Clinical decision support is an emerging type of healthcare information technology that aims to actively guide doctors' decision-making processes. In its various forms, it can help physicians design treatment regimens, regulate dosage, avoid potentially harmful drug interactions and allergies, order preventative screenings, and even establish a diagnosis. This project is a journalistic investigation of this new technology, its applications, and its effect on the medical profession. It examines two decision support systems, Partners Healthcare's clinical reminder system, and Logical Images' product VisualDx, in order to explore the potential for these technologies and how their use may change the practice of medicine. Through extensive interviews of experts in medicine, healthcare IT, and healthcare policy, it considers the major problems in implementing decision support, with emphasis on how the technology may affect doctors' autonomy, and how physicians' financial and professional incentives may influence how it is used.

Le curve B-spline comunemente usate in modellazione geometrica sono costituite da segmenti polinomiali aventi tutti lo stesso grado. Nella pratica, è spesso necessario modellare curve composte da segmenti polinomiali di grado differente. In questi casi l’utilizzo delle curve B-spline classiche complica inutilmente la modellazione ed necessita di informazioni superflue alla descrizione matematica della curva. Le curve MD-Spline (Multi Degree Spline) sono una generalizzazione delle curve B-spline costituite da segmenti polinomiali di gradi differenti. Con le MD-Spline un progettista CAD ha la possibilità di gestire il grado di ogni segmento in maniera indipendente dagli altri e può rappresentare una determinata forma specificando solo le informazioni strettamente necessarie a definirla. La conseguenza è che modellare con curve MD-Spline si rivela più semplice e rapido rispetto a quanto possibile con curve B-spline. La tesi tratta le MD-Spline descrivendone gli aspetti teorici alla base ed i principali algoritmi di modellazione. In particolare il lavoro di tesi ha prodotto il primo software di modellazione con curve MD-Spline.

The diploma thesis handles economic analysis of the company MD logistika, a.s. The work is divided into theoretical and practical part. The theoretical part describes the indicators of profitability, liquidity, activity, debt, costs and market indicators. DuPont analysis breaking down ROE was used and was followed by a more detailed decomposition of ROA. Within the value indicatiors was used economic value added, which was calculated by two methods. The credibility, bankruptcy and credibility - bankruptcy models were applied to evaluate the company. The operational leverage, financial leverage and more detailed analysis of stocks analysis were performed. In the practical part each procedure, which was elaborated in the theoretical part of company, was applied to the MD logistika, a.s. In the conclusion the company was compared with the closest competitors and the industry averages.

Abstract:
Knowledge about the structure, conformation and dynamics of carbohydrates is important in our understanding of the way carbohydrates function in biological systems, for example in intermolecular signaling and recognition. This thesis is a summary of five papers studying these properties in carbohydrate-containing molecules with NMR spectroscopy and molecular dynamics simulations. In paper I, the ring-conformations of the six-membered rings of two carbaiduronic analogs were investigated. These carbasugars could potentially be used as hydrolytically stable mimics of iduronic acid in drugs. The study showed that the equilibrium is entirely shifted towards the 4C1 conformation. Paper II is an investigation of the conformational flexibility and dynamics of two (1→6)-linked disaccharides related to an oligosaccharide epitope expressed on malignant tumor cells. In paper III, the conformational space of the glycosidic linkage of an alfa-(1→2) linked mannose disaccharide present in N- and O-linked glycoproteins, was studied. A maximum entropy analysis using different priors as background information was used and four new Karplus equations for 3JC,C and 3JC,H coupling constants, related to the glycosidic linkage, were presented. Paper IV describes a structural elucidation of the exopolysaccharide (EPS) produced by Streptococcus thermophilus ST1, a major dairy starter used in yoghurt and cheese production. The EPS contains a hexasaccharide repeating unit of d-galactose and d-glucose residues, which is a new EPS structure of the S. thermophilus species. In paper V, the dynamics of three generations of glycodendrimers were investigated by NMR diffusion and 13C NMR relaxation studies. Three different correlations times were identified, one global correlation time describing the rotation of the dendrimer as a whole, one local correlation time describing the reorientation of the C-H vectors, and one correlation time describing the pulsation of a dendrimer branch.

