Card tricks based on mathematical principles can be a great way to get students interested in exploring some important mathematical ideas. Bonomo (2008) describes several variations of a card trick that rely on nested floor functions, but these generally go beyond the reach of beginning algebra students. However, a simple spreadsheet implementation shows students why the card trick works and allows them to explore several variations. As an added bonus, students are introduced to composite functions, the floor function, and iteration, and they learn how to use formulas and the INT function in Microsoft Excel. The depth of the mathematical explanation can be varied according to students' background.

At the very least, we should be honest and explicit about the need for strong priors about the welfare effects of changes in averages of observables to warrant scaling. What we need is a healthy dose of theory and the implied econometrics.

It is devoted to Carl Tausig (1841–71) — one of the most prominent 19th century piano virtuosos, Liszts favorite apprentice, whose historical significance is still neglected in modern publications on piano playing. The introducing article contains biographical information on both — Carl Tausig and Wilhelm von Lenz (1809–83), Russian music critic, author of the notorious Beethoven studies.

Objective: We aimed to evaluate the impact of an individually tailored preventive oral health intervention on the use of oral health care services by older family caregivers (FCs) and their care recipients (CRs). Material and methods: A randomized controlled six and 12-month oral health intervention study included FCs and CRs aged ≥65 years living in Eastern Finland. The participants were randomly assigned to an intervention (FCs n = 53, CRs n = 47) and a control (FCs n = 39, CRs n = 35) group. Individually tailored oral health interventions for the FCs provided by a dental hygienist focused on oral hygiene and self-care. Generalized estimating equations were used to analyze the impact of intervention on the change in the use of oral health care services. Results: The intervention had no significant effect on the use of oral health care services by the FCs or their CRs. Traditional factors such as female gender, a higher number of teeth, toothache, no dental fear, and higher morbidity were significantly (p < 0.05) associated with an increased use of oral health care services in the FCs, but not among the CRs. Conclusions: Individually tailored preventive oral health intervention showed no effect on the use of oral health care services. To promote oral health among the elderly, specific interventions focusing on use of oral health care services are needed. Trial registration: clinicaltrials.gov/study/NCT04003493

Dans sa décision de principe sur les droits de chasse des Métis rendue à la suite de l’affaire R. c. Powley, la Cour suprême du Canada recourt au concept de mainmise effective pour marquer les limites de la période à considérer afin de déterminer si des pratiques particulières (p. ex., la chasse) font partie intégrante du mode de vie des Métis. Si ces pratiques n’ont pas été explicitement abolies par l’État avant 1982, alors elles font partie aujourd’hui des droits ancestraux des Métis et à ce titre, elles sont protégées par la Constitution. Déterminer la date de la mainmise effective de l’État est un problème empirique qui exige des recherches historiques approfondies. Devant les revendications de la Couronne quant à sa souveraineté et à l’extinction de droits autochtones, diverses activités peuvent servir de critères quant à l’établissement de la mainmise effective sur les régions pionnières (p. ex., les recensements). Des documents historiques montrent que des activités de topométrie et de cartographie ont matérialisé les efforts du gouvernement fédéral pour acquérir des connaissances géographiques. En ce qui concerne la nécessité constante de clarifier les droits ancestraux des Métis, la valeur des sources cartographiques historiques comme moyen de reconstruire la lente évolution des connaissances géographiques de l’État est illustrée par l’exemple de la région d’Ile-à-la-Crosse. La cartographie historique, par conséquent, peut contribuer à clarifier les droits des Métis.

Abstract:
Respiratory infections are one of the main causes of consultation and hospitalization in primary health care, including pneumonia. We present the case of a 3-year-old preschooler who came to the health services for presenting frequent wet cough and fever of up to 39 degrees Celsius of 8 days of evolution, who was previously treated with oral Amoxacillin in his health area without resolution of the symptoms. On physical examination, the patient was found to be in good general condition, where only the presence of crackling rales in the right lung base was positive; antimicrobial therapy was started, showing clinical improvement and cessation of the febrile symptoms. On the fifth day of evolution the febrile symptoms reappeared with peaks of up to 39 degrees Celsius. Physical and radiological examination showed worsening of the symptoms. Results of nasopharyngeal exudate were received, which showed Klebsiella pneumophila. An evolutionary chest X-ray was indicated, resulting in a denser and more homogeneous opacity with radiolucent areolar images towards its upper contour that kept occupied upper and middle segments of the right lung field, showing a phase of hepatization of the process with images suggestive of pneumatoceles. Antimicrobial spectrum was extended with Meropenen associated to Vancomycin according to antibiogram and possible sensitivity according to the most frequent etiology according to age, with satisfactory resolution of the picture. Atypical etiologies, including Klebsiella pneumophila, should be suspected in the case of slowly resolving pneumonia.

