In The Movement for Global Mental Health: Critical Views from South and Southeast Asia, prominent anthropologists, public health physicians, and psychiatrists respond sympathetically but critically to the Movement for Global Mental Health (MGMH). They question some of its fundamental assumptions: the idea that "mental disorders" can clearly be identified; that they are primarily of biological origin; that the world is currently facing an "epidemic" of them; that the most appropriate treatments for them normally involve psycho-pharmaceutical drugs; and that local or indigenous therapies are of little interest or importance for treating them. The contributors argue that, on the contrary, defining "mental disorders" is difficult and culturally variable; that social and biographical factors are often important causes of them; that the "epidemic" of mental disorders may be an effect of new ways of measuring them; and that the countries of South and Southeast Asia have abundant, though non-psychiatric, resources for dealing with them. In short, they advocate a thoroughgoing mental health pluralism.

Abstract:
Diffuse X-ray scattering is a rich source of local structural information over and above that obtained by conventional crystal structure determination (crystallography). The main aim in the book is to show how computer simulation of a model crystal provides a general method by which diffuse scattering of all kinds and from all types of materials can be interpreted and analysed. Part I gives a description of the experimental methods used to obtain diffuse scattering data including recent improvements both in synchrotron and neutron facilities and latest detectors that are now available. Part II describes a number of stochastic models of disorder, which allows various concepts to be established and enables simple examples to be generated to illustrate key principles. Part III describes example studies of a wide variety of real materials. These examples not only document the development of computer simulation methods for investigating and analysing disorder problems but also provide a resource for helping future researchers recognise the kinds of effects which can occur and for pointing the way to tackling new problems which are encountered. Part IV describes more recent studies in which the increased computational power, coupled with improvements in the quality and quantity of data that is now obtainable, has allowed more quantitative and detailed analyses to be undertaken.

This chapter reviews recent literature related to local anesthetic sympathetic ganglion blockade (SGB) in treatment of post-traumatic stress disorder (PTSD) and its effect on the overall function of patients, with specific emphasis on male sexual and marital function. This chapter also discusses a publically available video in which a patient who had the PTSD checklist (PCL) administered before, and following, SGB as well as his wife were interviewed. A 34 year-old male Army Ranger veteran who suffered severe PTSD with associated marital and sexual difficulties after deployment, the patient had reported decreased libido and avoidance of intimacy. After SGB, he noticed marked improvement of his PTSD symptoms and resolution of his sexual dysfunction and marital difficulties. His reported improvements were echoed by his wife. The chapter also discusses known mechanisms of sexual dysfunction as related to PTSD and sympathetic nerve blockade effects as related to the sympathetic nervous system.

Abstract:
Various disorders result from genetically determined abnormalities of enzymes, the metabolic consequences of which affect the development or functioning of the nervous system. The range of metabolic disturbances is wide, as is the resultant range of clinical syndromes. Although most occur in children, some can present in adult life, and increasing numbers of affected children survive into adult life. In some, specific treatments are possible or are being developed. The last 20 years has seen a considerable expansion in our understanding of the genetic and metabolic basis for many neurological conditions. Particular clinical presentations of neurometabolic disorders include ataxias, movement disorders, childhood epilepsies, or peripheral neuropathy. Detailed coverage of the entire range of inherited metabolic diseases of the nervous system is available in other texts (Brett 1997; Scriver et al. 2001; Menkes et al. 2005).Treatment is possible for some metabolic diseases. For instance, the devastating neurological effects of phenylketonuria have been recognized for many years. Neonatal screening for this disorder and dietary modification in the developed world has removed phenylketonuria from the list of important causes of serious neurological disability in children. This success has led to new challenges in the management of the adult with phenylketonuria and unexpected and devastating effect of the disorder on the unborn child of an untreated Phenylketonuria mother. More recently Biotinidase deficiency has been recognized as an important and easily treatable cause of serious neurological disease usually presenting with early onset drug resistant seizures. This and some other neurometabolic diseases can be identified on neonatal blood screening although a full range of screening is not yet routine in the United Kingdom. More disorders are likely to be picked up at an earlier asymptomatic stage as the sophistication of screening tests increases (Wilcken et al. 2003; Bodamer et al. 2007).Although individual metabolic disorders are rare, collectively such disorders are relatively common. In reality most clinicians will see an individual condition only rarely in a career. Furthermore, patients with certain rare conditions are often concentrated in specialist referral centres, further reducing the exposure of general and paediatric neurologists to these disorders. A recent study into progressive intellectual and neurological deterioration, PIND, gives some information about the relative frequency and distribution of some childhood neurodegenerative diseases in the United Kingdom (Verity et al. 2000; Devereux et al. 2004). Although primarily designed to identify any childhood cases of variant Creutzfeldt- Jakob disease, the study also provided much information about the distribution of neurometabolic disease in children in the United Kingdom. The commonest five causes of progressive intellectual and neurological deterioration over 5 years were Sanfilippo syndrome, 41 cases, adrenoleukodystrophy, 32 cases, late infantile neuronal ceroid lipofuschinosis, 32 cases, mitochondrial cytopathy, 30 cases, and Rett syndrome, 29 cases. Notably, geographical foci of these disorders were also found and correlate with high rate of consanguinity in some local populations.

The chapter includes information on common problems experienced by women after the birth and during their post-birth recovery. Post-partum haemorrhage is covered here, in particular that caused by uterine infection which may rapidly develop into life-threatening sepsis. Included are aspects of physical problems such as circulatory or urinary disorders. The midwife is responsible for early detection and rapid referral to enable prompt treatment. Psychological disorders often escalate after the birth and can be overwhelming for the mother. Timely support and management are essential to ensure the return of the normal parameters for each individual mother. Therapeutic interventions are required so knowledge of the local referral systems is a requisite.

This chapter explains how neuro-palliative care can be provided to patients with severe brain injury. Before arguing that the right to die must be preserved and that the right to care for patients who are minimally conscious must be supported, it defines and reviews brain states that constitute disorders of consciousness along with their differential biology. It then gives an overview of palliative care for patients with severe brain injury and the challenges involved in diagnosing the minimally conscious state. It proceeds by discussing advances in technology, particularly neuroimaging, that may help meet the needs of such patients. It also considers the neuroethics of diagnosis and concludes by suggesting ways to integrate the needs of individuals suffering from disorders of consciousness in both the local and national palliative care infrastructure.

Rheumatoid arthritis (RA) is the most common inflammatory complication and affects approximately 1 % of the global population. It affects three times more women than men.
