Abstract:
According to the Ministry of Health Statistics, there have been 87,710 confirmed HIV infectedcases in Malaysia. Objective: To determine how well MAHSA University medical studentsknew about HIV/AIDS. Method: This was a cross-sectional questionnaire-based studyconducted at MAHSA University's School of Medicine. The surveys focused on differentmechanisms of transmission, high-risk behaviours, and prevention strategies. Result: A total of360 medical students participate in this study. Medical students in pre-clinical years hadsubstantially higher knowledge ratings than those in clinical years (p=0.045). Comfortablenessdealing with HIV/AIDS patients had the highest stigmatizing score (3.78 ± 0.86). Attitudestoward imposed measures had the lowest stigmatization score (mean 2.60 ± 0.62), knowledgescore is significantly correlate with stigmatizing attitude (P=0.004). In addition, higherknowledge scores were shown among medical students who reported to have previousencounters with patients living with HIV/AIDS (PLWHA) (p<0.001). Knowledge scores weresignificantly low among the patients living with HIV (PEWHIV) (4.2 ± 1.67) (P=0.019).Concerning mother to child transmission of HIV/AIDS infection, clinical medical studentsshowed higher degree of knowledge as compare with preclinical years medical students with P<0.05. Conclusion: The knowledge of medical students in Mashsa University is generallyhigh, awareness of HIV transmission from mother to child through breast milk is still lacking,indicating that this is the most significant feature that has to be stressed and improved in futuremedical curriculum for better health care services and public education.

Abstract:
Abstract Background The Ministry of Health of Malaysia has invested significant resources to implement an electronic health record (EHR) system to ensure the full automation of hospitals for coordinated care delivery. Thus, evaluating whether the system has been effectively utilized is necessary, particularly regarding how it predicts the post-implementation primary care providers’ performance impact. Methods Convenience sampling was employed for data collection in three government hospitals for 7 months. A standardized effectiveness survey for EHR systems was administered to primary health care providers (specialists, medical officers, and nurses) as they participated in medical education programs. Empirical data were assessed by employing partial least squares-structural equation modeling for hypothesis testing. Results The results demonstrated that knowledge quality had the highest score for predicting performance and had a large effect size, whereas system compatibility was the most substantial system quality component. The findings indicated that EHR systems supported the clinical tasks and workflows of care providers, which increased system quality, whereas the increased quality of knowledge improved user performance. Conclusion Given these findings, knowledge quality and effective use should be incorporated into evaluating EHR system effectiveness in health institutions. Data mining features can be integrated into current systems for efficiently and systematically generating health populations and disease trend analysis, improving clinical knowledge of care providers, and increasing their productivity. The validated survey instrument can be further tested with empirical surveys in other public and private hospitals with different interoperable EHR systems.

Abstract:
IntroductionTranslation of evidence into clinical practice for use of intravenous thrombolysis in acute stroke care has been slow, especially across low- and middle-income countries. In Malaysia where the average national uptake was poor among the public hospitals in 2018, one hospital intriguingly showed comparable thrombolysis rates to high-income countries. This study aimed to explore and provide in-depth understanding of factors and explanations for the high rates of intravenous stroke thrombolysis in this hospital.MethodsThis single case study sourced data using a multimethod approach: (1) semi-structured in-depth interviews and focus group discussions, (2) surveys, and (3) review of medical records. The Tailored Implementation of Chronic Diseases (TICD) framework was used as a guide to understand the determinants of implementation. Twenty-nine participants comprising the Hospital Director, neurologists, emergency physicians, radiologists, pharmacists, nurses and medical assistants (MAs) were included. Thematic analyses were conducted inductively before triangulated with quantitative analyses and document reviews.ResultsFavorable factors contributing to the uptake included: (1) cohesiveness of team members which comprised of positive interprofessional team dynamics, shared personal beliefs and values, and passionate leadership, and (2) facilitative work process through simplification of workflow and understanding the rationale of the sense of urgency. Patient factors was a limiting factor. Almost two third of ischemic stroke patients arrived at the hospital outside the therapeutic window time, attributing patients' delayed presentation as a main barrier to the uptake of intravenous stroke thrombolysis. One other barrier was the availability of resources, although this was innovatively optimized to minimize its impact on the uptake of the therapy. As such, potential in-hospital delays accounted for only 3.8% of patients who missed the opportunity to receive thrombolysis.ConclusionsDespite the ongoing challenges, the success in implementing intravenous stroke thrombolysis as standard of care was attributed to the cohesiveness of team members and having facilitative work processes. For countries of similar settings, plans to improve the uptake of intravenous stroke thrombolysis should consider the inclusion of interventions targeting on these modifiable factors.

Abstract:
Objectives: Iodine is a trace element that is essential for the synthesis of thyroid hormone. Both chronic iodine deficiency or iodine excess have been associated with hypertrophy and hyperplasia of follicular cells in thyroid gland and the influence of thyroid hormone (T3, T4) and thyrotropin (TSH) secretion. Increase rates of the thyroid cancer are increasing after radiation exposure to 131I in children or aldolescents. Methodology: In respective published reports in literature and in combination of our previous study, dietary iodine excess goiter, iodine induced hyperthyroidism (IIH) and IIT, Iodine intake and the prevalence of papillary carcinoma (PTC), as well as the case-control and cohort studies of thyroid cancer and intake of seafood and milk products, were systematically reviewed. Relative factors that should be considered when studying the effect of dietary iodine in the development of thyroid cancer include screening programs, pathological criteria, diagnostic techniques, radioactive iodine, and standard of medical care in the studied population. Results and conclusion: In current surveys, papillary thyroid carcinoma forms the largest group of thyroid malignancies, after iodine intake excess or iodine prophylaxis where an increase in the papillary: follicular carcinoma ratio was uncovered. Also, there is clear temporal relationship in many countries between introduction of iodine intake excess especially as to radioactive iodine and an increase in incidence of PTC. Iodine goiter, IIH and IIT were also noted. Autoimmune hashimoto's thyroiditis are linked to dietary iodine. Dietary iodine intake is another care of environmental relevance factor in thyroid diseases and papillary carcinoma. Available evidence of oncogenic thyroid hormone receptor mutants from animal experiments and clinical investigation have been a shift toward the oncogenic function of human thyroid carcinoma, and also its target therapy.
