Abstract:
Czech names and Czech in Poland in the 20th centuryThe article was based on more than 600 Czech surnames from Słownik nazwisk współcześnie w Polsce używanych used by Kazimierz Rymut. The article shows the state of research of Czech anthroponymy in Poland, which was treated by researchers quite marginally. An outline of the history of contacts and Czech settlements in Poland was also presented, which had an important impact on the extension of the Polish anthroponymic system with the Czech genetic names. The four most important areas on which the Czech settlement was developed were: Galicia Galicja, “Czech Corner” Czeski Zakątek, Settlement areas areas of settlements of the Czech brothers and Silesia Śląsk. The article compares the data of names in Poland in the twentieth century with the map of the Czech population, which mostly overlapped. Česká příjmení a Češi v Polsku ve 20. stoletíTento článek byl založen na více než 600 českých příjmeních excerpovaných ze Słowniku nazwisk współcześnie używanych ed. Kazimierze Rymuta. Článek představuje současný stav výzkumu českých příjmení v polském antroponomastickém systému, který doposud byl považován vědci spíše okrajově. Zaroveň představuje také stručnou historii česko-polských vztahu a českého usázení v Polsku, které měly významný vliv na expanzi geneticky českých příjmení do polského antroponomastického systému. Autorka vyčlenila 4 nejdůležitější oblasti, ve kterých vznikaly největší české společnosti, a to: Galicie, Český koutek, místa, na kterých se usazeli čeští exulanti členové Jednoty bratrské a Slezsko. Ve svém článku autorka srovnává údaje tykající se českých příjmení v Polsku ve 20. století s mapou Čechů bydlících v Polsku, a ty se ve vetší míře shoduji.

Abstract:
Among the woody species found in Haiti, two are of primary importance: Ceiba pentandra, known in the region as mapou, and its counterpart, Ficus thonningii (or Figuier maudit). Both species are renowned in various places worldwide for their medicinal and pharmaceutical attributes. However, in Haiti, they are primarily associated with entrenched meanings related to the cultural beliefs and practices of native Haitians, who staunchly maintain the supernatural potentialities of these flora. Several researchers attest to their numerous healing benefits for the symptoms of many illnesses, but medical data have not been sufficiently compelling despite this pervasive belief system. Longitudinal inquiries could uncover a cultural motive for this fixation on unproven facts, at the risk of underestimating many other valuable attributes of the trees. In this study, general system theory was applied to analyze aspects of Haitian culture relative to individual beliefs about those species. The rationale of this phenomenon could be a case of communicative autopoiesis, whereby a system of operations creates a structure, thus establishing a network that continuously reproduces more operations within the system. Those traditional perceptions are trapped and buried, without the possibility of refinement or factual examination. The rigidity of those norms is so questionable that the presumption of a principle was essential. In the present study, it was assumed that those traditions derive from a series of linguistic operations that create a network wherein the process of autopoiesis occurs repeatedly in the system (L.O.A.S), as this network continues to replicate. The claim is that there is a method of communication that exists and maintains those trends. The assertion here is to cultivate an alternative to deep-rooted cultural views as much as to enlighten the locals with valuable knowledge and benefits regarding the flora under consideration.

