AbstractThis December Issue of Cardiology in the Young represents the 12th annual publication generated from the two meetings that compose “HeartWeek in Florida”. “HeartWeek in Florida”, the joint collaborative project sponsored by the Cardiac Center at the Children’s Hospital of Philadelphia, Pennsylvania, together with Johns Hopkins All Children’s Heart Institute of Saint Petersburg, Florida, averages over 1000 attendees every year and is now recognised as one of the major planks of continuing medical and nursing education for those working in the fields of diagnosis and treatment of cardiac disease in the foetus, neonate, infant, child, and adult. “HeartWeek in Florida” combines the International Symposium on Congenital Heart Disease, organised by All Children’s Hospital and Johns Hopkins Medicine and entering its 15th year, with the Annual Postgraduate Course in Pediatric Cardiovascular Disease, organised by The Children’s Hospital of Philadelphia and entering its 18th year. This December, 2014 Issue of Cardiology in the Young features highlights of Johns Hopkins All Children’s Heart Institute’s 14th Annual International Symposium on Congenital Heart Disease, which was held at the Renaissance Vinoy Resort & Golf Club, Saint Petersburg, Florida, from 15–18 February, 2014. This Symposium was co-sponsored by The American Association for Thoracic Surgery (AATS) and had as its special focus “Diseases of the Cardiac Valves from the Fetus to the Adult”. We acknowledge the tremendous contributions made to paediatric and congenital cardiac care by Duke Cameron and Joel Brenner, and therefore we dedicate this December, 2014 HeartWeek Issue of Cardiology in the Young to them. Duke Cameron is Professor of Surgery at Johns Hopkins University and Cardiac Surgeon-in-Charge at The Johns Hopkins Hospital. Joel Brenner is Professor of Pediatrics at Johns Hopkins University and Director of the Taussig Heart Center at Bloomberg Children’s Center, The Johns Hopkins Hospital. Together, Joel and Duke lead the proud paediatric and congenital cardiac programme at The Johns Hopkins Hospital.

Abstract:

Nondestructive evaluation has historically been used almost exclusively for detecting macroscopic defects after materials have been made or put into service. However, the role of NDE is now changing to include in-process control in an effort to increase yields and performance of materials. This includes materials stability during transport, storage, and fabrication, as well as degradation behavior during in-service life. The NDE community has implied that applying NDE in these ways is crucial to solving the economic problems of U.S. manufacturing industries. So called “intelligent manufacturing” is impossible without integrating modern NDE techniques into the production of today's advanced materials.The Johns Hopkins University Center for Nondestructive Evaluation (CNDE) was established in 1984 as an interdisciplinary center for research and instruction, drawing on the resources and talent of the School of Engineering, Applied Physics Laboratory, School of Medicine, and School of Arts and Sciences. Currently 31 faculty or senior staff work with the Center. The techniques being developed are aimed at reliable in-process control of materials and processes. Another important purpose is the education of talented students who will enter the NDE field; 150 students are associated with the Center, a third of whom are graduate students. The CNDE also provides both research collaborators and industrial sponsors with access to all NDE research and instruction at Johns Hopkins University. Twenty-six research institutions have formal cooperative research programs and 19 organizations are corporate sponsors (see Figure 1).

This article discusses the claim of a new paradigm in the knowledge production and diffusion process, and the need to assess the regional and local implications of this modal shift. After introductory remarks included in the first part of the paper, its next section introduces the theme of localisation of knowledge as a source of regional development; section three examines the lessons we can extract from the US university system (with a particular regard to the case of Johns Hopkins University and the recent project for a biotech park in the city of Baltimore); in section four an illustration of the Italian University system leads to a description of the current evolution of the University of Bologna toward a new entrepreneurial role. The last part of the paper discusses the embedded role of universities in the light of the two cases presented in the previous sections and draws the conclusions in terms of regional policy.

