Abstract:
In this paper, a new method is proposed to expand the family of lifetime distributions. The suggested method is named as Khalil new generalized family (KNGF) of distributions. A special submodel, termed as Khalil new generalized Pareto (KNGP) distribution, is investigated from the family with one shape and two scale parameters. A number of mathematical properties of the submodel have been derived including moments, moment-generating function, quantile function, entropy measures, order statistics, mean residual life function, and maximum likelihood method for the estimation of parameters. The proposed distribution is very flexible in its nature covering several hazard rate shapes (symmetric and asymmetric). To examine the performance of the maximum likelihood estimates in terms of their bias and mean squared error using simulated samples, a simulation study is carried out. Furthermore, parametric estimation of the model is conferred using the method of maximum likelihood, and the practicality of the proposed family is illustrated with the help of real datasets. Finally, we hope that the new suggested flexible KNGF may produce useful models for fitting monotonic and nonmonotonic data related to survival analysis and reliability analysis.

Abstract:
Background A national clinical registry was established in the Netherlands containing data directly sampled from electronic health record systems of physical therapists (PTs). This registry aims to evaluate the potential of patient reported outcome measures (PROMs) to develop quality indicators (QIs) in physical therapy care. Purpose To test to what extent the collected PROM data are reliable, valid and discriminatory between practices in measuring outcomes of patients with non-specific low back pain (NSLBP). Methods In this retrospective cohort study 865 PT practices with 6,560 PTs voluntarily collected PROM data of patients with NSLBP, using the Quebec Back Pain Disability Scale (QBPDS), the Numeric Pain Rating Scale (NPRS) and the Patient Specific Functioning Scale (PSFS). Reliability was determined by analysing the completeness of the dataset, the comparability by using national reference data, and through checking selection bias in the included patients. Validity was tested using the known-groups contrast between patients with (sub)acute vs. chronic NSLBP. To determine discriminative ability of outcomes between PT practices, case-mix corrected hierarchical multilevel analyses were performed. Results Reliability was sufficient by confirming fifteen of the sixteen hypotheses: 59% of all patients opted in for data analysis, 42% of these included patients showed repeated measurement, comparing with reference data and potential selection bias showed < 5% between group differences, while differences between (sub)acute and chronic NSLB-groups were significantly larger than 5% (less treatment sessions, lager differences in outcomes in (sub)acute NSLB patients). In addition, all nine adjusted hierarchical multilevel models confirm that the collected dataset on outcomes in PT care is able to discriminate between practices using PROM results of patients with NSLBP (ICC-scores range 0.11–0.21). Limitations Although we have shown the reliability, validity and discriminative ability of the dataset in the quest to develop QIs, we are aware that reducing missing values in patient records and the selective participation of PTs that belong to the innovators needs attention in the next stages of implementation to avoid bias in the results. Conclusion PROMs of patients with NSLBP collected in the national clinical registry of KNGF are reliable, valid and able to discriminate between primary care PT practices.

In biological particles such as Fusarium species, ice nucleation activity (INA) has been observed. Fusarium strains isolated from apple declined trees in Korea were identified with a multilocus sequence analysis using the tef1 and rpb1 genes. Droplet-freezing and tube-freezing assays were used to determine the INA of the strains, using Pseudomonas syringae pv. syringae KACC 21200 as a positive control and resulting in seven INA+ fungal strains that were identified as F. tricinctum (KNUF- 21-F17, KNUF-21-F18, KNUF-21-F29, KNUF-21-F32, KNUF-21-F38, KNUF-21-F43, and KNUF-21-F44). The effect of Fusarium INA+ KNUF-21-F29 was compared to that of INA– strains on Chrysanthemum morifolium cv. Shinma explants. A higher callus formation and noshoot formation were observed, suggesting that fungal INA could play a role in cold injuries and be a factor to consider in rapid apple decline. To the best of our knowledge, this is the first report of INA fungal strains isolated in Korea.

Abstract:
The present study aims to design, develop and characterize kNGR (Asn-Gly-Arg) peptide-conjugated lipid–polymer-based nanoparticles for the target-specific delivery of anticancer bioactive(s), i.e., Paclitaxel (PTX). The kNGR-PEG-DSPE conjugate was synthesized and characterized by using spectral analysis. The dual-targeted PLGA–lecithin–PEG core-shell nanoparticles (PLNs-kNGR-NPs) were synthesized using a modified nanoprecipitation process, and their physiological properties were determined. The results support that, compared to other NPs, PLNs-kNGR-NPs are highly cytotoxic, owing to higher apoptosis and intracellular uptake. The significance of rational nanoparticle design for synergistic treatment is shown by the higher tumor volume inhibition percentage rate (59.7%), compared to other designed formulations in Balb/c mice in the HT-1080 tumor-induced model. The overall results indicate that the PLNs-kNGR-NPs-based hybrid lipid–polymer nanoparticles present the highest therapeutic efficacy against solid tumor overexpressing the CD13 receptors.

Karsinoma nasofaring (KNF) adalah tumor ganas THTKL yang paling banyak dijumpai di Indonesia. Gejala dan tanda karsinoma nasofaring yang sering berupa benjolan di leher, obstruksi hidung, epistaksis dan diplopia.Penelitian ini bertujuan untuk mengetahui profil klinis karsinoma nasofaring di RSUP Dr. Mohammad Hoesin Palembang tahun 2014-2015.Penelitian ini merupakan penelitian deskriptif berupa serial kasus dengan menggunakan data sekunder dari rekam medis pasien di RSUP Dr. Mohammad Hoesin Palembang. Sampel penelitian ini adalah semua rekam medik pasien karsinoma nasofaring di RSUP Dr. Mohammad Hoesin Palembang tahun 2014-2015. Pada tahun 2014 terdapat sebanyak 60 kasus (16,85%) dan pada tahun 2015 terdapat 50 kasus (14,53%) dari total pasien KNF yang datang ke RSMH. Pasien KNF paling banyak (30,91%) terdapat pada kelompok usia 46-55 tahun. Sebanyak 72,73% pasien KNF adalah laki-laki. Gejala yang paling sering ditemui adalah hidung tersumbat (49,09%), benjolan di leher (43,64%), dan mimisan (36,36%). Sebanyak 81,82% kasus KNF adalah KNF WHO III, selebihnya adalah KNF WHO II (15,45%) KNF WHO I (2,73%). Mayoritas kasus KNF adalah KNF stadium IV (77,27%). Pasien KNF paling banyak terdapat pada kelompok usia 46-55 tahun. KNF lebih sering terjadi pada laki-laki daripada perempuan. Gejala klinik yang paling sering ditemui adalah hidung tersumbat, benjolan di leher dan mimisan. Mayoritas gambaran histopatologi KNF adalah KNF WHO III. Sebagian besar penderita KNF didiagnosis sebagai KNF stadium IV.

