Abstract:
Despite attempts to contain health care cost, healthcare expenditure has been surging worldwide. Healthcare financing remains high on the political agenda and nations are struggling hard to balance cost containment with service quality, accessibility, efficiency, etc (Froetschel 2011). Hong Kong, of no exception, faces increasing pressure to raise public expenditure on health and is seeking new ways to finance healthcare. This paper attempts to provide an overview of Hong Kong’s existing health financing system and identify possible reform options.

Abstract:
Introduction Accessibility to care is a key measure for quality health care. Waiting list resulted due to disequilibrium between demand and supply. Waiting time is a common issue in public health care services. Long waits and delays dissatisfy patients, affect clinical outcome and increase health care costs. Access management is therefore important to enhance patient safety, increase satisfaction and reduce service inefficiency. In Hong Kong, waiting lists for public specialist out-patient services have been increasing over the past years. Promotion of appropriate referrals and appropriate utilization between primary and secondary care was identified as one of the current strategies for service demand management. Referral guidelines were introduced to define the clinical conditions to be referred. Appropriate work up and trial of treatment was recommended before referral to specialist care. They were translated into standard referral letter templates and built into the existing electronic medical record system as an execution platform to facilitate workflow and enhance compliance. This electronic referral system was piloted since January 2010 in Department of Accident of Emergency and General Out-patient Clinic at one local public hospital in Hong Kong. Methods The objective of the study was to evaluate the effectiveness of current strategy in access management to public specialist out-patient services. All new case referrals to Medical and Surgical Specialist Out-patient Clinics (SOPC) of the pilot hospital from January 2010 to December 2010 were examined. While, the new case booking data from January 2009 to December 2009 in respective units of the same hospital was used as control group for comparison. Potential changes in number and distribution of new case bookings at medical and surgical SOPC as well as their corresponding waiting time were looked into. Comparison of referral pattern before and after the implementation of new referral system was performed. Results Changes in referral pattern in terms of distribution of triage categories have been observed. Number of semi-urgent cases was significantly increased in 2010 compared with 2009 in both Medical and Surgical SOPC (p=0.006 and p=0.048). Shortening of overall median waiting time was also seen in both Medical and Surgical SOPC of the pilot hospital in 2010. Consistent reduction with statistically significance was noted in all triage categories of both specialist clinics, except urgent cases in Surgical SOPC. Larger effect was seen in non-urgent cases, with 9 weeks and 4 weeks shortening of waiting time in Medical and Surgical SOPC respectively. For overall distribution of waiting time, persistent multimodal patterns were observed in both Medical and Surgical SOPC during the study period. Major peaks were identified within 2 and 8 weeks time of appointment, while scattered clustered bookings were seen along the timeline with waiting time up to years. Discussion The observed result was reinforcing the direction on current strategy, despite direct causal relationship could not be established at the moment. Changes in referral pattern could possibly be explained by the behavioral change of clinicians upon referral and triage of patients due to the introduction of the new referral system. Potential Hawthorne effect during the pilot period should therefore be considered. In view of the complexity and interconnectedness of various service components in the health care system, further studies should be of value to identify the change agent in the system and look into the efficiency gain as well as outcome improvement. Long term and regular monitoring mechanism of waiting time with specific set of performance indicators has to be in place for continuous quality improvement. Taking the public health approach by applying operational studies and simulation models should therefore be the way for further improvement of operational efficiency and service planning in the whole public health system. Conclusion Appropriateness of referrals and utilization between primary and specialist services was introduced for access management of public specialist out-patient services in Hong Kong. Changes in referral pattern with shortening of waiting time were observed. In order to manage the accessibility issue effectively, a balanced approach on demand, queue and capacity management was suggested to be adopted at the organizational level. Collaboration across sectors particularly with the direction to strengthen primary care would also be needed globally at the system level for a healthy, equitable and sustainable system.