AbstractFounded in 1986, an educational joint venture between Johns Hopkins University and Nanjing University has survived the trials of culture, geopolitics, and nature. As it entered its third decade, the Hopkins-Nanjing Center for Chinese and American Studies offered the first ever Sino-U. S. joint master's degree. This article traces the evolution of this bi-national institution, exploring its role in international relations, cultural exchange, economic globalization, and China's postMao drive toward modernization. A risk-taking Chinese university president, his American counterpart, a Chinese-American scientist who served as cultural mediator, the Tiananmen crisis, the SARS epidemic, Chinese and American students seeking to advance careers and mutual understanding–all these factors play a role in the story. Though continuous in many ways with early twentieth-century exchanges, the Hopkins-Nanjing Center diverges from the former pattern by being an equal partnership. This difference reflects China's growing economic and political power as well as a different American sensibility. In addition to reflecting the state of bilateral relations, the Center has contributed to relations through the symbolism of its survival and hopes, over the long term, to contribute by training future leaders in the government, business, and civil society sectors of the two societies.

As of August 20, 2021, the Center for COVID-19 Tracking at Johns Hopkins University reports more than 200 million cases and more than 4 million deaths.1 However, another of the great unknowns are the sequelae left by the acute phase of COVID-19 and the impact they have on health systems through parameters of health costs, morbidity, mortality and disability.2 The period of time when the sequelae are observed has been termed post-COVID-19 syndrome. Is defined as the persistence or appearance of signs and/or symptoms after the acute phase of the disease, specifically in two time periods, between 4 and 12 weeks and after week 12, and potentially compromising the functional capacity of the individual.2 This syndrome can occur in young patients as well as in patients with advanced disease, and in those with or without a history of comorbidities.2 One of the greatest challenges today is the implementation of strategies for early diagnosis and timely management of major complications presented in this period of time, especially in low- and middle-income countries where resources for health systems are scarce and where there is a very high overall disease burden.

Coronavirus 2019 (COVID-19) is currently a global pandemic. Public awareness about washing hands is important in preventing the spread of COVID-19. The purpose of this study was to determine the relationship between increased water sources, basic water services, improved sanitation facilities, basic hand washing facilities, soap and water availability with COVID-19 transmission in 44 countries. Using Demographic and Health Survey Data in 44 countries including data on populations living in households using improved water sources, basic water services, improved sanitation facilities, hand washing practices, basic hand washing facilities with soap and water availability, ownership of electricity, television, cellphones , and households consisting of only those aged> 65 years. Data is downloaded in the form of Excel. The COVID-19 case was reported by the Johns Hopkins University Center for Science and Systems Engineering (CSSE) on April 28, 2020 in 44 countries. DHS program data from 44 countries that are relevant to the CSSE Johns Hopkins University data are carried out linear regression tests and alternatives (spearman test) using SPSS 22. Relationships between variables by looking at the correlation coefficient and p value for each variable. The results of the study are presented in tabular form. Results: Increased water sources, basic water services, improved sanitation facilities, basic hand washing facilities, soap and water available in relation to COVID-19 transmission, with each correlation coefficient of 0.421, 0.506, 0.374, 0.243 and 0.399.

In both academic literature and the media, financial conflicts of interest in human subjects research have come center-stage. The cover of a recent edition of Time magazine features a research subject in a cage with the caption human guinea pigs, signifying perhaps that human research subjects are no more protected from research abuses than are laboratory animals. That magazine issue highlights three well-publicized cases of human subjects research violations that occurred at the University of Oklahoma, the University of Pennsylvania, and Johns Hopkins University.At St. John Medical Center in Tulsa, Oklahoma, a study that was co-sponsored by the University of Oklahoma Health Sciences Center investigated an experimental vaccine for malignant melanoma. In that case, the chair of the university's institutional review board (IRB) — the committee within each medical institution charged with ethics review of human research projects undertaken at that institution — and the dean of the University's College of Medicine allegedly concealed from both the IRB and the United States Food and Drug Administration (FDA) a report by an outside consulting firm that had found severe deficiencies with the melanoma vaccine study being conducted at the medical center.

