Abstract:
Abstract: The emergence of a wide variety of herbal medicinal products requires marketers to better understand consumer behavior. The various brands offered make consumers tend to have certain preferences before making a purchase decision. This study aims to determine the attributes of herbal medicinal products that determine the level of consumer preference in the purchasing process and the closeness of the attributes to consumer preferences. The method used is interviews with consumers which are then processed with conjoint analysis to determine the attributes that are most important to consumers in making a purchase. The research concludes that the most important attributes in purchasing herbal medicine in order from the most important are the properties, price, expiration time and packaging. There is a close relationship between the combination of attributes studied and consumer preferences for herbal medicine. This study is limited to four attributes and does not use ranking ratings on the stimulus or combination of attributes, consumer behavior. The uniqueness of this study is to analyze the attributes simultaneously. Keywords: Herbal medicine product attributes, consumer preferences, conjoin analysis, level of importance

Abstract:
Premature forecasting of hepatitis is extremely imperative to save an individual years and take appropriate steps to control the ailment. Decision Tree algorithms have been effectively useful in a variety of fields particularly in medicinal discipline. This manuscript investigates the premature forecasting of hepatitis by means of a variety of decision tree algorithms. In this manuscript, we build up a Hepatitis prediction model that can aid medical experts in envisaging Hepatitis condition supported on the medicinal data of patients. At the outset, we have chosen 19 imperative medicinal attributes viz., age, sex, antivirals, steroid, fatigue, anorexia, malaise, spleen palpable, etc., in addition to one target class. Secondly, we build up a prediction model using Pruned C4.5-J48 Decision Tree, Unpruned C4.5-J48, Reduced Error Pruned C4.5-J48 and Hoeffding Tree algorithms classifier for classifying Hepatitis based on these clinical attributes. Lastly, the precision of Pruned J48 decision tree approach proves to be more superior then the other approaches. Outcome acquired illustrates that Albumin and Ascites are the foremost predictive attributes which provides enhanced classification in opposition to the supplementary attributes.

Eifel, Raymond Leo. "Leadership Attributes of Physician Assistant Program Directors." Thesis, The George Washington University, 2014. http://pqdtopen.proquest.com/#viewpdf?dispub=3610781.

Full text
Abstract:

Physician assistant (PA) program directors perform an essential role in the initiation, continuation, and development of PA education programs in the rapidly changing environments of both health care and higher education. However, only limited research exists on this academic leader. This study examined the leadership roles of PA program directors in the context of full-range leadership theory. This theory distinguishes between transformational, transactional, and passive/avoidant behaviors of the program director from the perspectives of both leader and follower. This study also examined leadership outcomes.

Data were collected via an electronic survey (Multifactor Leadership Questionnaire) administered to PA program directors and other members in the same academic unit. The survey was deployed to 82 academic units. The response rates for completed MLQ surveys were 43.2% (54) for program directors and 24.6% (228) for others associated with the PA academic unit.

The results of this study indicated that program directors use transformational leadership factors frequently in the execution of their responsibilities, and those program directors perceive favorable outcomes with these behaviors. However, discrepancies existed between leader perceptions and the perceptions of individuals in the same academic unit in regards to leadership and outcomes, with program directors having more favorable perceptions. This research provides a foundation for future study of the PA program director and contributes towards professional development efforts and the measurement of associated outcomes.