Abstract High-molecular-weight kininogen (HK) is a major component of plasma kallikrein-kinin system (KKS), the activation of which mediates HK cleavage to release bradykinin (BK). BK is a nonapeptide with potent proinflammatory and vasoactive activities. Although patients with rheumatoid arthritis (RA) exhibits the activation of the KKS and the cleavage of HK in plasma, the role of HK in arthritis pathogenesis has never been characterized. In this study we evaluated the phenotype of our newly-generated HK-deficient (Kng1−/−) mice in arthritis. To induce autoantibody-induced arthritis, two groups of Kng1+/+ and Kng1−/− mice were injected intraperitoneally with anti-type II collagen antibody cocktail (4 mg) on day 0 and lipopolysaccharide (LPS, 50 μg) on day 3. Although wild-type (WT) mice developed severe arthritis, the Kng1−/− mice exhibited significantly attenuated disease severity as evaluated by joint diameter change and clinical scores (p&lt;0.0001). Compared with WT mice, Kng1−/− mice displayed reduced synovial hyperplasia and cartilage erosions, and decreased protein and mRNA levels of IL-1b and IL-6 in joint tissue (p&lt;0.001). BK level and kallikrein activity in Kng1−/− plasma were significantly lower than that of WT mice. Incubation with collagen-anti-collagen antibody complex mediated BK production in human plasma. Consistent with the phenotype of Kng1−/− mice, the mice lacking BK receptors also displayed a reduced arthritis severity, as well as a decrease in IL-1b and IL-6 production in joint tissue. Our study provides the first genetic evidence indicating an important role of HK in the pathogenesis of arthritis, and the blockade of HK cleavage to prevent BK production might be a novel therapeutic strategy for RA treatment.

Abstract:
Nonnegative matrix factorization (NMF) is a linear approach for extracting localized feature of facial image. However, NMF may fail to process the data points that are nonlinearly separable. The kernel extension of NMF, named kernel NMF (KNMF), can model the nonlinear relationship among data points and extract nonlinear features of facial images. KNMF is an unsupervised method, thus it does not utilize the supervision information. Moreover, the extracted features by KNMF are not sparse enough. To overcome these limitations, this paper proposes a supervised KNMF called block kernel NMF (BKNMF). A novel objective function is established by incorporating the intra-class information. The algorithm is derived by making use of the block strategy and kernel theory. Our BKNMF has some merits for face recognition, such as highly sparse features and orthogonal features from different classes. We theoretically analyze the convergence of the proposed BKNMF. Compared with some state-of-the-art methods, our BKNMF achieves superior performance in face recognition.

Kanker Nasofaring (KNF) merupakan keganasan yang sering ditemukan di Asia.1 Puncak insidensi KNF berada pada usia dewasa muda dan pada pasien usia 55 – 59 tahun.2 Radioterapi merupakan terapi standar untuk KNF stadium awal, sedangkan pada stadium lokal lanjut diberikan kemoradiasi.1 KNF telah terbukti memiliki keterkaitan dengan Epstein Barr Virus (EBV). Dengan berkembangnya biologi molekular, konsentrasi DNA tumor dalam plasma dan serum pada pasien KNF dapat diukur secara kuantitatif dengan polymerase chain reaction (PCR).3 Plasma EBV DNA berguna dalam hal diagnosis, pemantauan respon terapi, dan prediktor prognosis pada pasien KNF.1,3 Penulisan tinjauan pustaka ini ditujukan untuk menambah pengetahuan mengenai manfaat pemeriksaan kadar plasma EBV DNA pada pasien KNF.

Abstract Thrombosis and inflammation are hallmarks of ischemic stroke still unamenable to therapeutic interventions. High-molecular-weight kininogen (KNG) is a central constituent of the contact-kinin system which represents an interface between thrombotic and inflammatory circuits and is critically involved in stroke development. Kng−/− mice are protected from thrombosis after artificial vessel wall injury and lack the proinflammatory mediator bradykinin. We investigated the consequences of KNG deficiency in models of ischemic stroke. Kng−/− mice of either sex subjected to transient middle cerebral artery occlusion developed dramatically smaller brain infarctions and less severe neurologic deficits without an increase in infarct-associated hemorrhage. This protective effect was preserved at later stages of infarction as well as in elderly mice. Targeting KNG reduced thrombus formation in ischemic vessels and improved cerebral blood flow, and reconstitution of KNG-deficient mice with human KNG or bradykinin restored clot deposition and infarct susceptibility. Moreover, mice deficient in KNG showed less severe blood-brain barrier damage and edema formation, and the local inflammatory response was reduced compared with controls. Because KNG appears to be instrumental in pathologic thrombus formation and inflammation but dispensable for hemostasis, KNG inhibition may offer a selective and safe strategy for combating stroke and other thromboembolic diseases.

Background: Nasopharyngeal carcinoma (NPC) is one of the most common among head and neck malignancies, especially in some Asian countries. Epstein-Barr Virus (EBV) is one of the agent causing NPC, expressing many proteins such as Epstein-Barr nucleus antigen (EBNA). Expression of EBNA theoretically influencing therapy KNF outcome. Purpose: To differentiate therapy result between NPC expressed positive EBNA1 and EBNA2 compared with NPC expressed negative EBNA1 and EBNA2. Method: Nested case control study toward 28 complete remission NPC patients and 28 partial remission NPC patients post treatment. Result: There was no significant difference in therapy outcome between NPC with EBNA1 expression =4.90 compared to EBNA1 <4.90 (p: 0.160; OR: 0.222) and there was a significant difference therapy outcome between NPC with expression of EBNA2 =1.30 compared to EBNA2 <1.30 (p: 0.029; OR: 0.289). Conclusion: EBNA2 expression is one of protective agents in NPC post treatment outcome. Keterlibatan infeksi virus Epstein-Barr (EBV) merupakan salah satu faktor penyebab terjadinya KNF. Infeksi EBV mengekspresikan beberapa protein antara lain Epstein-Barr nucleus antigen (EBNA) yang secara teori mempengaruhi onkogenesis termasuk hasil terapi KNF. Tujuan:Menentukan adanya perbedaan hasil terapi antara KNF yang mengekspresikan EBNA1 serta EBNA2 dengan KNF yang tidak mengekspresikan EBNA1 serta EBNA2. Metode: Nested case-controlterhadap 28 penderita KNF yang mengalami remisi sempurna dan yang mengalami remisi parsial pascaterapi KNF. Hasil: Tidak terdapat perbedaan yang bermakna antara hasil terapi KNF yang mengekspresikan EBNA1 =490 dengan KNF yang mengekspresikan <4,90 (p: 0,160; OR: 0,222), dan terjadi perbedaan bermakna antara hasil terapi KNF yang mengekspresikan EBNA2 =1,30 dengan KNF yang mengekspresikan EBNA2 <1,30 (p: 0,029; OR: 0,289). Advanced triboelectric nanogenerator techniques provide a massive opportunity for the development of new generation wearable electronics, which toward multi-function and self-powering. Textiles have been refreshed with the requirement of flexible electronics in recent decades. In particular, knitted-textiles have exhibited enormous and prominent potential possibilities for smart wearable devices, which are based on the merits of high stretchability, excellent elasticity, comfortability as well as compatibility. Combined knitted textiles with nanogenerator techniques will promote the knitted textile triboelectric nanogenerators (KNGs) emerging, endowing conventional textiles with biomechanical energy harvesting and sensing energy supplied abilities. However, the design of KNGs and the construction of KNGs are based on features of human motions symbolizing considerable challenges in both high efficiency and excellent comfort. Currently, this review is concerned with KNGs construction account of triboelectric effects referring to knitted-textile classifications, structural features, human motion energy traits, working mechanisms, and practical applications. Moreover, the remaining challenges of industrial production and the future prospects of knitted-textile triboelectric nanogenerators of harvesting biomechanical energy are presented.

