Abstract:
This research paper explains the results of the prediction analysis of the number of lives lost in the event of a catastrophic dam collapse in Indonesia as a further consideration in assessing the level of risk of dam safety. The proposed procedure is to make a new prediction index of the number of lives lost (LoL) as the development of a risk index of evacuation requirements from Risk Affected Populations (PENRIS), on the Modified ICOLD Method which is always used in Indonesia. This study, resulting in a regression equation as a correlation between PENRIS and LoL, takes its source from various catastrophic dam collapse events that have occurred in the world including Indonesia. Furthermore the regression equation is integrated with the standard determination of the level of risk of dam safety used in Indonesia and the world, for conditions with and without a disaster early warning system based on the Graham formula (2010). Further analysis of the Emergency Action Plan (EAP or RTD) of 16 dams in Indonesia as a sample, gives an indication that the implementation of an early warning system will reduce the amount of LoL by almost 100% if implemented according to design. This research, with its focus on developing a prediction index for the number of LoL, proves that in Indonesia, where there are still many dams eventhough they already have RTDs, and have not conducted a disaster-based space arrangement based on predicted LoL numbers,the reduction in the value of dam security risks can only be optimal in the range of 50 % of the total dam studied.

Abstract:
In this work, the application of a feed-forward artificial neural network (FFANN) in predicting the degree of polymerization (DP) and loss of life (LOL) in oil-submerged transformers by using the solid insulation evaluation method is presented. The solid insulation evaluation method is a reliable technique to assess and predict the DP and LOL as it furnishes bountiful information in examining the transformer condition. Herein, two FFANN models are proposed. The first model is based on predicting the DP when only the 2-Furaldehyde (2FAL) concentration measured from oil samples is available for new and existing transformers. The second FFANN model proposed is based on predicting the transformer LOL when the 2FAL and DP are available to the utility owner, typically for the transformer operating at a site where un-tanking the unit is a daunting and unfeasible task. The development encompasses constructing numerous FFANN designs and picking networks with superlative performance. The training and testing procedures databank is based on the dataset of the 2FAL and DP from a fleet of transformers and measured from laboratory analysis. The correlation coefficient of 0.964 was ascertained when the DP was predicted using the 2FAL measured in oil. In the FFANN model, a correlation coefficient of 0.999 against the practical data where one can make a reliable prediction of transformer LOL concerning 2FAL was generated and the amount of DP present produced. This model can be used to predict the DP and LOL of new and existing transformers at the manufacturer’s premises and operating in the field, respectively. To the knowledge of the authors, no research work has been published addressing the methods proposed in this work.

Abstract:
This paper proposes a Home Energy Management System (HEMS) that optimizes the load demand and distributed energy resources. The optimal demand/generation profile is presented while considering utility price signal, customer satisfaction, and distribution transformer condition. The electricity home demand considers electric vehicles (EVs), Battery Energy Storage Systems (BESSs), and all types of non-shiftable, shiftable, and controllable appliances. Furthermore, PV-based renewable energy resources, EVs, and BESSs are utilized as sources of generated power during specific time intervals. In this model, customers can only perform Demand Response (DR) actions with contracts with utility operators. A multi-objective demand/generation response is proposed to optimize the scheduling of various loads/supplies based on the pricing schemes. The customers’ behavior comfort level and a degradation cost that reflects the distribution transformer Loss-of-Life (LoL) are integrated into the multi-objective optimization problem. Simulation results demonstrate the mutual benefits that the proposed HEMS provides to customers and utility operators by minimizing electricity costs while meeting customer comfort needs and minimizing transformer LoL to enhance operators’ assets. The results show that the electricity operation cost and demand peak are reduced by 31% and 18%, respectively, along with transformer LoL % which is reduced by 28% compared with the case when no DR was applied.

Abstract Access to effective communication is critical to the conversations that occur at end-of-life and represents an unaddressed need within palliative care. These challenges may disproportionately affect persons with dementia (PWD). Hearing loss is one of the most common comorbidities among PWD and is independently associated with neuropsychiatric symptoms. However, relatively little is known about the potential impact of hearing loss on PWD at end-of-life. We examined last month of life (LML) data from 971 proxies of deceased PWD from the National Health and Aging Trends Study (2011-2020). Hearing difficulty was associated with increased anxiety/sadness in PWD, χ2(1)=4.596, p=.032, such that 65.6% of persons with hearing difficulty reported anxiety/sadness in the LML. Binary logistic regression found that hearing difficulty was significantly associated with increased anxiety/sadness (OR=1.40, 95% C.I. 1.00 – 1.80, p < 0.05). Interventions that optimize communication for PWD may be a meaningful approach to improving the end-of-life experience.

AbstractWith the development of image recognition technology, face, body shape, and other factors have been widely used as identification labels, which provide a lot of convenience for our daily life. However, image recognition has much higher requirements for image conditions than traditional identification methods like a password. Therefore, image enhancement plays an important role in the process of image analysis for images with noise, among which the image of low-light is the top priority of our research. In this paper, a low-light image enhancement method based on the enhanced network module optimized Generative Adversarial Networks(GAN) is proposed. The proposed method first applied the enhancement network to input the image into the generator to generate a similar image in the new space, Then constructed a loss function and minimized it to train the discriminator, which is used to compare the image generated by the generator with the real image. We implemented the proposed method on two image datasets (DPED, LOL), and compared it with both the traditional image enhancement method and the deep learning approach. Experiments showed that our proposed network enhanced images have higher PNSR and SSIM, the overall perception of relatively good quality, demonstrating the effectiveness of the method in the aspect of low illumination image enhancement.

