This monograph presents the basics of super-element modeling method of two-phase fluid flows occurring during the development of oil reservoir. The simulation is performed in two stages to reduce the spatial and temporal scales of the studied processes. In the first stage of modeling of development of oil deposits built long-term (for decades) the model of the global dynamics of the flooding on the super-element computational grid with a step equal to the average distance between wells (200-500 m). Local filtration flow, caused by the action of geological and technical methods of stimulation, are modeled in the second stage using a special mathematical models using computational grids with high resolution detail for the space of from 0.1 to 10 m and time — from 102 to 105 C.
The results of application of the presented models to the solution of practical tasks of development of oil reservoir. Special attention is paid to the issue of value transfer in filtration-capacitive properties of the reservoir, with a detailed grid of the geological model on the larger grid reservoir models.
Designed for professionals in the field of mathematical and numerical modeling of fluid flows occurring during the development of oil fields and using traditional commercial software packages, as well as developing their own software. At the present time, the tourist destination offers a stimulating laboratory for the experimentation of theoretical models and good practices on the subjects of governance, knowledge management and sustainable competition. Growing interest in the study of this territorial context gains impetus from the new approaches and tools that local administrations are starting to introduce in the phases of implementation and control of local strategies. In this respect, the Tourist Destination Observatory (OTD) represents an important innovation, offering a nerve centre for the aggregation and networking of heterogeneous data scattered over the territory as well as a model for the implementation of permanent approaches to social dialogue as prerequisites for the creation of knowledge and for an aware, shared, competitive and responsible development of the destination. The OTD can act as an efficient agent of local change, facilitating the processes of governance, and as a tool of knowledge management for the valorisation of intellectual capital. It is consequently a crucial support for the strategic repositioning of mountain resorts, which can represent valid responses to the emerging new modes of interpreting the holiday.

Chronic neuropathic groin pain may be iatrogenic or posttraumatic and can be disabling or even crippling in some individuals. Patients may have significant sleep disturbances and may experience psychosocial effects along with significant physical limitations. A combination of pharmacologic treatments with physical therapy and local infiltrations may be useful. Neurostimulation techniques, including spinal cord stimulation, peripheral nerve stimulation, and dorsal root ganglion stimulation, have shown promising results in the treatment of chronic neuropathic pain. In certain cases, surgical approaches, including selective neurectomy, can be effective; in other cases, the pain will remain chronic and intractable despite all interventional measures. In summary, patients with neuropathic groin pain can be treated after a thorough pretreatment investigation. Dorsal root ganglion stimulation is a viable option and should be considered when treating this challenging patient population.

Lower limb nerve blocks gained popularity with the introduction of better nerve localization techniques such as peripheral nerve stimulation and ultrasound. A combination of lower limb peripheral nerve blocks can provide anaesthesia and analgesia of the entire lower limb. Lower limb blocks, as compared to central neuraxial blocks, do not affect blood pressure, can be used in sick patients, provide longer-lasting analgesia, avoid the risk of epidural haematoma or urinary retention, provide better patient satisfaction, and have acceptable success rates in experienced hands. Detailed knowledge of the relevant anatomy is essential before performing any nerve blocks in the lower limb as the nerve plexuses and the peripheral nerves are deep and obscured by bony structures and large muscles. The lumbosacral plexus provides sensory and motor innervation to the superficial tissues, muscles, and bones of the lower limb. This chapter covers different approaches and techniques for lower limb blocks, that is, the lumbar plexus, femoral nerve, fascia iliaca, saphenous nerve, sciatic nerve, popliteal nerve, ankle block, forefoot block, and the intra-articular infusion of local anaesthetics. Both peripheral nerve stimulator- and ultrasound-guided approaches are discussed. The use of ultrasound guidance is suggested as it helps in reducing the dose of local anaesthetic required and can ensure circumferential spread of local anaesthetic around peripheral nerves, which hastens the onset of block and improves success rate.

The pharmacotherapies divide into analgesics, such as non-steroidal anti-inflammatory drugs (NSAIDs); adjuvant analgesics, such as tricyclic antidepressants and anticonvulsants; oral anesthetic agents (cardiotropics); adrenergic agonists; topical agents such as capsaicin and local anesthetic solutions and ointments; and muscle relaxants such as cyclobenzaprine, tizanidine, and baclofen. Interventions include many best administered by anesthesiologists such as infusions of anesthetic agents; trigger point injections; local and regional blockade, spinal injections including corticosteroids; and electrical spinal cord stimulation. Pain-free surgery can be imposed on the hand and wrist without resort to general anaesthetic. Options include local anaesthetic infiltration which can, in higher volumes mixed with adrenaline, allow surgery without a tourniquet. This technique (wide awake local anaesthetic without tourniquet or WALANT) permits the patient to move the fingers without the muscle paralysis induced by the regional anaesthetic and tourniquet, adding invaluable information, e.g. in tendon transfers. The efficacy of specific peripheral nerve blockade and brachial plexus block can be enhanced by ultrasound or nerve stimulation. Intravenous blockade (Bier’s) is effective. Tourniquets (finger, forearm, above elbow) are essential in hand surgery to provide a view unimpeded by blood (although WALANT can achieve this at the expense of a more oedematous field for procedures in a small field).

