Abstract:
Objective: Congenital macrostomia is a lateral orofacial cleft between the maxillary and mandibular components of the first branchial arch. Bilateral macrostomia is a poorly characterized malformation, with only 14 cases reported in the literature. The purpose of this study was to compare our experience with the world literature. Method: A retrospective analysis of 20 cases of bilateral congenital macrostomia was conducted; 6 cases were drawn from 2 maxillofacial surgery units and 14 cases from the world literature. Cases of bilateral congenital macrostomia were compared with cases of unilateral forms using a review of the literature post-1954. Among the six cases identified from the two maxillofacial surgery units, three were treated with linear sutures and three with Z-plasty. Subsequent aesthetic and functional results were analyzed. Results: Compared to unilateral forms, bilateral macrostomia is more often isolated without ear or skin deformities. Moreover, there are a greater proportion of larger defects among cases with bilateral macrostomia when compared to unilateral macrostomia. Alimentation, phonation, and mouth opening were always normal. The two sides were always symmetric. Only one case presented with the complication of skin contractions during lip movement. Conclusion: The etiopathogenesis of bilateral macrostomia is unclear. Although over 50% of the reported cases of bilateral macrostomia are isolated, this condition presents a therapeutic challenge. In the case of bilateral forms, the surgeon must define the commissure position without a normal side. Repair thus requires extraoral landmarks and normal measurements.

Abstract:
Background: Many surgical techniques have been described for the repair of macrostomia. However, most were designed for the repair of unilateral macrostomia. Most reports of repairs involved small numbers of patients. This has not allowed for evaluation of complications and comparison of various techniques. Objective: This study describes a modification of an older technique designed for unilateral macrostomia. Details of the operative technique used for the repair are described, along with the principles underlying the various steps. Materials and Methods: This study involves 13 patients operated for macrostomia using the same surgical technique. The patients were seen at a recent screening and mass surgery exercise for patients with orofacial and craniofacial deformities. Results: Eleven patients had bilateral macrostomia; two had unilateral macrostomia. No anesthetic or surgical complications occurred in any of the patients. All patients tolerated the procedure well. Oral continence and acceptable facial symmetry were restored in all patients, with minimal scarring.

Abstract:
We report on an Italian boy, born to normal and nonconsanguineous parents with a prenatal diagnosis of ventriculomegaly and subependymal glial heterotopias. At birth bilateral macrostomia was diagnosed without other evident facial anomalies. Magnetic resonance imaging (MRI) showed triventricular hydrocephalus and aqueductal stenosis and confirmed the nodules of glial heterotopia. The bilateral macrostomia was surgically corrected with the vermilion square flap method and W-plasty technique and follow up MRI at 6 months showed mild increase of ventricular dilatation without signs of active hydrocephalus. The association between macrostomia and hydrocephalus has been reported only in rare cases of complex malformative syndromes but never with isolated macrostomia.