A análise desses documentos auxiliou a compreender os conflitos, negociações e impasses na seleção dos bolsistas enviados aos Estados Unidos, a formação na universidade norte-americana e o seu retorno ao Brasil, entre 1919 e 1924. Trajetórias de formação profissional internacional também foram marcadas por questões regionais e locais, e por trocas de conhecimentos sobre saúde pública.

Scholars within the fields of public health, health education, health promotion, and health communication look to specific theories to explain health behavior change. The purpose of this article is to critically compare four health theories and key variables within them with regard to behavior change in the area of reproductive health. Using cross-country analyses of Ghana, Nepal, and Nicaragua (data sets provided by the Center for Communication Programs, Johns Hopkins University), the authors looked at the Health Belief Model, Theory of Reasoned Action, Extended Parallel Process Model, and Social Cognitive Theory for these two defined objectives. Results show that all four theories provide an excellent fit to the data, but that certain variables within them may have particular value for understanding specific aspects of behavior change. Recommendations for the selection of theories to use as guidelines in the design and evaluation of reproductive health programs are provided.

Abstract Sherri Rose, Ph.D. is an associate professor at Stanford University in the Center for Health Policy and Center for Primary Care and Outcomes Research as well as Co-Director of the joint Harvard–Stanford Health Policy Data Science Lab. A renowned expert in machine learning methodology for causal inference and prediction, her applied work has focused on risk adjustment, algorithmic fairness, health program evaluation, and comparative effectiveness research. Dr. Rose’s leadership positions include current roles as Co-Editor of Biostatistics and Chair of the American Statistical Association’s Biometrics Section. She is also a Fellow of the American Statistical Association. Dr. Rose earned a BS in Statistics from The George Washington University and a PhD in Biostatistics from the University of California, Berkeley before completing an NSF Mathematical Sciences Postdoctoral Research Fellowship at Johns Hopkins University. Prior to joining the faculty at Stanford University, she was on the faculty at Harvard Medical School in the Department of Health Care Policy. AbstractThe hybrid remotely operated vehicle Nereus, developed by the Woods Hole Oceanographic Institution in collaboration with the Space and Naval Warfare Systems Center Pacific and Johns Hopkins University, is designed to provide a new level of access to a maximum depth of 11,000 m. Nereus operates in two different modes. The vehicle can operate untethered as an autonomous underwater vehicle for broad area survey, capable of exploring and mapping the seafloor with sonars, cameras, and other onboard sensors. Nereus can be converted at sea to become a remotely operated vehicle (ROV) to enable close-up imaging and sampling. The ROV configuration incorporates a lightweight fiber-optic tether to the surface for high-bandwidth real-time video and data telemetry to the surface to enable high-quality teleoperation, additional cameras and lights, manipulator arm, and sampling gear. Nereus underwent sea trials in May and June of 2009 during which it completed eight dives, including two dives to more than 10,900 m in the Challenger Deep of the Mariana Trench with a total bottom time in excess of 12 h.

Introduction The use of corticosteroids in systemic lupus erythematosus (SLE) patients requires difficult trade-offs between efficacy and risk of toxicity. This qualitative study examined SLE patients' most desired outcomes and their concerns with corticosteroid use in SLE treatment. Methods SLE patients with current/past experience with using corticosteroids were recruited from the clinics at the Johns Hopkins Lupus Center and the University of Maryland Medical Center. Five in-depth interviews ( N = 5) and four focus groups ( N = 15) were conducted during which discussions were transcribed and analyzed based on a grounded theory approach. Results We identified five major themes describing SLE patients' most desired outcomes: reduction in flares, maintenance of normal activities, minimization of treatment side effects, prevention of future organ damage, and finding a cure. Further, SLE patients reported these primary concerns with the adverse effects of corticosteroids: weight gain, organ damage (particularly bone-related damage), mood swings/irritability, sleep disturbances, and dental issues. Patients appeared to be more concerned with adverse effects that immediately affected their day-to-day lives. Conclusion Knowledge gained during this study better informs how patients view the benefits and risks of corticosteroids. This can facilitate discussions between physicians and patients as they work together to determine the appropriate use of corticosteroids.

