Abstract:
The aim of this research is to find out the role of the secretary in receiving leadership guests at the Medan Health Facilities Security Center (BPFK) Office. The techniques used by researchers are the observation method (Observation), the interview method (Interview), the library method (Library Research). In general, guests who come to the Health Facilities Security Hall are official guests from hospitals to take care of medical equipment or medical equipment certificates. Therefore, the role of the secretary is very important in receiving guests before meeting with the leadership. The results of the research are that the secretary plays an important role in helping the management's tasks run smoothly. If the secretary is not there then the work will be hampered in terms of energy, thought and time resulting in reduced work efficiency.

Law can greatly facilitate responses to public health emergencies, including naturally-occurring infectious disease outbreaks and intentional or accidental exposures to chemical, biological, radiological, or nuclear (CBRN) agents. At the federal level, the Secretary of the Department of Health and Human Services (HHS), as the lead for federal public health and medical responses to public health emergencies and incidents, has a range of authorities to support federal, state, tribal, local, and territorial responses. For example, under the Public Health Service (PHS) Act, the Secretary may provide temporary assistance to States to meet health emergency needs or deploy medicine and supplies from the Strategic National Stockpile. The Secretary also may determine that a disease or disorder presents a public health emergency, which may be the first step in triggering other critical legal authorities for response.Since the 2001 anthrax attacks, one focus of public health preparedness has been on developing, distributing, and rapidly dispensing medical countermeasures (MCMs) for CBRN emergencies and pandemics.

John Foster Dulles was the United States Secretary of State during the administration of President Dwight D Eisenhower. At the height of the Cold War, Dulles was Eisenhower’s emissary, traveling over 450,000 international miles, leading United States foreign policy. In November of 1956, during an international crisis involving the Suez Canal, Dulles became ill and underwent an operation for a perforated colon cancer. During much of his impactful term as Secretary of State, Dulles was being treated for this cancer that ultimately resulted in his death in May of 1959. This paper highlights the medical care of John Foster Dulles and the global events during his illness.

Abstract:
Purpose Ireland’s physicians have a legal and an ethical duty to protect confidential patient information. Most healthcare records in Ireland remain paper based, so the purpose of this paper is to: assess the protection afforded to paper records; log highest risk records; note the variations that occurred during the working week; and observe the varying protection that occurred when staff, students and public members were present. Design/methodology/approach A customised audit tool was created using Sphinx software. Data were collected for three months. All wards included in the study were visited once during four discrete time periods across the working week. The medical records trolley’s location was noted and total unattended medical records, total unattended nursing records, total unattended patient lists and when nursing personnel, medical students, public and a ward secretary were visibly present were recorded. Findings During 84 occasions when the authors visited wards, unattended medical records were identified on 33 per cent of occasions, 49 per cent were found during weekend visiting hours and just 4 per cent were found during morning rounds. The unattended medical records belonged to patients admitted to a medical specialty in 73 per cent of cases and a surgical specialty in 27 per cent. Medical records were found unattended in the nurses’ station with much greater frequency when the ward secretary was off duty. Unattended nursing records were identified on 67 per cent of occasions the authors visited the ward and were most commonly found unattended in groups of six or more. Practical implications This study is a timely reminder that confidential patient information is at risk from inappropriate disclosure in the hospital. There are few context-specific standards for data protection to guide healthcare professionals, particularly paper records. Nursing records are left unattended with twice the frequency of medical records and are found unattended in greater numbers than medical records. Protection is strongest when ward secretaries are on duty. Over-reliance on vigilant ward secretaries could represent a threat to confidential patient information. Originality/value While other studies identified data protection as an issue, this study assesses how data security varies inside and outside conventional working hours. It provides a rationale and an impetus for specific changes across the whole working week. By identifying the on-duty ward secretary’s favourable effect on medical record security, it highlights the need for alternative arrangements when the ward secretary is off duty. Data were collected prospectively in real time, giving a more accurate healthcare record security snapshot in each data collection point.

Abstract:
Non-Secretory Multiple Myeloma (NSMM) is characterized by typical morphological and pathological multiple myeloma (MM) characteristics and the absence of an M-protein on immunofixation electrophoresis with estimated prevalence of 3%. Among the NSMM cases there is a subset in which no cytoplasmic Immunoglobulin synthesis is detected, and this entity is called ‘’Non-Producing’’ Multiple Myeloma (NPMM). Plasma cell leukemia (PCL) is an aggressive form of MM characterized by high levels of abnormal plasma cells circulating in the peripheral blood. We present a rare case of non-producing variant of PCL. 75-year-old male was admitted due to anemia and thrombocytopenia. His CBC revealed hemoglobin of 9.0 g/dl and platelets were 9 k/ul. CMP showed creatinine of 1.34 mg/dl, total protein of 6 g/dl, albumin 3.6 g/dl and corrected calcium was normal. LDH was 204 IU/L. Peripheral smear review showed 8% circulating atypical plasmacytoid cells, normochromic normocytic anemia and thrombocytopenia. SPEP showed no monoclonal protein. IgA was normal. IgG, IgM were low 315 mg/dl and 20 mg/dl respectively. Serum beta-2 microglobulin was high (5.5, 1.1 – 2.4 mg/dl). Serum free kappa light chain was low (0.15, 0.33-1.94 mg/dl), lambda light chain and ratio was normal. Skeletal survey showed possible lytic lesions in right femur neck and subtrochanteric left femur. Bone marrow biopsy showed plasma cell myeloma involving 90-95% of bone marrow cellularity. The plasma cells show morphologic heterogeneity with prominent immature, plasmablastic and pleomorphic morphology. Flow cytometry shows a dominant abnormal CD45-dim population with expression of CD38, CD138, CD56 and CD117 (partial). The abnormal cells are negative for cytoplasmic kappa and lambda immunoglobulin light chains and negative for myeloid and lymphoid markers (by flow cytometry and immunohistochemical stains). Complex chromosomal analysis. Plasma cell FISH studies was positive for t(11;14). Based on suggested revised diagnostic criteria for PCL from outcomes of patients at mayo clinic, our patient was diagnosed with plasma cell leukemia. Given aggressive biology of this disease, he was started on VD-PACE chemotherapy. Bone marrow biopsy after cycle 1 chemotherapy showed no morphologic, immunophenotypic or flow cytometric features of a plasma cell neoplasm. Given excellent treatment response and discussion with transplant center subsequent cycle 2 was changed to Velcade, Revlimid and low-dose dexamethasone. He is scheduled for stem cell transplant later this month. Primary plasma cell leukemia (pPCL) is the most aggressive form of the plasma cell dyscrasias. The outcome of pPCL has improved with the introduction of autologous stem cell transplantation and combination approaches with novel agents, including bortezomib and immunomodulatory drugs, such as lenalidomide. This case highlights the challenges in diagnosis of non-producer primary plasma cell leukemia.

