Abstract:
Purpose Autologous hematopoietic cell transplantation, or autotransplantation, is effective in light-chain amyloidosis (AL), but it is associated with a high risk of early mortality (EM). In a multicenter randomized comparison against oral chemotherapy, autotransplantation was associated with 24% EM. We analyzed trends in outcomes after autologous hematopoietic cell transplantation for AL in North America. Patients and Methods Between 1995 and 2012, 1,536 patients with AL who underwent autotransplantation at 134 centers were identified in the Center for International Blood and Marrow Transplant Research database. EM and overall survival (OS) were analyzed in three time cohorts: 1995 to 2000 (n = 140), 2001 to 2006 (n = 596), and 2007 to 2012 (n = 800). Hematologic and renal responses and factors associated with EM, relapse and/or progression, progression-free survival and OS were analyzed in more recent subgroups from 2001 to 2006 (n = 197) and from 2007 to 2012 (n = 157). Results Mortality at 30 and 100 days progressively declined over successive time periods from 11% and 20%, respectively, in 1995 to 2000 to 5% and 11%, respectively, in 2001 to 2006, and to 3% and 5%, respectively, in 2007 to 2012. Correspondingly, 5-year OS improved from 55% in 1995 to 2000 to 61% in 2001 to 2006 and to 77% in 2007 to 2012. Hematologic response to transplantation improved in the latest cohort. Renal response rate was 32%. Centers performing more than four AL transplantations per year had superior survival outcomes. In the multivariable analysis, cardiac AL was associated with high EM and inferior progression-free survival and OS. Autotransplantation in 2007 to 2012 and use of higher dosages of melphalan were associated with a lowered relapse risk. A Karnofsky score less than 80 and creatinine levels 2 mg/m2 or greater were associated with worsened OS. Conclusion Post-transplantation survival in AL has improved, with a dramatic reduction in early post-transplantation mortality and excellent 5-year survival. The risk-benefit ratio for autotransplantation has changed, and randomized comparison with nontransplantation approaches is again warranted.

Abstract:
In 1997, a forty-three years old woman was diagnosed with CML and treated with alfa-Interferon, achieving complete haematological response (CHR). Three years later, patient was switched to hydroxiurea due to thyroid toxicity. For logistic reasons, therapy with imatinib 400 mg/die was initiated only in 2003, obtaining complete cytogenetic response (CCyR) and suboptimal molecular response in twelve months. CCyR and CHR were then lost three years later. Doubling imatinib dose to 800 mg/die gave no positive results. Mutational analysis performed in September 2007 showed F317L point mutation of the Bcr-Abl kinase domain. In October 2007 dasatinib was started and in April 2008 CCyR was reached with suboptimal molecular response. In March 2009 Bcr-Abl transcript progressively increased, and in August 2009 cytogenetic analysis showed loss of CCyR. Therapy with nilotinib 800 mg/die was started, and in October 2009 the patient obtained complete molecular response (CMR). Bcr-Abl kinase-domain point mutations, acquired during first line therapy, are a common cause of resistance to tyrosine kinase inhibitors. While several Bcr-Abl mutations have been identified, involvement of codon 317 has been reported in the literature following treatment with imatinib and dasatinib.

Abstract: Plastic Section Modulus of a section is the first moment of the cross sectional area about an axis which divides the sectional area into two equal halves. In this paper an attempt has been made to calculate and verify the Zpz values of tapered flange I and C Sections. I-Section has been divided into a total of 13 component areas and C-Section into 7 component areas. The area of each component is calculated and the position of centroid of each component area is identified and used in the calculation of the Plastic Section Modulus of the cross section about Z-Z axis. Eleven I-sections and One C-section Zpz values are found to differ from the values given in the IS 800 : 2007[2] by more than 100 mm3 , i.e., 0.1 cm3 and are reported here. These verifications gain significance due to the fact that, in case of four I-sections, the difference is found to be more than 10000 mm3 , i.e., 10cm3

