Abstract:
Abstract Background. Adjuvant denosumab treatment improved bone-health related outcomes in early breast cancer (BC) patients with discordant survival results. We undertook a systematic review and meta-analysis of randomized controlled trials (RCTs) to assess efficacy and safety of adjuvant denosumab in addition to standard anticancer therapy. Methods. PubMed, CENTRAL, Scopus, Embase, clinicaltrials.gov and key oncological meetings websites were screened to identify potentially eligible RCTs based on the PICOs model: P) Participant: pre and postmenopausal early BC patients; I) Intervention: adjuvant denosumab; C) Comparator: placebo; O) Outcomes: Disease-free survival (DFS), Bone Metastasis-free survival (BMFS), Overall Survival (OS), fracture incidence and time to first fracture were adopted as survival endpoints, and Adverse Events, Serious Adverse Events, Osteonecrosis of the Jaw (ONJ) and Atypical Femur Fractures (AFF) as safety endpoints; S) Study design: phase III RCTs. Risk of bias was assessed with Cochrane Collaboration Risk of Bias Tool. Pooled hazard ratios (HR), risk ratios (RR), risk differences (RD) and respective confidence intervals (CI) were computed using both a fixed and a random effect model. Subgroup analyses based on menopausal status, hormone receptor and HER2 status and immunophenotype were performed. Results. Two phase III RCTs were included (ABCSG-18, D-CARE), for an overall population of 7929 early BC patients receiving denosumab or placebo. Denosumab addition to standard of care anticancer treatment showed no difference in DFS (HR 0.93; 95% CI 0.75-1.16, p=0.53), BMFS (HR 0.90; 95% CI 0.75-1.07, p=0.23) and OS (HR 0.92; 95% CI 0.72-1.17, p=0.49). In hormone receptor-positive/HER2 negative patients, denosumab significantly prolonged both DFS (HR 0.88; 95% CI 0.78-0.99, p=0.04) and BMFS (HR 0.83; 95% CI 0.71-0.97, p=0.02). No interaction was found between denosumab addition and menopausal status. Fracture incidence (RR 0.79; 95% CI 0.70-0.89, p< 0.01) and time to first fracture (HR 0.76; 95% CI 0.66-0.87, p< 0.01) were also improved with denosumab. No association between denosumab addition and overall toxicity was seen and no difference was observed in terms of ONJ and AFF between the 60mg every 6 months schedule and placebo (RD 0.001, 95% CI from -0.001 to 0.002, p=0.48) Conclusions. Findings from this meta-analysis validate the role of denosumab as a highly effective anti-resorptive agent. We provide robust evidence that its addition to standard anticancer treatment significantly improves survival outcomes in hormone receptor-positive/HER2 negative early BC patients, suggesting that the implementation of denosumab use in combination with endocrine therapy in this patient population should be reconsidered. Abstract:
Abstract Background Reduction of Invasive meningococcal disease (IMD) burden is one of the public health main priorities in Argentina. Since 2017, MenACWY vaccination was implemented for infants (3-5 and 15 months) and adolescents (11 years). However, epidemiological studies have been scarce in recent years. Considering the changing dynamic of the disease, specially related to serogroups circulation, we decided to carry out an epidemiological review and update IMD information that contributes to assess the best prevention strategy. Methods Retrospective 2010-2019 observational and descriptive study, based on Argentina Ministry of Health secondary data. Main outcomes included IMD incidence rates, serogroups distribution, mortality and case fatality ratios (CFR) stratified by year, age groups, (infants: 0-11 months; 1 to 4 y; 5 to 19 y; >20 to 29 y; >30 to 39 y; 40 to 49 y; 50 to 59 y; >60 y) and regions. Results 1,972 IMD cases were notified in the study period. Incidence reached its maximum in 2013 (0.69/100,000) and steadily declined to 0.22-0.23/100,000 in 2018-2019. Highest incidence rates were observed in infants (exceeding 14/100,000 in 2012, with no adolescent’s peak) and in the center of the country (reaching a value of 0.9/100,000 in 2013). There were 167 meningococcal deaths during 2010-2019 with a downtrend since 2014. Mortality rates were 18 times higher in infants versus all other age groups. CFR for the entire period was 8.5% and increased with age (< 7% in < 20y to 26% in >60y). Serogroup identification from notified cases increased from 73% in 2010 to 91% in 2019. From all characterized strains 47% and 42% corresponded to serogroup B and W, respectively. A higher predominance of serogroup B over W was observed during the last 5 years, especially in infants and in the central region. Conclusion The highest burden of meningococcal disease focused on children < 1yoa with a growing predominance of serogroup B. This trend highlights the importance of vaccination at very early age adding protection against serogroup B in this age group. These results will contribute to the process of evidence-based decision making. Nel panorama degli studi di storia della critica figurativa del secondo dopoguerra, manca una disamina delle posizioni degli autori della pagina d’arte del periodico romano “Il Mondo” . Studi - raccolte di documenti, indagini sul rapporto tra arte e critica militante in Italia e affondi sulla trasformazione del vocabolario visivo tra anni Quaranta e Cinquanta - che, anche quando hanno centrato la propria attenzione su periodici o giornali non specialistici, hanno sempre finito per tenere fuori fuoco la definizione dei tratti propri della specola sulle arti costituita dal settimanale diretto da Mario Pannunzio. L'analisi delle pagine culturali de “Il Mondo” dimostra l'insufficienza di un ragionamento costruito sulla base di una mera contestualizzazione degli scritti figurativi comparsi nel settimanale nel dibattito critico del secondo Novecento. “Il Mondo” è stato espressione di un clan, della élite raccoltasi a partire dagli anni Trenta attorno a Mario Pannunzio; un gruppo che, in alcuni dei suoi protagonisti e in piena continuità politica e culturale, è poi confluito nell'avventura de “L’Espresso” di Benedetti e Scalfari. Sulla costituzione del gruppo vale la pena di indugiare, resistendo alla tentazione di una distinzione tra questioni prettamente storico-artistiche e culturali in senso generale, o addirittura tra scelte di campo in materia di arti visive e battaglie politiche e civili: sono gli stessi autori della pagina d’arte de “Il Mondo” ad indicare questa strada, in un dibattito che, negli anni dell’immediato dopoguerra, era giocoforza carico di ragioni ideologiche. Obiettivo del presente lavoro è quello di fare luce sulla sintesi operata nell’ambito della redazione del settimanale tra le due componenti salienti del gruppo gravitante attorno a Pannunzio. Serbatoi di uomini e di idee, essenzialmente cultura di fronda per quel che attiene agli autori delle pagine culturali, intellettualità liberaldemocratica per quanto attiene alla definizione della rotta politica, economica e civile del settimanale.
