Abstract:

The purpose of the project was to fund a medical respite program for homeless adults discharged from hospitals; too stabilized to remain in a hospital, and with recuperative needs too severe to remain on streets, or in a traditional shelter. This program will collaborate with other programs at the Weingart Center Association and local service providers to support homeless individuals. The Weingart Center Association, the hosting agency of this project, is located in Los Angeles County, California.

After conducting a review of the literature, the grant writer designed a medical respite program providing 24 hour shelter; access to care management; case management; access to medical professionals; nutrition needs; and psychosocial supports. After researching funding sources, the Ralph M. Parsons Foundation was determined to the most applicable match for funding of the project.

Actual submission and funding of the grant were not required for the successful completion of this project.

Abstract:
Incorporating spiritually-involved interventions into the treatment process for chronically ill patients is a trending subject in the field of medical social work. Literature suggests the integration of spirituality with patients diagnosed with a chronic medical illness appear to influence patient’s resiliency level. However, the lack of information and knowledge on the topic suggests medical social workers are still in the process of learning how to utilize spirituality as a form of intervention. This study explores how medical social workers in the Inland Empire perceive the practice of spirituality on resilience levels in adults diagnosed with a chronic medical illness. The data is collected through audio recordings from individual interviews with each participant. The audio recordings are transcribed into transcripts to identify themes and common categories among the participants. The results analyzed call for future research to continue on how to incorporate spirituality into the social work practice in the Inland Empire.

Abstract:
This thesis reports on an ethnography of a hospital social work team in Wales. The aim of this study was to explore the nature of the statutory social work role within hospitals, to examine how hospital social workers do their work, and to shed light on how social work fits into the hospital context. My findings indicate that hospital social workers face constant pressure from managers and clinicians to expedite patient discharges, and exclude almost all other tasks from their role. Their daily work is a sequence of bureaucratic tasks, focused on management of the failing body, often to the exclusion of considering the wider social or psychological needs of the patient. Drawing on the work of Bauman, I argue that the bureaucratic and managerial systems in which hospital social workers operate produce dehumanising practices and distance decision makers from the human consequences and moral dimensions of their decisions. Even within these systems, however, some levels of discretion are maintained and hospital social workers use their discretion in a variety of ways. The hospital social workers in this study consistently expressed values derived from anti-discriminatory practice and, despite the constraints they encountered, were able to perform work that showed a concern for social justice, human rights and empowerment at the individual’s level. Thus, I argue that hospital social work in the UK is driven by liberal, rather than radical values, and is largely unconcerned with addressing wider issues of structure, social disadvantage and oppression. The hospital social work role involves the co- ordination of knowledge provided by clinical professions, which must then be processed to match the needs of the patient to the services that are available. Social workers are outsiders within the hospital setting and there is a considerable amount of distrust between them and the clinical professionals, which occasionally manifests in open conflict. I draw on Goffman’s dramaturgical insights to analyse how social workers manage their position within the hospital and draw on his theory of frame analysis to understand the way conflicts arise. Hospital social workers maintain a distinct identity within the hospital that is tied to their liberal values. I argue that their practices can be interpreted both as arising from the zeitgeist of liquid modernity and as adapting to the human need brought about by liquid modernity. I suggest that social work must either pursue individual liberation further, following the liberal values currently underpinning these hospital social workers’ practice, or adopt a more radical or critical approach in seeking to influence government policies around social care.

Abstract:
Medical Social Workers are placed in fast pace environments with little time to process their feelings after being exposed to their client’s traumatic stories. The study assessed the implications of vicarious trauma on medical social workers and its impact on client outcomes. The current study utilizes a sample of 20 medical social workers employed in hospital settings across San Bernardino County. Social Work personnel were asked to take The Secondary Traumatic Stress Scale, which measurements are most congruent to the DSM-5 criteria of Secondary Traumatic Stress. To assess the relationship between Secondary Trauma and demographic characteristics of the social worker, an ANOVA and Post-Hoc test were conducted on SPSS to analyze data. This study will assist social workers in recognizing implications to their vicarious trauma and seek intervention before it manifests into their patient relationship. Policies that require social workers to be evaluated for vicarious traumatization would be advantageous and decrease turnover rates among medical social work.