Abstract Background Chronic Myeloid Leukemia (CML) is a myeloproliferative neoplasm with an incidence of 1-1.5 cases per 100,000 adults, accounting for ∼ 15-20 % of newly diagnosed patients with myeloid leukemia in adults. Treatment for CML has changed dramatically with the introduction of imatinib mesylate (IM), the first tyrosine kinase inhibitor (TKI) targeting the BCR-ABL1 oncoprotein. Previous population-based research (Björkholm et al. JCO, 2011) showed a major improvement in outcome of patients with CML up to 79 years of age diagnosed from 2001 to 2008. The elderly still had poorer outcome, partly because of a limited use of IM. However, increasing recognition of IM resistance and intolerance has led to the development of additional (second and third-generation) TKIs, which have demonstrated effectiveness as salvage therapies or alternative first-line treatments. Here we quantify how the life years lost due to a diagnosis of CML has changed between 1973 and 2013 using a measure called the loss in expectation of life (LEL). Methods This population-based study included3,684CML patients diagnosed in Sweden between 1973 and 2013; diagnoses were obtained from the Swedish Cancer Registry. The LEL was estimated using flexible parametric models. The LEL is the difference between the life expectancy in the diseased population and that in a matched subset of the general population. This measure has a simple interpretation as the number of life years lost, or the reduction in the life expectancy, due to a diagnosis of cancer. Results The life expectancy increased dramatically between 1990 and 2013 for CML patients of all ages; see figure. Patients in 2013, on average, lose less than 3 life years due to their diagnosis of CML. The largest increase in the life expectancy and thus the largest decrease in LEL over time was seen in younger patients; a diagnosis of CML in 1990 for a male 55-year old, on average, reduced his life expectancy by approximately 20.6 (95% CI: 20.3-21.1) years whereas a diagnosis in 2010 in the same male would on average reduce his life expectancy by only 2.6 (95% CI: 1.4-3.8) years. Although the greatest improvements were seen in those diagnosed at a younger age, those diagnosed at 85 years still benefitted in better survival over year of diagnosis; a diagnosis of CML in 1990 for a 85-year old, on average, reduced his life expectancy by approximately 3.6 (95% CI: 3.5-3.8) years whereas a diagnosis in 2010 in the same male would on average reduce his life expectancy by only 1.6 (95% CI: 1.0-2.2) years. Conclusions The reduction in life expectancy, or the number of life years lost due to a diagnosis of CML has greatly reduced over the years Patients who are diagnosed at a younger age lose dramatically fewer years in the most recent calendar years compared to previous years due to their CML diagnosis. Improvements in survival in the late 1990s were at least as great as those from 2001 in the youngest patients. Increased number of allogeneic stem cell transplantations, the introduction of interferon-alpha, improved supportive care and second line treatment with IM have all contributed. Less improvement was seen in the older patients which is probably explained by the relatively slow implementation of IM in this patient group. The impact of second generation TKIs on long-term survival remains to be determined. Figure 1. This article reports on the sampling and recruitment challenges, as well as the strategies used to address them in the Living After Loss (LAL) project, a bereavement intervention study conducted in Salt Lake City and San Francisco comparing two 14-week group conditions with follow-up. We encountered three major challenges: 1) difficulty determining eligibility for some potential participants who were contacted, 2) locating and recruiting nonwhites, and 3) unavailable phone numbers for approximately one-third of those we attempted to contact. Despite these challenges, we achieved a 42% response rate with a sample size of 328 participants comprising 15% non-white. Eighty-five percent of the participants completed all of the follow-up data points. Leading factors in participants' decisions to join and remain in the study were 1) opportunities to obtain help and support, 2) to potentially help other bereaved individuals, 3) to contribute to research, and 4) their on-going interactions with a professional, empathetic, and culturally sensitive project staff. Effectively focusing recruitment efforts and carefully and systematically training research staff were among the most effective strategies we employed and therefore suggest for those planning similar investigations.

Background: Obesity has become a major health issue in the world over past two decades. . This leads us to the need of immense exploration of life style modification options, dietary supplementation and weight loss friendly elements. Aim: Our study aims to highlight the comparative analysis of independent supplementation of vitamin E and iron to young adult females for their weight loss regime. Methods: 90 young adult non pregnant females of age between 25-39 years with BMI ranging between 30-34.9 (obese) were asked to take a fixed 1500 calorie diet for two months. They were divided into three groups with no supplement group, vitamin E group and iron group. Body mass index(BMI), LDL levels and prevalence of fatty liver was assessed among all individuals. Results: vitamin E and iron supplementation reduced BMI in their respective groups. Vitamin e and iron supplementation also reduce the prevalence of fatty liver among groups. Vitamin E reduces serum LDL levels. Conclusion: vitamin E and iron supplementation helps in weight loss. Keywords: obesity, vitamin E, iron, weight loss

Introduction and Aims: Homozygous-familial hypercholesterolemia (Ho-FH) is a rare condition due to biallelic mutations in low-density lipoprotein-receptor (LDL-R) genes characterized by high level of LDL-cholesterol (LDL-c) and huge risk of premature atherosclerotic cardiovascular disease (ASCVD), determining low quality of life and life expectancy. Lomitapide represents a therapeutic option for Ho-FH, but caution should be observed when used in fatty liver disease (FLD) and hypertransaminasemia since it is associated with onset/worsening of liver steatosis. We present a case of safe lomitapide therapy in an adult Ho-FH patient with pre-existing FLD. Case presentation: A 39-year-old man with severe hypercholesterolemia since childhood (LDL-c 405 mg/dl) and premature coronary heart disease history, was referred to our Modena Lipid Clinic. He presented an overt metabolic syndrome, FLD with hypertransaminasemia and elastosonographic significant liver fibrosis. Lipid-lowering-therapy (LLT) included rosuvastatin 20 mg, ezetimibe and evolocumab 140 mg twice a month without reaching LDL-c goal. Genetic analysis revealed homozygous pathogenic LDL-R gene mutation. Evolocumab was increased up to 420 mg twice a month and LDL-apheresis was started with quality of life worsening. Therefore, lomitapide 5 mg daily and low-fat diet were started, obtaining weight loss and lipid profile improvement. However, liver enzymes elevation higher than 5-fold was observed, leading to lomitapide discontinuation and baseline liver enzymes values restoration. After one-month wash-out, lomitapide was gradually reintroduced up to 5 mg daily without significant hypertransaminasemia recurrence, leading to LDL-c target achievement and LDL-apheresis discontinuation. Adherence to low-fat diet and weight loss resulted in FLD and fibrosis improvement. Conclusion: Ho-FH requires complex, combined treatment. Metabolic comorbidities co-existence makes Ho-FH management more difficult. Lomitapide can be safely used in Ho-FH patients with FLD and hypertransaminasemia, but strict follow-up of liver disease and a multidisciplinary approach are needed. Before lomitapide introduction, low-fat diet should be started advantageously and weight stabilization should be obtained.

Citrus fruits are susceptible to microbial infection during storage and transportation, leading to weight loss, quality deterioration and even physiological decay. In this study, a layer-by-layer (LbL) self-assembly edible coating based on chitosan and Sodium carboxymethyl cellulose (CMC) was prepared using electrostatic deposition. Postharvest citrus fruits (Citrus reticulate cv. ‘Chunxiang’) were coated either with the LbL coating, which exhibited an increased transmission rate of water vapor, or with single chitosan coating. The data of some physiological indexes of citrus coated with LbL coating and single coating were compared after being stored at 13 ± 2 °C and at relative humidity (RH) at 80–85% (shelf-life condition) for 10 days. Results demonstrated that the LbL deposited coating was effective at maintaining morphological and microstructural attributes, increasing the surface brightness, firmness and the content of titratable acid and ascorbic acid, as well as reducing the weight loss after storage by about 0.8%. Results in the present study indicated that the LbL edible coating could have the potential to maintain postharvest citrus quality during storage.