Abstract:
Group X secretory phospholipase A2 (GX sPLA2) hydrolyzes membrane phospholipids producing free fatty acids and lysophospholipids. Previous studies from our lab suggest that mice with targeted deletion of GX sPLA2 (GX KO) have increased age-related weight gain due to an increase in overall adiposity. Paradoxically, this increased adiposity is associated with improved age-related glucose intolerance. GX KO mice also demonstrate a reduced inflammatory response to lipopolysaccharide injection. In vitro studies indicate this phenotype may be attributable to blunted macrophage mediated inflammatory responses. Given the role of macrophages in promoting adipose tissue (AT) inflammation and metabolic dysfunction in response to diet-induced obesity, we hypothesized that GX KO mice would be protected from the obesity related metabolic derangements associated with overfeeding. Unexpectedly, GX KO mice were only partially protected from high fat (HFD) diet-induced glucose intolerance and showed no improvement in HFD-induced insulin resistance. Moreover, GX KO mice were not protected against HFD-induced AT inflammation. GX sPLA2 is produced as a proenzyme (pro-GX sPLA2), and propeptide cleavage is required for enzymatic activity. Furin-like proprotein convertases (PCs) have recently been implicated in the proteolytic activation of pro-GX sPLA2; however the identity of individual PCs involved is unclear. Previous findings from our lab have shown that GX sPLA2 is expressed in the adrenals where it regulates glucocorticoid production. GX KO mice have increased plasma corticosterone levels under both basal and ACTH-induced stress conditions. However, how GX sPLA2 is regulated in the adrenals is still uncertain. We hypothesized that PCs may be involved in the proteolytic activation of pro-GX sPLA2 in the adrenals. Here we report the novel findings that the PCs, furin and PCSK6, proteolytically activate pro-GX sPLA2 in Y1 adrenal cells. Furthermore, we demonstrate that PC dependent processing of pro-GX sPLA2 is necessary for GX sPLA2 dependent suppression of steroidogenesis. Finally, we provide evidence that pro-GX sPLA2 processing by PCs is enhanced in response to adrenocorticotropic hormone (ACTH), suggesting a novel mechanism for negatively regulating adrenal steroidogenesis. Cumulatively, these studies provide valuable insight into the function and regulation of GX sPLA2.

Abstract:
Rotavirus gastroenteritis is a preventable public health concern, and diarrhoeal deaths persist in low-income settings. Rotavirus is prevalent in children under five years, despite widespread vaccination. Rotavirus P[8] vaccines display lowered efficacy (30-50%) in low-income settings compared to high-income (~80%). Disparities in vaccine efficacy are multifactorial but may be influenced by host factors. Understanding the interplay between the host, pathogen infection, and gut microbes may allow for targeted interventions against diarrhoeal disease. The FUT2 secretor gene encodes the availability of human histo-blood group antigens (HBGAs) in the gut, and the VP4 spike of rotavirus displayed binding affinity in a strain-specific manner to HBGAs. Secretor status may, therefore, influence rotavirus susceptibility, strain distribution, and partially contribute to varied vaccine efficacy. Furthermore, gut microbiomes may be shaped by HBGAs, influencing gut health and the subsequent risk of diarrhoeal disease. The aim of this study was to investigate secretor status, rotavirus VP4 genotypes, and gut microbiomes in South African children to elucidate the interplay between these factors. Methods for determining secretor status were evaluated since unique strengths and limitations exist that should be tailored to study design and population. Genotyping of the FUT2 gene targeting a known non-secretor mutation proved to be limited in genetically diverse participants. Phenotyping methods inaccurately classified children potentially due to an underdeveloped secretor phenotype that is masked by maternal antibodies introduced via breastmilk. Both methods yielded comparable yet distinct information about the cohort, and the study aim is essential to consider prior to method choice. In this study, South African secretors were significantly more susceptible to rotavirus infection, while non-secretors displayed a natural resistance. Rotavirus susceptibility was also strain-specific, where secretors were susceptible to predominantly circulating P[8] and P[4] strains, while less prevalent rotavirus P[6] strains were more common in non-secretors. Our findings indicate that secretor status is an important mediator of VP4 strain-specific rotavirus susceptibility in the South African population. Non-secretors in a population could modulate the circulation of wild-type rotavirus and contribute to lowered vaccine efficacy. In our study, non-secretors made up ~30% of the population. Higher frequencies of non-secretors in a population could reduce P[8] and P[4] rotavirus circulation, causing increased prevalence of P[6] strains with phylogenetically different VP4 genes. P[6] rotavirus strains are relatively common in South Africa, which contrasts to regions where non-secretors and P[6] strains are uncommon. Non-secretors may also respond poorly to rotavirus P[8] vaccines, and although their resistance to wild-type P[8] strains negates the need for a strong P[8] response, it may be important in areas where rare rotavirus strains are prevalent. Investigating the gut microbiome of healthy children did not show that microbial composition was influenced by secretor status. However, this pilot study provided valuable insights into microbiome methods and analysis pipelines. The gut microbiome is an emerging field of study, with advancements in sequencing technologies making the field more accessible to young scientists. The fight against diarrhoeal disease will require comprehensive insight into the gut environment.

Abstract:
It is unknown why allergic symptoms do not develop in all sensitized children. We analyzed prospectively the postnatal secretory IgA (SIgA) development and whether high SIgA levels would protect sensitized infants from developing allergic symptoms. Salivary total IgA and SIgA levels were determined by ELISA, and allergy development was investigated at 3, 6, and 12 mo and at 2 and 5 y in two birth cohorts in Estonia (n = 110) and Sweden (n = 91), two geographically adjacent countries with different living conditions and allergy incidence. Total and SIgA levels increased with age, reaching adult levels at the age of 5. Virtually, all salivary IgA in Estonian children was in the secretory form, while a major part of IgA in Swedish saliva lacked the secretory component up to 2 y of age. In Sweden, high levels of salivary IgA without secretory component correlated inversely with house dust endotoxin levels. High SIgA levels were associated with less development of allergic symptoms in sensitized Swedish children. In conclusion, postnatal maturation of the salivary SIgA system proceeds markedly slower in Swedish than Estonian children, possibly as a consequence of low microbial pressure. SIgA may limit allergy-mediated tissue damage at mucosal surfaces in sensitized individuals.

Abstract:
The heterogeneity of salivary gland neoplasms have made classification and prognostication of these tumours sometimes difficult, and the in­troduction of techniques, such as enzyme and carbohydrate histochemis­try and electron microscopy have only to a certain extent increased our knowledge in these respects. In the present study immunohistochemical methods have been used to identify intermediate filament proteins (IFP) in normal fetal and adult parotid glands, as well as in salivary neo­plasms. The intermediate filaments (IF) make up the cytoskeleton in eucaryotic cells. Epithelial tissue contains IF composed of different cytokeratins (CK 1-19) whilst mesenchymal tissue generally contains IF composed of vimentin, and the IFP pattern is very stable even during cell transformation. It would thus be possible to further clarify the histogenesis of salivary neoplasms by identifying IFP, in addition the IFP pattern would probably be useful in tumour typing. Furthermore, ultrastructural cytochemical studies, microspectorphotometry on nuclear DNA as well as enzyme secretory studies of certain tumour types were carried out, in order to further characterize the biology of salivary neoplasms. The immunohistochemical investigations showed that in normal parotid tissue, the different cell types differed in IFP expression: acinar cells express mainly CK 18 and myoepithelial cells mainly CK 17 and 19, whilst duct cells contained a broad range of CK. Vimentin could in ad­dition to CK be detected in myoepithelial cells and basal cells of ex­cretory ducts. Fetal parotid cells showed a similar CK pattern as mature duct cells. In addition, vimentin could be found in some basal cells of the terminal tubules of the fetal glands. Salivary neoplasms could be divided into three types with regard to their IFP pattern:  Acinic cell carcinomas showed a CK-pattern similar to normal acinar cells but a co-expression of CK and vimentin was present in some cells.  Adenoid cystic carcinomas, mixed tumours and basal cell adenomas showed a CK-pattern of normal duct or myoepithelial cells. The peri­pheral cells were also vimentin positive. 3. Mucoepidermoid carcinomas and adenocarcinomas had a similar CK-pattern as duct cells, and no tu­mour cells contained vimentin. This indicates that typing of IFP may be useful for subgrouping of salivary neoplasms. By stereological measurements, the cells of acinic cell carcinomas were found to be very similar to normal parotid acinar cells. Furthermore, they contained amylase and after stimulation by norepiphrine a secre­tory response was induced, with a rise in intracellular cAMP as well as a release of amylase. By single cell measurements of nuclear DNA con­tent, no difference was found between acinic cell carcinomas with de­finite metastasis and those without recurrence, both in paraffin sec­tions and cytological smears.