Abstract:
Here we report a case of a man affected by metastatic GIST since march 2007. His oncologic history began in 2006 when he was submitted to ileal resection and diagnosis of abdominal GIST was made. He was free of disease at radiologic controls until march 2007, when peritoneal nodules were shown. He started imatinib as first line therapy at standard dose of 400 mg/day and he reached a partial response at 6 months according to Response Evaluation Criteria In Solid Tumors (RECIST). After 24 months he performed CT scan that showed progression disease. For this reason he increased imatinib dose to 800 mg/day. Imatinib was safely administered and radiologic analysis performed at 3 and 6 months demonstrated stable disease according to RECIST. The patient is continuing oral therapy with imatinib at 800 mg/day and the disease is still stable.

<p>Σκοπός της παρούσας εργασίας είναι η διερεύνηση των δαπανών για την δευτεροβάθμια εκπαίδευση στις αρχές του 21<sup>ου</sup> αιώνα των χωρών που ανήκουν στην Ευρωζώνη, μέσω της επισκόπησης ευρημάτων εμπειρικών μελετών. Ειδικότερα, πραγματοποιούνται διακρατικές συγκρίσεις και εξετάζεται η θέση της Ελλάδας σε σχέση με τις υπόλοιπες χώρες της Ευρωζώνης. Από τα ευρήματα της έρευνας προέκυψαν οι παρακάτω γενικές διαπιστώσεις – συμπεράσματα: Το 2005 η Ελλάδα βρίσκεται στις τελευταίες θέσεις κατάταξης ανάμεσα στις χώρες τις Ευρωζώνης όσον αφορά στις δημόσιες δαπάνες για δευτεροβάθμια εκπαίδευση ως ποσοστό του Α.Ε.Π. (Ελλάδα: 1,4%, μ.ο. Ευρωζώνης: 2,2%), ενώ τις πρώτες θέσεις καταλαμβάνουν η Γαλλία (2,7%), το Βέλγιο (2,6%) και η Αυστρία (2,5%). Το 2007, η Ελλάδα βρίσκεται επίσης στις τελευταίες θέσεις κατάταξης ανάμεσα στις χώρες της Ευρωζώνης σε ότι αφορά τις δημόσιες δαπάνες για την δευτεροβάθμια εκπαίδευση ως ποσοστό των συνολικών δημόσιων εκπαιδευτικών δαπανών (Ελλάδα: 38%, μ.ο. Ευρωζώνης: 42,9%), ενώ στις πρώτες θέσεις βρίσκονται η Γερμανία (47,1%), η Αυστρία (46,3%) και η Γαλλία (45,3%). Τέλος, το 2005, η Ελλάδα βρίσκεται στη πρώτη θέση κατάταξης ανάμεσα στις χώρες της Ευρωζώνης όσον αφορά στις δαπάνες ανά μαθητή για την δευτεροβάθμια εκπαίδευση ως ποσοστό του κατά κεφαλήν Α.Ε.Π. (από δημόσιες και ιδιωτικές πηγές χρηματοδότησης) με 33% και ακολουθούν η Πορτογαλία με 32% και η Γαλλία με 30%, ενώ ο μέσος όρος της Ευρωζώνης αγγίζει το 27%.</p>

β-phase (Bi2O3)1-x(Dy2O3)xsystem with tetragonal structure is synthesized for0.01<x<0.10molar doping. Unit cell parameters increased with increasing the doping. We have studied the dependence of total electrical conductvity on temperature, doping concentration ofβ-phase systems. The phase transition which manifests itself by the jump in the conductivity curve was also verified by DTA and both measurements are rather compatible. The electrical conductivity curves ofβ-phase structure revealed regular increase in the form of an Arrhenius curve. The activation energies are calculated from these graphs.Bi2O3-basedDy2O3doped ceramics show ionic oxygen conductivity. The conductivity increased as the doping concentration increased. The highest value of conductivity is 0.006 0.006ohm-1cm-1(600∘C)for theβ-phase (Bi2O3)0.91(Dy2O3)0.09(800∘C). The sample with the highest conductivity is (Bi2O3)0.91(Dy2O3)0.09(800∘C)binary system where 1.450 ohm−1cm−1(745∘C).