Abstract:
Background: Diabetics are more prone to periodontal diseases leading to poor oral function affecting their quality of life. The objective of the study was to assess the impact of periodontal health on the quality of life among diabetics using the short version of the Oral Health Impact Profile (OHIP -14). Methods: It was a descriptive cross-sectional study. Data was collected using translated and validated Nepalese version of OHIP-14 questionnaire and clinical examination for periodontal status (Community Periodontal Index and Loss of Attachment index) was done using mouth mirror and World Health Organization probe under natural light. Data was entered and analyzed using SPSS version 11.5. Mann- Whitney U test and Kruskal-Wallis tests were used to compare OHIP-14 scores between genders and periodontal status respectively. Statistical significance was established at p<0.05. Results: One hundred and forty-five subjects with confirmed diabetes participated in the study. Overall, 41% were male participants were as 59% were females. Majority of the participants had calculus with CPI score 1 (n=131, 90.3%) and another majority had loss of attachment 3-5 mm with LOA score 1 (n=55, 37.9%). There was no significant difference in mean scores between two genders (p=0.231). The OHIP scores among participants with highest CPI and LOA scores had statistically significant difference (p=0.011 and p=0.006 respectively). Conclusions: Periodontal status was poor among diabetics with significant impact on their oral health related quality of life. Glycemic control along with periodontal maintenance is required to enhance quality of life among such patients. This might be possible with comprehensive medical approach for diabetic patients.

Abstract:
Introduction: Patients who are overweight or obese have an increased risk of developing type 2 diabetes mellitus (T2DM). Weight loss can have a positive effect on glycemic control. Objective: We aimed to investigate glycemic control in patients with T2DM and overweight or obesity during a structured weight-loss program. Methods: This was a prospective, interventional study. We recruited 36 patients (14 men and 22 women) with a median age of 58.5 years and median body mass index (BMI) of 34.1, to a 15-week structured weight-loss program with a low-calorie (800 kcal) formula diet for 6 weeks. The primary end point, HbA1c level, and secondary end points, anthropometric data, medication, and safety, were assessed weekly. Laboratory values and quality of life were assessed at baseline and after 15 weeks. Results: HbA1c decreased from 7.3% at baseline to 6.5% at 15 weeks (p < 0.001), median body weight by 11.9 kg (p < 0.001), median BMI by 4.3 (p < 0.001) and median waist circumference by 11.0 cm (p < 0.001). Two participants discontinued insulin therapy, 4 could reduce their dosage of oral antidiabetic agents, and 6 completely discontinued their antidiabetic medication. Insulin dose decreased from 0.63 (0.38–0.89) to 0.39 (0.15–0.70) units/kg body weight (p < 0.001). No patient experienced hypoglycemic episodes or hospital emergency visits. Triglycerides and total cholesterol decreased as well as surrogate markers of liver function. However, the levels of high-density and low-density lipoprotein cholesterol (HDL-C and LDL-C) as well as uric acid remain unchanged. Regarding quality of life, the median physical health score increased from 44.5 (39.7–51.4) at baseline to 48.0 (43.1–55.3; p = 0.007), and the median mental health score decreased from 42.1 (36.1–46.7) to 37.4 (30.3–43.7; p = 0.004). Conclusions: A structured weight-loss program is effective in the short term in reducing HbA1c, weight, and antidiabetic medication in patients with T2DM who are overweight or obese. Levels of HDL-C and LDL-C were not affected by short-term weight loss. The decline in mental health and the long-term effects of improved glycemic control require further trials.

e20531 Background: We report results of a randomized pilot study assessing the impact of amifostine and a low weight tailored resistance exercise (LWTRE) on body composition in head and neck cancer (HNC) patients undergoing concurrent chemoradiation (CCR). Methods: Forty patients with stage III-IVB HNC scheduled for primary or adjuvant CCR were enrolled. Patients underwent primary randomization to CCR +/- amifostine and secondary randomization to nutrition counseling +/- LWTRE. IV amifostine was administered prior to radiation. Nutritional counseling and LWTRE began day 1 of CCR and continued through month 6 post-CCR. Weight was obtained at baseline, 1, 3 and 6-months post-CCR. Patients underwent lean mass measures by dual-energy X-ray absorptiometry (DEXA) at baseline, 1 and 3 months post-CCR. Results: Mean weight loss (WL) was 11.6 ± 6.3 kg (p < 0.001) from baseline to 6 months post-CCR. Mean lean mass loss (LML) was 3.5 ± 3.0 kg (p< 0.001) from baseline to 3 months post-CCR. The greatest change occurred from baseline to 1-month post-CCR: 10% and 7% decrease in weight and lean mass respectively. The LWTRE was feasible and safe (no adverse events). Neither amifostine nor LWTRE impacted weight or lean mass loss. Conclusions: Results failed to demonstrate decline in WL/LML with either intervention. Data confirmed WL/LML as profound acute treatment toxicity. It underscored the protracted course of WL/LML. LML has been shown to predict clinical outcomes such as reduced functionality leading to decreased quality of life. Long term follow-up of patients is needed to clearly understand the trajectory and sequellae of body composition changes in HNC patients post-CCR. <p>With the development of information technology, cold chain logistics monitoring systems are widely used in transportation of fresh food to detect the quality parameters in real time. Poor network transmission performance, high packet loss rate and high maintenance cost are the main problems in the current cold chain logistics monitoring. To solve these problems, the paper presents a low-power monitoring system based on Narrow Band Internet of Things (NB-IoT). Considering the characteristics of the moving carriage, the polling mode of Bluetooth is proposed for the communication of one sink node to three sensor nodes. An improved working sequence is designed to reduce the power consumption of Bluetooth modules. Subsequently, analyzing the standard NB-IoT terminal state transition diagram, combining with the practical applications of cold chain logistics, an improved terminal state transition diagram is proposed and its terminal power consumption is analyzed. In the experimental test, the packet loss rate is less than 1% within 8 km. And the battery life of improved Bluetooth module is 143 times of that of traditional one. The power consumption of main service of NB-IoT is discussed, the formula is given which includes the transmission quality, random access times, number of concurrent users, sleep interval and other parameters. Through simulation and calculate, the battery life of terminal is up to 17 years. These improvements will greatly reduce the cost of cold chain logistics monitoring system. In addition, the packet loss rate test experiment proves the high reliability of the communication system.</p> <p>&nbsp;</p>

