Abstract:
Engaging people experiencing a first episode of psychosis (FEP) in treatment is a priority. The duration of untreated psychosis (DUP) is the period of time between the onset of the FEP and the initiation of adequate treatment; longer DUP is associated with negative outcomes. Coordinated specialty care (CSC) is an evidence-based model that uses a team-based, multidisciplinary approach to engage people early in their psychotic illness in order to improve long-term psychosocial functioning and treatment outcomes. Best practices within CSC include assertive outreach, prescribing of low-dose antipsychotics, shared decision-making, family support, individual psychotherapy, and supported education and employment. Although people who have experienced an FEP may struggle to follow all treatment recommendations, they can still be engaged in their recovery by putting the focus on the individuals’ life goals.

The theory of inequality measurement can be founded on a few very simple principles concerning the comparison of income distributions. This chapter discusses the standard principles and the types of inequality indices that follow from them. It shows how these principles and indices can be related to conventional approaches to social-welfare analysis. Adjusting a few pieces within this same framework enables one to derive alternative, novel types of inequality indices and lays the basis for commonly-used types of poverty indices. The chapter also covers other general approaches to distributional comparisons including first-order and second-order dominance and their interpretation in terms of inequality and poverty.

The Shenzi Fragments is the first complete translation in any Western language of the extant work of Shen Dao (350–275 B.C.E.). Though his writings have been recounted and interpreted in many texts, particularly in the work of Xunzi and Han Fei, very few Western scholars have encountered the political philosopher’s original, influential formulations. This volume contains both a translation and an analysis of the Shenzi Fragments. It explains their distillation of the potent political theories circulating in China during the Warring States period, along with their seminal relationship to the Taoist and Legalist traditions and the philosophies of the Lüshi Chunqiu and the Huainanzi. These fragments outline a rudimentary theory of political order modeled on the natural world that recognizes the role of human self-interest in maintaining stable rule. Casting the natural world as an independent, amoral system, Shen Dao situates the source of moral judgment firmly within the human sphere, prompting political philosophy to develop in realistic directions. Harris’s sophisticated translation is paired with commentary that clarifies difficult passages and obscure references. For sections open to multiple interpretations, he offers resources for further research and encourages readers to follow their own path to meaning, much as Shen Dao intended. The Shenzi Fragments offers English-language readers a chance to grasp the full significance of Shen Dao’s work among the pantheon of Chinese intellectuals.

Obsessive-compulsive disorder (OCD) is often a chronic condition. Convergent evidence suggests that early-onset and adult-onset disease are importantly distinct: early-onset OCD is more highly genetic, has a male bias, and is more often associated with tic disorders and attention deficit disorder. Adult-onset OCD has an equal male–female ratio and is more often associated with anxiety and depression. Long-term follow-up studies from before institution of effective treatments suggest that a minority of individuals with adult-onset OCD remit, and many have persistent severe symptoms. There are few analogous studies of patients with childhood-onset OCD. Prognosis has improved over the past 30 years with the development of effective, evidence-based pharmacotherapy and psychotherapies. More recent long-term follow-up studies of both adult-onset and pediatric-onset OCD suggest remission rates of up to 50%. Refractory illness nevertheless remains an important clinical problem.

Neurocognitive deficits are acknowledged as integral features of bipolar disorder (BD) and are known to contribute to the compromised level of functioning in individuals with BD. This chapter provides an overview of the current state of cognitive enhancement in BD. Few pharmacological agents have been investigated with regard to their potential for pro-cognitive effects in BD. Dopaminergic agents (pramipexole) and stimulants (modafinil, armodafinil, and amphetamine) as adjunctive treatment in BD appear to be promising cognitive enhancers, and there are few ongoing randomized clinical trials targeting both cognitive dysfunctions and clinical symptomatology in BD. Glutamatergic agents (d-cycloserine) may hold promise as potential cognitive enhancing agents in BD; however, as for dopaminergic agents and stimulants, no conclusive data exist. Larger samples and longer follow-up are needed to obtain a deep understanding of the efficacy and safety of these compounds and their role in the neurobiological mechanisms underpinning cognition in BD.
