço e demandas, devido à sua inserção diferenciada no serviço. Os homens em situação de exclusão destacaram-se por apresentar diversas características que os tornam mais vulneráveis às questões de saúde, desde fatores sócio-demográficos a formas de uso dos serviços. Acreditamos que as diferenças observadas entre os quatro grupos de homens poderiam estar relacionadas às questões de gênero, além de outros fatores. Ainda, a maior vulnerabilidade dos homens em situação de exclusão às questões de saúde aponta para a necessidade de uma organização do CSEBF-AV baseada nas relações de gênero, elaborando ações e estratégias que atentem para as necessidades comuns e específicas dos diferentes grupos de homens que fazem uso desse serviço.
The mens health has emerged as an important concern to Public Health. Its because of the male deaths rates are higher then the womens rates. In last decades, mens studies have been looking for relations between these statistics and gender, and they have demonstrated that the behavior pattern followed by men, based on hegemonic masculinity, and socially expected, its related to mens behavior and the way they have been taken care of their health. The present study intends to know the adult male patient population from a primary health care the Centro de Saúde Escola Barra Funda Dr. Alexandre Vranjac (CSEBF-AV) and how these population have been using the health center: who are those men, which are their health demands and how they are insert in this health center. From the research project Homens, violência e saúde: uma contribuição para a linha de pesquisa e intervenção em gênero, violência doméstica e saúde we have selected a source of 237 men patients who are users of this health center. They were individually interwied by researches and a survey was applied to their medical records, intending to know their social aspects, their health and the way they have been using the health center, according to their service?s register category: people who resides around the health center, people who works around the health center, people from lower social-economic levels and people that was registered at the service because of some health condition involving epidemiologic surveillance. The data were analyzed by STATA v. 9 and Epi Info v. 6.04b programs. Mostly men were aged between 35-54, they had had 8 to 11 years study, they were from middle class, they were living stable relationships, they were residing in house/ apartment whit their family and they were mostly workers. About health issues, mostly men showed to be able to perceive signs of health problem, general demands appeared being their main health demand (43,5%), they were evidence for alcoholic abuse (62,4%), safe sex practices (54%), birth control depending on women (41,4%). About the use of the health center mostly men were used to come up to their doctors appointment (70%), they looked first for health center when they were feeling ill (57,8%). Men who resided around the health center and men who worked around the health center had similar use oh the health center, men that were registered at the service because of some health condition involving epidemiologic surveillance had specifics ways in using the health center. Men from lower socio-economic levels showed they were more vulnerable to diseases risk factors. We believe that the differences found among the groups of men could be related to gender, besides other factors. The CSEBF-AV organization based on gender will provide health actions improvement to all different mens groups.

Participant recruitment is a crucial component of clinical trial conduct yet an estimated 50% of trials fail to recruit to target. Trialists seeking strategies to boost participant recruitment are faced with a lack of good-quality evidence to guide their decision-making. In this context, recruitment activities are often conducted in a trial-and-error manner, resulting in wasted time and resources, and risking trial failure. Men have a lower life expectancy than women, and are also at increased risk of a number of chronic diseases including diabetes. Nevertheless, men have been underrepresented in diabetes prevention trials, and the published literature on how best to engage men in trials is limited. The aim of this research was to identify and evaluate strategies to recruit men into clinical trials. The research was set with the Testosterone for the prevention of Type 2 Diabetes (T4DM) study, a large, multi-centre, phase III, double-blind, placebo-controlled two-year trial of testosterone therapy combined with a lifestyle intervention (Weight Watchers®) compared to the lifestyle intervention alone for the prevention of Type 2 diabetes. Between January 2013 and February 2017, 19,022 participants were screened and 1007 randomised to the T4DM study. The final study visit was conducted in May 2019. A systematic review of strategies to recruit men aged 50 years and over to clinical trials was conducted. In the included studies, the most effective strategies for identifying prospective participants were referral from an affiliated health service, media coverage and mass mailing. Identification of participants through community outreach activities such as displaying posters and attending local community events was not effective. Site staff training that focused on trial-specific recruitment challenges was also effective in increasing recruitment. Of the 16 included studies, only one was assessed to be of good quality. The most common sources of bias in fair and poor quality studies were inadequate description of recruitment interventions, and failure to report cost data. Next, a program of three recruitment evaluations was conducted within the T4DM study. First, a range of promotional strategies to identify prospective T4DM participants was developed and evaluated. Repeated, high frequency bursts of radio advertising, infrequent but high reach television news coverage, and direct mass mail outs by a government health agency were the most effective recruitment strategies. Promotions through community groups and businesses, online advertising, referral by general practitioner, coverage on newspaper and radio news bulletins, and newspaper advertising were also implemented but were evaluated to be less effective. Promotional activities cost an average of AUD$594 per randomised participant (ranging from no direct cost for television, radio and newspaper news coverage to AUD$1941 per randomised participant for newspaper advertising). Second, a centralised, semi-automated approach to screening and enrolling T4DM participants was implemented and evaluated. The sequential screening process comprised: (i) web-based prescreening, (ii) laboratory eligibility screening at a network of third-party pathology centres, (iii) final on-site clinical screening. This approach delivered high-volume screening at low cost and required few staff. Efficiencies were achieved by pre-screening the majority of subjects (95%) online, automating email communications with prospective participants, outsourcing blood collection for laboratory screening to a third party pathology centre, and automating eligibility ascertainment up to the point of final clinical screening. Screening and enrolment cost on average AUD$1411 per randomised participant, including direct and indirect costs. This represented a considerable cost saving compared to previous studies in similar disease settings. Third, a ‘study within a trial’ (SWAT) was implemented to address an identified roadblock in trial recruitment. Phone call and Short Message Service (SMS) reminders were designed to increase screening uptake in participants who did not attend a further screening assessment after completing pre-screening. In a randomised comparison (N=709), there was no significant difference in screening uptake between those who received an SMS reminder and those who received a telephone call reminder (18% (62/354) and 23% (80/355) respectively) (RR=1.29, 95% CI 0.96–1.73, p=0.09). Both SMS (95% CI 14%–22%) and telephone call reminders (95% CI 18%–27%) increased screening uptake compared to neither reminder (12%). SMS reminders cost substantially less than telephone call reminders (AUD$0.53 versus AUD$6.21 per reminder), making them an adequate alternative to telephone call reminders to boost screening uptake. In total, this research provides a detailed and costed approach for recruiting men to diabetes prevention trials. The estimates of cost and effectiveness reported in this research will be of use to trialists seeking to recruit similar participant populations. More broadly, the methods for implementing and evaluating recruitment strategies presented in this thesis will be generalizable to many trial settings.

Background: The human secondary sex ratio has been the subject of long-standing medical, environmental and social scientific curiosity and research. A decline in male birth proportion in some industrialised countries is linked to endocrine disruption and is validated by some empirical studies. Increasing parental age and population stress and associated decreases in sex ratio have also been demonstrated. A thorough literature review of 123 relevant and diverse studies provides context for these assessments. Methods: A spatial-temporal investigation of birth sex ratio in Scotland and potential determinants of endocrine disruptor pollution, socio-economic factors including neighbourhood stress, deprivation, smoking, and maternal age, was conducted. This involved review of national and regional sex ratio time trends, and stratified/spatial analysis of such factors, including the use of GIS tools. Secondary data were sourced from Scottish Government web portals including Scottish Neighbourhood Statistics and the Scottish Environmental Protection Agency. Results: Regional differences in sex ratio between 1973 and 2010 are observed which likely lever the national male birth proportion downwards, with the region of poorest air quality from industrial emissions, the Forth Valley, displaying the greatest sex ratio reduction. Further analysis shows significant upwards skewing in sex ratio for the population cohort experiencing the least and 2nd most deprivation. Localised reductions in sex ratio for areas of high modelled endocrine disruptor pollution within the Central Region in Scotland are also displayed. Discussion: Limitations of the analyses include the danger of ecological fallacy in interpreting from area-based measurement and the simplified pollution modelling adopted. Despite this, and given elevated incidence of testicular cancer in Scottish regions mirrors the study’s results, tentative confirmation of the endocrine disruptor hypothesis can be substantiated. Further, elucidation on advanced parental age as a contributory factor to secondary sex ratio change is also given. Recommendations are made with respect to environmental monitoring and health protection, and preventative health strategies in Scotland.