Regional anaesthesia is now extremely safe in experienced hands. The vast majority of upper and lower limb procedures can now be performed with either a peripheral regional block alone or in combination with a general anaesthetic. Neuraxial blocks can provide reliable postoperative pain relief for operations on the trunk and lower limbs. There is no consensus on the maximum safe dose of local anaesthetics. It is important therefore to use a minimum optimal dose of a local anaesthetic for any nerve block to reduce the risk of toxicity and to improve the success rate. Adjuncts, such as clonidine and dexamethasone, can prolong the duration of the block. Advances in nerve localization methods and block needles have further improved the safety of nerve blocks. There is increasing evidence to show that ultrasound is superior to peripheral nerve stimulation for identifying nerves. Ultrasound also helps in real-time visualization of the spread of the local anaesthetic. Consent, sedation, and support from non-anaesthetic staff play a key role in the success of regional anaesthesia, especially in awake patients. Although serious complications from nerve blocks are uncommon, direct nerve injury is perhaps the most serious complication. Fortunately, these symptoms in the overwhelming majority resolve within a year. This chapter covers the history, factors affecting local anaesthetics, role of adjuncts, nerve localization techniques, and complications of regional anaesthesia. Finally, some suggestions to improve the success and safety of peripheral nerve blocks are discussed.

Piriformis syndrome is an entrapment neuropathy caused by compression or irritation of the sciatic nerve as it courses in proximity to the piriformis muscle. Conservative treatment modalities for piriformis syndrome include the use of anti-inflammatory analgesic medications or muscle relaxants. Physical therapy is often employed to correct the abnormal pelvic biomechanics and focus on stretching the piriformis muscle. Prior to proceeding with invasive surgical approaches, this chapter advocates the use of piriformis muscle injection. The technique both confirms the diagnosis and offers therapeutic value while avoiding the risks, expense, and potential adverse outcomes associated with surgical interventions. A combined fluoroscopic and nerve stimulator guided technique is recommended to identify bony landmarks, verify the perisciatic location, confirm intramuscular spread of the injectate, and avoid intravascular injection of particulate steroid. Transient sciatic nerve block caused by spillover of the local anesthetic administered into the piriformis muscle is a common complication.

The final chapter of the book summarises its main results and conclusions. It formulates two insights. First, prostitution policy is fragile. Legalisation and decriminalisation are easily reversed, and revert back to criminalisation and heavy-handed regulation and control. This is a complex process that, triggered by the ever present sigma on prostitution and a dominant neo-abolitionist discourse, largely occurs at the local level, thereby deviating from, and even undoing, national policymaking. Second, without a detailed exposition and analysis of the design and implementation of prostitution policy at different scales of governance, statements about its nature or outcomes remain necessarily superficial and are at worst misleading. We conclude with the question: What can policy makers do to negotiate the complexity and unpredictability of the prostitution domain? Stimulating variation, facilitating new communication lines and selecting and promoting solutions that work are general strategies for effectively navigating such complexity. This requires the inclusion of stakeholders, particularly of more vulnerable groups such as sex workers, in policy formulation and implementation.

It focuses upon the period 1830–80, exploring the interactions between the many different actors who developed, administered, and used one of the most important technologies of the period—the electric telegraph. Drawing upon evidence from Prussia, Bavaria, Bremen, and a number of towns across Central Europe, it reveals the channels through which knowledge circulated across the region, stimulating both collaboration and confrontation between the scientists, technicians, businessmen, and bureaucrats involved in bringing the telegraph to life. It highlights the technology’s impact upon the conduct of trade, finance, news distribution, and government in the tumultuous decades that witnessed the 1848 revolutions, the wars of unification, and the establishment of the Kaiserreich in 1871. Following the telegraph lines themselves, it weaves together the changes which took place at a local, regional, national, and eventually global level, revisiting the technology’s impact upon concepts of space and time, and highlighting the importance of this period in laying the foundations for Germany’s experience of a profoundly ambiguous, networked modernity.

In Portugal, global politics tend to dominate climate change communication. Policy-oriented news stories prevail, being very much influenced by international events, dynamics, and actors, especially European ones, whereas national politicians and officials tend to be given less space. Climate change is thus mainly (re)presented as a global issue, distant from local realities, in spite of the vulnerabilities that the country faces. National policy makers tend to adopt a technocratic discourse that comes across as “rational” and fairly optimistic, with little contestation by environmental groups or others. A “green economy” discourse has prevailed in the media, with investment on renewable energy being depicted as the way to both stimulating the economy and addressing climate change. Scientific knowledge tends to be represented as consensual and national scientists tend to avoid dramatization. Although public opinion surveys have shown that the population considers climate change a serious problem and skepticism regarding its anthropogenic causes is low, surveys have also revealed high levels of ignorance and self-evaluated lack of information. In spite of a traditionally weak environmental movement and lack of public engagement, the population has shown a consistent sense of collective responsibility to tackle climate change. The economic and financial crisis up until the mid-2010s considerably affected the already fragile media system and turned political and public attention to economy-related topics. News coverage of climate change, in all its complexity, has been constrained by a lack of specialized reporters and increased dependency on the pro-activity of news sources.
