Abstract:
Neuropsychological deficits in depression are a significant therapeutic challenge. Their occurrence means poor therapeutic prospects, worse social and professional functioning after therapy, as well as a higher risk of relapse. Despite clinical improvement, they often remain even in a state of complete remission. Beck’s model of depression does not include interventions directed at neuropsychological processes leading to neurocognitive mechanisms responsible for the development and maintenance of depression. More recent trends in cognitive-behavioral therapy seem to involve neuropsychological processes to a greater extent. This applies to Well’s metacognitive model, which focuses on the meta-level of thinking. Therapeutic process involves various aspects of attention, as well as detached mindfulness. Available empirical studies indicate that this therapy model is more effective in reducing neuropsychological deficits than Beck’s model. Acceptance and commitment therapy as well as mindfulness-based cognitivetherapy both focus on the development of skills that are related to the efficiency of executive functions and flexibility of attention, i.e. the cognitive processes whose deficits are characteristic of depression. However, research is needed to confirm their effectiveness in reducing neuropsychological deficiencies compared to other therapeutic models. Interventions in the field of cognitive remediationcan be used to enrich cognitive-behavioral therapies and increase their effectiveness. Until now, they have been used as a separate form of therapy, for example in anorexia.

Abstract:
Introduction. There is not much data about the composition of populations of the immune system in acute appendicitis. The basic histopathological criterion for the diagnosis of acute appendicitis is neutrophil infiltration of the muscle membrane. Aim. The subject of this publication is a semi-quantitative evaluation of B lymphocytes (CD20+), T lymphocytes (CD3+) and macrophages (CD68+), and the determination of the number of active lymph nodes during the course of inflammation.Material and Methods. The study material was obtained from 79 patients who had an appendectomy due to acute appendicitis. In this group, the tissue was obtained from: 34 women (aged 20 to 91) and 45 men (aged 20 to 72).Results. In the course of acute appendicitis, there is involvement of lymph node B lymphocytes, T lymphocytes and macrophages. Independent of the type of inflammation, the cellular make-up of the nodes is similar. The number of lymph nodes decreases with age and is gender dependent.Conclusions. In the course of acute appendicitis, there is involvement of lymph node B lymphocytes, T lymphocytes and macrophages. The number of lymph nodes decreases with age and is gender dependent. A statistically significant number of the examined cells of the immunological system in the lymph nodes changed due to inflammation (p<0.001). B and T lymphocytes in the lymph nodes and in the mucous membrane of the appendix differed depending on the sex, and the presence of B lymphocytes in the mucous membrane was significantly higher in the group of 20-40 years of age. T lymphocytes were predominant in the centres of the lymph nodes in groups 20-40 and 61-91 years of age, and in the peripheral zones in the group of 41-60 years of age.

Abstract:
This project aims to longitudinally assess changes in the psychosocial functioning of females with adolescent idiopathic scoliosis before and after completion of surgical treatment and implementation of cognitive-behavioural therapy. The planned study is a longitudinal randomised trial with 1-year follow-up. The cross-sectional aspect of the research concerns differences in perception of body shape from the perspective of the patients, their doctors, and healthy female adolescents. This study will recruit 106 patients treated at the Department of Pediatric Orthopaedics and Traumatology, Poznan University of Medical Sciences, their doctors and 106 healthy female controls. It will be the first study to use biometric self-avatars in virtual reality to investigate changes within body representation in scoliosis. The study findings will inform the development of guidelines for interdisciplinary rehabilitation of scoliosis patients following surgical treatment.

Cancers still constitute an important health burden, due to their high incidence and mortality. The discovery and clinical implementation of novel sensitive diagnostic markers could improve treatment outcomes. Circulating cell-free DNA (ccfDNA) offers great promise for the development of new molecular biomarkers. However, the analysis of ccfDNA methylation poses significant challenges due to ccfDNA fragmentation and low concentration in the blood. Techniques which have recently been developed for liquid biopsy studies have been adapted to the detection of trace amounts of ccfDNA. This mini-review focuses on recent technological advances which allow the discovery of new sensitive and specific liquid biopsy cancer biomarkers based on DNA methylation detection.