Há poucos estudos sobre saúde mental masculina, em especial quando a temática é a violência. Os trabalhos sobre homens e violência sofrida destacam eventos fatais, oferecendo pouca visibilidade à violência não fatal. No presente estudo, analisou-se a associação entre esses dois eventos, cuja relação é pouco explorada. Metodologicamente, realizou-se um estudo transversal, com 477 usuários masculinos de serviços de atenção primária, de 18 a 60 anos. A seleção amostral foi do tipo consecutivo, por ordem de chegada em dois serviços de atenção primária na cidade de São Paulo. Foram coletadas características sócio-demográficas, de saúde e relatos de experiência de violência sofrida ao longo da vida (adulta, na adolescência e/ou na infância) e de violência testemunhada na infância. Também foram coletadas informações sobre uso do serviço de saúde mental e/ou queixas/diagnósticos psicológicos em consulta da clínica médica, por meio de leitura de prontuários médicos, para categorização da variável dependente sofrimento mental. As variáveis foram descritas por frequências e proporções, apresentando 29,4% (140) de homens com sofrimento mental. Dentre esses homens, 45,7% sofreram violência física e/ou sexual mais de uma vez na vida. Em relação à violência sofrida na infância e/ou adolescência, metade dos homens com sofrimento mental havia tido algum episódio de violência psicológica, física e/ou sexual antes dos 18 anos. E 61,4% dos homens haviam perpetrado algum tipo de violência além de sofrê-la durante a vida. Por meio de análise multivariada desenvolveram-se três modelos de Poisson com variância robusta do tipo confirmatório, ajustados pelas variáveis sócio-demográficas, testemunho de violência na infância e uso de substância psicoativa. No modelo I, sofrimento mental foi significativamente associado apenas com sofrer violência física e/ou sexual recorrente. (RP:1,75; IC95%:1,13-2,72), já que a violência em episódio único perdeu a significância após a inclusão da variável de uso de substância psicoativa. No modelo II, o mesmo desfecho esteve associado a sofrer violência antes dos 18 anos, sendo que sofrer violência na infância e/ou adolescência aumenta o risco de sofrimento mental na vida adulta em 1,93 vezes (IC95% 1,25-3,00), e sofrer qualquer tipo de violência apenas na vida adulta não se mostrou associado a sofrimento mental. No modelo III, a exposição a sofrer e perpetrar violência foi associado a sofrimento mental, enquanto apenas sofrer ou apenas perpetrar não mostraram significância estatística. Em todos os modelos, o uso de substância psicoativa representou uma importante variável de ajuste. Conclui-se que há importante relação entre sofrimento mental e sofrer violência, e a ocorrência desses episódios nos primeiros anos de vida podem provocar agravos à saúde mental na vida adulta, chegando a ter associação com 61,4% desses agravos. Tal relação, já salientada nos estudos com mulheres, é relevante também para a saúde dos homens, chamando a atenção para a igual necessidade da identificação nos serviços de saúde de situações de violência experimentadas pela população masculina, bem como maior atenção aos agravos em saúde mental nessa população
There are few studies about mens mental health, particularly when the theme is violence. The studies on men and suffered violence highlight fatal events, providing poor visibility to non-fatal violence. In this study, it was analyzed the association between these two events, which relation is little explored. It was performed methodologically a cross sectional study, with 477 men using primary care services, aged from 18 to 60 years. The screening was consecutive, in order of arrival in two primary care services in the city of São Paulo. Social-demographic, health characteristics and reports of experience of violence suffered over lifetime (adult life, adolescence and/or childhood) and of violence witnessed during childhood were collected. It was also collected information on the usage of mental health service and/or psychological complaints/diagnoses during consultation at medical clinics by reading medical records, for categorization of the dependent variable mental suffering. The variables were described per frequency and proportions, showing 29.4% (140) of men with mental suffering. Among these men, 45.7% have suffered physical and/or sexual violence more than once in life. Regarding the violence suffered during childhood and/or adolescence, half of the men showing mental suffering had suffered some episode of psychological, physical and/or sexual violence before the 18 years of age. And 61.4% of them had perpetrated some type of violence besides suffering it during lifetime. Using multivariate analysis, three confirmatory Poisson models with robust variance of were developed, adjusted by variables of social-demographic, witness of violence during childhood and usage of psychoactive substance. In the Model I, mental suffering was significantly associated only to suffering recurrent physical and/or sexual violence. (PR:1.75;95%CI:1.13-2.72), since violence in only episode lost significance after inclusion of the variable of psychoactive substance usage. In the Model II, the same outcome had been associated to suffering violence before the 18 years of age, considering that suffering violence during childhood and/or adolescence increases the risk of mental suffering in adult life by 1.93 times, (95%CI:1.25-3.00), and suffering any type of violence only in the adult life was not observed to be associated to mental suffering. In the Model III, suffering and perpetrating violence was associated to mental suffering, while suffering only or perpetrating only did not show statistical significance. In all the models, psychoactive substance usage represented an important adjustment variable. This study came to the conclusion that there is an important relation between mental suffering and suffering violence, and the occurrence of these episodes during the early years of life can cause adverse effects to mental health in adult life, being associated to up to 61,4% of these aggravations. Such relation, already highlighted in studies with women, is also relevant for mens health, drawing attention to the similar need of identification in the health services for situations of violence experimented by the male population, as well as greater attention to the aggravations on mental health in such population

Abstract:
Men are less vulnerable to HIV acquisition than women, but have poorer HIV-related outcomes. They access HIV services less often and later, and are more likely to die while on antiretroviral therapy (ART). The adolescent HIV epidemic presents further challenges, and AIDS-related illness is the leading cause of death among adolescents in sub-Saharan Africa. Such deaths have tripled since 2000, while declining in all other age groups. There is a clear need to better understand health practices for adolescent boys and young men living with HIV, and the processes through which these practices are formed and sustained. This doctorate explores the biosocial lives of adolescent boys and young men living with HIV in the Eastern Cape Province of South Africa. It engaged health-focused life history narratives (n=36), semi-structured interviews (n=32) and analysis of health facility files (n=43), alongside semi-structured interviews with traditional and biomedical health practitioners (n=14). Young male participants were among the first generation to grow up with access to ART and democratic freedoms. In a context where HIV-positivity and men’s inability to fulfil traditional roles are considered signs of social and moral decay, they felt pressure to be ‘good’ HIV-positive patients and respectable young men. As younger children, they performed to norms of HIVpositive patienthood. As they became older, norms of masculinity, including financial achievement, ulwaluko (traditional initiation/circumcision), ‘moral’ behaviour and engaged fatherhood became more important and began to conflict with performances of ‘good’ patienthood. This was most apparent during and following ulwaluko, where societal norms made it difficult to engage with biomedical treatment and care. Despite this, participants and their families demonstrated agency, creativity and resilience in subverting and re-signifying these norms. Participants did not access traditional products or services for HIV-related issues, a finding that deviates from much of the literature. This study suggests that health practices are mediated not only by gender and culture, but also childhood experiences of growing up deeply embedded in the health system, through which participants forged additional health-seeking tools. Findings affirm the syncretic nature of traditional beliefs, documenting the plural and complementary ways that participants engaged with traditional products and services.

