Clinical decision support is an emerging type of healthcare information technology that aims to actively guide doctors' decision-making processes. In its various forms, it can help physicians design treatment regimens, regulate dosage, avoid potentially harmful drug interactions and allergies, order preventative screenings, and even establish a diagnosis. This project is a journalistic investigation of this new technology, its applications, and its effect on the medical profession. It examines two decision support systems, Partners Healthcare's clinical reminder system, and Logical Images' product VisualDx, in order to explore the potential for these technologies and how their use may change the practice of medicine. Through extensive interviews of experts in medicine, healthcare IT, and healthcare policy, it considers the major problems in implementing decision support, with emphasis on how the technology may affect doctors' autonomy, and how physicians' financial and professional incentives may influence how it is used.

Full textSpecific Aims: This study was performed to assess the accuracy of the presenting signs and symptoms, diagnostic procedures, and treatments depicted in episodes one through twelve of the fifth season of House, MD. Methods: This study was a descriptive, retrospective evaluation of the accuracy of the first twelve episodes of the fifth season of House, MD. Dependent variables in this study were presenting signs and symptoms, diagnostic procedures, and treatment of the final diagnosis for the primary patient case. A rating of one to four was assigned to each variable, with one being most accurate and four being inaccurate. Statistical analysis consisted of ANOVA and Tukey HSD post-hoc test Main Results: The signs and symptoms had a mean of 2.42 ± 0.669 (95% CI 1.99 to 2.84). The diagnostic tests had a mean of 2.42 ± 1.084 (95% CI 1.73 to 3.11). The treatment had a mean of 1.42 ± 0.9 (95% CI 0.84 to 1.99). ANOVA showed a statistically significant difference among the accuracy ratings of the groups (p = 0.013). Tukey HSD did not show a significant difference between the accuracy of the signs and symptoms and diagnostic tests (p = 1). The test did reveal a statistically significant difference between accuracy of treatment and both the signs and symptoms (p = 0.027) and diagnostic tests (p = 0.027). Conclusions: The treatments shown in House, MD, season five, episodes one through twelve are significantly more accurate than both the diagnostic tests and the presenting signs and symptoms.

In this study, models of the amino acids were developed for computer simulations. In these models, the amino acids are represented as a collection of two or more “beads” bonded together rather than as a collection of atoms bonded together. The beads were created in such a way that their characteristics reflect those of the molecules and atom groups that they represent. This was accomplished in part by selecting parameters for each bead that approximately reproduce experimental properties (density, heat of vaporization, and surface tension) and structures (bonds and angles) of the molecules and atom groups of which they are representative. Amino acids can link together to form short segments, known as peptides, or longer chains that form proteins. The bead models that were developed in this study can be linked together in the same way. They can also be linked with other beads that represent other atom groups—carbon groups of a carbon chain, for example. Certain types of molecules known as peptide amphiphiles (PAs) are often composed of amino acids and a carbon chain. The amino acid bead models were created especially to study these molecules, so once the models were developed, they were used in computer simulations to represent PAs. Many types of PAs can automatically assemble into structures that resemble fibers, and it is this behavior in particular that was studied. By using these models in computer simulations, we are able to see things that cannot be seen in a lab with a microscope or other lab tools. This may help with future efforts to study and design molecules such as PAs which show promise for medical applications like drug delivery.

O uso de computação móvel e ubíqua tem sido estimulado pela grande difusão dos dispositivos móveis, redes sem fio e sistemas de localização. Neste contexto, diversas aplicações estão surgindo em diferentes áreas, como educação, entretenimento, comércio e transportes. Esta dissertação propõe o MD-UTS, um modelo para o desenvolvimento de sistemas ubíquos de transporte. O modelo permite o gerenciamento de diferentes tipos de veículos em uma determinada região. Os veículos são gerenciados de acordo com seus perfis, criando um registro histórico de seu movimento. O modelo fornece informações através de serviços web que podem ser utilizadas por diferentes aplicações. A dissertação também descreve duas aplicações que usam o MD-UTS para gerenciar o transporte urbano. O MD-UTS foi integrado no simulador de transporte urbano chamado SUMO (Simulação da Mobilidade Urbana) para gerar dados sobre os veículos. Os resultados demonstram a viabilidade da proposta, facilitando a construção de aplicações direcionadas para o sistema de transporte.
The use of mobile and ubiquitous computing has been stimulated by the widespread diffusion of mobile devices, wireless networks and location systems. In this context, numerous applications are emerging in different areas such as education, entertainment, commerce and transport. This thesis proposes the MD-UTS, a model for development of ubiquitous transport systems. The model allows management of different types of vehicles in a given region. The vehicles are managed according to their profiles, making a historical record of their movement. The model provides information through web services that can be used by different applications. The thesis also describes two applications that use the MD-UTS to manage urban transport. The MD-UTS has been integrated into urban transport simulator called SUMO (Simulation of Urban Mobility) to generate data about the vehicles. The results demonstrate the feasibility of the proposed, facilitating the construction of applications directed to the transport system.

Full textOBJECTIVES: The purpose of this project was to analyze the accuracy of season two, episodes 13-‐24 of the popular medical drama, House, MD. METHODS: This study was a descriptive retrospective analysis of the second half of season two of House, MD. The accuracy of the presenting signs and symptoms, diagnosis, and treatment in each episode was rated on a scale of one to four. A rating of one described a correct and usual representation. A rating of two described a correct but somewhat unusual representation. A rating of three described a correct but extremely unusual representation. A rating of four described an incorrect representation. Each researcher independently rated the episodes, and then a collaborative rating was agreed upon by all researchers. RESULTS: Results of the ANOVA test demonstrated a statistically significant difference between the three dependent variables (p=0.003). The Tukey post-‐hoc test confirmed a significant difference between the accuracy of treatment when compared with signs and symptoms (p=0.003), and with diagnosis (p=0.022). The average rating for the treatment variable was 1.50 (±0.707), whereas the average ratings for the signs and symptoms and diagnosis variables were 2.80 (±0.919) and 2.50 (±0.707), respectively. CONCLUSIONS: The treatments presented in the last twelve episodes of season two of House, MD were more accurate than both the presenting signs and symptoms and the diagnosis.

Full textOBJECTIVES: The purpose of this work was to quantify the magnitude of factual inaccuracies in the first twelve episodes of the second season of the medical drama, House, MD. METHODS: This study was a descriptive retrospective evaluation of the first twelve episodes in the second season of the popular television show, House, MD. The degree of accuracy of the signs and symptoms (presentation), diagnosis, and treatment for the one major disease portrayed in each show was rated on a scale of one to four. Each researcher evaluated these ratings independently, and after subsequent collaborative discussion, a combined rating was determined for all the data. RESULTS: Both a three-way ANOVA and Kruskal-Wallis data analyses found significance among the data. The Tukey HSD post hoc test revealed a significant difference between the accuracy of signs and symptoms to treatment variables (p = 0.045). Analysis by the Mann-Whitney U test also determined that a significant difference was evident between the accuracy of signs and symptoms when compared with treatment (p = 0.022). CONCLUSIONS: The treatments presented in the twelve episodes evaluated were determined to be more accurate than the show's description of the patient's presenting signs and symptoms.