AbstractStakes at 2 km intervals were installed in January 1997 and remeasured in February 1998, January 1999, January 2005 and during the 2007/08 austral summer along a 1248 km traverse route from Zhongshan station to Dome A, East Antarctica. Based on topographical parameters, meteorological features and the records of ∼650 stakes and six stake arrays, the route is divided into five zones. We find that the snow accumulation rate decreases with increasing altitude as one progresses inland, except in the zone 800–1128 km from the coast, where the average annual accumulation rate is higher than in the zone 524–800 km from the coast. The Dome A zone (1128–1248 km) has the lowest accumulation rate (35 kg m−2 a−1, 2005–08) due to having the highest elevation and being furthest from the coast. The surface mass balance in the region 202–1128 km from the coast exhibits no temporal change from 1999–2005 to 2005–08, but there is a change in the accumulation distribution. The zone from 202 to 524 km shows a decrease in surface mass balance from 84 kg m−2 a−1 in 1999–2005 to 67 kg m−2 a−1 in 2005–08, while the zone between 800 and 1128 km shows an increase from 67 kg m−2 a−1 in 1999–2005 to 75 kg m−2 a−1 in 2005–08.

Abstract: Conventional steel frames are low rise steel frames with roofing systems of truss and roof coverings. Standard hot rolled sections are used for truss elements which are usually much heavier than what is actually required as per the design. Pre– Engineered steel frames are the steel frames in which excess steel is tapered as per the bending moment requirements. In present study, conventional (pratt type) steel roof truss and pre-engineered (portal type) steel roof frame with crane load is considered for an industrial warehouse construction located in Davangere city. 2D Modelling of both conventional and pre-engineered steel frame with crane load is done using STAAD Pro. software. Both the steel frames are subjected to different combinations of Dead load (DL), Live load(LL), Crane Load(CL) and Wind load(WL) as per IS 800 (2007) codal provisions. For the factored bending moments and shear force, referring IS 800 (2007) codal provisions, crane gantry girder is designed as a laterally unsupported member having a combination of I and channel sections. The members of both conventional and pre-engineered steel frames with crane load are designed for the worst load combination as per IS 800(2007) codal provisions. It is concluded that about 25% reduction in quantity of steel can be achieved by choosing pre-engineered steel frame than the conventional steel frame.