Abstract:
The project “Recovery from bone loss, diminished mineral density and strength in mice after treatment with steroidal and nonsteroidal anti-inflammatory drugs by injection of exosomes enriched with agomir miRNAs” is an animal experiment project, and an attempt to apply small RNA strands to reverse the harmful effects of anti-inflammatory drugs. Patients chronically treated with an anti-inflammatory drug suffer from musculoskeletal side effects, including reversed mineralisation and disabled bone fracture healing. The aims of the study are to measure changes in bone mineral density and bone strength in mice treated with methylprednisolone or diclofenac in combination with treadmill exercise. The reversal of the negative effects of these drugs will be assayed using modified miRNA agomir. Bones obtained from the treated mice will be analysed using micro-CT, dynamic mechanical analysis (DMA), nanoindentation, Raman spectroscopy and gene expression. We expect to find specific miRNA counteracting the demineralisation of the mice bones caused by methylprednisolone or diclofenac.

The L138ins mutation, found in Russian cystic fibrosis (CF) patients, is a duplication of three nucleotides (CTA) in exon 4 of the CFTR gene and is categorised as a small in-frame insertion/deletion. As a result, the CFTR protein molecule elongates by one amino acid residue, leucine, at position 138 (codon 138 (CTA)). In accordance with the new nomenclature, it should be called c.411_412insCTA (p.Leu138dup). The c.411_412insCTA (p.Leu138dup, L138ins) mutation is found in CF patients of Slavic origin (Russians, Ukrainians) and has been linked to a single haplotype of the intragenic DNA markers IVS1CA-IVS6aGATT-IVS8CA-IVS17bCA - 22-7-16-13.

Abstract:
Introduction. Benign tumour surgery of the parotid gland may cause iatrogenic injury of the facial nerve, with results of postoperative treatment depending on the type of injury. The study aimed to clarify the mechanism of facial nerve injury after benign tumour surgery of parotid gland. Materials and Methods. The effectiveness was verified preoperatively and 1, 3, 6 and 17 months postoperatively. House-Brackmann scales, electroneurography, blink reflex study and needle electromyography were performed. Pharmacological treatment (Galantamine, Cocarboxylase, Dexamethasone, Triamcinolone) and supervised physiotherapeutic procedures (Facial-Oral-Tract-Therapy, Proprioceptive neuromuscular facilitation) were applied for six months. Results. Tumour removal led to the total paralysis of the left facial nerve, IV, III and III House-Brackmann grades were ascertained at the subsequent 3rd–5th periods of observation. In postoperative studies, electroneurography results showed full functional recovery of the frontal branch and incomplete regeneration in the marginal mandibular branch. Blink reflex examination showed proper parameters of evoked potentials only during preoperative and the last observation period. Residual voluntary activity of the frontal muscle and weak voluntary activity of orbicularis oris muscle were recorded in the needle electromyography examination. Contracture of mimic muscles at rest and improvement of their voluntary activity on the left side was observed six months after surgery compared to the early period of observation. Conclusion. Consecutive studies showed the predominant axonal type of injury in the marginal mandibular branch and neuropraxia effect of the facial nerve, allowing the creation of a rehabilitation programme optimal for the functional recovery of the nerve.

Abstract:
Background: Intrahepatic cholestasis of pregnancy (ICP) is the most common liver disorder during gestation. The exact pathogenesis of ICP is multifactorial and still unclear. Therefore, our study aimed to check whether the selected ABCB4and ABCB11nucleotide variants are associated with an increased risk of ICP. Methods:ICP was diagnosed based on clinical symptoms characteristic of this disease and confirmed by increase in serum bile acids and transaminases, spontaneous resolution of clinical symptoms and normalization of laboratory tests after delivery. The total of 86 pregnant women meeting the criteria were included into the study. Healthy pregnant women with uncomplicated pregnancy served as control group (n=310). Sixcommon nucleotide variants in theABCB11and ABCB4genes were genotypedwith the use of high-resolution melting curve analysis. Conclusion:Our study did not show any significant association of analysed ABCB4and ABCB11nucleotide variants with the increased risk of intrahepatic cholestasis of pregnancy.