Abstract:
This study explored the roles and responsibilities of social workers providing services to nonmedical prescription drug users (NMPDU). Researchers have indicated NMPDU disproportionately affects people living in rural communities. The overarching research question sought to explore the concerns of social workers when providing services to patients coping with NMPDU in the rural community of Ulster County, New York. The intention was to examine systemic challenges facing rural social workers when attempting to decrease morbidity risks and increase the health of Ulster County residents. A total of 7 social workers participated in 3 focus groups to explore their ideas for defining, clarifying, and identifying solutions to the problem. The social exchange theory was used to frame the roles and responsibilities of social workers within rural communities at the macro, mezzo, and micro levels. Qualitative content analysis identified 5 themes: roles and responsibilities, barriers, education, treatment interventions, NMPDU and illicit substances of use. The results of the study included advocating for the fair and equitable distribution of resources for all residents coping with NMPDU in Ulster County, their responsibility to collaborate on pressing matters and educate physicians, community service providers, local legislators, individuals, and families of the warning signs and harmful effects of NMPDU. Findings may effect social change by enhancing the role of social workers by reducing overdose and death rates of NMPDUs.

Abstract:
Traumatic death survivors are an at-risk population that could benefit from social work interventions. This action research study explored social work perspectives on crisis practice with traumatic death survivors in acute medical settings. The research questions focused on whether early crisis interventions by social workers would benefit traumatic death survivors, which interventions were recommended in crisis practice with this population, the potential effects of repeated trauma exposure on the social workers providing the crisis interventions, and recommendations to manage this professional exposure to trauma. The purpose of the study was to explore social work perspectives on potential benefits of crisis interventions with traumatic death survivors and potential effects of crisis practice on the social workers providing the services. Crisis theory and family systems theory informed the study. Participants were selected from a convenience sample of licensed social workers, and data were collected in 1 focus group and 3 in-depth interviews. Data were analyzed using descriptive and thematic analysis. Findings supported the benefits of social work crisis interventions for traumatic death survivors and the need for self-care, self-awareness, and support to reduce the risks of negative effects of crisis practice on social workers. Specific crisis interventions recommended for practice include the roles of advocate and guide, grief support, viewing of the body, and explanation of next steps. An increase in knowledge and recommendations for future practice based on study findings might promote positive social change by raising awareness about the problem and improving social work practice with traumatic death survivors.

Abstract:
ABSTRACT This study explored the challenges medical social workers face that lead to burnout. Currently, there is literature on burnout among health care providers and social workers, but not specifically on social workers in the medical field. The current study aimed to fill this gap in literature. Due to the lack of literature, the study used an exploratory, qualitative design. The study utilized individual interviews with a non-random purposive sample of nine medical social workers currently employed at Kaiser Permanente’s Fontana/Ontario Social Services Department. Interviews with participants were recorded and transcribed. Transcriptions were analyzed using thematic analysis. Major themes that emerged were organizational challenges, challenges working in multidisciplinary teams, working in the medical field, and limited resources. The study’s findings aim to increase awareness of the issue of burnout among medical social workers and to contribute to the implementation of interventions or policies within health care settings to prevent burnout among medical social workers.

Abstract:
The purpose of this study was to compare the attitudes of medical social workers (MSWS) and mental health social workers (MHSWs) toward mental illness. The sample consisted of 87 subjects, 56 MSWs and 31 NHSWs. The instrument utilized was the Opinions About Mental Illness Scale which measured five attitudinal factors: Authoritarianism, Benevolence, Mental Hygiene Ideology, Social Restrictiveness and Interpersonal Etiology. Data was analyzed, using Pearson’s r. No significant differences were found on the Authoritarianism dimension. However, MSWs scored higher on Benevolence, Mental Hygiene Ideology, Social Restrictiveness, and Interpersonal Etiology as compared to MHSWs. One of the most significant implications was that the clinical environment should be less physically and socially restrictive so that the patient would not feel as though s/he was in prison and had committed a crime.

Abstract:
Patients receiving dialysis treatment experience immense changes in their health and well-being, therefore receiving adequate social support services is an integral aspect of their care plan. Thus, the purpose of this study was to gain an understanding about social support services offered by social workers to patients receiving dialysis care. Furthermore, this study sought to develop awareness about additional services need to assist social workers in providing social, emotional, and physical support to patients. This qualitative study used data from a non-probability snowball sample of 10 social workers that work with patients receiving dialysis care. Findings of this study are significant to social work practice as the analysis provided insight that may enhance current practices in dialysis centers. Furthermore, the qualitative analyses gathered through the one-on-one interviews led to the emergence of seven central themes. Themes anticipated by the researchers included the perception about the ability to provide social, emotional, and physical support, as well as to highlight the role of social workers in this healthcare setting. Additional themes identified during this study focused on reasons for patients lacking social support, the scarcity of resources, and recommendations corroborated with first-hand experiences in the field to better serve patients. Recommendations included increased focus on providing therapeutic services within dialysis, additional transportation options for patients, and greater availability of resources to meet the various needs of patients.

