Abstract:
Myocardial hypertrophy is present in many heart diseases, representing a strong predictor of adverse cardiovascular outcomes. Regarding therapeutic intervention, mesenchymal stem cells (MSCs) have been suggested to significantly reduce cardiac hypertrophy and progression to heart failure. Preconditioning of MSCs was previously demonstrated to highly improve their paracrine activity resulting in modulation of immune responses and the progression of diseases. Here, we studied the effects of bone marrow-derived preconditioned MSCs on hypertrophied induced pluripotent stem cell-derived cardiomyocytes (iPS-CM) and also sought to identify MSC-derived antihypertrophic molecules. Phenylephrine (PE) was used to induce hypertrophy in murine iPS-CM, and markers of hypertrophy were identified by microarray analysis. Murine MSCs were treated with IFN-γ and IL-1β to enhance their paracrine activity, and transcriptional profiling was performed by microarray analysis. Hypertrophied iPS-CM were subsequently cocultured with preconditioned MSCs or MSC-conditioned medium (CM), respectively. Effects on hypertrophied iPS-CM were studied by cell area quantification, real-time PCR, and western blot. In some experiments, cells were incubated with fractions of MSC-CM obtained by ultrafiltration or by MSC-CM supplemented with inhibitory antibodies. Intracellular and extracellular levels of vascular endothelial growth factor (VEGF) were evaluated by western blot and ELISA. PE-induced hypertrophy in iPS-CM was associated with an upregulation of neuron-derived orphan receptor (Nor1) expression, activation of Akt, and inhibition of both strongly prevented hypertrophy induction in iPS-CM. VEGF secreted by preconditioned MSCs provoked hypertrophy regression in iPS-CM, and a negative correlation between Nor1 expression and hypertrophic growth could be evidenced. Our results demonstrate that Nor1 expression strongly supports hypertrophy in iPS-CM. Moreover, the secretome of preconditioned MSCs triggered regression of hypertrophy in iPS-CM in a VEGF-dependent manner. We suggest that the delivery of the MSC-derived secretome may represent a therapeutic strategy to limit cardiac hypertrophy. However, additional in vivo studies are needed to prove this hypothesis.

Abstract Introduction/Objective Inflammatory pseudopolyps (IPs) often develop in response to chronic active inflammation in patients with inflammatory bowel disease (IBD). IPs can be diffuse and numerous in severe IBD. IPs can also occur from other chronic active injuries, such as ulcers, persistent infection, ischemic colitis, anastomotic site and diverticulitis. IPs arising from diverticulitis is uncommon and generally few in numbers. We report a rare case of a patient with segmental diffuse IPs associated with diverticulitis in the absence of IBD. Methods A 59-year-old male patient with a history of sigmoid diverticulitis presented with abdominal pain and was treated with antibiotics. Three months later he had severe recurrent diverticulitis. CT scan showed wall thickening of the sigmoid colon and multiple sigmoid diverticula. Colonoscopy showed extremely severe inflammatory colitis in the area of diverticulitis between 25.0 and 35.0 cm from the anal verge. Biopsies revealed cryptitis, crypt abscesses, crypt architecture distortion and Paneth cell metaplasia. The patient underwent laparoscopic left colectomy. Results The rectosigmoid colon specimen is 18.0 cm in length and 6.0 cm in diameter. There was a 7.5 cm segment with numerous polyps. Multiple diverticula with diverticulitis were also identified in this region. The rest of the colon was unremarkable. Microscopic examination revealed numerous IPs and multiple diverticulitis. No evidence of IBD was identified. Conclusion To our knowledge, this is the first time that segmental and diffuse IPs are described with associated diverticulitis in the absence of IBD. The pathogenesis of IPs in non-inflammatory bowel disease is not well described. Previously, it was reported that diverticulosis and subsequent diverticulitis has no significant impact on the development of IPs, and IPs may arise independent of location and time of diverticulitis. Sporadic IPs may also appear without any underlying pathology. In the current case, there were multiple diverticula and diffuse diverticulitis, which may have contributed to the diffuse IPs. Further clinical inquiries revealed no clinical signs and symptoms of IBD.

Abstract:
Aim: This study aimed at evaluating the impact of different adjustments and artificial aging on the color change (ΔE) and surface roughness (Ra) of monolithic zirconia (MLZ) and lithium disilicate (LD) ceramics. Materials and methods: Forty discs were prepared from two types of MLZs, including ceramill monolithic Zr (CM-Zr) and Prettau PSZ Zr (PP-Zr), and one LD (IPS e.max-LD, IPS-LD). After different clinical adjustments (performed with glazing, bur, bur + glazing, and bur + polishing) and artificial aging (in-vitro coffee immersion), the ΔE was assessed using a spectrophotometer. The roughness (Ra) was also evaluated after these adjustments. The ANOVA and multiple com-parisons test were used to compare the groups statisitcally and the level of significance was set at 5%. Results: In the CM-Zr group, the greatest ΔE was observed after bur adjustment (3.32, p < 0.05) while the lowest ΔE was seen after the glaze treatment (2.11, p < 0.05). In the PP-Zr group, the highest ΔE was observed after bur treatment (2.70, p < 0.05) while the lowest ΔE was seen after the glaze treatment (1.25, p < 0.05). Lastly, for the IPS-LD group, the highest ΔE was again observed after bur treatment (2.91, p < 0.05) while the lowest ΔE was seen after glaze treatment (1.0, p < 0.05). After coffee immersion, it was observed that the highest overall ΔE was observed for CM-Zr after the bur and polish treatment (3.45), while the lowest overall ΔE was observed in IPS-LD after the glaze treatment (2.10). All the ΔE values were within the clinical acceptable range. The highest overall Ra was witnessed for CM-Zr after bur adjustment (1.64), while the lowest overall Ra was seen for IPS-LD after glaze treatment (0.26). Different clinical adjustments and artificial aging influence the colour stability of different ceramic materials (CM-Zr, PP-Zr, and IPS-LD). Compared to the MLZ, IPS-LD ceramics were more stable as they presented with lower ΔE after surface adjustments and coffee immersion. Conclusions: The surface adjustment that minimally affected the colour of these ceramics was glaze treatment, while the ceramics were most affected by bur adjustments. Coffee aging affected colour stability of all ceramic materials. IPS-LD ceramics presented the lowest Ra values after different clinical adjustments.