Abstract:
The United States has a drug issue that is perpetually problematic. Efforts are being made on every level of government to reduce drug use and deter current and potential future users. Some of these efforts however are putting citizen's rights at risk in a manner that threatens the United States Constitution that hails over both the state and federal governments. My thesis will examine Florida's avant-garde approach to simplifying drug convictions through unprecedented legislation that has already been ruled unconstitutional on its face by the United States District Court for the Middle District of Florida. The decade long struggle will soon culminate in the Florida Supreme Court, and if found unconstitutional, could potentially impact thousands of inmates among other legal consequences. Through literature review and case study I will discuss the history of this issue and conclude by discussing possible rulings of the Florida Supreme Court in State v. Adkins, SC11-1878 (2D11-4559, 2nd DCA). In addition, I will analyze the case timeline that led to the legislative action which is being called into question in Adkins. I hypothesize that the ruling in Adkins will declare Florida's drug statute unconstitutional; however, I further presume that the currently incarcerated defendants will continue to serve their sentences virtually unaffected by the ruling, with some extraordinary exceptions. First, I will discuss the underlying legal premises, succeeded by an analysis of all pertinent case law and literature to assess the constitutionality of Florida's drug statute to further support my hypothesis. My goal for this thesis is to give perspective to the layperson as well as contribute to the statewide legal community through my organization of the subject, and analysis of case law.

This thesis explores the interconnections between masculinity, health and prison. It contests reductionist, individualist and biomedical approaches to health care management in prisons and challenges gender-blindness within criminology and social science where masculinities have been overlooked as key factors of prison culture and organisation. The research set out to explore how masculinities manifest at institutional, social and cultural levels in prison as key determinants of health. The study was conducted in an enhanced wing of an adult male training prison in Southern England. A reflexive ethnographic approach was used, comprising sustained (non-participant)observation, focus group interviewing, and in-depth, semi-structured interviews with thirty-five inmates and four prison officers. The research revealed how prison masculinities were produced and performed by inmates and prison staff, and through the discourses and practices of the prison regime. They were manifested at social and organisational levels as key determinants of health as 'deprivations' associated with imprisonment and as 'importation factors' reflecting inmates pre-prison health status. Values of the institution and those of inmates and staff combined to create a pervasively 'masculine' atmosphere and culture, which adversely affected the physical and mental health of many prisoners. This thesis recommends that health policy for prisons is developed and organised with consideration to issues of gender and power. The masculine ideology that underpinned the organisational and social fabric of the prison in this study was evident in the attitudes and behaviours of inmates and staff and in the 'progressive regime' advocated by the Prison Service. This research shows that a broad, holistic and 'gendered' view of prison health can provide alternative insight into men's health in prisons, and therefore offer a positive and productive way forward for future prison health policy, in line with the World Health Organisation's Healthy Prisons philosophy.

Asthma is a chronic condition affecting approximately 235 million people worldwide, yet international studies have identified that most adolescents with asthma have poor self-management skills. Asthma is common in adolescent males however only a few studies have specifically investigated the asthma experiences of this population group. The aim of this study was to explore how male adolescents understand and manage their asthma symptoms. A qualitative descriptive study design was selected to investigate the experiences of male adolescents, focusing on their perceptions of societal and masculine influences on their asthma management. Individual semi-structured interviews were undertaken with 15 male adolescents to capture data about their perceptions of masculinity, asthma management, interpersonal relationships, and their physical wellbeing. Findings support previous research illustrating that asthma restricts adolescent male’s lives, both at school and recreationally. This study established that these restrictions could unpredictably affect perceived masculine ideals and their ability to be independent, strong, muscular, and competitive. Consequently, the majority of participants felt their masculinity was challenged and described feeling different, isolated, and/or marginalised from their non-asthmatic peers. To counteract these feelings, maintain control, and seek normality in front of peers, most participants reported downplaying their asthma symptoms and/or did not adhere to their prescribed treatment regimes. In addition, hegemonic representations of males as ‘tough’ and ‘self-reliant’ influenced most participants to describe re-active, non-help seeking behaviours, and minimisation of their asthma symptoms. However not all participants described adverse outcomes, with a minority resisting hegemonic ideals by taking care of their health and asthma management. Findings illustrate how a variety of masculine ideals influenced young men’s health and asthma management. Further research is required to investigate the ways differential masculine ideals may be protective or detrimental to asthma medication.

Estudou-se a relação do exercício das masculinidades com o cuidado em saúde para homens na atenção primária, para se compreender como são percebidas e apreendidas as necessidades de saúde masculina nas interações entre homens e profissionais de saúde, tomando como referência a identidade masculina. Partiu-se do pressuposto de que o exercício das masculinidades pode representar riscos para a saúde dos homens e que os serviços de saúde não incorporam os referenciais culturais de masculinidades nas práticas de saúde efetivadas. Foram estudados dois serviços de atenção primária à saúde por meio de observação direta das atividades assistenciais e de entrevistas com homens usuários e profissionais de saúde. Encontrou-se uma diversidade de modelos de masculinidade que definem diferentes formas para pensar o cuidado de saúde dos homens. O estudo aponta que a identidade masculina de gênero passa por diferentes dimensões, as quais devem ser consideradas na apreensão das necessidades e nas práticas de cuidados de saúde empreendidas aos homens. No entanto, os padrões hegemônicos de gênero socialmente construídos criam dificuldades para o bom cuidado da saúde masculina. Não obstante, a população masculina procura as unidades básicas de saúde. Embora presente, percebe-se barreiras no uso desses serviços, relacionadas à invisibilidade e à não identificação dos homens com os serviços de atenção primária. Observou-se que as necessidades de saúde dos homens estão pautadas por questões como o trabalho, a sexualidade, a estrutura corporal, a vida comunitária e as relações familiares. Discute-se que as necessidades de saúde das masculinidades e suas particularidades não são acolhidas integralmente nas práticas de cuidado dos serviços de atenção primária à saúde.
This thesis studied the relation between the exercise of masculinities and health care of men in primary health care services. It consists of an attempt to comprehend how male health needs are apprehended in the interactions between men and health professionals. It was presumed that the exercise of masculinities could represent a risk to mens health and that health care services do not incorporate masculinities cultural references in effectuating health practices. Two primary health care services were studied by means of direct observation of assistential activities and interviews with men utilizing the services and health professionals. A diversity of models of masculinity that define different forms of reflecting upon mens health care was encountered. The interviews indicate that gendered masculine identity permeates different dimensions and that these dimensions should be taken into consideration when apprehending the needs and practices with respect to health care undertaken by men. The socially constructed hegemonic patterns of gender create difficulties for men to attain good health care. Nonetheless, the masculine population seeks the primary health care services for care. Although they are present, barriers related to invisibility or to the lack of identification of men with the primary health care services were perceived. This study indicates that mens health needs are regulated by issues such as work, sexuality, corporal structure, community life and family relationships. Masculinities and their needs are not entirely taken into account, as discussed, within the health care practices undertaken by the health services wherein this research was conducted.