Abstract:
The outbreak of interest in photodynamic therapy (PDT) at the end of last century to treat cancer and other diseases was based on the promise of localised treatment, cheaper therapy and fast ablation of the treated organ. One of the most attractive features of PDT is that it can evade cancer’s resistance to photosensitisers. PDT for cancer therapy depends on the absorption of a photosensitiser within the malignant tissue. The photosensitising drug is then activated by light (usually from a laser) and the active drug destroys the targeted tissue. However, one must consider that this is a complex mechanism involving many factors such as the diverse light and oxygen distribution in the treated organs, which has mitigated application of this technique in clinical practice. PDT is not a simple treatment that can be done by eyeballing; it requires precise planning that can be done with the help of complex computer programs. Computer simulation of PDT to optimise treatment depends heavily on intense calculations in all steps of the procedure, and desktop computers are only now sufficiently powerful to assist physicians during therapy in real-time. In this mini-review, the challenges of photodynamic therapy are described, and possible to solutions to overcome these are presented.

Abstract:
Introduction. The functional assessment of general movements (GMs) is a common test for the developing nervous system. The high predictive validity of abnormal GMs for cerebral palsy has been documented among preterm infants. Aim. The present study examined whether term infants without any documented risk factors for neurodevelopment delay may benefit from an assessment of GMs. Material and Methods. One hundred and four infants ranging in age from 1–4 months were evaluated using Prechtl’s method, of which, thirty-eight were younger than two months of age and the remaining sixty-eight were older than two months of age (with available detailed neonatal characteristics). The following movements were considered among younger infants, writhing, poor repertoire and cramped synchronised, whereas fidgety, cramped synchronised, poor repertoire, chaotic and abnormal GMs were evaluated in older infants. Infants were classified as ‘normal’ or ‘abnormal’ groups based on their presenting GMs. We determined postural positional preference, following Kaplan recommendations, with features categorised as either ‘present’ or ‘absent’, as well as activity level and general muscle tone (‘normal’ or ‘abnormal’). Results. Cramped synchronised GMs were observed in seven (18.4%) younger infants and in eleven (16.7%) older infants. There was no difference in the clinical characteristics of children with normal vs. abnormal GMs. Abnormal muscle tone was associated with a higher OR (p = 0.0039) of presenting with abnormal GMs (4.6063; 95%CI: 1.6303–13.0149). Although the infants studied were not at risk for developmental disorders, almost one-fifth required follow-up neurological consultation. Conclusions. An assessment of GMs should be considered as a universal screening tool among healthy infants without risk factor(s) for developmental deficits.

Abstract:
The primary objective of the study is to compare the effect of endurance and endurance‑strength training on endothelial function in women with abdominal obesity. The secondary objectives include the assessment of the effect of both types of training on anthropometric, densitometric and biochemical parameters. In total, at least 100 women will be recruited for the study. The study population will be randomly divided into two groups according to the type of training: endurance and endurance‑strength training. During the 3-month of intervention, both groups will be performed three times a week training of an equal exercise volume and duration of 60 minutes. Before and after the intervention selected anthropometric and densitometric parameters will be measured and body composition will be analysed. In addition, biochemical parameters related to glucose and insulin homeostasis, lipid metabolism, antioxidant status, oxidative stress, inflammatory markers and endothelial function will be assessed.

Aim. The aim of this systematic review was to assess the effect of vitamin K supplementation on anthropometric parameters and adipokine levels in adults.Material and Methods. Four databases (PubMed, Web of Sciences, Scopus and the Cochrane Library) were searched to select studies in which the effect of vitamin K supplementation on body weight, body mass index (BMI), fat mass, leptin and adiponectin levels were assessed.Results. We identified nine studies that included a total of 542 subjects. Vitamin K supplementation did not influence body weight, BMI and percentage of fat mass. In addition, the effect of vitamin K supplementation on adipokines levels was equivocal. Conclusions. Vitamin K supplementation did not affect anthropometric parameters and adipokines levels. Nevertheless, further studies are needed to clarify the effect of vitamin K supplementation on these parameters in adults.