This study explored the knowledge and perception between students that attended the Department of the Whitney M. Young Jr., School of Social Work and the remaining students that were attending the Atlanta University Centers in regards to attention deficit hyperactivity disorder (ADHD). This study analyzed the harms of medical treatment; it delineated the Afrocentric Perspective as well as the Theoretical Frameworks to describe how it could be correlated with the Afrocentric Perspective in regards to enhancing a social workers’ understanding of human behavior in the social environment as well as its relevance to attention deficit hyperactivity disorder (ADHD). The results from the study showed that there was no statistical significant relationship between students that attended the Department of the Whitney M. Young Jr., School of Social Work and the remaining students that were attending the Atlanta University Centers toward their knowledge and perception of attention deficit hyperactivity disorder (ADHD) and medical treatment among adolescents.

Abstract:
This thesis is about medical doctors with immigrant backgrounds who work in Sweden. Based on 15 qualitative interviews with medical doctors with immigrant backgrounds, this thesis explores the medical doctors’ feeling of professional belonging and boundary work. This thesis focuses mainly on the doctors’ experiences of being part of the Swedish medical profession while, at the same time, being regarded as ‘different’ from their Swedish medical counterparts. It starts off with the idea that medical doctors with immigrant backgrounds may have, or could be regarded as having, contradictory social positions. By virtue of being part of the Swedish medical profession, they belong to one of the most privileged groups in Swedish society. However, due to their immigrant background these doctors do not necessarily occupy a privileged position either within their profession or in society in general. This thesis shows that doctors with immigrant backgrounds feel that they are not perceived as full-fledged doctors, which seem related to how they are somewhat ‘othered'. The results show that these doctors cope with being seen as different from doctor with non-immigrant backgrounds, by using the notion of ‘migranthood’ as a resource in negotiations in everyday work life but they also do what they can to overcome the boundaries of ‘Swedishness’. Belonging should therefore be seen as having a formal and an informal side, as getting a Swedish license does not automatically mean that you feel belonging to, in this case, the Swedish medical profession. This seems to put doctors with immigrant backgrounds in a somewhat outsider within position, which seems having to do with boundaries between who is included in the ‘us’ and in the ‘them’. Lastly, these findings indicate that sociologists need to expand the understanding of professional groups to also include boundary work within these groups. In order to do so, this thesis argues that sociological theory on professional groups could be combined with sociological theory about social positions as that is one way to understand the outsider-within position that these doctors (and presumably other skilled migrants) have to cope with.

Abstract:
Undocumented immigrants have historically been excluded from the medical care system. This study explored the attitudes and experiences of undocumented Latino immigrants when seeking medical services in the San Bernardino County. It aimed to share the stories of undocumented immigrants that often go untold and provide valuable insight into the needs and concerns of this population. The exploratory study utilized a qualitative approach through interviews. Data was collected through in person and over the phone interviews, which were then transcribed and analyzed using a constant comparative method. The findings of this study identified barriers and challenges faced by undocumented immigrants including a lack of equal access to healthcare, a lack of knowledge in navigating the healthcare system, stigma and discrimination, language, finances, culture, and fear. Both negative and positive experiences were shared, along with advice provided by participants for undocumented immigrants. This study carried important implications for medical professionals, agencies, and policy makers to better address the needs and concerns of undocumented patients. Research recommendations included implementing programs based off this study’s findings and exploring the attitudes and experiences of this population when seeking mental health services. Further research is encouraged to work towards dismantling the barriers faced by the undocumented immigrant population.