Abstract:
Introduction: Nowadays, treatment decision making in advanced Hodgkin lymphoma (ad-HL) is based on pre-therapy risk factors and interim PET/CT (PET-2) results, used as a platform for therapy modification. In the past, a bulky mediastinal mass (BMM) was not found to be a significant risk factor and was not included in the final version of the International Prognostic Score (IPS) system (Hasenclever, NEJM 1998). The current study aimed to assess the effect of BMM presence on relapse-free survival (RFS) in patients (pts) with ad-HL and to determine the optimal cutoff of the mass size for outcome prediction in the PET/CT era. Methods: The Israeli Hodgkin Lymphoma Study Group selected to initiate therapy with standard BEACOPP (SB: bleomycin, etoposide, doxorubicin, cyclophosphamide, vincristine, procarbazine, and prednisone) or ABVD (adriamycin, bleomycin, vinblastine, dacarbazine) in HL pts with IPS 0-2 and with intensified therapy using escalated BEACOPP (EB: bleomycin, etoposide, adriamycin, cyclophosphamide, vincristine, procarbazine, prednisone) in pts with IPS ≥3. Treatment was modified according to PET-2 results. To pts with negative PET-2 another 4 cycles of SB/ABVD were administered. PET-2 positive pts received 4 EB cycles followed by radiation therapy (Dann, Blood 2007). In the present study, disease bulkiness, assessed using baseline PET/CT, was retrospectively evaluated in a cohort of HL pts. The data included measurements of the longest diameter of either the biggest single mass or a conglomerate of lymph node masses in the transverse or coronal plane in the PET/CT image. Prognostic values of mediastinal mass cutoffs of ≥5 cm, ≥7 cm, >10 cm for RFS and progression-free survival (PFS) were compared. Results: One hundred and ninety six pts (female/male: 46/54%) with ad-HL [stage IIB - 28, stage III - 74, stage IV - 94; IPS 0-2 - 104; IPS 3-7 - 92; median age 31 years (16-85)] treated at the Rambam Health Care Campus (n=121) and Memorial Sloan Kettering Cancer Center (n=75) between 2000-2016 were included in the analysis. Mediastinal masses ≥5 cm, ≥7 cm and >10 cm were observed in 125 (63%), 82 (42%), 36 (18%) pts, respectively. PET-2 was negative in 79% of pts. At a median follow-up of 66.5 (1-222) months [80 (4-222) for pts without disease progression and 12 (1-62) for those with relapse or progression], estimated 5-year RFS and PFS for the entire group were 82% and 79%, respectively. A mass of up to 7 cm was found in 62 % of pts (n=123); 22% (n=44) had a mass measuring between 7 and 9.9 cm and 15% (n=29) had a mass ≥10 cm. Mediastinal masses were prevalent in 126/196 (64%) pts; in 58 (30%) of them the mediastinal mass, while being biggest, did not exceed 7 cm, and 68 (34%) had BMM of ≥7 cm. We found no effect of bulky disease, either mediastinal or non-mediastinal, on the outcome of the PET-2 positive group. Univariate analysis showed that in pts with negative PET-2, BMM ≥7 cm was a significant adverse prognostic factor for both 5-year RFS and PFS (HR 2.85; 95% CI 1.09-7.41; p=0.032 and HR 2.86; 95% CI 1.1-7.45; p=0.032, respectively). Outcome comparison of PET-2 negative pts with BMM (≥7 cm) to pts with negative PET-2 and either non-mediastinal or mediastinal non-bulky (<7cm) masses (reference group) revealed an inferior 5-year RFS of 73% versus 89% (HR 2.97; 95% CI 1.37-6.42; p=0.006) in the former group. Of note, both groups received an ABVD/SB regimen. HR for RFS was insignificant when a mass size cutoff of ≥5 cm or >10 cm was used. The HR for RFS was particularly high in the comparison between the subgroup of PET-2 negative pts with IPS 0-2 and BMM (≥7 cm) and those with IPS 0-2 in the reference group, demonstrating a 5-year RFS of 59% versus 89% with HR of 4.20 (95% CI 1.59-11.05; p=0.004). PET-2 negative pts with IPS 3-7 and BMM (≥7 cm) did not have an inferior outcome compared to pts with IPS 3-7 from the reference group (5-year RFS of 82% versus 89%). Notably, the two groups were treated with an EB-containing regimen. Our results indicate that BMM is an important adverse prognostic factor predominantly for pts with IPS 0-2. Multivariate Cox regression analysis identified BMM ≥7 cm as the most significant adverse prognostic factor for RFS (Table 1; Fig. 1). Conclusions: In PET-2 negative pts with ad-HL, mediastinal masses ≥7 cm in any plane are associated with the highest risk for HL progression. These findings should be incorporated in a new prognostic scoring system upon more extensive evaluation in a larger cohort. Disclosures No relevant conflicts of interest to declare.

Background Cardiac injury associated with cytokine release frequently occurs in SARS-CoV-2 mediated coronavirus disease (COVID19) and mortality is particularly high in these patients. The mechanistic role of the COVID19 associated cytokine-storm for the concomitant cardiac dysfunction and associated arrhythmias is unclear. Moreover, the role of anti-inflammatory therapy to mitigate cardiac dysfunction remains elusive. Aims and methods We investigated the effects of COVID19-associated inflammatory response on cardiac cellular function as well as its cardiac arrhythmogenic potential in rat and induced pluripotent stem cell derived cardiomyocytes (iPS-CM). In addition, we evaluated the therapeutic potential of the IL-1β antagonist Canakinumab using state of the art in-vitro confocal and ratiometric high-throughput microscopy. Results Isolated rat ventricular cardiomyocytes were exposed to control or COVID19 serum from intensive care unit (ICU) patients with severe ARDS and impaired cardiac function (LVEF 41±5%; 1/3 of patients on veno-venous extracorporeal membrane oxygenation; CK 154±43 U/l). Rat cardiomyocytes showed an early increase of myofilament sensitivity, a decrease of Ca2+ transient amplitudes and altered baseline [Ca2+] upon exposure to patient serum. In addition, we used iPS-CM to explore the long-term effect of patient serum on cardiac electrical and mechanical function. In iPS-CM, spontaneous Ca2+ release events were more likely to occur upon incubation with COVID19 serum and nuclear as well as cytosolic Ca2+ release were altered. Co-incubation with Canakinumab had no effect on pro-arrhythmogenic Ca2+ release or Ca2+ signaling during excitation-contraction coupling, nor significantly influenced cellular automaticity. Conclusion Serum derived from COVID19 patients exerts acute cardio-depressant and chronic pro-arrhythmogenic effects in rat and iPS-derived cardiomyocytes. Canakinumab had no beneficial effect on cellular Ca2+ signaling during excitation-contraction coupling. The presented method utilizing iPS-CM and in-vitro Ca2+ imaging might serve as a novel tool for precision medicine. It allows to investigate cytokine related cardiac dysfunction and pharmacological approaches useful therein.

AbstractPitch canker, caused by Fusarium subglutinans (Wollenw. and Reink.) Nelson, Toussoun, and Marasas, a pine pathogen recently identified in California, may increase the distribution and abundance of Ips spp. on Monterey pines in Santa Cruz Co., CA. In all pine stands, Ips spp. tunneled into healthy branches and boles. Ips mexicanus (Hopkins) was the most abundant engraver beetle observed, followed by I. paraconfusus Lanier; I. plastographus (LeConte) was rare. Ips mexicanus was routinely captured in traps baited with racemic I. paraconfusus pheromone. During die winter, I. mexicanus excavated mass feeding cavities in shade-suppressed branches. In uninfected stands, 3% of the trees and only 0.3% of the branches were mass-attacked. In severely infected stands, 47% of the top one-third of the trees, 3% of all branches, and 91% of the branches declining due to infection by F. subglutinans were mass-attacked by Ips spp. On pitch canker-infected trees, I. mexicanus attacked cones and stems <1 cm in diameter. Sixty percent of the attacks on logs occurred inside or within 5 cm of the area covered with pitch canker-induced resin. When confined with logs, 45% of I. paraconfusus tunneled in or near pitch canker-induced resinous areas,21% near fresh chisel wounds, and 19% near resinous material produced by the Sequoia pitch moth. Abstract:
Novel polysilanes, antimony-substituted polysilanes (APS) and iron-substituted polysilanes (IPS), were synthesized and characterized in this paper. To prepare these new metal-containing polymers, high reactive polymers, polymethylsilane (PMS) and polychloromethylsilane (PCMS) were used as mother polymers. The APS was synthesized directly by the reaction of PMS with SbCl3, while IPS was obtained from the reaction of PCMS with Na2Fe(CO)4 in the solvent of n-hexane. The polymers were characterized by elemental analysis, FT-IR, 1H-NMR, 29Si CP/MAS NMR, GPC, and UV. The number average molecular weights of these two polymers are between 1400 and 1600. These polymers show good semiconductive properties, whose intrinsic conductivities are in the range of 10-8~10-7S/cm, and can be increased to10-5S/cm after being doped with iodine.

AbstractDual-frequency, multi-polarization airborne synthetic aperture radar (Pi-SAR; developed by the Communications Research Laboratory and National Space Development Agency of Japan) observations of the seasonal sea-ice region off the Okhotsk coast of Hokkaido, Japan, were carried out in February 1999 using X- and L-band radar frequencies with a resolution of 1.5 and 3.0 m. In conjunction with the SAR observations, the sea-ice thickness (draft) and velocity were measured by a moored Ice Profiling Sonar (IPS) and an Acoustic Doppler Current Profiler (ADCP). Tracks of the sea ice passing over the IPS were estimated from the time series of the ADCP ice-velocity and -direction data. Along these tracks, the SAR backscattering coefficient profiles were compared with the IPS ice-draft profiles. The results showed that the L-band SAR backs cattering profiles correlated well with the IPS ice-draft data, particularly in the thicker part (a few meters thick) of the rim of first-year ice, which had a large backscattering coefficient. Although the X-band SAR backscattering profiles did not correlate well with the IPS data, thin ice (<10 cm thick) showed a large backscattering coefficient. The L-band SAR and IPS data did not distinguish thin ice from open water.

Abstract:
In this work, we have studied the integration of an optical signal-based Indoor Positioning System (IPS) capable of supporting multi-access discrimination techniques. The research analyzes the different techniques and conditions that can be used to develop an IPS using a microcontroller unit (MCU)-based system-on-chip (SoC) systems. The main goal is to be able to integrate into the MCU both the hardware and software requirements for an IPS detector. In this way, different strategies that can implement multi-access discrimination using Frequency-division multiple access (FDMA) have been tested, such as I/Q demodulation, digital filtering, and discrete Fourier transform (DFT). This analysis has found a good technique to be executed in an MCU-based SoC, the DFT implemented through the Goertzel’s algorithm. The empirical tests carried out concluded that, using only one an MCU with the required HW and tuned SW, 15 position measurements per second were computed, with high accuracy in the 3-D positioning, with errors of less than 1 cm in a test area of 3.5 × 3.5 m 2 . The main contribution of the paper is the implementation of the optical signal based IPS in an MCU-SoC that includes signal acquisition and processing. The digital filtering or spectral processing for up to 16 received signals makes this IPS system very attractive from a design and cost point of view.