This study aimed to evaluate the influence of esophageal motility parameters, the characteristics of gastroesophageal reflux, the presence of anxiety and depression, the presence of dyspeptic symptoms and the gastric emptying on the laryngeal symptoms (hoarseness, throat clearing and coughing) in patients with posterior laryngitis attributed to GERD. Initially, we studied 20 patients with posterior larynx and typical symptoms of GERD and 12 patients with erosive reflux esophagitis without laryngeal symptoms. All patients underwent upper endoscopy, high-resolution manometry and answered the questionnaire of GERD symptoms (QS-GERD) and the questionnaire about quality of life in GERD (QVRS-DRGE). Then, only the group of patients with laryngitis underwent 24 hours impedance-pH monitoring, answered Hospital Anxiety and Depression Scale (HADS) and the questionnaire of Porto Alegre of dyspeptic symptoms (PADYQ) and underwent gastric emptying breath test. All protocols were approved by the Research Ethics Committee of HUWC (14464214.8.0000.5045). Data were analyzed using the unpaired Student t test, Mann-Whitney and the correlation of Spearman. Endoscopy was normal in 85% of patients with laryngitis. The need to use medication had greater impact on the quality of life of the group with esophagitis, while the degree of dissatisfaction with their situation was significantly higher in patients with laryngitis. Patients presented with laryngitis had pressure of the lower esophageal sphincter and contractility of the esophageal body significantly larger than esophagitis group. There was no correlation between esophageal motility parameters with the frequency of laryngeal symptoms (hoarseness, throat clearing and coughing) in the laryngitis group. There was a positive correlation between the frequency of symptoms of hoarseness, throat clearing and coughing with the number of reflux measured by impedance-pH monitoring. This correlation was found only with weakly acidic reflux (4.0 6.5) for symptoms throat clearing and cough. There was a high prevalence of anxiety / depression in patients with laryngitis (50%), with a positive correlation between the intensity of anxiety and the frequency of hoarseness and , but not coughing. Patients with laryngitis showed a high prevalence of gastrointestinal symptoms (75%), but it was not observed delay in gastric emptying of solids in this group when compared to normal subjects. From this results we conclude that the laryngeal symptoms attributed to gastroesophageal reflux are associated with a greater degree of dissatisfaction with clinical situation and loss in quality of life; with the presence of weakly acidic or alkaline reflux and with anxiety. It was unrelated to endoscopic mucosal injury , changes in esophageal motility and gastric emptying .

Haider, Sonia Ijaz. "The design and testing of an instrument to assess the teaching attributes of trainee doctors." Thesis, University of Warwick, 2012. http://wrap.warwick.ac.uk/56354/.

Full text
Abstract:
Background: In the UK specialty trainees are a major source of clinical teaching for junior doctors. Medical education and teaching skills are core competencies included in the generic curriculum for specialist training. Hence, there is a need for a validated assessment instrument that can measure the attributes of specialty trainees as effective teachers, leading to the research question; Is it possible to devise an instrument to measure the teaching ability of specialty trainee doctors? Methods: The study was conducted in two phases. In the first phase, the content of the instrument was generated from the literature and tested using the Delphi technique. This was followed by pilot testing the instrument. In the second phase, the instrument was field tested for validity and reliability by conducting factor analysis, Cronbach alpha and Generalizability coefficient. The instrument was also tested for feasibility by calculating the time taken to complete the instrument. Acceptability and educational impact were determined by qualitative analysis of written feedback from participants. The attributes of specialty trainees were assessed by clinical supervisors, peers and students. Results: The Delphi study produced a consensus on 15 statements for the final draft of the instrument. This draft was piloted and finalised using feedback from that pilot. The instrument was field tested. In the field study a total of 340 instruments were completed. The instrument exhibited internal consistency (Cronbach’s alpha 0.90) and the Generalizability coefficient was 0.92. Factor analysis demonstrated a three factor solution (learning-teaching milieu, teaching skills and learner-orientated). The mean time to complete the instrument was five minutes. Feedback from participants indicated that it was an acceptable method of assessment, and trainees also found it useful for improving their teaching performance. Discussion: Findings from the present study suggest that this instrument demonstrates robust validity and reliability. It is feasible to use it in a busy clinical setting. It is acceptable by stakeholders which indicate that it can be used for assessment of teaching in clinical settings. Further specialty trainees found it useful, thereby indicating a positive educational impact. Conclusion: This new instrument, specifically designed to test the teaching attributes of doctors-intraining, can be useful for providing formative and summative assessment of clinical teaching.

Creating a database to manage medical data is not the easiest. We create a database to be used for a presentation tool that presents medical data about patients that is stored in the database. We examine which of the three databases, MySQL with relational design, MySQL with EAV design and MongoDB that are best suited for storing medical data. The analysis i performed in two steps. The first step handles the database that is most effective to retriev data. The second step examines how easy it is to change the structure of the various databases. The results show that depending on whether efficiency or maintenance is most important, different databases are the best choise. MySQL with relational design proves to be most effective while MongoDB is the easiest to maintain.