يتناول هذا البحث دراسة وصفية تحليلية لمدخلات ومخرجات الشركة العامة للتجهيزات الزراعية للمدة 2004 – 2019 , من اصول ومصروفات وايرادات وارباح معتمدا على البيانات التي تصدرها الشركة من اقسام التخطيط , الحسابات , التسويق , التجارية , الادارية , اضافة الى تقارير ديوان الرقابة المالية, كذلك المقابلات التي اجريت من قبل الباحث مع الجهات المعنية والاشخاص ذوي العلاقة .من خلال النشاط التجاري حققت الشركة العامة للتجهيزات الزراعية ارباحا صافية طيلة مدة الدراسة بلغ مجموعها 614942302104 دينار عراقي (ستمائة واربعة عشر مليار وتسمعائة واثان واربعون مليونا وثلاثمائة واثنان الفا ومائة واربعة دينار عراقي) , كان اعلى هامش ربح عام 2006 حيث بلغ 113045918223 دينار عراقي وباعلى نسبة تطور في الربح مع سنة الاساس بلغ 329,027 % عن سنة الاساس ,فيما عانت الشركة من عجز قدره 10625860280 دينار عراقي عام 2015 وكانت في العام ذاته ادنى نسبة تطور للربح عن سنة الاساس 2004 بمقدار 140,327 - % , اما اعلى هامش لمجمل الربح فقد بلغ اعلى نسبة له في العام 2010 بواقع 59,26% واقل نسبة له كانت في العام 2015 بواقع 8,406 - %, ولو انتقلنا الى العائد على الاصول فقد كانت اعلى نسبة له في العام 2005 بواقع 12,67 % وبمعدل تطور بلغ 341,74 % فيما شهد العام 2015 اسوأ نسبة بواقع 0,84 - % وبمعدل تطور مع سنة الاساس انخفض الى 129,217 % , وبالنسبة للعائد على حقوق الملكية فايضا بلغ اعلى نسبة له في العام 2005 بواقع 105,774 % فيما انخفض الى اقل قيمة في العام 2015 بواقع 11,128 - % .فيما كانت اجمالي ايرادات النشاط الجاري للسنوات مدار البحث 2456970276934 دينار بلغت اعلى قيمة له في العام 2019 بواقع 292319780523 دينار بنسبة تطور عن سنة الاساس بلغت 195,8 % اما ادنى قيمة لتلك الايرادات فكانت في العام 2014 حيث بلغت 78271387483 دينار بسنبة تطور عن سنة الاساس 2004 بلغت 20,8 - % . بلغت اجمالي اصول الشركة اعلى قيمة لها في العام 2018 بواقع 1480395193331 دينار فيما كانت اجمالي الاصول عند ادنى قيمة لها في العام 2005 بواقع 782569009819 دينار .وفيما يخص معدل دوران المخزون فان اعلى معدل شهده العام 2006 بواقع 2,6 قابله اقل متوسط مدة احتفاظ بالمخزون بواقع 144 يوم اما اقل معدل دوران فكان في العام 2018 بواقع 0,45 قابله اعلى متوسط مدة احتفاظ بالمخزون مقداره 800 يوم . اما متوسط فترة التحصيل فقد كان افضل مؤشر له في العام 2006 بواقع 659 يوم اما الاضعف فقد كان في العام 2017 بواقع 1825 يوم . ولو عرجنا على معدل دوران الاصول الثابتة فقد حقق اعلى نسبة له في العام 2005 بواقع 79,757 اما اقل نسبة فقد كانت في العام 2014 بواقع 5,523 . اما معدل دوران اجمالي الاصول فقد كانت اعلى نسبة له في العام 2006 بواقع 0,25 وشهد العام 2014 اقل معدل دوران لتلك الاصول بواقع 0,067 .وعند المرور بمؤشرات نسب السيولة نجد ان الشركة وصلت لاعلى نسبة تداول في العام 2007 بواقع 1,16 فيما كانت النسبة الاقل في العام 2004 بواقع 1,065 , وفيما يخص نسب السيولة السريعة فقد حققت الشركة اعلى نسبة لها في العام 2011 بواقع 1,03 بينما وصلت هذه النسبة لادنى مستوى لها في العام 2016 بواقع 0,782 .واخيرا كانت لنسب المديونية حصة من هذا التحليل حيث بلغت الشركة اعلى نسبة لها في العام 2004 بواقع 0,937 اما افضل مؤشر لها كان في العام 2007 بواقع 0,86 , وبخصوص نسب المديونية الى حقوق الملكية فقد وصلت الى اعلى قيمة لها في العام 2004 ايضا بواقع 14,821 وكذلك حققت اقل قيمة لها في العام 2007 بواقع 6,097 .