INTRODUCTION: Appetite for Life is a six-week primary care–based programme for women who are overweight, and aims to achieve long-term health gain through establishing healthy eating and physical activity patterns and a healthier weight. AIM: To evaluate the outcomes of Appetite for Life, a primary care–based healthy lifestyle programme for women who are overweight. METHODS: Two hundred and sixty-one women enrolled and consented to take part in the six-week Appetite for Life programme via general practice and were followed for 12 months. Eating behaviours and physical activity levels were measured at baseline, six weeks, six months and 12 months. Anthropometric and biomedical data was collected at visits to the participants’ general practitioners at baseline and 12 months. RESULTS: Positive lifestyle changes were reported that were sustained for the duration of the 12-month follow-up period. Participants reported an increase in intake of fruit and vegetables, dairy products, healthy fats and an increased level of physical activity. There was also an increase in reported enjoyment and participation in exercise. Mean weight was maintained over this time period. There was a reduction in mean LDL and total plasma cholesterol. DISCUSSION: A healthy lifestyle programme offered through primary care that is based on a non-dieting approach may help overweight women develop and sustain positive lifestyle changes. The aim of this study was to determine the influence of chronic supplementation with grape juice (400ml), in modulating the anthropometric and biochemical parameters. Thirty-nine seniors participated and were evaluated at baseline after 30 days. The chronic consumption of grape juice reduced weight, BMI and waist circumference. Indeed, the consumption modulated biochemical parameters, decreased the total cholesterol levels, Low-density lipoprotein (LDL) cholesterol, Urea and GGT. In addition, the intake of juice improved the performance in the TUG test, and caused more stability in the elderly. The levels of protein oxidation declined and the antioxidant potential significantly increased as well as SOD and the ratio SOD / CAT. In contrast, levels reduced sulfhydryl groups to consumption. In nuclear changes there are a decrease in the frequency of MN and picnosis. In conclusion, grape juice could be an excellent option to improve quality of life in elderly.

Higher-protein diets, meal replacements, and greater early weight loss have separately been associated with greater weight loss. We compared a high-protein, meal replacement program with daily energy restriction (DER) to one which provided greater energy restriction adding alternate day fasting (ADF + DER; alternating days of modified-fasting and DER plus 1 ad libitum day/week) on retention, weight loss, physiological, nutritional, and behavioral markers. Participants were randomized to ADF + DER or DER for 16 weeks (n = 162, age 40 ± 8 years BMI 36 ± 6 kg/m2 (Mean ± SD)) plus 8 weeks weight maintenance. At week 16 weight change was −10.7 ± 0.5 kg and −11.2 ± 0.6 kg in ADF + DER and DER groups (treatment NS). Fat mass, visceral adipose tissue, and lean mass (p < 0.05) were similarly reduced between treatments. Weight loss was sustained to 24 weeks (treatment NS). Fasting LDL-cholesterol, triglycerides, insulin, hsCRP, glucose, and blood pressure all improved (p < 0.05; treatment NS). Transferrin saturation, ferritin, serum zinc, folate, and B12 improved (p < 0.05; treatment NS). Plasma thiamine and vitamin D levels decreased, reflecting lower carbohydrate intakes and seasonal changes, respectively. Food cravings, quality of life, and mood improved (treatment NS). Energy, fatigue, and pain improved slightly more in DER (p < 0.05). This study supports the use of higher protein, meal replacement programs with or without ADF in weight management.

A retrospective follow-up study showed an advantage of weight reduction diet (WRD) to fasting therapy concerning long-term weight loss.Objective: Prospective comparison of WRD to fasting therapy concerning long-term weight loss. Methods: From 08/2007 to 02/2009 90 patients with overweight or obesity were observed with a follow-up period of 6 months, who received either a fasting therapy or a WRD in the context of naturopathic complex in-patient treatment including physical activity.Results: During the in-patient treatment the fasting patients loosed significantly more weight than the WRD patients, but regained weight during the follow-up, while the weight loss of the WRD patients persisted. 32% of the fasting and 81% of the WRD patients achieved a 5% weight loss after 6 months. Total and LDL-cholesterol were significantly more reduced, controlled eating habits, physical leisure activity and tendentially physical quality of life increased more after 6 months in the WRD group, while the systolic blood pressure declined more in the fasting patients.Conclusion: Weight loss and improvement of obesity-associated parameters were long-term significantly inferior in fasting patients compared to patients treated with a WRD. The reduced success of the fasting patients is probably caused by the minor lifestyle modification.

In this study, single-layer coating using chitosan (Ch) and sodium alginate (SA) solutions and their gel coating (ChCSA) formed by layer-by-layer (LbL) electrostatic deposition using calcium chloride (C) as a cross linking agent were prepared to improve storage qualities and shelf-life of fresh-cut purple-flesh sweet potatoes (PFSP). The preservative effects of single-layer coating in comparison with LbL on the quality parameters of fresh-cut PFSP, including color change, weight loss, firmness, microbial analysis, CO2 production, pH, solid content, total anthocyanin content (TAC), and total phenolic content (TPC) were evaluated during 16 days of storage at 5 °C. Uncoated samples were applicable as a control. The result established the effectiveness of coating in reducing microbial proliferation (~2 times), color changes (~3 times), and weight loss (~4 times) with negligible firmness losses after the storage period. In addition, TAC and TPC were better retained in the coated samples than in the uncoated samples. In contrast, quality deterioration was observed in the uncoated fresh cuts, which progressed with storage time. Relatively, gel-coating ChCSA showed superior effects in preserving the quality of fresh-cut PFSP and could be suggested as a commercial method for preserving fresh-cut purple-flesh sweet potato and other similar roots.

Owing to the prohibitive costs of generating large amounts of labeled data, programmatic weak supervision is a growing paradigm within machine learning. In this setting, users design heuristics that provide noisy labels for subsets of the data. These weak labels are combined (typically via a graphical model) to form pseudolabels, which are then used to train a downstream model. In this work, we question a foundational premise of the typical weakly supervised learning pipeline: given that the heuristic provides all “label” information, why do we need to generate pseudolabels at all? Instead, we propose to directly transform the heuristics themselves into corresponding loss functions that penalize differences between our model and the heuristic. By constructing losses directly from the heuristics, we can incorporate more information than is used in the standard weakly supervised pipeline, such as how the heuristics make their decisions, which explicitly informs feature selection during training. We call our method Losses over Labels (LoL) as it creates losses directly from heuristics without going through the intermediate step of a label. We show that LoL improves upon existing weak supervision methods on several benchmark text and image classification tasks and further demonstrate that incorporating gradient information leads to better performance on almost every task.

