Abstract:
This chapter examines a category of green radicalism that focuses on green consciousness. The stress on green consciousness means that the way people experience and regard the world in which they live, and each other, is the key to green change. Once consciousness has changed in an appropriate direction, then policies, social structures, institutions, and economic systems are expected to fall into place. This prioritization of consciousness is widespread in the green movement, among deep ecologists, bioregionalists, ecofeminists, ecotheologists, and lifestyle greens, among others. The chapter begins with a discussion of deep ecology, ecofeminism, bioregionalism, ecological citizenship, lifestyle greens, and ecotheology. It then considers romanticism, the discourse analysis of green consciousness, and the impact of green consciousness change. Finally, it highlights the challenges confronting green consciousness.

Many of the most common chronic diseases in the Western world are now preventable and even reversible using comprehensive lifestyle changes; this has spawned a new field named lifestyle medicine. Lifestyle medicine is a subset of integrative preventive medicine that focuses on changing the four or more core behaviors: nutrition, movement, relaxation, and social support. The diseases that have been shown to be reversible include even severe coronary heart disease, type 2 diabetes, hypertension, obesity, hyperlipidemia, and early-stage prostate cancer. Many forces are converging that make this the right idea at the right time. This chapter focuses on reviewing randomized-controlled trials and demonstration projects illustrating both the medical effectiveness and cost-effectiveness of lifestyle medicine as a new paradigm of healthcare—one that integrates the best of conventional treatments such as drugs and surgery with the power of comprehensive lifestyle changes.

Changing one’s lifestyle is difficult and adherence to medication in people at high cardiovascular risk and established cardiovascular disease is low. Lifestyle is usually based on longstanding patterns and is highly determined by social environment and socioeconomic status. Additional factors such as chronic stress, cognitive impairment, and negative emotions (e.g. depression, anxiety) further impede the ability to adopt a healthy lifestyle, as does complex or confusing advice by medical caregivers. In clinical practice, increased awareness of these factors will facilitate empathetic counselling and the provision of simple and explicit advice. Established cognitive-behavioural strategies are important tools to help with behaviour change and medication adherence. Specialized healthcare professionals (e.g. nurses, dieticians, psychologists) should be involved whenever necessary and feasible. Reducing dosage demands to the lowest applicable level is the single most effective means for enhancing adherence to medication.

Erectile dysfunction (ED) can be improved by changing certain lifestyle factors such as sedentary lifestyle, unhealthy food, nicotine and alcohol abuse, or optimal management of risk factors/concomitant diseases causing or aggravating ED such as dyslipidaemia, hypertension, diabetes mellitus, depression, BPH/LUTS, or hypogonadism.First choice in the medical therapy of ED are PDE-5 inhibitors such as sildenafil, vardenafil, and tadalafil used p.r.n, or on a daily low-dose regimen regarding tadalafil, especially in patients suffering from ED and BPH/LUTS. Yohimbine and L-arginine may be considered in patients with mild PE, which also applies for topical alprostadil. Both transurethral alprostadil and self-injection therapy with alprostadil, papaverine/phentolamine, or the trimix combination consisting of all three compounds is mostly reserved for those patients non-or poorly responding to PDE-5 inhibitors. Finally, combination therapy with PDE-5 inhibitors and transurethral alprostadil or intracavernous self-injection therapy can be able to rescue non-responders to either monotherapy.

Abstract:
This epilogue examines how the distinctiveness of Italian food has been shaped by continuous transformations and adaptations to a changing Italian America and American culture since World War II. From domestic kitchens to luxurious restaurants, Italian immigrants framed a food culture that created a nation and shaped their self-representation as a group. However, Italian American food culture underwent various changes. The meanings of Italian American food were reworked in the neoliberal landscape of deindustrialization, globalization, and a postmodern culture in which “the self” was created through consumption and where cultural difference became just another commodity. A new group of middle-class Italian immigrants to New York City started to reshape Italian food in America by detaching it from its immigrant origins and relocating it within the “authentic” traditions of Italian regional cuisine. Despite all these changes, and even as the ground for Italian American identity has shifted, Italian American food continues to convey a lifestyle, a taste, and a history.

Taking the previous chapter’s argument one step further, this chapter suggests that because of this bodily awareness and of various shifts in lifestyles due to the break that imprisonment constitutes in their life course, women’s bodies change significantly as the result of imprisonment. The present chapter outlines various examples of physiological change on women’s bodies in order to explore the complexity of the effects of imprisonment on women’s bodies and identities. It argues that while physiological change is not always experienced as necessarily negative, its impact on women’s overall attitudes, body image, and subjectivities is one of ambivalence and emotional turmoil, marking their overall perceptions and identities long after release from prison.