This thesis presents men’s discussions and experiences of health and illness and its relation to, and implications for, the practices of masculinity amongst a diversity of men. Fifty five men participated in fourteen semi-structured focus group interviews. Diversity in men’s experiences of health and illness and in their constructions of masculinity was sought within the sample by age (range 15-72 years), occupational status, socio-economic background and current health status. Groups of men were recruited who had had ‘everyday’ or unremarkable experiences of masculinity and health and groups of men with health experiences that could have prompted reflection on masculinity and health. This included groups with men who had prostate cancer, coronary heart disease, mental health problems, and Myalgic Encephalomyelitis (ME). All of the men that participated in the study lived in central Scotland (Glasgow, Edinburgh, Dundee, Lanarkshire and Perthshire) and just one group was conducted with men of Asian origin, which reflects the limited ethnic diversity in this part of Britain. The first data chapter examines participants’ descriptions of their masculinity and their health-related beliefs and behaviours. The data capture both the experiences of men who felt pressured to engage in behaviours that may be harmful to their health in order to appear masculine and the accounts of those who regarded themselves as freer to embrace salutogenic health practices as they perceived there to be fewer consequences for their masculinities. These considerations are then followed by an examination of how participants re-negotiated male identity in the light of illness. The final data chapter presents participants’ discussions and experiences of help seeking and its relation to the practice of masculinity. The data suggests a widespread endorsement of a ‘hegemonic’ view that men ‘should’ be reluctant to seek help, particularly amongst younger men.

This study investigates the popular pleasures produced by readers of men's magazines, focusing primarily on the publication, Men's Health, which represents a new type of magazine catering for men. Using qualitative research methods such as textual analysis and reception analysis, the study explores the pleasures produced by both men and women from the consumption of such texts. The theoretical perspective of cultural studies informs this project, an approach that focuses on the generation and circulation of meanings in society. Focusing on the notion of the active audience and Hall's encoding/decoding model, this study examines readers' interpretations of the Men's Health text, focusing on the moment of consumption in the circuit of culture. Reception theory proposes the existence of "clustered readings" produced by interpretive communities that are socially rather than individually constructed. As a critical ethnography, the study interrogates these meanings with particular reference to questions of gender relations and power in society. Access to different discourses is structured by the social position of readers within relations of power and this study takes gender as a structuring principle. Therefore, this study also explores the particular discursive practices through which masculine and feminine imagery is produced by the Men's Health text and by its readers. The research findings support the more limited notion of the active audience espoused by theorists such as Hall (1980) offering further evidence to suggest that readers produce readings other than those preferred by the text and that therein lies the pleasure of the text for male and female readers. The research concludes that the popularity of Men's Health derives from the capacity of its readers to make multiple meanings of the text.

With a heritage rooted in the developing field of nineteenth century psychiatry, up until its classification within the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV-TR, American Psychiatric Association, 2000) today, disordered eating has been dominantly conceptualized as individual psy-medical pathology (Hepworth, 1999: 2). Drawing on a feminist post-structuralist theoretical framework which views dominant understandings of disordered eating as socially constructed through discourse, this research re-situates disordered eating within the multiple, complex, gendered, classed and racialised socio-cultures within which it is produced, indicating the wider discourses and social contexts which may be implicated in its development - particularly those across the formal and informal cultures of schools. Notwithstanding the volume of interest, literature and research on disordered eating, surprisingly little detailed attention has been given to the part that pedagogical processes of formal education may play in its development, either alone or in conjunction with social practices outside of schools. Yet, increasingly, research has pointed towards the intensification of the regulation, surveillance and monitoring of young people s healthy bodies, particularly in contexts such as physical education (Webb, Quennerstedt & Öhman, 2008). More specifically, this focus has been upon young people s weight in an effort to curtail the approach of an assumed obesity epidemic (Evans, Rich, Davies & Allwood, 2008b). This study examines how discourses of the body, weight and health are being translated into pedagogies enacted across a range of cultural sites (Wright, 2009) at school and outside of school, within the lives of a group of young women experiencing disordered eating. It explores the ways in which they are interpreting these health messages, illustrating how they co-mingle with performative neo-liberal discourses at school and at home, to propel these young women towards behaviours that are, in fact, damaging to their health. Although they are well-versed in dominant obesity/health discourses and the meanings they construct, the narratives of these young women illustrate the influence of these discourses, with their narrow focus on weight , upon problematic relationships with their bodies and food. The research concludes by pointing to the need for de-centring dominant obesity/health discourses in schools and for alternative ways of thinking about the body, weight and health to be filtered into education contexts.

Abstract:
The National Chlamydia screening programme in England screens only half the number of men compared with women, and the places men are being screened for chlamydia differ from that of women (NCSP, 2012). There is a wealth of data which shows that men are being screened in non-clinical settings, such as educational establishments, or that they are choosing to self-request screening via the internet as opposed to seeking alternative health service provision. However, we are unsure of the reasons for their choice. Furthermore, it is unclear what the impact of a positive or negative diagnosis for chlamydia has on subsequent sexual behaviour. By adopting a situational, qualitative methodology, this study aimed to understand the complex factors involved in men’s sexual health decision making following a request for a home testing kit for chlamydia. The focus for this study was young men aged 20 to 24 years who have a high rate of chlamydia infection, and who have been screened through the National Chlamydia screening programme in the North East of England. Data was collected through ten in-depth interviews, and seven follow up interviews after 12 months. Follow up interviews were primarily used to gauge any long term behaviour change. Patients’ sexual health records provided additional data which was used for triangulation. Data was analysed with the use of framework analysis. Findings from the research were presented to a focus group of professionals and the outcomes from that discussion have been implemented in sexual health provision locally. This research has also fed into a national working group which reviewed chlamydia testing guidelines for positive patients. Findings show that the decisions the young men made about sexual partners and sexual practice are based on a number of factors: pre-influencing factors, which were based on the men’s perceptions and beliefs about women, categorising them as “risky” with a sexually transmitted infection or “clean” with no infections, alcohol use and contraception vs STI prevention . Situational factors including sexual gratification and sexual arousal and post rationalisation factors such as peer pressure and masculinity. Factors influencing decisions to seek testing were triggered by unprotected sex with casual partners, strengthened by catalytic influences including media campaigns. The findings suggest a negative chlamydia test result gives respondents a clean bill of health allowing them to engage in further unprotected sex. A positive diagnosis resulted in the intention to change behaviour and modify sexual practice. After follow up interviews, intention did not lead to actual behaviour change and many became re-infected within 6 months. A conceptual model based on the study findings has been developed for use in professional practice. This model identifies the variables which influence the men’s decision making at different stages in the decision making process. This study has shown that the factors that influence young men’s sexual decision making and the impact of diagnosis on subsequent sexual behaviour have major implications for public health in terms of reinfection and further transmission.

Abstract:
This thesis draws on theoretical and empirical research on men and masculinities, and on social capital to inform understanding of the experiences of chronically ill men. Levels of chronic illness are increasing in the Western Global North, and this phenomenon has been linked with health-related behaviour (Crossley, 2000a). A growing literature has explored the ways that people experience chronic illness and the implications of that for their health and wellbeing (e.g. Williams, 2000; Bray et al, 2014). There is a need however for further research that seeks a gendered understanding of men’s experiences of chronic illness within a context of social relatedness. Connell’s (e.g. 1995) theoretical description of hegemonic masculinities set within the ‘gender order’ has stimulated much research that moves beyond a simplistic understanding of gender that often characterises work in Health Psychology. Similarly social capital has become widely used to conceptualise aspects of social connectedness and it’s links with health and illness (e.g. Hu et al, 2014). A qualitative, narrative approach has been employed here to explore the experiences of chronically ill men. Semi-structured interviews were conducted with twenty five men to investigate the processes linking social capital, masculinities and experiences of chronic illness. A multi-level narrative analysis (e.g. Murray, 2000) was conducted on the resulting interview transcripts, and both case study and cross-case studies are presented in the thesis. The thesis demonstrates the value of a multi-level analysis (Murray, 2000) that incorporates the examination of personal, interpersonal, positional and ideological facets of men’s illness narratives. Key findings concerned the contestation of meanings of illness in men’s social networks, the gendered context to family conflicts related to illness, and the complexities of reciprocal care and support amongst families over time. Issues around employment and the workplace such as stress, and the negotiation of accommodations in masculinist workplace contexts are also explored. These findings point to the complexity and importance of interdependence and masculine identities, and their links with chronic illness experience. The association of social capital with entirely positive or negative outcomes or experiences amongst chronically ill men oversimplifies the diverse and context specific processes involved. A gender-sensitive approach to family systems and broader social connectedness with community and work contexts however can aid in understanding how the experiences of chronically ill men are shaped. The importance of theories and interventions that do not rest on models of unconstrained individual choice is particularly highlighted. Such work may be useful to health practitioners and in contributing to public discourse around chronically ill men.