Abstract Objectives: To determine the association between eating window and time of last calorie intake with body composition and cardiometabolic health in obese older adults. Methods: We performed a cross-sectional analysis on 36 community-dwelling, overweight-to-obese (BMI 28.0-39.9 kg/m2) older adults, recruited to participate in a weight loss and exercise trial. Time of food intake were extracted from three 24-hour food recalls. Eating window was calculated as the time elapsed between the first and last food intake. We recorded the time of last calorie intake either from food or drink. Blood glucose, triglycerides, high-density (HDL) & low-density (LDL) lipoprotein cholesterols were measured as markers of cardiometabolic health. Total fat and lean mass were assessed by DXA. Partial correlation was used to determine the relationships between eating window and last calorie intake with body composition and cardiometabolic markers, while controlling for sex, age, and total calorie intake. Results: On average, participants’ eating window was 12.0±1.1 hours. Time of last calorie intake in 86% of participants was between 6:00-8:00 PM. After controlling for potential confounders, longer eating windows were associated with higher triglyceride levels (P=0.032) and lower HDL (P=0.035), while no association was observed with the other cardiometabolic markers. We observed negative trends, though not statistically significant, between longer eating windows and greater weight, BMI, and fat mass. No association was observed between time of last calorie intake, body composition and cardiometabolic markers. Conclusions: Our results suggest that timing of food intake may influence cardiometabolic risk and obesity in older adults.

Aim. The current study was aiming to investigate the relation between vitamin D, retinopathy, and hearing loss among type 2 diabetes mellitus (T2DM) patients. Methods. Cross-sectional study carried on 638 subjects aged between 20 and 60 years who visited the Endocrinology, Ophthalmology, and ENT Outpatient Clinics of the Medipol Hospital during the period from March 2016 to May 2017. Two audiometers Grason Stadler GSI 61 and Interacoustics AC40 Clinical audiometer were used to evaluate the hearing loss. Risk factors for diabetic retinopathy were evaluated, including age, sex, diabetes duration, glycated hemoglobin (HbA1c), hypertension, and lipid profiles. Results. The mean age (± SD, in years) for retinopathy with hearing loss versus normal subjects was 47.7 ±10.2 versus 48.5±9.1. The associated risk factors were significantly higher in T2DM with hearing loss, hypertension (32.6% versus 15.7%), tinnitus (40.0% versus 18.0%), vertigo (59.7% versus 26.8%), and headache (54.9% versus 45.3%), than in normal hearing diabetes. There were statistically significant differences between hearing impairment versus normal hearing for vitamin D [19.40±9.71 ng/ml versus 22.67±9.28 ng/ml; p<0.001], calcium, magnesium, phosphorous, cholesterol, HDL-C, LDL-C, albumin, systolic blood pressure [131.70±9.25 Hg versus 127.73±11.98 Hg], diastolic blood pressure [82.20±8.60 mm Hg versus 79.80±8.20 mm Hg], and microalbuminuria. Multivariate logistic regression analysis revealed that variables such as vertigo, duration of DM, mobile/I pad phone, vitamin D deficiency, sleeping disturbance, headache, frequently TV watching, tinnitus, cigarette smokers, and hypertension were considered at higher risk as a predictors of retinopathy with hearing loss among diabetic patients. Conclusion. Vitamin D deficiency is considered as a risk factor for diabetic retinopathy and hearing loss among diabetic patients. Meanwhile, hyperglycemia could be considered as a modifiable risk factor for diabetic retinopathy; tight glycemic control may be the most effective and important therapy for improving quality of life and substantially reducing the incidence of retinopathy and in T2DM patients.

Cholesteryl ester storage disease (CESD) results from loss-of-function mutations in LIPA, the gene that encodes lysosomal acid lipase (LAL). Hepatomegaly and deposition of esterified cholesterol (EC) in multiple organs ensue. The present studies quantitated rates of synthesis, absorption, and disposition of cholesterol, and whole body cholesterol pool size in a mouse model of CESD. In 50-day-old lal −/− and matching lal +/+ mice fed a low-cholesterol diet, whole animal cholesterol content equalled 210 and 50 mg, respectively, indicating that since birth the lal −/− mice sequestered cholesterol at an average rate of 3.2 mg·day−1·animal−1. The proportion of the body sterol pool contained in the liver of the lal −/− mice was 64 vs. 6.3% in their lal +/+ controls. EC concentrations in the liver, spleen, small intestine, and lungs of the lal −/− mice were elevated 100-, 35-, 15-, and 6-fold, respectively. In the lal −/− mice, whole liver cholesterol synthesis increased 10.2-fold, resulting in a 3.2-fold greater rate of whole animal sterol synthesis compared with their lal +/+ controls. The rate of cholesterol synthesis in the lal −/− mice exceeded that in the lal +/+ controls by 3.7 mg·day−1·animal−1. Fractional cholesterol absorption and fecal bile acid excretion were unchanged in the lal −/− mice, but their rate of neutral sterol excretion was 59% higher than in their lal +/+ controls. Thus, in this model, the continual expansion of the body sterol pool is driven by the synthesis of excess cholesterol, primarily in the liver. Despite the severity of their disease, the median life span of the lal −/− mice was 355 days.

Higher penetration of variable renewable energy sources into the grid brings down the plant load factor of thermal power plants. However, during sudden changes in load, the thermal power plants support the grid, though at higher ramping rates and with inefficient operation. Hence, further renewable additions must be backed by battery energy storage systems to limit the ramping rate of a thermal power plant and to avoid deploying diesel generators. In this paper, battery-integrated renewable energy systems that include floating solar, bifacial rooftop, and wind energy systems are evaluated for a designated smart city in India to reduce ramping support by a thermal power plant. Two variants of adaptive-local-attractor-based quantum-behaved particle swarm optimization (ALA-QPSO) are applied for optimal sizing of battery-integrated and hybrid renewable energy sources to minimize the levelized cost of energy (LCoE), battery life cycle loss (LCL), and loss of power supply probability (LPSP). The obtained results are then compared with four variants of differential evolution. The results show that out of 427 MW of the energy potential, an optimal set of hybrid renewable energy sources containing 274 MW of rooftop PV, 99 MW of floating PV, and 60 MW of wind energy systems supported by 131 MWh of batteries results in an LPSP of 0.005%, an LCoE of 0.077 USD/kW, and an LCL of 0.0087. A sensitivity analysis of the results obtained through ALA-QPSO is performed to assess the impact of damage to batteries and unplanned load appreciation, and it is found that the optimal set results in more energy sustainability.