Abstract:
Introduction: Amongst all public servants in Hong Kong, health care workers are one of the groups suffering from highest working pressure. They have long working hours and may have overnight shift duties, including Sundays or even public holidays. This may lead to poor job satisfaction, psychological stress and the recent high resignation rate in public hospitals. The aim of this study is to assess the prevalence of the psychological stress, psychological symptoms and job satisfaction of health care workers, the association between stress, psychological symptoms and job satisfaction; and also the factors associated with job satisfaction in a regional hospital in Hong Kong. Method: Health care workers in a large regional hospital of Hong Kong were surveyed by means of a questionnaire assessing basic demographic data, questions of the General Health Questionnaire (GHQ-12), Perceived Stress Scale (PSS), Warr-Cook and Wall job satisfaction scale (JSS). Four groups of health care workers (doctors, nurses, allied health workers and supporting staffs) were surveyed. Summary of descriptive statistics were calculated for each group to compare the prevalence of job dissatisfaction, perceived stress, and psychological symptoms. Two-stage analysis will be used. The first stage analysis will use ANOVA test to access the association between job satisfaction and different variables. The second stage analysis will use multivariate regression model to further assess the coefficient correlation of significant factors drawn from ANOVA test with job satisfaction. Results: There were 674 eligible questionnaires. About half (47%) of the health care workers reported having perceived stress and a third (33.8%) psychological symptoms. Doctors reported the (76.8%, 95% C.I = 69.43%, 84.17%) highest level of job satisfaction amongst all the health care workers surveyed. Among staff reporting a GHQ score equal to or more than three, supportive staff had significantly higher prevalence (38.7%, 95% C.I.=27.96%, 49.44%) and doctors the lowest prevalence (28.1%, 95% C.I.= 15.11%, 41.09%) of psychological symptom but proportions were compatible with their counterparts in other countries. The mean score for GHQ-12 was 2.41+/- S.D. 3.28. The overall mean perceived stress score was 18.14 with SD +/- 5.0. There was no significant difference when different subgroups were compared. The mean PSS scores of all subgroups were lower than their counterparts in other counties but were quite similar to the mean PSS reported during SARS period. Sixty eight percent of all health care workers surveyed were satisfied with their job (respondents indicating “moderately satisfied”, “very satisfied” and “extremely satisfied” on their overall job satisfaction). Values equal to or above 5 reflect being satisfied. The mean value for Job satisfaction was 4.58 +/-S.D. 1.21. The factors including shift duty, perceived stress, and psychological symptoms were negative correlated with job satisfaction. Factors such as clinical work, doctor and secondary school level were positive correlate with job satisfaction. Conclusion: Prevalence of perceived stress and psychological symptoms among health care workers were high when compared with the general population (14-17.6% for perceived stress and 28.1% for psychological symptom), but not as high as expected. In contrast to popular belief, doctors had the lowest perceived stress level, lowest prevalence of psychological symptom and the highest job satisfaction among different groups of health care workers. This may be related to higher income, social status and, education background that might help to protect them from depression and anxiety. Supportive staffs, who felt neglected by management, were found to have the highest prevalence of psychological symptom and higher stress levels. Nurses got highest prevalence of perceived stress. More attentions and resources should be devoted to these groups to cope with their psychological needs and stress.

Abstract:
Background “Over-crowding” in Accident and Emergency (A&E) departments of Hong Kong public hospitals with primary care cases not only threatens the service quality of emergency care and prolongs patients’ waiting time for treatment in the A&E departments, but also causes inefficient utilization of resources and deprives true emergency cases of quality care. A local study suggested that 57% of A&E attendances would have been managed better by general practitioner (GP) care. Reasons of the “over-crowding” include: lacking overnight outpatient services in the community, the convenience of A&E departments and the comprehensiveness of emergency services provided in the departments. As a result, there are urgent needs of patients, the society, A&E departments of Hong Kong public hospitals for a feasible solution to divert non-emergent A&E patients to more appropriate primary care services. Objectives To compare the costs and benefits of setting up the proposed 24-hour public emergency clinic inside the A&E department of Prince of Wales Hospital (PWH) for stable ambulatory non-emergent Categories IV and V A&E attendances by conducting a cost-benefit analysis. Methodology Retrospectively compare the costs and benefits of healthcare service provision to A&E attendances in PWH from Feb 2013 to Jan 2014 by cost-benefit analysis. By estimating the patients’ utilization rate of the emergency clinic, the total cost of the proposal is calculated by summarizing the estimated costs of employees’ salaries, capital costs, costs of equipments and treatments provided in the clinic in monetary values. The total benefit of the proposal is calculated by summarizing the estimated benefits of revenues, productivity gain from prevention of prolonged waiting and benefit from emergency diversions in monetary values. Results With the patients’ utilization rate of the clinic estimated as 44%, the total cost of the proposal is estimated as HKD $ 868,863 per month; while the total benefit is estimated as HKD $2,366,695 per month. The net benefit is estimated as HKD $1,497,832 per month with cost-benefit ratio greater than 2.72 of total benefit over total cost. Conclusions The proposal of setting up the 24-hour public emergency clinic in the A&E department of PWH is economically beneficial, and the results are generalizable to other A&E departments of similar acute urban public hospitals in Hong Kong.