Abstract:
Obesity is an increasing problem in the United States with 17% of youth currently classified as obese and an even higher prevalence of obesity among disadvantaged populations. The food environment may be contributing to these high rates as there has been a well documented association among increased away from home food consumption and excess adiposity, as well as evidence to support that children\'s diets are composed of a large portion of restaurant foods. The main purpose of this study is to describe the quality of restaurant food offered to children in a rural health disparate region. Two trained research assistants conducted systematic audits of all food outlets offering a children\'s menu in the Dan River region using the Children\'s Menu Assessment (CMA) tool. A composite score for each outlet for was calculated from the 29 scored items on the CMA. The total sample consisted of 137 outlets with CMA scores ranging from -4 to 9 with a mean score of 1.6+2.7. Scores were lowest in the predominantly Black block groups (0.2+0.4) when compared to the predominately White block groups (1.4+1.6) and Mixed block groups (2.6+2.4) with significantly lower scores in the predominantly Black block group than the Mixed block groups (F=4.3; p<0.05). The results of this study reveal a lack of few healthy food options available for children in this region. These findings have the potential to contribute to public health efforts in developing public policy changes or environmental interventions for the children\'s food environment in the Dan River Region.

The aim of this study was to investigate human-animal interactions in the triad comprising the veterinarian, the animal and the preschool and in so doing to elucidate the role of the veterinarian in the health and education of preschool children. The area of focus was a comparison of preschools in high and low income areas in and around Pretoria by means of qualitative and quantitative observational methods. It was found that all preschools investigated, included animals in the curriculum and a majority visited the zoo. Interactions included direct contact between children and animals kept permanently or temporarily at school, animal themes and topics, animal protagonists in books, videos, toys and games and excursions to the zoo, urban farms, agricultural museums and animal welfare societies. Teachers felt that child-animal interactions contributed positively to the holistic development of the preschool child, but were worried about management of animals at school and the possibility of zoonotic diseases. As protein of animal origin, such as milk, eggs, meat and fish, formed part of the diet of preschool children, this could also result in a risk of zoonotic disease, particularly in developing areas, where meat and milk hygiene were found to be inadequate. In order to address these fears, the literature was reviewed with regard to zoonotic diseases which could affect preschool children in South Africa. Incidence and prevalence were not well documented and the comparative significance could not be assessed. Therefore the comparative morbidity of diseases diagnosed by the paediatric department of a hospital serving the low income areas studied and a private practice serving the high income areas, were investigated. It was discovered that zoonotic disease formed a very minor proportion of diseases diagnosed. The diagnoses were, however, based on symptoms rather than aetiology and zoonotic causes for, in particular, respiratory and gastro-intestinal disease, could not be excluded. In the light of this, criteria were proposed for the prevention of zoonotic disease outbreaks at preschools. An outbreak of zoonotic disease at a preschool was documented and the application of primary health care principles suggested. Constraints were found to be mainly administrative. Co-operation between the departments of Health and Agriculture was complicated by financial implications which had not been budgeted. Despite this, the intervention was successful in controlling the disease and preventing further outbreaks. The presence of a veterinarian as part of the primary health care team was advocated. Within the preschool it was found that teachers lacked knowledge in the fields of animal ethology and management. Cost, ease of management and appeal to children were taken into account in the choice of animals by preschools. Rodents and birds were considered preferable to carnivores. Housing was evaluated and criteria suggested for management systems which would benefit both children and animals. Death and euthanasia of animals kept permanently at preschool was investigated and suggestions made for a teaching strategy to facilitate understanding by children of the abstract concepts of life, death and grief. A method was developed for the analysis of animal content in literature, games and toys at preschools. It was found that animals were central to the theme of a majority of the books and toys. Realistic fiction where anthropomorphic animals were the central protagonists, had most appeal for children and it was suggested that these could be used in veterinary extension materials for prevention of zoonotic diseases or promotion of animal welfare. An evaluation system was proposed which included the input of veterinary ethologists. Animal facilities visited by preschools during excursions were evaluated. It was found that they were not sufficiently child-centred and environmen¬tally safe for young children. It was suggested that veterinary public health officials should become involved in order to improve animal well-being, particularly with regard to handling facilities and hygiene. In conclusion, a schematic representation of the multitude of roles for veterinarians in the holistic development of the preschool child was drawn up. The roles for different veterinary specialities were also tabulated in order to illustrate the important part played by this profession in the health and education of preschool children.
Die doel van die studie was om mens-dier-interaksies in die driehoek tussen veearts, dier en die kleuterskool te ondersoek om sodoende die rol van die veearts in die gesondheid en opvoeding van die voorskoolse kind aan te dui. Kwalitatiewe en kwantitatiewe navorsingsmetodes is gebruik om kleuterskole in die lae en hoë inkomste-gebiede in en om Pretoria te vergelyk. Alle kleuterskole wat ondersoek is, het diere in die kurrikulum ingesluit en die meerderheid skole het uitstappies na die dieretuin onderneem. Kind-dierinteraksies het die volgende ingesluit: direkte kontak tussen kind en dier op skool; diere in temas, stories, boeke, speletjies en speelgoed; asook uitstappies na die dieretuin, landelike museums, stedelike plase en dierwelsynsorganisasies. Onderwyseresse het gemeen dat die interaksies positief bygedra het tot die holistiese ontwikkeling van die voorskoolse kind, maar het kommer uitgespreek oor bestuur en siektes van diere. Omdat proteïene van dierlike oorsprong deel uitmaak van die dieet van voorskoolse kinders, was soönose ook 'n moontlikheid, veral in kinders van ontwikkelende areas waar dit uitgewys was dat higiëne met betrekking tot vleis en melk, nie na wense was nie. 'n Literatuurstudie het aangetoon dat die algemeenheid en omvang van soönotiese siektes, wat moontlik gevaarlik kon wees vir voorskoolse kinders, nie volledig genoeg omskryf was nie. Die vergelykende morbiditeit van siektes by Ga-Rankuwa Hospitaal se kinderafdeling (pasiënte van lae-inkomste gebiede) en 'n private praktyk in die oostelike voorstede van Pretoria (hoë inkomste gebied), is dus ondersoek. Soönotiese siektes het 'n klein deel uitgemaak van die siektes wat gediagnoseer was, maar die diagnose was op simptome, eerder as die etiologie van die siektes, gebaseer. Die vermoede bestaan dat veral siektes wat gediagnoseer is as respiratories en gastro-enteries, 'n soönotiese oorsprong kon gehad het. 'n Uitbreek van 'n soönotiese siekte by 'n kleuterskool is ondersoek en daar is voorgestel dat primêre gesondheidsorgmetodes gebruik word om dit te bekamp. Teenkanting op administratiewe vlak was die belangrikste probleem wat ondervind was, maar ten spyte daarvan kon die ondersoek suksesvol verloop. Die rol van die veearts as deel van die primêre gesondheidspan is ook hierdeur beklemtoon. Binne die kleuterskool is gevind dat daar 'n gebrek aan kennis is oor diere-etologie en dierebestuur. Kostes, praktiese versorgingsmetodes en die stimuluswaarde van diersoorte, was belangrike aspekte in die keuse van diere wat by skole aangehou is. Daarom was knaagdiere, voëls en vissies meer gewild as honde en katte. Die behuising van die diere is geëvalueer volgens voorgestelde kriteria wat tot voordeel van beide diere en kinders kan strek. Die dood en genadedood van diere by twee kleuterskole is ondersoek en voorstelle is gemaak uit 'n opvoedingkundige oogpunt, om die kinders konsepte betreffende lewe, dood en rou te laat begryp en ook om dit te kan verwerk. 'n Metode is ook ontwerp vir die analise van boeke en speelgoed en daar is bewys dat diere 'n hoofrol gespeel het in die meerderheid boeke en speelgoed by voorskole. Kinders het boeke verkies waar die hoofkarakters antropomorfiese diere was en die omgewing vergelykbaar was met die lewenswêreld van kinders. 'n Voorstel is dus gemaak dat sulke karakters gebruik word om veeartsenykunde voorligtingsboodskappe oor soönotiese siektes of dierwelsyn oor te dra aan jong kinders.Stedelike plasies, die dieretuin, landboumuseums en die Dierebeskermingsvereniging in Pretoria is besoek en daar is bevind dat dit nie veilig genoeg vir jong kinders is nie. Daar word dus aanbeveei dat veeartse in diens van plaaslike owerhede se gesondheidsdienste sulke plekke besoek, om voorstelle oor verbeterde veiligheid en higiëne te maak.Ten slote is 'n skematiese voorstelling van die veelsydige rol van veeartsenykundiges in die holistiese ontwikkeling van die voorskoolse kind saamgestel. Die verskillende rolle vir verskillende spesialiteite is ook voorgelê om die belangrike bydrae van veeartsenykunde tot die gesondheid en opvoeding van voorskoolse kinders aan te dui.