Indoor Positioning System (IPS) adalah teknologi untuk mengakomodasi pemetaan objek di dalam ruangan. Sistem ini dibuat tersendiri karena keterbatasan GPS untuk memetakan objek dalam ruangan karena terhalang oleh bangunan sehingga akurasi yang kurang presisi. RFID menjadi salah satu alternatif tag yang digunakan pada IPS karena mampu menghadirkan sistem pemetaan dengan biaya terjangkau dan performa yang tidak kalah jauh dari tag lainnya. Tujuan dari penelitian ini adalah untuk membangun sistem pemetaan dalam ruangan yang ekonomis dengan akurasi yang baik. Pada penelitian ini diajukan sebuah sistem IPS dengan objek pemetaan RFID Passive Tag dengan menggunakan Machine Learning Regresi yang diteruskan ke Multilateration yang selanjutnya dioptimalkan dengan algoritma Nelder Mead. Pada penelitian ini dibandingkan dua model regresi yaitu Support Vector Regression dan Linear Regression, prediksi jarak dari model terbaik akan dimasukkan pada persamaan Multilateration untuk memprediksi koordinat. Untuk meningkatkan akurasi dari teknik Multilateration digunakan optimasi algoritma Nelder Mead untuk mengatasi error dari prediksi jarak. Setelah dilakukan pengujian didapatkan akurasi dari model ini yaitu rerata error sebesar 37.39 cm.

Reflectivity values (Yb and Yw) of all samples were determined with CM-5 chromatism instrument and contrast ratios (Yb/Yw) were then calculated. Chromatic values of IPS E.max LT blanks provided by the manufacturer were also determined. L valueS of ceramic blanks were significantly different among specimen groups (P< 0.01). From C1 to C4 and D2 to D4 L value declined. Contrast ratios were not significantly different for specimens of the same thickness and color series ranging from 0.3105 to 0.5134. Significant difference was found for specimens of different thicknesses but the same color (P< 0.05). Thickness is an important correlative factor for the relative translucency of IPS E.max LT framework materials.

Background: One of the main challenges of orthopedic surgery is adequate pain management after total knee arthroplasty. This work aimed to determine the anatomical safety area for infiltration through the posterior capsule of the knee in prosthetic surgery using Magnetic Resonance Imaging (MRI). Methods: A descriptive, observational, cross-sectional study was performed on 126 knee MRIs. The variables studied were age, sex, and distance between different neurovascular structures of the popliteal fossa (tibial nerve, common peroneal nerve, and vascular bundle). Data were analyzed for normality (Kolmogorov–Smirnov) and variance homogeneity (Levène). A value of p < 0.05 and a confidence interval of 9% were considered statistically significant for all comparisons. Student’s t-test was used to compare the means between independent samples. Results: We observed statistically significant differences between the sexes regarding EP–EPS (external plateau–external popliteal sciatic nerve (common peroneal)), EP–IPS (external plateau–internal popliteal sciatic nerve (tibial)), and IP–PA (internal plateau–popliteal artery) measurements. The average distance between both nerves, EPS–IPS (external popliteal sciatic nerve and internal popliteal sciatic nerve), was 25.96 mm in females, while the value obtained in males was 29.93 mm, but this difference was not statistically significant. Conclusions: The average distance from the posterior capsule to the EPS and IPS nerves is greater in males than in females, despite no statistical differences. The presence of a lateralized arteriovenous bundle reduces the infiltration area of the external compartment. Regarding the safety area, infiltration of the internal compartment is safe since the volume diffuses into the muscle mass of the internal gastrocnemius upon injection. To infiltrate the external compartment, the needle must move at least 2 cm from the midline toward the external side (to exceed the maximum displacement of the neurovascular bundle established at 1.82 cm), and not advance beyond 0.76 cm (minimum distance at which we located the common peroneal nerve in the external compartment).

Abstract Background: Despite its high overall accuracy in predicting ABVD outcome in advanced stage Hodgkin Lymphoma (HL), interim PET performed after 2 chemotherapy cycles (PET-2) showed a sub-optimal negative predictive value (PV) on treatment response. In fact, PET-2 negative patients (p.) treated with six ABVD cycles in four prospective trials (RATHL, GITIL/FIL HD 0607, SWOG 0816 and Echelon-1), showed a 3-Y PFS ranging between 79% to 87%, even declining to 74% after a 5-year follow-up (Stephens 2019). A high Total Metabolic Tumor Volume (TMTV) calculated in baseline PET (cutoff value 471 ml.) , along with a high IPS (≥2) proved able to identify a small p. subset (7%) of PET-2 neg. p. with a 3-Y PFS of only 56% (Gallamini 16° ICML, 2021). A new TMTV-derived parameter aimed to image tumor spread, the tumor distance (DMAX), proved able to predict ABVD outcome in a retrospective series of HL p. from a single center (Durmo, 16° ICML 2021). We report here the PV on ABVD outcome of DMAX combined with IPS in a large cohort of PET-2 negative p. prospectively enrolled in the HD0607 clinical trial. Methods: Out of 783 p. with advanced HL (IIB-IVB) included in the HD0607 clinical trial (NCT00795613), 630 (81%) of them with both negative PET-2 and end-of therapy PET, were randomly assigned to no further therapy or consolidation radiotherapy over the area of a large nodal mass detected at baseline. A single experienced nuclear medicine physician calculated DMAX and TMTV in 331 out of 630 (52%) PET-2 negative p. in which the baseline PET images were available for review. Three different tumor segmentation methods for TMTV computing were chosen, with (1) a relative threshold of 41% of SUVmax in each lesion, (2) a fixed threshold of SUV=2.5 or (3) of SUV=4. DMAX was calculated as the maximum distance among any pixel of the tumor belonging to any lesions in the body. Results: The demographics of the 331 p. included in the present study and of the overall cohort of 630 PET-2 negative p. were: median age 31 (14-60) Vs. 31 (14-60), M/F ratio 0.86 Vs. 0.89; WHO Performance Status 0-1 91.5% Vs. 91.4%; B-symptoms 81.8 Vs. 81.1%; Stage IIB, III and IV 35.0, 35.0, 29.9, Vs.36.3, 33.0 and 30.0%; IPS 0-1 39.3 Vs. 39.8%, IPS 2-3 48.3 Vs. 49.4%, IPS &gt;3 12.4 Vs. 10.6%, Bulky 18.1 Vs. 17.9%. No difference in 6-y PFS was found for p. randomized to NFT or cRT (p=.48; Gallamini JCO 2020). After a median follow-up of 40.6 (4.8-87.2) months, the 3-Y PFS for the 331 p. included in the present analysis and for all the 630 PET-2 negative p. was 84% (95% CI 81% to 87%) and 87% (95% CI, 84% to 89%), respectively. Treatment failure was recorded in 51/331 (15.4%) and 81/627 (12.9%), respectively. Based on a ROC analysis the three different segmentation methods for MTV computing proved to be equivalent (AUC values 0.620-0.525) and hence the 41% threshold was chosen for consistency with previous works. Median and average DMAX values were 12.5 cm. and 15.3 cm. The most accurate cutoff value for DMAX to predict treatment outcome (3-Y PFS) was 16.2 cm., with an AUC of 0.62 (95% CI 0.53-0.70). With this cutoff value DMAX was able to identify two cohorts of patients with a statistically different 3y-PFS: 90% (CI 85-93%) and 76% (95% CI 69-85%), p &lt; 0.001. In multivariate analysis (Cox regression model) including all the above demographics and clinical parameters, as well as TMTV and DMX, only DMAX turned out significant in predicting relapse, with a HR of 1.46 (95% CI1.06-2.01), p=0.02. Upon combining DMAX (higher or lower than 16.2 cm.) and IPS (0-1 Vs. ≥2) in a two-factor predictive model, three categories of p. with a statistically different treatment outcome (P &lt; 0.0001) have been identified: (1) both low DMAX and low IPS, N =30 (9%), 3-Y PFS 100% (95% CI 96-100); (2) either high DMAX and low IPS or high IPS and low DMAX, N= 198 (60%); 3-Y PFS 88% (95% CI 83-93), and (3) both high DMAX and IPS, N= 103 (31%); 3-Y PFS 72% (95% CI 65-82), p&lt;0.0001. Conclusions: DMAX and IPS combined in a predictive model were able to single out three classes of PET-2 negative p. with a statistically different ABVD outcome. P. with a high MTV and a high DMAX, accounting for nearly one-third of p. included in the study, showed the highest risk of failing ABVD, with only 72% of them sustaining a long-term disease control at three years, thus deserving a more aggressive or innovative treatment. The remaining two-thirds had a very good outcome, with a 3-y PFS of 90%, thus stressing that ABVD could remain the standard of care for most PET-2 negative p. Figure 1 Figure 1. Background. Shoulder pain is a common symptom following laparoscopic surgery. This systematic review was undertaken to assess updated evidence regarding the effectiveness and complications of the pulmonary recruitment maneuver (PRM) for reducing shoulder pain after laparoscopic gynecologic surgery. Methods. A number of databases for randomized controlled trials (RCTs) investigating PRM for reducing shoulder pain were searched up to June 2019. Two authors independently selected potentially relevant RCTs, extracted data, assessed risk of bias, and compared results. Network meta-analyses were employed to simultaneously compare multiple interventions. Effect measures were presented as pooled mean difference (MD) or risk ratio (RR) with corresponding 95% confidence intervals (CI). Results. Of the 44 records that we identified as a result of the search (excluding duplicates), eleven RCTs involving 1111 participants were included. Three studies had an unclear risk of selection bias. PRM with a maximum pressure of 40 cm H2O was most likely to result in the lowest shoulder pain intensity at 24 hours (MD −1.91; 95% CI −2.06 to −1.76) while PRM with a maximum pressure of 40 cm H2O plus intraperitoneal saline (IPS) appeared to be the most efficient at 48 hours (MD −2.09; 95% CI −2.97 to −1.21). The estimated RRs for analgesia requirement, nausea/vomiting, and cardiopulmonary events were similar across the competing interventions. Conclusion. PRM with 40 cm H2O performed either alone or accompanied by IPS is a promising intervention for alleviating shoulder pain within 48 hours following gynecologic laparoscopy.