As tecnologias estão presentes no cotidiano das instituições educativas, devido principalmente ao seu avanço. Dentre eles, destacam-se os smartphones. Este dispositivo tem a capacidade de processar dados com a mesma potência de um computador. O seu custo acessível democratizou o acesso à informação e possibilitou a disseminação de produtos digitais, tais como o vídeo. Além disso, os smatphones, com seus diversos aplicativos e recursos, como o gravador de voz e imagem, possibilitam seu uso pedagógico, tornando os estudantes verdadeiros protagonistas na construção do seu conhecimento. Ciente dessa prerrogativa, neste artigo descrevemos como foi elaborado um manual instrucional orientando o aluno a construir vídeos na escola, indo além dos seus muros, de tal modo que este aluno, ao realizar uma produção audiovisual está elaborando uma narrativa videogeográfica, tendo a oportunidade de mostrar ao mundo, o mundo sob as lentes do seu olhar. Para a produção do manual, foi realizada uma revisão sistemática e integrativa sobre o Ensino de Geografia e Vídeo, caracterizando-se assim como uma pesquisa exploratória. Foi desenvolvida uma pesquisa documental nos currículos a nível nacional e estadual, bem como aplicada uma sequência didática de forma remota com estudantes da zona rural do ensino fundamental visando testar a questão norteadora da pesquisa e verificar a eficácia da primeira versão do produto do Mestrado Profissional em Geografia - GEOPROF-UFRN, buscando melhorá-lo. Após o desenvolvimento do trabalho investigativo, constatou-se que a produção de vídeos por alunos constitui-se como estratégia eficaz para a construção da aprendizagem da geografia escolar, demonstrando a potencialidade do Manual Instrucional: aprendendo a gravar vídeos nas aulas de Geografia.

O artigo apresenta a discussão vygotskiana sobre as ideias do Formalismo Russo expostas no capítulo “Arte como procedimento”, do livro Psicologia da arte (de 1925). Trata-se de uma resposta crítica contemporânea ao desenvolvimento da escola. Apesar de dialogar mais intimamente com as vertentes marxistas do estudo da arte em sua obra, Vygótski faz uma crítica muito refinada, que ressalta o potencial da Escola Formal e, ao mesmo tempo, aponta suas contradições e limitações, sem que as mesmas invalidem a profunda inovação, muito cara a Vygótski, oferecida por vertente dos estudos literários.

El confort visual de los présbitas ha mejorado considerablemente con la evolución en el diseño y la fabricación de los lentes progresivos. Para que la adaptación a estos sea inmediata, es necesaria la elección adecuada del lente progresivo y la toma de medidas exactas como lo permite hacer el I-Terminal. Objetivo:evaluar la adaptación de lentes progresivos de sexta generación en pacientes présbitas usuarios de lentes bifocales tradicionales por más de dos años, utilizando el I-terminal para la toma de medidas. Método: se realizó un estudio de tipo observacional descriptivo, con una muestra de 20 pacientes usuarios de lentes bifocales durante más de dos años. Para la toma de medidas de la adaptación del lente progresivo se utilizó el equipo I-Terminal de la casa Carl Zeiss Vision. La adaptación se evaluó usando la encuesta afa2’q y afa4q. Resultados: se encontró que el 100 % de los pacientes obtuvieron una adaptación inmediata con los lentes progresivos. Conclusión se observó una adaptación inmediata de los lentes progresivos y a la toma de medidas con el I-Terminal; además, se demostró, al evaluar la visión y el confort, que los pacientes usuarios de lentes bifocales por mucho tiempo son buenos candidatos para el uso de lentes progresivos.

El disqueratoma de Warty es considerada una lesión tumoral benigna , poco frecuente, generalmente solitaria aunque se han reportado casos de lesiones múltiples. Se describe como una pápula o nódulo de alrededor de 5 mm de diámetro pudiendo llegar hasta 1-2 cm , de crecimiento lento en personas de edad media (1). Las leishmaniasis son infecciones crónicas causadas por protozoarios intracelulares del género Leishmania, son transmitidas al ser humano mediante la picadura de flebótomos infectados. Las lesiones cutáneas aparecen principalmente en áreas expuestas como cara y extremidades. El diagnóstico es difícil por la clínica inespecífica y el crecimiento lento del parásito en cultivos. Se presenta el caso clínico de una niña de 8 años de edad, con lesión nodular en la mejilla izquierda, eritematosa, no dolorosa ni pruriginosa, diagnosticada en biopsia de piel. Se administró anfotericina B complejo lipídico por 21 días, obteniendo mejoría clínica.

Abstract:
Filipinos consider Holy Week as the holiest of days of Lent and Easter. During this time, the country is shrouded with centuries-old rituals and practices that persist in contemporary times. Using the framework of gender performativity, this study examines three forms of pamamanata (devotion): pagsasanto (taking care of a religious image), penitensya (penance), and salubong (Easter procession). The aim is to identify pamamanata practices that align with the feminine, masculine, gay, and those bordering between masculine and feminine tropes. The study maximized data from the author’s fieldwork and ethnographic materials written by academics. This study found that the three pamamanata traditions are gendered practices and that these are affected by the agencies of the family, community, and religion.