Penelitian ini bertujuan: (1) Untuk menghasilkan rancangan dan mengimplementasikan hasil rancangan film dokumenter: ”Pelestarian Lingkungan Melalui Permainan Tradisional Mapoh-pohan”, (2) Untuk mengetahui respon masyarakat terhadap hasil akhir film dokumenter: ”Pelestarian Lingkungan Melalui Permainan Mapoh-pohan”. Metode penelitian yang digunakan pada film dokumenter Mapoh-pohan adalah Research and Develompent dengan model cyclic strategy. Tradisi Mapoh-pohan yang dipilih sebagai subjek penelitian adalah Mapoh-pohan yang ada pada Banjar Ole, Desa Dauh Puri, Kecamatan Marga, Kabupaten Tabanan. Adapun tahap – tahap dari Cyclic Strategy diantaranya adalah brief, tahap 1, tahap 2, evaluasi 1, tahap 3, evaluasi 2, tahap 4 dan Outcome. Hasil penelitian menunjukkan bahwa film dokumenter: ”Pelestarian Lingkungan Melalui Permainan Tradisional Mapoh-pohan” dalam kriteria sangat baik. Hasil yang diperoleh dari uji ahli isi dan uji ahli media seluruhnya sudah sesuai. Sedangkan dari uji respon penonton disebar ke 30 responden didapatkan hasil rata-rata presentase yaitu 93,66% dengan tingkat pencapaian “sangat baik”. Kesimpulan yang didapatkan film dokumenter: ”Pelestarian Lingkungan Melalui Permainan Tradisional Mapoh-pohan” sudah sangat baik dan bisa digunakan sebagai media pelestari dan promosi permainan tradisional “Mapoh-pohan” dalam bentuk dokumentasi digital. Kata kunci: Film Dokumenter, Mapoh-pohan, Permainan Tradisional, Cyclic Strategy This study aims: (1) To produce a design and implement the result of documentary film: "Environmental Preservation through Mapoh-pohan Traditional Game", (2) To find out the public response to the final result of documentary film: "Environmental Preservation Through Mapoh-pohan Traditional Game". The research method used in the Mapoh-pohan documentary film was Research and Develompent with a cyclic strategy model. Mapoh-pohan tradition chosen as the research subject was Mapoh-pohan in Banjar Ole, Dauh Puri Village, Marga District, Tabanan Regency. The results showed that the documentary film: "Environmental Preservation through Mapoh-pohan Traditional Game" was in very good criteria. The results obtained from the content expert test and the media expert test are all appropriate. Whereas from the audience response test spread to 30 respondents, the average percentage was 93.66% with a "very good" achievement level. The conclusion obtained by the documentary film: "Environmental Preservation through Mapoh-pohan Traditional Game" has been very good and can be used as a preservation media and promotion of "Mapoh-pohan" traditional games in the form of digital documentation. Abstract:
Purpose Model-assisted probability of detection (MAPOD) is an important approach used as part of assessing the reliability of nondestructive testing systems. The purpose of this paper is to apply the polynomial chaos-based Kriging (PCK) metamodeling method to MAPOD for the first time to enable efficient uncertainty propagation, which is currently a major bottleneck when using accurate physics-based models. Design/methodology/approach In this paper, the state-of-the-art Kriging, polynomial chaos expansions (PCE) and PCK are applied to “a^ vs a”-based MAPOD of ultrasonic testing (UT) benchmark problems. In particular, Kriging interpolation matches the observations well, while PCE is capable of capturing the global trend accurately. The proposed UP approach for MAPOD using PCK adopts the PCE bases as the trend function of the universal Kriging model, aiming at combining advantages of both metamodels. Findings To reach a pre-set accuracy threshold, the PCK method requires 50 per cent fewer training points than the PCE method, and around one order of magnitude fewer than Kriging for the test cases considered. The relative differences on the key MAPOD metrics compared with those from the physics-based models are controlled within 1 per cent. Originality/value The contributions of this work are the first application of PCK metamodel for MAPOD analysis, the first comparison between PCK with the current state-of-the-art metamodels for MAPOD and new MAPOD results for the UT benchmark cases.