Abstract:
The use of electronic health records (EHR) has the potential to improve relationships between physicians and patients and significantly improve care delivery. The purpose of this study was to analyze the relationships between hospital attributes and EHR implementation. The research design for this study was the cross-sectional approach. Secondary data from the Health Information and Management Systems Society (HIMSS) Analytics Database was utilized (n = 169) in a correlational crosssectional research design. Normalization Process Theory (NPT) and implementation theory were the theoretical underpinnings used in this study. Multiple linear regressions results showed statistically significant relationships between the 4 independent variables (region, ownership status, number of staffed beds [size], and organizational control) and the outcomes for the dependent variables of EHR software application attributes (Clinical Decision Support Systems (CDSS) components), EHR software application attributes (major systems), and successful implementation of Meaningful Use (MU) (p = .001). A statistically significant relationship (p = .001) was also found between the 2 independent variables (EHR software application attributes [CDSS components] and EHR software application attributes [major systems]) and the outcome of successful implementation of MU when combined. This evidence should provide policy makers and health practitioners support for their attempts to implement EHR systems to result in positive Meaningful Use which has been shown to be more cost effective and result in better quality of care for patients. The potential social change is improved medication prescribing and administration for hospitals and, lower cost and better quality of care for patients.

Kwok, Sze Man Simon. "Attribute-driven segmentation and analysis of mammograms." University of Western Australia. Centre for Intelligent Information Processing Systems, 2005. http://theses.library.uwa.edu.au/adt-WU2005.0010.

Full text
Abstract:
[Truncated abstract] In this thesis, we introduce a mammogram analysis system developed for the automatic segmentation and analysis of mammograms. This original system has been designed to aid radiologists to detect breast cancer on mammograms. The system embodies attribute-driven segmentation in which the attributes of an image are extracted progressively in a step-by-step, hierarchical fashion. Global, low-level attributes obtained in the early stages are used to derive local, high-level attributes in later stages, leading to increasing refinement and accuracy in image segmentation and analysis. The proposed system can be characterized as: • a bootstrap engine driven by the attributes of the images; • a solid framework supporting the process of hierarchical segmentation; • a universal platform for the development and integration of segmentation and analysis techniques; and • an extensible database in which knowledge about the image is accumulated. Central to this system are three major components: 1. a series of applications for attribute acquisition; 2. a standard format for attribute normalization; and 3. a database for attribute storage and data exchange between applications. The first step of the automatic process is to segment the mammogram hierarchically into several distinctive regions that represent the anatomy of the breast. The adequacy and quality of the mammogram are then assessed using the anatomical features obtained from segmentation. Further image analysis, such as breast density classification and lesion detection, may then be carried out inside the breast region. Several domain-specific algorithms have therefore been developed for the attribute acquisition component in the system. These include: 1. automatic pectoral muscle segmentation; 2. adequacy assessment of positioning and exposure; and 3. contrast enhancement of mass lesions. An adaptive algorithm is described for automatic segmentation of the pectoral muscle on mammograms of mediolateral oblique (MLO) views

Abstract:
The purpose of this study was to evaluate the influence of facial convexity on the perception of social attributes in a young adult population. Nine models were asked to pose for a 3 dimensional photograph that was then modified to represent ideal, retrognathic, and prognathic facial convexity angles. Survey evaluators were shown digital videos of models with ideal and non-ideal profiles and asked to rate their perception of the following 4 social attributes: intelligence, athleticism, popularity, and leadership ability using a visual analog scale. Results gathered from 271 evaluators showed that the model images with ideal facial convexity angles were rated higher on average than the same model images with retrognathic and prognathic profiles. The differences in ratings between ideal and non-ideal profiles were significant for intelligence (P = 0.0009), athleticism (P = 0.0002), and leadership ability (P = 0.0008). Differences in perceived popularity (P = 0.2169) showed no significant differences among facial convexities.