ABSTRACTThis article is about dementia disease in the context of transnational migration. Focusing on the example of Pakistani immigrants in Norway, the article explores response processes surrounding signs and symptoms of dementia. Particular attention is lent to understanding how Norwegian-Pakistani families ‘negotiate dementia’ in the space between their own imported, culturally defined system of cure and care, and the Norwegian health-care culture, which is characterised by an inclination towards public care and biomedical intervention. Based on field observations and in-depth interviews with Norwegian-Pakistani families and hospital professionals working with dementia, we show that the centrality of the traditional family in Norwegian-Pakistanis' identity claims has significant implications for how Norwegian-Pakistanis relate to the Norwegian health-care culture, and for how signs and symptoms of cognitive decline are read and responded to in a migratory context.

La segunda parte aborda el modelo de desarrollo chileno actual desde el lente de la teoría de las Variedades de Capitalismo. El modelo de desarrollo chileno ha significado crecimiento económico que sin embargo, no ha repercutido en el bienestar de la ciudadanía, este trabajo propone generar debates en torno a este tema que impliquen revisar otras formas de articular la industria y el mercado. Asimismo, ofrece reflexiones que permiten ampliar el debate que gira en torno al Capitalismo, proponiendo alternativas para organizar la economía, poniendo el foco de atención en el debate constitucional que está vigente en el Chile de hoy.

Então é preponderante passar pela Idade Média, descobrir a épica cavalheiresca e os contos de fadas, depois, pelas obras de Italo Calvino: três dos principais momentos que criaram um solo fértil para a fantasia nascer e crescer, pois se trata de uma longa e complexa viagem que dura séculos, abrange a literatura do bel paese, mas, ao mesmo tempo, a sua história e a sua cultura também; ou seja, todos os aspectos que, de uma forma ou de outra, afetam um gênero tão fantástico como a fantasia.

Ante la avalancha de información rápida y superficial que caracteriza al periodismo fast-food digital contemporáneo, ha surgido una corriente conocida como 'periodismo lento' que busca lo opuesto: perspectiva, contexto y profundidad en sus narrativas. Ante este panorama, las revistas se erigen como el formato más apropiado para abrazar esta filosofía por sus características y frecuencia de publicación y, en el caso español, Yorokobu destaca como una de las publicaciones pioneras en este enfoque. Este artículo, a partir de un estudio de caso mediante análisis de contenido, tiene como objetivo principal analizar el contenido publicado por esta emblemática cabecera como contraposición a las publicaciones periodísticas estándar. El resultado permite determinar la relevancia que esta cabecera otorga a la información reposada abordando temáticas alejadas de la actualidad inmediata y la réplica de notas de prensa, así como al uso de géneros periodísticos reflexivos.

Introduction: Mutations in RAS-pathway genes ( NRAS, KRAS, CBL) are amongst the most common somatic mutations in cancer and historically resistant to most therapies. Targeted therapies that impact the RAS-pathway constitute a major unmet need in oncology. The proliferative form of chronic myelomonocytic leukemia (CMML) is commonly associated with RAS-pathway mutations and has a high propensity to develop into acute myeloid leukemia. In pre-clinical models, Lenzilumab (LENZ; Humanigen, Inc., Short Hills, NJ), a proprietary Humaneered ® first-in-class monoclonal antibody with best-in-class off-rate and affinity that neutralizes GM-CSF, resulted in a reduction of colony numbers and viability of CMML cells, with the greatest sensitivity in cells possessing RAS-pathway ( NRAS/ KRAS/ CBL) mutations, suggesting benefit in the targeted treatment of CMML. The PREcision Approach to CHronic Myelomonocytic Leukemia (PREACH-M) trial assesses the efficacy of LENZ, in addition to azacytidine (AZA), in CMML subjects with RAS-pathway mutations and high dose sodium ascorbate (ASC) and AZA in CMML subjects without RAS-pathway mutations. Interim data from 11 subjects with RAS-pathway ( NRAS/ KRAS/ CBL) mutations in PREACH-M receiving LENZ/AZA demonstrate reductions in circulating GM-CSF and CRP with 8 subjects achieving complete response or optimal marrow response. This report describes improvements in variant allele frequencies (VAF) for RAS-pathway mutations and hematologic improvements associated with LENZ/AZA treatment in CMML. Methods: PREACH-M is a Phase 2/3 non-randomized, open-label trial in 72 subjects, aged at least 18 years, with newly diagnosed CMML based on the WHO 2016 criteria; and RAS-pathway mutations at a variant allele frequency ≥3%. Subjects received 24 cycles (every 28 days) of AZA (SC; 75 mg/m 2 for 7 days) and LENZ (IV; 552 mg; d1 & d15 of cycle 1 and d1 only for all subsequent cycles). Subjects without RAS-pathway mutations received the same AZA regimen and sodium ascorbate (IV; 30g for 7 days (15g for 1st dose only; 30g thereafter if no evidence of tumor lysis syndrome); PO; 1.1g on all other days). VAF was determined from bone marrow mononuclear cells using a 41-myeloid panel using Illumina Hi-Seq with a depth of 1000x performed on bone marrow aspirates obtained at baseline and Day 1 of treatment cycles 4, 7, and 12. Results: As of July 2023, 11 subjects completed at least 3 months of LENZ/AZA treatments and follow-up sequencing data were available for 10 subjects. Five of 10 subjects showed &gt;10% decrease in VAF for at least one mutation detected at baseline, including 5 decreased alleles in the RAS-pathway ( KRAS, CBL) out of 22 total VAF mutation responses. CBL mutations showed the largest change with a decrease in &gt;50% VAF in three subjects. Decreases in RAS-pathway clones by LENZ/AZA were durable at 6 months. No decreases in VAF for any mutations were observed in 2 evaluable subjects following ASC/AZA treatment. Following 3 months LENZ/AZA treatment, blood monocyte count improved from a baseline of 11.0 ± 6.0 x 10 9/L to 2.0 ± 1.0 x 10 9/L (p=0.030). Blast differential improved from 10.5 ± 5.0% to 4.0 ± 3.5% (p=0.038). Platelet count increased from 90.0 ± 50.0 x 10 9/L to 150.0 ± 60.0 x 10 9/L (p=0.010). Statistical improvements remained durable for at least 6 (n=6) and 12 (n=3) months. Hemoglobin concentration increased 100.0 ± 22.0 g/L to 115.0 ± 10.0 g/L at 3 months (p=NS, n=11) and progressively increased to 122.0 ± 10.0 g/L at 12 months (p=0.024, n=3). Spleen length decreased from 15.0 ± 4.0 cm to 13.0 ± 4.0 cm (p=0.03) and decreased further to 11.0 ± 2.0 cm in subjects who received 12 months of treatment (n=3). No evidence of disease progression or relapse was observed in any subject. Conclusion: In 11 evaluable subjects with proliferative CMML and RAS-pathway mutations, GM-CSF neutralization with LENZ in addition to AZA standard of care, resulted in significant decreases in the proportion of KRAS and CBL mutant leukemic cells, accompanied by clinically significant hematologic improvements and a reduction in splenomegaly. Lenzilumab may have efficacy in preventing outgrowth of RAS-pathway mutations, specifically KRAS and CBL, in the context of CMML and other myeloid malignancies.