Abstract:
This study examines the HIV/AIDS knowledge, attitudes and perceptions of medical social workers that provide services to older adult populations and the effects that may impact service delivery. It was hypothesized that medical social workers believe that they lack sufficient knowledge to provide social work services to older adults with HIV/ AIDS. The study sample consists of medical social workers who work with older adult patients and residents at a southeast Atlanta hospital and nursing home. The study reported the findings of a 23-item questionnaire that measured knowledge of HIV/AIDS older adult related issues, professional preparation and attitudes regarding sexuality and older adults. A descriptive approach was used to analyze the data and report significant findings. Findings showed that social workers were knowledgeable, yet lacked knowledge in critical areas. Overall social workers believed that they were not prepared to provide effective service delivery. This research is of importance because in order to enhance the quality of life for at risk older adults, social workers need to evaluate their level of knowledge, their attitudes and become aware of any gaps in knowledge or biases that may impede effective service delivery.

Abstract:
This study explored the concept of medical tourism and the subjective experiences of international patients seeking inpatient eating disorder (ED) treatment at a private inpatient ED clinic in South Africa. This research aimed to shed light on what patients with Anorexia Nervosa (AN) feel are essential features of a high-quality ED service at Akeso Montrose Manor specifically, and what they found to be concerning about ED treatments received prior, especially in the United Kingdom. The research aimed to explore the participant's reasons for seeking treatment at Akeso Montrose Manor, their expectations of treatment, their experience of treatment and their recommendations regarding the treatment of international patients. Ex-patients from the United Kingdom were chosen for this study, as a high number of admissions for treatment at Akeso Montrose Manor are from the United Kingdom. This therefore provoked an interest in exploring the reasons why these patients search for specialised out-of-country treatment. The research was conducted using a qualitative research design. Twenty participants were selected using purposive sampling by the researcher. They were interviewed face-to-face and via Skype using a semi-structured interview schedule. The data obtained was analysed using qualitative methods. The findings of this study highlighted several difficulties experienced in the United Kingdom when seeking treatment and also highlighted the value of residential inpatient treatment centres, extended treatment for ED's and the importance of specialised ED treatment teams and units. The results included that participants came to South Africa, due to challenges experienced in accessing treatment in the United Kingdom and treatment largely being dictated by weight and focused mostly on weight restoration. Expectations met during their stay in South Africa, included structure, safety and specialised ED treatment and staff and expectations not met whilst in treatment at Akeso Montrose Manor included limited time with the dietician and psychiatrist, and their belief that they would be fixed post-discharge. Participants made recommendations for the clinic with regards to the treatment of international clientele, which included more focus on post discharge planning and support. Further recommendations were made for improved service delivery at Akeso Montrose Manor as well as further research.

Abstract:
The Patient-Centered Medical Home (PCMH) is an innovative, team-based health care model that was applied during the implementation of the Affordable Care Act (ACA). However, the competencies for PCMH health care social worker team members are not identified within this model. Thus, the purpose of this phenomenological study focused on identifying the core competencies that will enable social workers to perform competently in PCMHs. This study also explored the roles and training needs as related to improving the competence of social workers. Sandberg's and Parry's conceptualization of the competency model was used as the theoretical framework. Data were acquired through interviews with 10 PCMH social workers. These data were then inductively coded and analyzed using a modified Moustakas method. Key findings indicated that these social workers believed that improvements in competencies may include training and knowledge with mental health and physical health knowledge which consist of diagnoses, interventions, medications, symptoms, and terminology. It was also noted that knowledge of evidence-based practices for mental health interventions and patient-centered, team-based principles were essential to ACA policy implementation. The positive social change implications of this study include recommendations to health care leadership, educational institutions, and other PCMH providers to develop competency-based training for social workers. Recommendations are also put forth to adapt social work curriculum to ensure the effective implementation of the principles of the ACA policy and to improve social work practice in PCMH health care settings.

Therefore an attempt was made to determine the psycho-social characteristics of patients referred to the Rehabilitation Clinic. Furthermore an attempt was made to determine the role and task of the medical social worker in relation to these psycho-social characteristics. The research method was a descriptive one, using archival data; the latter being social work records of interviews with patients. Prior to the research a literature study was done on various aspects of physical disability and vocational rehabilitation. A sample group of 45 subjects was compiled by a random sampling technique. Two structured schedules were used to extract data from both the social work file and the Rehabilitation Clinic patient file. The research and processing of the information was done manually. The findings revealed the following 1. The majority of the physically disabled patients were Coloured males in the age category of 31-50 years. 2. These patients had poor educational qualifications and were previously unskilled or semi-skilled labourers. 3. The patients were unemployed and had no income. 4. These patients had a passive lifestyle with no hobbies or little religious involvement. They also habitually abused liquor and/or cannabis. 5. The housing conditions of these patients were inadequate and primitive. 6. The various roles of the medical social worker included those of rehabilitation counsellor, team member, co-ordinator/resource manager and teacher, whereas the tasks included assessment, dissemination of information, referrals, co-ordinating of services and education. The final conclusion in this study was that the role and task of medical social worker was influenced by both the psycho-social characteristics of referred patients as well as the expectations of the rehabilitation team of the Rehabilitation Clinic.