Karsinoma nasofaring (KNF) merupakan tumor ganas pada sel epitel nasofaring dan merupakan penyakit multifaktor yang bersifat endemik. Tipe KNF tidak berdiferensiasi (KNF WHO-3) telah terbukti konsisten dengan infeksi EBV. Gen litik BRLF1 berperan sebagai mediator transisi dari fase laten menjadi litik. Pengukuran aktivitas mRNA EBV di lokasi tumor primer perlu dilakukan karena lebih mencerminkan patogenesis KNF yang sesungguhnya dari pada diagnosis serologi dan pengukuran DNA EBV di sirkulasi darah. Penelitian ini bertujuan untuk mengetahui nilai ekspresi relatif mRNA BRLF1 Epstein-Barr Virus pada beberapa tingkat stadium tumor penderita KNF sebagai petanda biologi potensial dalam patogenesis KNF. Metode penelitian yang digunakan adalah metode deskriptif yang berbentuk studi seran lintang. Sampel penelitian ini adalah biopsi jaringan tumor dalam blok parafin penderita KNF sebanyak 24 sampel. Sampel tersebut telah didiagnosis pasti sebagai KNF WHO-3 dari hasil pemeriksaan Patologi Anatomi pada Poli Patologi Anatomi, RSUD Prof. Dr. Margono Soekarjo, Purwokerto serta memenuhi kriteria inklusi sampel. Analisis statistik dilakukan untuk membandingkan ekspresi relatif mRNA BRLF1 stadium yang berbeda. Nilai ekspresi relatif mRNA BRLF1 EBV pada 24 sampel KNF berkisar 99,04415959-1097,496026. Hasil analisis statistik menunjukkan nilai ekspresi relatif mRNA BRLF1 EBV pada stadium awal (n = 5; 544,72420 + 142,614733) lebih tinggi 1,8 kali dibandingkan dengan stadium lanjut (n = 17; 395,68612 + 293,172201), namun peningkatan ekspresi relatif mRNA BRLF1 EBV tidak bermakna secara statistik karena didapatkan nilai p=0,130 (p>0,05). Oleh karena itu, ekspresi relatif mRNA BRLF1 EBV dari biopsi jaringan tumor dalam blok parafin tidak berpotensi sebagai petanda biologi molekul patogenesis KNF, khususnya progresivitas tumor pada stadium lanjut KNF.

Abstract Three single nucleotide polymorphisms (SNPs) were recently found to be associated with activated partial thromboplastin time (aPTT). Because shortened aPTT levels have been observed in patients experiencing venous thrombosis (VT), we investigated the effects of these 3 aPTT-associated SNPs, rs2731672, rs9898, and rs710446, on the risk of VT in a sample of 1110 healthy patients and 1542 patients with VT. Among the 3 tested SNPs, only rs710446 was associated with VT risk; the rs710446-C allele was associated with an increased risk of VT (odds ratio 1.196, 95% confidence interval 1.071-1.336, P = .0012). This association also was observed in an independent sample of 590 controls and 596 patients (odds ratio 1.171, 95% confidence interval 0.889-1.541, P = .059). We also confirmed that the rs710446-C allele was associated with decreased aPTT levels, making this nonsynonymous Ile581Thr variant a new genetic risk factor for VT.

Tensile properties and morphological studies of linear low density polyethylene (LLDPE)/poly (vinyl alcohol) (PVA)/kenaf (KNF) composites were investigated. The composites with different KNF loading (0, 10, 20, 30, 40 phr) were prepared using a Thermo Haake Polydrive internal mixer at 150°C and 50 rpm for 10 min. The results indicated that tensile strength and elongation at break were decreased with increasing KNF loading, whereas tensile modulus showed the opposite trend. Tensile fractured surfaces observed by scanning electron microscopy showed better interfacial adhesion between LLDPE/PVA and KNF at 10 phr KNF loading.

Hubungan antara lama waktu pasca kemoradiasi dengan derajat disfagia orofaringeal pada KNF sampai saat ini belum diketahui secara jelas. Tujuan : Menganalisis hubungan lama waktu pasca kemoradiasi dengan derajat disfagia orofaringeal pada KNF. Metode : Penelitian observasional analitik dengan desain belah lintang. Subjek adalah penderita KNF pasca kemoradiasi di RSUP Dr. Kariadi Semarang yang memenuhi kriteria penelitian. Penentuan status disfagia dan derajat disfagia dengan pemeriksaan Gugging Swallowing Screen (GUSS). Data lengkap subjek didapatkan dari anamnesis dan rekam medis. Uji statistik yang digunakan adalah Chi-Squared. Hasil : Sebanyak 55 subjek KNF mengalami disfagia (100%), 48 (87,3%) subjek mengalami disfagia derajat ringan, tidak didapatkan subjek yang mengalami disfagia derajat sedang, dan 7 (12,7%) subjek mengalami disfagia derajat berat. Lama waktu pasca kemoradiasi (p = 0,451), jenis kemoterapi (p = 0,267), dan modalitas terapi (p = 0,402) tidak berhubungan dengan derajat disfagia orofaringeal pada KNF. Kesimpulan : Mayoritas subjek terjadi disfagia derajat ringan. Lama waktu pasca kemoradiasi, jenis kemoterapi, dan modalitas terapi tidak berhubungan dengan derajat disfagia orofaringeal pada KNF. Abstract Although experimental and clinical studies have suggested that plasma kallikrein-kinin system (KKS) is involved in arthritis, their role in the disease pathogenesis has never been studied using gene deficient mouse model. High molecular weight kininogen (HK) is a major component of KKS, the activation of which induces HK cleavage to generate proinflammatory bradykinin (BK). In mice, kininogen-1 (Kng1) gene in liver drives production of HK in plasma. We generated a Kng1-/- mouse strain whose liver Kng1 mRNA and plasma HK protein were absent. To induce collagen antibody-induced arthritis (CAIA), three groups of mice (WT, Kng1-/-, and B2RB1R-/- that lack double BK receptors) were injected I.P. with anti-type II collagen antibody cocktail (3 mg) on day 0 and LPS (50 μg) on day 3. WT mice started to develop joint swelling and erythema on day 5, and the increase in ankle joint diameter became marked on day 6 (1.32±0.13 mm) and peaked on day 8 (1.41±0.17 mm). As detected by ELISA, plasma levels of IL-1β were increased on day 8 than day 0 (p&lt;0.01). Real time PCR indicated that IL-1β mRNA in monocytes of WT mice was increased by 3 fold. In contrast, either Kng1-/- or B2RB1R-/- mice did not develop marked joint swelling, their plasma IL-1β and monocytic IL-1β mRNA levels were not significantly increased. Our results demonstrate an essential role of HK in the pathogenesis of arthritis, BK serving as a key mediator. Blockade of HK cleavage has a potential for anti-arthritic treatment.