La psicoterapia experiencial y el focusing fue desarrollada por Eugene Gendlin en Estados Unidos, mientras era parte del equipo de investigación de Carl Rogers. La psicoterapia experiencial es un desarrollo del enfoque centrado en la persona que surge de la investigación en psicoterapia, observando a personas cuyos procesos fueron exitosos. En estas investigaciones, se observó que las personas con mayores avances en sus terapias tenían cierta particularidad en cómo expresaban sus vivencias. Esto se manifestaba en que hablaban con un lenguaje tentativo, más lento y chequeaban con algo interno lo que iban expresando. Gendlin desarrolla una filosofía, una teoría experiencial y el focusing como herramienta aplicada a la psicoterapia. En la presente revisión teórica se realizará una descripción de los fundamentos básicos de la psicoterapia experiencial orientada al focusing, sus influencias, el focusing como método y técnica, y sus aplicaciones.

Este artigo, decorrente de uma pesquisa de mestrado, buscou identificar, na perspectiva de dez professores bacharéis com tempo de docência entre 7 e 15 anos, todos com graduação e pós-graduação em áreas técnicas e com vínculo efetivo a uma universidade pública mineira, como se dá a constituição da sua identidade profissional docente. Originados de entrevistas semiestruturadas, os dados obtidos foram analisados com o fim de identificar seus saberes e práticas docentes, com base na lente teórica de Maurice Tardif, e de responder aos objetivos propostos neste estudo. Segundo os sujeitos da pesquisa, os saberes provenientes dos cursos de formação inicial em bacharelado não compreendem toda a complexidade que envolve os saberes necessários à docência. Admitem ser necessário ter aptidão para construir novos saberes e ressignificar aqueles existentes em sua prática pedagógica, comprovando ser imprescindível que as Instituições de Ensino Superior promovam ações voltadas à qualificação e desenvolvimento profissional destes docentes.