AbstractIntroductionCommunity disaster preparedness plans, particularly those with content that would mitigate the effects of psychological trauma on vulnerable rural populations, are often nonexistent or underdeveloped. The purpose of the study was to develop and evaluate a model of disaster mental health preparedness planning involving a partnership among three, key stakeholders in the public health system.MethodsA one-group, post-test, quasi-experimental design was used to assess outcomes as a function of an intervention designated Guided Preparedness Planning (GPP). The setting was the eastern-, northern-, and mid-shore region of the state of Maryland. Partner participants were four local health departments (LHDs), 100 faith-based organizations (FBOs), and one academic health center (AHC)—the latter, collaborating entities of the Johns Hopkins University and the Johns Hopkins Health System. Individual participants were 178 community residents recruited from counties of the above-referenced geographic area. Effectiveness of GPP was based on post-intervention assessments of trainee knowledge, skills, and attitudes supportive of community disaster mental health planning. Inferences about the practicability (feasibility) of the model were drawn from pre-defined criteria for partner readiness, willingness, and ability to participate in the project. Additional aims of the study were to determine if LHD leaders would be willing and able to generate post-project strategies to perpetuate project-initiated government/faith planning alliances (sustainability), and to develop portable methods and materials to enhance model application and impact in other health jurisdictions (scalability).ResultsThe majority (95%) of the 178 lay citizens receiving the GPP intervention and submitting complete evaluations reported that planning-supportive objectives had been achieved. Background A new ARCHITECT® alpha fetoprotein (AFP) assay was developed to improve the linearity at the upper end of the calibration curve and to enhance other performance characteristics. In addition, this reformulation eliminated the possibility of falsely depressed samples at high AFP concentrations. The purpose of this study was to evaluate its analytical performance at multiple sites. Methods The assay configuration, the diluent formulation, and the manufacturing process were redesigned. Analytical performance was evaluated at Abbott Laboratories, Sapporo Medical University, VU University Medical Center, and Johns Hopkins University. Results The limit of quantitation of the assay was 1.00–1.30 ng/mL. Total precision (%CV) across the assay range varied between 1.41 and 3.52. The assay was linear from 1.19 to 2535 ng/mL, and the range of the assay was expanded from 200 ng/mL to 2000 ng/mL. Comparison of this assay with the on-market ARCHITECT, AxSYM, ADVIA Centaur, DxI, AIA-1800, and E 170 systems yielded regression slopes of 0.91–1.08 and correlation coefficients of ≥0.99 for serum samples. No falsely depressed results were observed in 174 serum samples with AFP concentrations of 2018–1,196,856 ng/mL and in a spiked sample containing up to 10 mg/mL of purified AFP. Conclusions The new AFP assay has improved an issue of the on-market ARCHITECT AFP assay and demonstrated excellent assay performance.

Abstract:

Background : One of the challenges in handling Covid-19 is that there are people who are against government policies. Some of these people still don't believe in the Covid-19 virus, distrust the information and reality about Covid-19 that the government provides, and not complying with government policies, including the implementation of health protocols. According to data from the Center for Systems Science and Engineering (CSSE) at Johns Hopkins University (JHU), Indonesia is in the top 20 list in the world. To be precise, it is in the 18th position, with the number of cases reaching 1,837,126 (as of June 24, 2021), and the death toll reaching 51,059 people. Methods: The research design is a descriptive quantitative survey with a cross sectional approach and the sampling technique used is accidental sampling and a sample of 71 respondents, which was then performed a chi-square statistical test, with a confidence degree of 95%. Results: The results of the statistical test show that there is a relationship between communication (p value 0.003), attitude (p value 0.001), and attitude of the implementer (p value 0.004) with a decrease in the number of cases of covid-19.

