Abstract:
Objectives: 1) To present a technique for implanting the Med-El Combi 40+™ using a small incision with minimal access. 2) To describe the short term postsurgical outcomes in these patients. Methods: Two patients (1 child and 1 adult) underwent a novel small incision technique for implantation of the Med-El Combi 40+™ cochlear implant device. The short term outcomes in these two patients were described and compared with previous experience using the standard implantation technique citing advantages and possible limitations. As these two patients had bilateral implantation utilizing different techniques on the two sides interesting comparisons could be made on the same individuals. Results: The preliminary experience with a novel small incision technique for the Med-El Combi 40+™ implantation shows encouraging results in terms of healing and initial performance of these patients. Conclusion: This small incision technique may be offered to patients especially to those who wish to have bilateral implantations as this allows a less invasive approach, good cosmesis without sacrificing the safety and performance outcomes at least in the short term. Some process: 40 years of re-organising Norwegian archaeologyThe main objective of this paper is to investigate the historical background of the so-called ‘regionreform’ recently proposed by the Norwegian Government with special emphasis on the Cultural Heritage Management (CHM). The paper presents a critical inquiry into re-organisations within the Norwegian CHM in the 1980- and 90’s, a process known within academic research and official State documents simply as ‘the re- organisation’. The re-organisation involved four vital events: (1) the transformation of the Directorate for Cultural Heritage into a central body in the administration and located directly below the Department of Environmental Protection in 1988, (2) the de-centralisation of the district authority from the university museums to the county councils in 1990, (3) the creation of NIKU (The Norwegian Institute for Cultural Heritage Research) in 1994, and (4) the displacement of exempt authority from the university museums to the Directorate in 2001. The paper focuses on the political processes these events were part of, especially on political strategies, discursive turning points and speech acts, drawing on theoretical and methodological aspects from Actor-Network-Theory. It is found that the ideas and strategies that were realised through ‘the re-organisation’ dated back to academic circles and cultural policies forged in the 1960- and 1970’s. It is concluded that the ‘regionreform’ was formed within a new network of textual references, but that the ideas behind it is thoroughly connected with the old re-organisation, and thus to discursive elements from the 1960’s.

Abstract:
Presented here are the results of speech perception measures and subjective scales from 68 children with 6 months of device experience and from 49 children with 12 months of experience in using the Med-El Combi 40+ cochlear implant system. All children demonstrated bilateral profound hearing loss before operation, and 90% experienced onset before 3 years of age, but their hearing losses had a broad range of causes. A battery of age-appropriate objective speech perception measures was administered, in addition to a battery of subjective auditory skill assessments. The group data indicate that the children showed a statistically significant improvement in a wide array of speech perception skills and auditory behaviors with this device. In addition, subjective therapist survey data indicated changes in primary receptive and expressive communication method with Combi 40+ device use. Individually, all children showed significant improvement on at least 1 of the speech perception measures used in the test battery. This evidence suggests that the Med-El Combi 40+ system can provide meaningful auditory information for profoundly deaf children, which in turn supports meaningful functional improvement in speech perception and communicative skill development.

Abstract:
Fifty consecutive patients with degenerative lumbar spondylolisthesis were treated surgically. The first 17 patients (FU group) underwent decompression with fusion, the second 33 patients (MED group) underwent microendoscopic decompression (MED), and the outcomes following the two surgical methods were compared. The duration of follow-up ranged from 27 to 40 months in the FU group and from 25 to 40 months in the MED group. Clinical outcomes were evaluated with use of the Japanese Orthopaedic Association Back Pain Evaluation Questionnaire. Although statistically not significant, the degrees of improvement in all five functional scores were greater in the MED group than in the FU group. There was no influence of instability before MED to the clinical outcome, and a large percentage of slipping in the neutral position could lead to inadequate improvement in low back pain after MED, but MED is a useful, minimally invasive operation and can lead to a good clinical outcome for degenerative lumbar spondylolisthesis. Considering the lower invasiveness of MED, this procedure is recommended in case of degenerative lumbar spondylolisthesis with less than 20% slipping. Posterior decompression and fusion using pedicular screws is recommended in case of degenerative lumbar spondylolisthesis with over 20% slipping.

Abstract:
The whitefly, Bemisia tabaci (Gennadius), is one of the most serious agricultural pests worldwide. Bemisia tabaci is a cryptic species complex of more than 40 species among which the invasive MEAM1 and MED species are the most widespread and economically important. Both MEAM1 and MED present intraspecific genetic variability and some haplotypes are reported to be more invasive than others. MED can be further deconstructed into different genetic groups, including MED—Q1 and MED—Q2. However, distinct biological phenotypes discerning the different MED mitochondrial haplotypes are yet to be characterized. Competitive displacement and life-history trials were carried out between MED-Q2 and MEAM1 populations collected in Florida, USA. In addition, a phylogenetic analysis was carried out including populations from previous whitefly competitive displacement studies for identification and comparison of the MED mitochondrial groups. In contrast to other studies with MED—Q1, the MED–Q2 population from Florida is less likely to displace MEAM1 on pepper. In addition, both pepper and watermelon were a more favorable host to MEAM1 compared to MED–Q2 according to the life history trials.

Stimulation of the pedunculopontine nucleus (PPN) is known to induce changes in arousal and postural/locomotor states by activation of such descending targets as the caudal pons and the medioventral medulla (MED). Previously, PPN stimulation was reported to induce prolonged responses (PRs) in intracellularly recorded caudal pontine neurons in vitro. The present study used intracellular recordings in semihorizontal slices from rat brain stem (postnatal days 12–21) to determine responses in MED neurons following PPN stimulation. One-half (40/81) of MED neurons showed PRs after PPN stimulation. MED neurons with PRs had shorter duration action potential, longer duration afterhyperpolarization, and higher amplitude afterhyperpolarization than non-PR MED neurons. PR MED neurons were significantly larger (568 ± 44 μm2) than non-PR MED neurons (387 ± 32 μm2). The longest mean duration PRs and maximal firing rates during PRs were induced by PPN stimulation at 60 Hz compared with 10, 30, or 90 Hz. The muscarinic cholinergic agonist carbachol induced depolarization in all PR neurons tested, and the muscarinic cholinergic antagonist scopolamine reduced or blocked carbachol- and PPN stimulation-induced PRs in all MED neurons tested. These findings suggest that PPN stimulation-induced PRs may be due to activation of muscarinic receptor-sensitive channels, allowing MED neurons to respond to a transient, frequency-dependent depolarization with long-lasting stable states. PPN stimulation appears to induce PRs in large MED neurons using parameters known best to induce locomotion.