Familial hypercholesterolemia is a rare, monogenic, co-dominant, life-threatening disorder resulting from loss of function mutations in the genes responsible for synthesis of low-density lipoprotein receptors or apo-B genes or gain of function mutations in PCSK9 genes in the liver which affects 0.2% of the population. It is characterized by severe lifelong elevation of LDL cholesterol and by development of xanthelasma, xanthomas, premature coronary artery disease and peripheral artery occlusive disease. Most patients develop PCAD and aortic stenosis before the age of 20 years and die before 30 years of age. The diagnosis of FH is usually based on clinical presentation and commonly used criteria are the Dutch lipid clinic network criteria, Simon Broome criteria or the WHO criteria. We encountered four cases of familial hypercholesterolemia over last 10 years. All the four patients presented with effort angina and all were found to have obstructive coronary artery disease oncoronary angiogram and two of them had severe supravalvular aortic stenosis. All four patients were on dietary modifications, high intensity statin and cholesterol absorption inhibitor. Two patients underwent coronary artery bypass grafting with aortoplasty, one patient underwent coronary artery bypass grafting and one patient underwent percutaneous transluminal coronary angioplasty. Objective This systematic review evaluated the effect of testosterone replacement therapy (TRT) in men with obesity having low testosterone levels (LTLs). Design and methods Search strategies were performed in MEDLINE, Embase, LILACS, and CENTRAL databases. Two reviewers selected the studies, assessed the risk of bias, and extracted data from the included studies. A random-effects model was used to pool results across studies, and the Grading of Recommendations Assessment, Development, and Evaluation was used to evaluate the certainty of evidence. Results A total of 16 randomized controlled trials were included. With moderate certainty of the evidence, no difference was found between TRT and placebo regarding total adverse events, TRT led to a 2-kg lean body mass gain and slightly improved low-density lipoprotein (LDL), without effects on the blood pressure. Due to imprecision/heterogeneity, effects in cardiovascular events (relative risk: 0.52, 95% CI: 0.26 to 1.05, 7 trials, 583 participants), high-density lipoprotein, hematocrit, prostate-specific antigen, HbA1c, and quality of life were unclear. TRT was effective for waist circumference and BMI; however, large between-study heterogeneity was found, with 95% prediction intervals crossing the null effect line. Meta-regression revealed that the average age of participants was a significant modifier for both outcomes. Conclusion TRT slightly improved the lean body mass and LDL in men with obesity having LTLs but did not affect the blood pressure. The effects of TRT on cardiovascular events, HbA1c, and quality of life are unclear. The mean age of participants significantly modified the effect of TRT on weight loss.

Adult neurons are believed to be in a state of growth arrest. The generation of neurons is complete at the time of birth in most of the brain regions. However neurogenesis is present through life in the dentate gyrus of hippocampus and the lateral ventricles due to the presence of neural stem cells (NSC). This postnatal neurogenesis in hippocampus plays a critical role in cognitive development mainly in learning and memory functions. NSC are self-renewing, multipotent cells that generate the neurons and glia of the nervous system. Due to their high proliferation, NSC are highly sensitive to ionizing radiation. This review describes the current knowledge on impact of ionizing radiation on neural stem cells biology. Widening the knowledge of mechanisms involved in radiation-induced neurotoxicity at the level of NSC may help to overcome in the future the side effects occurring after anti-cancer therapies of the brain and help to protect and maintain neurogenesis.

Here we describe a full-length TFPI molecule, expressed in mouse C127 cells (TFPIC127), which exhibits virtually no cellular binding yet contains the intact carboxy terminus. This TFPI (TFPIC127) is neither internalized nor degraded via the TFPI endocytic receptor, LDL-receptor–related protein. Covid-19 has been threatening human life and is now the most serious public health issue in the world. During this pandemic, wearing masks is one of the most effective ways to inhibit virus transmission. However, existing ubiquitous identity recognition requires people to remove their masks to complete facial recognition, which is highly risky. Iris recognition, as a safer applicable identification method, has its fatal weakness of not being able to achieve satisfactory real-time recognition on end devices. This paper presents an edge deployment of a low-power iris recognition system based on FPGA with approximate multipliers. We adopted a serial-parallel hybrid method for the preprocessing stage, trained the CNN model on PC and then deployed the architecture and parameters on FPGA. We further reduced power and resource consumption by designing approximate multipliers for the key calculation. Experimental results show that design achieves up 28% and 43% gain in terms of area and latency energy product, while incurring a negligible accuracy loss. The recognition speed increased by 40% compared with Raspberry Pi, 11 times better than Jetson Nano power latency production.

Background: By 2025, it is projected that one billion adults will be classified as having obesity worldwide. Unfortunately, the disease of obesity remains vastly undertreated. As an example, in the United States, nearly half of the adults meet recommendations for anti-obesity pharmacotherapy (defined as having a BMI ≥ 30, or a BMI ≥ 27 with weight-related diseases). Owing to its impact on weight loss, remission of diabetes mellitus, and metabolic syndrome, treatment of obesity with medical therapy like Liraglutide 3.0 mg injection has offered hope for obese individuals. In recent years, obesity has increased in the UAE and the use of medical treatment for obesity has increased in-line with this trend. However, data regarding medical treatment for obesity outcomes in diabetic and non-diabetic Emirati people is scarce. Objectives: The primary objective of the study is to evaluate the effect of Liraglutide 3.0 mg injection for weight management in patients with diabetes mellitus. In addition, we evaluated the impact of weight reduction in improving the metabolic parameters (HbA1c and LDL-c for a patient living with type2 diabetes). Secondary objectives include improvement of infertility in females with PCOS and improvement of psychological well-being, functional capacity and quality of life. Also, we evaluated the response post-bariatric group who received Liraglutide 3.0 mg to treat their post-bariatric weight regain. Results: In terms of weight changes, the average weight at baseline was 100.98 kg and had decreased to 97.54 kg, 96.00 kg, and 96.37 kg at 3, 6, and 12 months, respectively. Males experienced a greater rate of weight reduction than females, while diabetic patients had a lower percentage of weight loss than non-diabetic patients. Additionally, patients with PCOS were able to conceive after reducing 10% of their body weight after using Liraglutide, while the remaining 15 patients who were married but unable to conceive had an improvement in their menstrual cycle and hirsutism. Conclusion: The results demonstrated that Liraglutide helps to induce and sustain weight loss in patients with obesity. It is an effective treatment for weight reduction and offers the unique benefit of improved glycaemic control. In addition, the study identified a correlation between weight reduction and improvements in psychological health, quality of life, and fertility in females with PCOS. Additional studies are needed to determine its long-term efficacy and safety profile.

Low-illumination image restoration has been widely used in many fields. Aiming at the problem of low resolution and noise amplification in low light environment, this paper applies style transfer of CycleGAN(Cycle-Consistent Generative Adversarial Networks) to low illumination image enhancement. In the design network structure, different convolution kernels are used to extract the features from three paths, and the deep residual shrinkage network is designed to suppress the noise after convolution. The color deviation of the image can be resolved by the identity loss of CycleGAN. In the discriminator, different convolution kernels are used to extract image features from two paths. Compared with the training and testing results of Deep-Retinex network, GLAD network, KinD and other network methods on LOL-dataset and Brightening dataset, CycleGAN based on multi-scale depth residuals contraction proposed in this experiment on LOL-dataset results image quality evaluation indicators PSNR = 24.62, NIQE = 4.9856, SSIM = 0.8628, PSNR = 27.85, NIQE = 4.7652, SSIM = 0.8753. From the visual effect and objective index, it is proved that CycleGAN based on multi-scale depth residual shrinkage has excellent performance in low illumination enhancement, detail recovery and denoising.

