It is now well accepted that communication between the health professional and the patient is a critical component of quality patient care, and that poor communication can adversely affect both patient and health professional outcomes. However, audits of doctor and nurse communication with patients have consistently revealed deficits, prompting the growth of communication skills training for both junior and senior clinicians, and the publication of communication guidelines for various challenging situations. Interaction analysis systems (IAS) enable the analysis of communication between the doctor, patient, family, and other health professionals in a qualitative and quantitative fashion. They are used as descriptive and outcome measures in research into medical communication, as well as to provide feedback to individual clinicians on their behaviour. Two types of IAS can be identified: ‘content’ systems, which describe task-oriented behaviour; and ‘process’ systems, which measure socio-emotional behaviour. This chapter describes and compares a variety of IAS.

The National Institutes of Health, the Agency for Healthcare Research and Quality, the CDC, and a number of private foundations have expressed the need for advancing the science of dissemination and implementation. Interest in dissemination and implementation research is present in many countries. Improving health care requires not only effective programs and interventions, but also effective strategies to move them into community based settings of care. But before discrete strategies can be tested for effectiveness, comparative effectiveness, or cost effectiveness, context and outcome constructs must be identified and defined in such a way that enables their manipulation and measurement. Measurement is underdeveloped, with few psychometrically strong measures and very little attention paid to their pragmatic nature. A variety of tools are needed to capture health care access and quality, and no measurement issues are more pressing than those for dissemination and implementation science.

The fair and equitable treatment (FET) standard is an ‘absolute’, or ‘non-contingent’, standard of treatment, i.e. a standard that states the treatment to be accorded in terms that have their own normative content, as contrasted with the ‘relative’ standards embodied in the ‘national treatment’ and ‘most-favoured-nation’ principles, which define the required treatment by reference to the treatment accorded to other investments in similar circumstances. The FET is the most often invoked treaty standard in investor-state arbitration, present in almost every single claim brought by foreign investors against host States. This chapter tackles the FET standard from two angles: its position in the international law context and the elements identified by arbitral tribunals as forming part of this standard (and their balance).

The artefacts and activities identified and discussed during the tour of the Hospice, and indeed the basic assumptions and espoused values that underlie them (as described in previous chapters), have provided for a rich and varied account of the culture within the organization. Whilst there are elements of discord and ambiguity in the accounts given so far, the theme of patient-centred care and compassion seems largely to act as a binding mechanism to uphold a generally cohesive culture in the Hospice. In many ways this mirrors Schein’s view of culture as the ‘glue’ which binds the organization together. However, in line with Meyerson and Martin’s well-known contention that culture can only be fully described if elements of ‘ambiguity’ and ‘differentiation’ are included, as well as those ‘integrative’ forces that are agreed upon, this chapter focuses on issues of discord and friction identified during the study.

This chapter reviews the challenges in providing safe anaesthesia outside of the operating theatre complex and the particular risks this may pose to the elderly. Generic issues with remote sites are discussed, and key points are identified: these include patient assessment, appropriate staffing and support, safety drills, environmental safety, and anaesthetic equipment and drug provision. Sedation in the elderly is identified as an area of risk, and the chapter details the problems of providing safe sedation. The areas where sedation is administered vary from endoscopy and imaging including CT and MRI, to interventional suites such those used for cardiothoracic catheterization and GI stenting. Major advances in technology have allowed the development of endovascular aortic aneurysm repair (EVAR) and transthoracic aortic valve implantation (TAVI). Psychiatric units still prescribe electroconvulsive therapy (ECT) for severe suicidal depression, especially in the elderly because of the fast response, and the related challenges for anaesthetists are reviewed.