Abstract:

Picropodophyllin (PPP) is an analogue of the anti tumour lignan podophyllotoxin with the unique ability to selectively inhibit the receptor of Insulin like growth factor 1(IGF-1). IGF-1 is believed to play an important part in development of the endometrium facing implantation. With PPP treated mice, studies can be made to measure gene expression from tissue of both treated and untreated mice to compare the role of IGF-1 regarding ovulation, implantation and receptivity. The aim of this study was to analyze gene expression of some steroid hormone receptors and cytokines in ovaries from mice treated with PPP. In this study, seven mice were treated with PPP at different times and tissue was collected. PCR-primers for cDNA sequences of estrogene receptor α, estrogene receptor β, progesterone receptor A, progesterone receptor B, growth hormone receptor, interleukin 1 α, interleukin 1 β, tumour necrosis factor α and androgen receptor were used. Real Time PCR was run with the samples and gene expression was measured. The results of this study showed that the inhibition of IGF-1 receptor interacted with IGF-1 which lead to altered levels of estrogene receptor alpha, progesterone receptor, growth hormone receptor and androgen receptor that can decrease ovulation. The results also showed the differences in gene products between treated and untreated samples, suggesting that IGF-1 plays an important role regarding ovulation.


Studier med hjälp av den selektiva insulinlika tillväxtfaktor 1 receptorn (IGF-1R) antagonisten; picropodof?phyllin (PPP), hur samspelet mellan livmoderslemhinnan och implantationsprocessen, samt hur ovulationen påverkas av insulinlika tillväxtfaktorn 1 (IGF-1) kan nu utföras. IGF-1 tros ha en viktig roll för den reproduktiva processen, där den påverkar ovulation, implantation och embryoutveckling. IGF-familjen består av tre ligander; insulin, IGF-1 och IGF-2. IGF transporteras bundet till bindarprotein (IGFBP). Medlemmarna i IGF receptorfamiljen kan binda IGF-1, IGF-2 och insulin fast med olika affinitet. PPP som är en cykloligan, är en analog från podofyllotoxin och fungerar som en syntetisk IGF-1 receptorantagonist, som selektivt inhiberar receptorns aktivitet. PPP tros även kunna nedreglera genexpression av receptorn. Tre tidigare projektarbeten har utförts på vävnader från möss injicerade med PPP. Tyngdpunkterna i dessa arbeten har legat på immunhistokemiska studier av IGF-1 i reproduktionsorgan från möss, uttryck av IGF-1, dess receptor och bindarprotein 1 i ovarier och uterus efter behandling med PPP. I denna studie användes vävnad samt cDNA från sju möss behandlade med PPP, i olika stadier av reproduktionen samt även icke behandlade möss. Studiens syfte var att med sanntids-PCR jämföra genuttryck från östrogenreceptor α och β, progesteronreceptor A och B, tillväxthormonreceptor, Interleukin 1 α och β, ’tumor necrosis’ faktor α samt androgenreceptor i vävnad från PPP-behandlade och obehandlade möss och genom de erhållna resultaten från ovarievävnaden utläsa effekten på ovulationen och från uterusvävnaden effekten på implantation och receptivitet. O estudo das faltas e licenças médicas pode tornar-se referencial privilegiado para observar-se a conjuntura na qual se edificam as diretrizes e os produtos que se constituirão nas ações educativas para jovens e crianças que dependem da Secretaria da Educação do Estado de São Paulo. Para medir o absenteísmo no trabalho relacionado a motivos de saúde optou-se em descrever a magnitude média das taxas de faltas e de licenças médicas do magistério e do pessoal de apoio, segundo as 91 diretorias de ensino de todo o Estado de São Paulo, entre maio de 2008 e julho de 2009 e comparar as proporções de faltas e de licenças entre as funções do magistério e de apoio, segundo coordenadorias de ensino. Trata-se de um estudo ecológico que utilizou em sua construção, dados secundários, coletados a partir de registros oficiais. Verificou-se que a região periférica da metrópole paulistana foi a que apresentou um número superior de ausências, em detrimento do interior e da capital, que tiveram registradas médias aproximadas. As licenças médicas, contudo, têm, principalmente no interior, significativa elevação. Este trabalho pôde registrar que aproximadamente 20 por cento da força de trabalho contratada esteve ausente durante o período letivo observado, sendo o tempo fator importante para a predição do afastamento das pessoas do trabalho. Identifica-se que, em janeiro, quase não há afastamentos por motivo de saúde e que ocorre aumento progressivo ao longo do semestre estudado. Em junho/julho, inverte-se essa tendência, que se modifica novamente em agosto, o que sugere um padrão de repetição (ciclo). Nítida é a elevação de ausências por motivo de saúde no período definido como letivo
The study of absences and sick leave can become privileged reference point for observing the situation in which the guidelines are built and the products that will serve on the educational activities for adolescents and children who depend on the Bureau of Education of the State of São Paulo. Measuring absenteeism in work related to medical reasons was decided to describe the magnitude of the average rates of absenteeism and medical leave from teaching and support staff, according to the 91 boards of education throughout the State of São Paulo between May 2008 and July 2009 and compare the proportion of absences and licenses among the functions of teaching and support, according to coordinators of education. This is one ecological study that used in its construction, the secondary data collected from official records. It was found that the peripheral region of the metropolis was the one with a higher number of absences at the expense of the interior and the capital, who had recorded approximate average. Sick leave, however, have mainly within, a significant increase. This work could report that approximately 20 per cent of the workforce employed was absent during the school year observed, the time factor is important for predicting the removal of people from work. We found that in January, there was almost no leaves due to health reasons and that it had increased progressively throughout the semester studied. In June / July, that trend reversed itself, which was modified again in August, which suggested a pattern of repetition (cycle). Um estudo epidemiológico da prevalência da infecção por Candida sp. em mulheres não grávidas foi realizado em uma população do Norte do Brasil (Belém-Pará, 2002). Este estudo teve como objetivo principal contribuir para esclarecer os mecanismos de infecção por Candida sp e sua possível associação com os antígenos de grupos sangüíneos ABO e Lewis e estado secretor de substâncias ABH. Tal estudo compreendeu um total de 165 mulheres, atendidas no Ambulatório do Hospital da Polícia Militar e Laboratório de Análises Clínicas do centro de Ciências Biológicas da Universidade Federal do Pará, que procuraram essas unidades para a realização de exames ginecológicos, das quais foram coletadas amostras de sangue, saliva e muco vaginal. A presença da infecção por Candida sp foi feita através do exame a fresco de secreção vaginal e bacterioscopia da secreção vaginal. A identificação dos fenótipos ABH e Lewis no sangue foi determinada pelos testes de hemaglutinação direta e na saliva pelo dot-blot Elisa. Aplicou-se um questionário padrão para obtenção das informações epidemiológicas. A prevalência da infecção por Candida sp foi de 47,9%. Dentre os sintomas mais comumente associados à infecção por Candida sp, destacaram-se o prurido e corrimento. Mulheres com idade inferior a 40 anos, que não faziam uso de preservativo e que já tiveram infecções anteriores apresentaram um maior risco de infecção vulvovaginal por Candida sp. Mulheres infectadas e não infectadas por Candida sp tiveram distribuição similares para os fenótipos de grupos sangüíneos ABO, Lewis e estado secretor de substâncias ABH. E na comparação da expressão dos antígenos Lewis no sangue, saliva e muco vaginal, verificou-se que ambas as secreções expressavam antígenos Lewis sem qualquer estreita correlação dos fenótipos eritrocitários Lewis. Neste contexto, considera-se que a natureza da diversidade genética na interação entre hospedeiro e Candida sp requer estudos complementares envolvendo a identificação de diferentes cepas para determinar estas prováveis associações entre os fenótipos de grupo sangüíneos ABH e Lewis com candidíase.
Na epidemiologic study of Candida sp infection prevalence in non-pregnant women was performed in a population of the north of Brazil (Belem-Pa, 2002). This paper aims to contribute to clear up the infection mechanisms of Candida sp and its possible association to ABO and Lewis group blood antigens and ABH substances secretory status. Such paper comprehended a total of 165 women admitted at the out-patient clinic of "Hospital da Policia Militar" and "Laboratorio de Analises Clinicas do Centro de Ciencias Biologicas da Universidade Federal do Para", who came to those units to undergo gynaecologic examinations, where blood, salive and vaginal mucus samples were taken. Candida sp infection presence was determined through the examination of fresh vaginal secretion and bacterioscopy of vaginal secretion. The identification of ABH and Lewis phenotipes in the blood was determined through direct hemaglutination tests and, in the salive, by Elisa Dot-blot. It was performed a standard form for the obtaining of epidemiologic information. Candida sp prevalation was 47.9%. Among the most commom signals and symptoms associed to Candida sp infections are pruritus and gleet. Women under 40 who did not use condoms and who have already had previous infections presented a higher risk of vulvo-vaginal infection caused by Candida sp. Infected and non-infected women with Candida sp had similar distribuition to the phenotypes of ABO, Lewis blood groups and ABH substances secretory status. And, comparing the expressions of Lewis antigens in the blood, save and vaginal mucus, it was verified that both secretions expressed Lewis antigens without any close relationship to erithrocitic Lewis phenotypes. In such context, it is considered that the nature of genetics diversity in the interaction between Candida sp and host requires more studies involving the identification of different types to determine their probable associations among ABH and Lewis blood groups phenotypes with cadidiasis.