Kuta Mapot yang berada di wilayah Desa Tumbang Lapan, Kecamatan Rungan Hulu, Kabupaten Gunungmas, Provinsi Kalimantan Tengah, merupakan hunian berbenteng di tepian anak Sungai Tumbang Lapan, di Daerah Aliran Sungai (DAS) Kahayan bagian hulu. Kuta ini termasuk istimewa karena masih nampak beberapa tiangnya, baik dari bagian pagar keliling maupun bangunan di dalamnya. Penelitian ini bertujuan untuk merekonstruksi bentuk, simbol, dan kronologi hunian berbenteng Mapot. Penelitian bersifat deskriptif dengan pendekatan induktif. Pengumpulan data menggunakan metode survei, ekskavasi, wawancara, dan studi pustaka. Hasil penelitian memberi gambaran bahwa Kuta Mapot berbentuk persegi, yang dibentuk oleh pagar keliling dari balok ulin, dilengkapi dengan beberapa patung yang bermakna simbolik sebagai penolak bala sekaligus penjaga, dan bangunan patahu. Secara kronologi (absolut) berdasarkan analisis 14C, Mapot berada di kisaran abad ke-5--20 Masehi, sedangkan secara relatif (keramik Cina) berada pada abad ke-18--20 Masehi. Pendukung Kuta Mapot adalah masyarakat asli, yang sekarang dikenal dengan nama masyarakat Ngaju yang tinggal di Desa Tumbang Lapan, Kecamatan Rungan Hulu. Kuta Mapot, located in the Tumbang Lapan Village, Rungan Hulu Subdistrict, Gunungmas Regency, Central Kalimantan Province, is a fortified residence on the banks of the Tumbang Lapan tributary, in the upper of Kahayan River Basin. Kuta is specially considered because there can still be found some pillars, both from the part of the fences and the buildings inside. This study aims to reconstruct the shape, symbols, and chronology of Mapot fortified dwellings in the village of Tumbang Lapan. Research is descriptive with an inductive approach. The data were collected by survey, excavation, interview, and literature study methods. The study can illustrate that Kuta Mapot is square in shape, formed by a perimeter fence of ironwood beams, equipped with several sculptures that have symbolic meanings as repellent as well as guards, and patahu building. Chronologically (absolute) based on C14 analysis, Mapot is in the range of the 5th-20th century AD, while relatively (Chinese ceramics) is in the 18th-20th century AD. Supporters of Kuta Mapot were indigenous people, now known as Ngaju people who live in Tumbang Lapan Village, Rungan Hulu District.

AbstractIn light of previous results, we assessed whether liposomes functionalized with ApoE-derived peptide (mApoE) and phosphatidic acid (PA) (mApoE-PA-LIP) impacted on intracellular calcium (Ca2+) dynamics in cultured human cerebral microvascular endothelial cells (hCMEC/D3), as an in vitro human blood-brain barrier (BBB) model, and in cultured astrocytes. mApoE-PA-LIP pre-treatment actively increased both the duration and the area under the curve (A.U.C) of the ATP-evoked Ca2+ waves in cultured hCMEC/D3 cells as well as in cultured astrocytes. mApoE-PA-LIP increased the ATP-evoked intracellular Ca2+ waves even under 0 [Ca2+]e conditions, thus indicating that the increased intracellular Ca2+ response to ATP is mainly due to endogenous Ca2+ release. Indeed, when Sarco-Endoplasmic Reticulum Calcium ATPase (SERCA) activity was blocked by cyclopiazonic acid (CPA), the extracellular application of ATP failed to trigger any intracellular Ca2+ waves, indicating that metabotropic purinergic receptors (P2Y) are mainly involved in the mApoE-PA-LIP-induced increase of the Ca2+ wave triggered by ATP. In conclusion, mApoE-PA-LIP modulate intracellular Ca2+ dynamics evoked by ATP when SERCA is active through inositol-1,4,5-trisphosphate-dependent (InsP3) endoplasmic reticulum Ca2+ release. Considering that P2Y receptors represent important pharmacological targets to treat cognitive dysfunctions, and that P2Y receptors have neuroprotective effects in neuroinflammatory processes, the enhancement of purinergic signaling provided by mApoE-PA-LIP could counteract Aβ-induced vasoconstriction and reduction in cerebral blood flow (CBF). Our obtained results could give an additional support to promote mApoE-PA-LIP as effective therapeutic tool for Alzheimer’s disease (AD).

Buscando contribuir nesta direção, este artigo considera a sugestão recente de que a unidade política liderada por Saul teria causado a incursão de Sheshonq/Sisac nas terras de Israel, e de que este faraó teria matado Saul no final do séc. X a.C. (FINKELSTEIN, 2013, p. 38-44 e 59-61; seguido por KAEFER, 2014, p. 160 e 2015, p. 41-42). Este artigo discorda desta proposta e apresenta argumentos defendendo que quem matou Saul foram os filisteus, uns 20 anos antes da invasão de Sheshonq.

This paper proposes a max-product threshold unit (maptu) that can successfully perform dichotomous classifications of pattern vectors. Maptu, with weight vector, w, classifies a pattern vector, x, by comparing x max-prod w to 0.5. Results obtained by other methods in classification of benchmark data are used for comparison to the method using maptu. The benchmark data consists of the Australian credit data set, cervical cell data set, diabetes data set and the iris data set.

