Full textThe subject of this work is the study of the woman’s body in the field of ancient medical literature, from both physiological and cultural aspects. The study is based on the testimony of two authors who were key figures in this reserach : Hippocrates (5th century B.C.) and Soranus (1st century B.C.). Genealogical and embryological treatises of the Hippocratic Corpus highlight the great importance of the maternal role in the image of the woman, a function by which the female body is interpreted, matter that is also confirmed in several pieces by Sorano’s Gynaecia which show an important interest for the expectant mother. Indeed, because of the negative opinion about virginity and the problems arising from infertility, motherhood became not only the guarantor of the family continuity, but also a therapeutic method which provided a state of well-being. A philological and historical-literary analysis of the treatises can clearly demonstrate the complexity of the social and anthropological indications which are abound in the treatises

Full textThe cerebral microvascular system is central in the remarkable machinery of the brain. It is responsible for the delivery of vital molecules (e.g. oxygen, glucose) and clearance of metabolic wastes (e.g.carbone dioxide, amyloid) to and from brain cells. Such a system is composed of small vessels (i.e. arterioles, venules and capillaries), embedded in brain tissue, which form a very large and intricate network spanning over the whole brain. Due to its critical role in brain homeostasis, the cerebral microvascular system is also involved in various pathologies, ranging from stroke toneurodegenerative diseases. During the last decades, significant advances in imaging techniques have been made, such asmulti-photon microscopy, that enabled the observation of the cerebral microvascular system with anunprecedented level of accuracy. However, these techniques generate large amounts of data that aredifficult to interpret without a proper theoretical framework. In this thesis, we address this need bybuilding computationally efficient models that accurately describe blood flow and solute transport in the cerebral microvascular system, more specifically in large anatomical networks.The biggest challenge in the resolution of blood flow and solute transport problems dwells inthe scale of such anatomical networks. Indeed, even though they represent only a fraction of thecomplete cerebral microvascular system, they are made of tens of thousand of vessels and exhibithighly complex geometries. Consequently, this prohibits the resolution of the blood flow and solutetransport problems by means of classic numerical methods, e.g. finite volume or finite elements. Toget around this computational issue, we combine a pore network approach to upscaling methods(volume averaging and multiscale asymptotics) and Green’s functions to simplify the formulation ofboth blood flow and solute transport problems while still capturing the underlying physics. In orderto assess the relevance of such simplifications we systematically validate our models against in vitroand in-vivo measurements, and against reference analytical solutions otherwise. We then use our models to investigate the role of the cerebral microvascular system during theonset of Alzheimer’s disease. Indeed, it has been recently shown that a significant decrease in the cerebral blood flow is the earliest biophysical marker of the disease. Coincidently, our collaborators,Professors Schaffer and Nishimura from Cornell University, have observed in Alzheimer’s diseasedmice that a small proportion (2% to 4%) of capillaries were abnormally occluded by white blood cells adhering to the inflammatory vessel walls. They subsequently injected antibodies inhibiting the whiteblood cells adhesion. This resulted in the unclogging of the capillary vessels, causing a substantialincrease in cerebral blood flow and ultimately leading to a significant cognitive improvement of thediseased animals. Assuming that such antibody injections restored the cerebral blood flow to itsbaseline value leads to estimate that capillary occlusions were previously reducing the cerebral blood flow by 20% to 30%. This raises the critical question: can 2% to 4% of capillary occlusions cause up to 30% reductionin cerebral blood flow? This question is challenging to answer only using experiments since it istremendously difficult to isolate the contribution of such a biophysical process in vivo in mice or humans. Instead, we use our models to numerically investigate the impact of such capillaryocclusions on cerebral blood flow. We find that 2% to 4% capillary occlusion cause up to 12% cerebral blood reduction making them a significant mechanism in the onset of Alzheimer’s disease. Finally,we go on to investigate the consequences of such occlusions on molecule exchange

Full textThere is increasing evidence from experiments in rodents and non-human primates, as well as from human studies, to suggest that the different structures within the medial temporal lobe (MTL) differentially contribute to declarative memory. In the human brain, two neural networks implicating MTL structures have been described: an anterior MTL network that includes brain areas that contribute to context-free memory (object memory and semantic memory or memory for « what ») and a posterior MTL network that contributes to context-rich memory (spatial memory, episodic memory or memory for “where” and “when”). In Alzheimer’s disease (AD), neurofibrillary tangles (NFT), associated with cognitive signs, initially appear in the sub-hippocampal (transentorhinal and entorhinal) cortex, which are part of the anterior MTL network, before reaching the hippocampus. Potential cognitive deficits related to the dysfunction of this brain area in AD are not clearly identified. In the presented studies, the emphasis is placed on the investigation of sub-hippocampal corteces using a neuropsychological approach and neuroimaging techniques. Our findings suggest that the very earliest stages of AD could represent a “model” leading to a better understanding of memory systems that involve the MTL. They also provide evidence that evaluating context-free memory may be useful in the diagnosis of early AD