Nicotinamide adenine dinucleotide (NADH/NAD+) is involved in many important biochemical reactions in human metabolism, including participation in energy production by mitochondria. Flow Mediated Skin Fluorescence (FMSF) is a non-invasive method to study dynamic changes in the content of the reduced form of NADH by measuring the optical properties of NADH related to the emission of the autofluorescent light (460 nm) after an earlier excitation by ultraviolet light. This review summarises the available studies using this method to describe its potential and limitations.

Abstract:
4-Thiazolidinones, as examples of privileged scaffolds, have been the focus of medicinal chemistry since 60th. Among them, 5-substituted thiazolidinones with a C5 exocyclic bond (5-ene derivatives) are of special interest due to chemical characteristics and pharmacological profiles, possessing anticancer, antimicrobial, and antiviral properties, as well as being high-affinity ligands to a number of biological targets. A new medicinal chemistry trend claims that the aforementioned compounds are frequent hitters or pan assay interference compounds, which are useless because of the possible low selectivity. This is argued by the Michael acceptor property of 5-ene-4-thiazolidinones, which is actively discussed in the literature and requires further investigation. Based on SAR analysis, the main vectors for the design of 5-ene-4-thiazolidinone-based molecules were proposed: complication of C5 fragment; introduction of the substituents in the N3 position; synthesis of isosteric heterocycles; combination with other pharmacologically attractive fragments; annealing of thiazolidinone core; utilisation of 5-ene-thiazolidinones in synthesis of other compounds. The affinity of 5-ene-4-thiazolidinones toward various targets can be regarded as an advantage in polypharmacological approaches. Michael acceptors are considered as the “new old tool” for new drug creation, especially anticancer agents. One of the possible solutions within privileged substructure-based diversity-oriented synthesis is the fixation of 5-ene-4-thiazolidinone fragment in the fused heterocycles, for example, thiopyrano[2,3-d]thiazoles obtained from 5-ene-thiazolidinones.

Abstract:
Introduction. T helper cells (Th)-1 and -2 cytokines homeostasis control or predict clinical outcome of infected persons, especially those with HIV/AIDS. This case-control study evaluated the leucocyte differentials, TNF-α, IL-2 and -10 levels, among HIV-infected persons with serological evidence of leishmaniasis attending the University of Abuja Teaching Hospital, Nigeria. Material and Methods. This study involved blood samples from 28 HIV-infectedwith Leishmania donovani rK39 and IgG positive (group 1), 30 age- and sex-matched HIV-infected individuals without Leishmania antibodies (group 2) and 30 apparently healthy persons without HIV and Leishmania antibodies (group 3). Full blood counts, TNF-α, IL-2 and IL-10 levels were analysed using an automated haematology analyser and ELISA, respectively. Structured questionnaires were used to collate biochemical and clinical data from participants. Results. Ten (35.7%) participants in group 1 were on ART, 15 (50%) in group 2 were on ART, while group 3 were ART naïve. There were significantly higher values in basophil (4.4 ± 2.5%) and eosinophil counts (12.9 ± 3.8%) in HIV/Leishmania coinfected persons (p˂0.005), whereas other white cell subpopulations were significantly lower in the HIV/Leishmania coinfected participants (p˂0.05). There were significantly reduced CD4+ T cell counts (119 ± 26 vs 348 ± 63 vs 605 ± 116 cells/mm3), TNF-α (36.82 ± 8.21 vs 64.67 ± 12.54 vs 254.98 ± 65.59 pg/mL) and IL-2 levels (142.14 ± 20.91 vs 507.6 ± 84.42 vs 486.62 ± 167.87 pg/mL) among HIV/Leishmania coinfected participants compared to group 2 and group 3 participants, respectively. However, higher IL-10 levels (80.35 ± 14.57 pg/mL) were detected in HIV/Leishmania coinfected participants compared to the HIV mono-infected (62.2 ± 10.43 pg/mL) and apparently healthy persons (23.97 ± 4.88 pg/mL; p˂0.001). Conclusion. Eosinophil and basophil counts, and serum IL-10 level were high in HIV/Leishmania coinfected patients demonstrating parasite-induced hypersensitivity and immunosuppression.