Resumo: Saúde é um direito social conquistado, e a efetivação deste direito só se cumpre com o enfrentamento dos seus dois principais entraves: financiamento em saúde e gestão dos recursos humanos em saúde. O Brasil tem lidado com a questão dos recursos humanos em saúde através de programas governamentais que têm por finalidade o provimento de profissionais e a melhoria da qualidade da formação destes. O PROVAB ¿ Programa de Valorização do Profissional da Atenção Básica e o Programa Mais Médicos são os dois mais recentes programas na área. O presente trabalho objetiva refletir e analisar, a partir da experiência do Sujeito-Pesquisador como Supervisor do PROVAB e do Programa Mais Médicos, sobre o papel da supervisão prática nestes programas, e também sobre suas potencialidades e limites como ferramenta pedagógica. Trata-se de um estudo de natureza qualitativa e exploratória, baseado em relatos de experiências do Sujeito-Pesquisador, ocorridas no período de 2013 a 2015. Realiza-se um estudo descritivo dos programas, em seguida são relatadas as experiências selecionadas de supervisão, e por fim realiza-se a construção de categorias de análise. São delimitados três grupos de categorias: supervisão em contexto, papel da supervisão e ferramentas de supervisão. A pesquisa encontra seus limites na contemporaneidade aos fatos, que dificultam o encontro de dados e fontes distintas às fornecidas pelo governo federal, além da própria escolha metodológica, que remete os achados ao olhar implicado do autor. Com este trabalho, verifica-se que a prática de supervisão no PROVAB e no Programa Mais Médicos tem grande potência pedagógica, e que uma melhor definição do papel do supervisor, assim como a incorporação de novas ferramentas, tal qual o Grupo Balint-Paidéia e a Supervisão Ampliada, são algumas das propostas para o avanço dessas políticas. Novos estudos são necessários, para ampliar o olhar dos profissionais, gestores, equipe, usuários sobre os programas e, verificar as mudanças ocorridas, e a influência/potência da supervisão nestas mudanças
Abstract: Health is a conquered social right, and the fulfillment of this right only occurs with the confrontation of its two main obstacles: financing and management of the health workforce. Brazil deals with the issue of human resources for health through government programs to provide professionals and improve the quality of training. The PROVAB - "Professional Enhancement Program Primary Care" (Professional Improvement Program Primary Care) and (More Medical Program) "More Doctors Program" are the two most recent programs in this field. This article aims to reflect and analyze, from the experience of the subject-researcher as a Supervisor on the role of practical supervision in these programs on their potential and limits as a pedagogical tool. This is a study of qualitative and exploratory nature, based on reports of subject-researcher experience, during the period from 2013 to 2015. The author develops a descriptive study of the programs and reports selected supervisory experiences, and finally performs the construction of analysis categories, organized into three groups: supervision in the context, the role of supervision and supervision tools. The research finds its limits in the contemporaneity to the facts, making it difficult to collect different data and sources provided only by the federal government, also in from the specific methodological choice, referred the findings to the implicated author's look. With this work, it seems that the practice in supervision on the PROVAB "More Doctors Program" has great educational power, and that a better definition of the role of the supervisor as well as the incorporation of new tools, such as Balint-Paideia¿s Groups and the "enhanced supervision" are some of the proposals for the advancement of these policies. More studies are needed to broaden the perspectives of professionals, managers, team members about the programs and verify the changes occurred, and the influence / power of the supervision in these changes

AbstractAzoospermia is defined as the absence of sperm in the ejaculate. All etiologies can be categorized in secretory and obstructive azoospermia. Some conditions of obstructive azoospermia are suitable of surgical recanalization to restore natural fertility. Conversely, in cases of nonobstructive etiologies, untreatable obstructive azoospermia, and when female factor is present, sperm retrieval techniques for intracytoplasmatic sperm injection (ICSI) represent the only therapeutic option to reach paternity. The role of the clinical andrologist is to offer to the couple the best treatment option: medical or surgical treatment or sperm retrieval for ICSI.

In the summer of 1634 the grand-ducal ambassador resident in France Giovan Battista Gondi made an undercover journey to Spanish Netherlands to meet Maria de’ Medici and tried to persuade her to leave the Habsburg territories for Florence. Despite the failure of the negotiations, a series of unpublished letters exchanged between Gondi and the first secretary of the Grand Duchy Andrea Cioli will serve as an opportunity to analyze which cultural and artistic affairs involved the Tuscan agent alongside the political events he primarily dealt with. In his letters Gondi described different artefacts that could have been acquired for the Medici collection, and publications that could have contributed to the reputation of the Grand Duke or, on the contrary, jeopardized it.

Abstract In this chapter I will examine the éloges and funeral orations of Etienne Pariset, the first permanent secretary of the Academy of Medicine, serving from 1823 until his death in 1847. My intent is to ascertain through these writings the values and self images of the medical elite during the first half of the nineteenth century. I do not claim that Pariset’s depictions of medicine and its practitioners are necessarily “true” in any literal sense. I merely suggest that they constitute one representation among many of the medical elite of this era. This representation, however, was uniquely influential because of its thematic coherence and because it was reiterated regularly by the authorized representative of the most prestigious medical institution in France.