Abstract:
Abstract Recent evidence from our laboratory provided proof-of-concept for therapeutic potential for glioblastoma (GBM) of a combination strategy based on radiation and adjuvant doxorubicin-loaded liposomes (LIPs) conjugated with a modified Apolipoprotein E-derived peptide (mApoE), known to facilitate Blood Brain Barrier (BBB)-crossing. Significant glioma stem cell (GSC) apoptosis, tumor growth inhibition and increased overall survival were observed in vivo upon combined treatment offering attractive and innovative therapeutic possibilities for GBM. To strengthen therapeutic efficacy and lower off-target effects, we implemented mApoE-LIPs with a matrix metalloproteinases (MMP)-activable element that allows controlled payload release only in the MMPs rich tumor microenvironment, thus concurring to reduce unspecific interaction in healthy tissue where MMPs are low or absent Given the MMP2 overexpression in GBM, a MMP2-activable block (M2AB) was included in the phospholipid bilayer of mApoE-LIPs. The M2AB efficacy was evaluated on patient-derived GSCs displaying different MMP2 enzymatic activities by means of calcein-loaded M2AB/mApoE-LIPs. Human endothelial cells (hCMEC/d3), not expressing MMP2, were included to validate the targeted strategy as well as to assay the cytotoxicity on non-tumoral cells. Intracellular calcein quantification showed that: 1) calcein uptake correlates with MMP2 activity level; 2) M2AB/mApoE functionalization augmented calcein internalization into GSCs compared to mApoE alone. The MEK/ERK pathway, known to supports GBM cell survival, migration, and radio-resistance was considered as target strategy against GBM. A survey of the MEK/ERK inhibitors Trametinib (TRAM) and Pimasertib (PIMA) in several GSC lines indicated significant induction of GSC apoptosis associated to reduction of ERK phosphorylation. TRAM and PIMA were then encapsulated into mApoE-LIPs and their anti-GSC activity was investigated. Indeed, a dose dependent inhibition of GSC survival and induction of apoptosis combined to a significant lower level of phospho-ERK was observed upon 72h treatment. The genome of salmonid species contains numerous short interspersed repetitive elements (SINEs), which are known as members of the Hpa I family of SINEs. We have isolated and characterized eight loci at which Hpa I SINEs have been inserted in a species-specific manner in the genome of Oncorhynchus masou (cherry salmon). All of these SINE units were fixed in each of the local populations examined. This observation suggests that the SINE insertion events must have occurred in the genome of the single ancestral species of all the subspecies of O. masou and that the SINEs must have spread throughout the various populations before the divergence of subspecies. This provides unequivocal evidence that O. masou is monophyletic. These species-specific SINE units provide a very convenient and reliable tool for identification of O. masou by the polymerase chain reaction.

Objective To validate the effectiveness of MAPO method (Movement and Assistance of Hospital Patient) after the introduction of some changes to improve assessment objectivity. Background The number of operators exposed to patient manual handling is increasing considerably. MAPO, proposed in 1999 as a useful tool to estimate the risk of patient manual handling, is a method characterized by analytical quickness. It has recently been improved to better match the 2012 ISO (International Organization for Standardization) technical report. Methods A multicenter study was conducted between 2014 and 2016 involving 26 Italian hospitals in the Apulia Region. MAPO method was used to assess the risk of patient manual handling in 116 wards. A total of 1,998 exposed subjects were evaluated for the presence or absence of acute low back pain in the previous 12 months. Results Only 12% of the investigated wards fell in the green exposure level (MAPO index = 0.1–1.5), 37% resulted in the average exposure level (MAPO index = 1.51–5) and the remaining 51% in the higher exposure level (MAPO index >5). The results confirmed a positive association between increasing levels of MAPO index and the number of episodes of acute low back pain (adjusted p trend = .001). Conclusion The improvements made over the past years led to a more objective assessment procedure. Despite the changes, the study confirmed the effectiveness of MAPO method to predict low back pain. Application MAPO method is an accurate risk assessment tool that identifies and evaluates workplace risks. The proper application of the method significantly improves working conditions.