Full text
Abstract:
Unani Medicine in the Making examines the institutions and practices of Unani medicine, the Graeco-Islamic healing practice based on the humoral theory attributed to Hippocrates and officially recognized as a system of medicine in India. Drawing on diverse materials, including Urdu sources, interviews with practitioners, and observations in clinics, the book explores what Unani medicine is today by attending to its multiplicity, scrutinizing apparent tensions between the understanding of Unani as a system of medicine and its multiple enactments as Islamic medicine, medical science, or alternative medicine. Ethnographic details provide vivid descriptions of the current practice of Unani in India, and invite readers to rethink the idea that humoral medicine is incommensurable with modern medicine and science, and that the modernization of Asian medicines invariably leads to their biomedicalization. Ultimately, the book also discusses the relationship of Unani with Muslim communities, examining the growing practice of Prophetic Medicine in Urban India and increasing representations of Unani as Islamic Medicine.

Abstract:
Patient, disease and physician were the three corners of the ‘medical triangle’ according to one of the texts attributed to Hippocrates, a famous ancient Greek doctor. This volume, covering a period from roughly 800 BCE to 800 CE, examines and deconstructs these three aspects of ancient medicine in the Mediterranean world. It shows that, while physicians sought to assert themselves as experts in the medical art, they had to contend with numerous other healers whose methods, remedies and tools patients often favoured. It explores the ways in which civic entities, cities, kingdoms and empires, and their officials directly and indirectly shaped medical encounters and discoveries. It examines the interaction between medicine and the environment, non-human animals and plants. To attempt a cultural history of medicine in antiquity requires bringing together a wealth of sources: the texts attributed to Hippocrates, Galen and other medical authors are not neglected, but they are studied alongside other literary and historical works, letters on papyri, funerary inscriptions celebrating healers, surgical tools and bioarchaeological remains. While discussing the enduring cultural impact of classical Greek and Roman medicine in the West, through texts such as the Hippocratic Oath or names of diseases and types of medicines, this volume reveals the various ways in which health, disease and medical treatments were experienced diversely in the ancient world, according to gender, socio-economic class and ethnicity.

Abstract:
The protected landscape (PL) of Biambare is one of the most famous green sites for local visitors and tourists of Sarajevo. This area is distinguished by beautiful Bosnian Alps (Dinarids) meadows and plenty of spectacular natural phenomena, especially caves; the Central Biambare Cave is currently the most popular. The terrain is situated at an elevation range of 915–1044 m above sea level, so it mostly has a pleasant pre-mountain climate, etc. Therefore, tourism supply is based on different products and activities: speleology; walking in nature; bicycling; riding; hiking; alpinism; fishing; hunting; skiing; and collecting medicinal herbs and mushrooms. The aim of the paper is to represent the Biambare tourist image based on identifying its central natural attributes and according to the evaluation of the visitor’s experience. The two relevant parameters for understanding tourist impressions are: “satisfaction” and “loyalty”. These are higher among foreign than resident visitors, particularly among Middle Eastern tourists, who favour this site above many others in Canton Sarajevo.

Abstract:
Abstract Medicinal plants are widely used in many parts of the world. In Brazil, due to the influence of indigenous roots, this practice has been widespread until today, especially in northern and northeastern regions, where there is the largest concentration of indigenous tribes still alive. The precariousness of public services in remote communities has resulted in the use of plants for medicinal purposes as the main therapeutic resource of these populations and ethnic groups, given that the presence of knowledge about herbs and plant materials, which is passed on among families, is the primary source of treatment and cure of various diseases based on empirical experimentation in these communities (LEITE, 2014). Thus, it should be noted that the use of plants for curative purposes comes from the earliest reports of human societies and, currently, it can still be observed that the wealth attributed to the ancient knowledge of traditional peoples has been a potentiating factor in innovations in research and the search for new substances from what has already been described in these cultures.

Abstract:
This paper analyzes gender differences in three Chilean professional labor markets, business, law and medicine, utilizing a new and rich data set collected for this purpose. The results show that differences in wages attributed to gender are only present in the legal profession. In business/economics, a vector of current family condition eliminates the gender effect and in Medicine, taking into account hours worked, size of firm and region also eliminates gender differences. The paper further shows that individuals' perceived locus of control (internal or external) is relevant in explaining the distribution of earnings.

Uche, Chidi, Zita Ekeocha, Stephen Robert Byrn, and Kari L. Clase. Retrospective Study of Inspectors Competency in the Act of Writing GMP Inspection Report. Purdue University, December 2021. http://dx.doi.org/10.5703/1288284317445.