This thesis explores the discourses and organisations through which girls' development towards adult womanhood was framed and managed during the inter-war period. It examines how contemporary perceptions of social change following the First World War resulted in widespread scrutiny of girls' circumstances and behaviour, particularly their sexual conduct. It argues that representations and responses to girls were increasingly underpinned by the conceptualisation of adolescence as a critical period of change and instability. This understanding of adolescence pervaded both medical and lay discourses. It was interpreted through the prisms of gender and class, and served to legitimise increasing levels of intervention into girls' lives, mainly on the basis of their sexual behaviour or perceived exposure to sexual risk. Adolescence was also represented as the period in which individuals developed moral agency. This study examines the increased importance ascribed to the moral training of the adolescent, in the context of widespread agreement of the need to express traditional moral values in ways that took account of social change. This was seen as particularly important for girls, not only because of their changing circumstances, but because of women's new status as enfranchised citizens. The thesis explores the work of the Girl Guides Association and the Young Women's Christian Association in some detail. These organisations drew upon the discourse of social change, adolescence and citizenship to claim an enhanced role in shaping the development of young women. While histories of girls' youth organisations have tended to portray them as conservative movements intent on socialising girls into their future role as wives and mothers, this study highlights these organisations' commitment to preparing girls to understand and exercise their future responsibilities as citizens, and argues that such organisations were more complex in their purposes, and more varied in their approaches, than has previously been recognised.

This research explored social worker’s perception of the services received among patients labeled with a diagnosis or labeled negatively, such as non-compliant, in comparison to non-labeled patients in a medical setting. Data for this project were gathered through seven in person interviews with social workers. The participants were all social workers in a medical setting at different DaVita Dialysis centers throughout San Bernardino County. The participants experience and education level ranged from master level social work interns to licensed clinical social workers. The findings indicated that the social workers do believe patients with diagnoses do not receive the same level of care as patients without a diagnosis or label.

The overall objective of this study was to determine the level of satisfaction with medical treatment and social work counseling among women diagnosed with Acquired Immune Deficiency Syndrome (AIDS). The author examined the following factors: First, how quickly and frequently the female respondents received medical treatment for symptoms related to a diagnosis of AIDS. Second, the type and form of medical insurance coverage provided for females diagnosed with AIDS. Third, the level of the respondents’ satisfaction with the medical treatment received for the treatment of AIDS. Fourth, the type of social work counseling received for the treatment of AIDS. Fifth, the respondents’ level of satisfaction with the social work counseling received for the treatment of AIDS. A self-administered questionnaire was given to thirty adult female respondents that have been diagnosed with AIDS. Two hypotheses were tested concerning the relationship between gender and the level of satisfaction with medical and social work treatment received for the treatment of AIDS. Both of the null hypotheses were rejected. The results of this study indicated that the female respondents were dissatisfied with the counseling they received from the social worker, with a felt sense of having their concerns inadequately addressed during counseling sessions. This study demonstrated that social workers must be sensitive to gender specific issues with special populations of clients in order to provide adequate, acceptable treatment.