Introduction: Chronic myelomonocytic leukemia (CMML) is a rare cancer orchestrated by granulocyte-macrophage colony-stimulating factor (GM-CSF), a pro-inflammatory cytokine that drives leukemic monocyte proliferation. Standard of care (SOC) for CMML treatment includes azacitidine (AZA), with a complete response (CR) rate of 16-21% 1,2. The PREcision Approach to CHronic Myelomonocytic Leukemia (PREACH-M; ACTRN12621000223831) trial investigates novel CMML therapies directed by molecular profiling. Lenzilumab (LENZ; Humanigen, Inc., Short Hills, NJ) a proprietary Humaneered® first-in-class monoclonal antibody with best-in-class off-rate and affinity that neutralizes GM-CSF. PREACH-M interim results show that LENZ/AZA improves hematologic parameters, decreases spleen size, and dampens pro-inflammatory responses in CMML with RAS-pathway mutations. This report details the objective clinical responses from an interim analysis of the first 11 subjects who completed at least three months LENZ/AZA treatment. Methods: PREACH-M is a Phase 2/3 non-randomized, uncontrolled, open-label trial in 72 adults aged at least 18 years, newly diagnosed with WHO 2016 criteria for CMML. Key exclusion criteria include prior treatment with investigational agents; radiotherapy within 28 days before treatment; treatment with G-CSF within 7 days of screening; GM-CSF within 28 days of screening; and uncontrolled medical conditions. Subjects exhibiting RAS-pathway mutations ( NRAS, KRAS, CBL) receive 24 cycles (every 28 days) of AZA (SC; 75 mg/m2 for 7 days) and LENZ (IV; 552 mg; d1 & d15 of cycle 1 and d1 only for all subsequent cycles); while those with only TET2 mutations receive the same AZA regimen and sodium ascorbate (IV; 30 g for 7 days [15 g for 1st dose only, 30 g thereafter if no evidence of tumor lysis syndrome]; PO; 1.1g on all other days). Subjects who complete 24 cycles of treatment are followed every 6 months for an additional 24 months. The primary endpoint is the frequency of complete response (CR) or partial response (PR) during the first 12 cycles according to Savona Criteria. Secondary endpoints include responses according to modified 2006 International Working Group criteria, 2 year overall survival, and symptom improvement. Results: As of July 2023, 15 subjects were enrolled in the LENZ/AZA arm (8 females, 7 males with mean age 69; mean white cell count 21x10 9/L, mean Hb 121 g/L; mean platelet count, 74x10 9/L, mean blast count, 10.1%). Mutations included; CBL (47% of subjects), NRAS (27%), KRAS (47%), NRAS and KRAS (13%), and TET2 (93%). Subjects exhibited CPSS-MOL scores, of intermediate risk 1 (n=1), intermediate risk 2-3 (n=8), and high risk 4-6 (n=6). All the 11 evaluable subjects at 3 months responded to LENZ/AZA. CR was achieved within 3 cycles in 55% of subjects. Six of the subjects demonstrated CR including 2 with a high risk CPSS-MOL profile and 8 achieved either CR or complete marrow response (blasts&lt;5%) within 12 months. One subject had a platelet response, 1 subject each had PR and stable disease with blasts &lt;5%. CMML progression was absent and 1 subject became eligible for allogeneic transplant. These findings exceed historical CR rates for hypomethylating agents (16%; 95%CI, 12-21% 1 and 21%;13-29% 2). Self-reported symptom scores from the standardized MPN-SAFFS improved from baseline (mean of 22 vs 12, P=0.06). Fifteen grade 3 and 9 grade 4 adverse events were reported of which 2 were “probably” ascribed to both LENZ/AZA and 7 were “possibly” ascribed to LENZ. No unexpected adverse events were observed. Conclusion: Interim analysis of the PREACH-M trial demonstrated that GM-CSF neutralization with LENZ/AZA, for the treatment of CMML with RAS-pathway mutations resulted in 55% CR, achieved early in treatment, durability up to 18 months, thus far, and no unexpected serious adverse events. Dental ceramics have an excellent ability to reproduce natural teeth appearance, but once the substrate has an altered coloration, it is necessary to mask this intercurrence, considering the characteristics of the ceramic and cement selected. This article aims to present a clinical case of rehabilitation with ceramic laminates to mask a darkened central incisor by planning a more opaque porcelain structure under a translucent contact lens. A female patient sought dental care complaining of dissatisfaction with smile aesthetics. It was clinically identified the presence of a Class IV darkened restoration in composite resin in element 11. In treatment planning harmony in the shape and color of elements 11 and 21 was aimed at through lithium disilicate lenses. An opaque intermediate structure was used to cover the darkening in tooth 11. The double molding technique was performed to obtain the working model. Prior to cementing, the surfaces were treated. In ceramics, conditioning with 10% hydrofluoric acid and 37% phosphoric acid, silane application and universal adhesive; in dental substrate, conditioning with 37% phosphoric acid and application of universal adhesive were performed. In laminates definitive cementation Allcem Venner cement was chosen. Patient reported satisfaction with treatment final aesthetics. Thus, cementation technique of a translucent laminate over an opaque facet is a good option for masking substrates with darkened coloration.