Instead of a long and detailed account of the wide variety of cases, issues, procedures, and the like, the Secretary gives only a terse statement of the highlights of the year’s work. Most of you who attend these annual meetings have closely followed the work during the year and are already familiar with most of the cases reported. We attempt, therefore, to spare you the necessity of listening to an over-long account. A committee numbering among its members some of the most distinguished medical authorities of America has at the invitation of the Association made a detailed study of the difficulties in Harlem Hospital, the only tax-supported institution in the country which has to so large an extent a mixed staff. The inquiry was broadened to include the whole field of medical opportunity for Negro doctors and nurses. The committee’s report of more than 80,000 words establishes a new yardstick in the matter of opportunity for service in tax-supported hospitals and in medical schools which, because of the distinction of the committee making the report, is of nation-wide significance. Two of the recommendations which will have most effect are for the ending of discrimination against Negroes in medical schools and of denial of opportunity to serve on the staffs of tax-supported hospitals through out the country.

This Article also applies to a person who seeks leave to enter the United Kingdom for a purpose not covered by the immigration rules or otherwise on the grounds that those rules should be departed from in their case. In relation to the giving or refusing of leave to enter by the Secretary of State under Article 2, paragraphs 2 (examination by immigration officers, and medical examination), 4 (information and documents), 7(1), (3), and (4) (power to require medical examination after entry), 8 (removal of persons refused leave to enter), and 9 (removal of illegal entrants) of Schedule 2 to the Immigration Act 1971 shall be read as if references to an immigration officer included references to the Secretary of State.

Introduction: Secretory carcinoma of the breast is a rare disease, accounting for approximately 0.15% of breast cancer cases. This entity was first described in a child in 1966. However, one of the largest case series with SEER data, encompassing a total of 190 patients, showed that the median age at diagnosis was 56 years, and it can affect both sexes, being much more common in women. In this same series, 58% and 40% of patients were positive for estrogen and progesterone hormone receptors, respectively. Most cases (86.86%) were well to moderately differentiated tumors without lymph node involvement. Older patients had a worse prognosis. In general, the secretory breast carcinoma has a more indolent course with excellent prognosis. The treatment is based mainly on surgery, followed by radiation therapy. The role of chemotherapy and hormone therapy in these cases is not yet well established in the literature. Parallel to basal-like breast cancer, the indolent clinical course as well as prolonged survival seems opposite to that of common triple-negative breast cancer. In most cases of secretory breast carcinoma, reaching a 92% positivity rate, there is a fusion of the ETV6-NTRK3 genes, activating aberrant cell proliferation pathways. Studies with NTRK inhibitors are being developed and will bring this therapeutic possibility soon. Due to the rarity of secretory carcinoma of the breast, notably in men, we report the case of a young man with this neoplasm. A.S.R., 20 years old, male, from Guaraciaba do Norte (CE), white, single, telecommunications technician, reported that he noticed a painless, small, stable nodule in his left breast in 2012. He did not seek medical help at the time. In June 2021, the patient suffered trauma to her left breast during a soccer match. After this event, she noticed a considerable growth of a nodule in the left breast, which became painful to palpation. He then sought medical attention in a health center and underwent an ultrasonography of the left breast in August 2021, which detected a hypoechoic nodular image, oval, with well-defined limits, and regular contours, measuring 16.1×9.6×13.7 cm, 1.8 mm away from the skin, without vascularization inside the nodule (CATEGORY: BIRADS 3); little amount of stromal and glandular tissue were observed. A core biopsy of the nodule, performed in August 26, 2022, showed atypical epithelial proliferation. Immunohistochemistry was compatible with hypersecretory atypical epithelial proliferation. After the diagnosis of secretory carcinoma of the breast, the patient was referred to the Haroldo Juaçaba Hospital, a reference hospital in oncologic treatment in the North/Northeast of Brazil, where he underwent a slide review and immunohistochemistry, which confirmed invasive carcinoma of the secretory type of breast. Staging CT scans and bone scintigraphy were performed in September 2021. Chest CT showed a nodule with irregular contours and contrast medium concentration in the left breast, in close contact with the retropectoral musculature, measuring 19×10×12 mm, in addition to adenomegaly in the right axillary region (levels I and II), measuring up to 38×27 mm. There were no other relevant findings, with no evidence of secondary disease in the abdomen and bones. Investigation of right axillary adenomegaly with core biopsy continued and was negative for neoplasia. Histological picture and immunohistochemical profile were compatible with mixed lymphoid hyperplasia, follicular, and interfollicular. In December 28, 2022, the patient underwent a left mastectomy with sentinel lymph node biopsy, which revealed a secretory invasive carcinoma, measuring 1.9×1.5 cm, grade I, lymphovascular invasion, and negative margins, no lymph nodes were involved, nipple with compromised dermis and intraductal extension. Pathological staging: pT1c pN0 (sn-). The patient is currently on adjuvant systemic treatment (chemotherapy) with good tolerance.

Background: Platelet activity plays a role in the occurrence of diabetic angiopathy with an increase in mean platelet volume (MPV) as a marker of platelet activity. Platelet activity is influenced by phospholipase A2 type IIA (sPLA2 type IIA), which is a lipid-mediating enzyme that connects the pathogenesis of Diabetes Mellitus (DM) with complications of diabetic angiopathy. This study aimed to examine the relationship between levels of type IIA sPLA2 and MPV among type II DM patients. Subjects and Method: This was a cross-sectional study. A total of 63 patients with type II DM was selected for this study. The inclusion criteria for the study subjects were type 2 diabetes mellitus patients who did not experience an infectious disease, acute inflammation, trauma, surgery or malignancy, anemia, taking antiplatelet drugs, having abnormal platelet counts, and smoking. The dependent variable was levels of type IIA sPLA2. The independent variable was MPV. The data were obtained from the medical records of Prof. Dr. Soerojo Mental Hospital, Magelang. The data were analyzed using Spearman correlation test. Results: The study showed the median level of sPLA2 type IIA was 3841.50 ng / dL and the average MPV value was 7.36 fl. The results of the Spearman correlation analysis showed that there was no relationship between sPLA2 type IIA and MPV (p = 0.551), but there was a tendency for an increase in type IIA sPLA2 followed by an increase in MPV value (r = 0.077). There was a difference in the average MPV value in the subject group with DM ≤ 10 years and> 10 years (p = 0.009), and it was statistically significant. Conclusion: There is a tendency for an increase in type IIA sPLA2 followed by an increase in the MPV value among type II DM patients. Abstract: Pneumococci (Streptococcus pneumoniae) are one of the leading causes of morbidity and mortality among people over 60 years of age and workers in some professional groups. According to the medical literature, the frequency of invasive forms of pneumococcal infection among people of working age is 3.8 per 100,000 population. Increased susceptibility to colonization of the respiratory tract and subsequent morbidity may be due to concomitant pathology, exposure to immunocompromising, including harmful production factors. It should be noted that the source of the pathogen is not only sick people, but also healthy carriers. The level of asymptomatic colonization in the adult population is 5-7%, and in families with children increases to 30%. Vaccination is a way to effectively prevent respiratory diseases caused by this infection. The purpose of our study is to substantiate immunological indications for the formation of immunocompromised groups among workers exposed to the aerogenic factor at work for subsequent vaccination against pneumococcal infection. Results: It has been shown that low bactericidal activity of neutrophils (NBT-test) and a high level of secretory immunoglobulin can be used as a marker of immunodeficiency in workers of a ferrous metallurgy enterprise. When a doctor assesses the immune status of workers, he needs to take into account the presence of diseases that are part of the groups of immunological syndrome complexes (infectious-inflammatory, autoimmune, allergic, immunoproliferative) and the composition of industrial aerosols

