Full text
Abstract:
This chapter begins with a discussion of Neil MacCormick’s institutionalist approach to legal phenomena, and argues that this theoretical framework has value as a way to study multiple offense sentencing (MOS). The most thorough completed empirical research into MOS, by Austin Lovegrove in Victoria, Australia, is then considered, alongside the leading Victorian case of Azzopardi v. R. Congruently with the expectations of institutionalism, this analysis uncovers several separate normative principles used by judges in MOS practice. These results are discussed through the lens of what can be described as “post-desert theory.” Overall, the analyses in the chapter are intended to pave the way for the development of a more coherent answer to the question: “what principles should optimally guide sentencers when dealing with cases involving multiple offenses?”

Abstract:
Aside from its paramount diagnostic role, imaging techniques, particularly magnetic resonance imaging (MRI), provide unparalleled insights into multiple sclerosis (MS) by assessing the spatiotemporal dynamics of the associated inflammation and neurodegeneration. This dynamical view, predicated on interrogation of individuals with MS at multiple time points, is impossible with pathology. The chapter approaches MRI in MS from this perspective, describing features related to lesion development and location, as well as assessment of global and regional damage. It summarizes current knowledge, addresses the limitations of that knowledge, and suggests ways in which imaging can advance future research.

Abstract:

Abstract:
This chapter sets out the theoretical framework of the book, and develops the hypotheses to be tested. It argues that the attitudes of MPs to party unity will be shaped by the career, electoral, and other political incentives they face, and so reflect the extent to which MPs’ experiences of the expectation to maintain party unity is positive or negative. It then develops three measurements of policy incentives to dissent in floor votes in multiparty legislatures, each corresponding to a different assumption about how MPs approach floor voting. Further, the chapter discusses the case selection and the quality of the data used in the book.

Full text
Abstract:
Mechanical circulatory support (MCS) offers life-saving treatment options for patients with advanced heart failure, cardiogenic shock, and acute cardiopulmonary failure. With the development of short-term, long-term, left-sided, right-sided, biventricular, and cardiopulmonary support options, the complexity surrounding this field has evolved substantially in the past decade. This textbook seeks to provide a logical and systematic framework for managing patients who require MCS therapies. In chapters 2–9, a comprehensive approach to selecting and preparing patients for MCS interventions is described. In chapters 10–23, the range of currently available devices is reviewed with updates on the most up-to-date clinical experiences based on recently published outcomes. Chapters 24–30 describe the current state of the art in perioperative strategies for achieving long-term success. Finally, chapters 31–46 outline the myriad of clinical challenges that often occur in the postoperative period, including some of the frequently encountered adverse events that are unique to the physiology associated with this technology. The fundamental principles included in this textbook will provide a framework for the MCS provider that can serve as a road map amidst the rapidly evolving landscape created by the technology pipeline within this industry.

Full text
Abstract:
The kidneys play a critical role in maintaining normal serum calcium and phosphorus concentrations, under the regulation of three main hormones: parathyroid hormone, calcitriol, and fibroblast growth factor 23. With the progression of chronic kidney disease (CKD), most patients develop CKD–mineral and bone disorder (CKD-MBD), which is a systemic disorder involving derangement in mineral metabolism, renal osteodystrophy, and extraskeletal calcification. Disturbances in mineral metabolism develop early in CKD and include phosphate retention, hypocalcaemia, vitamin D deficiency, and hyperparathyroidism. Renal osteodystrophy involves pathologic changes of bone morphology related to progressive CKD and is quantifiable by histomorphometry, based on bone biopsy. CKD-MBD is associated with significant morbidity, including bone loss, fractures, cardiovascular disease, immune suppression, as well as increased mortality. As the disorder begins early in the course of CKD, a proactive approach with intervention is important. Therapeutic strategies could then be employed to prevent and correct these disturbances, aiming to improve cardiovascular outcomes and survival. Current practice guidelines for CKD-MBD are based on insufficient data and high-quality studies are required before specific treatment can be advocated strongly.