The pro-inflammatory cytokine granulocyte-macrophage colony-stimulating factor (GM-CSF) plays a central role in stimulating leukemic monocyte proliferation. Lenzilumab (LENZ) is a proprietary Humaneered® first-in-class monoclonal antibody with best-in-class specificity and affinity that neutralizes GM-CSF to prevent signaling through its receptor. The PREcision Approach to CHronic Myelomonocytic Leukemia (PREACH-M) trial assesses the efficacy of LENZ in CMML (ACTRN12621000223831p) to improve outcomes beyond those afforded by SOC. Methods: PREACH-M is a Phase 2/3 non-randomized, open-label precision medicine trial in 72 adults aged at least 18 years, newly diagnosed with WHO 2016 criteria for CMML; cytopenia (hemoglobin &lt; 100 g/L, platelets &lt; 100 × 109/L or absolute neutrophil count &lt; 1.8 × 109/L): white blood cell count ≥ 13 × 109/L; as well as TET2 and/or RAS pathway mutations (NRAS, KRAS, CBL). Key exclusion criteria include prior treatment with investigational agents; radiotherapy within 28 days before treatment; treatment with G-CSF within 7 days of screening; GM-CSF within 28 days of screening; and uncontrolled medical conditions. Subjects exhibiting RAS pathway mutations, with or without TET2 mutations, receive 24 cycles (28 days) of A (SC; 75 mg/m2 for 7 days) and LENZ (IV; 552 mg; d1 & d15 of cycle 1 and d1 only for all subsequent cycles); while those with non-RAS pathway mutations receive the same A regimen and sodium ascorbate (IV; 30 g for 7 days [15 g for 1st dose only, 30 g thereafter if no evidence of tumor lysis syndrome]; PO; 1.1g on all other days). Secondary endpoints include overall survival and progression-free survival at 2 years; proportion of subjects with clinical benefit at any point during the 24 cycles; impact on physical and functional capacity; social well-being according to Multidimensional Geriatric Assessment and quality of life; as well as hematological and non-hematologic safety. Results: As of December 31, 2022, eight subjects were treated with A and LENZ (5 females, mean age of 67 years; 3 males, mean age of 69 years); among them 6 were evaluable based on at least 3 months of follow-up. CR or objective responses were observed in all evaluable patients including 2 with high risk based on molecular profiling. 10 grade 3/4 Serious Adverse Events were observed of which 2 were assessed by the investigator as possibly related to LENZ. Conclusion: The ongoing PREACH-M trial evaluates GM-CSF neutralization with LENZ in addition to SOC, in the treatment of CMML with RAS pathway mutations. Abstract:
Carl Rogers desenvolveu uma teoria da personalidade que pode servir como lente para entendermos diversos fenômenos clínicos. Considerando pesquisas que correlacionam os problemas de autoimagem corporal e autoestima, este artigo objetiva realizar uma releitura da teoria rogeriana da personalidade sobre esses fenômenos. Nesse sentido, apresentam-se os aspectos que: (1) organizam a personalidade pelas relações entre experiência, consciência, campo fenomenológico, comportamento, self (real e ideal), tendência à autorrealização, autoimagem e autoestima; (2) desorganizam a personalidade e geram problemas de autoimagem e autoestima, a partir de reações a ameaça, avaliações condicionais, regulações em função do self ideal, incongruências e desajustamento psicológico; (3) reorganizam a personalidade em uma autoimagem e autoestima baseadas nas experiências organísmicas diretas, regulações em função de um self real atrelado à tendência à autorrealização, e expressas por um funcionamento pleno, abertura à experiência e processo de ser o que se é. Conclui-se, por fim, apontando para estudos empíricos sobre o tema.

Rezumat În cadrul articolului cu titlul „Beton monolit versus beton prefabricat la execuția în consolă a podurilor” vor fi abordate o serie de probleme care apar atât la proiectarea cât şi la execuţia structurilor de poduri din beton precomprimat utilizând metoda execuţiei în consolă. Această metodă a revoluţionat podurile din beton, din punct de vedere al deschiderilor şi al uşurinţei traversării unor obstacole dificile. În afara avantajelor realizării unor structuri din beton monolit prin execuţie în consolă, utilizarea elementelor prefabricate conduce la micşorarea semnificativă a timpului de execuţie şi la diminuarea problemelor care pot apărea din cauza curgerii lente şi a contracţiei betonului. Materialul prezentat în cadrul articolului cu titlul „Beton monolit versus beton prefabricat la execuția în consolă a podurilor” va prezenta principiul metodei de execuţie în consolă, avantajele utilizării acestei metode, cazurile particulare pentru care se pretează o astfel de structură, scheme statice şi secţiunile transversale adoptate pentru acest tip de structuri, cât şi modul de îmbinare al tronsoanelor prefabricate în funcţie de generaţia din care fac parte. De asemenea, se va prezenta un studiu de caz în care vor fi analizate cele doua variante de execuție în consolă (turnare monolită a tronsoanelor respectiv montare de tronsoane prefabricate) și vor fi prezentate concluzii privind oportunitatea utilizării uneia dintre cele două variante.

Para tanto, foi proposta a adoção da perspectiva analítica da multifuncionalidade agrícola para análise das atividades exercidas pelas mulheres rurais, uma vez que esta perspectiva almeja ir além das abordagens de orientação meramente produtivista e pretende evidenciar as funções não produtivas da agricultura. Na pesquisa empírica, foram identificadas as diversas e diferentes atividades que compõem os trabalhos produtivo, doméstico e de care de um grupo de 18 agricultoras integrantes do Movimento de Mulheres Camponesas (MMC) residentes no município de Quilombo, no oeste de Santa Catarina, na região Sul do Brasil. Verificou-se que as agricultoras têm papel fundamental no que concerne à manutenção e reprodução social da agricultura familiar, uma vez que as suas atividades cotidianas estão relacionadas integralmente às suas famílias e a seus estabelecimentos agrícolas. Nessa perspectiva, a lente da multifuncionalidade funciona como uma poderosa ferramenta que possibilita visibilizar o papel da mulher no campo.

