Cancer continues to be a leading cause of mortality worldwide and ranks second among the pediatric population. Radiation therapy (XRT) is a potent tool used by oncologists to manage this disease. Although treatment is typically painless, the need for immobilization has resulted in anesthesiologists frequently being consulted to provide service to these patients during their treatment. In addition to the challenges of providing anesthesia in a non–operating room environment, the provider must tailor an appropriate anesthetic for the simulation or XRT treatment sessions. Complications are rare but may be exacerbated by the distance between the patient and the anesthesia provider. Finally, the anesthesiologist must be prepared to discuss issues related to multiple anesthetic exposures with the patient and caregivers.

There are numerous leadership opportunities and a great need for more effective leadership in the nonprofit sector. While community leadership is one of the 18 community psychology competencies, it is rarely addressed by community psychologists or taught in community psychology graduate education programs. In this chapter, a framework and rationale for community leadership is provided along with ideas and encouragement for community psychologists to become more intentional in using a community leadership framework. Principles for effectively working with and within nonprofits are provided along with the qualities needed for effective leadership. Community psychology students are motivated to work in the nonprofit sector because they care deeply about the social justice and health issues addressed by these organizations, so we included ideas for how students could develop a leadership framework and pursue experience with nonprofits as part of their education. Working with the assumption that all community psychology activity should have empowerment as one of its outcomes, a community leadership framework ensures giving voice to nonprofit staff and those they serve.

This book analyses the complex power dynamics and interpersonal politics that lie at the heart of leadership in professional organizations, such as accounting, law, and consulting firms, investment banks, hospitals, and universities. It is based on scholarly research into many of the world’s leading professional organizations across a range of sectors, including interviews with over 500 senior professionals in sixteen countries. Drawing on the latest academic theory to analyse exactly how professionals in organizations come together to create ‘leadership’, it provides new insights into how leaders lead when there is no traditional hierarchy to support them, their own authority is contingent, and they must constantly renegotiate relationships with relatively autonomous professional peers. It explores how leaders persuade highly intelligent, educated, and opinionated professionals to work together; how change happens within professional organizations; and why leaders so often fail. Part I introduces the concept of plural leadership, analysing how leaders establish and maintain their positions within leadership constellations, and the implications for governance in the context of collective or distributed leadership. Part II examines the complex, challenging relationships between professionals as they seek to influence their organizations, including the phenomena of leadership dyads, insecure overachievers, social control, and the rise of the management professional. This chapter focuses on the rank-and-file fee-earning professionals. It examines the origins of insecurity among professionals and explains how their organizations amplify and exploit it. Elite professional organizations provide insecure overachieving individuals with the security of exceptional psychic as well as financial rewards through offers of employment. This chapter explains how and why professionals enjoy the comforts of being associated with an elite organization and how they are able to incorporate its elite status into their own identity. ‘Comforting’ social control mechanisms embodied in strong cultures can translate into cult-like conformity among senior professionals. This chapter, therefore, also explores the dark side of social control and its most typical manifestation—overwork. It asks: why do senior professionals ‘choose’ to exercise their autonomy by overworking to such an extent that they risk their personal relationships and physical and mental health? It concludes by examining the responsibilities of leaders in this context.

This chapter provides background for the 2011 Egyptian Revolution by highlighting the grievances of the upper and lower classes regarding economic factors, police brutality, and corruption. The purpose of the chapter is to explain why a large portion of the Egyptian population was unhappy with the Mubarak regime in the years leading up to the Revolution and where they attributed the blame for their plight. The chapter also demonstrates how social movement organizers frame grievances in a manner that leads to successful protest mobilization. This chapter, found in the back pain section of the book, provides a succinct synopsis of a key study examining the use of magnetic resonance imaging (MRI) for low back pain. This summary outlines the study methodology and design, major results, limitations and criticisms, related studies and additional information, and clinical implications. Researchers concluded that although spinal MRIs (compared with plain radiographs) are reassuring for patients with low back pain, they do not lead to improved functional outcomes; also, spinal MRIs detect anatomical abnormalities that would otherwise go undiscovered, possibly leading to spinal surgeries of uncertain value. In addition to outlining the most salient features of the study, a clinical vignette and imaging example are included in order to provide relevant clinical context.

Abstract:
Fetal alcohol spectrum disorders (FASD) represent a range of physical, mental, and behavioral disabilities caused by alcohol use during pregnancy, or prenatal alcohol exposure (PAE). FASDs are considered to be one of the leading preventable causes of developmental disability. Despite its high prevalence, FASD is often misdiagnosed or underdiagnosed, making interventions more challenging or delayed. Earlier diagnosis yields greater benefits for affected children, which include a reduction in secondary disabilities such as substance use disorders and learning and cognitive disabilities leading to school failure, and improved life outcomes. Most importantly, diagnosis provides a context for understanding a child’s behavior. When the environment surrounding a child with an FASD opts to focus on the child’s strengths as a means for intervention, there is a greater likelihood of that child achieving success as an adult. Diagnosis of FASD is further beneficial to the extent that it leads to a reduction of future births of children with FASD. This publication was initially developed for use in Spanish-speaking countries of the Americas and is intended to serve as a training workbook for providers of various disciplines to learn about the fundamentals of diagnosing FASD and to apply them to several case scenarios. It also discusses ethical implications of diagnosing FASD to the mother and child. Target audiences include physicians, psychologists, allied health professionals, social workers, and other providers that may encounter individuals affected by FASD. It is ideally used as a supplement for in-person training by experts in the fields of dysmorphology, epidemiology, and neuropsychology.

The Preamble of the Principles is based on the premise that the Principles will provide guidance to States and other entities in developing effective measures for combating impunity. A leading normative component of the Preamble is the trilogy of the core rights to know, to justice and to reparation, including guarantees of non-recurrence. This chapter first describes the functions and characteristics of a Preamble and provides an overview of the report of Special Rapporteur Louis Joinet before discussing the Updated Set of Principles and the evolving notion of ‘transitional justice’; the campaign against impunity; and the orientation and scope of the Principles, including the Preamble, in the broader settings of restoring peace and democracy. Issues concerning broad participatory involvement of victims, human rights defenders and other sectors of civil society are also considered.