Clinical observations have shown that KKS activation and HK cleavage are involved in inflammatory bowel disease (IBD). However, the pathogenic role of HK in IBD have never been characterized using a genetic approach. Our goal is to characterize the role of HK in the pathogenesis of IBD,using HK-deficient (Kng1-/-) mice we recently generated. Dextran sulfate sodium (DSS)-induced colitis in mice were induced by 2.5% (wt/vol) DSS in drinking water for 7 days. Plasma bradykinin level in wild-type (WT) mice was elevated on day 7 (p&lt;0.001). Weight loss index and disease activity index (DAI) of DSS-treated Kng1-/- mice were significantly lower than those of WT mice (p&lt;0.05). Kng1-/- mice had a decrease in colonic cytokines (TNF-α, IL-1β, IL-6 and INF-γ) and myeloperoxidase (MPO) levels (p&lt;0.001). As indicated by H&E staining, Kng1-/- mice exhibited a reduced colonic tissue damage and neutrophil infiltration in colonic lamina propria (p&lt;0.001). Similar with Kng1-/- mice, the mice lacking double bradykinin receptors (B1RB2R-/-) exhibited an amelioration of DSS-induced colitis, including the amelioration of weight loss, DAI, colonic tissue damage, and colonic cytokine levels. This study provides the first genetic evidence demonstrating that HK and its cleavage product bradykinin play a critical role in the pathogenesis of IBD.

High-molecular-weight kininogen (HK), together with factor XI, factor XII and prekallikrein, is part of the contact system that has proinflammatory, prothrombotic, and vasoactive properties. We hypothesized that HK plays a role in the host response during pneumonia-derived sepsis. To this end mice were depleted of kininogen (KNG) to plasma HK levels of 28% of normal by repeated treatment with a specific antisense oligonucleotide (KNG ASO) for 3 wk before infection with the common human sepsis pathogen Klebsiella pneumoniae via the airways. Whereas plasma HK levels increased during infection in mice treated with a scrambled control ASO (Ctrl ASO), HK level in the KNG ASO-treated group remained reduced to 25–30% of that in the corresponding Ctrl ASO group both before and after infection. KNG depletion did not influence bacterial growth in lungs or dissemination to distant body sites. KNG depletion was associated with lower lung CXC chemokine and myeloperoxidase levels but did not impact neutrophil influx, lung pathology, activation of the vascular endothelium, activation of the coagulation system, or the extent of distant organ injury. These results were corroborated by studies in mice with a genetic deficiency of KNG, which were indistinguishable from wild-type mice during Klebsiella-induced sepsis. Both KNG depletion and KNG deficiency were associated with strongly reduced plasma prekallikrein levels, indicating the carrier function of HK for this zymogen. This study suggests that KNG does not significantly contribute to the host defense during gram-negative pneumonia-derived sepsis.

Keberadaan EBV pada penderita KNF dapat diketahui dengan ditemukannya DNA EBV dalam spesimen biopsi jaringan penderita KNF. Genom EBV dan ekspresi sebagian dari produk gen laten virus secara konsisten terdeteksi hampir di setiap sel dalam kanker ini, salah satunya adalah gen Latent Membrant Protein (LMP). Aktivitas mRNA EBV lebih mencerminkan patogenesis KNF yang sesungguhnya dari pada diagnosis serologi dan pengukuran DNA EBV di sirkulasi. Penelitian dilakukan di Laboratorium PA RSUD Margono Seokarjo, Purwokerto/Lab PA Fakultas Kedokteran dan laboratorium genetika/molekuler Universitas Jenderal Soedirman Purwokerto. Analisis Ekspresi mRNA LMP2A EBV dengan teknik one step RT-PCR dan produk RT-PCR (amplikon cDNA) divisualisasi dengan elektroforesis gel agarosa 1%. Hasil ekspresi mRNA LMP2A EBV adalah 27,3% (6 dari 22 sampel). Kesimpulan, metode one step RT-PCR dapat digunakan untuk menganalisis ekspresi mRNA LMP2A EBV dari sampel biopsi jaringan KNF dalam blok paraffin dan hasil positivitas ekspresi mRNA LMP2A EBV sedang, sehingga berpotensi digunakan sebagai petanda biologi molekul diagnosis KNF.

Yin-deficiency-heat (YDH) syndrome is a concept in Traditional Chinese Medicine (TCM) for describing subhealth status. However, there are few efficient diagnostic methods available for confirming YDH syndrome. To explore the novel method for diagnosing YDH syndrome, we applied iTRAQ to observe the serum protein profiles in YDH syndrome rats and confirmed protein levels by ELISA. A total of 92 differentially expressed proteins (63 upregulated proteins and 29 downregulated proteins), which were mainly involved in complement and coagulation cascades and glucose metabolism pathway, were identified by the proteomic experiments. Kininogen 1 (KNG1) was significantly increased (p<0.0001), while apolipoprotein C-III (APOC3, p<0.005) and paraoxonase 1 (PON1, p<0.001) were significantly decreased in the serum of YDH syndrome rats. The combination of KNG1, APOC3, and PON1 constituted a diagnostic model with 100.0% sensitivity and 85.0% specificity. The results indicated that KNG1, APOC3, and PON1 may act as potential biomarkers for diagnosing YDH syndrome. KNG1 may regulate cytokines and chemokines release in YDH syndrome, and the low levels of PON1 and APOC3 may increase oxidative stress and lipolysis in YDH syndrome, respectively. Our work provides a novel method for YDH syndrome diagnosis and also provides valuable experimental basis to understand the molecular mechanism of YDH syndrome.

Novel mutations in the RSW1 and KNOPF genes were identified in a large-scale screen for mutations that affect cell expansion in early Arabidopsis embryos. Embryos from both types of mutants were radially swollen with greatly reduced levels of crystalline cellulose, the principal structural component of the cell wall. Because RSW1 was previously shown to encode a catalytic subunit of cellulose synthase, the similar morphology of knf and rsw1-2 embryos suggests that the radially swollen phenotype of knf mutants is largely due to their cellulose deficiency. Map-based cloning of the KNF gene and enzyme assays of knf embryos demonstrated that KNF encodes α-glucosidase I, the enzyme that catalyzes the first step in N-linked glycan processing. The strongly reduced cellulose content of knf mutants indicates that N-linked glycans are required for cellulose biosynthesis. Because cellulose synthase catalytic subunits do not appear to be N glycosylated, the N-glycan requirement apparently resides in other component(s) of the cellulose synthase machinery. Remarkably, cellular processes other than extracellular matrix biosynthesis and the formation of protein storage vacuoles appear unaffected in knf embryos. Thus in Arabidopsis cells, like yeast, N-glycan trimming is apparently required for the function of only a small subset of N-glycoproteins.

