Resumo: Este estudo foi desenvolvido para avaliar, prospectivamente, os resultados do tratamento de varizes com úlcera em atividade com laser endovenoso (EVL), comparando com um grupo sob tratamento clínico, durante um ano. Foram estudados 52 pacientes consecutivos portadores de varizes com úlcera em atividade há mais de um ano, divididos em dois grupos aleatórios. O Grupo I, tratamento clínico, formado por 25 indivíduos; o Grupo II, tratamento com EVL, constituído por 27 indivíduos. Todos os pacientes foram estudados com ultra-som no início e término do estudo. Os pacientes do Grupo II foram seguidos com ultra-som com 7 dias, 30 dias e a cada 3 meses. As áreas das feridas foram avaliadas a cada 3 meses. O laser utilizado para o tratamento endovenoso das varizes tronculares foi um laser de diodo, com comprimento de onda de 980 nanômetros, com potência nominal de 15W com fibra óptica condutora de laser de 600 microns, introduzida endovenosamente por punção percutânea dirigida por ultra-som e com emprego de anestesia local por infiltração intumescente associado a sedação leve por via oral com 15mg de Midazolam®. As medidas de temperaturas intra e perivenosa foram realizadas com um termômetro digital acoplado ao computador. Em 12 meses, 81,5% das feridas dos pacientes do Grupo II estavam cicatrizadas enquanto no Grupo I apenas 24% estavam cicatrizadas. A recorrência de úlcera foi de 44,4% no Grupo I, sem nenhuma recorrência no Grupo II. A área média das feridas no Grupo I reduziu de 18,04cm² para 13,16cm² ao final de um ano, enquanto no Grupo II reduziu de 22,7cm² para 3,64cm² (p<0,05). A temperatura média registrada foi de 79,3ºC no intravenoso e de 43,0ºC nos tecidos perivenosos. Não houve efeito adverso importante. O tratamento de varizes com laser endovenoso em pacientes com úlcera venosa em atividade mostrou-se seguro, com taxa de cicatrização das feridas superior à dos pacientes com tratamento clínico no prazo de um ano, e não houve recorrência
Abstract: Conventional saphenous vein stripping is difficult to be indicated for the treatment of varicose veins in patients classified as CEAP C4, C5 or C6. This study was developed to consecutively evaluate treatment results for varicose veins with active ulcers using endovenous laser (EVL), compared to a groupundergoing clinical treatment, during a year. Fifty-two consecutive patients presenting with varicose veins with active ulcers for more than a year were divided for treatment into two randomized groups: Group I, clinical treatment, composed of 25 subjects, were submitted to elastic or inelastic compression therapy according to individual medical recommendation; Group II, EVL treatment, composed of 27 subjects, were submitted to great and or small saphenous vein ablation with a 980 nm diode endovenous laser, plus the clinical treatment. Intravenous and perivenous temperatures were measured continuously with a digital thermometer connected to a computer during the EVL treatment. All patients were followed for 12 months and studied with ultrasound at the beginning and end of the study. The ulcers¿areas were evaluated initially and at every 3 months. In 12 months, 81.5% of the wounds in patients in Group II and only 24% in patients in Group I had healed. Ulcer recurrence rate was 44.4% in Group I. The average wound area in Group I decreased from 18.04cm² to 13.16cm² at the end of the year. In Group II, the wound area decreased from 22.7cm² to 3,64cm² (p<0,05). Mean intravenous and perivenous temperatures of 79.3ºC and 43.0ºC were recorded. In conclusion, the treatment for varicose veins with endovenous laser (EVL) as described is safe in patients with active ulcers. Wounds healed faster than in patients undergoing clinical treatment alone during a one-year period. There was no ulcer recurrence in patients treated with EVL

Resumo: O diagnóstico de varizes é comum, sendo encontrado em até 25% das mulheres e 15% dos homens. Tais pacientes procuram tratamento médico por várias razões, inclusive preocupação estética. O desenvolvimento de instrumentais mais apropriados e flexíveis, bem como a busca por técnicas menos invasivas com resultados aceitáveis, a curto e longo prazos, possibilitou o aparecimento de novas modalidades terapêuticas. Com o objetivo de comparar a fotocoagulação endovenosa com a fleboextração total da veia safena interna, foi realizado um estudo prospectivo, analítico, do tipo ensaio clínico controlado e cego. Um total de 20 pacientes com insuficiência de veia safena interna bilateral foi operado de varizes no Hospital Estadual Sumaré durante o período de março de 2002 a fevereiro de 2004. Para cada caso, eram realizadas, aleatoriamente, as duas técnicas, sendo uma em cada membro inferior. Todas as variáveis pesquisadas foram submetidas à análise estatística. Como resultados mais expressivos, a técnica que utilizou o laser endovenoso apresentou menor dor, menor edema e menor hematoma durante o pós-operatório. Também no que se refere ao beneficio da cirurgia, a grande maioria dos pacientes respondeu que o membro operado com o laser foi o mais beneficiado dos dois. O exame de pletismografia a ar demonstrou melhora hemodinâmica nos dois grupos, sem, contudo obter a normalização dos valores do tempo de enchimento venoso em todos os casos. Conclui-se que a fotocoagulação endovenosa para o tratamento da veia safena interna em pacientes com varizes de membros inferiores é segura e bem tolerada, e apresenta resultados comparáveis aos da fleboextração convencional