Background: Young men account for a substantial proportion of reported sexually transmitted infection (STI) and HIV cases in Canada. However, STI/HIV testing rates remain low among young men. While men’s health-related behaviour have been linked to masculine expectations that demand stoicism, independence and denial of illness, little is known about how dominant masculine and/or heteronormative expectations may affect men’s sexual health-related practices that can put them at both an elevated risk of acquiring an STI(s)/HIV and/or affect their ability to access STI/HIV testing services. The objectives of this thesis are to: (1) explore how heteronormative and heterosexist discourses function within clinical settings where young men access STI testing services to better understand the extent to which dominant masculine ideals are (re)produced or resisted in these clinical contexts; (2) identify the social and contextual conditions which facilitate or create barriers to effective sexual health communication amongst men, paying special attention to how idealised masculinities influence these interactions; (3) develop recommendations for sexual health services and future research to improve the sexual health of young men in BC. Results: The findings drawn from this research highlight how idealized masculinity influences young men’s sexual health, including their sexual health-seeking behaviour, sexual practices and the ways in which they talk about sexual health. Specifically revealed are instances in which dominant heternormative expectations ‘hurt’ all men in clinical encounters (e.g., by stereotyping gay men as ‘risky’, thereby alleviating STI/HIV concern for straight men by virtue of their sexual identity). Men’s conversations about sexual health focused primarily around their sexual encounters (e.g., using ironic/teasing humour to embody masculine identities that neither dismiss nor actively express concerns about sexual health), amid processes of ‘manning up’ to break with dominant masculinity (e.g., stoicism) to permit talk about sexual health with peers or sex partners. Discussion: By examining situations in which men (and clinicians) align with or socially reconfigure idealized notions of masculinity related to sexual health, theorists and interventions will better understand how more equitable gender relations can be produced, thereby improving the sexual health of men (and women).

Abstract:

The purpose of this study was to investigate factors that influence consumer choice in menu items at fast food chain restaurants. The study utilized social media, specifically Facebook, to collect data from various areas of the United States.

Previous literature on the effectiveness of nutrition menu labeling on decreasing calories ordered reported mixed results. Some studies point to groups that may be least affected by menu labeling: men, individuals of certain races, obese individuals, individuals of lower socioeconomic status, unemployed individuals, and individuals who dine outside of the home frequently.

The results of this study indicated that men and individuals who dine outside of the home more than three times per week are less influenced by nutrition information than their counterparts. Additionally, about half the participants in this study reported that they do not notice or look for nutrition information. Further research and outreach programs are needed in this area.

Abstract:
There is a prominent discourse in academic literature, and society at large, that presents men as ‘damaged and damage doing’ (Mac an Ghaill and Haywood, 2012: 483). Incorporated within this idea is the notion that ‘masculinity’ itself is problematic and represents a ‘risk factor’ for health (Gough, 2006). For example, traditional masculine norms, like ‘toughness,’ have been linked to poor emotional management skills in men, which in turn are implicated in mental health problems (Aldao et al., 2010). However, it is increasingly acknowledged that there is diversity within and across men and masculinities, and that men are capable of positively managing their well-being, although little research exists exploring how they do so. To address this deficit, this study sought to find men – meditators – who were likely to have found ways to positively manage well-being to examine factors relating to this engagement. Meditation was selected as it is associated with positive outcomes on a range of mental health indicators (Mars and Abbey, 2010). Thirty male meditators, mainly from one organisation in London, were selected using principles of maximum variation sampling. The study employed a longitudinal mixed methods design, including in-depth narrative interviews analysed using a modified constant comparison approach (Strauss and Corbin, 1998), and also a cognitiveneuroscience component, involving EEG measurement across a battery of cognitive tasks and a meditation sitting. All participants were interviewed and tested twice,1 a year apart, between 2009 and 2010. Drawing on various theories, including Connell's (1995) notion of hegemonic (i.e. dominant) masculinity, and Mayer and Salovey's (1997) model of emotional intelligence, the analysis explored themes relating to men’s involvement with meditation, including how engagement came about, and its impact upon well-being.2 The findings suggested that men negotiated difficult journeys towards meditation: for example, they came up against traditional and other hegemonic forms of masculinity, and most described subsequent strategies to be emotionally 1 One participant did not complete the cognitive neuroscience component. 2 Pollard and Davidson (2001: 10) define well-being as ‘a state of successful performance across the life course integrating physical, cognitive and social-emotional function.’ However, well-being is a contested concept, used in diverse ways according to different theoretical frameworks (De Chavez et al., 2005). The range of meanings attached to the concept is discussed in the theoretical review. tough and/or disconnect from difficult emotions. Meditation itself was linked to well-being in various ways, notably through the cultivation of emotional intelligence via the development of attention – this was indicated by emergent themes in the qualitative analysis, and results from the cognitive neuroscience component. Overall, the analysis was unusual in exploring masculinities and meditation, as well as the wider social context of practice, and how the social dimensions of meditation also impacted upon well-being. For example, many men meditated within a ‘community of practice’ (Lave and Wenger, 1991), which influenced their behaviour, e.g. reducing alcohol use. The findings also highlighted various problems linked to meditation that have received less attention in the literature, including mental health disorders, and ostracism from peers. In summary, the study discusses implications for helping men to better manage their well-being.