Full textThe present study aimed at studying autobiographical memory (AbM) in patients at early stages of Alzheimer’s disease, as well as at analyzing the link between AbM and the Self components (as defined by Prebble et al., 2013), and finally, at investigating its neuro-anatomical correlates. The results we obtained confirmed AbM is damaged in patients with regards to episodic and emotional components, whatever the age of the memory. The deficit in episodic memory was associated with medial temporal lobe atrophy, with the left hippocampus seemingly involved in the temporal context of the memories and the right amygdala in the emotional component. Conversely, specificity of remaining emotional memories was relatively preserved, as well as semanticized memories, which rely on the temporal neocortex. In the context of the Self more generally, our results highlight a relationship between the conceptual-Self and autobiographical memories, through semanticization and integration processes, which allow the formation of the most abstracted forms of self-representations. Moreover, the subjective sense of Self appears as a prerequisite to all other Self components. Based upon a case study and a volumetric group study, we were able to show that the implication of the medial prefrontal cortex is common to all Self components, suggesting its key role for the subjective sense of Self. Our results point to a potential rehabilitation therapy based on reinforcing self-defining memories to strengthen the Self. Full textCell therapy is an innovative method of the future, currently used in the treatment of multiple diseases (autoimmune, cancer, inflammatory pathologies, allografts ...) and the regeneration of tissue loss. Mesenchymal stem cells (MSC), regardless their origins, exhibit very interesting properties for therapy, including a potential for multi-line differentiation, and important immunomodulation properties. My project focuses on the use of oral stem cells recently isolated from the gingiva by our team (GSC), and having a functional advantage over traditional mesodermal (bone marrow) cellular sources. The bone defects of the jaws, due to their multitude of etiologies (trauma, dysmorphoses, cancer...) and the generated handicap, represent a preferred therapeutic target. GSCs have the same neurectodermal embryological origin as the maxillary bones and thus a similar phenotype, explored in our team. This gingival source of non-traumatic removal is an alternative to current mutilating surgical techniques for the donor site. Our goal is twofold: To establish a preclinical GSC culture protocol in osteoblasts, to be compatible with human therapy, in order to achieve optimal bone regeneration. The immunomodulatory capacities of the GSCs are themselves studied under these new conditions, with the aim of controlling the inflammatory reaction and preserving the bone graft, thanks to the exceptional platform made available to us by the French blood establishment, and A highly specialized team in the study of immune regulation mechanisms. Our results will not only allow transposable bone regeneration in humans but also use these cells for the treatment of other pathologies (cancerous, autoimmune ...) using their immunomodulatory capacity

Full textOur research mainly focuses on tasks with an application purpose: depression and anorexia detection on the one hand and aggression detection on the other; this from messages posted by users on a social media platform. We have also proposed an unsupervised method of keyphrases extraction. These three pieces of work were initiated at different times during this thesis work. Our first contribution concerns the automatic keyphrases extraction from scientific documents or news articles. More precisely, we improve an unsupervised graph-based method to solve the weaknesses of graph-based methods by combining existing solutions. We evaluated our approach on eleven data collections including five containing long documents, four containing short documents and finally two containing news articles. We have shown that our proposal improves the results in certain contexts. The second contribution of this thesis is to provide a solution for early depression and anorexia detection. We proposed models that use classical classifiers, namely logistic regression and random forest, based on : (a) features and (b) sentence embedding. We evaluated our models on the eRisk data collections. We have observed that feature-based models perform very well on precision-oriented measures both for depression or anorexia detection. The model based on sentence embedding is more efficient on ERDE_50 and recall-oriented measures. We also obtained better results compared to the state-of-the-art on precision and ERDE_50 for depression detection, and on precision and recall for anorexia detection. Our last contribution is to provide an approach for aggression detection in messages posted by users on social networks. We reused the same models used for depression or anorexia detection to create models. We added other models based on deep learning approach. We evaluated our models on the data collections of TRAC shared task. We observed that our models using deep learning provide better results than our models using classical classifiers. Our results in this part of the thesis are in the middle (fifth or ninth results) compared to the competitors. We still got the best result on one of the data collections

Full textFor the past 30 years, Alzheimer’s disease (AD) has been considered as a crippling memory disorder impairing any possibility of new learnings in declarative memory. However, in the history of neuropsychology, cases of residual encoding have been reported with amnestic patients presenting different etiologies despite showing lesions very similar to AD. Although using neutral verbal and pictural items in laboratory settings failed to report preserved learning capacities from the mild stages, we investigated how using music and other artistic items in ecological settings may reveal these capacities in AD patients at a moderate to late stage, notably by passive repeated exposition. By relying on a behavioral scale designed to study the evolution of the sense of familiarity in these patients, we were able to show and describe new learnings in this population, and inferring their nature in view of both classical and contemporary memory models. Finally, we offer suggestions to discuss how acknowledging these capacities could change the way AD is perceived, and how it could help caring for people affected by it and their familial and professional caregivers