Abstract:
This thesis examines the literary uses of biblical typology in the last three decades of the nineteenth century. It aims to show how late Victorian writers, having opted out of the orthodox Christian beliefs of the age, were still writing from within a cultural discourse shaped by, and based upon, such faith. Covering works as diverse as Sartor Resartus, De Profundis, and The Island of Doctor Moreau, and discussing writers who range from Mary Augusta Ward via Hardy to Strindberg and Dostoevsky, my contention is that these writers not only used the structure, terminology, and imagery of biblical typology to express their religious doubts, but that they 'reclaimed' what was strictly seen as a mode of exegesis and transformed it into a richly suggestive signifying system. Through this reconstructed mode of expression, they could offer to their readers ideas of a new 'religion' or, at least, a possible way out of the despair caused by the ultimate failure of Christian faith. The thesis is presented in three parts, the first of which briefly details the various available definitions of biblical typology itself. Following this, each sub-section of Part One traces a different aspect of late Victorian typology usage. Parts Two and Three deal with what I claim to be the two major strains of the late nineteenth century's secular use of typology - those concerned, respectively, with the 'imitation of' or 'association with' biblical types in their relation to literary characters. The changes made to the traditional biblical typology by late Victorian writers, as examined in this thesis, brought the biblical anti-type closer to the Jungian archetype, just as it brought the Nineteenth Century closer to our twentieth-century view of our religious and textual inheritance.

Les sections efficaces differentielles pour la reaction #3he(,pd) a des energies de photons entre 200 et 800 mev ont ete mesurees pour tous les angles de protons dans le centre de masse de 30 a 150. Cette experience utilise le systeme d'etiquetage de photons obtenus par rayonnement de freinage du faisceau d'electrons de 855 mev delivre par l'accelerateur mami a mayence. Les resultats confirment une decroissance rapide des sections efficaces aux angles arrieres tandis qu'aux angles avants on peut observer clairement une bosse grace a notre precision statistique. Cela pourrait etre une signature des mecanismes a trois corps. Une seconde reaction #3he(,ppn) est aussi mesuree afin d'apporter plus d'informations sur ces processus a trois corps. Les resultats d'une analyse preliminaire de cette reaction sont montres

Nosso olhar foi dirigido atrav?s das imagens escritas, faladas e fotografadas para melhor entendimento de como, a comunidade dos Negros do Riacho, foi incorporada a pol?tica de igualdade racial dos Governos Municipal, Estadual e Federal, e transformada em produto publicit?rio de governo. Tra?adas entre uma parte e outra est?o registradas as quest?es da identidade, mas sobretudo sua (des)constru??o, favorecida pela linguagem publicit?ria, que utiliza a riqueza de suas t?cnicas, apresentando o Riacho e sua gente atrav?s da mostra Etno-Fotogr?fica Retratos da Dignidade. Logo, torna-se relevante compreender a divulga??o do Projeto Dignidade, tendo como suporte da propaganda a imagem dos personagens do Riacho divulgada na m?dia local. Fato este, que nos conduz de certa forma, a quest?o da ratifica??o de antigos estere?tipos aos negros atribu?dos, assim como a forma que o Governo do RN no per?odo acima referido se compromete com a problem?tica das comunidades remanescentes de quilombo existente em nosso estado. O Estado ao tomar a comunidade do Riacho para a efetiva??o de sua pol?tica de a??o afirmativa a coloca na condi??o de laborat?rio social (RN, GOVERNO DO ESTADO, 2005). Assim, buscamos atrav?s da an?lise de documentos oficiais, teses, pesquisa bibliogr?fica e de campo, bem como, de material jornal?stico e publicit?rio revelar as mensagens veiculadas na m?dia local acerca da quest?o proposta para este estudo. Para tanto, nos ancoramos metodologicamente na pesquisa de cunho qualitativo, sem perder de vista a an?lise cr?tica dos conte?dos, visto que os sujeitos em estudo n?o devem ser analisados numa via de m?o ?nica, dado sua complexidade. AbstractIn this chapter we review some examples of general Land Use Cover (LUC) mapping at a supra-national level in America and Asia. These datasets provide a general overview of the land uses and covers in specific American or Asian regions, without focusing on any particular land use or cover. For Asia, we have only identified one dataset mapping the Himalayan region, whereas for America five different datasets were identified. Only three of these are reviewed here, as the other two (SERENA, South America 30 m) are not available for download. The most ambitious project of all those reviewed is NALCMS, which coordinates the production of a LUC map for the whole of North America (Canada, Mexico, USA) at detailed scales (30–250 m) and using the same classification legend. It is the only dataset of all those reviewed that provides a time series of LUC maps (2005, 2010 and 2015). The Himalaya Regional Land Cover database is a vector-based map that provides information on LUC changes over the period 1970/80–2007 at a scale of 1:350,000. The other two American datasets—LBA-ECO LC-08 (1 km, 1987/91) and MERISAM2009 (300 m, 2008/10)—are raster-based and only available for one date, therefore making change detection impossible.