Abstract:
In order to better manage progress toward improved human welfare, governments and organizations around the world have begun to report on more comprehensive indicators of environmental, social, and economic conditions. The Genuine Progress Indicator (GPI) has proven useful as a measure of economic welfare by incorporating changes in environmental conditions, resource stocks, social capital, income distribution, and other non-marketed economic activity. Studies at the local scale have also found the GPI to be an effective tool for informing debate and stimulating questions about the nature of the economic development process. In this study, the GPI methodology is applied to Baltimore City, Baltimore County, and Maryland in order to explore how sustainable economic welfare in the Baltimore region has changed from 1950-2005. A comparison among per capita GPI trends in four US cities shows Baltimore to have the highest average annual growth rate over the study period. Comparisons are made between per capita GPI and Gross Domestic Product (GDP), the most widely recognized measure of national economic performance. Analysis of the trends at all three scales show that GDP growth does not correlate well with changes in welfare as measure by GPI. This implies that Baltimore City, Baltimore County, and Maryland could be in a period of uneconomic growth, when the social and environmental costs of further economic growth outweigh the benefits of such growth. However, the underlying methods used in sub-national applications of the GPI inevitably lead toward certain results, giving rise to an indicator framework that favors particular policy and development outcomes. This situation is defined as indicator bias. Since indicator bias can inadvertently lead society toward undesirable conditions, key assumptions that contribute to indicator bias in the GPI are tested for how they influence the final GPI results. The costs of crime, long-term environmental damage, and depletion of non-renewable natural resources categories are explored in more depth. GPI is found to be an imperfect measure of true progress, but it is believed to be an improvement over GDP for guiding modern society towards a more sustainable and desirable future. More work is needed to incorporate uncertainty, fine-tune the underlying GPI methodology, and build broad consensus about how to measure economic performance and social progress. By providing information about social, ecological, and economic conditions of the region, though, the Baltimore GPI does inform citizens and decision-makers about a wide range of impacts resulting from the modern ‘GDP growth’ paradigm

Specific Aims: This study was performed to assess the accuracy of the presenting signs and symptoms, diagnostic procedures, and treatments depicted in episodes one through twelve of the fifth season of House, MD. Methods: This study was a descriptive, retrospective evaluation of the accuracy of the first twelve episodes of the fifth season of House, MD. Dependent variables in this study were presenting signs and symptoms, diagnostic procedures, and treatment of the final diagnosis for the primary patient case. A rating of one to four was assigned to each variable, with one being most accurate and four being inaccurate. Statistical analysis consisted of ANOVA and Tukey HSD post-hoc test Main Results: The signs and symptoms had a mean of 2.42 ± 0.669 (95% CI 1.99 to 2.84). The diagnostic tests had a mean of 2.42 ± 1.084 (95% CI 1.73 to 3.11). The treatment had a mean of 1.42 ± 0.9 (95% CI 0.84 to 1.99). ANOVA showed a statistically significant difference among the accuracy ratings of the groups (p = 0.013). Tukey HSD did not show a significant difference between the accuracy of the signs and symptoms and diagnostic tests (p = 1). The test did reveal a statistically significant difference between accuracy of treatment and both the signs and symptoms (p = 0.027) and diagnostic tests (p = 0.027). Conclusions: The treatments shown in House, MD, season five, episodes one through twelve are significantly more accurate than both the diagnostic tests and the presenting signs and symptoms.