The results indicated that composites with 3-(trimethoxysilyl) propyl methacrylate gives higher tensile strength and modulus but lower elongation at break than composites without. The presence of 3-(trimethoxysilyl) propyl methacrylate found to enhance the interfacial adhesion between LLDPE/PVA matrix and KNF fiber. Morphological studies on tensile fractured surfaces showed good adhesion between LLDPE/PVA matrix and KNF, and better dispersion of KNF for the composites with 3-(trimethoxysilyl) propyl methacrylate.

The Polish Financial Supervision Authority (hereinafter: KNF) is the key element of the economic security system of the Polish State. By establishing the KNF, the Polish legislator applied the concept of integratedfi nancial supervision (covering bank supervision, insurance supervision and capital market supervision) located outside the central bank. The KNF has been vested with a broad mandate, including powers to supervise authorised entities. However, the scope and nature of measures available to the KNF in the prevention of economic crime are determined by the institutional position of the KNF, which has not been established as a law enforcement authority or a uniformed or special service but rather an institution engaged in the fi ght against economic crime by providing expertise, supporting other state services specialised in this area. The KNF and the Offi ce of the Polish Financial Supervision Authority (hereinafter: UKNF) actively support, among others, the police units that fi ght crime in the fi nancial market and work to increase the economic security of the Polish State. Therefore, it is imperative to ensure broad and effi cient cooperation between the police and the KNF. However, such collaboration must not be limited to relations between the institutions’ management; it should also involve, as far as possible, working and operational activities carried out at both institutions by individuals responsible for day-to-day tasks. This paper provides a closer perspective on the formal and organisational framework of said cooperation and discusses its examples.

Abstract:
Abstract Background- High molecular weight Kininogen (HK) is a nonenzymatic co-factor of the contact activation system. HK binds prekallikrein (PK) and FXI to surfaces in proximity to FXII, amplifying PK activation by FXIIa and the reciprocal activation of FXII by activated PK (PKa), as well as FXI activation of FXIIa. PKa cleavage of HK also liberates bradykinin-a proinflammatory and vasoactive nanopeptide. The aim of this study was to define the pro-thrombotic role of kininogen in venous thrombosis (VT) and to use in vivo serial analysis of thrombus development to understand the recruitment and retention of platelets in the growing thrombus in the absence and presence of kininogen. Methods- The development of VT in mice deficient in kininogen (mKng1-/-) was compared to that in their wild-type littermates. A femoral-saphenous stasis VT model was prepared by ligating both saphenous and femoral veins. Next VT formation, growth, and dissolution (n=3 for each group) was monitored using intravital microscopy (IVM) via a multichannel epifluorescence microscope (Nikon Eclipse 90i). To induce stasis VT, FITC-dextran (10 mg/kg, ex/em 488/520 nm) was injected retro-orbitally, and then continuous light irradiation (20x objective, 475nm/35nm) of the saphenous vein was applied for 5 minutes. FITC-dextran fluorescence angiography monitored thrombus formation and dissolution. Immediately after VT formation, platelet accumulation at the thrombus site was monitored in the Cy5 channel (630/38 nm) via injection of a GPIbβ antibody conjugated with Dylight-649 (150nmol/kg), over time. All images were identically windowed in each channel, and thrombus area was measured using NIH ImageJ software. To corroborate IVM studies, we also evaluated a complete stasis model of inferior vena cava (IVC) ligation (n=7-8 per group). Thrombi were harvested after 48 hours and thrombus weight and length were measured to estimate thrombus mass. FXI circulates in blood as a homodimer along with HK. We determined the effect of kininogen deficiency on FXI activity. FXI activity assay used a combination of inhibitors, serially, to monitor the cleavage of substrate specific to activated FXI and release of chromogen, as a function of FXI activity. Finally, to determine the effects of Kng1 deficiency on bleeding, tail vein bleeding times were also determined (n=8 per group). Results- In femoral-saphenous stasis VT, thrombus developed in both groups immediately following FITC-channel light irradiation. However, thrombus size was smaller in Kng1-/- as compared to WT (Figure 1). Results from serial IVM of VT indicated faster thrombus dissolution in the Kng1-/- group. Lower platelet signals, as shown at 2 and 6 hours in the Kng1-/- mice may be consistent with this hypothesis. Thrombus area analysis suggested decreased thrombus formation in the Kng1-/- animals, and temporal analysis indicated faster dissolution by 6 hours (Figure 2). IVC ligation results corroborated the findings of femoral-saphenous DVT model, demonstrating that thrombus weight was significantly lower in Kng1-/- mice as compared to WT (p&lt;0.001, Figure 3). FXI activity was also decreased in the Kng1-/- group (p&lt;0.10). Tail vein bleeding times, however, showed no increased bleeding in Kng1-/- mice. Conclusion- These initial results suggest a pro-thrombotic role of kininogen and a protective role of kininogen deficiency in two murine venous thrombosis models, without incurring a bleeding penalty. Thrombus dissolution was faster and platelet accumulation was inhibited in Kng1-/- mice. These findings suggest that targeting kininogen may provide a new approach to prevent and treat venous thrombosis. Figure 1 Figure 1. Latar Belakang: Karsinoma nasofaring (KNF) merupakan keganasan yang tersering ditemukan, dan berdampak pada penurunan kualitas hidup serta memiliki mortalitas tinggi. Penanganan KNF selama ini terkendala oleh waktu tunggu yang cukup lama dalam menentukan staging KNF terutama untuk antrian pemeriksaan computed tomography scan (CT scan) dan Ultrasonography (USG). Pemeriksaan kadar DNA EBV (Deoxyribonucleic acid Epstein-Barr Virus) pada pasien yang relatif lebih mudah dan terjangkau dapat digunakan untuk memprediksi stadium dan prognosis KNF. Dengan mengetahui prognosis KNF lebih dini, maka diharapkan penanganan terhadap KNF dapat segera dilakukan. Tujuan: Mengetahui apakah kadar DNA EBV dapat dipakai untuk memprediksi stadium dan prognosis KNF dengan cara mencari hubungan antara kadar DNA EBV dengan stadium KNF. Metode: Penelitian cross sectional melibatkan 15 subjek penelitian yang terdiagnosis KNF WHO tipe 3 kemudian dilakukan staging dengan CT scan, USG abdomen, dan foto toraks, serta diambil sampel darah untuk diukur kadar DNA EBV. Hasil: Seluruh subjek penelitian mengalami peningkatan kadar DNA EBV sesuai dengan peningkatan stadium KNF. Peningkatan stadium KNF berhubungan signifikan dengan peningkatan kadar DNA EBV (p=0,001). Ukuran tumor (T) berhubungan signifikan dengan kadar DNA EBV (p=0,023), ukuran nodul (N) berhubungan signifikan dengan kadar DNA EBV (p=0,005), ada tidaknya metastasis tidak berhubungan signifikan dengan kadar DNA EBV (p=0,398). Nilai cut off kadar DNA EBV sebesar 952 kopi/ml. Kesimpulan: Terdapat hubungan yang signifikan antara kadar DNA EBV dengan stadium klinis, dengan demikian kadar DNA EBV dapat dipertimbangkan untuk digunakan sebagai prediktor stadium dan prognosis KNF. Background: Nasopharyngeal carcinoma (NPC) is the predominant tumor type arising in the nasopharynx, with a high mortality and affecting quality of life. NPC treatment management is hindered by long queues of Computed Tomography Scan (CT scan) and Ultrasonography (USG) examinations to ascertain the NPC staging. The examination of Epstein-Barr Virus (EBV) DNA level is relatively simpler and inexpensive to predict the NPC staging and prognosis, thus, it can speed up NPC treatment. Objective: To determine whether EBV DNA level can be used to predict the NPC stage and prognosis by finding a correlation between EBV DNA level and NPC stage. Method: This was a cross-sectional study involving 15 respondents who were diagnosed as WHO type 3 NPC, and examined by CT scan, abdominal ultrasound, chest X-ray, and blood test for measuring the levels of EBV DNA to determine the stage. Results: All respondents had elevated levels of EBV DNA in accordance with NPC stage elevation. Increased NPC stages were significantly correlated with elevated levels of EBV DNA (p=0.001). The size of tumor (T) was significantly correlated with EBV DNA (p=0.023), the size of nodule (N) was significantly correlated with EBV DNA (p=0.005). The presence or absence of metastasis did not significantly correlate with EBV DNA (p=0.398). The EBV DNA cut off value was 952 copies/ml. Conclusions: There was a significant correlation between EBV DNA levels and clinical stages, hence EBV DNA can be considered to be used as NPC staging and prognosis predictor.