Modulated adiabatic passage on oriented nuclei (MAPON) is a technique developed specifically to measure very small electric quadrupole interactions (EQIs) at radioactive probe nuclei even when such interactions are masked in the frequency domain by inhomogeneous magnetic broadening associated with a dominant magnetic hyperfine interaction. In this paper an overview of the technique is presented with particular reference to 60CoCo EQI measurements which were an important step in establishing MAPON methodology. MAPON results for dilute nd impurities in ferromagnetic Fe and Ni metal hosts are reviewed including presentation of new data for 58CoNi and 60CoNi. Priorities for future studies are provided on the basis of this review.

Both an increase in atmospheric temperature and a decrease in atmospheric pressure can lead to the degradation of the maximum allowable power output (MAPO) or the thermal efficiency of a gas turbine. In order to reduce these adverse effects, this paper provides a simulated study on the improvement of the MAPO and the variations of the thermal efficiency due to the fuel flow and variable area nozzle control under different atmospheric conditions. Simulation results indicate that the MAPO increases with the power turbine nozzle area, while the thermal efficiency shows a parabola trend. With the same increment of the power turbine nozzle area, the improvement of the MAPO goes up as the atmospheric temperature rises and the decrease of thermal efficiency is alleviated. Analyses show that the slow degradation of the compressor isentropic efficiency is a key factor that enables the MAPO to increase significantly and the thermal efficiency to remain almost constant.

AbstractWe investigated the risk of diphyllobothriasis from ingestion of wild Pacific salmon in Japan by surveying Diphyllobothrium plerocercoids in 182 salmon samples obtained from Japan. The plerocercoids were not detected in chum salmon (Oncorhynchus keta) (0/26), called Akizake in Japan, caught between September and November. However, the detection rate of plerocercoids in chum salmon, called Tokishirazu in Japan, caught between early April and June, was 51.1% (24/47) with an average of two plerocercoid larvae per fish. The detection rates of cherry salmon (Oncorhynchus masou) and pink salmon (Oncorhynchus gorbuscha) were 12.2% (10/82) and 18.5% (5/27), respectively, and the average number of plerocercoids per fish was 0.45 (37 larvae/82 fishes) and 0.22 larvae (6 larvae/27 fishes), respectively. Plerocercoids isolated from O. keta and O. masou were identified as Diphyllobothrium nihonkaiense on the basis of molecular analysis of the cox1 and nad3 genes. Moreover, four tapeworms (three from O. keta and one from O. masou) were obtained by infecting golden hamsters with plerocercoids. The morphological features of these tapeworms were similar to those of D. nihonkaiense isolated from humans. Therefore, we think that O. keta and not O. masou is the most important source of plerocercoid infections in Japan.

PIXE was adopted for analysis of trace elements in otoliths of anadromous fish, masu salmon Oncorhynchus masou masou and amago salmon O. masou ishikawae. Mn, Zn and Sr were detected in order of ppm. Two different types of spectra were obtained from masu salmon otoliths. The concentrations of Sr and Zn in the two types were quite different. The difference seems to be a result of the difference in periods when the individuals stayed in freshwater environment until they were caught. Amago salmon otoliths contain significantly less Sr than masu salmon otoliths. It is considered to be due to different migration pattern between the two subspecies. Anadromous fish generally experiences freshwater and sea water environment in its migration. Trace elemental concentrations in sea water is quite different from those in freshwater. The environment, which varies along with the migration, would affect the incorporation of trace elements into otoliths of anadromous fish.

The deposition of amyloid-β (Aβ) plaques in the brain is a significant pathological signature of Alzheimer’s disease, correlating with synaptic dysfunction and neurodegeneration. Several compounds, peptides, or drugs have been designed to redirect or stop Aβ aggregation. Among them, the trideca-peptide CWG-LRKLRKRLLR (mApoE), which is derived from the receptor binding sequence of apolipoprotein E, is effectively able to inhibit Aβ aggregation and to promote fibril disaggregation. Taking advantage of Atomic Force Microscopy (AFM) imaging and fluorescence techniques, we investigate if the clustering of mApoE on gold nanoparticles (AuNP) surface may affect its performance in controlling Aβ aggregation/disaggregation processes. The results showed that the ability of free mApoE to destroy preformed Aβ fibrils or to hinder the Aβ aggregation process is preserved after its clustering on AuNP. This allows the possibility to design multifunctional drug delivery systems with clustering of anti-amyloidogenic molecules on any NP surface without affecting their performance in controlling Aβ aggregation processes.