Indoor Positioning Systems (IPSs) have multiple applications. For example, they can be used to guide people, to locate items in a warehouse and to support the navigation of Automated Guided Vehicles (AGV). Currently most AGVs use local pre-defined navigation systems, but they lack a global localisation system. Integrating both systems is uncommon due to the inherent challenge in balancing accuracy with coverage. Visible Light Position (VLP) offers accurate and fast localisation, but it encounters scalability limitations. To overcome this, this paper presents a novel Image Sensor-based VLP (IS-VLP) identification method that harnesses existing Light Emitting Diode (LED) lighting infrastructure to substitute both navigation and localisation systems effectively in the whole area. We developed an IPS that achieves six-axis positioning at 90 Hz refresh rate using OpenCV’s solvePnP algorithm and embedded computing. This IPS has been validated in a laboratory environment and successfully deployed in a real factory to position an operative AGV. The system has resulted in accuracies better than 12 cm for 95% of the measurements. This work advances towards positioning VLP as an appealing choice for IPS in industrial environments, offering an inexpensive, scalable, accurate and robust solution.

Barth syndrome (BTHS) is an X-linked recessive multisystem disorder caused by mutations in the TAZ gene (TAZ, G 4.5, OMIM 300394) that encodes for the acyltransferase tafazzin. This protein is highly expressed in the heart and plays a significant role in cardiolipin biosynthesis. Heart disease is the major clinical manifestation of BTHS with a high incidence in early life. Although the genetic basis of BTHS and tetralinoleoyl cardiolipin deficiency in BTHS-affected individuals are well-established, downstream metabolic changes in cardiac metabolism are still uncovered. Our study aimed to characterize TAZ-induced metabolic perturbations in the heart. Control (PGP1-TAZWT) and TAZ mutant (PGP1-TAZ517delG) iPS-CM were incubated with 13C6-glucose and 13C5-glutamine and incorporation of 13C into downstream Krebs cycle intermediates was traced. Our data reveal that TAZ517delG induces accumulation of cellular long chain acylcarnitines and overexpression of fatty acid binding protein (FABP4). We also demonstrate that TAZ517delG induces metabolic alterations in pathways related to energy production as reflected by high glucose uptake, an increase in glycolytic lactate production and a decrease in palmitate uptake. Moreover, despite mitochondrial dysfunction, in the absence of glucose and fatty acids, TAZ517delG-iPS-CM can use glutamine as a carbon source to replenish the Krebs cycle.

Purpose The International Prognostic Score (IPS) is the most widely used risk stratification index for Hodgkin's lymphoma (HL). It is based on patients treated before 1992 and predicts 5-year freedom from progression (FFP) and overall survival (OS) ranging from 42% to 84% and 56% to 89%, respectively. The IPS has not been validated in a recently treated population in which outcomes have improved compared with historic results. Patients and Methods By using the British Columbia Cancer Agency Lymphoid Cancer Database, we identified all patients age ≥ 16 years newly diagnosed with advanced-stage HL (stage III to IV, or stage I to II with “B” symptoms or bulky disease ≥ 10 cm) from 1980 to 2010, treated with curative intent with doxorubicin, bleomycin, vinblastine, and dacarbazine (ABVD) or an ABVD-equivalent regimen with complete clinical information. Results In all, 740 patients were identified. Five-year FFP and OS were 78% and 90%, respectively. The IPS was prognostic for both FFP (P < .001) and OS (P < .001), with 5-year FFP ranging from 62% to 88% and 5-year OS ranging from 67% to 98%. Analysis limited to patients age 16 to 65 years (n = 686) demonstrated a narrower range of outcomes, with 5-year FFP ranging from 70% to 88% and 5-year OS ranging from 73% to 98%. Conclusion The IPS remains prognostic for advanced-stage HL, but the range of outcomes has narrowed considerably. This improvement in outcome with ABVD should be acknowledged before consideration of alternate initial therapies and when comparing results from current trials with those of historic controls.

Abstract KGF (FGF-7), an epithelial cell growth factor, can lessen the injurious effects of post-BMT lung injury (IPS) and GVHD when given prior to conditioning of the recipient. We reported that mice treated with KGF pre-conditioning had elevated systemic levels of Th2-type cytokines at the time of BM infusion which was associated with less GVHD. In non-conditioned SCID mice that developed lethal GVHD from allogeneic T cells, KGF pretreatment also increased Th2 cytokines implying that KGF alone was responsible for the systemic Th2 milieu and not a response that was dependent on conditioning. In the post-BMT lung, KGF pretreatment did not alter the cellular composition of the inflammatory infiltrate but resulted in decreased levels of immune activation markers compared to non-treated control IPS mice. The mechanism of this immunomodulatory effect is unknown. Since Th2 responses are dependent on the STAT-6 transcription factor, we studied the effects of KGF in our established murine IPS model using lethally-conditioned STAT-6−/− mice (B6 background) as recipients of allogeneic B10.BR BM and T cells and compared to WT hosts. STAT-6−/− mice pretreated with KGF (5 mg/kg s.c. for 3 days) had an accelerated mortality rate and poorer survival compared to WT recipients. Examination of GVHD target organs revealed that KGF pretreatment ameliorated liver GVHD in WT and STAT-6−/− recipients. In contrast, the benefit of KGF for GVHD of the colon was only seen in the WT but not in STAT-6−/− recipients, consistent with post-BMT body weights and survival. Whole body plethysmograph was done to evaluate lung function at 30 cm water pressure on day 7 post-BMT, a time when acute IPS is manifested in this model. WT but not STAT-6−/− mice pretreated with KGF had improved lung compliance compared to their non-treated counterparts (p<0.05). KGF-pretreated STAT-6−/− recipients also had increased lung weights compared to control- or KGF-treated WT mice with IPS (p<0.05). Histological analysis of lung cryosections taken on day 7 post-BMT showed that all recipients of allogeneic BM and T cells had increased inflammatory cells in their lungs, consistent with acute IPS. However, STAT-6−/− hosts with IPS had elevated numbers of CD4+ T cells in their lungs compared to WT hosts with IPS (18.3%±1.5 vs 11.6%±2.7, p=0.01). Other cell phenotypes did not differ. However, KGF-pretreated STAT-6−/− mice had significantly elevated levels (10-fold) of IL-17, CCL2 and CCL8 in their bronchoalveolar lavage fluid on day 7 post-BMT compared to their WT KGF-treated counterparts implying a skewing to a Th17 inflammatory response in STAT-6−/− mice. These data indicate that the STAT-6 pathway plays a role in the cytoprotective function of KGF and this STAT-6-dependency is tissue-specific. To achieve the full benefits of KGF and other cytoprotective agents, it will be important to take into account the immune modulatory mechanisms operative in individual GVHD target organs.