The mechanical properties of polypropylene (PP)/waste tire dust (WTD)/kenaf (KNF) composites with and without 3-aminopropyltriethoxysilane (APTES) coupling agent were investigated. The composites were prepared using a Thermo Haake Polydrive internal mixer, where different KNF loading (0, 5, 10, 15, 20 phr) were used. The results revealed that the tensile modulus increased with increasing KNF loading but tensile strength and elongation at break were found decreased. Composites with APTES give higher tensile strength and modulus but lower elongation at break than composites without APTES. The presence of APTES enhanced the interfacial adhesion between PP/WTD matrices and KNF which result in higher tensile strength and modulus of the composites. These findings were supported by the morphological study of the tensile fractured surfaces of the composites.

The pathogenesis of NPC is more directly reflected by carcinoma-specific viral transcriptional activity at the site of primary tumor. Therefore, EBV infection in NPC is also reflected by the expression of EBV latent and lytic gene, particularly in the expression of mRNA LMP1 EBV as a biomarker for EBV latent infection. The aims of this study were to determine the relative expression of mRNA LMP1 EBV in undifferentiated NPC (NPC WHO-III) patient as a biomarker of tumour progressivity in NPC. The samples were the tissue biopsies formalin-fixed embedded paraffin (FFPE) of 28 undifferentiated NPC patients at Pathology Anatomy Departement, R.S.U.D. Prof. dr. Margono Soekarjo, Purwokerto. The relative expression of mRNA LMP1 EBV was determined by RT-qPCR technique and calculated by formula 2-ΔCt. The relative expression of mRNA LMP1 EBV in undifferentiated NPCpatients on advanced staging was approximately start from 5,832.90 to 39,786.70. Latar belakang: Karsinoma nasofaring (KNF) berpengaruh terhadap kualitas hidup penderita, baik dari kankernya sendiri, maupun pengobatan serta efek sampingnya. Penilaian kualitas hidup penderita KNF dapat secara unidimensional menggunakan parameter status tampilan Karnofsky Performance Scale (Karnofsky PS) atau multidimensional memakai parameter European Organization For Research and Treatment of Cancer Quality of Life Questionnaire C30 (EORTC QLQ-C30) dan European Organization For Research And Treatment Of Cancer Head and Neck Cancer Quality of Life Questionnaire (EORTC QLQ-H&N35).Tujuan: Menilai kesesuaian hasil skor Karnofsky PS, EORTC QLQ-C30 dan EORTC QLQ-H&N35 dalam menilai kualitas hidup penderita KNF dengan menentukan korelasi antar parameter dan menentukan estimasi skor Karnofsky PS menggunakan skor EORTC QLQ-C30 dan skor EORTC QLQ-H&N35. Menganalisis hubungan antar parameter menurut stadium.Metode: Penelitian adalah observasional analitik dengan pedekatan cross sectional. Populasinya semua kasus KNF yang berobat ke Bagian THT-KL, Rumah Sakit Wahidin Sudirohusodo, Makassar. Sampel sebanyak 48 orang dipilih secara purposive. Penilaian kualitas hidup menggunakan parameter Karnofsky PS, EORTC QLQ-C30 dan EORTC QLQ-H&N35. Hasil: Didapati kesesuaian hasil skor dari ketiga parameter, ditandai adanya korelasi bermakna antara skor Karnofsky PS, skor EORTC QLQ-C30 dan skor EORTC QLQ-H&N35. Semakin tinggi skor Karnofsky PS, akan semakin rendah skor EORTC QLQ-C30 dan skor EORTC QLQ-H&N35 maka kualitas hidup penderita KNF semakin baik. Skor EORTC QLQ-C30 dan skor EORTC QLQ-H&N35 dapat mengestimasi skor Karnofsky PS. Terdapat hubungan bermakna antara Karnofsky PS, EORTC QLQ-C30 dan EORTC QLQ-H&N35 menurut stadium (p<0,05). Kesimpulan: DEORTC QLQ-C30 dan EORTC QLQ-H&N35 dapat melengkapi Karnofsky PS dalam penilaian kualitas hidup penderita kanker kepala leher terutama KNF. Latar belakang: Epstein-Barr Virus (EBV) adalah anggota herpes virus berkaitan dengan etiologi karsinoma nasofaring (KNF). Pada pasien KNF jumlah monosit dalam sel darah tepi mengalami penurunan dan kebanyakan masih dalam bentuk immature sehingga menurunkan respon imun pasien serta meningkatkan kemungkinan terjadinya penyakit periodontal. Radioterapi merupakan salah satu metode terapi yang banyak digunakan untuk kasus KNF. Calprotectin diproduksi dalam sitoplasma sel monosit dan levelnya meningkat pada beberapa penyakit inflamasi, termasuk inflamasi jaringan periodontal, ditandai dengan peningkatan kadar calprotectin pada cairan sulkus gingiva (CSG). Tujuan: mengkaji perbedaan kadar calprotectin pada pasien KNF sebelum dan setelah dilakukan radioterapi, pada sel monosit, serum dan CSG. Metode Penelitian: sepuluh pasien KNF akibat infeksi EBV digunakan sebagai subjek dalam penelitian ini. Lima orang sebagai sampel kelompok sebelum radioterapi dan 5 orang sebagai sampel kelompok sesudah radioterapi. Dari masing-masing pasien diambil sel monosit dan serum darah tepi serta CSG. Kadar calprotectin diukur menggunakan metode ELISA. Hasil: kadar calprotectin pada kelompok sampel sebelum radioterapi lebih rendah dibandingkan kelompok sam pel sesudah radioterapi dilihat melalui sel monosit dan serum darah tepi. Sementara dari CSG, kadar calprotectin kelompok sampel sebelum radioterapi nampak lebih tinggi dibanding kelompok sesudah radioterapi. Hasil analisis statistik Anova menunjukkan perbedaan yang signifikan (p<0,05). Kesimpulan: terdapat perbedaan kadar calprotectin pada sel monosit, serum darah tepi dan CSG pasien KNF antara sebelum dan sesudah radioterapi. Pada sel monosit dan serum darah tepi, terjadi penurunan kadar calprotectin, sementara pada CSG terjadi peningkatan kadar calprotectin antara sebelum dan sesudah radioterapi.