Full textMy PhD thesis aims to analyse the fibromyalgia syndrome understood as an entity defined by contemporary and located biomedical knowledge and practices. While a transnational biomedical vision of the syndrome is analysed, the ethnographic research field is located within the Italian context. The first chapter is focussed on biomedical theories which analyse chronic pain without organic lesions, also framed here by the concept of pain as a disease; it ends by analysing how complex chronic painful states are considered at one legislative and political level in the Italian national context. The second chapter analyses fibromyalgia syndrome seen as a biomedical category. A first section examines how the syndrome is framed by transnational biomedical scientific knowledge; the second section focuses on data collected during the ethnographic research. The third chapter is dedicated to an investigation of the medical reasons that outline fibromyalgia as a female syndrome. It also proposes, in its second section, the testimony of Marzia, a woman affected by fibromyalgia; beyond a purely biomedical interpretation of the disease, through Marzia’s story the syndrome emerges in contrastive, opposite and unpredictable ways
La mia tesi di dottorato analizza la sindrome fibromialgica intesa come entità presa in carico e trattata da saperi e pratiche biomediche contemporanee e localizzate. Pur focalizzandomi sulla visione biomedica transnazionale della sindrome, il mio terreno di ricerca è situato all’interno del contesto nazionale italiano. La prima parte della tesi è centrata sull’analisi della visione biomedica del dolore cronico senza lesione, inquadrato qui anche nei termini di “dolore-malattia”. Essa propone anche una osservazione di come sono inquadrate nella contemporaneità forme dolorose croniche complesse a un livello più propriamente legislativo e politico all’interno del contesto italiano. La seconda parte della tesi si concentra sulla fibromialgia intesa nei termini di categoria medica. Un primo capitolo analizza come attualmente la sindrome è inquadrata attraverso un sapere scientifico transazionale; un secondo capitolo è focalizzato sui dati raccolti durante la ricerca etnografica. La terza parte della tesi esamina la costruzione medica della fibromialgia nei termini di una sindrome femminile. Propone infine la testimonianza di Marzia, una donna colpita da fibromialgia che, attraverso la sua storia e opponendosi a un’interpretazione puramente biomedica della malattia, definisce la sindrome in modo contrastante, mutevole e imprevedibile

Full textIntroduction.
Le premier chapitre de l’introduction expose les problèmes posés à la sécurité sociale par le vieillissement de la population et plus particulièrement par le développement de la dépendance des personnes âgées. Ces problèmes sont illustrés par l’évolution des dépenses en soins de santé pour les aides aux actes de la vie journalière en institutions d’hébergement des personnes âgées, et à domicile.
Le deuxième chapitre décrit les instruments d’évaluation qui sont étudiés dans la présente dissertation. En effet, en Belgique, le financement des soins à la dépendance est réalisé sur base d’une évaluation à l’aide d’une échelle de l’INAMI ;une allocation à la personne âgée peut être obtenue par les personnes âgées dont la dépendance est alors évaluée à l’aide de l’échelle de la prévoyance sociale que nous appellerons aussi APA. Nous les comparons à l’outil d’évaluation utilisé en France et dénommé AGGIR pour autonomie gérontologique, groupes iso-resources.
Méthodes et populations.
Méthodes.
Nous validons les trois instruments sur le plan du construit à l’aide la Classification Internationale du Fonctionnement (CIF) de l’OMS. La validité est évaluée de différentes manières :validité concurrentielle entre les trois instruments ;validité concomitante des trois instruments par comparaison aux temps de soins nécessaires par les méthodes de corrélation, leur capacité de discriminer des catégories de dépendance ;la fidélité des instruments est également étudiée.
Populations
Quatre enquêtes ont été réalisée :la première en institutions de personnes âgées, la seconde à domicile, la troisième à domicile et la quatrième en institutions de personnes âgées avec la collaboration de différents professionnels :infirmières soignantes, infirmiers conseils de mutualité et l’auteur de la dissertation.
Résultats.
La troisième partie de la dissertation expose les résultats démographiques et des tests de validation.
Discussion.
La quatrième partie évalue les résultats. L’échelle de l’INAMI est de conception ancienne pour son contenu. Sa validité est suffisante pour étudier les charges en soins de populations de patients mais insuffisantes pour l’évaluation des besoins individuels, car elle n’évalue pas les besoins pour les actes instrumentaux de la vie journalière.
L’échelle APA a une mauvaise validité de contenu ;sa validité de construit est la moins bonne des trois instruments étudiés. Sa fidélité est médiocre. Cette échelle est à déconseiller.
La grille AGGIR a une validité de contenu moderne et bonne, une bonne validité de construit et une bonne fidélité.
Conclusions.
Des trois instruments étudiés, la grille AGGIR est la plus performante et permet à la fois le financement des soins à des populations des patients et l’évaluation de critères d’éligibilité pour l’octroi d’avantages sociaux.