Coronaviruses are a large family of viruses that cause illness ranging from the common cold to more severe diseases such as Middle East Respiratory Syndrome (MERS-COV) and Sever Acute Respiratory Syndrome (SARS-COV). The Coronavirus Disease 2019 (COVID-19) refers to the cluster of viral pneumonia cases which first started occurring in Wuhan city, which has a population of 11 million people and is the largest city in Hubei province in central China, since December 2019. On Tuesday, the 11th of February 2020, the WHO officially named the disease caused by the new coronavirus "COVID-19", with more than 43,000 patients, and more than 1,000 deaths cases were reported in China. The Government of Israel, the Prime Minister's Office in collaboration with the Ministry of Health, the Ministry of Defense and the Treasury, have decided to take early first steps to prevent the spread of the virus in the Country. The first step was banning Chinese tourists from entering the country via Ben-Gurion International Airport, and this was only the beginning, as afterwards the decision was made to close all border crossings. The idea was to reduce the number of entrances to the State of Israel as much as possible, and every citizen coming back from countries such as China and European countries was directly sent to home quarantine for a period of 14 days.

The project “Searching for new genes and loci involved in cleft lip and palate in the Polish population – genome-wide association study” is a case-control study in a group of unrelated subjects with non-syndromic cleft lip with or without cleft palate (NSCL/P) and healthy individuals with no family history of clefting or other congenital disorders. The overall goal of this grant proposal is to identify novel genetic factors, which can play a significant role in the pathogenesis of orofacial clefts in the Polish population. To accomplish the proposed aim, a two stage genome-wide association study will be performed. In the first stage, Illumina's HumanOmni Express BeadChips arrays will be used to genotype over 700,000 polymorphisms in NSCL/P patients and controls. In the second stage, SNPs showing the most compelling association with the risk of orofacial clefts will be tested in an independent sample set using standard genotyping methods. This research project is expected to be completed in July 2015.

II la Legea-cadru nr. 153/2017 privind salarizarea personalului plătit din fonduri publice, precum şi a condiţiilor de acordare a acestuia, pentru familia ocupaţională de funcţii bugetare ''Sănătate şi asistenţă socială'' – aprobat prin Hotărârea de Guvern nr.153/2018 - potrivit căruia sporurile acordate personalului medical creează diferențe mari de venituri între specialitățile medicale, aceste sporuri având cuantum între 5% și 85%. Acordarea diferențiată a sporurilor salariale, în funcție de specialitatea medicală, este lipsită de o justificare obiectivă și rezonabilă, iar amplitudinea diferențelor procentuale dintre sporuri nu se regăsește în alte state europene. Studiul analizează aceste aspecte legislative și are ca obiectiv propunerea unor modificări care să elimine inechitățile identificate.

introduction: Medico-legal cases are very crucial from a legal point of view. Lots of controversies and conflicts are associated with such types of cases so there was a need to analyze these cases. To fulfill this objective present study was conducted. Objectives: To analyze medico-legal cases referred to the hospital of ACPM Medical College, Dhule, Maharashtra. Methodology: A retrospective study was done by assessing the MLC cases referred to A.C.P.M. Medical College during the period from 1st January 2021 to 31st December 2021. Observations: In the year 2021 total 341 MLC cases were reported to the hospital of ACPM medical college. Out of these males were 276 (80.93%) & females were 65 (19.06%). Most commonly involved age group was 21-30 years (n= 108, 31.67%). Most commonly affected people were Hindu (n=303, 88.85%) which was followed by Buddhist (n=21, 6.15%). Maximum incidences occurred in rural regions (n=266, 78%). Road traffic accidents (n=205, 60.11%) comprised of the most of medico-legal cases, followed by fall (n=42, 12.31%) and assault (n=30, 8.79%). A maximum number of cases were noted in month of July (n=42, 12.31%) followed by October (n=40, 11.73%). When seasonal variation was considered, maximum incidences occurred during winter months of October to January (40.76%, n=139). Total 241 MLC cases (70.67%) were treated on OPD basis. Majority of MLC cases (n=269, 78.88%) were discharged after complete recovery. conclusion: It was observed that the majority of medico-legal cases were due to road traffic accidents. So the burden of these cases can be reduced by minimizing road traffic accidents. In view of this, appropriate measures are required. Casualty Medical Officers should be trained to handle such MLC cases and consult with the Department of Forensic Medicine for appropriate documentation of these cases.