The purposes of the study were to investigate homeless men's experiences of taking medications while living in a homeless shelter and the factors that affect medication adherence; and to describe medication adherence, as it pertains to homeless men residing in a shelter, from the perspective of the shelter staff. This is a manuscript-based thesis. The first manuscript provides a description of a proposed ecological model, based on clinical experience of the author and from the literature. The second paper is a summary of the findings of the qualitative research on medication adherence and homeless men's shelters. The third manuscript, directed at pharmacists, offers recommendations and strategies based on results of the qualitative study, to improve medication adherence. This thesis suggests that when health professionals view adherence to medications as an individual, cognitive choice, it does not take into account the challenges imposed by a homeless individual's environmental context.

This study investigated the viability of community health insurance (CHI) as a means of increasing access to health care for rural households in Uganda. This was against the background that health care is a basic need and right and that, despite this, households especially in the rural parts of Uganda are still lacking effective access to health care. The study is informed by different theories of justice in health care delivery, namely, the libertarian, egalitarian and utilitarian theories. It also borrows concepts from Andersen's (1968) behavioural model of health services access and utilization as well as Kutzin's (2001) framework for analysis of health financing arrangements to assess the viability of CHI as a strategy to increase access to health care.

Esta pesquisa teve como proposta examinar e descrever os estilos de vida do homem contemporâneo depreendidos dos periódicos Gentlemen’s Quarterly e Men’s Health, bem como investigar os modos pelos quais os programas narrativos, nelas desenvolvidos, demonstram os diferentes estilos de vida desse novo homem. Para isso, o trabalho se alicerça na Sociossemiótica, principalmente nas postulações de Eric Landowski. Além do mais, foi também possível identificar como se deu a interação entre enunciador, por meio de sua projeção no texto, e enunciatário, que se fez presente pelas marcas deixadas no enunciado. O trabalho também se fundamenta na teoria semiótica de origem francesa, desenvolvida por Algirdas Julien Greimas, e tem como núcleo de suas preocupações o modo de significar, mais precisamente nos encontros entre sujeitos e objetos de valor, de onde a comunicação faz suscitar inúmeras significações. This research aimed at examining and describing the lifestyles of the contemporary men gathered from the magazines Gentlemen’s Quarterly and Men’s Health as well as investigating the way in which the narrative programmes developed demonstrate the different lifestyles of the new men. For this, the ivestigation places its foundations on the sociossemiotics, mainly on the assumptions made by Eric Landowski. Furthermore, it was also possible to identify how the interaction between the enunciator, through its text projection, and the enunciatee, present by the signs left in the text, occurred. The research is also based on the French semiotics theory, developed by Algirdas Julien Greimas and one of its main worries is the way to signify, especially when the subjects and the value objects meet and where the communication raises significance. These encounters between the subject and the object which comprise its conjunctions and disjunctions, fusions and separations produce distinct and varied sense effects that pass by the analysis in which the sense is being built and displayed. Thus, the sense comes up oriented by the immanence of the texts which show it up. The corpus was selected based on the extensive scenario offered to deal with the universe of the contemporary men on the media. On account of the diversity of the mediatic products offered to the men, the two mentioned magazines were selected, from April 2011 to April 2012. By establishing the differences between the magazines to represent the masculine, it was possible to orientate the following hypothesis: was it possible to recognize the configuration of the same lifestyles in both magazines? Based on the analysis, it possible to notice the differences which distantiate the two magazines, taking into account that in GQ, it was possible to identify that the lifestyles of the men represent... Resumo: Esta pesquisa teve como proposta examinar e descrever os estilos de vida do homem contemporâneo depreendidos dos periódicos Gentlemen's Quarterly e Men's Health, bem como investigar os modos pelos quais os programas narrativos, nelas desenvolvidos, demonstram os diferentes estilos de vida desse novo homem. Para isso, o trabalho se alicerça na Sociossemiótica, principalmente nas postulações de Eric Landowski. Além do mais, foi também possível identificar como se deu a interação entre enunciador, por meio de sua projeção no texto, e enunciatário, que se fez presente pelas marcas deixadas no enunciado. O trabalho também se fundamenta na teoria semiótica de origem francesa, desenvolvida por Algirdas Julien Greimas, e tem como núcleo de suas preocupações o modo de significar, mais precisamente nos encontros entre sujeitos e objetos de valor, de onde a comunicação faz suscitar inúmeras significações. Abstract:
Medical errors occur despite precautionary measures. Limited research has focused on intuition in preventing medical errors. The problem addressed in this study explored the role of intuition by health care team members in preventing medical errors from reaching hospitalized patients. The research questions focused on the differences in response to medical errors by health care team members from 3 West Coast hospitals. The theoretical framework included human error, personality typing, skill acquisition, and a model of intuition. In this exploratory mixed method study 1,836 unusual occurrence reports submitted over 6 months were analyzed. Of the 710 health care team members surveyed, 201 (28%) completed an intuitive score instrument. Eight health care team members were interviewed, with responses analyzed for themes of knowledge management implicit to intuition. The unusual occurrences results were categorized as near miss or adverse event and analyzed using t tests. There were no differences in a comparison of mean intuition scores for type of error and levels of intuition by participant age or gender. Differences were found in the number of documented constructs of intuition by type of error and discipline in the comparison of pharmacy to nursing and diagnostic imaging. Interview excerpts were compiled for use by managers to role model through storytelling how intuition can prevent medical errors. Research is needed to understand how to incorporate skills of tacit recognition and intuition. Preventing costly and potentially life-threatening medical errors is fundamental to addressing the societal need to lower costs and provide safer patient care.

Despite the prediction that religion would wane in the 21st century, evidence suggests otherwise. It is important therefore for mental health professionals to understand the role of religion in the lives of their clients and to appreciate its impact on mental health. Research suggests a moderate inverse relationship between religion and depression and anxiety with some cross-cultural support. The relationship is however complex and context needs to be taken into account. The effect of culture and the importance of intrinsic religiosity are discussed. Much research is conducted in the US in the field of psychiatry, suggesting a gap in the British psychological literature. Some implications for clinical practice are discussed. The interplay between religion, culture and wellbeing is discussed

This thesis outlines research undertaken by formal collaboration between Everton Football Club's Football in the Community (FitC) scheme; Everton in the Community (FitC) and Liverpool John Moores University, School of Sport and Exercise Sciences. In recent years, there has been recognition of the influence that English Premier league (EPL) football clubs can have in attracting men to physical activity and health engagement programmes. Despite attempts to align FitC programmes with suitable evaluation procedures, there still remains limited evaluative empirical evidence. '. Study 1 adopted ethnographic principles to explore the effectiveness of, and identify the barriers to, promoting positive health behaviours and messages to male football fans at an EPL football stadium on match days. Results showed that in general, men did not wish to engage in health related behaviours on match days however approaches that did not impose on, nor contaminate, the men's match day experience were more successful. Study 2 adopted a multi-method approach to explore the distinct barriers that hard-to-reach (HTR) male populations encounter when attempting to commit to regular participation in physical activity and health behaviours and to examine the impact of engaging in a 12 week FitC intervention. Economic, environmental and social barriers to engagement in regular physical activity and positive health behaviours are highlighted and specific biopsychosocial effects of engaging in the FitC programme are identified. Study 3 utilised informal semi structured interviews with programme participants to explore the contextual, environmental and psychosocial barriers experienced by men from HTR populations. Psychosocial motivations for programme uptake and the impact of regular engagement in the FitC men's health programme are discussed. It is recommended that commissioning agencies should endorse and fund men's health initiatives delivered in and by professional sports clubs. To maintain participant engagement and maximise improvements to men's health and wellbeing, alterations to current practice and research are discussed.