This study aimed to improve students’ mastery of pronunciation through words spelling strategies in the fourth grade of SD Inpres Mappu in Mamuju. This research was a classroom action research (CAR) conducted in two cycles, namely, cycle one and cycle 2. Each cycle was carried out four times. There are four steps in this research; planning, action, observation, and reflection. The data in this study used quantitative (test) as the primary data and qualitative (field notes, recording, and documentation) as supporting data. The subjects in this study were 15 students in grade IV SD Inpres Mappu in Mamuju. The results showed that the use of the Words Spelling Strategy succeeded in improving the pronunciation master of the fourth-grade students of SD Inpres Mappu. It can be seen from the increased percentage of student learning outcomes from cycle I to cycle II, with an average value of 83.6. Therefore, it can be conducted that using the Words Spelling Strategy can improve student learning outcomes in mastering pronunciation.

O impulso à realização deste trabalho foi, primeiramente, o estranhamento diante do universo pesquisado, a comunidade de São Pedro de Joselândia, aproximadamente 170 km de Cuiabá, distrito de Joselândia, município de Barão de Melgaço – Mato Grosso, localizada entre dois importantes rios do Pantanal: Rio Cuiabá e Rio São Lourenço. As comunidades que compõem o distrito (São Pedro, Mocambo, Pimenteira, Retiro São Bento, Colônia Santa Izabel, Capoeirinha e Lagoa do Algodão) são interligadas não somente pela definição geográfica, mas também por uma rede de parentesco (consanguinidade, afinidade e compadrio), que nos permite referenciá-la como campesina, tendo a família e a terra como eixos axiais de seu modo de vida. Na primeira inserção em campo estava ocorrendo a Pareia Pantaneira, uma corrida de cavalos que acontece uma vez ao ano durante o feriado de Independência Nacional. Na Pareia aparecem ainda mais expressivamente elementos de uma sociabilidade agonística. O que no cotidiano pode estar velado, no evento ganha uma dimensão expressiva. Foi durante a primeira viagem de campo, no evento da Pareia, que conheci Manoel Gonçalo Amorim, pouco antes de ele ser assassinado naquele mesmo dia. Os primeiros estranhamentos se seguiram nos anos seguintes ao acontecimento, diante das expressões usadas na comunidade para falar sobre o assassinato acima citado: “aquele que dizem que matou”, ou, “foi um suicídio”. Os códigos acionados por estes agentes sociais que dão significado ao assassinato constituem o objeto de uma etnografia, da qual apresentamos aqui alguns elementos significativos.

PIXE was adopted for analysis of trace elements in otoliths of Masu salmon Oncorhynchus masou masou to examine relationship between trace elements and environmental salinity. The otoliths were removed from salmon juveniles reared in four values of salinity and wild ones. The otolith Sr concentrations of reared individuals are positively related to salinity and there is significant difference between freshwater and seawater. The otoliths of smolts contain more Sr than those of parrs. It seems that the Sr concentrations in otoliths of Masu salmon reflect salinity where they had stayed and show the migration pattern.

The objective of this study was to evaluate the impact of a 10-bed MAPU in Royal Melbourne Hospital (RMH) on hospital length of stay. Methods.A pre-post study design was used. Cases were defined as all general medical patients admitted to the RMH between 1 August 2003 and 31 January 2004. MAPU patients were defined as general medical patients who had been discharged from RMH MAPU unit as part of their RMH inpatient admission. Historical controls were defined as all general medical patients admitted to the RMH between 1 August 2002 and 31 January 2003. Results.There was a reduction in median length of stay that did not reach statistical significance. During the study period, median emergency department length of stay for MAPU patients was 10.3 h compared with 13.2 h for non-MAPU patients who were admitted directly to general wards. Conclusions.The reductions in length of stay are likely to be of clinical significance at the emergency department (ED) level. The MAPU model also contributes to providing care appropriate care for older admitted patients. What is known about the topic?There is increasing interest in models of acute medical management in public hospitals in Australia. One of the key factors driving interest in these models has been the need to improve patient flow to improve hospital efficiency and contribute to reducing bed access block. There are very little published data pertaining to the effectiveness of these models of care. What does the paper add?The paper reports non-statistical, but probably important clinical reductions in hospital and ED length of stay using a before and after cohort analysis. It highlights the difficulties evaluating these models of care in the absence of well designed controlled studies and suggests evaluation of length of stay needs to be powered to detect small changes in ED efficiency rather than overall hospital length of stay. What are the implications for practitioners?Practitioners in the area can draw on the results of this paper to design an acute medical planning unit and develop an evaluation framework.

