According to the latest data, in Italy 200.000 patients are affected by the Parkinson’s Disease, and the trend is bound to go up. Generally the diagnosis, crucial in this pathology, is based on the clinic observation. But the absence of typical signs or symptoms increases the possibility of diagnostic mistakes. Ioflupane (DATSCAN ®) is a new diagnostic agent indicated for detecting loss of functional dopaminergic neuron terminals in the striatum of patients with clinically uncertain Parkinsonian Syndromes. The use of Ioflupane (DATSCAN ®) in association with the single-photon emission computed tomography (SPECT) allows to improve the diagnosis. Applying systematically this diagnostic model could cause a significant reduction in hospedalization cost, with less trouble for patients and a better quality of life. Aim of this study is to show the clinical and economic consequences of the use of Ioflupane (DATSCAN â) in association with SPECT as a new standard for the diagnosis of Parkinson’s Disease, considering the perspective of the National Heath System (NHS) and a single hospital structure.

Abstract:
Aim. High order control of movements and gestures is attributed to a cortico-subcortical network involving the fronto-parietal cortex and connections, and the striatum. Damage within the basal ganglia would be neither necessary nor sufficient to cause clinically evident apraxia, but would add to praxis errors caused by fronto-parietal dysfunction. While the cortical component of the praxis network has been repeatedly investigated, this is the first study directly exploring the contribution of the striatum: we correlated dopaminergic uptake measured with 123-ioflupane Single Photon Emission Tomography (SPET) and PET FDG cerebral metabolism with severity of ideomotor apraxia in Cortico-Basal Syndrome (CBS). Patients and methods. Patients were evaluated with De Renzi’s Test for Ideomotor Apraxia, which allows to score separately movements performed with hand and fingers or with arm and hand, and meaningful and meaningless gestures. Both upper limbs were assessed. Occipital-striatum ratios of dopaminergic uptake were measured semiquantitatively for left and right caudate and putamen. Partial correlation analysis between apraxia scores and SPET parameters was conducted using Unified Parkinson’s Disease Rating Scale III, MMSE scores, disease duration and age as covariates. Bonferroni correction for multiple comparisons was applied. PET FDG uptake was quantified using SPM software. Results. Eleven patients diagnosed with CBS according to recent standardised criteria were included into the study. They were eight women and three men with a mean age of 70.3 years +7.8, a mean disease duration of 28.2 months +14.6, and a MMSE score of 19.7 +4.1 on average. Partial correlation analysis after Bonferroni correction showed a statistically significant relationship between reduced dopaminergic uptake in the left striatum (caudate and putamen) and poor performance of gestures with the left fingers. A correlation was demonstrated between a cluster of reduced metabolism at PET FDG in the frontal left hemisphere and right apraxia scores in the CBS subjects. Discussion. Even if confirmation in a larger sample is needed, our findings confirm a cortical involvement in the praxis network and support a role of the basal ganglia on praxis dysfunction in subjects with CBS. Conclusions. In CBS, damage to the basal ganglia is not only responsible for extrapyramidal signs and symptoms, but also contributes to the development of ideomotor apraxia.

O Iodo, situado no grupo 17 e no período 5 da tabela periódica é um elemento químico não-metálico essencial à vida, assumindo um papel vital no crescimento e desenvolvimento do organismo. Ao longo deste trabalho o iodo será descrito enquanto elemento químico, dando especial atenção aos vários isótopos de iodo, principalmente àqueles que possuem aplicações terapêuticas e de diagnóstico no âmbito da Medicina Nuclear. Assim, no decurso desta revisão bibliográfica abordar-se-á as características físicas e químicas do Iodo, enquanto elemento químico, e dos seguintes isótopos: 123I, 124I, 125I e 131I. Para além disto, serão ainda referidos os seus métodos de obtenção, e as suas propriedades nucleares que tornam possível a sua aplicação a nível médico. Neste contexto abordar-se-ão pormenorizadamente os radiofármacos de iodo utilizados em diagnóstico, nomeadamente: para estudos da glândula da tiroide (NaI-[131I]), para diagnóstico da Doença Isquémica Cerebral (iofetamina-[123I]) e ainda para diagnóstico da doença de Parkinson (Ioflupano-[123I]). Para além destes, serão ainda abordados outros radiofármacos de iodo que são aplicados a nível terapêutico, tais como: o NaI-[131I] (hipertiroidismo e carcinoma da tiroide), as sementes de 125I (cancro da próstata), o Tositumomab-[131I] (linfoma folicular não-Hodgkin) e o anticorpo monoclonal anti-tenascina 81C6 marcado com 131I (glioma). Por último, será ainda abordada a possível aplicação de nanopartículas marcadas com radioisótopos de iodo no âmbito da Medicina Nuclear.