Mental health problems are conditions that do not meet the criteria for disease but affect the normal quality and continuity of everyday life. They are the emotional and psychological difficulties of the individual, which occur as a result of circumstantial or persistent stresses or of a reflection of the emotions between the individual and the environment where he lives (WHO, 2005). Health is seen as "not just a lack of sickness or disability," but rather as "a complete physical, mental and social well-being." This definition significantly expands the concept of mental health, which should not be defined in a restrictive manner as the absence of mental disorders, but should be defined in a positive and comprehensive view (WHO, 1985). Ultimately, mental health is defined as "a state of well-being in which the individual realizes his or her own potential, is able to cope with daily life concerns, is productive and fruitfully in his work, and is capable of delivering his contribution to the community where he lives (WHO, 2001). Mental health problems are usually associated with important concerns in social, professional, or other important activities. Electronic sports (eSports) are a category of video games that are played competitively following rules, either in teams or individually. Currently, the top videogame is League of Legends (LoL), a game in the genre known as Multiplayer Online Battle Arena (MOBA) with more than 100 million monthly active players worldwide. Re­search has shown that video games can satisfy basic psychological needs and be experienced as a passion, and that passion is a useful construct to help understand the different motivational patterns of video game use. In addition, no relevant studies have been found with professional players. There­fore, the objective of this study is to explore whether knowledge of the degree of frustration of gamers’ basic psy­chological needs in everyday life, users’ passion and their motives to play makes it possible to distinguish between types of players (amateurs or semi-professionals). The participants were 195 Spanish-speaking LoL players (156 amateurs and 39 semi-profes­sionals) who were passionate about LoL. The results indicate that the game is most often played for competitive, social and exploration reasons. As for passion, players play more out of harmonious passion than out of obsessive passion, the latter being associated with the frustration of basic psychological needs. As a whole, being younger, less motivated by the plot of the game, having greater motivation to improve mental abilities, and spending more hours playing are predictors of the type of player (semi-professional). It is concluded that eSports players are not characterized as obsessed or frustrated people, and that the use of videogames is not necessarily harmful.Los deportes electrónicos (eSports) son un conjunto de videojuegos que se juegan de manera competitiva siguiendo unas reglas, ya sea por equipos o en solitario. Actualmente, el videojuego por excelencia es el League of Legends (LoL), del género Mul­tiplayer Online Battle Arena (MOBA), con más de 100 millones de jugadores mensuales activos a nivel mundial. La investigación ha demostrado que los videojuegos pueden satisfacer necesidades psicológicas básicas y ser vividos como una pasión, y que la pasión es un constructo útil para ayudar a comprender los diferentes patrones motivacionales del uso del videojuego. Además, no se han encontrado estudios relevantes con jugadores profesionales. Así pues, el objetivo de este estudio es explorar si la frustración de las necesidades psicológicas básicas en la vida cotidiana, la pasión y los motivos de juego permiten diferenciar el tipo de jugador (amateur y semi-profesional). Los participantes fueron 195 jugadores de LoL (156 amateurs y 39 semi-profesionales) castellano-hablantes y apasionados al LoL. Los resultados indican que el uso predominante del juego son los motivos competitivos, sociales y de exploración. En cuanto a la pasión, los jugadores juegan más por pasión armoniosa que por pasión obsesiva, siendo esta última junto con la frustración de las necesidades psicológicas básicas las más bajas. En conjunto, tener menor edad, una menor motivación por la historia del juego, una mayor motivación por mejorar las capacidades mentales, y dedicar más horas de juego por semana son predictores del tipo de jugador (semi-profesional). Se concluye que los jugadores de eSports no se caracterizan por ser personas obsesivas o frustradas, y que el uso del videojuego no es necesariamente perjudicial.

MATERIALS AND METHODS: A cross sectional study was conducted to assess dental caries, periodontal health status, and oral health-related quality of life (OHRQoL) in prisoners of Nellore district. The Oral Health Impact Profile -14 questionnaires was given to the participants followed by Oral health status was assessed using the WHO Oral Health Assessment Form (2013). RESULTS: A total of 115 prisoners were included in the study. The prevalence of dental caries was 53.1% with mean DMFT of 2.6±3.5. Pockets measuring 4-5mm was observed among 10.4% and >6 mm was observed among 21.7% of prisoners. LOA was seen among 27.8% of prisoners. A significant correlation was found between OHIP-14 with Dental caries and periodontal diseases. CONCLUSION: This study concluded that periodontal health and dentition status was poor among prisoners due to lack of accessibility to dental care and dental health education. Abstract:
Background: Stroke is defined as abrupt onset of symptoms and/or sign of focal and global loss of cerebral function lasting for at least 24 hrs with no apparent cause other than of vascular origin. Dyslipidemia in stroke is not widely studied especially from the population of Northeast India (Manipur). Moreover, most prior studies did not specifically compare hemorrhagic stroke risk among those with elevated lipid levels to those with optimal or near optimal lipid levels. Therefore, we studied the levels of different Lipids in stroke patients (hemorrhagic and ischemic) to understand its role in pathogenesis of stroke. Methods: This cross-sectional study was conducted in RIMS, Manipur from 2017-2019 and included 185 patients above 18 years with stroke within 48 hrs of onset of symptoms. NCCT brain and serum lipids, other related blood investigations were performed.Results: We found that serum total cholesterol, LDL-C and triglycerides are significantly higher in infarction patients with average levels of 210.82 mg/dl, 143.64 mg/dl and 151.50 mg/dl respectively which is statistically significant (p<0.001). The average serum total cholesterol, LDL-C and triglycerides are significantly lower in ICH patients with average levels of 143.34 mg/dl, 84.67 mg/dl and 113.94 mg/dl respectively which is statistically significant. (p<0.001).Conclusions: We concluded that raised level of serum total cholesterol, serum LDL-C, serum triglyceride are associated with cerebral infraction (p=0.001) and lower level of serum total cholesterol, serum LDL-C, serum triglyceride are associated with cerebral hemorrhage (p=0.001). Thus, serum lipid levels should be carefully monitored to prevent life threatening stroke.