ABSTRACT Objective: To describe characteristics of hippocampal sclerosis dementia. Methods: Convenience sample of Hippocampal sclerosis dementia (HSD) recruited from the Johns Hopkins University Brain Resource Center. Twenty-four cases with post-mortem pathological diagnosis of hippocampal sclerosis dementia were reviewed for clinical characterization. Results: The cases showed atrophy and neuronal loss localized to the hippocampus, amygdala and entorrhinal cortex. The majority (79.2%) had amnesia at illness onset, and many (54.2%) showed abnormal conduct and psychiatric disorder. Nearly 42% presented with an amnesic state, and 37.5% presented with amnesia plus abnormal conduct and psychiatric disorder. All eventually developed a behavioral or psychiatric disorder. Disorientation, executive dysfunction, aphasia, agnosia and apraxia were uncommon at onset. Alzheimer disease (AD) was the initial clinical diagnosis in 89% and the final clinical diagnosis in 75%. Diagnosis of frontotemporal dementia (FTD) was uncommon (seen in 8%). Conclusion: HSD shows pathological characteristics of FTD and clinical features that mimic AD and overlap with FTD. The findings, placed in the context of earlier work, support the proposition that HSD belongs to the FTD family, where it may be identified as an amnesic variant.

Abstract:

Yitzhak Rabin left a complex and paradoxical legacy. The man who for decades embodied a national security policy based on forceful deterrence adopted, in the 1990s, a rhetoric of coexistence and cooperation. Rabin began to articulate positions identified with the Israeli left: that the Palestinians were at the heart of the Israeli–Arab conflict, and that the conflict was at least partly a product of “psychological walls” on both sides. Despite his traditionally hawkish views and staunch opposition to recognizing Yasir Arafat and the PLO, Rabin oversaw a reversal of policy toward Israel's former arch-enemy. Indeed, it was Rabin's stature as “Mr. Security” that made the Israeli–PLO Declaration of Principles possible. Unlike Shimon Peres, his rival for Labor Party leadership, Rabin's unrivaled role as an architect of national security policy and practice afforded him relative immunity to charges of excessive dovishness. Efraim Inbar, professor of politics at Bar-Ilan University, offers a lucid account of Rabin's contribution to Israeli security. Concluding that Rabin's approach to security was pragmatic, ad hoc, and without an overarching strategic vision (p. 169), Inbar nevertheless begins from the premise that for Rabin and other leaders of the Israeli Defense Force (IDF), the anarchic environment of Middle Eastern politics conditions Israel's security needs. In this environment, governments consider international law and treaties unreliable, so “self-help” and deterrence are the sole path to security. For Rabin, “What has assured Israel's existence…is primarily Israel's comprehensive power, with military might as the decisive element” (p. 11).

Abstract:

The use of opioid analgesics for pain management has increased dramatically over the past decade, with corresponding increases in negative sequelae including overdose and death. Physicians, policymakers, and researchers are focused on finding ways to decrease opioid use and overdose. This crisis calls for a coordinated response that includes the entire healthcare sector. In this work, the authors lay out a blueprint for such a response at the level of the academic medical center. The proposed model is a comprehensive opioid overdose prevention, response, and education program to evaluate, monitor, and address prescription opioid-related adverse events and addiction among all patients within a healthcare system. The approach includes three inter-related elements: (1) creation of an organizational structure that is subdivided into subcommittees to facilitate cross-functional collaboration and implementation. These subcommittees will focus on Research and Design, Implementation, Advisory, and Compliance with the recommendation. (2) Development of an effective communication plan throughout the institution to enable the organization to function seamlessly and efficiently as a single unit, (3) development of a data tracking and reporting system that intended to have a 360° view of all aspects of opioid prescription and downstream patient outcomes. The most effective response system will require an organizational structure that facilitates the ad hoc constitution of cross-functional teams with members drawn from all levels of the organizational hierarchy (executive leadership to frontline staff). Such a structure provides the teams with immediate solutions as developed by the frontline staff and authority to remove institutional barriers that may delay or limit the successful implementation. The model described was developed in our institution by a cross-functional team that included members from the Johns Hopkins School of Medicine and Johns Hopkins University Carey Business School, Department of Operations Management. The multidisciplinary nature of collaboration allowed us to develop a model for an immediate institution-wide response to the opioid crisis, and one that other healthcare organizations could adopt with local modification as a template for execution. The model also meant to serve as a template for an institutional rapid-response that can be seamlessly implemented during any future drug-related crisis or epidemic.