La presente tesi di ricerca dottorale -dal titolo "Il diritto in Movimento. Hans Kelsen e l'evoluzione macrostorica degli ordinamenti giuridici"- è dedicata alla complessa e articolata questione del carattere evolutivo del fenomeno giuridico. Essa si compone di tre capitoli. Il primo capitolo, dal titolo "I due volti dell’evoluzione giuridica", è dedicato alla proposta ed elaborazione di una tesi metateorica di fondo, relativa alla possibilità e necessità di distinguere tra "teorie dell’evoluzione nel diritto" e "teorie evolutive del diritto" come linee di ricerca che in modi essenzialmente differenti trattano dell’evoluzione giuridica. Nel primo capitolo, si procede, dunque, con una breve ricostruzione storico-critica della genesi, degli sviluppi e della fortuna delle teorie dell’evoluzione nel diritto, con un’analisi delle ragioni che hanno condotto a misconoscere il valore delle teorie evolutive del diritto e con un tentativo di rivalutare la rilevanza teoretica e pratico-politica delle stesse. Il secondo capitolo, dal titolo "Evoluzione macrostorica del diritto: la vasta storia di un’idea", è invece dedicato alla storia delle teorie evolutive del diritto. Obiettivo di tale sezione storiografica della presente ricerca è mettere in evidenza come i tentativi di elaborare ricostruzioni dell’evoluzione macrostorica del diritto non possano essere circoscritti al XIX secolo come da vulgata, essendo quella delle teorie evolutive del diritto una linea di ricerca che può vantare una lunga tradizione che, risalente fino all’antichità classica, viene portata avanti ancora oggi da alcuni studiosi. Il terzo capitolo della presente ricerca è dedicato invece all’esame dettagliato delle riflessioni concernenti l’evoluzione macrostorica degli ordinamenti giuridici che Hans Kelsen ha proposto in alcune opere degli anni Quaranta del XX secolo. La scelta di procedere con tale approfondimento deriva, in primo luogo, dalla constatazione che non è mai stata tentata una ricostruzione unitaria di tale versante della riflessione giusfilosofica di Kelsen. In secondo luogo, ancora dal punto di vista storiografico, l’analisi della teoria kelseniana dell’evoluzione macrostorica degli ordinamenti giuridici si rivela certamente interessante per una rivalutazione di alcuni assunti o pregiudizi relativi all’ontologia giuridica e all’epistemologia giuridica di Kelsen. Infine, la scelta di procedere con tale approfondimento deriva dalla convinzione che la teoria di Kelsen sull’evoluzione macrostorica degli ordinamenti giuridici risulti essere ricca di intuizioni che ancora oggi si rivelano di estremo valore.
The doctoral research thesis "The Law in Movement. Hans Kelsen and the Macro-historical Evolution of Legal Orders" is dedicated to the complex problem of the evolutionary nature of the legal phenomenon. It is made up of three chapters. The first chapter, entitled "The Two Faces of Legal Evolution", is dedicated to the proposal and elaboration of a basic metatheoretical thesis, relating to the possibility and need to distinguish between "theories of evolution in law" and "evolutionary theories of law "as lines of research which in essentially different ways deal with legal evolution. The first chapter proceed with a brief historical-critical reconstruction of genesis, developments and success of the so called theories of evolution in law; with the analysis of the reasons that led legal scholars to the rejection of evolutionary theories of law; and, finally, with an attempt to re-evaluate the theoretical and practical-political relevance of evolutionary theories of law. The second chapter, entitled "Macro-historical Evolution of Law: the Story of an idea", is dedicated to the history of evolutionary theories of law. The objective of this historiographic section of the present research is to highlight how attempts to elaborate reconstructions of the macro-historical evolution of law cannot be limited to the nineteenth century as usually claimed by legal scholars, being thei elaboration dating back to classical antiquity and still carried out today by some scholars. The third chapter of this research is dedicated to the detailed examination of the reflections concerning the macro-historical evolution of legal orders that Hans Kelsen proposed in some works of the 1940s. The choice to proceed with this analysis derives, first of all, from the observation that an unitary reconstruction of this side of Kelsen's philosophical reflection has never been attempted. Second, still from the historiographic point of view, the analysis of the Kelsenian theory of the macro-historical evolution of the legal systems certainly proves interesting for a re-evaluation of some assumptions or prejudices related to his legal ontology and legal epistemology. Finally, the choice to proceed with this analysis derives from the belief that Kelsen's theory on the macro-historical evolution of the legal systems turns out to be rich in intuitions that still prove extremely valuable today.