Full text
Abstract:
The research was a retrospective study of twenty-five Good Manufacturing Practice (GMP) inspection reports (from March 2017 through to December 2018) of a national medicine regulatory agency, drug Inspectorate, in West Africa, designed to assess the inspectors’ expertise in the act of inspection report writing. The investigation examined a paper-based tool of thirteen pre-registration Inspection reports and twelve GMP reassessment reports written prior and following an intervention program by external GMP trainers to enhance inspectors’ skill in pharmaceutical cGMP inspection. The study made use of quantitative analysis to investigate each team’s expertise in the act of writing GMP inspection report. Likewise, each report’s compliance with the requirements of three regulatory standards on GMP inspection report writing was ascertained. Impact of intervention program on lead inspectors’ competence was assessed. Lastly, gap in each team writing effectiveness, and lead inspectors’ abilities to deliver an effective report were determined. The results showed one of the inspection team (4.0%) wrote an excellent report. Two (8.0%) of the twenty-five inspection teams penned good inspection reports. Eleven (44.0%) teams drafted needs improvement reports and the remaining eleven teams (44.0%) prepared unacceptable reports. The excellent report and the two good reports had report format that meet expectation. One (50.0%) of the good reports showed the authors possess excellent knowledge of cGMP technical areas. The remain good report (50.0%) revealed the writers’ knowledge.as good. The excellent report showed the authors displayed partial mastery in the use of objective evidence while the two good reports disclosed theirs as having partial and evolving abilities. One of the teams (50.0%) that wrote good reports displayed good use of third person narrative past tense in report writing whereas the other team used the same tense and voice excellently. Generally, a sort of marginal level of performance was prominent among the inspection teams. A gap, if not tackled, will slow down regulatory process through increase report review, litigations that query report factual accuracy (AIHO, 2017) and delay in issuance of marketing authorization. In conclusion, trainings on quality attributes, such as technical content (Quality Management System (QMS) and Site), the use of objective evidence, assignment of risk levels to GMP violations and citing of applicable laws, regulation and guidelines that substantiate GMP observations, were recommended, to enhance knowledge sharing and regulators’ performance in the act of writing inspection report.

Schwartz, Bertha, Vaclav Vetvicka, Ofer Danai, and Yitzhak Hadar. Increasing the value of mushrooms as functional foods: induction of alpha and beta glucan content via novel cultivation methods. United States Department of Agriculture, January 2015. http://dx.doi.org/10.32747/2015.7600033.bard.