The United States relies on international and osteopathic medical graduates (“non-USMDs”) to fill one third of residency positions because of a shortage of American MD graduates (“USMDs”). Non-USMDs are often informally excluded from top residency positions, while USMDs tend to fill the most prestigious residencies. Little is known, however, about whether the training in these different settings is comparable or how it impacts patients. Drawing on 23 months of ethnographic fieldwork and 123 interviews, I compare training at two internal medicine programs: a community hospital staffing 90% non-USMDs and a university hospital staffing 99% USMDs. The community program’s structure lent itself to a hands-off approach resulting in “inconsistent autonomy.” In contrast, the university hospital supervised its residents much more regularly, resulting in “supported autonomy.” I conclude that medicine may be stratified in unexpected ways between USMDs and non-USMDs and that stratification may matter for patients.

ABSTRACT Hearing loss (HL) is a common and complex condition that can occur at any age, be inherited or acquired, and is associated with a wide number of etiologies. HL is the most common sensory deficit in newborn children. In developed countries, genetic causes are considered the most frequent etiology of HL, and are estimated to account for 75% of the causes of HL. Current estimates suggest 1% of human genes (200–250 genes) are associated with genetic HL, and to date, more than 80 genes with over 1000 mutations and 140 loci have been identified associated with non-syndromic HL. The Online Mendelian Inheritance in Man reports more than 400 syndromes with HL. Syndromic and non-syndromic HL can be caused by different mutations within the same gene. Establishing the genetic cause of HL in prelingual children facilitates the medical course of action, rehabilitation choices and long term care in children. Patients with HL of undiagnosed etiology should be evaluated by a clinical geneticist and consider genetic testing as a part of their multidisciplinary evaluation. Introduction: Chronic myelomonocytic leukemia (CMML) is characterized by accumulation of classical CD14+CD16- inflammatory monocytes driven in part by hypersensitivity to granulocyte-macrophage colony-stimulating factor (GM-CSF), a pro-inflammatory cytokine. Standard of care in CMML includes hypomethylating agents such as azacytidine (AZA), with complete response (CR) rates of 16-21% 1,2 and no reliable biomarkers that predict response. The complete pro-inflammatory profile of CMML is unknown and no treatment addresses the hematologic aberrations of CMML. Lenzilumab (LENZ; Humanigen, Inc., Short Hills, NJ) is a proprietary Humaneered ® first-in-class monoclonal antibody with best-in-class off-rate and affinity that neutralizes GM-CSF. The interim analysis of the PREcision Approach to CHronic Myelomonocytic Leukemia (PREACH-M; ACTRN12621000223831) trial showed LENZ/AZA treatment in 11 subjects with RAS-pathway ( NRAS/ KRAS/ CBL) mutations resulted in 8 subjects with complete response or optimal marrow response and improvements in hematological parameters. This report describes the cytokine profiles and systemic C-reactive protein levels of these subjects. Methods: PREACH-M is a Phase 2/3 non-randomized, open-label trial in 72 subjects, aged at least 18 years, with newly diagnosed CMML based on the WHO 2016 criteria; and RAS-pathway mutations at a variant allele frequency ≥3%. Subjects received 24 cycles (every 28 days) of AZA (SC; 75 mg/m 2 for 7 days) and LENZ (IV; 552 mg; d1 & d15 of cycle 1 and d1 only for all subsequent cycles). Subjects without RAS-pathway mutations received the same AZA regimen and sodium ascorbate (IV; 30g for 7 days (15g for 1st dose only; 30g thereafter if no evidence of tumor lysis syndrome); PO; 1.1g on all other days). Cytokine profiling from bone marrow plasma was performed after 4,7,12 and 24 months using Milliplex Human Cytokine/Chemokine Magnetic bead panel and compared with 24 age-matched healthy subjects. Unsupervised hierarchical clustering with Ward's method sought distinct CMML patterns based on cytokine expression. C-reactive protein (CRP) was determined from blood samples using a routine assay. Variant allele frequencies were determined from bone marrow mononuclear cells using a 41-myeloid panel using Illumina Hi-Seq with a depth of 1000x. Results: As of July 2023, 15 subjects were enrolled in the LENZ/AZA arm (8 females, 7 males with mean age 69; mean white cell count, 21x10 9/L; mean Hb, 121 g/L, mean platelet count; 74x10 9/L, mean blast count, 10.1%). CRP decreased from a median of 5.6 at baseline to 2.1 mg/L after 6 months of LENZ/AZA (P=0.02). GM-CSF and other pro-inflammatory cytokines were increased in CMML bone marrow plasma compared with healthy subjects. Hierarchical clustering of all subjects revealed 2 patterns of CMML, distinct from healthy subjects. Cluster “INNATE-1” was comprised of medium increases in inflammatory cytokines associated with innate immune response and M1 macrophage activation (IFN-g, IL-1b, TNFa, IL-12p70, IL-12p40, IL-17, Fractalkine, MCP-3). Cluster “INNATE-2” was associated with extreme increases of these cytokines. The mean pro-inflammatory innate immune score (sum of z-scores) was 20-fold greater in INNATE-2 compared with INNATE-1 ( P=0.055). Somatic mutations SRSF2, WT1, PHF6 were enriched in INNATE-2. All patients (5/5) in INNATE-1 showed 100% response (CR or optimal marrow response resulting in &lt;5% blasts) to AZA/LENZ. Two subjects (2/6) in INNATE-2 demonstrated partial responses or haematological improvement thus far. Conclusion: CMML is a disorder of profound innate immune activation, driven by GM-CSF and other pro-inflammatory cytokines. Early treatment with LENZ/AZA, a precision immunotherapeutic approach, leads to a) efficacy in INNATE-1 that exceeds historical CR rates for hypomethylating agents 1,2; and b) evolving efficacy in INNATE-2, in which pro-inflammatory activity is more robust. 1. Os Serviços Residenciais Terapêuticos (SRT) são uma importante estratégia de inclusão de pessoas com transtornos mentais na cidade. Nas últimas décadas, no Brasil, tem sido estimulada a criação desses serviços como forma de concretizar os ideais do movimento da Reforma Psiquiátrica, cuja contestação ao modelo asilar representado pelo hospital psiquiátrico implicou no surgimento dos serviços substitutivos. Este estudo discutiu a implementação dos SRT em Belo Horizonte e problematizou as interações sociais entre os vizinhos e os moradores dessas casas e suas percepções sobre os transtornos mentais, a partir de 60 entrevistas. Concluiu-se que esse serviço tem extrema relevância e que os atores envolvidos devem promover a integração dos moradores com a vizinhança. Além disso, o ritmo de instalação dos SRT é lento e insuficiente, se considerada a necessidade e sua potência terapêutica.