Abstract:
Background: As in the rest of the world, cancer has been a leading killer in Hong Kong. Though technology has been growing rapidly, expensive cancer treatments have continuously been problematic to patients and their families. There are some known risk factors that make some people have a higher risk for cancer than others, but the reason why some develop cancer and some do not is mostly still unknown. In addition, the expensive cancer treatments can distress patients and their families psychologically during the painful and long chemotherapy process which is a common cancer treatment. While it is important for experts to research on effective cancer treatment, it is also important for the government and health care experts to solve associated financial problems. In response to help patients to ease their financial burden of expensive medical treatment, the Hong Kong government has proposed a new health protection scheme (HPS), “My Health, My Choice.” Objective: In this paper, a systematic review on different published literatures is conducted to analyze the prospective outcome of HPS and if it can help patients to ease their financial burden. Results and Discussion: The Health scheme provides a financial aid option for patients who suffer from chemotherapy through monthly premium. However, the implementation of this HPS seems to be difficult both on the patients’ and the providers’ sides. Case study of health care systems in US and Canada is included in this paper to find out what Hong Kong can learn from other countries with completely different payment systems would manage to deal with this problem. Australia which with a universal coverage health care system has also proposed a similar HPS plan aiming to help lower health care cost by increasing individual responsibility on medical expenses. However, it failed by lack of support from the general public. The Australian example would be used to criticize some essential elements that would contribute to the failure of the HPS, and how Hong Kong would use this example to yield a better proposal. Conclusion: As HPS might not be able to ease the burden on cancer patients in Hong Kong, it is suggested for government to allocate more effective and direct resources on helping cancer patients, especially those who are receiving chemotherapy or improve services through better primary care. However, the final outcome remains unknown, and the final option still depends on the ultimate need from the general public.

Abstract:
Background Emergency department (ED) overcrowding has become a worldwide problem over the past few years, which has been reported in USA, Canada, New Zealand and Australia. For the past two decades, ED overcrowding has also become a controversial issue in Hong Kong, due to high demand for emergency service and misuse of emergency services. In 2002, although there was a charge for emergency department visit which led to a markedly decrease (19.1%) of total attendance, but now the rising trend of ED overcrowding seems to resurface during past few years. This paper aims to review and synthesis causes of ED overcrowding and possible interventions so as to provide possible recommendations for emergency care in Hong Kong. Methods Literatures on ED overcrowding and potential interventions were searched from PubMed, Google Scholar and Google to locate all relevant articles in English up to May 2013. Through PubMed, ED was described using “Emergency Medicine [MeSH]” OR “emergency department” OR “emergency”, and overcrowding was described using “Crowding [MeSH]” OR “crowded” OR “overcrowding” OR “overcrowded” OR “congestion”, and interventions was described using “interventions” OR “solutions”. Besides, relevant emergency medicine literatures published from the Hong Kong Journal of Emergency Medicine were also reviewed. Results I identified and reviewed relevant articles and found that ED attendance has been steadily rising during the past decades in Hong Kong. Although the causes may be somewhat different between different countries, causes of ED overcrowding could be related to easy access to emergency services, barriers to primary care as well as specialist care, and the rising aging population which might be an important underlying cause. As the problem of ED overcrowding will have significant negative impact on patient outcomes, such as unnecessary death, two common interventions to the problem are increasing the resources and demand management. Apart from increasing resources within emergency departments to cater for the increasing demand, it is of highly significance to improve community and primary care for the needs of older people who will contribute a great proportion to ED overcrowding in the future. Conclusion Semi-urgent and non-urgent visits do account for a great proportion among the total attendance, so it is important triage these patients to alleviate the overcrowding. What’s more, pressure on EDs can be related to a significant increase in the number of elderly patients who may require more investigation or admissions, and need much longer time to manage. As a result, future health policies should focus more on the aging population to improve emergency care.

In Hong Kong, access to elective surgeries in public hospitals is often associated with lengthy waiting times. Facing resource constraints and increasing demands from a rapidly ageing population, the Hospital Authority (HA) is constantly confronted by the healthcare rationing dilemma. To date, publicized data on elective surgery waiting times at the HA remain limited, and a standardized way of measuring waiting time is currently lacking. Recognizing that the definition of waiting time will form the basis for future policies in addressing the issue, a three-step approach will be taken in this paper. First, a comparison will be drawn for the varying definitions of waiting time worldwide. Next, a suitable definition will be proposed for Hong Kong, followed by analysis of where policy interventions are most needed for reducing waiting times. Finally, best practices for managing waiting times will be extrapolated from England, Canada, Australia, and Spain to serve as guidance for Hong Kong’s future policy direction.

Abstract:
Background: Medical tourism is arising in Asian countries and in Hong Kong, the maternal tourism is evolving due to the influx of non-entitled pregnant women from mainland China for deliveries. The visitors were attracted not only by the technical advancement of Hong Kong healthcare system, but its special administrative background which grants the babies with a permanent resident identity as well as the citizenship benefits regardless to their parental status. Besides, the parents could avoid penalties from the "One Child Policy" in China. With the limited healthcare resources, non-local mothers started to compete with the local mothers for obstetric services. The community raised their concerns in developing of maternity tourism and verbalised their demands in protecting local rights and equity to resources. Aims and Objectives: There are limited published researches available on maternal tourism or the specific situation faced by Hong Kong. By analysing grey literature, this paper would like to suggest the impact exerted on the healthcare system by the influx of non-eligible mothers with a medical tourism framework. Results: The impact of non-local-mother deliveries on healthcare system were discussed in the areas of governance, delivery, financing, human resources and regulation. Future research could be done on assessing the priorities in the framework components and the direction, effectiveness of the management strategies for non-entitled deliveries.