Abstract:
Understanding molecular recognition of small molecules by proteins in atomistic detail is key for drug design. Molecular docking is a widely used computational method to mimic ligand-protein association in silico. However, predicting conformational changes occurring in proteins upon ligand binding is still a major challenge. Ensemble docking approaches address this issue by considering a set of different conformations of the protein obtained either experimentally or from computer simulations, e.g. from molecular dynamics. However, holo structures prone to host (the correct) ligands are generally poorly sampled by standard molecular dynamics simulations of the unbound (apo) protein. In order to address this limitation, we introduce a computational approach based on metadynamics simulations called ensemble docking with enhanced sampling of pocket shape (EDES) that allows holo-like conformations of proteins to be generated by exploiting only their apo structures. This is achieved by defining a set of collective variables able to sample different shapes of the binding site, ultimately mimicking the steric effect due to the ligand. In this work, we assessed the method on re-docking and cross-docking calculations. In first case, we selected three different protein targets undergoing different extent of conformational changes upon binding and, for each of them, we docked the experimental ligand conformation into an ensemble of receptor structures generated by EDES. In the second case, in the contest of a blind docking challenge, we generated the 3D structures of a set of different ligands of the same receptor and docked them into a set of EDES-generated conformations of that receptor. In all cases, for both re-docking and cross-docking experiments, our protocol generates a significant fraction of structures featuring a low RMSD from the experimental holo geometry of the receptor. Moreover, ensemble docking calculations using those conformations yielded in almost all cases to native-like poses among the top-ranked ones. Finally, we also tested an improved EDES recipe on a further target, known to be extremely challenging due to its extended binding region and the large extent of conformational changes accompanying the binding of its ligands.

Abstract:
There are twenty standard amino acids that are the structural units of biomolecules and biomaterials such as proteins and peptide amphiphiles (PAs). The focus of this study was to develop accurate transferable coarse-grained (CG) models of those amino acids. In CG models, several atoms are represented together as a single pseudo-atom or "bead," which can allow the modeling of processes like self-assembly of biomolecules and biomaterials through reduction of degrees of freedom and corresponding increased computational speed. A 2:1 to 4:1 mapping scheme, in which a CG bead is comprised of two to four heavy atoms, respectively, and associated hydrogens, has been employed to represent functional groups in the amino acids. The amino acid backbone atoms are modeled as two beads while the side chains are modeled with one to three beads, and each terminus is modeled as one bead. The bonded parameters for the CG models were obtained from bond, angle, and dihedral distributions from all-atom molecular dynamics (MD) simulations of dipeptides. Non-bonded parameters were optimized using the particle swarm optimization (PSO) method to reproduce experimental properties (heat of vaporization, surface tension, and density) of analogues of the side chains, termini, and backbone groups of the amino acids. These CG models were used to study the self-assembly pathways and mechanisms of the PA c16-AHL3K3-CO2H in the presence of explicit CG water.
Master of Science
In this study, models of the amino acids were developed for computer simulations. In these models, the amino acids are represented as a collection of two or more “beads” bonded together rather than as a collection of atoms bonded together. The beads were created in such a way that their characteristics reflect those of the molecules and atom groups that they represent. This was accomplished in part by selecting parameters for each bead that approximately reproduce experimental properties (density, heat of vaporization, and surface tension) and structures (bonds and angles) of the molecules and atom groups of which they are representative. Amino acids can link together to form short segments, known as peptides, or longer chains that form proteins. The bead models that were developed in this study can be linked together in the same way. They can also be linked with other beads that represent other atom groups—carbon groups of a carbon chain, for example. Certain types of molecules known as peptide amphiphiles (PAs) are often composed of amino acids and a carbon chain. The amino acid bead models were created especially to study these molecules, so once the models were developed, they were used in computer simulations to represent PAs. Many types of PAs can automatically assemble into structures that resemble fibers, and it is this behavior in particular that was studied. By using these models in computer simulations, we are able to see things that cannot be seen in a lab with a microscope or other lab tools. This may help with future efforts to study and design molecules such as PAs which show promise for medical applications like drug delivery.