Optical fiber sensors (OFSs) represent an efficient sensing solution in various structural health monitoring (SHM) applications. However, a well-defined methodology is still missing to quantify their damage detection performance, preventing their certification and full deployment in SHM. In a recent study, the authors proposed an experimental methodology to qualify distributed OFSs using the concept of probability of detection (POD). Nevertheless, POD curves require considerable testing, which is often not feasible. This study takes a step forward, presenting a model-assisted POD (MAPOD) approach for the first time applied to distributed OFSs (DOFSs). The new MAPOD framework applied to DOFSs is validated through previous experimental results, considering the mode I delamination monitoring of a double-cantilever beam (DCB) specimen under quasi-static loading conditions. The results show how strain transfer, loading conditions, human factors, interrogator resolution, and noise can alter the damage detection capabilities of DOFSs. This MAPOD approach represents a tool to study the effects of varying environmental and operational conditions on SHM systems based on DOFSs and for the design optimization of the monitoring system.

Abstract:
Objective To investigate malnutrition prevalence on presentation to a Medical Assessment and Planning Unit (MAPU) in a setting designed to prevent hospital admission, the association of nutritional status with hospital readmission at 90 days, and agreement of nutritional risk between the Malnutrition Screening Tool (MST) and Subjective Global Assessment (SGA). Methods Prospective longitudinal cohort study of consecutive patients admitted to MAPU during the first 6 weeks of operation. The main outcome measures were prevalence of malnutrition and hospital readmission at 90 days. Sensitivity and specificity of the MST was assessed against the criterion standard of SGA. Results The mean participant age was 62 years (n = 153, s.d. 17.4 years) with 50% male (77/153, 95% CI 42–58%). According to the SGA, 17% (95% CI 8–26%) were assessed as malnourished on admission. The MST identified that 18% (95% CI 12–24%) were at nutritional risk, and participants screening positive for nutritional risk had significantly increased odds of hospital readmission at 90 days (OR 3.4, 95% CI, 1.3–9.1, P < 0.029). The MST was practical and successfully identified patients assessed as malnourished within the MAPU setting (sensitivity 73%, specificity 76%, negative predictive value 93%, positive predictive value 38%). Conclusions Malnutrition is a significant problem in a MAPU setting, and patients screened at nutritional risk are at significantly higher risk of hospital readmission within 90 days. What is known about the topic? The prevalence of malnutrition in Australian hospitals is estimated to be between 30 and 53%. It is important to identify malnutrition, given that the consequences contribute to increased morbidity, mortality, length of hospital stay, readmission and healthcare costs. Malnutrition screening is strongly advocated as the first step to routinely identify nutritional problems early in acute care settings. To date only one Australian study has documented the prevalence of malnutrition in an acute medical unit setting, estimating it to be 53%. What does this paper add? The acute medical unit in the previous study operates under a different model of care acute assessment unit, in which patients are triaged to the wards, and no known study to date has investigated the prevalence of malnutrition in a MAPU setting aiming to avoid hospital admissions. Furthermore, no other studies have explored malnutrition and its association with risk of hospital readmission in these settings. Our study reveals that malnutrition is a problem in MAPU settings, demonstrating the important role malnutrition screening plays in identifying malnutrition risk in a MAPU setting due to the increased risk of readmission at 90 days evident in this group. What are the implications for practitioners? The study identified that malnutrition risk significantly increased the odds of hospital readmission at 90 days. The MST was confirmed as a practical tool for use in the MAPU setting, showing acceptable sensitivity and specificity. Incorporation of malnutrition screening and timely intervention may increase the overall effectiveness of the MAPU clinic through avoidance of hospital readmission at 90 days.