Abstract:
A Síndrome Ablefaria Macrostomia (SAM) é uma condição rara, onde os pacientes apresentam características clínicas marcantes como o encurtamento ou ausência das pálpebras superiores e inferiores, ausência de sobrancelhas e cílios, macrostomia por defeitos na fusão dos lábios, entre outros. O padrão de herança da síndrome não está elucidado, tendo a herança autossômica dominante com expressividade variável sido sugerida. SAM possui sobreposição fenotípica com a Síndrome de Barber-Say e com a Síndrome de Fraser, porém nenhum gene já descrito apresentou mutação nos pacientes portadores da SAM. A abordagem genômica no estudo de doenças raras tem sido amplamente utilizada, devido principalmente ao surgimento da Nova Geração de Sequenciamento, que possui alto poder de descriminar as seqüencias nucleotídicas com grande cobertura, em um curto período de tempo. No presente estudo o sequenciamento completo do exoma foi realizado, com cinco indivíduos de uma mesma família, três membros afetados e dois não, e permitiu a análise das regiões codificantes nestes indivíduos. A base molecular da Síndrome Ablefaria Macrostomia é aqui sugerida como autossômica dominante, e decorrente da mutação nova não sinônima c.223G>A (p.E75K) no gene TWIST2. Essa mutação patogênica ocasiona a troca de um aminoácido pequeno de carga negativa, o ácido glutâmico, para um aminoácido de cadeia maior carregado positivamente, a lisina. A modelagem in silico da proteína Twist2 mostrou que a estrutura geral tridimensional da proteína não foi alterada, mas a troca do aminoácido ocorre na posição 75 dentro do domínio básico HLH, e pode impedir a formação de dímeros, ou a própria ligação ao DNA. Sugere-se ainda que a heterogeneidade de fenótipos associados a mutações no gene TWIST2, pode ser atribuída às interações que essa proteína é capaz de formar, e a ampla ação regulatória que ela desempenha em diversos genes do desenvolvimento.
Ablepharon-Macrostomia Syndrome (AMS) is a rare condition characterised by absent or hypoplastic eyelids, absent eyebrows and eyelashes, macrostomia caused by fusion defects of the mouth with unfused lateral commissures, as well as other clinical features. The inheritance pattern has not been confirmed and while autosomal dominant inheritance with variable expressivity has been suggested, recessive inheritance has not been ruled out. The phenotype of AMS overlaps that of Barber-Say and Fraser Syndrome, but any reported gene for these syndromes is mutated on AMS patients. The genomic approach for rare disease studies has been widely used mainly due to the emergence of Next Generation Sequencing, which is very effective at determining nucleotide sequences with large coverage in a short period of time. The whole exome sequencing of five family members was undertaken, with three affected and two unaffected, and the coding regions of the individuals were subsequently analysed. The molecular basis of AMS is suggested here as autosomal dominant, and due to a novel non-synonymous mutation c.223G>A (p.E75K), in TWIST2 gene. This pathogenic mutation causes glutamic acid, a small negatively charged amino acid, to be substituted for a larger and positively charged lysine. The in silico protein modeling of Twist2 shows that the general 3D-structure of the protein is not affected, but the amino acid change is located inside the basic Helix-Loop-Helix domain which could disrupt dimerization and DNA binding. It has also been suggested that the phenotype heterogeneity associated with mutations on TWIST2 gene can be attributed to the interactions that this protein is capable of, and the role that it plays in the regulation of several developmental genes.

<em>Abstract</em>.—There are 11 lamprey species in Canada: Vancouver lamprey <em>Entosphenus macrostomus</em> (formerly <em>Lampetra macrostoma</em>), Pacific lamprey <em>Entosphenus tridentatus</em> (formerly <em>Lampetra tridentata</em>), chestnut lamprey <em>Ichthyomyzon castaneus</em>, northern brook lamprey <em>Ichthyomyzon fossor</em>, silver lamprey <em>Ichthtyomyzon unicuspis</em>, river lamprey <em>Lampetra ayresii</em>, western brook lamprey <em>Lampetra richardsoni</em>, Alaskan brook lamprey <em>Lethenteron alaskense</em>, American brook lamprey <em>Lethenteron appendix</em> (formerly <em>Lampetra appendix</em>), Arctic lamprey <em>Lethenteron camtschaticum</em> (formerly <em>Lampetra camtschatica</em>), and sea lamprey <em>Petromyzon marinus</em>. <em>Entosphenus</em> and <em>Lethenteron</em> were previously synonymized with <em>Lampetra</em>, but Nelson (2006) recognized these as three distinct genera. Conservation status has been assessed in only five species and in two of these (western brook lamprey and chestnut lamprey), only for portions of their Canadian population. The 2007 Committee on the Status of Endangered Wildlife in Canada list indicates that the enigmatic population of western brook lamprey in Morrison Creek, British Columbia, is endangered; the Vancouver lamprey in British Columbia is threatened; the chestnut lamprey in Saskatchewan and Manitoba is special concern; the northern brook lamprey in Manitoba, Ontario, and Quebec is of special concern in the latter two provinces and data deficient in the former; and the Alaskan brook lamprey in the Northwest Territories is data deficient. The threats to the four species at risk were collectively related to habitat degradation and loss, sensitivity to a catastrophic event, and sensitivity to lampricide used to control the invasive sea lamprey in the Great Lakes basin. Despite much lamprey work being conducted in the past decade, there still remain a number of knowledge gaps. These gaps include unequivocal evidence as to whether parasitic and nonparasitic members of a paired species should be considered distinct species and information on the distribution and population sizes and trends of the native lamprey species.