Syftet med denna uppsats har varit att studera hur mansroller uttrycks utifrån manlig vänskap i tv-serien ”House M.D.” Till min hjälp har jag använt mig av frågeställningarna: Hur skildras mansrollen hos karaktären Gregory House i umgänget med James Wilson? Hur skildras mansrollen hos karaktären James Wilson i umgänget med Gregory House? Hur skildras relationen mellan Gregory House och James Wilson? De centrala teorierna har utgått ifrån Connells beskrivning av maskuliniteter, med betoning på hegemonisk maskulinitet och underordnad maskulinitet samt Birds beskrivning av homosocialitet som beskriver sociala praktiker i manligt, heterosexuellt umgänge med utgångspunkt ur en hegemonisk maskulinitet. Fokus har också legat på att ge läsaren en tydlig bakgrund i hur synen på mannen förändrats, hur vänskap mellan män sett ut historiskt och förändrats över tid samt vilka mekanismer som legat till grund för dynamiker inom manlig vänskap. Materialet har analyserats utifrån en metod som är framtagen för just TV-program, ”How to Study Television” men jag har skapat en specifikt anpassad metod utifrån detta som fokuserar på narrativet. Studien visar på tydliga inslag av de homosociala reglerna i umgänget mellan House och Wilson, främst i form av tävlan och känslomässig otillgänglighet. Dock tycks detta samt den typ av hegemonisk maskulinitet som House följer vara förlegad och närmast självdestruktiv. Wilson som skildras som varande i god kontakt med sina känslor får hjälpa House med Houses känslomässiga hämningar, men Wilson gör det via verktyget tävlan – en homosocialt accepterad form. Slutsatsen är att ”House M.D.” kritiserar mansrollen som den skildras hos Gregory House samt visar på de svårigheter som följer av att umgås inom de homosociala ramarna.

This thesis is an exploration and analysis of what audiences may be learning about medical professionalism from the fictional television (TV) medical drama House MD. Fictional TV medical dramas are an important form of medical narrative in that they are usually created by writers with no medical training. As such, they carry a higher risk of portraying the practice of medicine inaccurately. A review of the scholarly literature reveals that there is a precedent for fictional TV medical dramas to affect viewers' perception of the practice of medicine and health behaviours, and viewers' understanding of medical ethics issues. It also reveals strong empirical evidence that TV medical dramas can affect audience's perceptions of physicians' character. A thorough review of the first two seasons of House MD reveals 20 lessons on professionalism (i.e. lessons on interactions with colleagues and patients, medical ethics, and professional competence) that the title character, Gregory House, is imparting to viewers. All 20 lessons are in direct conflict with established charters on professionalism. Arguments are made for the programme's potential to negatively affect patient access to care, physician-patient relations, interactions between healthcare professionals, and applications to medical school.

Modularization (MD) in the auto industry is relatively new when compared to its use in other industrial areas. It is regarded as the third revolution in the history of the auto industry after Henry Ford's assembly line production system and Toyota's JIT (just in time) management. Modularization brought a major reorganization to the automotive parts supplier industry by realizing the firm's strategic positional advantage through mass customization While academic interest in this area also experienced significant growth in recent years, few empirical studies have been conducted because it is a difficult task to operationalize the multi-faceted, complex modularization. Although modularization has become a global trend in the auto industry, studies show that different characteristics of modularization are exhibited in various international automobile markets. China has been recognized as the largest car market and manufacturer in the world in recent times, yet the industrial structure is quite different from leading countries such as the U.S. and Japan. More than sixty percent of the vehicles in China are produced under foreign brands by joint venture factories. Despite the importance and uniqueness of the Chinese auto market, only a few conceptual scholarly works have been conducted touching on the concept of modularization. This means that there is not a deep understanding of this topic as it exists in the Chinese auto market. To emphasize, no literature was found among the existing works about the cultural impact on modularization and its outcomes in China. The purpose of this study is to fill in such a gap with an empirical analysis on the impact of modularization on the auto industry in China. Guanxi as a unique cultural phenomenon in China is covered in this study. Internalization theory, transaction cost economics, the knowledge based view of the firm, and the OLI model is reviewed as a base for the study. In practice, this study will help managers in the auto industry make a more scientific decision of whether and how they should go into modularization, especially in the Chinese market. It is also helpful for automakers like GM and Ford who have an ambitious parts procurement plan from China to have a better understanding of the Chinese auto industry.

