Full textIn this thesis, we model the life-cycle evolution of individual healthcare expenditures, expressed as a function of the aging process, health shocks and conditions, and distance to death. All the analyses are carried out by using a unique dataset, which allows us to focus on different types of healthcare services and different subsamples of individuals. The population of interest consists of individuals aged 50-70, the age window where the first adverse health events are expected to arise. In the first chapter, we use a two-way fixed effects model to examine the effect of age, morbidity, and time to death (TTD) on individual healthcare expenditures (HCE). The estimation is carried out by controlling for several confounding factors, including individual and General Practitioner (GP) fixed effects. We also investigate to what extent patients’ and GP’ characteristics contribute to the overall variability in expenditures among individuals. Our main results show that age, morbidity, and TTD are all important determinants of HCE and are among the elements that contribute most to the variability in HCE among individuals. Total HCE is increasing in age, with the latter found to be negatively correlated with the time to death, a result in contrast with the ‘red herring’ hypothesis. Such an increase with age of overall expenditures is mainly driven by expenses for out-of-hospital services; in contrast, no difference in hospital costs is observed over the considered lifespan once the other factors are taken into account. On the other hand, inpatient expenditures mainly drive the morbidity and end-of-life profiles of total HCE. Concerning heterogeneous analysis, we find that chronic and disabled individuals with health shocks requiring hospitalization are those who place the greatest burden on the costs borne by the Italian healthcare system. It suggests that the enhancement of preventive approaches before the onset of such shocks is a priority goal to reduce the incidence of long-lasting diseases and prevent them from deteriorating to the point of exacerbation in acute cases requiring hospital admissions. Given the results obtained in the first chapter, in the second one, we use a difference-in-difference event study approach to estimate the short- and long-run impact of the hospitalization on HCE, with hospital admissions analyzed here as a measurable subset of those first adverse health events individuals aged 50-70 experience in their life. Our main findings confirm the existence of a large effect of the first hospitalization on HCE and show that the first access is associated with substantial future medical expenses in all healthcare settings, accounted for the largest part by acute inpatient care. Indeed, the analysis of hospital expenditures indicates the occurrence of subsequent hospitalizations, mainly required for complications of cardiovascular diseases and cancer. The latter are responsible for the highest increase in inpatient expenditures and present a persistent post-admission increase also in outpatient and pharmaceutical expenses, a result driven by the high incidence of chronic and disabled individuals within the group of those affected by these two conditions. From a policy perspective, it indicates need for a strengthening of territorial care and tertiary prevention improvements, necessary to soften the impact of ongoing illnesses with lasting effects. On the one hand, it would improve patients’ health by preventing complications and acute cases; on the other hand, it would also generate significant savings through reduced avoidable additional hospitalizations.

Full textNeurodevelopmental disorders (NDDs) are a group of heterogeneous disease affecting the nervous system, including autism spectrum disorders (ASD), and present in about 1-3% of children. ASD is characterized by communicative impairments, difficulties in social interaction, behavior abnormalities and complex pathogenetic mechanisms. Increasing findings support an oligogenic model of inheritance with a combination of inherited and/or de novo variants, involved in different pathways and biological processes. The presence in the same individual of multiple deleterious variants in different genes could contribute, according to an additive model, to heterogeneity of clinical features observed in NDD patients. The genetic etiology of NDDs has not yet been fully clarified. Poland Syndrome (PS, OMIM 173800) is a rare congenital condition (incidence of 1-9/100.000) with variable phenotype characterized by pectoral muscle agenesis/hypoplasia that may include ipsilateral thoracic and upper limb anomalies. Most cases of PS are sporadic, familial recurrence has been observed (4%) with phenotypic heterogeneity. Different inheritance patterns have been reported and polygenic/multifactorial mechanisms have been hypothesized in some cases. The genetic etiology of PS remains unknown. Recent studies proved that the use of Whole Exome Sequencing (WES) together with the copy number variations (CNVs) analysis can help to the identification of the molecular bases of heterogeneous genetic disorders. In this study we aimed at investigating the genetic mechanisms underlying NDDs and PS. i) A retrospective Array-CGH analysis was performed on 526 NDD cases. The pathogenicity of CNVs was investigated through mining of literature and searching through databases reporting genes associated with NDD, particularly intellectual disability (ID) and ASD. (ii) Fifty-three ASD families were investigated by Whole exome sequencing, and potentially deleterious variants prioritized by custom filtering strategies including the use of Oligogenic Resource for Variant Analysis Platform (ORVAL) and enrichment analysis of candidate genes with GeneCodis4. (iii) A cohort of 30 PS patients were analysed by WES, and potentially deleterious variants prioritized by custom filtering strategies including oligogenic variant analysis. Functional analyses of identified variants included in vitro mutagenesis followed by cell imaging and gene reporter assays. Pathogenetic CNVs have been found in 42% of NDD cases, while 58% of the identified CNVs remain of uncertain significance. The study of arrays data led to the identification of new genes and "double-hit" mechanisms that could account for the NDDs. By using an oligogenic approach to the analysis of WES data, we identified inherited variants in different genes that globally could explain the complex phenotype in some cases. The study also revealed the involvement of different genes in pathways and biological processes common to several patients with NDD. In patients with PS, deleterious variants were found in two different genes, one involved in the pectoral muscle and cartilage development, and one involved in mechanisms of cell polarity and asymmetric cell divisions that could contribute synergistically to the PS phenotype, suggesting a digenic mechanism.