Mid year of 2007 Siemens Oil & Gas and Industrial Applications introduced an enhanced SGT-800 gas turbine. The new power rating is 47MW at a 37.5% electrical efficiency in open cycle (ISO). Four components in the gas turbine are modified and are fully interchangeable between the existing 45MW and the 47MW rating. Improved cooling designs were introduced in the turbine stages #1 and #2 requiring no change to the casting design. The only modification is to the machining of the parts and to the guide vane cooling inserts and impingement screens. A re-stagger of the stage #1 compressor blade was made to give a slightly increase in the airflow. The combustor outlet temperature (COT) remains the same. The first SGT-800 with 47MW rating was successfully tested during the autumn of 2006 and the expected performance figures were confirmed. This unit has, up to November 2007, accumulated 8 000 hours and a planned follow up inspection made after 4 000 hours by borescope of the hot section showed that the parts were in excellent condition. This article presents some details of the design work carried out during the development of the enhancement from 45MW to the mature rating of 47MW and the operation experience from the first units.

Introducción: La vigilancia de la mortalidad materna es una herramienta para el monitoreo y toma de decisiones que pueden impactar en su reducción, su análisis es fundamental. Según datos de la OMS cada día mueren en todo el mundo cerca de 830 mujeres por complicaciones relacionadas con el embarazo o el parto, en Colombia según datos oficiales del Departamento Administrativo Nacional de Estadísticas (DANE) la razón de mortalidad materna presenta una tendencia decreciente pasando de 73,3 casos en el 2007 a 51,0 casos por 100 000 nacidos vivos en el 2017. En 2018 y 2019 se presentaron 46,1 y 47,1 / 100 000 nacidos vivos respectivamente. Objetivo: describir el comportamiento de la mortalidad materna durante el 2019, realizando un análisis con el año anterior y relacionando las principales variables socio-demográficas. Metodología: Se realizó un análisis descriptivo retrospectivo de los casos de mortalidad materna notificados al Sistema Nacional de Vigilancia en Salud Pública, donde se incluyó el análisis de las principales variables sociales, demográficas y del sistema de salud. Resultados: Durante el 2019 se notificaron 511 muertes maternas, de las cuales 298 se clasificaron como mortalidades maternas tempranas, se incluyeron 24 casos de nacionalidad extranjera ya que cumplen con la definición de residencia habitual. La tendencia de la razón de mortalidad materna durante los años 2009 a 2019 presento tendencia a la disminución la cual es estadísticamente significativa (P=0,000359). Se presentó aumento en las entidades territoriales de Vichada (p=0,0000), Guainía (p=0,000), Vaupés (p=0,0003), Atlántico (p=0,0070), Barranquilla (p=0,0065) y Cartagena (p=0,0276). La principal causa de muerte es la hemorragia obstétrica seguida del trastorno hipertensivo asociado al embarazo. Conclusiones: La mortalidad materna en Colombia durante los años 2009 a 2019 presenta una tendencia a la disminución; sin embargo, la región atlántica y amazónica muestran tendencia al aumento de casos.