Die Visualisierung bisher nicht nachweisbarer Defekte in hochwertigem Silizium konnte durch die Entwicklung neuer hochempfindlicher Mikrowellendetektionsverfahren erreicht werden. Dies eröffnet eine Vielzahl von Möglichkeiten zur kontaktlosen und zerstörungsfreien Charakterisierung von Halbleitern. Insbesondere sind bisher unzugängliche Defekte in Silizium und selbst in dünnen epitaktischen Schichten nachweisbar. Elektrische Eigenschaften von Halbleitern wie Lebensdauer, Beweglichkeit und Diffusionslänge können bei kleinen Injektionsraten mit einer Ortsauflösung, die nur durch die Diffusionslänge der Ladungsträger limitiert ist, gemessen werden. Aufgrund der um mehrere Größenordnungen verbesserten Empfindlichkeit kann über ein Mikrowellenabsorptionssignal die Ladungsträgeremission aus Defekten selbst bei qualitativ hochwertigstem Silizium untersucht werden. Dies ermöglicht die Vorhersage elektrischer Eigenschaften von Bauelementen bereits an den Ausgangswafern.

O uso de computação móvel e ubíqua tem sido estimulado pela grande difusão dos dispositivos móveis, redes sem fio e sistemas de localização. Neste contexto, diversas aplicações estão surgindo em diferentes áreas, como educação, entretenimento, comércio e transportes. Esta dissertação propõe o MD-UTS, um modelo para o desenvolvimento de sistemas ubíquos de transporte. O modelo permite o gerenciamento de diferentes tipos de veículos em uma determinada região. Os veículos são gerenciados de acordo com seus perfis, criando um registro histórico de seu movimento. O modelo fornece informações através de serviços web que podem ser utilizadas por diferentes aplicações. A dissertação também descreve duas aplicações que usam o MD-UTS para gerenciar o transporte urbano. O MD-UTS foi integrado no simulador de transporte urbano chamado SUMO (Simulação da Mobilidade Urbana) para gerar dados sobre os veículos. Os resultados demonstram a viabilidade da proposta, facilitando a construção de aplicações direcionadas para o sistema de transporte.
The use of mobile and ubiquitous computing has been stimulated by the widespread diffusion of mobile devices, wireless networks and location systems. In this context, numerous applications are emerging in different areas such as education, entertainment, commerce and transport. This thesis proposes the MD-UTS, a model for development of ubiquitous transport systems. The model allows management of different types of vehicles in a given region. The vehicles are managed according to their profiles, making a historical record of their movement. The model provides information through web services that can be used by different applications. The thesis also describes two applications that use the MD-UTS to manage urban transport. The MD-UTS has been integrated into urban transport simulator called SUMO (Simulation of Urban Mobility) to generate data about the vehicles. The results demonstrate the feasibility of the proposed, facilitating the construction of applications directed to the transport system.

OBJECTIVES: The purpose of this project was to analyze the accuracy of season two, episodes 13-­‐24 of the popular medical drama, House, MD. METHODS: This study was a descriptive retrospective analysis of the second half of season two of House, MD. The accuracy of the presenting signs and symptoms, diagnosis, and treatment in each episode was rated on a scale of one to four. A rating of one described a correct and usual representation. A rating of two described a correct but somewhat unusual representation. A rating of three described a correct but extremely unusual representation. A rating of four described an incorrect representation. Each researcher independently rated the episodes, and then a collaborative rating was agreed upon by all researchers. RESULTS: Results of the ANOVA test demonstrated a statistically significant difference between the three dependent variables (p=0.003). The Tukey post-­‐hoc test confirmed a significant difference between the accuracy of treatment when compared with signs and symptoms (p=0.003), and with diagnosis (p=0.022). The average rating for the treatment variable was 1.50 (±0.707), whereas the average ratings for the signs and symptoms and diagnosis variables were 2.80 (±0.919) and 2.50 (±0.707), respectively. CONCLUSIONS: The treatments presented in the last twelve episodes of season two of House, MD were more accurate than both the presenting signs and symptoms and the diagnosis.