Introdução: A sífilis é uma doença infectocontagiosa que vem apresentando crescimento significativo no país na última década. Pode ser dividida em primária, secundária, terciária e congênita, em que cada apresentação possui sintomatologia específica. A avaliação neonatal de sífilis é realizada após o nascimento, aplicando-se critérios pré-estabelecidos que auxiliam na realização de diagnóstico e seguimento do tratamento. Esses, definidos como critérios de cura, possuem diferenças quando comparados com os definidos pelo Ministério da Saúde — MS (2020) e pela Secretaria Municipal de Saúde do Rio de Janeiro — SMS-RJ (2016), podendo levar a desfechos diferentes. Objetivos: Comparar as diretrizes atuais de rastreio de sífilis congênita por meio do protocolo adotado pelo MS e pela SMS-RJ e seus possíveis desfechos na incidência e na prevalência. Material e Métodos: Foi realizada revisão por meio de artigos em PubMed, MEDLINE e periódicos. Resultados e Conclusão: Os protocolos de rastreio da sífilis congênita do MS e da SMS-RJ possuem pontos de divergência no que consta a avaliação de valores de Venereal Disease Research Laboratory (VDRL) e do risco de reinfecção por parceiros. O município não estipula redução dos valores de VDRL no neonato, aceitando aumento de sua titulação em até uma vez. A medida vai de encontro ao critério usado pela Federação, que prevê redução deles, abrindo possibilidade para o subtratamento do paciente. Em recente atualização, o MS se aproxima do protocolo da SMS e retira obrigação ao rastreio do parceiro, incentivando avaliação do risco de reinfecção para cada caso. Tal medida flexibiliza o rastreio e possibilita aumento das chances de reinfecção, prejudicando o controle da disseminação. Tal fato é corroborado por dados recentes que indicam prevalência de sífilis no município com proporção de 1,5 homem para 1 mulher (Sistema de Informação de Agravos de Notificação — SINAN). No período entre 2015 e 2018 houve um aumento nos casos de sífilis gestacional, de 2.500 para 4.353, com discreta redução nos casos de sífilis congênita, de 1.410 para 1.136 (Secretaria de Vigilância em Saúde do Rio de Janeiro). A discordância protocolar, no que diz respeito ao controle de cura e rastreio, aumenta o risco de reinfecção, além de complicações gestacionais atuais e futuras. A omissão do rastreio do parceiro acaba por favorecer a permanência elevada em sua incidência e prevalência na comunidade. Inferindo-se a presença de uma falha nas estratégias secundárias e a necessidade de uma ação da SMS-RJ para conter a epidemia em curso, tem-se como medida a revisão e a atualização do protocolo oficial utilizado ou a troca para o mesmo em uso nos hospitais federais, associado ao controle mais rígido dos contactantes sexuais na tentativa de reduzir a disseminação e a reinfecção.

Dehydroepiandrosterone sulphate (DHEAS) is a major secretory product of the human adrenal gland. Its precise functions are uncertain, but it has been postulated as a discriminator of life expectancy and aging. We have previously reported significantly lower plasma levels of DHEAS pre- and postoperatively in men developing DVT after major abdominal surgery, as compared to patients who remained free of DVT. Recent data suggest that the DHEAS concentration is also independently and inversely related to death from cardiovascular disease in men over 50 years of age. We here report data on preoperative plasma concentrations of DHEAS (measured with RIA) in 96 patients over 40 years of age, who underwent major abdominal surgery, and correlated the result to the development of postoperative DVT as diagnosed by the 125I-fibrinogen uptake test.Thirty patients (31%) developed postoperative DVT during the first ten postoperative days. The plasma levels of DHEAS were significantly lower in the patients with postoperative DVT compared to those without (median 1.95 umol/1; Q1−Q3, 1.30−3.00 vs. median 3.35 umol/1; Q1−Q3 1.60−4.70; p < 0.02). Ninety per cent of the patients who developed postoperative DVT had a DHEAS value lower than 3.9 umol/1, while 45 % of the patients who remained free of DVT had a DHEAS value higher than 3.9 umol/1.In conclusion, the data suggest that a low preoperative value of DHEAS predisposes the development of postoperative DVT in patients undergoing major abdominal surgery. It seems like the DHEAS concentration in plasma can be a valuable factor in a predictive index for postoperative DVT in those patients.

A logística de materiais nas organizações tem sofrido um significativo processo de aprimoramento e desenvolvimento de suas aplicabilidades nos processos de gestão de materiais, portanto é importante para aprimorar os processos internos das empresas, assim como minimizar os desperdícios e erros na seqüência das operações. O objetivo dessa pesquisa foi contribuir para a otimização da gestão de materiais nas empresas de pequeno porte, a fim de melhorar seus processos internos. Para a elaboração dos diagnósticos realizou-se um estudo de caso, na sede administrativa da empresa ASEFE-Associação dos Servidores da Secretaria de Educação, por meio de questionários e entrevistas com o gerente geral e com o coordenador de logística. Além disso, foram realizadas observações nos setores de compras, recebimento, armazenagem e expedição para se complementarem as informações recolhidas. Através da análise das informações obtidas identificou-se alguns pontos positivos e negativos no processo de gestão de materiais. Em seguida, sugere-se medidas mitigatórias para estancar os pontos negativos identificados e outras que promovessem resultados positivos a um custo menor. A gestão de materiais representa uma questão de interesse na gestão de operações que, pela aplicação da curva ABC, pode reduzir ou promover a flexibilidade do ambiente produtivo, e por conseqüência, impactar os resultados globais da organização. Finalmente, é salientada a importância da gestão contínua dos materiais, envolvendo a segmentação da gestão nos diferentes quadrantes da matriz e utilizando um mecanismo de indicadores de desempenho adotadas pela empresa ,

O presente trabalho busca compreender as relações entre o documento “Melhores práticas em escolas de ensino médio no Brasil”, uma parceria do Banco Interamericano de Desenvolvimento (BID) com a Secretaria de Educação Básica do MEC, e a Reforma do Ensino Médio iniciada pela Medida Provisória 746/2016, hoje a Lei 13.415/2017. Para tanto utilizamos a citada Lei e cinco peças publicitárias veiculadas no YouTube pelo Ministério da Educação (MEC). A análise foi feita com base no Materialismo Histórico-Dialético, pois entendemos que a Reforma e o documento só podem ser compreendidas se forem levadas em conta as condições materiais em que foram produzidas e na qual se efetivam. A análise das propagandas nos permitiu ver que seus principais pontos de defesa convergem com o defendido pelo documento do BID. Especialmente, quanto a flexibilização curricular, posta em prática na Reforma por meio dos itinerários formativos. O documento argumenta que o Ensino Médio Brasileiro é “conteudista”, fragmentado e sem sentido. As propagandas defendem que agora a aprendizagem se tornará mais estimulante e compatível com a realidade porque os estudantes poderão escolher o que estudar baseados em seus desejos para o futuro. As propagandas buscam dar a Reforma um caráter democrático e estabelecer como um consenso a necessidade da sua implementação.