Full textIn the last decade lots of research efforts were put in the field of immunotherapy, a relatively new class of treatments that boost the body’s natural defenses to fight against cancer. In medical research, despite growing evidence that sex (i.e., differences between men and women at biological level) might be a disease treatment modifier, sex-based differences in the presentation, progression, adverse events as well as in the prophylactic or therapeutic treatment of diseases were rarely analyzed and reported. Only in 2018, Conforti and colleagues found, for the first time, that considering patients affected by several types of advanced cancers, and treated with immune checkpoint inhibitors as monotherapy or as a combination therapy, males derive larger effect on OS then females do. The overall aim of this doctoral thesis was to extend previous (limited) knowledge on sex-based differences in cancer immunotherapy efficacy and to further explore mechanisms at molecular level that regulate anticancer immune response in men and women. First, we investigated whether, and confirmed that, women with advanced lung cancer derive larger benefit than men from the combination of chemotherapy to an anti-PD-1/PD-L1. We found also an interaction between patients’ sex and the efficacy in OS of two therapeutic strategies (anti-PD-1/PD-L1 alone or in combination with chemotherapy compared to standard chemotherapy) with opposite direction of the effect in men and women: men derive larger benefit than women with an anti-PD-1 treatment alone, while women have better survival with anti-PD-1/PD-L1 plus chemotherapy. Then we deeply studied the sex-based dimorphism of the response to lung cancer immunotherapy, conjecturing that the heterogeneity of response to different immunotherapeutic strategies might be due to differences in the molecular mechanisms that drive anticancer immune response in men and women. We observed a less efficient tumor recognition and infiltration by immune system in men compared to women. In particular, in men, we found a lower abundance of a number of immune cell types in the tumor microenvironment (TME), a significantly higher T-cell exclusion score, a smaller T-cell receptors repertoire diversity and a lower amount of ubiquitous expanded T-cell receptors. We found that such poorer immune infiltration of tumors in men may depend on a less efficient tumor neoantigens presentation to the immune-system, due to lower expression levels of human leukocyte antigen (HLA) class I and II molecules, higher frequency of HLA type I loss of heterozygosity and/or alterations in other component of the antigen presentation machinery. We also showed that, among the molecular pathways and biological processes most significantly enriched in the TME of women, there were many directly related to the anticancer immune response. Contrary, none of the gene sets found significantly enriched in tumors arising in men were directly related to anticancer immune responses. Moreover, we provided a clear example of the potential clinical implications of our findings, showing significant differences in the association between tumor mutational burden and survival benefit observed in men and women treated with anti-PD-1/PD-L1 antibodies. We implemented several statistical methods to answer the different questions depending on the aim of each study. We used meta-analyses to combine results from several studies and to produce estimates of the overall sex-effect of interest. We used cox proportional hazard regression model to analyze survival data and, as a mean to investigate departures from linearity, restricted cubic splines were applied to model the relationship between continuous covariates and the survival outcome of interest. Several bioinformatic tools were used to process the data. Moreover, we implemented the Gene Set Enrichment Analysis methodology in the statistical software SAS with an extension to meta-analysis.

Full textPreterm infants’ cribs in Neonatal Intensive Care Unit (NICU) of “Women’s and Children’s Hospital G. Salesi” of Ancona are surrounded by many devices for the monitoring, diagnosis and treatment of diseases, which provide a huge amount of data that, until now, was only displayed on monitors and periodically transcribed in a paper medical record. Then manual notes were regularly, but not immediately, transcribed in an electronic sheet on a NICU PC with the risk of errors and forgetfulnesses. In this context, physicians have expressed the need to automatically gather data from all these devices to ensure that no key details for patient care were overlooked, as they are aware that the automation of this process could improve the implementation of the procedures of their daily clinical practice. The objective of this thesis is to allow the interfacing of NICU biomedical instruments into a single cloud-based infrastructure that enables the communication between different medical devices and a unique DB. The proposed architecture permits the automation of the process of device data collection, transmission, storage, processing and availability for medical staff, that is guaranteed through the implementation of a web interface that exceeds the functionalities of a simple Electronic Medical Record, thanks to the development of innovative clinical tools. They contain data extraction and analysis techniques and decision-making algorithms, which provide in output advices, reminders, alarms, and indicators that can support physicians in predicting and diagnosing diseases, such as neonatal jaundice or motor disabilities, in monitoring physiological parameters, such as respiratory rate and growth parameters, and in making decisions about clinical problems, such as daily nutritional intakes and follow-ups. All the solutions described above have been validated through experiments conducted in the NICU under the supervision of the physicians and the Head Physician.