A study was taken up to evaluate the performance of cost-free surge irrigation including two amendments viz., Raw Coconut Fibre Waste (RCFW) and Farm Yard Manure (FYM) @ 12.5 t ha1, in Sunflower (Co.2). The flow rate was 1 Ips with an 'ON-OFF' time of 10 minutes. The total distance was 100 m. The spacing was 60/2 30x30 cm under double row planting. The cost-free surge irrigation involved inlet pipes of 52 cm length and 6.0 cm diameter from head channel to the furrows and manually operated. The over all trend was that higher number of surges to start with, decreasing in between and increasing at the end. There was a water economy (44.3-44.7 per cent) and labour economy (48.1- 51.5 per cent) due to surge irrigation. Next ranking in labour economy was surge irrigation with RCFW. Labour saving was relatively lesser in continuous irrigation. Water use efficiency was higher for surge irrigation as compared to continuous irrigation except sector three (51-75 M) due to 'penultimate depression. Also continuous irrigation suffered from penultimate depression.

Abstract Background: The prognostic score for Hodgkin’s lymphoma was defined as the number of adverse prognostic factors presented at diagnosis. Seven factors had similar independent prognostic effects. This model was validated retrospectively in advanced disease using different therapeutic approaches (D Hasenclever et al N Eng J Med339:1506–14, 1998). Methods: From December 1996 up to October 2005, the GATLA completed a risk-adapted therapy with ABVD and IFRT. Patients with stages I-IIIA without bulky disease, who achieved complete remission (CR) after three cycles of ABVD, favorable group (FG) received only IFRT 25 GY to areas of &gt;2 cm at diagnosis. Patients with FG not in CR after three cycles of ABVD, slow responders (FGSR), all stages IIIB-IV and all bulky disease, unfavorable group (UG) received six cycles of ABVD and IFRT 30 GY at remaining areas after 3 cycles of ABVD. A total of 584 patients, completed therapy; of them 513 were evaluated with the IPS. Patients were divided in three groups according to the number of adverse prognostic factors 0–1, 2–3, and ≥ 4. Results: The number of patients, complete remission (CR) rate, event-free survival (EFS) and overall survival (OSV) at 5 years according to prognostic factors in the 513 patients were as follows: IPS # patients (%) # CR (%) % EFS % OSV 0–1 224 (44) 217 (97) 86 95 2–3 241 (47) 213 (88) 73 90 ≥4 48 (9) 40 (83) 65 72 P&lt; 0.020 0.001 0.001 A total of 200 patients with FG had a 5 years EFS and OSV of 89% and 98% while 53 patients with FGSR had an EFS and OSV of 66% and 88% respectively (P&lt;0.001). The IPS in FG and FGSR was 0–1 of 61% versus 49%, 2–3 of 38.5% versus 43% and ≥4 of 0.5% versus 8% respectively (p=0.003). In UG with an EFS and OSV of 72% and 87%, the incidence of IPS 0–1 was 29%, 2–3 was 54% and ≥4 was 17%. Conclusion: The IPS is an excellent tool to predict outcome. Patients with stages I-IIIA without bulky tumour who did not achieve CR after three cycles of ABVD (FGSR) had poorer IPS than FG. In spite of receiving six cycles of ABVD, those with FGSR instead of three of those with FG had statistically a poor outcome. In the PET-TC era, patients who remain positive after three cycles of ABVD will need an intensified therapy with the purpose of improving the bad prognosis.

This study aimed to examine the presence of neuropsychological deficits and their relationships with clinical, pharmacological, and neuropsychiatric characteristics in chronic migraine (CM) patients assessed during a headache-free period. We enrolled 39 CM patients (mean age: 45.4 years; male/female ratio: 3/36) and 20 age-, sex-, and education-matched healthy controls (HCs, mean age: 45.5 years; male/female ratio: 2/18) in a case–control study. All CM patients underwent a full and extensive clinical, neuropsychiatric, and neuropsychological evaluation to evaluate cognitive domains, including sustained attention (SA), information processing speed (IPS), visuospatial episodic memory, working memory (WM), and verbal fluency (VF), as well as depressive and anxiety symptoms. CM patients exhibited higher scores than HCs for all clinical and neuropsychiatric measures, but no differences were found in personality characteristics. Although more than half of the CM patients (54%) showed mild-to-severe neuropsychological impairment (NI), with the most frequent impairments occurring in short- and long-term verbal episodic memory and inhibitory control (in approximately 90% of these patients), almost half of the patients (46%) showed no NI. Moreover, the severity of NI was positively associated with the number of pharmacological treatments received. Remarkably, disease-related symptom severity and headache-related disability explained global neuropsychological performance in CM patients. The presence of cognitive and neuropsychiatric dysfunction during the interictal phase occurred in more than half of CM patients, increasing migraine-related disability and possibly exerting a negative impact on health-related quality of life and treatment adherence.

Unlike GNSS-based outdoor positioning, there is no technological alternative for Indoor Positioning Systems (IPSs) that generally stands out from the others. In indoor contexts, the measurement technologies and localization strategies to be used depend strongly on the application requirements and are complementary to each other. In this work, we present an optical IPS based on a Position-Sensitive Detector (PSD) and exploiting illumination infrastructure to determine the target position by Angle of Arrival (AoA) measurements. We combine the proposed IPS with different positioning strategies depending on the number of visible emitters (one, two, or more) and available prior or additional information about the scenario and target. The accuracy and precision of the proposal is assessed experimentally for the different strategies in a 2.47 m high space covering approximately 2.2 m2, using high-end geodetic equipment to establish the reference ground truth. When the orientation of the target is known from external measurements, an average positioning error of 8.2 mm is obtained using the signal received from only one emitter. Using simultaneous observations from two emitters, an average positioning error of 9.4 mm is obtained without external information when the target movement is restricted to a plane. Conversely, if four signals are available, an average positioning error of 4.9 cm is demonstrated, yielding the complete 3D pose of the target free of any prior assumption or additional measurements. In all cases, a precision (2σ) better than 5.9 mm is achieved across the complete test space for an integration time of 10 ms. The proposed system represents a prospectively useful alternative for indoor positioning applications requiring fast and reliable cm-level accuracy with moderate cost when smart illumination infrastructure is available in the environment.