ENGLISH ABSTRACT: Medical social workers working in children's wards are more prone to burnout because of the stressful nature of their work. Because social work requires emotional involvement, it is not uncommon for workers to be emotionally drained and frustrated, which might lead to burnout. Stress amongst social workers is an important issue, as a stressed workforce can negatively affect the quality of service. This study was undertaken with the aim of providing guidelines for handling stress amongst medical social workers working in children's wards, in order to promote job satisfaction and enhance work performance. Chronically ill children who are hospitalised, often suffer from behavioural and emotional problems that require social work intervention, which in turn puts pressure on the social worker. Concurrent stressors on the children and their families may exhaust their normal coping capacity and further complicate the child's adaptation to hospitalisation. The role of the medical social worker is to provide social support to the child and the family in order to promote positive coping in the child during and after hospital treatment. The demands put on the social worker by hospitalised children can cause stress, which could adversely affect their work performance and job satisfaction. Social workers therefore, have to carefully select an appropriate practice framework when dealing with hospitalised children. This entails the selection of appropriate perspectives, theories and models to ensure effective intervention. These frameworks provide structure and directive to social work intervention, and combined with supervision, can promote job satisfaction amongst medical social workers. The study was undertaken amongst medical social workers working in paediatric wards within hospitals in the Cape Metropolitan area. The empirical study focused on an investigation of factors, which might contribute to the experience of burnout amongst the social workers. Work-related stressors playa major role in increasing the likelihood of burnout amongst social workers. If these stressors are not properly dealt with they might lead to burnout amongst medical social workers, which will have a direct bearing on the child's benefit from medical services. A combination of both organisationally based and individually based strategies should be implemented in order to overcome job-related stress and to prevent burnout.
AFRIKAANSE OPSOMMING: Mediese maatskaplike werkers wat in kindersale werk, is meer geneig tot uitbranding as gevolg van die stresvolle aard van hulle werk. Maatskaplike werk vereis emosionele betrokkenheid. Dit is dus nie ongewoon vir maatskaplike werkers om emosioneel gedreineerd en gefrustreerd te voel nie. Laasgenoemde is kenmerke van uitbranding. Oormatige stres by maatskaplike werkers kan 'n negatiewe uitwerking op die kwaliteit van hul dienslewering hê. Hierdie navorsingstudie was onderneem met die oog op die daarstelling van riglyne vir die hantering van stres by mediese maatskaplike werkers, wat in kindersale werk om sodoende werkstevredenheid en -verrigting te bevorder. Kroniese siek kinders wat gehospitaliseer word, presenteer dikwels met gedrags- en emosionele probleme wat uiteraard tydens intervensie addisionele druk op die maatskaplike werker plaas. Gepaardgaande stressors kan daartoe lei dat pasiënte en hul gesinne afwyk van hul normale funksionering wat weer die pasiënt se hanteringsvermoë en aanpassing by die hospitaalopset belemmer. Die rol van die mediese maatskaplike werker is om sosiale ondersteuning aan bogenoemde partye te gee ten einde positiewe hanteringsvermoë tydens en na mediese behandeling, by pasiënte te vestig. Die eise wat deur pasiënte aan maatskaplike werkers gestel word kan stres veroorsaak wat hul werksverrigtinge en tevredenheid negatief kan affekteer. Maatskaplike werkers moet dus versigtig te werk gaan in die keuse van 'n gepaste praktykraamwerk vir intervensie ten opsigte van gehospitaliseerde kinders. 'n Verantwoordbare keuse van perspektiewe, teorieë en modelle lê ten grondslag van effektiewe maatskaplike werk intervensie. Hierdie raamwerke bied struktuur en rigting aan maatskaplike werk intervensie. Laasgenoemde, tesame met Stellenbosch University http://scholar.sun.ac.za superviste kan werkstevredenheid onder mediese maatskaplike werkers bevorder. Stressors wat met werk verband hou, speel 'n groot rol in die toename van uitbranding onder mediese maatskaplike werkers. Indien hierdie stressors nie effektief hanteer word nie, kan dit tot uitbranding by maatskaplike werkers ly wat dan weer die pasiënt se benutting van mediese dienste negatief beïnvloed. Die ondersoekgroep het bestaan uit tien mediese maatskaplike werkers wat werksaam was in paediatriese sale in hospitale binne die Kaapse Metropool. Tydens 'n empiriese ondersoek is gefokus op faktore wat moontlik aanleiding kan gee tot die uitbranding van maatskaplike werkers. Dit word dus aanbeveel dat 'n kombinasie van strategieë van beide die orgarusaste en die persoon geïmplementeer moet word om werksverwante stres te oorkom en dus uitbranding by mediese maatskaplike werkers te voorkom.

It is generally accepted that spirituality can play an important role in the end stage of the life of a person. Spirituality can provide a source of comfort and guidance and in so doing be a coping strategy. Social workers should take into account this spiritual component of their work particularly with clients who use spirituality as a form of social support. This thesis provides a theoretical understanding of spiritual based practice. It identifies the principles and competencies central to this practice, including the need for worker self awareness. It then illustrates how certain social work students have applied these principles in their practice with dying clients.
The author's own identification as an individual from a religiously oriented Muslim country is used to illustrate the importance of understanding and working from the world view of clients. The author concludes that spirituality should not be ignored by social workers in their practice.