Glejak wielopostaciowy (glioblastoma multiforme, GBM) należy do najbardziej złośliwych nowotworów pierwotnych mózgu. Charakterystyczną cechą GBM jest jego naciekający wzrost, zdolność komórek do szerzenia się wzdłuż dróg nerwowych istoty białej oraz możliwość inwazji przeciwnej półkuli mózgu. Agresywna natura glejaka wielopostaciowego sprawia, że u nieleczonych pacjentów przeżycia wynoszą jedynie 3 - 6 miesięcy od rozpoznania choroby. Po 65 roku życia odnotowuje się gwałtowny wzrost przypadków glejaka wielopostaciowego. Obecnie na świecie około połowa pacjentów z tym nowotworem ma więcej niż 65 lat. Biorąc pod uwagę starzenie się społeczeństwa oblicza się, że coraz bardziej rosnąć będzie liczba nowo stwierdzanych przypadków GBM u pacjentów w podeszłym wieku, a problem odpowiedniego podejścia terapeutycznego do tej szczególnej grupy chorych będzie coraz istotniejszy. Przez wiele lat klinicyści odnosili się z niechęcią do wdrażania u osób starszych metod leczenia z powodzeniem stosowanych u młodych pacjentów. Tak więc poprzez samo kierowanie się wiekiem terapia starszych chorych z GBM była często suboptymalna. Tymczasem zmiana w podejściu do leczenia pacjentów w starszym wieku udowodniła, że najważniejszym czynnikiem branym pod uwagę przed rozpoczęciem leczenia powinien być stan ogólny chorego oceniony według powszechnie uznawanych skal (np. skali sprawności Karnofsky’ego). Wiele badań klinicznych potwierdziło, że zastosowanie technik operacyjnych, metod radioterapii i chemioterapii z temozolomidem przynosi również istotny zysk starszym pacjentom (wydłuża przeżycia całkowite, poprawia jakość życia), w tym nawet chorym po 80 roku życia. Glioblastoma multiforme (GBM) is one of the most malignant primary brain tumour.There is a rapid increase in the number of glioblastoma multiforme cases found in patients older than 65 years old. Currently about half of patients with this cancer are more than 65 years old. Taking into account the aging of the population it is calculated that the number of newly diagnosed GBM cases in older patients will grow more and more and the problem of an appropriate therapeutic approach to this specific group of patients will become more and more important. For many years clinicians were reluctant to implement in older patients methods of treatment successfully used in young patients. Thus by taking into consideration only the age the therapy of older patients with GBM was often suboptimal. Meanwhile, a change in the approach to the treatment of elders proved that the most important factor taken into account before starting treatment should be the general condition of the patient assessed according to commonly accepted scales (eg Karnofsky score). Many clinical trials have confirmed that the use of surgical techniques, radiotherapy methods and chemotherapy with temozolomide brings significant benefits to older patients (prolongs overall survival, improves quality of life), including those over 80 years old. INTRODUCTION: Long-term results from bariatric surgery amongst individuals with obesity is considered good in general, with regard to weight loss, complications of obesity or quality of life. However, risk of nutrient deficiency might be increased. The aim of the study was to assess vitamin D status of patients before and after metabolic and bariatric surgery at Landspitali. METHODS: Data on 25(OH)D concentration and parathyroid hormone (PTH) was retrieved from medical records for patients undergoing metabolic and bariatric surgery at Landspitali from 2001-2018 (n=539). Insufficient vitamin D status was defined as 25(OH)D concentration <45 nmol/L in 2001-2012 but <50 nnmol/L from 2013-2018, due to changes in analytical method during the study period. Vitamin D deficiency was defined as 25(OH)D <30 nmol/L for both time periods. Guidelines on supplement use are provided before discharge from the hospital and up to 18 months after surgery, RESULTS: Mean concentration of 25(OH)D before surgery was 51 nmol/L (SD 30 nmol/L) and 278 (52%) had insufficient vitamin D status, of which quarter of subjects were defined as being vitamin D deficient. Concentration of 25(OH)D increased after surgery in majority of subjects (85%). However, about third of those defined as having insufficient vitamin D status before surgery still had insufficient status 18 months after surgery. When comparing time periods, 2001-2012 and 2013-2018 it can be seen that insufficient vitamin D status was less common in the second period, although still persistent in about 25% of cases before surgery and 8,5% 18 months after surgery. CONCLUSION: Insufficient vitamin D status is relatively common before metabolic and bariatric surgery at Landspitali. In large majority of subjects, 25(OH)D concentration increased after surgery, following recomendations on supplement intake. The results highlight the need for greater emphasis on correcting possible nutrient deficiencies before surgery.

Abstract:
Objective: To observe the efficacy and safety of Lishi Oral Liquid (LOL), a Chinese herbs formula, combined with levocetirizine, an antihistamine drug, in patients with damp-heat atopic dermatitis (AD). Methods: A randomized double-blind placebo-controlled clinical trial was conducted. Ninety patients diagnosed with damp-heat atopic dermatitis were randomly assigned to Lishi Oral Liquid group and control group at the ratio of 1:1. Patients were given one Lishi Oral Liquid or placebo three times per day. Both groups were treated with allantoin and vitamin E cream by topical use and levocetirizine by oral administration. Patients were followed up at the second and fourth week. The primary outcome was the scoring of atopic dermatitis (SCORAD). The secondary outcomes were visual analogue scale (VAS) and dermatology life quality index (DLQI). Results: After two weeks of treatment, there were statistical differences in SCORAD between these two groups ([Formula: see text]), whereas VAS and DLQI showed no statistical differences. There were significant differences in SCORAD and VAS between two groups at the end of four weeks of treatment ([Formula: see text]). However, no significant differences were observed in DLQI after four weeks of treatment. No serious adverse event was found during this clinical trial. Conclusion: LOL combined with levocetirizine is effective and safe for damp-heat AD treatment.

IntroductionIn type 1 diabetes, potential loss of life-years is greatest in those who are youngest at the time of onset. Using data from a nationwide cohort of patients with type 1 diabetes, we aimed to study risk factor trajectories by age at diagnosis.Research design and methodsWe stratified 30 005 patients with type 1 diabetes aged 18–75 years into categories based on age at onset: 0–10, 11–15, 16–20, 21–25, and 26–30 years. HbA1c, albuminuria, estimated glomerular filtration rate (eGFR), body mass index (BMI), low-denisty lipoprotein (LDL)-cholesterol, systolic blood pressure (SBP), and diastolic blood pressure trends were analyzed using mixed models. Variable importance for baseline HbA1c was analyzed using conditional random forest and gradient boosting machine approaches.ResultsIndividuals aged ≥16 years at onset displayed a relatively low mean HbA1c level (~55–57 mmol/mol) that gradually increased. In contrast, individuals diagnosed at ≤15 years old entered adulthood with a mean HbA1c of approximately 70 mmol/mol. For all groups, HbA1c levels stabilized at a mean of approximately 65 mmol/mol by about 40 years old. In patients who were young at the time of onset, albuminuria appeared at an earlier age, suggesting a more rapid decrease in eGFR, while there were no distinct differences in BMI, SBP, and LDL-cholesterol trajectories between groups. Low education, higher age, and poor risk factor control were associated with higher HbA1c levels.ConclusionsYoung age at the diabetes onset plays a substantial role in subsequent glycemic control and the presence of albuminuria, where patients with early onset may accrue a substantial glycemic load during this period.