Abstract The malignant Hodgkin and Reed-Sternberg (HRS) cells of classical Hodgkin lymphoma (HL) rarely survive outside their microenvironment, which is illustrated by the difficulty in establishing HRS-derived cell lines. Reactive B and T cells, monocytes, and eosinophils have been reported to express growth and survival factors such as CD30 ligand, CD40 ligand, RANK ligand, and BLyS that may support HRS cell survival. To examine the contribution of reactive B-cells to the survival of HRS cells in vivo, we hypothesized that depleting B-cells from the HL microenvironment by rituximab may deprive HRS cells from critical survival and resistance factors and thereby improving the efficacy of chemotherapy. To test this hypothesis, we evaluated the safety and efficacy of the novel combination of rituximab and ABVD (R-ABVD) chemotherapy in newly diagnosed patients with classical HL. Rituximab was given at 375 mg/m2 weekly for 6 weeks to rapidly deplete B lymphocytes, while ABVD was given at a standard dose and schedule. The first dose of rituximab was given concurrently with the first dose of ABVD (schedule A) or 3 weeks before the first dose of ABVD (schedule B). Patients with lesions larger than 5 cm received involved field radiation therapy at the end of R-ABVD. Patients were eligible if they were older than 16 years of age and had biopsy-confirmed classical HL irrespective of CD20 expression on HRS cells, bidimensionally measurable disease, adequate bone marrow reserve (ANC > 1,000/uL, Platelets > 100,000/uL) and adequate cardiac and renal functions. They were excluded if they had HIV infection, or were pregnant women. To date 70 newly diagnosed patients are enrolled, of whom 65 patients had at least 12 months of follow up and are evaluable for treatment response. The median age is 28 years (Range; 18–72 years). Patients had stage II (50%), stage III (31%), or stage IV (19%) disease. Using the IPS prognostic score model, 36 patients (55%) had a score of 2 or higher. With a median follow up of 32 months, the estimated event-free survival (EFS) for the entire group is 85% and the overall survival is 98%. EFS for patients with different IPS groups is shown in the table and is compared with the expected EFS for ABVD alone as reported by Hasenclever and Diehl (NEJM 1998). As shown, R-ABVD improved EFS in all IPS groups with the biggest impact seen in patients with IPS > 2. In two patients, fine needle aspiration of a peripheral lymph node was performed before the first and after the third dose of rituximab (schedule B-before starting ABVD). Rituximab induced complete depletion of CD20 and CD19 positive B cells from HL node. We conclude that in patients with classical HL, the addition of 6 weekly doses of rituximab to standard dose and schedule of ABVD chemotherapy is effective in terms of remission rate and remission duration irrespective of CD20 expression on HRS cells or IPS category. A randomized phase-II study is planned to evaluate this strategy in patients with high IPS score. AbstractThree white spruces, Picea glauca (Moench) Voss (30–35 cm diameter at breast height), felled in central Alberta, were colonized by Ips perturbatus (Eichhoff) beginning in late May 1999. The mean (±SE) density of breeding galleries on the trees was 217 ± 23/m2 (n = 30). Harem size ranged from one to four females per gallery, with a mean of 2.1 ± 0.6 (n = 90). Tunnel excavation and oviposition occurred primarily during the first 3 weeks after gallery initiation. Individual females laid 48.9 ± 2.5 eggs (n = 30) in galleries that reached 10.0 ± 0.3 cm in length (n = 45). Males assisted their mates by removing frass and woody debris from the tunnels. Males remained in their galleries for at least 1 week, although there was gradual attrition such that < 15% of males remained after 6 weeks. Large males abandoned their galleries sooner than small males. In contrast, females were present in almost half of the oviposition tunnels examined after 6 weeks, and there was no significant relationship between female size and residence time. Mortality from egg to adult was high (98%) during this 1-year study, likely a result of the intense crowding of galleries. Adult offspring were found beneath the bark in mid-July, although the main emergence did not begin until mid-September. Because such late emergence would be too late for these individuals to reproduce before winter, I conclude that I. perturbatus has only one generation per year in central Alberta.

Enhanced scintillations in the direction of the quasar 2204+29 (3C441) were observed on 13 May 1990 when the tail of Comet Austin passed in front of it. Comparison with previous observations at 103, 327 and 408 MHz of Comet Halley and at 408 MHz of Comet Wilson show that proper occultation geometry is essential for observing enhanced scintillations. It has been shown that the solar elongation ? during such observations should be large, typically greater than 60� and in no case less than 30� at 103 MHz. At the time of the occultation the scintillation index (r.m.s./mean source flux) was greater than that expected for this source by a factor of 3. The r.m.s. electron density variation /IN, at a distance of 0�9 A.U. from the sun and 7�3� downstream of the nucleus, was found to be 6 cm-3 as compared with 1 cm-3 for the normal solar wind at 1 A.U. The corresponding scale sizes of the turbulence were found to be much finer than normally found in interplanetary scintillation (IPS) caused by the solar wind.

Trong bài báo này, nhóm tác giả đưa ra thiết kế của hệ thống định vị trong nhà IPS (Indoor Positioning System) sử dụng sóng siêu cao tần băng thông rộng kết hợp với phép đo ba cạnh. Hệ thống gồm 3 nút Anchor được dùng làm các điểm mốc và 1 nút Tag được gắn trên đối tượng cần định vị. Thông qua khoảng cách từ nút Tag đến 3 điểm mốc, IPS xác định được vị trí của đối tượng theo thời gian thực. Sau thiết kế, hệ thống được đánh giá bằng phương pháp thực nghiệm kết hợp với phần mềm chuyên dụng. Kết quả cho thấy hệ thống định vị đã xác định được đối tượng với sai lệch dưới 10 cm. Sai số này có giá trị nhỏ hơn nhiều so với sai số của hệ thống định vị toàn cầu GPS (Global Positioning System). Với các thông số thiết kế đạt được, hệ thống định vị phù hợp để ứng dụng vào hệ thống dẫn đường cho Robot và xe tự hành.

Monitoring external training load (eTL) has become popular for team sport for managing fatigue, optimizing performance, and guiding return-to-play protocols. During indoor sports, eTL can be measured via inertial measurement units (IMU) or indoor positioning systems (IPS). Though each device provides unique information, the relationships between devices has not been examined. Therefore, the purpose of this study was to assess the association of eTL between an IMU and IPS used to monitor eTL in team sport. Retrospective analyses were performed on 13 elite male National Collegiate Athletic Association (NCAA) Division I basketball players (age: 20.2 ± 1.2 years, height: 201.1 ± 7.6 cm, mass: 96.8 ± 8.8 kg) from three practices during the off-season training phase. A one-way analysis of variance was used to test differences in eTL across practices. Pearson’s correlation examined the association between the Distance traveled during practice captured by IPS compared to PlayerLoad (PL), PlayerLoad per Minute (PL/Min), 2-Dimensional PlayerLoad (PL2D), 1-Dimensional PlayerLoad Forward (PL1D-FWD), Side (PL1D-SIDE), and Up (PL1D-UP) captured from the IMU. Regression analyses were performed to predict PL from Distance traveled. The eTL characteristics during Practice 1: PL = 420.4 ± 102.9, PL/min = 5.8 ± 1.4, Distance = 1645.9 ± 377.0 m; Practice 2: PL = 472.8 ± 109.5, PL/min = 5.1 ± 1.2, Distance = 1940.0 ± 436.3 m; Practice 3: PL = 295.1 ± 57.8, PL/min = 5.3 ± 1.0, Distance = 1198.2 ± 219.2 m. Significant (p ≤ 0.05) differences were observed in PL, PL2D, PL1D-FWD, PL1D-SIDE, PL1D-UP, and Distance across practices. Significant correlations (p ≤ 0.001) existed between Distance and PL parameters (Practice 1: r = 0.799–0.891; Practice 2: r = 0.819–0.972; and Practice 3: 0.761–0.891). Predictive models using Distance traveled accounted for 73.5–89.7% of the variance in PL. Significant relationships and predictive capacities exists between systems. Nonetheless, each system also appears to capture unique information that may still be useful to performance practitioners regarding the understanding of eTL.

Object. Broca identified the posterior third of the inferior frontal gyrus as a locus essential for the production of fluent speech. The authors have conducted this retrospective analysis in an attempt to find readily identifiable landmarks on magnetic resonance (MR) imaging that correspond to intraoperative cortical stimulation-induced speech arrest. These landmarks demonstrate novel structural—functional relationships that can be used preoperatively to predict the location of the Broca area. Methods. Using a neuronavigation system, sites where stimulation produced speech arrest (Broca area) were recorded in a consecutive series of patients undergoing awake tumor resections in the perisylvian territory of the dominant hemisphere. The authors reviewed 33 consecutive patients by projecting the MR imaging data sets and marking the site where the Broca area was identified. Sulcus topography was analyzed with respect to this site by scrolling into neighboring planes and classifying the frontal operculum into one of the four schemes of sulcus variability described by Ebeling, et al. The following categories of frontal opercula were found: 18 (69%) of 26 were Type I, eight (31%) of 26 were Type III, and seven cases eluded classification because of sulcal effacement. For patients with Type I anatomy, the Broca area was adjacent to, and distributed evenly around, the inferior precentral sulcus (IPS). Quantitatively, the site of speech arrest was located a mean of 2.4 ± 0.25 cm from the anteroinferior aspect of the pars opercularis, where it abuts the subarachnoid space surrounding the apex of the pars triangularis. For all patients with Type III anatomy, the Broca area was adjacent to the accessory sulcus that lies immediately posterior to the IPS. In these patients the mean distance from the anterior inferior pars opercularis was 2.3 ± 0.29 cm. The mean distance from the Broca area to the edge of the tumor for the 26 patients with clear sulcal anatomy was 1.29 ± 0.12 cm. Conclusions. The results indicate a correlation between the structure of the frontal operculum as seen on MR imaging and the functional localization of speech arrest in the dominant hemisphere. Additionally, sulcal landmarks that can be used preoperatively to predict the location of the Broca area within the inferior frontal gyrus are described based on the patient population. This information will allow the surgeon to determine if an awake craniotomy is necessary to identify the Broca area when planning a surgical procedure near the dominant frontal operculum.

