Abstract:
This thesis traces the efforts of rural county in Utah attempting to create a professional medical system and addresses the challenges community faced in this effort including divisions among local and national medical societies, women and gender issues, and opposition to religious hierarchy. Navigating these conflicts, the local leaders in San Juan County established a medical cooperative which enable the permanent residence of a physician and later the construction of a hospital. San Juan County provided these medical services for its residents at a time when many of counties in the United States failed to expand their health services. San Juan succeeded due to dynamic leadership, support of local medical association, and the slow expansion of the medical system.

Abstract:
To date, very little is known about the aetiology of the plasma cell disorder multiple myeloma (MM) and its precursor condition monoclonal gammopathy of undetermined significance (MGUS). Chronic antigenic stimulation and, more recently, medications have been investigated as potential aetiological risk factors however findings from observational studies have been largely inconsistent. This thesis aimed to explore the impact of medical history and drug exposure on the risk of developing MGUS and MM. A systematic review of the literature revealed an elevated risk of MGUS/MM in association with prior autoimmune disease and in particular pernicious anaemia. The findings of this study were further supported by a population-based nested case-control study carried out within the UK Clinical Practice Research Oatalink (CPRO). Similarly an increased risk of MGUS and MM following exposure to common community-acquired infections was observed within studies carried out within the CPRO and the USA SEER-Medicare dataset. Autoimmune disease and infections diagnosed after MGUS were not associated with progression to MM or associated Iymphoproliferative disorders within the CPRO dataset. Oral statin and bisphosphonate use was investigated as a risk factor for the development of MGUS/MM and MGUS progression using the UK CPRO dataset. While there was evidence of a reduced risk of MGUS/MM in association with oral statin use, an increased risk of both MGUS and MM was observed among oral bisphosphonate users most likely as a result of detection bias and/or reverse causality. Post-diagnostic statin use was also associated with a reduced risk of MGUS progression to any Iymphoproliferative disorder but not MM. Overall, the studies conducted as part of this thesis support a role for chronic antigenic stimulation in the development of MGUS and MM, and suggest a potential role for statins as chemopreventive agents within the MGUS/MM setting. Further research is however warranted to confirm these findings.

PGHD reflects medications and treatment, lifestyle choices, and health history. Unlike traditional medical visits, where clinicians collect and manage data within their offices, PGHD is collected by patients throughout the course of their day and provides an insight of how they are responding to treatments or lifestyle choices. For centuries, St. John’s wort (SJW; Hypericum perforatum L., Hypericaceae) has been used as an herbal remedy for various medical conditions both externally and internally in many countries. Although it is a well-known and widely used traditional medicinal plant, concerns about its safety and herb-drug interactions caused a significant decrease in market sales starting in the early 2000s. The adulteration history of St. John’s wort (SJW) goes back to 1875 in the United States; the American Pharmaceutical Association mentioned Ascyrum stans and A. crux-andreae as the substitutes of SJW in its report on adulterations and sophistications. More recently, many Hypericum species (H. androsaemum, H. barbatum, H. crux-andreae, H. hirsutum, H. maculatum, H. montanum, H. patulum, and H. tetrapterum) and synthetic dye mixtures (E123 Amaranth, E133 Brilliant Blue, E110 Sunset Yellow, and E102 Tartrazine) are reported as adulterants of SJW. This Laboratory Guidance Document presents a review of the various analytical methods used to differentiate between authentic SJW plant, powder/extracts and ingredients containing adulterating materials. This document can be used in conjunction with the Hypericum perforatum Botanical Adulterants Bulletin published by the ABC-AHP-NCNPR Botanical Adulterants Prevention Program in 2017.

This study was a diagnostic research, so the content was decomposed according to PIRO : P: Patients diagnosed with sudden death; I: C-reactive protein; R: There is no gold standard for sudden death, and the definition of sudden death varies from literature to literature. The World Health Organization defines sudden death: "Patients who are normally healthy or seemingly healthy die suddenly due to natural diseases in an unexpectedly short period of time." In our study, sudden death is determined by the history, symptoms, physical examination and electrocardiogram results assesed by doctor. If death events were collected from the patients’ medical records, deaths coded using the International Classification of Diseases-9th Revision, codes 410 to 414 for non-SCD and 798.1 for SCD; or the International Classification of Diseases-10th Revision, codes I20 to I25 for non-SCD and I46 for SCD. All deaths registered as sudden deaths were confirmed in interviews with the patient’s physician or family members again. O: sudden death.

Abstract:
Introduction and Importance: A double cystic duct with a single gallbladder is one of the extremely uncommon variations of the cystic duct and only a few cases were reported in literature. Case Presentation: A 33-year-old female, with an unremarkable medical history, presented to the emergency department with a 2-day history of right upper quadrant abdominal (RUQ) pain. The abdominal pain was gradually increasing in intensity radiating to the back and was associated with anorexia and multiple episodes of vomiting. Abdominal examination revealed RUQ pain and tenderness. Abdominal ultrasonography was performed, showing a markedly distended gallbladder with evidence of a few calculi one of which was impacted at the neck. laparoscopic cholecystectomy was done within 2 days of admission during which another luminal structure was identified that suggested a double cystic duct. Clinical Discussion: Anomalies of the biliary tree are common with the classical anatomical picture presenting in only 33% of cholecystectomy cases. However, the presence of a double cystic duct is a rare variation, especially in the case of a single gallbladder. The identification of such anomaly can be achieved preoperatively using imaging modalities or it can be identified during the surgical procedure itself. such identification reduces the chances of postoperative comorbidities. Conclusion: Pre-operative identification of biliary tract anomalies by different imaging modalities is limited. Hence the importance of cautiousness and achieving a proper critical view of safety intra-operatively to prevent possible complications intra- and post-operatively. Our case report emphasizes the diagnostic and surgical challenges of the double cystic duct.

Abstract:
Review question / Objective: Gallbladder cancer is a rare tumor that is mostly advanced once detected. The efficacy of surgical treatment is still controversial. Therefore, primary prevention of gallbladder cancer is important. There are many studies on risk factors for gallbladder cancer, but at present it is difficult to identify independent risk factors for gallbladder cancer, except for a history of symptomatic chronic cholecystitis and malignant transformation of a single polyp. Laparoscopic cholecystectomy is popular worldwide and can be a preventive procedure for gallbladder cancer in addition to resolving benign lesions. This study makes a meta-analysis of the latest research results exploring the risk factors of gallbladder cancer in the last decade , expecting to provide evidence-based medical support for the prevention of gallbladder cancer at the clinical level, and to provide some ideas to guide the surgical indications for LC and future research related to gallbladder cancer.