ENGLISH ABSTRACT: The aim of this study was to compile practical guidelines to assist medical practitioners in utilizing social work services in their practices. Exploratory, descriptive and applied research was done to determine and describe medical practitioners' knowledge of psychosocial problems and their present and potential utilization of social work services. Results were generalized in respect of the population as a whole. In the theoretical study social functioning was conceptualized by the description of micro, mezzo and macro levels of functioning. The approach towards rendering of services (in the Medical field) and intervention (in the Social Work field) was described, followed by recommendations for the eclectical utilization of the approaches. The eclectical application of the intervention approaches requires that social workers should have certain knowledge, values and skills to interpret certain roles. This occurs within certain social work areas, with the achievement of the aims and functions of Social Work as goal. The ethical and value basis of Social Work and Medical Science shows certain similarities and disparities. The values of Social Work, as contained in the Behavioural- and Ethical Code for Social Work and the Oath of Hippocrates, in Medical Science, are described in this study. In the empirical study qualitative and quantitative information was gathered, inter alia, about the existence of psychosocial problems of patients in medical practitioners' practices, how medical practitioners handled them, how well equipped they were to do it, their present and potential utilization of Social Work services, the role of third parties in the rendering of services and referral procedures. Thirty nine (39) medical practitioners, selected by random sampling procedure, were involved in the study and completed selfadministered questionnaires. The majority respondents indicated that their undergraduate training was inadequate in enabling them to handle psychosocial problems of patients. The entire spectrum of psychosocial problems existed in all the respondents' practices. The majority of respondents handled such problems themselves, while specialists, psychologists and psychiatrists were at times used as resources. The majority of respondents indicated that they could perhaps utilize Social Work services and that there is a great need for such a resource. Most medical practitioners preferred to send written referrals via patients, with the responsibility being with the patient for making an appointment with the social worker. Minimal use was made of third parties in the patients' frame of reference in the assessment and treatment of patients. Third parties, within the reference framework of patients, were used minimally by medical practitioners in the making of assessments and the rendering of services. Respondents had divergent opinions regarding their competence in the psychosocial field of service rendering. The majority of respondents indicated the need for training programs focusing on the handling of patients' psychosocial problems. Limited time for attending such training programs was however mentioned as the biggest obstacle. These findings of the empirical study, together with the theoretical framework, were used as a guideline for the composition of a practical guideline for the referral of services to social workers in private practice.

Verkennende, beskrywende en toegepaste navorsing is gedoen, ten einde geneeshere se kennis van psigososiale probleme en hul huidige en potensiele benutting van maatskaplikewerkdienste te bepaal en te beskryf. Bevindinge is ten opsigte van die populasie as geheel veralgemeen. In die teoretiese studie is maatskaplike funksionering deur die beskrywing van mikro-, meso- en makrovlakfunksionering, gekonseptualiseer. Die benaderings tot dienslewering (in Geneeskunde) en intervensie (in Maatskaplike Werk) is beskryf. Daarna is 'n aanbeveling vir die eklektiese benutting van die benaderings gedoen. Die eklektiese benutting van die intervensiebenaderings vereis dat maatskaplike werkers oor sekere kennis, waardes en vaardighede moet beskik en sekere rolle moet vertolk. Dit geskied binne sekere maatskaplikewerkvelde en het as mikpunt die verwesenliking van die doelstellings en funksies van Maatskaplike Werk. Die etiese en waardegrondslag van Maatskaplike Werk en Geneeskunde toon sekere ooreenkomste en verskille. Die meeste respondente het aangetoon dat hulle moontlik van maatskaplike werkers gebruik kan maak en dat daar 'n groot behoefte aan sodanige hulpbron bestaan. Die verwysingsprosedure wat deur die meeste geneeshere verkies is, was die stuur van skriftelike verwysings saam met pasiente. Derde partye, in pasiente se verwysingsraamwerk, is minimaal deur geneeshere by assessering en dienslewering betrek. Geneeshere het uiteenlopende menings gehad oor hul bevoegdhede in die psigososiale veld van dienslewering. Die behoefte aan opleidingsprogramme, wat fokus op die hantering van psigososiale probleme by pasiente, is deur die meeste respondente aangedui. Beperkte tyd is egter as die grootste struikelblok vir die bywoning van sodanige opleidingsessies aangevoer. Hierdie bevindinge van die empiriese studie, saam met die teoretiese raamwerk, is as riglyn benut vir die ontwikkeling van 'n praktykriglyn vir verwysing van dienste na maatskaplike werkers in privaat praktyk.