Background: The Multifactor Offender Readiness Model (MORM) acknowledges the importance of gender-sensitive interventions in enhancing engagement in the rehabilitation of offenders. However, to date, little attention has been paid to the role of gender identity in psychological therapy. Even though men in forensic settings are likely to endorse hypermasculine attitudes, such as promoting aggression and restricting emotional expression, which run counter to therapeutic ideals. Methodology: Nine men residing in medium-secure forensic mental health hospitals were interviewed. Interpretative Phenomenological Analysis (IPA) was used to obtain first-person accounts of the men’s lived experiences to gain an insight into the subjective meanings they attached to their experiences of psychological therapy. Results: An analysis of the individual transcripts highlighted the men’s journey over the course of therapy, marked by an internal struggle against the external pressures. Three superordinate themes were identified: ‘shifting self’, ‘relationship with other’ and ‘therapeutic journey’, alongside their subsequent sub-themes. Discussion: A gradual, non-linear process of change was evident in the men’s narratives, who at the various phases of psychological therapy were faced with the challenge of questioning and redefining their identity. This involved lowering their guard, learning to become comfortable with vulnerability and face their past in the presence of a supportive ‘other’, in order to move towards building a new or better future for themselves.

Recent research has indicated that men do not tend to seek help from mental health services when experiencing emotional distress. In the Introduction, I outline the men's help-seeking literature, before describing a social constructionist approach to masculinity and the possibilities of this framework for investigating the problem of men's help-seeking. I then examine literature to argue that Western conceptions of mental health and masculinity draw upon similar discourses and I suggest that men's help-seeking for emotional distress involves behaving in a way contrary to the construction of Western masculinity. This discussion leads to the present research: a discourse analysis of men's perspectives on seeking help from mental health services. The research aim was to identify which discourses and identities were available to men when they talked about help-seeking from services, and therefore to identify which individual actions and social practices seemed reasonable. I interviewed eight men, four of whom had sought help, and four who had not. In the Analysis, I examine the men's use of language and the effects that this had when accomplishing them as masculine. Similar discourses were drawn upon by both groups however these were deployed in different ways, constructing male help-seekers as masculine or not so. The analysis also demonstrates the extent of the rhetorical work that help-seekers had to employ to construct help-seeking as masculine. In the Discussion, I summarise the analysis and examine the implications for individual action and broader social practice and policy. I outline the implications for the help-seeking literature and for social constructionist understandings of masculinity. I then examine the application of my findings to individual psychological therapy and broader mental health promotion for men. I reflexively examine the research with regards to my own identity and positioning, and the validity of my reading of the transcripts.

The study findings were thematically grouped into three themes including the coordination and partnerships, culture and implementation challenges. Poor coordination and failure of systems in place appeared to characterise the many challenges. Gender inequalities and masculine dominated cultural practices like polygamy and widow inheritance are associated with consequences of ill health among women; including high HIV/AIDS prevalence, early marriage, high teenage pregnancies and high maternal mortality. Although these practices are in favor of men, they hardly protect them from the wrath of poor RH like STDS, HIV/AIDS, stressful big families and vast poverty. Hence, Tanzanian men are also victims of their own behavior.

Resumo: Este estudo nasceu a partir da percepção do grande espaço que, atualmente, as mídias dedicam ao tema da saúde e de como, muitas vezes, a beleza parece ser um pré-requisito para que qualquer indivíduo seja reconhecido como saudável. Visto que, em grande parte, a imagem de um corpo saudável disseminada pelos meios de comunicação é constituída por corpos belos, objetiva-se, a partir desse quadro, investigar como o discurso da saúde utilizado pela edição brasileira da revista Men's Health é construído, pois, em um primeiro momento, parece-nos que seu objetivo é o de retratar temas relacionados e pertencentes ao gênero do discurso da beleza masculina, como é possível verificar pelo conteúdo anunciado em suas capas. Para isto, primeiramente, estudamos o conceito de saúde, tomando por base a ideia de que um corpo saudável é um corpo sem doenças e, em seguida, a partir de Charaudeau, buscamos compreender como a mídia constrói seu discurso. Só então, utilizando o conceito de gênero do discurso de Bakhtin, foram analisados os discursos com os quais a revista trabalha, dando ênfase aos que estão presentes em suas capas. Concluímos que os discursos da Men's Health fazem parte do gênero do discurso da saúde. Entretanto, a revista faz uso de um imaginário de saúde no qual a beleza é um elemento indispensável deste discurso. Isto é, há uma simbiose entre os discursos da saúde e da beleza. Abstract: This study was born from the perception of a large space that currently the media dedicate to the topic of health and how often beauty seems to be a prerequisite for any individual to be recognized as healthy. This is due to, in large part, the image of a healthy body disseminated by the media consisting of beautiful bodies. Within this framework, the aim of this study was to investigate whether, as indicated by its name, the Brazilian edition of Men's Health magazine belongs to the branch of publications pertaining to the health discourse genre. Conversely, at first, it might seem its goal was to portray issues belonging to the male beauty discourse genre, as can be verified by the content on their covers. For this analysis, we first studied the concept of health, based on the idea that a healthy body is a body without disease. Secondly, from Charaudeau's framework, we seek to understand how the media construct its discourse. Only then, using the concept of Bakhtin's discourse genre, the discourses of the magazine were analyzed, with an emphasis on those present in its covers. We conclude that Men's Health's discourses are part of the health genre. Nevertheless, the magazine makes use of a health imaginary in which beauty is an essential element of this discourse. That is, there is a symbiosis between the discourses of health and beauty. However, in this case, beauty is a symbolic value related to the seduction and erotization of the healthy body. Therefore, the image has a great importance to this imaginary and a relevant participation in the discourse conveyed by this publication. The analysis also showed that this imaginary is not accessible to everyone. Consequently, it meets a market logic in which health is the result of a lifestyle permeated by product and behavior consumption which give the magazine's target audience not only the image of healthy people ... Abstract:
This study explores the defining moments in six men’s lives. The empirical dimension of the research is built around the personal narratives these men tell of their lives across a series of four interviews. The central research theme is the notion of the defining moment as a key element in the processes of establishing how men understand and interpret the events and incidents that have shaped their lives. In the context of this study, the defining moment is seen as the moment or period in time when an individual gives definition to a specific event or experience, as a transition point with (potentially) life-altering consequences. Some of the thematic structures presented include relationships with significant adults (parents, teachers), masculinity, self-harm, schooling, mental illness, isolation, loneliness, stress and relationships with peers. In my pursuit of a methodology that could accommodate the aims of this study, I explored the process of meaning through the qualitative paradigm. Drawing on the principles of qualitative research, as applied through narrative inquiry, I deployed a semi-structured interview format to collect the lived experiences of participants. By privileging the stories that individuals tell of their experiences, the narrative method recognises that data are inexorably located in the contextual and contingent. The experiences and narratives that are presented in this thesis are built around the authentic voices of participants. The study presents a warrant for working with men’s defining moments to disrupt, alter and redefine their attitudes and behaviours in order to improve their lives. Based on the insights gleaned through this study, I argue that there are defining times/points in people’s lives where their experiences can be life altering. When these experiences involve uncertainty, anxiety, stress and other pernicious effects, their longer-term consequences can be devastating. The study confirms existing research, that men are reluctant to seek help or reveal their insecurities during such times, therefore making them particularly vulnerable to defining moments. The conclusion of this thesis establishes some broad recommendations pertaining to working effectively with men and their defining moments. I focus particular attention on the place of schooling and education in helping individuals recognise and respond to the early symptoms of what is potentially a life-altering experience. Schools and, by association, teachers need to be actively and strategically involved in this process. To this end, I argue the need for targeted interventions that are both sensitive and timely. In their engagements with young males, parents, teachers, coaches and mentors need to be particularly attuned to their silent screams for help.