It is assumed that employees display favorable attitudes (e.g., high organizational commitment) and behaviors (e.g., high work effort) when identified as organizational talent. If they did not, the idea that talent management creates value by making disproportionate investments into organizational talent would need to be reconsidered. We reviewed the literature to explore whether the assumed favorable reactions among talent are valid and the results are not straightforward. Many studies found evidence for the assumption; however, several studies revealed that talent designation bears considerable risks: Being identified as talent creates (overly optimistic) expectations of receiving rewards and benefits from the organization and it increases the felt pressure to meet high performance standards. We discuss the findings in the light of social exchange theory, psychological contract theory, and others commonly used in talent-management research, highlighting key issues regarding talent designation and identifying avenues for future research.

This text presents a study on corruption in international investment arbitration. The book considers the limited effectiveness of efforts to combat transnational corruption in international law and the emergence of international investment arbitration as a singular means for effective control of corruption within the international legal order. The case law on corruption by investment tribunals is studied exhaustively, jurisprudential trends are identified, and reforms aimed at enhancing the effectiveness and fairness of investment arbitration as a mechanism to combat corruption are proposed. The book is divided into three parts. Part I focuses on the phenomenon of corruption in foreign investment and attempts at its control through international law. Part II analyses the available case law in international investment arbitration dealing with corruption. The text identifies nine distinct trends emerging from the case law and provides a table summarizing the key areas of corruption decision-making and each relevant tribunal's approach, which is an invaluable tool for practitioners engaging in ‘live’ issues of corruption within arbitral proceedings. It became international later in the century, and problems surfaced almost immediately as skaters trained in their own national styles competed against skaters trained in other styles. The chapter then sets out the book's focus, namely World and Olympic championship skaters from 1896 through 2002. Their successes and their failures are identified; their contributions are celebrated. The narrative through the postwar period progresses generally by decades. It emphasizes major issues, some short-lived but others extending over longer periods, that are identified with the skaters most directly affected. General concerns of the International Skating Union are addressed throughout.

The figure of a woman reclining, in repose, displayed, abandoned, fallen, asleep, or dreaming, returns in the work of women filmmakers and photographers in the twentieth and twenty-first centuries. Filmmakers Agnès Varda and Catherine Breillat, and American photographer working in Paris, Nan Goldin, return to the paintings of Titian, Velázquez, Goya, Courbet, and others, re-imagining, and re-purposing, their images of female beauty, display, (auto)eroticism, and intimacy. This book, a sensuous evocation of these feminist works, claims a female-identified pleasure in looking. The artists explored align images of repose and sensuality with other images of horizontality and proneness, of strong emotional content, images of erotic involvement, of vulnerability, of bodily contortion, of listlessness, grief, and depression. The reclining nude is for all three artists a starting point for a reflection on the relation of film, projections, and still photography, to painting, and a sustained re-imagining of the meanings conjured through serial returns to a particular pose. This book claims that the image of the reclining nude is compelling, for female-identified artists – and for all allied in feeling and picturing femininity – in the sensitive, ethically adventurous, politically complex feminist issues it engages. The reclining nude is an image of passivity, of submission, of hedonism. It allows thought about passivity as pleasure, about depression and grief figured posturally, about indolence as a form of resistance and anarchy. Through this image, female-identified artists have claimed freedom to offer new focus on these extremes of emotion. They are re-imagining horizontality.

Understanding each individual patient and their goals for sexual health can lead to identifying and addressing sexual issues effectively. Information is provided about some of the traditional medical treatment options typically used to treat men with common sexual problems. In addition, some lifestyle modification and psychological approaches to treating sexual dysfunction are discussed.

The chapter provides an analysis of teamwork processes involved in the “reimagining” ethnographies and their impact on procedural ethics (formal ethics reviews), practice ethics (issues emerging while conducting the research), project ethics (issues related to the international, interdisciplinary, and collaborative project design). Processes identified include building consensus through meetings of many kinds, problem-solving consultation, team reflexivity and the use of discretion when democracy was not possible. Situating these processes within the frame of critical feminist research, the chapter makes links between these ethical processes and research that aims to create change.

The present project of grounding morality is distinguished from much recent work in metaethics. The argumentative framework of non-eliminative naturalism is also presented.