Abstract:
The Patient-Centered Medical Home (PCMH) is an innovative, team-based health care model that was applied during the implementation of the Affordable Care Act (ACA). However, the competencies for PCMH health care social worker team members are not identified within this model. Thus, the purpose of this phenomenological study focused on identifying the core competencies that will enable social workers to perform competently in PCMHs. This study also explored the roles and training needs as related to improving the competence of social workers. Sandberg’s and Parry’s conceptualization of the competency model was used as the theoretical framework. Data were acquired through interviews with 10 PCMH social workers. These data were then inductively coded and analyzed using a modified Moustakas method. Key findings indicated that these social workers believed that improvements in competencies may include training and knowledge with mental health and physical health knowledge which consist of diagnoses, interventions, medications, symptoms, and terminology. It was also noted that knowledge of evidence-based practices for mental health interventions and patient-centered, team-based principles were essential to ACA policy implementation. The positive social change implications of this study include recommendations to health care leadership, educational institutions, and other PCMH providers to develop competency-based training for social workers. Recommendations are also put forth to adapt social work curriculum to ensure the effective implementation of the principles of the ACA policy and to improve social work practice in PCMH health care settings.

Cultural bias in health assessment has been asserted in various scientific reports. The aim of this study was to determine whether ethnicity effects how patients are represented in such medical certificates that are needed to obtain a certain Swedish social insurance. The setting was an analysis of 18 medical certificates for patients whose parents were born in Sweden, respectively of 18 medical certificates of patients whose parents immigrated to Sweden from non European countries. The main objective was comparison of the two groups to test for ethnical heteronormative representations of the patients and their parents. The method used is mainly based on critical discourse analysis in combination with Fowlers (1996) semantic tools used to decipher disparity in the discourse. The text analysis is combined with theories of Said (1978), Johansson (1993) and de los Reyes (2006) regarding social representations of the ethnically non European “The Others”. Results of the study show that presentations of the two ethnical groups generally do differ both when it comes to representations of the patient as well as of the parent. Substantial differences are found in descriptions of patient aggression and asocial behaviour. The results render the conclusion that ethnical heteronormative representations can be found even in such documents that have the purpose of objectively certifying individual circumstances, conditions and difficulties.

In this article, the authors present the Willingness to Foster Scale - Disabilities and Medical Conditions (WFS-DMC) and report results concerning its psychometric properties. The WFS-DMC is a new measure designed to accurately and efficiently assess the willingness of parents to foster children with special needs, in particular, disabilities and special medical conditions. The authors tested the WFS-DMC with a national sample of 298 foster mothers. Internal consistency reliability was excellent (α =. 90). With reference to construct validity, mothers with higher WFS-DMC scores fostered longer, fostered and adopted more children, and requested the removal of a smaller proportion of foster children. Furthermore, the mothers' WFS-DMC scores were unrelated to demographic characteristics. The WFS-DMC could help guide the decision-making process involved in matching children who have special needs with parents willing to care for them.

Abstract:
The premise for this research initially stemmed from a perceived crisis facing the provision of the nuclear medicine service within the United Kingdom, the possible impact posed by the shortage of nuclear medicine clinicians and the untapped potential of a body of non-medical healthcare practitioners working within the nuclear medicine sector to whom recognised additional roles, such as reporting of images, may sensibly be delegated. Yet, despite the support by various professional bodies and colleges, uptake is not widespread and appears to be ill provided for in terms of educational programmes. From an educational perspective, with ever advancing technology and the ubiquity of web based resources, eLearning within healthcare is still in its infancy. Certainly its ability and flexibility to reach geographically diverse populations of learners, is undisputed, yet whilst advantageous to the professional leaner in accessing material away from a restricted campus based environment, its efficacy to teach a skill, or competence, and indeed to translate this to clinical practice remains largely unproven. With both these issue in mind, the project question was posed as to whether it was possible to establish the efficacy and credibility of an eLearning resource to prepare and support the training of non-medical healthcare practitioners working within the field of nuclear medicine in reporting of bone scans.