Cost-free surge irrigation method was tested involving two amendments of Farm Yard Manure (FYM) and Raw Coconut Fibre Waste (RCFW) @ 12.5 t ha in sunflower crop. The cultivar was Co.2 raised with a spacing of 60 x 230 x 30 cm under double row planting. Cost-free surge irrigation includes waste aluminium sheets of off-set press made into inlet pipes of 52 x 6 cm and placed in the head channel to draw the water. The designed rate of flow was I Ips and the 'ON-OFF time was 10 minutes. The study revealed that the available soil moisture (ASM) was higher in the head and gradually decreased upto third sector and again increased in the fourth sector. Available soil moisture status was higher under RCFW as compared to FYM. There was not much difference in depletion pattern between surge and continuous flow. There was a marked reduction in the moisture status in third sector (51-75 M). Under farmers' method the depletion pattern did not vary as compared to surge or continuous flow. Soil moisture depletion was faster during the initial periods and was slowed down during later periods.

Dentine porcelain disc specimens 13.0 mm in diameter with thicknesses of 1.0, 1.5, 2.0, 2.5, 3.0, 3.5, 4.0, 4.5, 5.0, 5.5, and 6.0 mm, were made using IPS E.max color systems B, C, and D and their chromatic aberration values (ΔE) under the standard black background and the standard white background were measured using a Minolta CM-5 spectrophotometer. Then, the cubic regression formula of best fit was used to calculate the thicknesses of dentine porcelains of various color numbers at a ΔE of 1.0 and 1.5. The determination coefficient of the regression formula of color system B was 0.990–0.995, that of color system C was 0.989–0.997, and that of D was 0.993–0.998; and R2is close to 1. The results show that the regression formulas of the 11 thickness groups have good fit. The infinite optical thickness values of the IPS E.max dentine porcelains were as follows: for B1–B4, 3.263–2.711 (ΔE = 1.0) and 2.910–2.493 (ΔE = 1.5); for color system C, 2.727–2.611 (ΔE = 1.0) and 2.473–3.365 (ΔE = 1.5); and for color system D: 2.928–3.694 (ΔE = 1.0) and 2.656–2.458 (ΔE = 1.5). When the chromatic aberration tolerance is 1.5, the thickness of dentine porcelain required to reach the infinite optical thickness is less than the numerical value at ΔE = 1.0. As the thickness increases, the influence of the background diminishes and the specimen looks truer to its inherent color. The results prove that thickness of dentine porcelain positively correlates with its masking ability and that its thickness must exceed its infinite optical thickness so that its color will not be influenced by the background color.

ABSTRACT This paper deals with the studies on the biology and development of the bark beetle Ips stebbingi Strohmeyer, 1908 (Coleoptera: Curculionidae: Scolytinae), a pest of Himalayan blue pine Pinus wallichiana A.B.Jacks. in Kashmir Himalaya. This pest is an aggressive and notorious one, but little is known of its biology. This beetle pest overwinters in adult stage under the bark of host trees P. wallichiana. After emergence, the adults fly to suitable trees and undergo maturation feeding for 4–6 days. Reproduction is polygamous type; two to six females join a male in his nuptial chamber. After mating, each of the females makes one gallery with an average length of 10.76 (±3.30 standard deviation [SD]) cm. The female lays 23.16 (±7.03 SD) eggs on an average. The eggs hatch in 8–18 days. The larvae have five instars and complete their development in 30–45 days constructing larval galleries of 3.83 (±0.63 SD) cm in length. The larvae pupate for 18–28 days and finally the adults emerge to attack new suitable trees. The adults live for 45–70 days, and the total lifespan of this species ranges from 101 to 156 days. The seasonal distribution of various life stages and the number of generations were also recorded.

Se trata de un paciente de sexo masculino de 70 años de edad, oriundo de la ciudad de Pasto, con antecedentes de colelitiasis, sin colecistitis, de 2 años de evolución, quien ingresa el 27 de diciembre del 2012 a urgencias de una Institución Prestadora de Salud (IPS) de la mencionada ciudad, donde se realiza colecistectomía laparoscópica, encontrándose colangitis, vesícula necrótica, piocolecisto, cálculo de 5x6 cm, impactado en cuello vesicular y vesícula fibrótica adherida al lecho hepático. El síndrome de Mirizzi fue descrito por primera vez por Pablo Mirizzi en el año de 1947 y señala factores etiológicos de colestasis extrahepática en algunos pacientes con litiasis vesicular. Esta patología es infrecuente a nivel mundial. En Colombia, y más específicamente en el departamento de Nariño, se carece de publicaciones sobre de este tema; por tal razón se trae a discusión este caso clínico.

Abstract:
The objective of this study was to test the effect of water stress and pruning on the resistance of young Scots pines (Pinus sylvestris L.) to a bark beetle associated fungus. Six-year-old potted trees were either pruned (70% of needles removed) or subjected to several successive episodes of severe water stress, prior to inoculation of inner bark with the fungus Ophiostoma ips (Rumb.) Nannf., which is usually associated with the bark beetle Ips sexdentatus Boern. Well-watered, nonpruned trees served as controls. Predawn needle water potential reached -2.5 MPa and net CO2 assimilation rates were reduced to almost zero during each water stress episode. The length of the reaction zones around inoculation points reached 3-4 cm after 3 weeks. It was higher during Spring than during Autumn. Impact of water stress on the length of the reaction zone was very limited and independent of the number of drought episodes that had been imposed prior to inoculation. The only visible change was a slight decrease when the inoculation was done during the period of maximum water stress intensity. Growth of the pathogen in the phloem was not affected by water stress. Correspondingly, pruning had no effect on either of these two parameters. Induced reaction zones accumulated monophenolic compounds that were undetectable in unwounded phloem. These included pinocembrin and pinosylvin and its monomethylether known to contribute to the defence against the fungus. Neither drought nor severe pruning induced any change in the nature or concentrations of these compounds in the unwounded phloem or in the reaction zone.

Este artículo presenta la evaluación de escenarios de coordinación de actores en la red de suministro de medicamentos oncológicos utilizando la dinámica de sistemas. Se describe el contexto actual y la problemática del abastecimiento resultado de la investigación realizada en instituciones prestadoras de servicios de salud (IPS) con servicios de oncología de diferentes ciudades de Colombia. Se desarrolla una revisión de los principales aspectos relacionados con la coordinación en sistemas de suministro, se evalúan y analizan dos escenarios de coordinación basados en el uso de información compartida y la toma de decisiones conjunta utilizando parámetros de referencia de medicamentos de alto costo que requieren adecuación en centrales de mezcla (CM) para aplicación de tratamientos de quimioterapia en pacientes con cáncer. Los resultados dieron ahorros cercanos al 14% y niveles de cumplimiento alrededor del 98%, lo que implica una contribución importante al sistema de salud colombiano.

The purpose of this study was to determine the friction coefficients and wear rates of six commercially available dental ceramics including IPS Empress 2 (E2), Cergo Pressable Ceramic (CPC), Cercon Ceram (CCS) and Super porcelain EX-3 (SPE). Bovine enamel (BE) was also tested as a reference material for comparison purposes. Samples of the dental ceramics were prepared according to the instructions described by the manufacturers in disk-shape with nominal dimensions of 12 mm × 2 mm. The wear tests were performed by means of a pin-on-disk type tribometer. The friction coefficients and specific wear rates of the materials were determined at a load of 10 N and rotating speed of 0.25 cm/s without lubrication. Surface morphology of the wear tracks was examined using a scanning electron microscope. Statistical analyses were made using one-way ANOVA and Turkey's HSD (P < 0.05).

Water front advance in relation to crop geometry T on-off timing under continuous and surge flow was studied in Tamil Nadu Agricultural University. The soil was sandy clay loam. The slope was 0.5 per cent. Cost free surge layout was adopted using inlet pipes (50 x 7.5 cm) and manually operated. Water front advance took more time for continuous irrigation as compared to surge flow. Among the crop geometry, water front advance was faster under double row as compared to single row. Out of three T on-off timings there was not much difference in water front advance between 10 and 15 minutes. Water front advance was faster in 20 minutes. Surge irrigation in maize with single crop row geometry and T on-off timings of 10-15 with a flow rate 1.5 Ips is optimum from the point of view of water front advance.