Abstract:
Chronic kidney disease-mineral and bone disorders (CKD-MBD), calcium, phosphate, and parathyroid hormone are biomarkers of mortality and cardiovascular risk. Hyperphosphataemia is a prominent and pathophysiologically most plausible risk indicator. Calcium balance and load appear to be more important than serum concentrations. Parathyroid hormone is a less reliable marker with a relatively wide range extending above that applicable for a normal population especially when used as a singular laboratory parameter without additional assessment of bone metabolism, for example, bone-specific alkaline phosphatase and bone biopsy. There is not a single prospective controlled hard-outcome study that provides us with unequivocal evidence that such an isolated laboratory parameter-based treatment approach will lead to significant clinical improvements. As CKD-MBD is complex, clinical decisions would be made easier by informative prospective trials.

Full text
Abstract:
Woolf scholars have been at the forefront of bringing economic and especially Keynesian perspectives to bear on modernist literature. This chapter makes the case instead for a sociological approach. Whereas Keynes promoted a separation of egoism and altruism, Marcel Mauss called for a “new morality” that would be a mixture of egoism and altruism. For her part, Woolf, too, conceptualized the modern mind as a mix of contradictory feelings both in her nonfiction and in Mrs. Dalloway, published the same year as Mauss’s The Gift. Drawing historical and conceptual connections between the novel’s representation of feminine consciousness and Claude Lévi-Strauss’s structural re-reading of Mauss, the chapter argues that, in Mrs. Dalloway, everyday metropolitan experience is an experience of gift exchange and, more specifically, of hospitality—of being at once open and closed to the thought of other people in ways that are shaped by gender, class, and nationality.

Full text
Abstract:
At each point across the disease spectrum, the MS nurse plays a pivotal role in the symptom management and comprehensive care of patients with multiple sclerosis. Multiple sclerosis most often affects young adults in the prime of life, derailing and interrupting goals and plans for the future. Whereas a neurologist might look at the disease burden of the brain, the MS nurse looks at the burden on a patient’s life. This nursing approach enables the MS nurse to help patients adjust and adapt to changes and symptoms as disease progression occurs and to empower them to live with the highest quality of life possible. As patients adjust to major life changes or advancing levels of disability, MS nurses can offer support and instill a sense of hope. This support is also important in promoting empowerment and wellness for persons with MS.

Full text
Abstract:
This chapter examines the crucial role that political parties play in maintaining a unified voting bloc in parliament. This party-based approach sets it apart from most existing studies in this area. The focus of this chapter is on the factors that incentivize MPs to vote in a unified manner. The chapter tests three hypotheses: (1) whether party unity is improved by greater party organizational strength; (2) whether the greater threat of disciplinary sanctions increases party unity; and (3) whether greater access to resources by MPs reduces party unity. The authors use the Political Party Database (PPDB) dataset to test these hypotheses in thirteen of Europe’s democracies, finding strong support for the third hypothesis, some support for the first hypothesis, but little support for the second hypothesis. This study adds an important new dimension to research on how institutions affect party unity by showing the distinct role party organizations can play in this regard.

Full text
Abstract:
In the post-financial crisis era, conduct regulation has permeated every facet of an institution’s operations. Not only does the legal framework for the delivery of redress impact the legal risk profile of an activity, but the very ‘approach’ of the regulator exacerbates the legal risk inherent in that activity. This issue is of specific relevance in the context of mis-selling cases, as the regulator may direct (or otherwise sponsor or influence) the terms of redress and in ways that may not be perfectly aligned to a strict application of liability under English common law. This chapter first explores post-financial crisis litigation and criminal charges and the influence of government and regulators on the provision of redress. It then analyzes a legal brake on liability — specifically, the impact of contractual estoppel in mis-selling cases. It concludes with a review of the regulatory factors relevant to redress and the leveraging of post-crises sentiment to promote extra-regulatory schemes.

Abstract:
This chapter “lifts the roof of the household” across the irrigation and water, sanitation, and hygiene (WASH) sub-sectors in agrarian low- and middle-income settings. Focusing on age-old intersections between gender, class, and agrarian technology, the chapter explores how colonial conquest was served by the ideology of the male breadwinner‒ female housewife as a divide-and-rule process to vest control over people, land, and water. After independence, the same ideology enabled top-down services in both sub-sectors and also marginalized women. This is contrasted with implications of global policy commitments to gender-equal households for the water sector. In particular, evidence of the multiple-use water services (MUS) approach is examined. This inclusive, people-driven water services approach meets both women’s and men’s multiple domestic and productive needs. Overcoming the same administrative silos in human rights frameworks, a gender-equal human right to water for livelihoods is proposed.