OBJECTIVES: The purpose of this work was to quantify the magnitude of factual inaccuracies in the first twelve episodes of the second season of the medical drama, House, MD. METHODS: This study was a descriptive retrospective evaluation of the first twelve episodes in the second season of the popular television show, House, MD. The degree of accuracy of the signs and symptoms (presentation), diagnosis, and treatment for the one major disease portrayed in each show was rated on a scale of one to four. Each researcher evaluated these ratings independently, and after subsequent collaborative discussion, a combined rating was determined for all the data. RESULTS: Both a three-way ANOVA and Kruskal-Wallis data analyses found significance among the data. The Tukey HSD post hoc test revealed a significant difference between the accuracy of signs and symptoms to treatment variables (p = 0.045). Analysis by the Mann-Whitney U test also determined that a significant difference was evident between the accuracy of signs and symptoms when compared with treatment (p = 0.022). CONCLUSIONS: The treatments presented in the twelve episodes evaluated were determined to be more accurate than the show's description of the patient's presenting signs and symptoms.

Molecular dynamics simulations were conducted on a system consisting of a decamer DNA solvated by trehalose and water (molecular ratio= 1:2), to mimic a relatively dry state for the DNA molecule. Simulations were performed at two different temperatures, 300 K and 450 K. The B-form DNA structure was shown to be stable at both temperatures. The analysis of hydrogen bonds between trehalose/water and DNA revealed that trehalose and backbone DNA formed the largest number of hydrogen bonds and thus constituted the major effect of structural protection for DNA. The number of hydrogen bonds formed by each OH group of trehalose with the backbone DNA was compared. Different types of trehalose-DNA interactions were analyzed, with no prevalent pattern recognized. Diffusion constants for trehalose and water were also calculated, suggesting a glassy/viscose state of the simulation system. It is believed that trehalose protects DNA in the dry state through the network of hydrogen bonds built by the sugars, which reduces the structural fluctuations of DNA and prevents its denaturation.

Abstract:
In this thesis, we focus on theoretical investigations on metal interfaces where many heterogeneous chemical reactions take place. Surface-enhanced Raman scattering (SERS) spectroscopy and the modern electrochemical methods are important in-situ techniques that have been widely employed for a variety of applications. Theoretical simulations have become an indispensable tool to infer the molecular details of interfacial structures that are not directly accessible from experimental measurements. In this context, we have proposed several new theoretical models for both SERS and interfacial electrochemistry, which allow us to provide molecular-level understanding of the interfacial structures under the realistic experimental conditions.   The first part of the thesis has addressed the basic theory of SERS that offers the vibrational structure of the interfacial molecules. It is well known that the huge enhancement of Raman intensity in this technique can be attributed to two independent factors, namely the physical and chemical enhancements. The former is resulted from the enhanced electromagnetic field induced by the plasmonic excitations, while the latter comes from the changing of interaction between the molecule and the surface. The interplay between these two enhancement factors, which has long been an issue of debate, is revealed in this thesis. They are coupled through molecular polarizability. A practical computational approach is proposed and used to demonstrate the importance of the coupling on different molecular systems. It is found that for certain systems the coupling factor can be as large as 106. This finding is of great importance towards a comprehensive understanding of the SERS mechanisms and a quantitative prediction of the enhancement factor.   The other part of the thesis is devoted to the theory of interfacial electrochemistry, in particular the effects of water solution. A novel protocol that combines classical molecular dynamics (MD) and the first principles density functional theory (DFT) calculations is proposed to address the statistical behavior of interfacial properties. Special attention has been paid to the work function of Pt(111) surface and CO adsorption energy on Pt(111) surface in aqueous solution. It has been found that in this case the work function of Pt surface illustrates a surprisingly broad distribution under the room temperature, sheds new light on the understanding of reaction activity of the surface. The proposed protocol is able to provide results in very good agreement with experiments and should be applied routinely in future studies.