Full text
Abstract:
During the granting period, we performed the following projects: Firstly, we differentially measured glucan content in several pleurotus mushroom strains. Mushroom polysaccharides are edible polymers that have numerous reported biological functions; the most common effects are attributed to β-glucans. In recent years, it became apparent that the less abundant α-glucans also possess potent effects in various health conditions. In our first study, we explored several Pleurotus species for their total, β and α-glucan content. Pleurotuseryngii was found to have the highest total glucan concentrations and the highest α-glucans proportion. We also found that the stalks (stipe) of the fruit body contained higher glucan content then the caps (pileus). Since mushrooms respond markedly to changes in environmental and growth conditions, we developed cultivation methods aiming to increase the levels of α and β-glucans. Using olive mill solid waste (OMSW) from three-phase olive mills in the cultivation substrate. We were able to enrich the levels mainly of α-glucans. Maximal total glucan concentrations were enhanced up to twice when the growth substrate contained 80% of OMSW compared to no OMSW. Taking together this study demonstrate that Pleurotuseryngii can serve as a potential rich source of glucans for nutritional and medicinal applications and that glucan content in mushroom fruiting bodies can be further enriched by applying OMSW into the cultivation substrate. We then compared the immune-modulating activity of glucans extracted from P. ostreatus and P. eryngii on phagocytosis of peripheral blood neutrophils, and superoxide release from HL-60 cells. The results suggest that the anti-inflammatory properties of these glucans are partially mediated through modulation of neutrophileffector functions (P. eryngiiwas more effective). Additionally, both glucans dose-dependently competed for the anti-Dectin-1 and anti-CR3 antibody binding. We then tested the putative anti-inflammatory effects of the extracted glucans in inflammatory bowel disease (IBD) using the dextran sulfate sodium (DSS)–induced model in mice. The clinical symptoms of IBD were efficiently relieved by the treatment with two different doses of the glucan from both fungi. Glucan fractions, from either P. ostreatus or P. eryngii, markedly prevented TNF-α mediated inflammation in the DSS–induced inflamed intestine. These results suggest that there are variations in glucan preparations from different fungi in their anti-inflammatory ability. In our next study, we tested the effect of glucans on lipopolysaccharide (LPS)-induced production of TNF-α. We demonstrated that glucan extracts are more effective than mill mushroom preparations. Additionally, the effectiveness of stalk-derived glucans were slightly more pronounced than of caps. Cap and stalk glucans from mill or isolated glucan competed dose-dependently with anti-Dectin-and anti-CR-3 antibodies, indicating that they contain β-glucans recognized by these receptors. Using the dextran sulfate sodium (DSS)-inflammatory bowel disease mice model, intestinal inflammatory response to the mill preparations was measured and compared to extracted glucan fractions from caps and stalks. We found that mill and glucan extracts were very effective in downregulatingIFN-γ and MIP-2 levels and that stalk-derived preparations were more effective than from caps. The tested glucans were equally effective in regulating the number of CD14/CD16 monocytes and upregulating the levels of fecal-released IgA to almost normal levels. In conclusion, the most effective glucans in ameliorating some IBD-inflammatory associated symptoms induced by DSS treatment in mice were glucan extracts prepared from the stalk of P. eryngii. These spatial distinctions may be helpful in selecting more effective specific anti-inflammatory mushrooms-derived glucans. We additionally tested the effect of glucans on lipopolysaccharide-induced production of TNF-α, which demonstrated stalk-derived glucans were more effective than of caps-derived glucans. Isolated glucans competed with anti-Dectin-1 and anti-CR3 antibodies, indicating that they contain β-glucans recognized by these receptors. In conclusion, the most effective glucans in ameliorating IBD-associated symptoms induced by DSS treatment in mice were glucan extracts prepared from the stalk of P. eryngii grown at higher concentrations of OMSW. We conclude that these stress-induced growing conditions may be helpful in selecting more effective glucans derived from edible mushrooms. Based on the findings that we could enhance glucan content in Pleurotuseryngii following cultivation of the mushrooms on a substrate containing different concentrations of olive mill solid waste (OMSW) and that these changes are directly related to the content of OMSW in the growing substrate we tested the extracted glucans in several models. Using dextran sulfate sodium (DSS)–inflammatory bowel disease (IBD) mice model, we measured the colonic inflammatory response to the different glucan preparations. We found that the histology damaging score (HDS) resulting from DSS treatment reach a value of 11.8 ± 2.3 were efficiently downregulated by treatment with the fungal extracted glucans, glucans extracted from stalks cultivated at 20% OMSWdownregulated to a HDS value of 6.4 ± 0.5 and at 80% OMSW showed the strongest effects (5.5 ± 0.6). Similar downregulatory effects were obtained for expression of various intestinal cytokines. All tested glucans were equally effective in regulating the number of CD14/CD16 monocytes from 18.2 ± 2.7 % for DSS to 6.4 ± 2.0 for DSS +glucans extracted from stalks cultivated at 50% OMSW. We finally tested glucans extracted from Pleurotuseryngii grown on a substrate containing increasing concentrations of olive mill solid waste (OMSW) contain greater glucan concentrations as a function of OMSW content. Treatment of rat Intestinal epithelial cells (IEC-6) transiently transfected with Nf-κB fused to luciferase demonstrated that glucans extracted from P. eryngii stalks grown on 80% OMSWdownregulatedTNF-α activation. Glucans from mushrooms grown on 80% OMSW exerted the most significant reducing activity of nitric oxide production in lipopolysaccharide (LPS) treated J774A.1 murine macrophages. The isolated glucans were tested in vivo using the Dextran Sodium Sulfate (DSS) induced colitis in C57Bl/6 mice and found to reduce the histology damaging score resulting from DSS treatment. Expression of various intestinal cytokines were efficiently downregulated by treatment with the fungal extracted glucans. We conclude that the stress-induced growing conditions exerted by OMSW induces production of more effective anti-inflammatory glucans in P. eryngii stalks.