The effect of untreated and treated (chromium sulfate and sodium bicarbonate) kenaf (KNF) on the water uptake and morphological properties of linear low density polyethylene/poly(vinyl alcohol)/kenaf (LLDPE/PVOH/KNF) composites was studied. The composites were prepared by using an internal mixer (Haake Polydrive) at 150 °C and 50 rpm for 10 min. The results showed that the untreated composites tend to absorb greater amount of water compared to the treated composites. This shows that the chromium sulfate and sodium bicarbonate treatment on KNF had improved the interfacial adhesion between KNF and LLDPE/PVOH matrices. Consequently, the capability of water absorption by the treated composites was reduced. Scanning electron microscope (SEM) result shows that lesser traces of filler pull-out and better filler-matrix interfacial adhesion were observed in the tensile fractured surfaces of treated composites.

Latar belakang: Protein Epstein-Barr Virus Encoded Small RNA (EBER) ditemukan pada sebagian besar jaringan karsinoma nasofaring (KNF). Hal ini merupakan bukti bahwa infeksi virus Epstein-Barr (VEB) menjadi salah satu penyebab terjadinya KNF. Penurunan ekspresi EBER dipengaruhi oleh maturasi sel dan diferensiasi sel karsinoma. Tujuan: Mengidentifikasi hubungan ekspresi EBER dengan stadium dan jenis histopatologi pada penderita KNF. Metode: Penelitian observasional analitik dengan pendekatan cross sectional. Penelitian dilakukan di Unit Rawat Jalan Telinga Hidung Tenggorok - Bedah Kepala Leher Rumah Sakit Umum Daerah Doktorr. Soetomo Surabaya mulai bulan November 2015 hingga Oktober 2016 dengan consecutive sampling. Fisher exact test untuk melihat hubungan ekspresi EBER dengan stadium KNF. Hasil: Ekspresi EBER dari seluruh sampel didapati negatif sebanyak 23,33%, positif lemah sebanyak 10%, positif sedang sebanyak 33,33%, dan positif kuat sebanyak sebanyak 33,33%. Berdasarkan hasil uji statistik menunjukkan hubungan antara ekspresi EBER dengan stadium KNF didapatkan p=0,623, sedangkan ekspresi EBER dengan jenis histopatologi KNF diketahui p=0,204. Kesimpulan: Tidak terdapat hubungan antara ekspresi EBER dengan stadium KNF, maupun antara ekspresi EBER dengan jenis histopatologi KNF. Purpose: To identify association between EBER expression and histopathological type of NPC. Method: This study was an observational analytic with cross-sectional design. This study occured at Inpatients Unit of Otorhinolaryngology - Head and Neck Surgery of Dr. Soetomo Hospital Surabaya, November 2015 - October 2016. Samples were collected by consecutive sampling. Statistical analysis was using Fisher’s exact test. Result: The EBER expression from all sample there were 23.33% negative, 10.00% weak positive, 33.33% moderate positive, and 33.33% strong positive. Statistical analysis was obtained p=0.623 and the association between EBER expression with stage of NPC was obtained p=0.204. Conclusion: There was no association between EBER expression and stage NPC, nor histopathological type of nasopharyngeal carcinoma. ABSTRAK Pendahuluan: Karsinoma nasofaring (KNF) merupakan salah satu keganasan regio kepala dan leher yang paling sering ditemukan di dunia. Hingga saat ini, radioterapi masih menjadi terapi pilihan utama untuk penderita KNF, terutama jenis KNF loko-regional yang belum metastasis jauh. Radioterapi dapat memberikan efek samping terhadap sistem hematopoietik, sehingga pemeriksaan darah lengkap merupakan prosedur yang wajib dilakukan sebelum, selama, dan setelah radioterapi. Sayangnya, data penelitian terkait efek radioterapi terhadap parameter darah lengkap pasien KNF di Indonesia masih terbatas. Oleh karena itu, penelitian ini bertujuan untuk mengetahui efek radioterapi terhadap sistem hematopoietik pada penderita KNF dengan melihat jumlah leukosit, kadar hemoglobin, dan jumlah trombosit pasca radioterapi. Metode: Penelitian ini merupakan penelitian analitik observasional dengan desain cross sectional menggunakan data rekam medik penderita KNF yang menjalani radioterapi siklus lengkap di Rumah Sakit Pendidikan Universitas Hasanuddin periode 2016-2018. Data yang diperoleh diolah dengan menggunakan uji hipotesis Friedman. Hasil: Dari 41 pasien yang memenuhi kriteria ekslusi dan inklusi, perbedaan rata-rata yang signifikan pada parameter darah lengkap pasca radioterapi hanya ditemukan pada variabel jumlah leukosit (p<0,05), dibandingkan dengan variabel hemoglobin dan trombosit yang perubahannya tidak begitu bermakna (p>0,05). Pembahasan: Radiasi yang mengenai sumsum tulang akan menyebabkan depresi jumlah sel darah karena destruksi sel punca hematopoietik dan sel progenitor yang sangat sensitif radiasi. Berdasarkan tingkat sensitifitas terhadap radioterapi, leukosit merupakan sel darah yang paling radiosensitif, diikuti oleh trombosit kemudian eritrosit. Kesimpulan: Radioterapi memberikan efek yang signifikan terhadap perubahan jumlah leukosit pasien karsinoma nasofaring. Kata kunci: Darah lengkap, karsinoma nasofaring, radioterapi

Karsinoma nasofaring (KNF) adalah tumor ganas yang tumbuh di daerah nasofaring dengan predileksi pada fossa Rossenmuller dan atap nasofaring. Histopatologi dari biopsi nasofaring merupakan baku emas untuk diagnosis KNF. Biopsi dapat menyebabkan nyeri, risiko perdarahan, dan efek traumatis. Oleh karena itu tindakan biopsi pada anak dan remaja membutuhkan fasilitasi anestesi. Laporan kasus ini bertujuan mendiskusikan remaja 14 tahun dengan KNF yang menjalani biopsi untuk evaluasi hasil radioterapi dan kemoterapi. Manajemen nyeri pasca-biopsi membutuhkan pengkajian, dan pengukuran yang benar. Komunikasi yang baik antara klinisi, orangtua, dan remaja penting dalam membangun kesepakatan bersama untuk keberhasilan pengobatan yang optimal dan pemulihan pasien.