<p><strong>Objective:</strong> To evaluate the effect of shade and curing mode on cure efficiency of two dual-cure resin cements. <strong>Material and Methods:</strong> Two shades (A2 and TRANS) of two different dual-cure resin cements (G-CEM, GC Dental and SET PP, SDI) were submitted to different curing modes: light curing through a 2 mm thick ceramic disc (IPS Empress Esthetic, A2, Ivoclar Vivadent) for 20 s (16 J/cm<sup>2</sup>), 40 s (32 J/cm<sup>2</sup>) or 80 s (64 J/cm<sup>2</sup>) performed immediately or with 1 or 5 min of delay. Fourier transform infrared spectroscopy (FT-IR) was used to evaluate the degree of conversion (DC) after 48 h. For the statistical analysis, data were submitted to three-way analysis of variance, followed by Tukey’s test for multiple comparisons (p=0.05). <strong>Results:</strong> Shade influenced DC (%) for the dual-cure resin cements tested (p=0.00001). TRANS shade showed lower DC (%) for both cements when there was no delay before light-curing (p=0.00001). Curing mode also influenced DC (%) for the dual-cure resin cements tested: radiant exposure greater than 32 J/cm<sup>2</sup> and delaying light-curing for 1 to 5 min increased the DC (%) for both dual-cure resin cements evaluated. <strong>Conclusions:</strong> One min delay prior to light curing improved the cure efficiency and may be a more clinically acceptable approach to increase the degree of conversion of dual-cure resin cements.</p>

There are several technologies and techniques available when developing indoor positioning systems (IPS). Recently, the development of positioning systems based on optical signals has aroused great interest, mainly those using visible light from the lighting infrastructure. In this work, we analyze which techniques give better results to lay the foundations for the development of a Visible Light Positioning system (VLP). Working only with a receiver, it is analyzed what the result of determining the position of different emitters is when they emit simultaneously and without any synchronism. The results obtained by Frequency Division Multiple Access (FDMA) (with digital bandpass filters, I/Q demodulation, and FFT) and Code Division Multiple Access (CDMA) are compared. The interference between signals when emitted simultaneously from multiple emitters is analyzed as well as the errors they cause and how these effects can be mitigated. As a result of the research, the advantages and disadvantages using different multiple-access determination techniques are determined. In addition, advantages and disadvantages of using FDMA and CDMA techniques as well as hardware requirements that make one more feasible than the other are presented. The system behavior, in terms of errors, is established using FDMA and different configurations such as: I/Q, RMS, or FFT. The work also determines the error rates that can be obtained with the different FDMA and CDMA configurations, considering different error scenarios and integration time. Synthetic emulations and empirical tests were performed, which concluded that IPS systems based on optical signals and PSD sensors can achieve very high measurement accuracies and a high measurement rate. Obtained positioning errors in a room of 3 m height are less than 1 cm when working in noisy environments.

Background Inverted papilloma (IP) is the most common benign sinonasal neoplasm. Endoscopic techniques, improved understanding of pathophysiology, and novel surgical approaches have allowed rhinologists to treat IPs more effectively, with surgery being the mainstay of therapy. Frontal sinus IP poses a challenge for surgical therapy due to complex anatomy and potentially difficult surgical access. Objectives We reported a unique case of a massive frontal sinus IP that presented with intracranial and orbital extension, with near resolution after chemotherapy. Methods A retrospective case review of a patient with a frontal sinus IP treated at a tertiary academic medical center. Results A 75-year-old male patient presented with nasal obstruction, purulent nasal discharge, and a growing left supraorbital mass. Endoscopy demonstrated a mass that filled both frontal and ethmoid sinuses, with orbital invasion. There also was substantial erosion of the posterior table, which measured 1.73 × 1.40 cm. A biopsy specimen demonstrated IP with carcinoma in situ. The patient was deemed unresectable on initial evaluation and, subsequently, underwent chemotherapy (carboplatin and paclitaxel). The tumor had a dramatic response to chemotherapy, and the patient elected for definitive surgery to remove any residual disease. During surgery, only a small focus of IP was found along the superior wall of the frontal sinus. No tumor was found elsewhere, including at the site of skull base erosion. The final pathology was IP without carcinoma in situ or dysplasia. Conclusion This was the first reported case of chemotherapeutic “debulking” of IP, which facilitated surgical resection, despite substantial intracranial and orbital involvement. Although nearly all IPs can be treated surgically, rare cases, such as unresectable tumors, may benefit from systemic chemotherapy.

Chemical thinning was applied in blue spruce (Picea pungens Engelm.) stands by the application of Roundup Klasik and Garlon 4 EC. The thinning efficacy of Garlon 4 EC was negligible, while that of Roundup Klasik reached nearly 100% when the full strength concentration was applied in quantities of 1 ml per 10 cm of stem girth using the method of drilling holes for the arboricide injection. Treated blue spruce trees died within 6 month after application. No difference was found in the response of silver and green varieties of blue spruce to the application of arboricides. Several species of cambioxylophages colonizing blue spruce were killed by Roundup. The economically insignificant Hylurgops palliatus (Gyll) was the most abundant negatively affected insect species found on the stem, followed by significant pests such as Ips amitinus (Eichh.) and Pityogenes chalcographus (L.). In these species, partial mortality of larvae was probably caused by the lowered quality of phloem. A significant mortality of Cryphalus abietis (Ratz.) was noted on branches.

This paper presents an algorithm for the estimation of open boundary conditions (OBCs) which force tides in the interior region by an adjoint data assimilation approach. Assuming that OBCs are position dependent, OBCs can be approximated by linear interpolation among values at certain independent points (IPs). Twin experiments are performed to examine the sensitivity of the model to the IP distribution and interpolation radius. It is proved that the prescribed OBCs can be well recovered with appropriate number of IP and interpolation radius. In the Bohai Sea model domain with horizontal resolution of10’×10’, the appropriate number of IP is 3 and the interpolation radius is 60′. In the practical experiment, the M2constituent in the Bohai Sea is simulated by assimilating the T/P data and tidal gauge data. The mean absolute errors in amplitude and phase are 5.0 cm and 5.7°, respectively, and the cochart obtained shows the character of M2constituent in the Bohai Sea.

Nonviral reprogramming of bovine embryonic and adult fibroblasts was undertaken using electroporation of a polycistronic plasmid carrying human reprogramming factors Oct4, Sox2, Lin28, and Nanog (Addgene Plasmid #20922). Because of difficulties encountered in reliably and reproducibly reprogramming bovine cells to a pluripotent state, several small-molecule combinations have been tested to support reprogramming. Previously, a combination of three molecules, (sodium butyrate, PD0325901 and SB431542; NaB-PD-SB) in induced pluripotent stem (iPS) medium [Minimum Essential Medium Alpha, 20% FBS, 1x insulin-transferrin-selenium (ITS), 2 mM Glutamax (Gibco, Grand Island, NY, USA), 100 μM nonessential amino acids (NEAA), 50 U mol–1 penicillin, 50 mg mL–1 streptomycin, 0.1 mM β-mercaptoethanol, 4 ng mL–1 human leukaemia inhibitory factor, and 10 ng mL–1 basic fibroblast growth factor] was found to accelerate the reprogramming process of bovine iPS cells, with colonies observed at 12 days post-electroporation, as opposed to 21 days without the addition of the small-molecule cocktail. The addition of 1 mM valproic acid (VPA), a histone deacetylase (HDAC) inhibitor, in combination with the other small-molecule cocktail (NaB-PD-SB), was found to improve the reprogramming efficiency of bovine adult and embryonic fibroblasts, with colonies observed at Day 10 post-electroporation, and the average number of colonies present per 10-cm dish, rising from an average of 5 colonies observed in the NaB-PD-SB cocktail to 27 with the addition of VPA. Colonies grown in the presence of VPA had a consistent morphology, forming compact domed colonies consisting of small round cells with well-defined borders. It was observed that colonies growing in the presence of VPA tended to have a better defined border and grew larger in size than those grown in the presence of NaB-PD-SB alone. Colonies began to differentiate after 21 days and were no longer alkaline phosphatase positive after this time. Cells were either harvested for mRNA extraction or differentiated into embryoid bodies (EB). Expression of pluripotency genes, Oct4, Sox2, Klf4, Nanog, and Alkaline Phosphatase were significantly increased in the presence of VPA compared to nonreprogrammed somatic bovine fibroblasts, with expression profiles similar to those grown in the NaB-PD-SB cocktail. Embryoid bodies were analysed for gene expression of different germ layer markers, FOXA2 (endoderm), Nestin and TUBB3 (ectoderm), and Desmin (mesoderm) using quantitative RT–PCR. For both EBs derived from the NaB-PD-SB cocktail and those with VPA, at least one marker from each germ layer was present, demonstrating the potential potency of these cells. At least a 10-fold increase in expression of these was observed in comparison to somatic fibroblast cells. It is apparent that the addition of small molecules can assist in the reprogramming of bovine iPS cells, and addition of VPA to the cocktail results in more consistent putative bovine iPS colonies. Further work is needed to identify the causes for early differentiation of colonies in order to obtain fully reprogrammed pluripotent stem cells.