Nonviolent action is activity undertaken to call for, struggle for, or achieve change without using violence. This paper examines St. John XXIII’s historic encyclical on world peace, Pacem in terris, and its relationship to nonviolent action. It focuses on two nonviolent actions that contributed to this historic magisterial teaching: John’s efforts to foster a resolution to the Cuban Missile Crisis and a fast undertaken by the spiritual activist Lanza del Vasto during Lent 1963. It argues that the very writing of this papal letter was a form of nonviolent action and that it has inspired nonviolent action around the world since it was promulgated, including Catholic leadership in the People Power Revolution in the Philippines.

Pneumonia reprezintă o inflamaţie a parenchimului pulmonar care, în ciuda multiplelor clase de antibiotice disponibile şi în ciuda progreselor medicinei, deţine încă o rată crescută de mortalitate, mai ales în cazul copiilor. Deltfia acidovorans şi Stenotrophomonas maltofilia sunt bacili Gram-negativi care pot determina infecţii respiratorii. Prezentăm cazul unui pacient de sex masculin, în vârstă de 2 ani şi 3 luni, internat în clinica noastră pentru următoarele motive: tuse productivă, febră şi inapetenţă. Pe baza examenului obiectiv, a analizelor de laborator şi a investigaţiilor imagistice, am stabilit diagnosticul de pneumonie dreaptă asociată cu pleurezie şi am instituit antibioterapie cu spectru larg. Evoluţia a fost însă nefavorabilă, pacientul necesitând pleurostomie şi drenaj. Din lichidul pleural au fost izolaţi Delftia acidovorans şi Stenotrophomonas maltophilia. Am modificat schema de tratament conform antibiogramei, cu evoluţie ulterioară lent favorabilă. Particularitatea cazului constă în diagnosticarea unei pneumonii cu pleurezie la un copil în vârstă de 2 ani şi 3 luni, cu un episod de pneumonie în antecedente, fără factori de risc, care a necesitat pleurostomie cu drenaj toracic, la care cultura din lichidul pleural a evidenţiat doi germeni întâlniţi de obicei la persoanele imunocompromise, cu evoluţie favorabilă după iniţierea antibioterapiei conform antibiogramei.

I n Te Ao Mãori, the Toroa (albatross) is sacred. Roimata Toroa, albatross tears, is a widely used tukutuku pattern. Derived from the Te Tairawhiti Ngati Porou story of Pourangahua, the pattern speaks of themisadventures of travelers who take shortcuts in haste to get to port. Pourangahua was an agriculturist who traveling a return journey to Aotearoa to grow kumara, gifted to him by Ruakapenga, a tohunga and learned scientist. Lent two pet albatrosses, Harongarangi and Tiungarangi, by Ruakapenga, Pourangahua is given strict instructions on which hazards to avoid, the care of the birds, and a karakia to give thanksgiving for their safe return. In his hurriedness to see his wife Kaniowai, Pourangahua takes a shortcut, runs into a taniwha (a denotation of hazards), and forgets the karakia and fails to care for the birds,leading to their grief and eventual demise. Realising he has dishonoured Ruakapenga, Pourangahua tries to cover his mistake, by belatedly doing the karakia, but it is too late. The damage was done...