However, different medical departments have different kinds of medical resources, and customizing the scheduling system for each department is not an effective approach. To solve this problem, we surveyed different medical departments in a (Medical Center in Southern Taiwan, summarize the needed, implemented a modularization Medical Scheduling System (MSS), for a Medical Center in Southern Taiwan, and resolve the complex appointment scheduling problem in a multi-clinic. The MSS improves work assignment effective and sensible utilization in medical device. Furthermore, it can be used for epidemic prevention and the control of Covid-19 pandemic. Based on the confirmed cases, we can identify potential contacts via MSS and contain the coronavirus.

Objective. The purpose of this study is to draw attention of dental medicine specialist’s team on risks they are exposed to, by modifying treatment plans in order to fit medical tourism programs. Material and method. In this study a questionnaire was applied to a number of 173 dentists with different specializations, regarding the adaption of treatment options for patients from medical tourism programs and risks posed by changes they had applied. Results. The analysis of results shows that the treatment approach for medical tourists is mostly based on a multidisciplinary team. In terms of quality of the treatment it is distinguished the use of specialists focused on specific fields of dentistry, but we must not neglect the fact that the therapeutical act is adapted to a reduced treatment time. Conclusions. For most of professionals of dental tourism, the financial aspect comes first and not the ethical aspect of medical care.

Changing doctor-patient relationship and commercialization of modern medical practice has affected medical practice. Objective: To assess the knowledge and attitudes towards medical ethics among doctors in a Teaching Medical College, Hospital. Study design: Observational descriptive study. Material and methods: A self-administered structured questionnaire was distributed to all doctors in a Teaching Medical College, Hospital, a total 110 practitioners of various clinical departments participated. Result: 90% of the total participants were well aware about medico legal cases. All the participants doing ethical practice out of them 12% practice as per the demand of the situation. Dichotomy should not be a part of medical practice expressed by 88% of participants. Awareness regarding MCI code of medical ethics 2002 shown by 52% of respondents and 57% knows the role of ethical committee in their institute. Majority, 88% of them expressed that 15 days duty in the department of Forensic Medicine should be mandatory during internship to handle the medicolegal cases properly.

Simulation is a synthetic representation of a real-world process with sufficient reliability to facilitate learning through contemplation and practice without the hazard, innate in a real-life experience. Nowadays, simulation is a useful accompaniment to medical education as pre-exposure to necessary clinical skills as exposure in the real clinical setting may be insufficient. Clinical skills and performance are considered core proficiency and are crucial to the professionals. This can enable the students to familiarize themselves with patient examination and hands-on- training by using models before coming across patients directly. Simulators are broadly classified into two broad categories: 1. High-fidelity Simulators and 2. Low-fidelity simulators. The fidelity of a simulator is decided by the extent to which it provides realism through characteristics. Simulation Based Medical Education (SBME) provides a safe environment for the students to acquire their psychomotor skill but is not necessarily better than other types of instruction as there is a high degree of variability between studies. SBME has been introduced in the health care field and now it is becoming one of the most popular teaching techniques for improving patient safety and care. Physiological rehabilitation is applied in medical practice to restore a person's functional ability after musculoskeletal injuries. Movements in the joints are the main functional indicator of the activity of the organs of support and movement. The paper considers a resistive goniometric sensor in the form of a ball joint. Based on such a sensor, a medical rehabilitation goniometric system is developed. It includes also a data collection device (autonomous data recorder) and a portable modem. That is why the system works autonomously in real-time and transmits data over a distance. This allows the system to be mobile and control joint mobility without the direct involvement of a doctor.