Abstract:
Background: Nasopharyngeal carcinoma (NPC) is a malignancy originated from nasopharyngeal epithelial cells. NPC therapy response could be predicted from histopathological type, but some patients with the same histopathological type, showed a different therapy response. Interleukin (IL)-10 expression is expected to be able to predict a better response of therapy in NPC patients. Purpose: To find out the correlation between IL-10 expression and histopathological type in NPC patients. Method: An analytic observational study with cross sectional approach towards 33 samples taken from the Oncology Polyclinic of Outpatient Unit of Otorhinolaryngology Head and Neck Surgery Department, Dr. Soetomo General Hospital. Formalin-fixed paraffin-embedded biopsy specimens were obtained. The IL-10 expression was studied with immunohistochemistry using rabbit polyclonal antibody Anti IL-10. Assessment of the staining used Allred scale. The Fisher exact test was utilized to determine the correlation of IL-10 expression and histopathological type of NPC, with p value = 0.05. Result: The result of IL-10 expression in NPC patients with histopathological WHO type I NPC obtained 1 sample (8.3%) with strong positive expression and 2 samples (9.5%) with weak positive expression. In patients with histopathological WHO type II NPC obtained 2 samples (16.7%) with strong positive expression and 12 samples (57.1%) with weak positive expression. In patients with histopathological WHO type III NPC obtained 9 samples (75%) with strong expression and 7 samples (33.3%) with weak positive expression. Respon terapi KNF selama ini dapat dinilai berdasarkan tipe histopatologi, namun pada kenyataannya penderita KNF dengan tipe histopatologi sama dapat menunjukkan respon terapi berbeda. Pemeriksaan ekspresi interleukin (IL)-10 diharapkan dapat memberikan prediksi lebih baik mengenai respon terapi pada penderita KNF. Tujuan: Mengetahui hubungan antara ekspresi IL-10 dengan tipe histopatologi pada penderita KNF. Metode: Penelitian observasional analitik dengan pendekatan cross sectional pada 33 sampel yang diperoleh dari Poliklinik Onkologi Unit Rawat Jalan, Departemen THTBedah Kepala Leher, RSUD Dr Soetomo. Ekspresi IL-10 diperiksa dari blok parafin dengan teknik pemulasan imunohistokimia menggunakan rabbit polyclonal antibody Anti IL-10. Ekspresi IL-10 dinilai dengan menggunakan skala Allred. Uji Fisher exact digunakan untuk menentukan korelasi antara ekspresi IL-10 dan tipe histopatologi KNF, dengan p = 0,05. Hasil: Ekspresi IL-10 pada KNF WHO tipe I didapatkan ekspresi positif kuat 1 penderita (8,3%) dan ekspresi positif lemah 2 penderita (9,5%). Hasil ekspresi IL-10 pada KNF WHO tipe II didapatkan ekspresi positif kuat 2 penderita (16,7%) dan ekspresi positif lemah 12 penderita (57,1%). Abstract:
The adsorption isotherms and regeneration study of the removal of Pb(II) ions from water using kenaf-chitosan-alginate (KNF-CHT-ALG) beads were evaluated. The effects of initial Pb(II) ions concentration on the adsorption capacity of KNF-CHT-ALG beads were carried out in a batch study mode and analysed using the Inductively Coupled Plasma (ICP) technique. In the present research, the linear models of Langmuir and Freundlich were used to predict the adsorption isotherms. The adsorption process was excellently well fitted with the Langmuir isotherm model. The maximum adsorption capacity recorded 33.557 mg/g. For the regeneration study, after five times of the recycling process, the KNF-CHT-ALG beads still showed good adsorption towards Pb(II) ions with maximum removal of 95% and regeneration of 98%. Clearly, from the research conducted, the KNF-CHT-ALG beads are found to be a suitable adsorbent for Pb(II) ions removal.

Abstract:
BackgroundIn national and international management guidelines physical therapy (PT) is a recommended treatment for people with inflammatory arthritis (IA). Based on multiple systematic reviews, concluding that supervised exercise therapy and exercise promotion is an effective and safe PT treatment option in patients with IA, recommendations in particular endorse active PT treatment. In order to monitor and enhance the quality of PT in clinical practice, knowledge about its delivery, including its active content, is needed.ObjectivesTo assess the use, frequency, duration and content of PT in patients with IA.MethodsIn this cross-sectional, national study a link to an electronic questionnaire addressing people with rheumatic and musculoskeletal diseases was published between December 2020-July 2021 by the Dutch Arthritis Foundation via their website, newsletter and various social media. It comprised questions on demographic and health characteristics, the EuroQol 5-Dimensions 5-Levels (EQ5D5L) and 29 questions on the usage of PT currently and/or in the past 12 months and, if applicable, the duration, frequency and content of the PT treatment. The content of PT was categorized into exercises (active aerobic, muscle strengthening, range of motion and/or functional exercises), manual treatment, physical modalities and counseling/education. Only data from patients self-identifying as diagnosed with rheumatoid arthritis (RA) and/or axial spondyloarthritis (axSpA) were used for the current study.ResultsA total of 267 people with RA (mean (SD) age 54 (12) years, 96% female) and 104 people with axSpA (age 47 (13) years, 88% female) (10 patients with both diagnoses) completed the questionnaire. More than 90% were treated by a rheumatologist; the mean (SD) EQ5D5L of people with RA was 0.62 (0.2) and of axSpA 0.59 (0.2). 172 RA respondents (64%) reported the use of PT related to their RA (162 individual PT, 1 group PT, 9 both individual and group PT). In axSpA, the rate was 87 (84%; 77 individual PT, 2 group PT, 8 both individual and group PT).Of those reporting individual PT treatment, the duration was long-term (> 3 months) in 134 (89%) and 70 (89%) of the respondents with RA and axSpA respectively, with a duration of more than 2 years in 67 (44%) and 38 (48%). In all users of individual PT, the mean frequency was once per week or less in 116 (77%) of the RA and 63 (80%) of the axSpA patients. Proportions were based on number of respondents to questions on individual PT.Concerning the content of individual PT, and taking into account both short-term and long-term use, for active exercises, the proportion of patients reporting aerobic, muscle strengthening, range of motion or functional exercises ranged from 40-66% in RA and 24-73% in axSpA patients. Regarding manual treatment, the proportions were 61% and 58%, in RA and 73% and 72% in axSpA for massage and passive mobilizations, respectively. With respect to physical modalities (passive), the modality most frequently mentioned were kinesiotaping and dry needling (40 and 25% and 43 and 32% in RA and axSpA). For counseling/education, exercises to perform at home (70 and 78%) and physical activity (PA) promotion (48 and 49%) were most often reported in both RA and AxSpA.ConclusionLong-term use of PT is common in patients with RA and AxSpA. Apart from active treatment modalities (exercises) and promoting recommended PA, passive treatment options appear to be relatively often used.