Abstract: The diagnosis of varicose veins is common, being found in up to 25% of women and 15% of the men. Such patients seek medical treatment for some reasons, also aesthetic concern. The development of more appropriate and flexible instruments, as well as thesearch for less invasive techniques with acceptable results, the short and long term period, made possible the appearance of new therapeutically modalities. With the objective to compare the endovenous photocoagulation with the total stripping of the internal safenous vein, a study of the type was carried through controlled, randomized and blind clinical assay. A total of20 patients with insufficiency of bilateral internal safenous vein were operated of varices during the period of March of 2002 and February of 2004. For each in case that, two techniques, being one in each inferior member were carried through. The entire searched variable had been submitted to the analysis statistics. As major results, the technique that used the endovenous laser presented minor pain, minor edema and minor hematome during the postoperative course. Also as for the benefits of the surgery, the great majority of the patients answered that the member operated with the laser was the most benefited of the two. The air pletismography examination demonstrated hemodynamic improvement in the two groups, without however getting the normalization of the values of the venous filling time in all of them. The endovenous photocoagulation for the treatment of varicose veins is insurance and tolerated good. This technique presents resulted comparable to the ones of the conventional stripping of the internal safenous vein and can successfully be used with low index of complications

Full textResumo: Introdução: A doença venosa em sua apresentação mais grave, úlcera venosa em atividade ou cicatrizada, afeta de 2 – 6 milhões de pessoas, se considerarmos a população acima de 70 anos. A terapia de compressão (EC) é considerada o tratamento clínico mais eficaz para úlcera venosa(UV), mas a recorrência deve ser prevenida com o tratamento da hipertensão venosa. Evidências sugerem que a recorrência da úlcera é diminuída com a cirurgia venosa superficial, com superioridade de intervenções endovenosas minimamente invasivas, como laser (EVLA) ou ablação por radiofrequência (RF), sobre o tratamento cirúrgico clássico. A escleroterapia com espuma (EE) tem demonstrado também eficácia na correção da hipertensão venosa, sendo seu uso cada vez mais difundido. Entretanto, ainda faltam evidências comparando as técnicas entre si com a terapia compressiva clássica. Objetivo: Comparar as técnicas endovenosas, como radiofrequência (RF) e endolaser (ELVA), com escleroterapia por espuma (EE) e o tratamento clínico por meio de elastocompressão em pacientes com insuficiência venosa crônica (IVC) avançada (CEAP 6). Casuística e Métodos: Estudo prospectivo, controlado e randomizado. Foram incluídos 126 (cento e vinte e seis) pacientes, num total de 139 (cento e trinta e nove) pernas com úlceras venosas ativas, randomizados de maneira independente em 4 grupos: Radiofrequência ou Laser ou Espuma ou Elastocompressão. Abstract: ABSTRACT: Introduction: Venous disease in its more serious presentation, healed or open venous ulcers, affects 2 – 6 million people worldwide. Compression therapy (EC) is considered the most effective clinical treatment for venous ulcers (VU) ; however, recurrence is prevented by treating underlying venous hypertension. Current evidence suggests that ulcer recurrence is reduced by superficial venous surgery. Minimally invasive endovenous treatment modalities, such as laser (EVLA) or radiofrequency ablation (RF) have shown superiority over the classic open surgical treatment. Foam sclerotherapy (EE) has also been shown to be effective in treating venous hypertension, Nonetheless, there isn’t enough evidence comparing all these techniques against classic compression therapy. Objective: To compare catheter-based endovenous techniques, such as radiofrequency (RF) and endolaser (ELVA) with foam sclerotherapy (EE) and clinical treatment using elastic compression in patients with advanced chronic venous disease (IVC) (CEAP 6). Methods: Randomized, open label, controlled trialy. One hundred twenty-six (N=126) patients in a total of 139 (one hundred thirty-nine) legs with active venous ulcers, were randomized and enrolled in an independent manner into 4 groups: RF, EVLA, EE or EC..
