Abstract:
Background: On February 6, 2024, the Korean government announced a drastic increase in the medical schools’ admission quota to 2,000, sending shockwaves through the medical community. In protest, residents submitted their resignations and medical students submitted their leaves of absence.Current Concepts: The government has used many shocking tactics in pushing for greater medical school quotas. First, the government used lies to incite public opinion and socially isolate a group of doctors. Second, it violently imposed law enforcement against residents in the process of promoting the medical school expansion policy. Third, it pursued a medical school expansion policy that did not consider the quality of medical education. Triggered by the government’s rapid expansion of medical schools, fierce resistance from doctors and medical students has revealed structural contradictions in the Korean healthcare system.Discussion and Conclusion: In essence, the medical crisis of 2024 has been created by the government. The medical community cannot stop its legitimate criticism and resistance for the following reasons. First, this is not just a medical crisis but a crisis of the Korean constitution, rule of law, and democracy. Second, the fate of future generations depends on the resolution of the 2024 medical crisis. Third, the government’s policies are not saving essential medicine, but pushing it off the cliff.

Abstract:
Abstract Despite numerous studies of Iran in recent decades, the country’s concerted efforts to expand its medical diplomacy have been largely overlooked by experts and observers alike. This is despite the fact that the Islamic Republic has sought to employ medical diplomacy as a viable tool for expanding its soft power for a number of years. Over the course of the last two decades, in fact, Iranian authorities and health professionals have considered their medical advances to greatly add value to the country’s overall diplomatic profile. This article examines three basic questions: first, how has Iran used medical diplomacy to increase its regional influence in the Middle East? second, what are the factors that have contributed to the country’s success in this area? And, third, what are likely to be the regional consequences of Iran’s enhanced medical diplomacy in the future? The answers to these questions will present an overall picture of the current and future depth and voracity of Iran’s soft power.

Abstract:
Abstract Since so many mishandling of medical waste in various places in Indonesia that has endangered our environment by the potential disease that easily can spread to our society , it is very important for all of us to address it on how to improve the medical waste management. There are area of improvement needed in our regulation, public policies disciplines and social awareness and especially the importance of proper handling of medical waste by the medical waste producers such as hospital, clinic and laboratories. This study is not intended to produce scientific theory but more focus on how to solve the problem of medical waste with the limit time and less costly manner or in the other words as a macro diagnosis of “the main causes of the problems” of medical waste management in Indonesia. This study has made clear conclusion that Indonesia has to follow the basic principle and guidelines of International standards and recommends improvement of the coordination between Ministry of Health and Ministry of Environment, toward “one roof policy”, “strong political will of top government to empower MOE and also Law No. 18, 2008 and Law No. 32, 2009 must be reviewed toward “Polluter pay the price” Policy. The government also must allocate sufficient fund to function as regulator and controlling body and increase social awareness for environmental health. Key note: Medical waste, public policy, environmental health

Abstract:
The purpose of this study was to generate national estimates of the prevalence of medically unnecessary emergency medical services (EMS) transports to emergency departments (EDs) over time and to identify characteristics that may be associated with medically unnecessary transports. A previously published algorithm was applied to operationalize medical necessity based on ED diagnosis to 10 years of data from the National Hospital Ambulatory Medical Care Survey. The trend over time was reported using descriptive statistics weighted to produce national estimates. Nationally, the proportion of EMS transports that were medically unnecessary increased from 13% to 17% over the 10-year study period. Individual demographic characteristics, including insurance status, were not predictive of inappropriate utilization. EMS transports for medically unnecessary complaints increased from 1997 to 2007. Our findings from a nationally representative sample highlight the opportunity for alternative patient delivery strategies for select patients seeking EMS services.