Avaliar o desempenho energético operacional é importante para conhecer o uso de energia nas edificações e proporcionar estratégias de eficiência energética baseadas na realidade. Este artigo apresenta uma análise do desempenho energético de um estoque de escolas com base nas faturas de energia, área construída e ocupação. Os dados foram fornecidos pela Secretaria Estadual de Educação de Santa Catarina, e as análises foram divididas em escolas com fornecimento de energia em baixa tensão e alta tensão. As escolas em baixa tensão foram analisadas com base nos indicadores de intensidade de uso de energia em função da área construída e número de alunos (kWh/m².ano e kWh/aluno.ano). Com base nos quartis do estoque, foram determinados os desempenhos típicos, máximos e mínimos. Nas escolas em alta tensão, analisou-se a diferença entre demanda contratada e medida, consumo na ponta e fora da ponta, e relacionou-se o consumo com as informações de área construída e número de alunos. Além disso, percebeu-se que um ajuste na contratação de demanda pode proporcionar economia financeira significativa nas escolas em alta tensão. Por fim, concluiu-se que há a necessidade de um método que considere aspectos subjetivos na avaliação comparativa de desempenho energético de escolas

Introdução: Recentemente, ações desenvolvidas pela USP contribuíram para que a universidade atingisse alto padrão de qualidade. As diversas medidas e programas instituídos exigem a modernização dos instrumentos de gestão. O Estatuto e o Regimento Geral são instrumentos que regulamentam a finalidade, estrutura, competências e atribuições dos órgãos da Universidade, possibilitando atender ao princípio da legalidade e promovendo agilidade/transparência à gestão. Objetivo: Incorporar as alterações e emendas realizadas no Estatuto e no Regimento Geral da USP, bem como identificar dispositivos contraditórios, tornaram-se incorretos ou irrelevantes frente às alterações ocorridas durante os anos. Ainda, de acordo com a experiência do grupo, sugerir modificações para aprimorar a aplicação das normas no cotidiano universitário, sobretudo quanto às eleições e ao funcionamento dos órgãos colegiados, além dos concursos da carreira docente. Metodologia: Foi instituído um grupo de trabalho composto por assistentes acadêmicos, um Analista da Procuradoria Geral e três servidores da Secretaria Geral. O RG será revisado por artigos, que receberão as consolidações necessárias e as propostas de modificações que sejam condizentes com a realidade atual da USP. A partir da análise do RG, se necessário, propor adequações no Estatuto, a fim de evitar descompasso entre as normas. Resultados: A revisão está em andamento, com previsão de término para 13.11.2023, quando pretendemos tramitar o novo Regimento nas instâncias competentes da Universidade, até sua aprovação pelo Conselho Universitário, em 2024. Conclusão: O Regimento modernizado, além de ficar mais objetivo, garantirá aos usuários o suprimento efetivo de suas demandas, contribuindo para as boas práticas na Universidade.

Esta nota técnica presenta un panorama de las estrategias empresariales utilizadas para reducir las brechas de género en Panamá, utilizando los datos provenientes de la Herramienta Empresarial de Género (conocida como herramienta WEPs), una Encuesta de Medidas de Cuidado (EMC), y las Buenas Prácticas para la Igualdad de Género que ha recolectado la Secretaría Técnica de la Iniciativa de Paridad de Género (IPG) de Panamá. Los resultados indican que las empresas panameñas analizadas reconocen su rol para promover la igualdad de género y tomar medidas concretas para introducir políticas y prácticas, pero todavía se encuentran en vías de lograr un enfoque estratégico para implementar su compromiso, así como para medir e informar sobre su avance. Asimismo, las empresas encuestadas reconocen los beneficios de incorporar acciones de promoción de cuidados para sus organizaciones y 30% de ellas afirman estar interesadas en implementar medidas de corresponsabilidad o cuidado en el corto plazo, y 60% en el largo plazo. Este documento recomienda medidas a ser implementadas por la IPG, las empresas y el Gobierno de Panamá para fortalecer el compromiso con la equidad y cerrar brechas.

Introducción: El 07 de junio de 2021 la Secretaría de Salud de Cúcuta notificó la presencia de casos con sintomatología respiratoria en el centro carcelario de Cúcuta, de inmediato se inició el abordaje para determinar posible brote de COVID-19. Se caracterizó el brote por SARS-CoV-2, se identificó la cadena de transmisión y los factores de riesgo para establecer las medidas de prevención y control. Metodología: se realizó la caracterización en tiempo, persona y lugar. Las fuentes de información fueron la base de reporte del Sivigila de casos positivos, base de caracterización de conglomerados, casuística del centro penitenciario. Se realizó Búsqueda Activa Institucional (BAI) y Comunitaria (BAC) en patios, revisión de registros y de fichas de notificación. La tabulación de los datos se hizo con Microsoft Excel 2007 y el análisis de la información en EpiInfo. Se calcularon tasas de ataque por patio; identificación de factores de riesgo y de hacinamiento en la cárcel. Resultados: se confirmaron 79 casos de COVID-19: 65 en internos, 11 en personal de custodia, y 3 administrativos de INPEC. La tasa de ataque más alta se identificó en población con antecedente de permiso con el 100 %, seguida de la parte administrativa con el 70 %, y en los patios el UME reclusorio de las mujeres con el 40 %. La letalidad fue el 2,5 %. El nivel de significancia del 5 %, se encuentran diferencias (p<0,05) entre el sexo y la ocupación (administrativos, cuerpo de custodia, PPL). El suministro de agua, hacinamiento y las medidas de protección personal no eran suficientes. Conclusiones: se confirmó brote de COVID-19 en la cárcel, y se controló la situación con la implementación de acciones preventivas, tanto a nivel individual como colectivo. Se recomienda mantener comunicación entre la autoridad de salud y el personal del INPEC, con el fin de dar respuesta a situaciones de emergencia en salud pública.

La presentación expone las funciones de la Organización de los Estados Americanos (OEA) en torno a la seguridad. La OEA actúa en diplomacia preventiva, manejo de crisis y gestión posterior al conflicto con un mismo objetivo: defender el sistema democrático; afianzar las conquistas democráticas que se realizan en los países y fortalecer sus instituciones representativas, procurando siempre la solución pacífica de los conflictos. El tema de las medidas de fomento de la confianza y la seguridad en la agenda de la Organización de los Estados Americanos se introdujo en 1993, cuando la Asamblea General de la Organización aprobó una resolución en el sentido de iniciar un proceso de consultas sobre seguridad hemisférica. Esta presentación fue preparada para el Foro Sobre Convivencia y Seguridad Ciudadana en el Istmo Centroamericano, Haití y República Dominicana, que tuvo lugar en San Salvador, el 4 de junio de 1998.

Abstract:
Introducción: en Colombia, para controlar el SARS-CoV-2, se desarrolló la estrategia PRASS (pruebas, rastreo, y aislamiento selectivo sostenible), que busca detectar casos, aislarlos, controlar la cadena y velocidad de transmisión del virus. El objetivo de esta investigación es analizar las características sociodemográficas y clínicas de los casos positivos detectados en la búsqueda activa comunitaria para COVID-19 en la Isla de San Andrés. Materiales y métodos: estudio transversal descriptivo, realizado entre el 4 al 29 de septiembre del 2020. Para la recolección de información se hizo BAC (Búsqueda Activa Comunitaria), teniendo en cuenta los criterios establecidos para la definición de caso probable para COVID-19, en los 13 barrios priorizados. Se realizaron análisis descriptivos de las características sociodemográficas y clínicas de los casos utilizando Microsoft Excel®, Access, el software Epi- info 7.2. Resultados: en la BAC se notificaron por RT-PCR y Antígeno 592 casos, de los cuales el 56,4 % pertenece al sexo femenino, la mayor proporción se observó en el grupo de 30 y 39 años (21,6 %), el mayor número de casos se presentó en los barrios Natania (14,9 %) y School House (8,6 %). El 78,7 % de los casos, no presentó comorbilidades, el 7,3 % presentaba diabetes y el 3 % asma. El 24,3 % de los casos fueron asintomáticos, los síntomas más frecuentes fueron tos (25,0 %), fiebre (16,4 %) y cefalea (14,7 %). En el análisis de la distribución de las comorbilidades y síntomas por grupo etario se evidenció que no es estadísticamente significativo. Conclusiones: se confirmó el brote por COVID-19 en los 11 barrios priorizados de San Andrés Islas, lo que permitió caracterizar y aislar los casos positivos. A su vez se generaron recomendaciones a la secretaria de salud municipal acerca de las medidas de control para la contención del virus.

Abstract:
Los sistemas de ensilado de forrajes se presentan como alternativa para garantizar la disponibilidad de alimento en diferentes épocas del año, manteniendo el valor nutricional de los mismos. Además, el forraje ensilado puede sustituir o completar el uso de concentrados en la alimentación, reduciendo los costos, pero al mismo tiempo manteniendo calidad en la alimentación de los animales. Sin embargo, es importante realizar un proceso de ensilaje utilizando prácticas adecuadas que mantengan la estabilidad aeróbica del silo y que no permita la colonización de microorganismos indeseables, entre otros (Herrera 2014). Algunas enzimas fibrolíticas se adicionan como aditivos a los ensilajes para favorecer el proceso de fermentación, así como para aumentar el valor nutricional del mismo. Estas enzimas son producidas principalmente por hongos filamentosos mediante fermentación en estado sólido (SSF) (Daguerre et al. 2014). Actualmente, Agrosavia utiliza los conidios de un hongo filamentoso nativo de Colombia, Trichoderma koningiopsis Th003 para producir por SSF el bioplaguicida Tricotec®. Durante este proceso, queda un sobrenadante producto de la separación de los conidios del sustrato sólido, el cual contiene las enzimas que el hongo ha secretado, y que podría ser utilizados como un subproducto, dando un valor agregado al proceso. Por lo tanto, en este estudio se presenta la caracterización de las enzimas del sobrenadante, y el efecto de la temperatura y tiempo de almacenamiento sobre actividad enzimática. Se encontró que el sobrenadante presenta una baja actividad enzimática de proteasa (U/gss) 0,509± 0,060, amilasa (U/gss) 2,563 ± 0,117, una actividad enzimática media de CMCasa (U/gss) 10,949 ± 0,276, y Fpasa (U/gss) 11,149 ± 0,523. Mientras que presenta alta actividad enzimática de β (1,3)-glucanasa (U/gss) 20,873 ± 1,666, demostrando la capacidad de esta cepa para producción de enzimas pertenecientes al complejo celulolítico. En cuanto al efecto de la temperatura se concluye, que el extracto enzimático se puede almacenar a cualquier temperatura (-20 °C, 4 °C o 16 °C) sin tener ninguna diferencia significativa entre actividades enzimáticas en el día siete. La actividad enzimática de CMCasa y Fpasa aumento en el tiempo, mientras que la actividad enzimática que β (1,3)-glucanasa y proteasas disminuyo en el tiempo

A utilização de dados para a tomada de decisão sempre foi, em grande medida, adotada pelas corporações empresariais e, nas últimas décadas, pelas gestões das cidades. A elaboração do Plano Plurianual e demais Planos das diversas dimensões do desenvolvimento contam com os dados das diversas plataformas de dados setoriais do governo estadual e federal. Com recursos da Consulta Popular e acompanhamento da Secretaria Estadual de Inovação, Ciência e Tecnologia foi possível disponibilizar aos gestores municipais uma única plataforma onde é possível verificar 68 indicadores que traduzem a fotografia atual dos ODS – Objetivos do Desenvolvimento Sustentável, dos indicadores que mostram o que é uma cidade inteligente (baseada na ISO 37.122) e os indicadores que mostram os capitais existentes nas cidades e que possibilitam a estruturação de redes que apoiam o desenvolvimento. Além dos indicadores oficiais, a plataforma disponibiliza dados de percepção dos cidadãos do COREDE SERRA. Foram mais de 1500 respondentes, que expuseram as suas percepções sobre aspectos de suas cidades nos três diferentes modelos: ODS, Cidades Inteligentes e Sistema de Capitais. Será possível olhar para cada cidade e compará-la com cidades vizinhas, com a região, com o estado do RS e com o país. Caso a cidade decida perseguir os indicadores da cidade mais bem colocada numa determinada variável, haverá possibilidade de criar estratégias para alcançar tais indicadores. Neste momento em que a UCS - Universidade de Caxias do Sul oferece o Observatório das Cidades aos gestores municipais, o COREDE SERRA sente-se honrado por ter possibilitado esta construção, no momento da escolha do projeto. Entende que os números em si são altamente relevantes para o planejamento do futuro que se deseja. Associar a análise dos dados a Pareceres do grupo de especialistas envolvidos com o Observatório demonstrará, de fato, a integração entre a UCS e o setor público buscando elevar, ainda mais, o compromisso com o desenvolvimento regional. O Observatório de Cidades agregará, no curto prazo, muitas possibilidades de apoio à construção de Planos de Desenvolvimento Locais estruturados e baseados no estado da arte. Nossa expectativa é que agentes públicos, instituições, cidadãos e o setor empresarial se apropriem das informações para que, em aliança, se busque qualificar as cidades para atendimento de uma sociedade em profunda transformação. Este relatório vem em complemento à Plataforma Web do Observatório de Cidades, trazendo uma versão resumida dos dados da região.

Abstract:
The subject of the study is the cooperation of S. Efremov with Western Ukrainian periodicals as a page in the history of Ukrainian journalism which covers the relationship of journalists and scientists of Eastern and Western Ukraine at the turn of the XIX-XX centuries. Research methods (biographical, historical, comparative, axiological, statistical, discursive) develop the comprehensive disclosure of the article. As a result of scientific research, the origins of Ukrainocentrism in the personality of S. Efremov were clarified; his person as a public figure, journalist, publisher, literary critic is multifaceted; taking into account the specifics of the memoir genre and with the involvement of the historical context, the turning points in the destiny of the author of memoirs are interpreted, revealing cooperation with Western Ukrainian magazines and newspapers. The publications ‘Zoria’, ‘Narod’, ‘Pravda’, ‘Bukovyna’, ‘Dzvinok’, are secretly got into sub-Russian Ukraine, became for S. Efremov a spiritual basis in understanding the specifics of the national (Ukrainian) mass media, ideas of education in culture of Ukraine at the end of XIX century, its territorial integrity, and state independence. Memoirs of S. Efremov on cooperation with the iconic Galician journals ‘Notes of the Scientific Society after the name Shevchenko’ and ‘Literary-Scientific Bulletin’, testify to an important stage in the formation of the author’s worldview, the expansion of the genre boundaries of his journalism, active development as a literary critic. S. Yefremov collaborated most fruitfully and for a long time with the Literary-Scientific Bulletin, and he was impressed by the democratic position of this publication. The author’s comments reveal a long-running controversy over the publication of a review of the new edition of Kobzar and thematically related discussions around his other literary criticism, in which the talent of the demanding critic was forged. S. Efremov steadfastly defended the main principles of literary criticism: objectivity and freedom of author’s thought. The names of the allies of the Ukrainian idea L. Skochkovskyi, O. Lototskyi, O. Konyskyi, P. Zhytskyi, M. Hrushevskyi in S. Efremov’s memoirs unfold in multifaceted portrait descriptions and function as historical and cultural facts that document the pages of the author’s biography, record his activities in space and time. The results of the study give grounds to characterize S. Efremov as the first professional Ukrainian-speaking journalist.
